Backgroung

Previous studies showed that maximal oxygen uptake and maximal heart rate were not different during prolonged fasting (ramadan) compared to normal feeding period. However, the effect of ramadan on the blood pressure response during incremental exercise has not been investigated.

Objective

This study aimed to evaluate the evolution of blood pressure during incremental trial in ramadan period.

Methods

Twelve young trained male aged 24 ± 4 yrs participated as voluntary subjects. Their anthropometric parameters, maximal aerobic power and maximal heart were measured in fasting and in normal feeding periods during incremental trial on cycle ergometer.

Results

No significant difference was observed in any anthropometric parameter. Maximal aerobic power of fasting period was significantly less (P < 0,05) compared to normal feeding. Heart rate at rest, at maximal exercise and during a 15 min period of recovery was not significantly affected. Systolic blood pressure of fasting period at maximal power was significantly lower than during the control period (P < 0,05).

Conclusion

Ramadan negatively influences the capacity of maximal power and cardiovascular response at maximal power.  相似文献

Objective

The aim of this study was to elaborate and validate a specific test to evaluate the physical condition of judo players.

Subjects and methods

Twenty-three volunteers, males, aged 22 ± 3.62 years old took part in our experiment. They did the progressive test of Leger et al. (1984), vertical Jump test (Sargent test), Australian shuttle run test and a specific judo test.

Results

The observed results showed significant correlations between muscular power and the number of Uchi-komi on the judo test reference scale (R = 0.52, P < 0.01). Furthermore, there were other correlations between the number of Uchi-komi at the two first sets of specific judo test and the anaerobic power represented by the distance covered in 30s at the Australian shuttle test (R = 0.86, P < 0.01), also between the anaerobic capacity represented by the whole distance covered and the total number of Uchi-komi achieved at the judo test (R = 0.88, P < 0.01).

Conclusion

The test reproduces the physiological characteristics of judo fight. It is a good indicator of the judoka's physical fitness and their cardiovascular adaptation in a physical effort.  相似文献

Purpose

This study was focused on the serial changes in magnetic resonance images (MRI) of the skull-base bone that occur after endoscopic nasopharyngectomy in patients with local recurrent nasopharyngeal carcinoma (rNPC).

Materials and methods

Nine patients with histologically proven rNPC were enrolled in this study. Two experienced radiologists independently reviewed all presurgical and postsurgical MR images for each patient.

Results

At 36 sites on the skull base, the MRI signal underwent a change after surgery, which took the form of a heterogeneous pattern of hypointense regions with moderate contrast enhancement on T1WI. The onset of changes ranged between 2 weeks and 3 months after surgery. For 21 of the sites, the changes subsided over the course of follow-up, while in 6 they remained stable. At 9 sites, the alteration MRI signal became more pronounced with time. Changes were more common on the homolateral side of the skull base with respect to the recurrent tumor (P < 0.05). The skull-base bone adjacent to the resection boundary had a higher incidence of signal change than nonadjacent areas (P < 0.05).

Conclusions

MRI changes in the skull base bone, having a number of distinguishing characteristics, appear to be a common sequel to endoscopic nasopharyngectomy for rNPC.  相似文献

Introduction

The aim of this study was to determine the effects of endurance training on IGF-1, corticosterone and insulin levels in male sedentary and trained rats.

Facts

IGF-1 concentrations decrease after training (p < 0.05), those of corticosterone increase (p < 0.01) whereas insulin levels remain stable.

Conclusion

A short period of endurance training leads to catabolic state with a decrease in IGF-1 concentrations and increase in corticosterone levels.  相似文献

Objective

To assess the sensitivity and specificity of detecting subarticular bone erosion of sacroiliac (SI) joint in patients with spondyloarthritis (SpA) using MRI three-dimensional water selective balanced steady-state free precession sequence (3D-WS-bSSFP) and T1-weighted (T1W) sequence.

Materials and methods

Radiography, CT and MRI of SI joint from 43 SpA patients were retrospectively analyzed. MRI examination sequences include T1W, short tau inversion recovery (STIR) and 3D-WS-bSSFP. Two radiologists, blinded to clinical data, independently determined bone erosion at bilateral sacral and iliac sides of the SI joint on radiography, CT, T1W and 3D-WS-bSSFP respectively. X2 test was used to compare the sensitivity of detecting bone erosion among different diagnostic methods.

Results

Of the 86 sacral and 86 iliac articular surfaces from the 43 cases, radiography, CT, MRI T1W and 3D-WS-bSSFP showed the presence of bone erosion in 40, 74, 50 and 71 articular surfaces respectively. CT and MRI 3D-WS-bSSFP demonstrated similar sensitivity (x2 = 0.11, P = 0.74), and both were superior to radiography (x2 = 15.17, P < 0.01 and x2 = 12.78, P < 0.01, respectively) and T1W (x2 = 7.26, P < 0.01 and x2 = 5.62, P < 0.05). Using CT diagnosis as the gold standard, the sensitivity and specificity of detecting bone erosion for MRI 3D-WS-bSSFP and T1W sequences were 91.8%, 96.9%, and 60.8%, 94.9% respectively.

Conclusion

MRI 3D-WS-bSSFP sequence is associated with short scanning time, zero ionizing radiation, high sensitivity and specificity of displaying the subarticular bone erosion of SI joints in patients with SpA. Therefore, it can be considered an alternative to CT.  相似文献

Purpose

The purpose of this study was to investigate whether 3D-double echo steady state (3D-DESS) with improved contrast by setting the FA (Flip angle) at 90° is useful in 3D isotropic cartilage imaging of the knee at 3 T.

Materials and methods

Imaging was performed in 10 healthy volunteers using 3 methods: with 3D-DESS using FA of 25° and 90°, and with true fast imaging with steady-state precession (True-FISP). The signal-to-noise ratio (SNR) of the synovial fluid and cartilage, and contrast-to-noise ratio (CNR) were measured, and mean values were compared. Visual assessment of artifacts was performed with the cartilage divided into 6 regions.

Results

There were no significant differences in synovial fluid SNR in the comparison between FA-90° 3D-DESS and True-FISP (P = 0.364). A significantly higher cartilage SNR was observed with FA-90° 3D-DESS than with True-FISP (P = 0.031). There were no significant differences in synovial fluid-cartilage CNR between FA-90° 3D-DESS and True-FISP (P = 0.892). In the evaluation of artifacts, FA-90° 3D-DESS imaging showed a significantly higher score than True-FISP imaging in the patella and trochlea cartilage (P < 0.001, P < 0.002).

Conclusions

FA-90° 3D-DESS is useful in 3D isotropic cartilage imaging of the knee at 3 T.  相似文献

Background

Vertebral collapse is a common problem due to benign bone marrow lesions, trauma or malignant process. The diagnosis is often correctly predicted from characteristic imaging appearance. Some vertebral collapses have atypical imaging appearance that may cause diagnostic confusion.

Aim

To evaluate the value of the ADC obtained in DW-MR sequences for the differentiation between benign and malignant bone marrow lesions.

Patients

Sixty patients were included in this study, referred from Neurosurgery and Radiotherapy Departments and proved to have vertebral compression based on conventional MR imaging.

Results

The ADC value resulted in statistically significant characterization between (osteoporotic and post-traumatic collapse) and (malignant vertebral collapse) (P < 0.0001) while there was no statistically significant findings between infective spondylodiscitis and malignant vertebral collapse (P > 0.05). The sensitivity, specificity, PPV, NPD of DWI and ADC values in differentiating benign from malignant vertebral collapse were 100%, 83.3%, 60% and 100% respectively.

Conclusions

ADC values are a useful complementary MRI tool to characterize bone marrow lesions, in order to distinguish acute benign fractures from malignant or infectious bone marrow lesions. However, ADC values are not valuable in order to differentiate malignancy from infection with diagnostic overlap in the subacute traumatic vertebral collapse.  相似文献

Purpose

The effects of vitamins and minerals complex supplementation on maximal voluntary contraction decrease (FMV) and biological markers following an eccentric exercise at old people.

Method

Sixteen elderly subjects took either placebo (Pl group) or vitamins and minerals (Isoxan Senior, NHS, Rungis, France) (group S) for 21 d before an eccentric exercise and for 3 d after the exercise. The FMV and surface EMG activity (RMS) of the vastus lateralis (VL), vastus médialis (VM) and rectus fémoris (RF) were recorded before (Pre), immediately after (Post), 24 h (Post 24) and 48 h (Post 48) after the exercise. CCVThe creatine kinase (CK), lactate déshydrogénase, malondialdéhyde, and tumor necrosis Factor (TNFα) levels were analyzed.

Results

The reduced MVC (S: 11,2 ± 4,8%; Pl: 17,8 ± 10,4%, P < 0,01) after exercise was associated with a significant reduction in RMS VL, RMS VM and RMS RF values for both groups. A faster FMV recovery appeared at 48 h for the S group (P < 0.05). CK and TNFα values increased in post-exercise.

Conclusion

A dietary supplementation of a vitamin and mineral complex does not attenuate the loss of contractile function immediately after the running exercise, and it may accelerate the recovery of maximal force capacity after 48 h by limiting the post-exercise pro-inflammatory processes.  相似文献

Objective

To qualitatively and quantitatively compare T2-weighted MR imaging of the liver using volumetric spin-echo with sampling perfection with application-optimized contrast using different flip angle evolutions (SPACE) with conventional turbo spin-echo (TSE) sequence for fat-suppressed T2-weighted MR imaging of the liver.

Materials and methods

Thirty-three patients with suspected focal liver lesions had SPACE MR imaging and conventional fat-suppressed TSE MR imaging. Images were analyzed quantitatively by measuring the lesion-to-liver contrast-to-noise ratio (CNR), and the signal-to-noise ratio (SNR) of main focal hepatic lesions, hepatic and splenic parenchyma and qualitatively by evaluating the presence of vascular, respiratory motion and cardiac artifacts. Wilcoxon signed rank test was used to search for differences between the two sequences.

Results

SPACE MR imaging showed significantly greater CNR for focal liver lesions (median = 22.82) than TSE MR imaging (median = 14.15) (P < .001). No differences were found for SNR of hepatic parenchyma (P = .097), main focal hepatic lesions (P = .35), and splenic parenchyma (P = .25). SPACE sequence showed less artifacts than TSE sequence (vascular, P < .001; respiratory motion, P < .001; cardiac, P < .001) but needed a longer acquisition time (228.4 vs. 162.1 s; P < .001).

Conclusion

SPACE MR imaging provides a significantly increased CNR for focal liver lesions and less artifacts by comparison with the conventional TSE sequence. These results should stimulate further clinical studies with a surgical standard of reference to compare the two techniques in terms of sensitivity for malignant lesions.  相似文献

Objective

To determine the benefit of DWI in diagnosis of benign and malignant solid or predominantly solid gynecological adnexal or ovarian masses.

Material and Methods

This study is carried out on 23 patients with histologically proven solid or predominantly solid adnexal or ovarian masses out of which 5 cases (21.8%) have benign and 18 cases (78.2%) have malignant neoplasms. Among these 19 cases (82.6%) have unilateral disease and 4 cases (17.4%) have bilateral disease which was metastatic ovarian carcinoma.

Result

On DWI, high signal intensity is noted in malignant lesion more frequently than in benign lesion. (P < 0.001) in adnexal lesions, while in ovarian lesions (P = 0.001).The differentiation between benign and malignant adnexal lesions revealed no significant difference in the apparent diffusion coefficient (ADC) value (P = 0.22).

Conclusion

DWI is a helpful tool in differentiation between predominantly solid and solid benign and malignant adnexal lesions because there is an increased frequency of higher signal intensity (diffusion restriction) in malignant lesions.  相似文献

Objectives

To investigate whether rice pads can eliminate lingering fat signals of the complex surface shape of both hands that occur with chemical shift selective (CHESS) at 1.5 T and 3.0 T.

Materials and methods

T1-weighted images were obtained with CHESS using 1.5 T and 3.0 T systems. The same imaging parameters were used with and without rice pads on the coronal plane of both hands in 10 healthy volunteers. The fat-suppression effects were classified into four categories and scored for images, and visual evaluations were performed by one radiologist and one radiologic technologist.

Results

At 1.5 T, the mean evaluation score was 1.55 for images obtained without rice pads and 3.50 for images obtained with rice pads. At 3.0 T, the mean evaluation score was 1.10 for images obtained without rice pads and 3.20 for images obtained with rice pads. With both systems, images obtained with the rice pads showed significantly better fat suppression effects than images obtained without rice pads (P < 0.0001, P < 0.0001).

Conclusions

It was confirmed that lingering fat signals are eliminated and good fat-suppressed images are obtained with the use of rice pads at 1.5 T and 3.0 T. Rice pads are therefore useful with at 1.5 T and 3.0 T, which are currently becoming more widely used.  相似文献

Objective

CT-guided transthoracic biopsy is a well-established method in the cytologic or histologic diagnosis of pulmonary lesions. The knowledge of its diagnostic performance and complications for cavitary pulmonary lesions is limited. The purpose of this study was to determine the diagnostic accuracy and safety of CT-guided fine needle aspiration biopsy (FNAB) in cavitary pulmonary lesions.

Materials and methods

102 consecutive patients with pulmonary cavitary lesions received CT-guided FNAB with use of an 18-gauge (n = 35) or 20-gauge (n = 67) Chiba for histology diagnosis. The sensitivity, specificity, and diagnostic accuracy of FNAB were calculated as compared with the final diagnosis. Complications associated with FNAB were observed. The diagnostic accuracy and complications were compared between patients with different lesion sizes and different cavity wall thickness.

Results

The overall sensitivity, specificity, and accuracy of FNAB were 96.3%, 98.0%, and 96.1%, respectively. The sensitivity, specificity, and diagnosis accuracy in different lesion size (<2 cm vs ≥2 cm), or different cavity wall thickness (<5 mm vs ≥5 mm) were not different (P > 0.05; 0.235). More nondiagnostic sample was found in wall thickness <5 mm lesions (P = 0.017). Associated complications included pneumothorax in 9 (8.8%) patients and alveolar hemorrhage in 14 patients (13.7%) and hemoptysis in 1 patient (1%). No different rate of complications was found with regard to lesion size, wall thickness, length of the needle path and needle size (P > 0.05).

Conclusion

CT-guided FNAB can be effectively ad safely used for patients with pulmonary cavitary lesions.  相似文献

Goal

The aim of this study was to test the relation between the reactive power and the stiffness of the legs in high trained athletes (sprinters and skiing racers).

Method

A simple method to measure, in field conditions, the leg stiffness during maximal bouncing was used on twenty-eight sprinters, and twenty-nine skiing racers. In addition, the mechanical power called reactive power was calculated according to the method proposed by Bosco. The relationship between stiffness and power was observed in the different groups.

Results

The value of stiffness obtained was 29,866 ± 7372 N.m−1. The power was equal to 52.33 ± 8,72 W.kg−1. The main result of this study was that a relation between stiffness and power was obtained only in the sprinters (r2 = 0,68, P < 0,001). This result stressed the influence of the sport speciality on the stiffness control.

Conclusion

The power measured during maximal bouncing, called reactive power is correlated with the leg's stiffness when subjects are trained to produce power in similar conditions. No correlation was obtained in skiing racers. The stiffness measurement was then a complementary test in the physical evaluation of the athletes.  相似文献

Background

Hyperdynamic circulatory state in liver cirrhosis is characterized by increased splanchnic blood flow and renal vasoconstriction.

Aim

To evaluate the relationship between renal resistive indices (RI) and HCV liver cirrhosis severity and RI value in predicting 6 month survival of those patients. Also we aimed to assess the effect of midodrine on RI.

Patients and methods

120 patients with HCV liver cirrhosis and 40 healthy controls were enrolled in the study. INR, total bilirubin, albumin, creatinine and sodium were measured in all patients. Both patients and controls underwent abdominal ultrasound with duplex Doppler examination of the kidneys with RI calculated. Patients were followed for 6 months. Surviving patients with highest risk underwent renal duplex with RI calculation (RI2). They then received oral midodrine at a dose of 7.5 mg three times daily for 3 months with revaluation of RI (RI3).

Results

57 (47.5%) patients had high RI (RI > 0.7) while 63 (52.5%) patients had normal RI. Patients had significantly higher RI than healthy controls (P < 0.001). There was a significant positive correlation between RI and MELD, MELD-Na, and Child class (r = 0.859, r = 0.769, rho = 0.56 respectively and P < 0.001). Patients with RI > 0.73 are at higher risk of death within 6 months (P < 0.001). Administration of midodrine resulted in no significant difference in RI in the 37 surviving patients with baseline RI > 0.73 (P = 0.1605).

Conclusion

RI is strongly correlated with liver cirrhosis severity and had comparable prognostic value with MELD score. Midodrine had no significant effect on RI in high risk patients.  相似文献

Objective

To determine whether contrast enhancement of CT and apparent diffusion coefficient on diffusion-weighted MR imaging are important parameters that can predict outcomes for patients with pancreatic ductal adenocarcinoma.

Materials and methods

Ninety-two patients with histologically confirmed pancreatic ductal adenocarcinoma who underwent quadriphasic CT (including unenhanced, pancreatic parenchymal, portal venous and delayed phases) and fat-suppressed single-shot echo-planar diffusion-weighted MR imaging at 3.0 T were retrospectively analyzed to investigate prognostic factors. Overall survival curves were drawn using the Kaplan–Meier method. Effects on survival of variables including age, sex, tumor location, tumor size, TNM stage, carbohydrate antigen 19-9, carcinoembryonic antigen, treatment, tumor contrast enhancement and apparent diffusion coefficient values were analyzed in univariate analysis using the log-rank test. Variables were analyzed in multivariate analyses using the Cox proportional hazards regression model.

Results

Median survival for the entire patient population was 18.2 months. Higher contrast enhancement during all phases was associated with significantly longer overall survival (P < 0.001 for all phases). The difference in overall survival between groups divided by median apparent diffusion coefficient value was not significant (P = 0.672). TNM stage (P = 0.026) and tumor contrast enhancement on CT (P = 0.027) were significantly related to survival in multivariate analysis.

Conclusions

Poor enhancement of pancreatic adenocarcinomas on enhanced CT is associated with reduced patient survival.  相似文献

Purpose

Femoral neck fracture is a major public health problem in elderly persons, representing the main source of osteoporosis-related mortality and morbidity. In this study, we aimed at comparing radiographic texture analysis with three-dimensional (3D) microarchitecture in human femurs, and at evaluating whether bone texture analysis improved the assessment of the femoral neck fracture risk other than that obtainable by bone mineral density (BMD).

Materials and methods

Thirteen osteoporotic femoral heads from patients who fractured their femoral neck and twelve non-fractured femoral heads from osteoarthritic patients were studied using respectively (1) a new high-resolution digital X-ray device (BMA™, D3A Medical Systems) allowing for bone texture analysis with fractal parameter Hmean, and (2) a micro-computed tomograph (CT) for 3D microarchitecture. BMD was measured postoperatively by DXA in all patients in the contralateral femur.

Results

In these femoral heads, we found that fractal parameter Hmean was correlated with 3D microarchitecture parameters: bone volume fraction (BV/TV), trabecular number (Tb.N), trabecular separation (Tb.Sp) and fractal dimension (FD) respectively (p < 0.05). Then, fractal parameter Hmean was significantly lower in the femoral heads from the fractured group than from the non-fractured group (p < 0.01). Finally, multiple regression analysis showed that combining bone texture analysis and total hip BMD significantly improved the estimation of the femoral neck fracture risk from adjusted r2 = 0.46 to adjusted r2 = 0.67 (p < 0.05).

Conclusion

Radiographic bone texture analysis was correlated with 3D microarchitecture parameters in the femoral head, provided accurate discrimination between the femoral heads from the fractured and non-fractured groups, and significantly improved the estimation of the femoral neck fracture risk when combined with BMD.  相似文献

Purpose

To investigate the diagnostic value of T1 mapping imaging of evaluating fibrosis in patients with hypertrophic cardiomyopathy (HCM).

Materials and methods

21 subjects with HCM and 18 healthy volunteers underwent conventional late gadolinium enhancement (LGE) imaging and T1 mapping imaging. The region of myocardium in HCM is divided into remote area of LGE, peri-LGE, LGE (halo-like LGE and typical patchy LGE). These regions combined with normal volunteers’ myocardium were calculated by the reduced percent of T1 value (RPTV).

Results

The RPTV in healthy volunteers was no significant comparing with that in the remote area of LGE in HCM subjects (3.98 ± 3.19 vs. 3.34 ± 2.75, P > 0.05). There were significant statistical differences in pairwise among the remote area of LGE, peri-LGE, halo-like LGE and typical patchy LGE in the RPTV (P < 0.0001). ROC curves indicated that the T1 mapping imaging has a greater area under the curve comparing with that of traditional LGE imaging (0.975 ± 0.07 vs. 0.753 ± 0.26, P < 0.0001).

Conclusions

HCM has a high prevalence of fibrosis and with varying severity. T1 mapping imaging can be a useful method to evaluate the severity of the fibrosis in HCM.  相似文献

Objective

Medial tibial condyle bone marrow edema (BME), associated with soft tissue edema (STe) surrounding the medial collateral ligament, was incidentally observed in MRI examinations of young and athletic individuals. The aim of the present study was to 1. Prospectively investigate the association between these findings and coexistence of localized pain, and 2. Explore the possible contribution of the tibial morphology to its pathogenesis.

Methods

The medial tibial condyle crest was evaluated in 632 knee MRI examinations. The angle and depth were measured by two separate evaluators. The presence of STe and BME was recorded. A third evaluator blindly assessed the presence of pain at this site.

Results

BME associated with STe was found in 24 patients (with no history of previous trauma, osteoarthritis, tumor or pes anserine bursitis). The mean crest angle was 151.3° (95%CI 147.4–155.3°) compared to 159.4° (95%CI 158.8–160°) in controls (Mann–Whitney test, P < 0.0001). MRI findings were highly predictive of localized pain (sensitivity 92% specificity 99%, Fisher's exact test, P < 0.0001).

Conclusion

Friction at the medial tibial condyle crest is a painful syndrome. MRI is a highly specific and sensitive imaging modality for its diagnosis.  相似文献

Purpose

To retrospectively analyze the degree of artefacts in CT and MRI scans caused by coils and vascular plugs used for portal vein embolization (PVE).

Materials and methods

All patients who underwent PVE between July 2011 and December 2012 and received either plug or coil embolization additional to particle embolization were retrospectively analyzed. Artefacts causes by embolization materials were measured in CT and MRI scans following PVE.

Results

The sum of the artefact diameters caused from coils was significantly higher in CT (188.3 ± 26.1 mm) than in MRI T1 (50.5 ± 6.5 mm) or T2 imaging (39.5 ± 9.7 mm) (P < 0.05). In MRI, the diameter in T1 and T2 sequences did not differ significantly (P = 0.977). The sum of the artefact diameters from vascular plugs in MRI T1 sequences (62.5 ± 8.7 mm) was significantly higher than in CT (46.6 ± 4.8 mm; P < 0.05) and MRI T2 sequences (52.8 ± 3.8 mm; P < 0.05).

Conclusion

PVE with particles and vascular plug causes significantly fewer artefacts than PVE with particles and coils on CT scans following embolization, which can be important in the event of vascular complications or in follow-up scans, should the patient become unresectable.  相似文献

Purpose

This study aims to determine the use of diffusion-weighted (DW) magnetic resonance imaging (MRI) in differentiating typical and atypical meningiomas.

Patients and methods

In total, 31 patients aged 37–77 years with meningiomas were included in this study. Using routine MRI sequences, the meningiomas were diagnosed and DW images were performed using factor of b-0 and b-1000. Apparent diffusion coefficient (ADC) values were measured in the lesion, in the normal area of brain parenchyma. Student t-test was used for statistical analysis. P < 0.05 was considered significant.

Results

Showed that the mean ADC of atypical/malignant meningiomas (0.42–0.69 × 10−3 mm2/s; P < .0001) was significantly lower compared with benign meningiomas (0.72–1.5 × 10−3 mm2/s). Mean NADC ratio in the atypical/malignant group (0.61) was also significantly lower than the benign group (1.21; P < .0001), without overlap between groups. The difference between the ADC values of the subtypes of typical meningiomas was insignificant.

In conclusion

Typical meningiomas have higher ADC values than atypical cases. DW MRI may be of help in differentiating typical and atypical meningiomas.  相似文献
