基底膜抗原在声门上型喉鳞癌中表达的意义   总被引:8,自引:0,他引:8  
目的 探讨基底膜(basement membrane,BM)抗原的表达及其在声门上型喉鳞癌中的屏障意义。方法 应用抗IV型胶原纤维单克隆抗体以链霉素亲生物素-过氧化酶免疫组化法观察IV型胶原在声门上型喉鳞癌中的表达分布情况,以评价BM完整性与声门上型喉鳞癌的临床分期、分级、细胞分化程度、浸润方式及颈淋巴结转移的关系。结果 BM完整者占42.5%,BM缺失者占57.5%。在低分化和肿瘤弥散浸润的喉癌组织中,BM缺失率明显高于分化良好及肿瘤边界良好的组织(P〈0.05)。BM缺失者发生颈淋巴结转移率高于BM完整者,二者之间有较强相关性(P〈0.01)。BM表达与临床分期之间并未现现出明确的相关性(P〉0.05)。结论 BM完整性可以用来评价声门上型喉鳞癌细胞分化程度及喉癌组织浸润能力,也可作为预测声门上型喉鳞癌发生  相似文献

目的 :探讨细胞外基质糖蛋白 (tenascin ,TN)在喉部良、恶性病变中的表达 ,及其与喉肿瘤发生及发展的关系。方法 :将 5 8例石蜡包埋喉部病变组织用鼠抗人单克隆抗 TN抗体行免疫组化染色 ,用HPIAS 10 0 0型医学彩色全自动图像分析系统进行分析。结果 :TN在喉部良性病变及正常组织的细胞外间质中无阳性染色 ,在喉的原位癌组织的细胞外间质中有弱阳性染色 ,且二者差异有显著性意义 (P <0 .0 0 1)。在喉的侵袭性鳞状细胞癌组织的细胞外间质中有强的TN染色 ,且在有淋巴结转移的肿瘤组织中染色更强 ,染色面积更广。在喉癌的各病理分级之间阳性染色差异无显著性意义 (P >0 .0 5 )。结论 :提示TN与喉癌的发生、发展和转移有关 ,而与其病理分级无关。  相似文献

目的:探讨Syndecan—1和E—candherin两种黏附分子在声门上型喉鳞状细胞癌中的表达情况及与喉癌生物学行为之间的关系。方法:应用SP免疫组织化学法检测了38例声门上型喉鳞状细胞癌和7例喉黏膜慢性炎症组织中的Syndecan-1和E—candherin的表达并根据阳性瘤细胞占肿瘤细胞总数的比率进行半定量分析和统计检验。结果:Syndecan-1和E—candherin的阳性染色部位主要集中于细胞膜,E—candherin也可表达于细胞间质。Syndecan-1和E-candherin在恶性程度高的癌细胞上膜表达明显缺失,并与组织学分化、肿瘤大小密切相关。Syndecan-1和E-candherin在喉鳞状细胞癌细胞上的表达结果有相关性,但无敏感性差异。结论:Syndecan-1和E-candherin的表达可能在声门上型喉鳞状细胞癌的发生发展等生物学行为中起着重要的作用。  相似文献

目的 研究喉鳞癌中明胶酶A和明胶酶B的表达与临床病理特征及预后的关系。方法 应用免疫组化方法测定 70例喉鳞癌组织和相应癌旁组织中明胶酶A和明胶酶B的表达 ,分析其表达结果与喉鳞癌病人临床病理及预后的相关性。结果 明胶酶A和明胶酶B在喉鳞癌组织中阳性表达率均显著高于癌旁组织 (P <0 .0 1) ,并与T分级、淋巴结转移及临床分期呈正相关 (P <0 .0 5 )。Log rank检验显示明胶酶A和明胶酶B表达强阳性组术后生存率明显低于其表达弱阳性组和阴性组 (P <0 .0 5 )。结论 明胶酶A和明胶酶B在喉癌的发展过程中起重要作用 ,检测明胶酶A和明胶酶B的表达 ,有助于评估喉癌病人的预后情况。  相似文献

从喉鳞癌患者的手术切除标本中提取的可溶性抗原具有免疫活性,在抗CD3单抗和rIL-2的协同作用下,能刺激外周血单核细胞(PBMC)增殖产生CD8+细胞毒T淋巴细胞为主的杀瘤细胞,称其为TAK细胞。它是异质性细胞群,其细胞表型以CD8+细胞为主,具有活化的淋巴母细胞的形态学特征,常形成集落,并表达活化的淋巴细胞的表面标记,如IL-2受体。  相似文献

目的检测血管内皮抑素(Endostatin)在喉癌声门上型组织中的表达,分析其与临床病理特征的关系。方法收集40例喉癌声门上型组织,取大于癌旁2 cm以上的正常喉黏膜组织40例作为对照组,采用免疫组化SP法检测Endostatin的表达情况。结果 Endostatin的表达在喉癌组织中明显高于癌旁组织,差异有统计学意义(χ2=10.368,P〈0.05)。无淋巴结转移的癌组织中Endostatin阳性表达率明显高于有淋巴结转移的癌组织,差异有统计学意义(χ2=9.726,P〈0.05)。结论 Endostatin可抑制喉癌的生长和转移,为预测喉癌的预后提供了一定的理论依据。  相似文献

目的:了解细胞外基质金属蛋白酶诱导因子(EMMPRIN)在喉鳞状细胞癌(LSCC)及喉癌前病变的表达情况,探讨其与LSCC浸润和转移的关系。方法:用免疫组织化学SP法对42例LSCC、28例喉癌前病变和20例正常喉组织中的EMMPRIN表达情况进行检测。结果:EMMPRIN在LSCC和喉癌前病变的阳性表达率分别为88.1%和57.1%,明显高于正常喉组织的5.0%,Ⅲ~Ⅳ期LSCC组织的强阳性表达率为76.0%,高于Ⅰ~Ⅱ期的强阳性表达率17.6%。伴有颈淋巴结转移的LSCC组织的EMMPRIN强阳性表达率高于无淋巴结转移组。结论:EMMPRIN在LSCC有较高的阳性表达。EMMPRIN的表达与LSCC的分期和是否存在淋巴结转移有关。LSCC组织EMMPRIN的表达影响LSCC的浸润和转移。  相似文献

采用免疫组化ABC法检测了28例喉鳞癌组织中淋巴细胞亚群的分布状态及数量,并与残喉的健侧组织对照研究。发现喉鳞癌局组织浸润的T淋巴细胞多于健侧喉正常组织,癌间质的T细胞多于实质,癌组织中CD^+8细胞数多于CD^+4细胞数多于CD^+4细胞。喉低分化鳞癌和晚期患者癌组织中浸润的CD^+4和CD^+8细胞少于高分化癌和早期患者。喉癌局部细胞免疫处于抑制状态,其T细胞亚群的分布与肿瘤临床分期及组织分化  相似文献

为更全面地了解喉癌局部细胞免疫状况,收集14例喉高分化浸润性鳞状细胞癌病例,用其冰冻切片进行了免疫组织化学染色,以CD3(T3/T总淋巴细胞)、CD4(T4/辅助性T淋巴细胞)、CD8(T8/抑制性T淋巴细胞)、CD(22)(B淋巴细胞)及CD(68)(吞噬细胞)抗体,标记了五种免疫细胞。结果喉癌局部浸润的免疫细胞主要为T淋巴细胞,其中以CD3为最多,其次为CD8和CD4细胞,两者中CD8较CD4为多,CD(22)阳性细胞相对少见,CD(68)阳性细胞最少。据此认为喉癌局部主要是T淋巴细胞,尤其是CD8细胞介导的弱细胞毒排斥反应,过继免疫疗法应能使喉癌的局部免疫反应增强。  相似文献

Basaloid squamous cell carcinoma is a rare variant of squamous cell carcinoma, Larynx has been an uncommon site of this tumour which is said to have aggressive biological behaviour with high incidence of cervical and distant metastasis along with second primary. Two cases of laryngeal basaloid squamous cell carcinoma are reported with relevant review of literature. Basaloid squamous cell carcinoma (BSC) is regarded as a variant of squamous cell carcinoma, but displays distinct morphological and biological features as well as a different clinical course. The tumor is frequently seen in the head and neck and is preferentially located in the larynx, especially in supraglottic sites. Ten patients with BSC of the supraglottic larynx were treated from 1991 to 1995 at the Medical Faculty of the University of Istanbul. Results of treatment were compared retrospectively with a control group consisting of 44 patients with well-differentiated squamous cell carcinomas. Ages, ¶localizations, stages and treatment procedures were similar. In both groups mean survival, nodal involvement and distant metastases were comparable although the local ¶(laryngeal) recurrence rate in patients with early supraglottic (T2) disease in the BSC group after conservative partial surgery was distinct compared to the control group (P < 0.05). These results indicate that conservative surgery should be assessed with caution in patients with BSC, and postoperative irradiation be taken into consideration.  相似文献

OBJECTIVES/HYPOTHESIS: Head and neck cancer tumor cell invasion is responsible for both local destruction and distant metastasis. Invasion is largely mediated by matrix metalloproteases that are thought to be induced by tumor cell derived extracellular matrix metalloprotease inducer (EMMPRIN) in surrounding fibroblasts. We hypothesize that EMMPRIN is overexpressed in laryngeal cancer. STUDY DESIGN: Retrospective analysis of patients with supraglottic laryngeal cancer. METHODS: Total protein immunoblotting and immunohistochemical analysis of normal and malignant tissue were performed to determine EMMPRIN expression. EMMPRIN immunoreactivity in 33 patients was correlated with clinicopathological features and survival. RESULTS: Whole-tissue lysates of tumors (n = 8) and metastatic lymph nodes (n = 2), but not normal skin (n = 8) or mucosa (n = 6), expressed significant amounts of EMMPRIN by immunoblotting. EMMPRIN membrane immunoreactivity (transmembrane EMMPRIN score) was associated with nodal positivity (P =.07), and it was a borderline significant predictor of survival (Hazards Ratio = 2.4; 95% CI, 0.88-6.55). As a categorical variable, higher transmembrane EMMPRIN score was associated with higher mortality. CONCLUSIONS: The present study helps to establish EMMPRIN as a widely expressed protein in dysplastic mucosa and supraglottic laryngeal cancer, but not in normal epithelial counterparts.  相似文献

目的:探讨Survivin蛋白在喉癌组织中的表达与患者术后生存期的关系。方法:采用免疫组织化学 (SP法)染色技术对40例喉癌组织(喉癌组)和5例正常喉黏膜组织(对照组)石蜡标本进行Survivin蛋白表达的 检测,并采用图像分析对表达结果进行定量分析。结果:Survivin在喉癌组中的阳性表达显著高于对照组(P< 0.01);在有淋巴结转移的喉癌组织中Survivin蛋白的表达高于无淋巴结转移的喉癌组织(P<0.05);不同性别、 年龄的患者喉癌组织中Survivin蛋白的表达差异均无统计学意义(均P>0.05);Survivin蛋白的表达与喉癌的 TNM分期有关(P<0.05),与患者生存期呈负相关关系(r=-0.646,P<0.01)。结论:喉癌组织Survivin蛋 白的表达与TNM分期、淋巴结转移和预后有密切关系,它的高表达提示肿瘤患者预后不良;Survivin基因在喉癌 组织中表达上调,提示该基因在喉癌的发生、发展中起重要作用  相似文献

Conclusion. Contrary to most reports, our study shows that the expression of cyclin D1 is not an independent prognostic factor in patients with laryngeal cancer (LC). No correlation between cyclin D1 expression and survival rates in LC was found in a multivariate analysis. Objectives. The aim of this study was to determine the possible relevance of the expression of cyclin D1 protein in LC as prognostic criteria and to analyse correlation of the expression with clinicopathological features and survival rates. Materials and methods. Immunohistochemistry staining was used to detect the expression of cyclin D1 in 130 samples of laryngeal cancer and in 22 specimens of laryngeal nodules. Results. Cyclin D1 expression was found in 52 (40%) LC samples and in 3 (13.6%) samples of laryngeal nodules. There was no significant correlation between cyclin D1 expression and clinicopathological features of LC. A multivariate analysis of survival confirmed that cyclin D1 expression was not an independent prognostic factor in LC.  相似文献

OBJECTIVES: This study is designed to report the clinical and pathologic features and outcome of cases of basaloid squamous cell carcinoma (BSCC) of the larynx treated in our clinic. STUDY DESIGN: A retrospective review of the medical records of these patients. METHODS: Four cases of BSCC of the larynx were treated in our department. Histopathologic slides were reevaluated to confirm the diagnosis. Immunohistochemical studies were performed, and file records were reviewed. Follow-up was available for all patients and ranged between 11 and 72 (mean 37) months. RESULTS: All patients were male (mean 57), with supraglottic or transglottic larynx tumors. Two patients presented with stage-II disease and the other 2 with stage-IV disease. Initial diagnosis was invasive squamous cell carcinoma in 3 patients and BSCC in one patient. Two patients who had stage-II disease underwent partial laryngectomy and bilateral neck dissections; total laryngectomy and bilateral neck dissections were performed in stage-IV patients. Three patients received adjuvant postoperative radiotherapy, and 2 of them also received additional chemotherapy. Patients with stage-IV disease were found to have 4 and 27 metastatic lymph nodes on histopathologic examination and died because of distant metastases at 11 and 14 months, respectively. Patients with stage-II disease did not have cervical metastasis on histopathologic examination and were alive and free of disease at 52 and 72 months respectively. CONCLUSION: In contrast with the literature reporting the tendency of more aggressive clinical behavior of the BSCC, we can say that BSCC has a behavior similar to conventional squamous cell carcinoma based on our 4 cases.  相似文献
