Purpose:
To determine whether Doppler ultrasonography could be useful in the prediction of relapse of hyperthyroidism in patients with Graves' disease.
Material and Methods:
Forty patients with Graves' disease confirmed by laboratory tests were examined for a number of blood flow parameters in the inferior thyroid artery before and after they were subjected to proper antithyroid drug treatment. Data were retrospectively reviewed and compared with findings for a control group of 16 age-matched subjects.
Results:
Significantly increased blood flow parameters were observed both in patients with active hyperthyroidism before treatment and in euthyroid patients who presented a relapse shortly after withdrawal of proper antithyroid drug treatment versus normal controls. Conversely, no significant differences were observed between patients who remained in stable remission and normal controls.
Conclusion:
Our results support the concept that Doppler ultrasonography evaluation of patients with Graves' disease may contribute to the detection of a relapsing course of hyperthyroidism.  相似文献

PURPOSE: To evaluate the role of microbubble ultrasound contrast media in detecting stenosis of the post-liver transplant extrahepatic hepatic artery (HA) in the presence of the tardus parvus spectral Doppler waveform of the intrahepatic HA. MATERIALS AND METHODS: All post-liver transplant patients with a prolonged systolic acceleration time (SAT>0.08s) and/or a reduced resistant index (RI<0.50) of the HA (the tardus parvus waveform) on colour Doppler ultrasound (CDUS), were assessed with microbubble contrast medium for a focal arterial stenosis. Following microbubble contrast-enhanced CDUS, patients underwent arteriography or follow-up CDUS. RESULTS: A total of 2038 examinations were performed in 529 liver transplant recipients; 16 (3.02%) tardus parvus waveforms were identified. The median SAT of the intrahepatic HA was 0.18s (range 0.11-0.38s) and the RI 0.47 (range 0.22-0.58). No extrahepatic elevated peak systolic velocity (PSV), defined as above 1.00m/s, was detected on the baseline examinations. Following the administration of microbubble contrast, medium, PSV in the extrahepatic HA was elevated in 14 of 16 patients, (median=2.15m/s, range=1.44-3.10m/s); flow was not identified in two patients. Arteriography was performed in 10 patients and confirmed stenosis in eight (median grade of stenosis 93%, range 60-99%) and occlusion in two. The measured median PSV at contrast-enhanced CDUS in the stenosis group was 2.03m/sec (range 1.44-2.71m/sec). Repeat CDUS in six patients not undergoing arteriography showed resolution in four; one underwent re-transplantation before arteriography and one patient maintains a tardus parvus waveform. In transplant recipients undergoing arteriography during the study period (n=55), no hepatic artery stenosis without a tardus parvus waveform was seen. CONCLUSION: The tardus parvus waveform pattern is an excellent screening test for the presence of post-liver transplantation hepatic artery stenosis. There is only a limited role for microbubble ultrasound contrast agent in the presence of a tardus parvus waveform. It could be used following equivocal colour Doppler ultrasound, but arteriography will still be necessary.  相似文献

BACKGROUND AND PURPOSE: Several recent studies have shown that sonographic contrast agents may affect transcranial Doppler evaluation of the arterial peak systolic velocity (PSV). Some investigators reported an increase in PSV, and others reported no change in PSV compared with baseline values. This study was conducted to determine the effect of sonographic contrast agent on PSV measured in normal middle cerebral arteries. METHODS: Continuous spectral Doppler sonography was performed on the right middle cerebral artery of 20 participants with angiographically proven normal intracranial vasculature. Videotaping was performed in each case from the initiation of the administration of contrast medium until the effect of the contrast agent on the PSV subsided. The PSV values were normalized for each participant, were pooled, and were plotted as a function of time. RESULTS: PSV increased in all participants after the administration of contrast material; the mean maximum increase was 24+/-7.4% (mean +/- standard deviation) (range, 15-36%). The mean duration of PSV increase was 320+/-97 s (range, 165-465 s). CONCLUSION: The middle cerebral artery PSV increased substantially after the administration of contrast material. This effect needs to be considered if velocity thresholds developed for disease detection without the use of contrast materials are used when contrast agents are administered.  相似文献

PURPOSE: To prospectively evaluate the clinical response and hemodynamic changes in cavernosal arteries after oral administration of sildenafil without and with audiovisual sexual stimulation and to compare those responses with responses from intracavernosal injections of vasoactive agents. MATERIALS AND METHODS: Institutional review board approval and written informed consent were obtained. Thirteen consecutive patients (age range, 22-77 years; mean, 60.4 years) with erectile dysfunction were evaluated with clinical assessment and cavernosal duplex ultrasonography (US). The patients were examined at two sessions 3 weeks apart. First, each patient received 100 mg of sildenafil citrate orally and was examined 60 minutes later without any sexual stimulation. Each patient then underwent repeat clinical and duplex US assessment after audiovisual sexual stimulation. Three weeks later, the patients underwent identical clinical evaluation and duplex US after intracavernosal injection of a commercially available combination of papaverine, prostaglandin E1, and phentolamine. Clinical and duplex US data (ie, peak systolic velocity [PSV]) were examined by using the Wilcoxon signed rank test for matched pairs. RESULTS: At rest, the overall mean cavernosal artery PSV was 1.08 cm/sec and remained unchanged after intake of sildenafil without any audiovisual stimulation, with no clinical evidence of erection. With the addition of audiovisual sexual stimulation, eight (62%) of 13 patients had penile congestion or erection, and six (46%) had a PSV greater than 25 cm/sec. With intracavernosal injection of the combination of three drugs, all 13 patients achieved congestion or erection, and 10 (77%) had a PSV greater than 25 cm/sec. Both clinical and duplex US responses to intracavernosal injection were significantly greater than they were to sildenafil with audiovisual sexual stimulation (P = .04 and .003, respectively). CONCLUSION: Oral sildenafil with audiovisual sexual stimulation led to a significant clinical response and increment in blood flow in the cavernosal arteries. However, more patients responded to intracavernosal injection of the combination of three drugs than to sildenafil, and the clinical response was significantly better.  相似文献

PURPOSE: The aim of the study is to find out whether the measurement of peak systolic velocity in the inferior thyroid artery (ITA) is a valuable parameter to differentiate autoimmune thyroid diseases (hyper-, normo- or hypofunctional) and to evaluate the efficacy of medical treatment. MATERIAL AND METHODS: The ITA of 31 patients (eight with Graves' disease, 23 with subclinical hypothyroidism) was examined with color Doppler and pulsed Doppler. The final diagnosis was obtained by cytology and by hormonal and antibodies assays. The patients were monitorized by ultrasound for a period of 8 months. RESULTS: In all the patients with Graves' disease the peak systolic velocity was always over 150 cm/s, while in other autoimmune thyroiditis the peak systolic velocity was within the normal range, and never exceeding 65 cm/s. In the first group, the measurement taken in the ITA showed also the efficacy of the pharmacological treatment earlier and more reliably than the color Doppler pattern obtained in the parenchyma. CONCLUSIONS: The color Doppler measurement of the ITA seems to be a promising technique with low-cost and easy approach. In our experience, the color Doppler of the ITA could have a clinical role in the differential diagnosis of diffuse thyroid diseases and in the follow-up of the Graves' disease during medical treatment.  相似文献

PURPOSE: Duplex Doppler ultrasound allows the evaluation of thyroid gland vascular flow by spectral analysis assessment. The objective of this prospective study was to employ Doppler ultrasound to evaluate the possible functional changes produced in normal subjects following intravenous iodized contrast medium injection. MATERIAL AND METHODS: The study was performed on 30 non-consecutive subjects, enrolled according to some inclusion criteria: male gender, age ranging between 19 and 70 years, absence of chronic liver disease, absence of thyroid disease, absence of sonographic changes in the thyroid gland, negative history for recent intravenous contrast medium administration. All patients were scheduled for a contrast-enhanced CT study. A non-ionic contrast medium (iomeprol) was employed at 350 mgI/mL concentration, 130-140 mL volume, and 2-4 mL/sec. injection rate. The Doppler ultrasound examination was carried out immediately before and about 30 minutes after the CT study. The systolic velocity, diastolic velocity, the resistive index, and the pulsatility index were measured at the level of one of the four thyroid arteries. RESULTS: In most cases a slightly increasing trend of the four parameters considered was noted but none of these showed a statistically significant change. The mean systolic velocity was of 33 cm/sec. in the basal measurement and of 39 cm/sec. in the post-contrast measurement. The variation was positive in 17 cases, negative in 12, and absent in 1. The mean diastolic velocity was of 13 cm/sec. in the pre-contrast evaluation and of 14 cm/sec. in the second evaluation. The change was positive in 14 cases, negative in 12, and absent in 4. The mean resistive index was of 0.55 in the basal analysis and of 0.59 in the post-contrast one. The variation was positive in 20 patients, negative in 9, and absent in 1. The mean pulsatility index was of 0.99 in the basal measurement and of 1.14 in the second measurement. The change was positive in 23 subjects, negative in 5, and absent in 2. CONCLUSION: Our series shows that no significant change in Doppler spectral analysis of thyroid flow is evident after administration of iodized contrast media, at least in euthyroid subjects and with the contrast doses and the examination intervals employed in our study. Doppler ultrasound is confirmed as an accurate and objective procedure in the evaluation of thyroid gland flow and may represent a useful tool in functional studies, as well as clinical applications in the assessment of thyroid gland disorders.  相似文献

The Society of Radiologists in Ultrasound convened a multidisciplinary panel of experts in the field of vascular ultrasonography (US) to come to a consensus regarding Doppler US for assistance in the diagnosis of carotid artery stenosis. The panel's consensus statement is believed to represent a reasonable position on the basis of analysis of available literature and panelists' experience. Key elements of the statement include the following: (a) All internal carotid artery (ICA) examinations should be performed with gray-scale, color Doppler, and spectral Doppler US. (b) The degree of stenosis determined at gray-scale and Doppler US should be stratified into the categories of normal (no stenosis), <50% stenosis, 50%-69% stenosis, > or =70% stenosis to near occlusion, near occlusion, and total occlusion. (c) ICA peak systolic velocity (PSV) and presence of plaque on gray-scale and/or color Doppler images are primarily used in diagnosis and grading of ICA stenosis; two additional parameters, ICA-to-common carotid artery PSV ratio and ICA end-diastolic velocity may also be used when clinical or technical factors raise concern that ICA PSV may not be representative of the extent of disease. (d) ICA should be diagnosed as (i) normal when ICA PSV is less than 125 cm/sec and no plaque or intimal thickening is visible; (ii) <50% stenosis when ICA PSV is less than 125 cm/sec and plaque or intimal thickening is visible; (iii) 50%-69% stenosis when ICA PSV is 125-230 cm/sec and plaque is visible; (iv) > or =70% stenosis to near occlusion when ICA PSV is greater than 230 cm/sec and visible plaque and lumen narrowing are seen; (v) near occlusion when there is a markedly narrowed lumen at color Doppler US; and (vi) total occlusion when there is no detectable patent lumen at gray-scale US and no flow at spectral, power, and color Doppler US. (e) The final report should discuss velocity measurements and gray-scale and color Doppler findings. Study limitations should be noted when they exist. The conclusion should state an estimated degree of ICA stenosis as reflected in the above categories. The panel also considered various technical aspects of carotid US and methods for quality assessment and identified several important unanswered questions meriting future research.  相似文献

OBJECTIVE: A wide range of Doppler threshold values for carotid stenosis is found in the literature. We undertook this study to compare methods of derivation and to determine if an optimum strategy of threshold selection exists for a high-risk population. MATERIALS AND METHODS: From the sonograms of all patent internal carotid arteries, peak systolic velocity in the internal carotid artery (ICA(PSV)) and the ratio of peak systolic velocity in the internal carotid artery to that of the common carotid artery (ICA(PSV)/ CCA(PSV)) were compared with the percentage of angiographically determined stenosis. Receiver operating characteristic curves were generated for levels of stenosis > or =60% and > or =70%. Doppler thresholds were chosen on the basis of maximum accuracy and on the basis of > or =90% sensitivity and specificity. Patients were then segregated into symptomatic and asymptomatic cohorts, and the above process was repeated. An effectiveness analysis was also conducted using various Doppler thresholds. Thresholds derived using these three methods were compared and optimal values chosen. RESULTS. Of 333 carotid arteries that fit inclusion criteria, 132 were found in asymptomatic patients and 201 in symptomatic patients. Maximum accuracy, > or =90% sensitivity and specificity, and effectiveness analysis each produced different ranges of thresholds. We chose final thresholds that maintained patient outcome profiles. For asymptomatic patients at the > or =60% stenosis level, thresholds were ICA(PSV) = 200 cm/sec and ICA(PSV)/CCA(PSV) = 3.0. For symptomatic patients with stenosis > or =70%, thresholds were ICA(PSV) = 175 cm/sec and ICA(PSV)/CCA(PSV) = 2.5. CONCLUSION: Considerable latitude exists in the choice of carotid Doppler thresholds. We propose a rational strategy for threshold selection based on a combination of three commonly used methods. Our observations indicate that it appears advisable to consider symptomatic and asymptomatic patients separately and to apply appropriately derived thresholds.  相似文献

PURPOSE: To determine the optimal peak systolic velocity (PSV) threshold at duplex ultrasonography (US) required to establish the need for carotid endarterectomy in symptomatic patients on the basis of the long-term cost-effectiveness outcomes of diagnostic testing and subsequent treatment. MATERIALS AND METHODS: From January 1997 through January 2000, a prospective medical ethics committee-approved multicenter study was conducted. After giving informed consent, patients with amaurosis fugax, transient ischemic attack, or minor stroke who underwent duplex US and digital subtraction angiography were included in the study. Selective ipsilateral carotid angiograms were obtained in at least three planes. Arteries that were nearly or totally occluded at duplex US were excluded because the PSV cannot be reliably measured in these vessels. Receiver operating characteristic (ROC) curves were constructed for the diagnoses of 70%-99% and 50%-99% stenoses. Optimal likelihood ratios were calculated on the basis of lifetime costs and quality-adjusted life-years derived at cost-effectiveness analysis and the prevalence of disease. The associated optimal sensitivities, specificities, and PSV thresholds were derived from the ROC curves. RESULTS: In this clinical study, 350 patients were included. The nonoccluded arteries in a total of 236 patients were assessable for ROC analysis. For the diagnosis of 70%-99% stenosis, the optimal likelihood ratio was 0.21, which was associated with a PSV threshold of 220 cm/sec, a sensitivity of 97% (127 of 131 patients; 95% confidence interval [CI]: 94%, 100%), and a specificity of 48% (50 of 105 patients; 95% CI: 38%, 57%). For the diagnosis of 50%-99% stenosis, the optimal likelihood ratio was 0.38, which was associated with a PSV threshold of 180 cm/sec, a sensitivity of 95% (182 of 191 patients; 95% CI: 92%, 98%), and a specificity of 69% (31 of 45 patients; 95% CI: 55%, 82%). CONCLUSION: On the basis of the lifetime outcomes of diagnostic testing and subsequent treatment, the optimal PSV thresholds for the diagnosis of 70%-99% and 50%-99% carotid artery stenoses in patients with amaurosis fugax, transient ischemic attack, or minor stroke were 220 cm/sec and 180 cm/sec, respectively.  相似文献

Eighty-five men with 92 vascular grafts placed for peripheral vascular disease of the lower extremity underwent a total of 264 examinations with duplex Doppler over a 2.5-year period. In 64 patients who underwent more than one examination, the total follow-up encompassed 740 months. In 220 native femoral arteries (96.0%) the peak systolic velocity (PSV) was higher than that in the graft. Arteriovenous shunting was associated with a normal PSV and a markedly elevated diastolic component at spectral analysis. Focal fluid collections were common initially near the graft and usually disappeared uneventfully. An average PSV of 32 cm/sec or less was always associated with impending occlusion. The sensitivity of an average PSV of 40 cm/sec or less to indicate impending graft occlusion by the next visit was only 33%; the specificity, 94%. At initial examination, stenoses were associated with high PSV focally in the graft or low PSV with absent diastolic flow.  相似文献

OBJECTIVE: The aim of this study is to assess the effect of long-term antithyroid drug intake on the success rate of iodine-131 (131I) treatment of Graves' hyperthyroidism, and to explore other clinical/laboratory factors that may predict/affect the treatment outcome. MATERIALS AND METHODS: Fifty-eight patients with Graves' disease were referred for radioactive iodine therapy after failure of medical treatment, which was given for at least 6 months. Antithyroid drug (carbimazole) was stopped for at least 2 days before administration of a fixed dose of 370 MBq. Treatment outcome was determined at the end of 1-year follow-up after iodine administration. Treatment success was reported if the thyroid hormonal profile indicated euthyroid or hypothyroid state. RESULTS: One year after 131I administration, 19% of our patients were still hyperthyroid (treatment failure), 15.5% became euthyroid and 65.5% were hypothyroid (treatment success, 81%). No statistically significant correlation was found between treatment outcome and patient's age at the time of I administration (P=0.20); duration of medical treatment before 131I administration (P=0.22) and duration of stoppage of medical treatment before 131I intake (P=0.15). In contrast, there was significant association between treatment outcome and pretreatment Tc99m-thyroid uptake (P=0.0001), thyroid size (P=0.001) and TSH level (P=0.04). Using receiver operator characteristic curve analysis, we generated a cut-off value for thyroid uptake (18%) and thyroid weight (70 g) to predict response to 370 MBq of 131I. The 18% thyroid uptake cut-off value predicted treatment outcome with 93.6% sensitivity, 100% specificity and 94.8% accuracy, whereas the 70 g thyroid weight predicted treatment outcome with sensitivity, specificity and accuracy of 80.9, 72.7 and 79.3%, respectively. CONCLUSION: Long-term carbimazole treatment will not increase the failure rate of 131I treatment in patients with Graves' disease if the drug was discontinued for at least 2 days before iodine administration. A fixed dose of 370 MBq is efficient in patients with Tc99m-pertechnetate thyroid uptake less than 18% and gland weight less than 70 g. Patients with larger goitres and/or higher thyroid uptake level will probably need a higher dose of radioactive iodine.  相似文献

PURPOSE: To examine the variability of Doppler measurements along the extra-cranial courses of the nondiseased common carotid artery (CCA) and internal carotid artery (ICA) and determine the effect of this variability on assessment of carotid arterial stenosis. MATERIALS AND METHODS: During the study period, 580 patients were referred for carotid arterial ultrasonography (US), including Doppler measurements of flow velocities in the proximal, middle, and distal portions of the CCA, in the bulb, and in the proximal and distal portions of the ICA. Eighty-five patients (average age, 59 years) with normal ICAs and CCAs formed the cohort for this study. RESULTS: The range of peak systolic velocity (PSV) measurement (maximum minus minimum) averaged 20 cm/sec +/- 13 in the CCA and 15 cm/sec +/- 13 in the ICA. ICA/CCA velocity ratios varied, depending on the CCA measurement location. In five arteries, PSV ratios exceeded a threshold of 1.8 (suggesting > or = 60% stenosis); in 23 arteries, end diastolic velocity ratios exceeded a threshold of 2.4 (also suggesting > or = 60% stenosis). Right-to-left CCA PSV ratios were abnormal in up to 26 patients (suggesting > 50% ICA stenosis), depending on where CCA measurements were obtained. When the CCA ratios were obtained at the same level, 16 were in the abnormal range. CONCLUSION: Variability of Doppler measurements in the CCA and ICA in patients without visible disease is substantial and could lead to inaccuracies in carotid arterial stenosis assessment.  相似文献

Purpose: To determine the accuracy and optimal threshold values of duplex ultrasonography (US) in assessing restenosis of renal artery stents. Methods: Twenty-four consecutive patients with 33 renal arteries that had previously been treated with placement of a Palmaz stent underwent duplex US prior to intraarterial digital subtraction angiography (DSA), which was the reference standard. Diagnostic accuracy of in-stent peak systolic velocity (PSV) and reno-aortic ratio (RAR = PSV renal stent/PSV aorta) in detecting > 50% in-stent restenosis were evaluated by the receiver operating characteristic curve. Sensitivity and specificity were determined using the optimal threshold values, and using published threshold values: RAR > 3.5 and in-stent PSV > 180 cm/sec. Results: Six examinations were technically inadequate. Nine stents had residual or restenosis > 50% at DSA. The two duplex parameters were equally accurate since areas under the curves were similar (0.943). With optimal threshold values of 226 cm/sec for PSV and 2.7 for RAR, sensitivities and specificities were 100% and 90%, and 100% and 84%, respectively. Using the published duplex criteria resulted in sensitivities and specificities of 100% and 74% for PSV, and 50% and 89% for RAR. Conclusion: Duplex US is a sensitive modality for detecting in-stent restenosis if laboratory-specific threshold values are used.  相似文献

Fast cine phase contrast magnetic resonance imaging (fast cine phase contrast MRI) can measure phasic coronary flow velocity in humans. The purpose of this study was to compare the coronary flow velocity reserves measured by MR IMAGING with those obtained by Doppler guide wire. Nineteen patients with ischemic or valvular heart disease were studied. Fast cine phase contrast MR images of the left anterior descending (LAD) artery were acquired during breath-hold time in the basal state and after administration of dipyridamole. Flow velocity in the LAD artery was also measured with Doppler guide wire before and after venous injection of dipyridamole in all subjects. Flow velocity in the coronary artery measured with MR IMAGING in the basal state (12.5 +/- 4.9 cm/sec) was significantly lower than that obtained with Doppler guide wire (32.4 +/- 12.1 cm/sec, P < 0.01). However, MR assessments of coronary flow velocity reserve showed a good linear correlation with those measured by Doppler guide wire (r = 0.91). In conclusion, fast cine phase contrast MR imaging is a useful technique, which can provide a noninvasive assessment of flow reserve ratios in patients with coronary artery disease. J. Magn. Reson. The administration of a contrast agent to obtain optimal images at three-dimensional computed tomographic (CT) angiography of the hepatic artery by using multi-detector row helical CT was investigated. Three-dimensional CT angiographic images were evaluated visually, and quantitative evaluation was performed by measuring the postcontrast CT number of the aorta. The injection rate of 5 mL/sec was significantly superior to that of 4 mL/sec in both evaluations. At administration of a contrast agent with 300 or 350 milligrams of iodine per milliliter, there were no significant differences in either evaluations. The preferable injection rate to obtain sufficient three-dimensional CT angiographic data was 5 mL/sec.  相似文献

Hepatocellular carcinoma (HCC) is characterized by tumor vascularization from the hepatic artery. The objective of our work was to compare color Doppler ultrasonography (CDU), including power Doppler ultrasonography (PDU) with radiographic imagings with contrast medium in regard to the detection of the arterial tumor vascularity of small hepatocellular carcinomas (HCC). We examined 42 small HCC lesions 2 cm or less in diameter in 37 patients for arterial tumor vascularity by conventional CDU, PDU, dynamic computed tomography (dCT), digital subtraction angiography (DSA), and CT hepatic arteriography (CTA). Color images were detected in 25 (59.5%) and 28 (66.7%) of the 42 lesions with conventional CDU and PDU, respectively, and tumor vascularity was detected in 26 (61.9%) by dCT, 23 (54.8%) by DSA, and 29 (69.0%) by CTA. Tumor vascularity could be detected in 51.9% by PDU and CTA, more than by conventional CDU, dCT, and DSA (44.4, 44.4, and 33.3%, respectively) in well-differentiated HCC, whereas the detection rates by these techniques were almost equal (86.7% by CDU, 93.3% by PDU, dCT, and DSA, 100% by CTA, respectively) in moderately and poorly differentiated HCC. PDU is superior to CDU, dCT and DSA and equal to CTA for the detection of tumor vascularity in small HCC, particularly in well-differentiated HCC.  相似文献

PURPOSE: To evaluate the clinical effectiveness of a simplified dosimetric approach to the iodine-131 treatment of hyperthyroidism due to Graves' disease or uninodular and multinodular toxic goiter. MATERIAL AND METHODS: We enrolled 189 patients with biochemically confirmed hyperthyroidism and performed thyroid ultrasonography and scintigraphy obtaining the diagnosis of Graves' disease in 43 patients, uninodular toxic goiter in 57 patients and multinodular toxic goiter in 89 patients. In 28 patients we found cold thyroid nodules and performed fine-needle aspiration with negative cytology for thyroid malignancy in all cases. Antithyroid drugs were stopped 5 days till radioiodine administration and, if necessary, restored 15 days after the treatment. Radioiodine uptake test was performed in all patients and therapeutic activity calculated to obtain a minimal activity of 185 MBq in the thyroid 24 hours after administration. The minimal activity was adjusted based on clinical, biochemical and imaging data to obtain a maximal activity of 370 MBq after 24 hours. RESULTS: Biochemical and clinical tests were scheduled at 3 and 12 months posttreatment and thyroxine treatment was started when hypothyroidism occurred. In Graves' disease patients a mean activity of 370 MBq (distribution 259-555 MBq) was administered. Three months after treatment and at least 15 days after methimazole discontinuation 32 of 43 (74%) patients were hypothyroid, 5 of 43 (11%) euthyroid and 6 of 43 (15%) hyperthyroid. Three of the latter were immediately submitted to a new radioiodine administration while 32 hypothyroid patients received thyroxine treatment. One year after the radioiodine treatment no patient had hyperthyroidism; 38 of 43 (89%) were on a replacement treatment while 5 (11%) remained euthyroid. In uni- and multinodular toxic goiter a mean activity of 444 MBq (distribution 259-555 MBq) was administered. Three months posttreatment 134 of 146 (92%) patients were euthyroid and 12 of 146 (8%) patients hyperthyroid. Two patients were immediately submitted to a new radioiodine administration. One year posttreatment 142 of 146 (97%) patients were euthyroid while only 4 of 146 (3%) patients showed TSH levels above the normal range. Only 2 of them required thyroxine treatment. CONCLUSIONS: The simplified dosimetric method illustrated in our paper is very effective in clinical practice because it permits to avoid resorting to sophisticated but also imprecise quantitative methods. Hypothyroidism should not be considered as a major collateral effect of radioiodine treatment, particularly in Graves' disease. In fact, the pathogenesis of the disease requires an ablative treatment with both surgery and radioidine treatment and the control of hyperthyroidism and the prevention of relapse are the major clinical targets. Vice versa, hypothyroidism was very uncommon in uni- and multinodular toxic goiter when our dosimetric approach was applied.  相似文献

 目的 探讨脾动脉环阻术(splenic artery banding, SAB)预防肝移植术后脾动脉盗血综合征(splenic artery ateal syndrome, SASS)的效果及其安全性。方法 2004-01至2013-12,对127例肝硬化、脾脏增大、术前脾动脉直径/肝动脉直径(SA/CHA)≥1.5、术中HA血流<30 cm/s的肝移植患者(SASS高危者)采用预防性SAB(干预组),观察其预防SASS效果及安全性。分析术中HA血流≥30 cm/s未接受环阻术的191例患者(对照组)手术前后情况,同时对两组部分资料进行比较。结果 干预组患者处理后肝动脉(CHA)血流量立即改善,环阻前(19.34±5.45) cm/s,环阻后(45.89±9.13)cm/s, P<0.001;阻力指数(RI)全部恢复到正常水平(0.5~0.8) ,移植术后无SASS发生,亦未观察到移植术后受者动脉相关并发症。而对照组术后发现SASS 17例(8.90%),其中11例继发肝动脉血栓形成。结论 高风险患者预防性SAB具有可靠的疗效和安全性。  相似文献

PURPOSE: The aim of this study was to optimize the protocol for multislice spiral CT angiography and to assess the ability of the technique to detect the anatomy of splanchnic vessels, using volume rendering as reconstruction algorithm. MATERIAL AND METHODS: The anatomy of splanchnic vessels was studied in 19 patients (11 men, 8 women, age range 38-83 years) undergoing CT of the abdomen and pelvis. All examinations were performed with a multislice spiral CT scanner (Somatom Plus 4 Volume Zoom; Siemens, Erlangen, Germany) using the following parameters: collimation, 1 mm; slice width, 1 mm; table feed, 8 mm/sec; reconstruction interval, 1 mm; mAs, 120; kVp, 120. Before the study, the patients received 800 ml of water as oral contrast agent to opacify the stomach and small bowel. A nonionic contrast medium (130-140 ml; Xenetix 350, Guerbet, Aulnay-Sous-Bois, France) was infused intravenously at a rate of 3-5 ml/sec. Two scans of the abdomen and pelvis were obtained at 20-25 sec (arterial phase) and 60-65 sec (venous phase) after starting contrast medium injection. Image elaboration was performed using Vitrea 2.2 (Vital Images; Minneapolis, Minn., USA), a software with volume-rendering capabilities. RESULTS: All major arterial (celiac trunk, superior mesenteric artery, and inferior mesenteric artery) and venous (portal vein, superior mesenteric vein, inferior mesenteric vein, and splenic vein) vessels could be evaluated with excellent detail in all patients. Side branches, including small collaterals, could also be easily visualized. Volume rendered images always provided better understanding of the 3D anatomic relationships among splanchnic vessels and surrounding organs. DISCUSSION AND CONCLUSIONS: Multislice spiral CT angiography allows for optimal depiction of the anatomy of splanchnic vessels. It can be anticipated that the clinical applications of this imaging modality in the study of splanchnic vessels will be greatly expanded. However, further studies are necessary to rigorously compare the results of multislice spiral CT angiography with conventional angiography in terms of diagnostic accuracy.  相似文献
