BACKGROUND: Gene promoter methylation is an epigenetic event leading to gene silencing. This mechanism is particularly relevant in cancer since it can interfere with the activity of specific "suppressor" genes. AIM: To evaluate promoter methylation of CDH1, p16, APC, MLH1, and COX2 in patients with H. pylori (Hp) infection before and after eradication. METHODS: Fifty-seven dyspeptic outpatients who had never performed previous endoscopy or Hp testing and treatment underwent clinical interview, endoscopy with three paired gastric biopsy specimens from the antrum, angulus, and corpus, and (13)C-urea breath test (UBT). Biopsies were scored for the presence of Hp and intestinal metaplasia (IM). DNA methylation of five tumor-related genes (CDH1, p16, MLH1, APC, and COX2) was evaluated by methylation-specific PCR in each biopsy. Infected patients were given a standard eradicating treatment and, after 1 yr, underwent endoscopy with biopsies and UBT. RESULTS: Hp infection was found in 45 patients. IM was detected in 17 out of 45 (38%) infected patients. Mean number of methylated genes was 0, 1.1 +/- 0.9, and 1.6 +/- 0.9 among the 12 Hp-/IM-, the 28 Hp+/IM-, and the 17 Hp+/IM+ patients, respectively (P < 0.0001). Specifically, promoter hypermethylation of CDH1, p16, APC, MLH1, and COX2 was found in 68%, 25%, 7%, 0%, and 14% of Hp+/IM- patients and in 71%, 29%, 35%, 12%, and 12% of Hp+/IM+ patients. No significant difference was found among the three groups of patients as far as age, smoking, alcohol, meat and vegetable consumption, and family history of gastric cancer were considered. Twenty-three out of 45 (51%) infected patients underwent the 1-yr follow-up endoscopy: 17 out of 23 (74%) were successfully eradicated. After Hp eradication, CDH1, p16, and APC methylation significantly decreased while COX2 methylation completely disappeared. Conversely, MLH1 methylation did not change significantly in patients with IM. CONCLUSION: Hp infection is associated with promoter methylation of genes which are relevant in the initiation and progression of gastric carcinogenesis. While CDH1 methylation seems to be an early event in Hp gastritis, MLH1 methylation occurs late along with IM. Hp eradication is able to significantly reduce gene methylation thus delaying or reversing Hp-induced gastric carcinogenesis.  相似文献

目的 观察胃癌高发区中幽门螺杆菌(Hp)阳性者根除Hp5年后胃黏膜组织的炎症变化,以探讨。Hp感染与胃黏膜组织炎症及胃癌的关系。方法 对胃癌高发区山东省烟台市成年人群随机抽样1006例,做内镜、快速尿素酶试验及胃窦、胃体部黏膜组织学检查,将Hp阳性者随机分为治疗组(奥美拉唑20mg、克拉霉素500mg、阿莫西林1000mg)及对照组,2组入选者分别于1年后、5年后进行内镜复检,本研究是将5年后复查胃镜及相同部位胃黏膜组织病理检查与5年前结果进行比较并做χ^2检验。结果 552例Hp阳性者随机分为治疗组及对照组各276例,5年后Hp持续阴性者161例,持续阳性者198例。2组结果统计显示:(1)入选前2组胃窦部炎症及活动度发生率与体部相比差异无显著性,P值分别为0.105及0.084,但萎缩及肠化生发生率明显高于体部,P值均为0.000;(2)根除Hp 5年后胃窦、胃体部炎症及活动度均明显减轻,P值均为0.000;(3)根除Hp5年后胃窦部肠化生减轻或未进展,而Hp持续阳性组肠化生发生率明显增加,P=0.032;(4)根除Hp 5年后窦、体部萎缩改善差异无显著性,两组比较P值分别为0.223及0.402。结论 根除Hp可使胃慢性炎症及活动度明显减轻,窦部肠化生得到显著控制,溃疡病发病明显减少;持续Hp感染可使萎缩及肠化生呈进行性加重。  相似文献

目的:研究高清晰放大胃镜下幽门螺杆菌(Hp)相关性胃炎胃粘膜的特点,观察Hp感染根除治疗后胃粘膜的变化。方法:使用Fujinon EG485ZH85万像素高清晰放大电子胃镜,对50例门诊病例经常规胃镜检查后转换高清晰放大功能,详细观察胃体和胃窦粘膜的微细结构,判断Hp感染情况。结果:50例中Hp阳性26例,普通胃镜判断Hp感染的敏感性为34.62%,准确性为58.00%,阳性拟然比为2.08,阴性拟然比为0.78;高清晰放大胃镜判断Hp感染的敏感性为69.23%,准确性为78.00%,阳性拟然比为5.54,阴性拟然比为0.35。两种胃镜检查的Kappa值分别为0.176和0.563。结论:Hp相关性胃炎的胃镜下表现主要是胃小凹不均匀发红,小凹变粗、变宽,集合静脉的模糊、混乱或消失。高清晰放大胃镜判断Hp感染的准确性明显高于普通胃镜。  相似文献

Background: Duodenal gastric metaplasia (DGM) is commonly found in association with Helicobacter pylori (Hp)-associated gastritis in adults. DGM is also considered a risk factor for duodenal ulcer development. The prevalence of DGM in children and its association with gastritis, duodenitis, or the presence of Hp organisms is not clear. We investigated the prevalence of DGM in children and explore its association with several possible risk factors, including age, gender, gastritis, duodenitis, or Hp presence in the gastric antrum. Methods : A retrospective analysis of 173 upper endoscopy procedures performed between 1993 and 1995 at Cabell Huntington Hospital, Huntington, WV, was done. Gastric and duodenal biopsies were stained with Giemsa for Hp detection, periodic acid-Schiff for DGM, and hematoxylin and eosin for histologic assessment. Gastric mucosal inflammation was graded according to Sydney criteria. Results : Duodenal gastric metaplasia was identified in 23 of 173 (13%) patients. Duodenitis hut not age, gender, gastritis, or the presence of Hp in the gastric antrum was associated with DGM development. In 4 of 23 DGM foci, Hp was identified. Conclusions : In children, DGM is not the consequence of Hp infection.  相似文献

[目的]观察以胃舒散为主的三联疗法(胃舒散、呋喃唑酮和克拉霉素)治疗幽门螺杆菌(Hp)阳性慢性萎缩性胃炎(CAG)的临床效果及其对核因子-κB(NF-κB)表达的影响。[方法]41例Hp阳性CAG患者服用胃舒散2.0g,呋喃唑酮0.1g,各3次/d,克拉霉素0.25g,2次/d,1周后再继服胃舒散4周。治疗前及治疗结束1年后行内镜及病理组织学检查,取活检观察病理组织学改变及NF-κB表达变化,采用银染色法、^14C-尿素呼气试验或快速尿素酶试验检测Hp。[结果]三联疗法结束1年后,Hp根除率为85.4%;根除Hp能显著减轻患者胃窦部慢性炎症(P〈0.05)和活动程度(P〈0.01),下调NF-κB表达(P〈0.01),但胃炎的萎缩和肠化生等病理无明显改变。[结论]以胃舒散为主的三联疗法对Hp有较高根除率。根除Hp可抑制NF-κB的表达,减轻活动性炎症,但近期观察对萎缩、肠化生等病理改变无明显作用。  相似文献

Background and Aim: Many studies have shown that Helicobacter pylori infection is associated with chronic gastritis, peptic ulcers and gastric carcinoma in adults. However, little is known about these associations in the younger population. The aim of this study was to clarify endoscopic and histological findings in H. pylori‐positive young adults. Methods: Two hundred consecutive outpatients younger than 29 years old undergoing esophago‐gastroduodenal endoscopy at four hospitals between 2001 and 2002 were eligible for this study. At endoscopy, three biopsy specimens were obtained from the mid‐antrum, the angulus and the mid‐corpus. Endoscopic and histological interpretations were based on the updated Sydney System. H. pylori infection was determined by histology and serology. Results: The rates of H. pylori infection were 1.2% (1/86) in normal, 95.8% (46/48) in gastritis, 97.8% (45/46) in duodenal ulcers, 100% (17/17) in gastric ulcers, and 100% (3/3) in gastric carcinomas. Endoscopic findings in 112 H. pylori‐positive patients were 25 normal (22.3%), 38 atrophy (33.9%), 18 erosion (16.1%), and 31 nodularity (27.7%). Histological findings of H. pylori‐positive patients in the anturm revealed mononuclear cell and neutrophil infiltration in 100%, and atrophy in 27.7%. Histological findings of H. pylori‐positive patients in the corpus revealed mononuclear cell infiltration in 75%, neutrophil infiltration in 60.7%, and atrophy in 28.6%. Conclusions: The study shows that H. pylori infection is strongly associated with chronic gastritis and peptic ulcers, and that histological corpus gastritis was found with high frequency in Japanese young adults.  相似文献

目的探讨感染幽门螺旋杆菌(Hp)及Hp阴性患者胃镜下黏膜表现的差异,旨在为临床诊疗工作提供参考依据。 方法回顾性分析2017年12月至2019年3月经天津医科大学附属第二医院消化内科门诊收治的14C呼气试验(+)及14C呼气试验(-)并同时间段行胃镜检查的受检者共1 792例内镜下观察,对胃黏膜表现进行记录,并分析Hp阳性及Hp阴性的胃镜下黏膜差异。 结果(1)不同Hp感染状态胃镜下黏膜表现差异分析上,单因素分析显示,消化性溃疡、点状发红、斑片状发红、线状红斑、黏膜出血、结节样改变、黏膜水肿、黏膜萎缩、皱襞肿大、增生性息肉在Hp感染者及Hp阴性患者的胃镜下黏膜表现有显著的统计学差异(P<0.05),且以上表现在Hp感染者中发生率高。反流性食管炎(RE)、规则的集合静脉(RAC)、胃底腺息肉在Hp阴性者中发生率较Hp阳性者高,有明显统计学差异(P<0.05)。黏膜糜烂在Hp阳性者及Hp阴性者中无显著统计意义(P>0.05),但胃窦部隆起型糜烂在Hp感染者中发生率明显高于未感染Hp患者,差异有统计学意义(P<0.05)。多因素Logistic回归分析显示,RE、RAC、胃底腺息肉在Hp阴性者中发生率高,差异有统计学意义(P<0.05)。消化性溃疡、点状发红、结节样改变、黏膜水肿、黏膜萎缩、线状红斑、斑片状发红、胃窦部隆起型糜烂在Hp感染者中发生率高,差异有统计学意义(P<0.05)。(2)不同胃镜下黏膜表现对于预测Hp感染与否的诊断价值分析上,RAC对预测Hp阴性的AUC、敏感性、特异性、阳性预测值及阴性预测值分别为0.750、65.6%、84.5%、77.8%及74.7%;胃底腺息肉预测Hp阴性的特异性、阳性预测值分别为99.8%、96.4%。RE患者诊断Hp阴性的特异性高达94.7%;黏膜点状发红作为预测Hp阳性的敏感性为80%,特异性为74.9%,AUC为0.774,阳性预测值为79.3%;结节样改变对于预测Hp阳性其特异性可高达99.9%,阳性预测值高达92.3%。消化性溃疡、增生性息肉、黏膜萎缩及线状红斑对于预测Hp感染的特异性分别为90.8%、98.2%、86.3%及85.1%,阳性预测值分别为78.3%、71.4%、80.4%及75%。 结论(1)RE、RAC、胃底腺息肉为Hp阴性的独立的胃镜下黏膜征象;消化性溃疡、点状发红、结节样改变、黏膜水肿、黏膜萎缩、线状红斑、斑片状发红、胃窦部隆起型糜烂为Hp阳性独立的胃镜下黏膜征象;(2)RAC、黏膜点状发红预测Hp感染与C14呼气试验结果的符合率较高。结节样改变、黏膜萎缩、消化性溃疡、胃窦部隆起型糜烂、线状红斑作为预测Hp感染的表现有较大价值;黏膜水肿、皱襞肿大、增生性息肉、黏膜出血、斑片状发红作为单独诊断Hp阳性的指标不推荐;胃底腺息肉对于预测Hp阴性有较大价值;RE作为单独诊断Hp阴性的指标不推荐。  相似文献

BACKGROUND: The purpose of this study was to determine the usefulness of magnifying endoscopy for the diagnosis of Helicobacter pylori-induced histopathologic gastritis. METHODS: A total of 92 patients scheduled to undergo routine endoscopic examination were enrolled. After routine endoscopic examination, 3 sites in the stomach were studied by magnified observation. Visualized collecting venulae were classified into the following 3 patterns: regular, irregular, and obscured. The sites observed by magnifying endoscopy were assessed histopathologically with an Updated Sydney System; 4 morphologic parameters (activity, inflammation, atrophy, metaplasia) were assessed and graded from 0 to 3. RESULTS: The regular pattern cases were negative for H pylori infection at all sites observed by magnifying endoscopy (antrum greater curve, 0/11; body greater curve, 0/24; body lesser curve, 0/23). The scores for all 4 morphologic parameters were significantly lower in the regular pattern group than the irregular and obscured groups (p < 0.01). The value of the atrophy parameter in the irregular group was significantly higher than that in the obscured group (p < 0.05 for a single test of hypothesis; correction for multiple testing of data removed significance). CONCLUSION: Visibility of collecting venulae in the gastric mucosa is influenced by H pylori-induced histopathologic gastritis. Magnifying endoscopy is useful for the diagnosis of histopathologic gastritis.  相似文献

OBJECTIVES: To clarify the histological features and endoscopic classifications of nodular gastritis (NG). METHODS: Overall 40 996 patients who had undergone an upper gastrointestinal endoscopy were enrolled. NG is defined as a uniform and diffuse protrusion from the antrum to angulus, which has two types at endoscopy: nodular (N) and granular (G). Three biopsy specimens were taken from the antrum, angulus and corpus. The histological features were evaluated using the updated Sydney System (USS). The topography of gastritis (antrum‐predominant, pangastritis or corpus‐predominant) and the prevalence of lymphoid follicles were also investigated. RESULTS: Overall 89 patients (0.22%) were diagnosed with NG, which tended to decrease in prevalence over age and predominantly affected women. All the patients were Helicobacter pylori‐positive. Among these, 65 patients underwent biopsy. Activity and inflammation were mostly moderate or severe, while intestinal metaplasia and atrophy were mostly absent at all three sites. Pangastritis was the most frequent (72%) type of gastritis. Lymphoid follicles were found in 69% at the antrum, 65% at the angulus and 51% at the corpus. There were no significant differences between N and G types in clinical features, USS scores, topography of gastritis, and prevalence of lymphoid follicles. CONCLUSIONS: Atrophy and intestinal metaplasia are rare but activity and chronic inflammation are severe at the antrum, angulus and corpus in NG. Pangastritis is the commonest type of gastritis. Lymphoid follicles affect everything up to the upper corpus, contrary to endoscopic protrusion found only at angulus. There was no correlation with pathological features between N and G types.  相似文献

The successful isolation of C. pyloridis from human gastric mucosa has renewed interest in these bacteria and their role as a causative agent for gastritis, and possible causal relationship between chronic gastritis and peptic ulceration. To determine the incidence of C. pyloridis in gastric biopsies we studied 51 consecutive Brazilian patients with a wide range of alimentary disorders presenting for endoscopy. At least three biopsies were taken from each site: antrum, any ulcer or cancer. Microbiological and histological studies were performed to identify the bacteria. The organism was found in 40/51 (78%) of patients. These was a close correlation between culture (100%), Gram (90%) and Gimenez staining (80%) in identifying the bacteria. All C. pyloridis positive patients had histological evidence of antral chronic gastritis (active or quiescent) even if the endoscopic appearance looked normal. All peptic ulcer patients (n = 17) showed C. pyloridis in the antrum. In the duodenum the bacteria were mainly seen in gastric type of mucosa. Our findings support the hypothesis that C. pyloridis is etiologically related to gastritis and possibly peptic ulceration.  相似文献

The transient lower oesophageal sphincter relaxations which allow reflux may be due to altered afferent pathways from the fundus. We aimed to determine whether fundal inflammation is the underlying cause. Two endoscopic biopsies were taken from each of the gastric antrum and fundus in 25 asymptomatic controls with a normal endoscopy (median age 54 range 13-83 years), and 33 patients with erosive oesophagitis (median age 52, 11-78 years). No patient had taken acid suppression therapy or antibiotics for at least 1 month. Sections were stained with haematoxylin and eosin and Giemsa stain and examined in a blinded fashion by one pathologist for the presence of gastritis (Sydney classification) and Helicobacter pylori. Chronic gastritis was common in both groups, but was usually mild. In Helicobacter pylori-negative subjects, there was significantly less chronic gastritis in the antrum and the fundus in oesophagitis patients than in controls (p < 0.05). When present, gastric atrophy was usually antral and mild in severity. There was no difference in the incidence of gastric atrophy in patients with oesophagitis compared with controls (24% compared with 40%; p > 0.05). Chronic gastritis is not more common in patients with oesophagitis, and is unlikely to play a part in the pathogenesis of this disease.  相似文献

背景幽门螺杆菌(H.pylori)感染已被确认为慢性胃炎的主要病因,由慢性非萎缩性胃炎、慢性萎缩性胃炎至肠化生,经过数十年最终可能导致胃癌发生。目的评价H.pylori感染与胃镜检查正常者、慢性胃炎、早期胃癌和进展期胃癌患者胃黏膜组织学特点的关系。方法在受检者胃窦大弯侧、胃体大弯侧和胃角处各取一块黏膜活检标本,以Giemsa染色和免疫组化染色检测H.pylori感染情况;以HE染色评价胃黏膜炎症、活动性、萎缩和肠化生情况。结果慢性胃炎、早期胃癌和进展期胃癌患者的总体H.pylori感染率均显著高于胃镜检查正常者(52.4%、52.4%和81.2%对44.9%,P<0.05),慢性胃炎与早期胃癌患者的感染率无显著差异,但均显著低于进展期胃癌患者(P<0.05)。胃镜检查正常和慢性胃炎组H.pylori感染者的胃黏膜炎症、活动性、萎缩和肠化生检出率均显著高于无感染者(P<0.05);早期胃癌和进展期胃癌组H.pylori感染者的炎症活动性检出率显著高于无感染者(P<0.05),而炎症、萎缩和肠化生检出率与无感染者无显著差异。结论由H.pylori感染引起的胃黏膜慢性炎症、萎缩和肠化生可能在胃癌的发生、发展过程中起直接或间接作用。  相似文献

AIM: To develop a serum or histological marker for early discovery of gastric atrophy or intestinal metaplasia. METHODS: This study enrolled 44 patients with gastric adenocarcinoma, 52 patients with duodenal ulcer, 14 patients with gastric ulcer and 42 consecutive healthy adults as controls. Each patient received an endoscopy and five biopsy samples were obtained. The degrees of histological parameters of gastritis were categorized following the Updated Sydney System. Anti-parietal cell antibodies (APCA) and anti-Helicobacter pylori (H pylori) antibodies (AHPA) were analyzed by immunoassays. H pylori infection was diagnosed by rapid urease test and histological examination. RESULTS: Patients with gastric cancer and gastric ulcer are significantly older than healthy subjects, while also displaying higher frequency of APCA than healthy controls. Patients with positive APCA showed higher scores in gastric atrophy and intestinal metaplasia of corpus than patients with negative APCA. Patients with positive AHPA had higher scores in gastric atrophy, intestinal metaplasia, and gastric inflammation of antrum than those patients with negative AHPA. Elderly patients had greater prevalence rates of APCA. Following multivariant logistic regression analysis, the only significant risk factor for antral atrophy is positive AHPA, while that for corpus atrophy is positive APCA. CONCLUSION: The existence of positive APCA correlates with glandular atrophy in corpus and the presence of positive AHPA correlates with glandular atrophy in antrum. The existence of serum APCA and AHPA betokens glandular atrophy and requires further examination for gastric cancer.  相似文献

目的 探讨幽门螺杆菌 (Hp)感染及慢性萎缩性胃窦炎伴肠上皮化生与CD4 4V6表达程度之间的关系。方法 以单克隆抗体及免疫组化技术等方法对Hp阴性单纯慢性胃炎、Hp阴性萎缩性胃炎伴肠上皮化生、Hp阳性萎缩性胃窦炎伴肠上皮化生、胃窦腺癌的胃镜下活检组织标本进行测定分析。结果 在Hp阴性单纯性胃炎组粘膜上皮未见CD4 4V6表达 ,Hp阴性萎缩性胃窦炎伴肠上皮化生、Hp阳性萎缩性胃窦炎伴肠上皮化生、胃窦腺癌各组CD4 4V6表达程度依次逐渐升高 ,各组之间比较差异有显著性 (P <0 .0 5 )。结论 CD4 4V6的表达可能是上皮细胞癌前病变出现的早期生物学信号 ,肠上皮化生细胞可能诱导CD4 4V6的表达 ,而Hp感染则有促进这种诱导表达的作用。  相似文献

目的通过智能电子分光技术(FICE)结合高分辨率放大内镜,描述正常及胃黏膜病变的特征性改变,并探讨其与幽门螺杆菌(H.pylori)及组织病理学的相关性。方法选择32例消化不良患者及5例正常志愿者,在内镜检查中分别于胃窦及胃体部行放大内镜及FICE观察,对胃黏膜按胃小凹形态做出相应分型(Ⅰ~Ⅲ型),并行快速尿素酶^13C-尿素呼气试验及组织病理学检查。分析胃窦及胃体FICE下的分型对诊断H.pylori的价值,并对FICE观察部位的组织病理学改变(活动性、炎症度、萎缩、肠化)进行分级评估。结果对照组5例胃窦及胃体黏膜的FICE分型均为Ⅰ型,提示无H.pylori感染。研究组32例中,胃窦黏膜FICE分型为Ⅰ型14例,其中1例H.pylori感染(7.1%);Ⅱ型13例中10例H.pylori感染(76.9%),且9例同时有萎缩改变;Ⅲ型5例,均H.pylori感染,且3例同时有萎缩及肠化。胃窦黏膜各FICE分型间H.pylori感染情况的差异具有统计学意义(P〈0.01);Ⅲ型结构与组织病理学诊断的一致性较好(kappa=0.890)。胃体黏膜FICE分型为Ⅰ型15例,其中1例H.pylori感染(6.7%);Ⅱ型13例中11例(84.6%)H.pylori感染;Ⅲ型4例均存在H.pylori感染。胃体黏膜各FICE分型间H.pylori感染情况的差异具有统计学意义(P〈0.01)。组织病理学改变(炎症性、活动度、萎缩及肠化)的分级在无H.pylori感染组中显著低于H.pylori感染组(P〈0.01)。结论H.pylori感染与胃黏膜的炎症程度及萎缩、肠化生有明显相关性;FICE技术结合高分辨率放大内镜对预测H.pylori的存在及判断胃黏膜的病变具有一定临床价值。  相似文献

BACKGROUND: We wanted to evaluate the course of chronic gastritis and its association with Helicobacter pylori and CagA seropositivity in an adult sample from Saaremaa (Estonia) during an 18-year follow-up. METHODS: Seventy persons (31 men, 39 women; median age, 57.5 years) from a primary sample of 304 subjects endoscoped in 1979 were reinvestigated by endoscopy and biopsy in 1997. The state of the gastric mucosa and the presence of H. pylori in histologic sections from the antrum and corpus were assessed both in 1979 and 1997 in 66 subjects in accordance with the Sydney system, and H. pylori status in all 70 subjects was determined with the enzyme-linked immunosorbent assay (ELISA). Anti-CagA IgGs were determined with the ELISA, using the recombinant fragment of CagA. RESULTS: During an 18-year follow-up 11% of the subjects developed atrophy in the antrum, whereas 35% developed it in the corpus. Development of atrophy in the corpus and the appearance of intestinal metaplasia in the antrum were associated with increased activity of gastritis both in the initial and last follow-up biopsies. Anti-CagA positivity was found in 71% of H. pylori-seropositive persons (94% of subjects). There was a significant association between CagA positivity and the activity of gastritis, the presence of atrophy or damage to surface epithelial cells in the antrum and in corpus mucosal biopsy specimens at the last follow-up endoscopy. CONCLUSION: The CagA-positive strains of H. pylori enhance the development of atrophic gastritis compared with CagA-negative strains.  相似文献

METHODS: We examined 176 Japanese patients including 53 with H. pylori eradication. We evaluated the antrum by magnifying observation and ordinary endoscopic findings, and compared these results. Biopsy specimens were taken from the sites observed. RESULTS: The magnified views were classified into four types. Histology of the biopsy specimens allowed us to match the four magnified views with normal mucosa with fundic glands, normal mucosa with pyloric glands, mucosa with gastritis and intestinal metaplasia/epithelial hyperplasia. The types of magnifying appearances were specific enough for the diagnosis of histologic gastritis (148 out of the 176 (82.4%) cases; sensitivity, 96.3%; specificity, 73.7%). We could accurately diagnose the histologic gastritis by magnifying endoscopy in 49 out of the 53 (92%) cases with H. pylori eradication, while only in 38% by ordinary endoscopy. The accuracy of diagnosis was statistically higher with the use of magnifying endoscopy than with ordinary endoscope (P < 0.001). CONCLUSION: Magnifying gastroendoscopy is useful to judge the histologic gastritis, especially, in cases with H. pylori eradication.  相似文献

Abstract

Background/aims: Interactions between Helicobacter pylori (Hp) and gastroesophageal reflux disease (GERD), which are common diseases worldwide, are confusing. In this study, the aim was to compare and evaluate the relationship between reflux esophagitis (RE) and Hp infection in adult patients with both the gastric localization of Hp and its histopathologic features.

Materials and methods: 248 patients with RE were compared with 249 age and sex matched control groups. Biopsy specimens obtained from the gastric antrum and corpus were histologically evaluated.

Findings: The incidence of Hp infection was significantly lower in patients with RE than in the control group (Ratio 1.53, 95% CI 1.07–2.20; p?=?.02, p?<?.05). Corpus Hp colonization and corpus gastritis scores were notably lower in the study group (p?=?.01, p?<?.05), whereas there was no significant difference in Hp colonization and antrum gastritis scores in the antrum. Corpus Hp colonization and gastritis scores were found to be negatively correlated with esophagitis development (r?=??0.11; p?=?.01; (r?=??0.14; p?=?.00 respectively, p?<?.05). There was no difference between the groups in terms of atrophy development (p?>?.05).

Conclusion: This study showed that the presence of Hp infection in the corpus and corpus gastritis score was significantly lower in patients with erosive reflux esophagitis than in the control group. It also showed that Hp colonization and corpus gastritis scores were negatively correlated with esophagitis development. This inverse relationship was independent of atrophy.  相似文献
