目的分析冠状动脉介入术后Angioseal血管缝合器机械止血和人工压迫止血的血管并发症发生率。方法947例冠状动脉介入诊治病例,179例术后使用Angioseal血管缝合器止血(机械组),另外768例进行人工压迫止血(人工组),比较二组的止血时间、制动时间和血管并发症发生率。结果机械组显著缩短止血时间和制动时间(P〈0.01),但其与人工组的血管并发症发生率差异无统计学意义(P〉0.05)。结论使用Angioseal血管缝合器能显著缩短止血时间和制动时间,但不能减少股动脉穿刺并发症的发生率,不推荐介入术后常规使用机械止血装置。  相似文献

BACKGROUND: Factors leading to subacute vessel closure after percutaneous coronary intervention (PCI) have not been well established in lesions treated with balloon angioplasty or atherectomy. METHODS AND RESULTS: We used intravascular ultrasound (IVUS) to determine the pre- and post-PCI characteristics involved in subacute vessel closure after PCI. Of 3,403 patients undergoing IVUS imaging during coronary balloon angioplasty or atherectomy, 8 patients (0.2%) developed angiographically documented subacute vessel closure within 1 week post-PCI and were compared to a group matched by procedure date (within 6 months), age, gender, stable or unstable angina, lesion location and treatment (balloon angioplasty or atherectomy). IVUS identified at least one cause for subacute vessel closure in 87% of patients (vs 25% in matched lesions, p < 0.01). Causes were all procedure-related and included inadequate final lumen (60%), dissection (50%) and thrombus (25%). Pre-procedure lesion characteristics were not different from matched lesions. CONCLUSIONS: Subacute vessel closure after balloon angioplasty or atherectomy is mostly related to insufficient post-procedure lumen dimension or dissection.  相似文献

To determine predictors of acute coronary closure after PTCA performed with steerable catheter systems, we compared 140 procedures complicated by acute closure and 311 representative successful attempts from 4,772 procedures performed between April 1982 and March 1986. Sixteen clinical, 35 angiographic, and seven procedural variables were analyzed. Multivariate analysis found seven independent preprocedural factors related to closure: stenosis length of 2 or more luminal diameters, female gender, stenosis at a bend point of 45 degrees or more, stenosis at a branch point, stenosis-associated thrombus (filling defect or staining), other stenoses in the same vessel, and multivessel disease. In addition, four procedural factors were found to be associated with closure by univariate analysis: post-PTCA percent stenosis (p less than .001), intimal tear or dissection (p less than .001), use of prolonged heparin infusion (p less than .001), and post-PTCA gradient of 20 mm Hg or more (p = .004). Multivariate analysis of both preprocedural and procedural variables found six factors independently related to closure: post-PTCA percent stenosis, dissection, prolonged post-PTCA use of heparin, branch point location, fixed bend point location, and other stenoses in the vessel dilated. The risk of coronary closure after PTCA has many determinants. While an estimation of risk can be made before performing PTCA, the most powerful predictors of closure can only be assessed during the procedure itself.  相似文献

OBJECTIVES—To investigate the effect of intracoronary dipyridamole on the incidence of abrupt vessel closure, myocardial infarction, necessity for bypass grafting, and death following percutaneous transluminal coronary angioplasty (PTCA).
PATIENTS—Patients were randomly allocated to receive either conventional pretreatment (heparin 15 000 IU and aspirin 500 mg intravenously) or additional intracoronary dipyridamole (0.5 mg/kg bodyweight). Dipyridamole was administered in 550 PTCA procedures (455 interventions in men, mean (SD) age 59.2 (8.4) years; 74 acute coronary syndromes), while conventional pretreatment was administered in 544 interventions (444 interventions in men 58.3 (7.9) years old; 81 acute coronary syndromes). In 53 interventions bail out stenting was performed for threatened abrupt vessel closure.
RESULTS—Intracoronary dipyridamole significantly reduced the incidence of abrupt vessel closure (odds ratio 0.42. 95% confidence interval (CI) 0.22 to 0.79). While abrupt vessel closure occurred in 6.1% of interventions following conventional pretreatment, dipyridamole reduced the incidence to 2.5%. Restricting the analysis to balloon angioplasty, this reduction was observed in patients with stable angina (odds ratio 0.49, 95% CI 0.23 to 0.96) as well as in those with acute coronary syndromes (odds ratio 0.29, 95% CI 0.09 to 0.87). Reduction of secondary end points in the dipyridamole treated patients failed to reach significance in the PTCA group.
CONCLUSIONS—Intracoronary dipyridamole before PTCA reduces the incidence of abrupt vessel closure following PTCA for stable angina and acute coronary syndromes.

To assess the clinical, angiographic and procedural correlates of outcome after abrupt vessel closure during coronary angioplasty, results were analyzed of 109 patients (8.3%) who had abrupt vessel closure during 1,319 consecutive coronary angioplasty procedures performed between July 1, 1988 and June 30, 1990. These 109 patients had a mean age of 59 +/- 11 years; 63% were male, 57% had had a prior myocardial infarction and 61% had multivessel disease. Coronary angioplasty was performed in the settings of acute myocardial infarction (14%), recent myocardial infarction (36%), unstable angina (34%) and stable ischemia (29%). Abrupt vessel closure occurred at a median of 27 min (range 0 min to 5 days) from the first balloon inflation. By angiographic criteria, thrombus or coronary dissection was identified in 20% and 28% of cases, respectively; both thrombus and dissection were present in 7% of closures, and 45% were due to indeterminate mechanisms. Successful reversal of abrupt vessel closure, defined as restoration of normal Thrombolysis In Myocardial Infarction (TIMI) grade 3 flow without resultant Q wave myocardial infarction, emergency bypass surgery or death, was achieved in 47 patients (43%). By hierarchal analysis, the incidence of death, emergency coronary bypass surgery, Q wave and non-Q wave myocardial infarction was 8%, 20%, 9% and 11%, respectively. Univariate analysis using 23 clinical, morphologic and procedural variables demonstrated that successful outcome after abrupt closure was associated with prolonged balloon inflations (greater than 120 s) (odds ratio = 6.87, p less than 0.001), unstable angina (odds ratio = 2.37, p = 0.034) and placement of an intracoronary stent (odds ratio = 5.33, p = 0.062). By multivariate analysis, independent correlates of successful outcome were prolonged balloon inflations (odds ratio = 5.11, p = 0.001) and intracoronary stenting (odds ratio = 4.37, p = 0.049). Thus, although prolonged balloon inflations and intracoronary stents may improve outcome after abrupt vessel closure, the cumulative risk of morbidity or mortality remains significant and mandates investigation into improved strategies for its prevention and treatment.  相似文献

From October 1, 1986 to December 31, 1989 directional coronary atherectomy was performed during 1,020 procedures (1,140 lesions) at 14 clinical centers. Abrupt vessel closure, defined as a total coronary occlusion or subtotal occlusion associated with clinical evidence of myocardial ischemia, occurred in 43 procedures (4.2%). It developed in the catheterization laboratory in 34 patients, but was delayed 1 to 96 h after directional atherectomy in 9 patients. By univariate analysis the incidence of abrupt closure was higher in directional atherectomy of de novo lesions (p less than 0.001), lesions in the right coronary artery (p = 0.001) and diffuse lesions (p = 0.04). The incidence of abrupt closure tended to be lower in directional atherectomy of saphenous vein grafts as opposed to native coronary arteries (1.6% vs. 4.4%; p = 0.08). Clinical findings during abrupt closure included severe angina in 26 patients, myocardial infarction in 17 patients, hypotension in 5 patients and death in 2 patients. Balloon angioplasty was attempted in 32 patients after abrupt vessel closure. In 16 patients balloon angioplasty resulted in initial resolution of the closure episode, although 1 patient died 96 h after the procedure. Fifteen of 16 patients without initial improvement after balloon angioplasty underwent coronary bypass operation; 9 additional patients with abrupt closure were referred directly for bypass operation. It is concluded that abrupt vessel closure develops relatively infrequently after directional coronary atherectomy. In the absence of severe coronary dissection, abrupt closure after directional atherectomy may be effectively managed with balloon angioplasty in some cases, although coronary bypass operation is often required.  相似文献

AIM: To elucidate factors related to acute vessel closure (AVC) after transluminal coronary intervention. METHODS: From population of 10439 patients subjected to transluminal coronary intervention 2 groups were formed: with (n=885) and without (n=885) acute vessel closure (AVC). Twenty five clinical, angiographical and procedural characteristics of patients of these 2 groups were included into mono and multifactorial logistic regression analysis. RESULTS AND CONCLUSION: The following factors were univariate predictors of acute vessel closure: smoking [odds ratio (OR) 1.42], unstable angina (OR=2.130, acute myocardial infarction within previous 24 hours (OR 2.76), cardiogenic shock (OR 4.31), urgent procedure (OR 1.94), eccentric stenosis (OR 1.67), calcified lesion (OR 2.21), preexisting thrombosis (OR 3.79), lacerated complicated stenosis (OR 2.02), tortuous lesion (OR 1.35), low operator experience (OR 3.37), balloon angioplasty as sole procedure (OR 1.66), concomitant rheolytic thrombectomy (OR 1.95), urgent stenting (OR 1.45). Elective stenting significantly lowered risk of acute vessel thrombosis. Multifactorial step-up analysis selected the following independent predictors of AVC: smoking, acute myocardial infarction within previous 24 hours, cardiogenic shock, preexisting thrombosis, lacerated complicated stenosis, and concomitant rheolytic thrombus extraction. Thus only elective stenting significantly reduced risk of AVC.  相似文献

A case is reported of a man, aged fifty years, who developed pericardial effusion following an acute coronary vessel closure. This diagnosis was established from both clinical and roentgenographic signs. Following paracentesis of the pericardium there was great relief in symptoms, although the fluid reaccumulated subsequently. With further rest in bed and a thoracentesis the patient became well and is at present ambulatory.  相似文献

Vessel rupture and perforation are important complications of percutaneous treatment of coronary and peripheral arterial disease. These complications can result in abrupt vessel closure, distal organ injury, bleeding into the surrounding tissue, and death. Prompt management of such complications is therefore critically important. This paper reviews the management of vessel rupture and perforation, including the use of different types of covered stents (balloon-expandable and self-expanding), as well as the various types of embolization coils. Particular focus will be placed on percutaneous coronary artery and peripheral arterial interventions.  相似文献

Objectives. The purpose of this study was to evaluate the effectiveness of radiofrequency thermal balloon angioplasty and rescue procedure after abrupt or threatened vessel closure complicating elective percutaneous transluminal coronary angioplasty.Background. Coronary angioplasty is an established therapy for ischemic heart disease. However, abrupt closure after successful angioplasty remains a serious problem.Method. We utilized a unipolar radiofrequency balloon in which a radiofrequency potential of 13.56 MHz was transmitted between the coil within the balloon and a plate electrode attached to the patient's body. The temperature within the balloon could be monitored through a thermistor within the balloon. From October 1991 through December 1993, 31 patients who had abrupt or threatened vessel closure during 1,005 consecutive elective coronary angioplasty procedures were randomly assigned to radiofrequency balloon angioplasty or to other procedures as rescue treatment.Results. Fifteen patients were assigned to radiofrequency balloon angioplasty (5 with abrupt vessel closure and 10 with threatened closure). The average balloon temperature and inflation time were 62 ± 9 °C and 129 ± 62 s, respectively. Percent diameter stenosis decreased from 87 ± 14% to 36 ± 25% (p < 0.01). The procedure was successful in 14 patients. The rate of restenosis was 67%, but the success rate of repeat conventional coronary angioplasty for restenosed lesions was 86%.Conclusions. Radiofrequency balloon angioplasty is effective in the treatment of abrupt or threatened vessel closure complicating elective coronary angioplasty even though the procedure is associated with a relatively high rate of restenosis.  相似文献

In order to compare the efficacy of two different stent types in case of bailout stenting, 65 patients, with abrupt or threatened vessel closure following coronary angioplasty, were randomly assigned to either Wiktor (Medtronic Inc., Minneapolis, MN, 33 patients) or Palmaz-Schatz (Johnson & Johnson Interventional, Warren, NJ, 32 patients) stent implantation. Stenting was technically feasible in all except one patient and immediately successful in reverting ischemia and vessel closure in 60 patients (92%). At hospital discharge, complication rates were comparable: early vessel closure, 18% (Wiktor) versus 13% (Palmaz-Schatz) (P=0.53); any clinical event (such as death, myocardial infarction, and surgical revascularization): 18% (Wiktor) versus 22% (Palmaz-Schatz) (P=0.71). At 6 months follow-up, these complication rates remained equal: restenosis, 38% (Wiktor) versus 27% (Palmaz-Schatz) (P=0.42); any clinical and angiographic (vessel closure and restenosis) event: 45% (Wiktor) and 41% (Palmaz-Schatz) (P=0.69). Baseline, direct postprocedural, and follow-up quantitative coronary analysis data were similar, with, however, an exception for the postprocedural residual stenosis [28% (24-32%) (Wiktor) and 21% (18-23%) (Palmaz-Schatz] (means and 95% confidence intervals). In conclusion, despite a discrete postprocedural angiographic benefit observed with the Palmaz-Schatz stent, the long-term clinical and angiographic outcome is similar in both treatment groups. The choice whether to implant a Wiktor or Palmaz-Schatz stent may probably be left to the discretion of the operator and his experience with one particular device.  相似文献

Background: Emergency coronary artery bypass grafting (CABG) has previously been the only option in the treatment of refractory abrupt vessel closure complicating percutaneous transluminal coronary angioplasty (PTC A), and has been associated with high rates of morbidity and mortality. Intracoronary stenting now provides an alternative to emergency CABG. Aim: To assess our initial experience with emergency coronary artery stenting as a new technique. Methods: Retrospective case study review with clinical and angiographic follow-up. Results: The Gianturco-Roubin (GR) stent was deployed in 13 patients in whom PTCA was complicated by abrupt vessel closure refractory to standard balloon techniques. Indications for PTCA were unstable angina (six), stable angina (six) and acute myocardial infarction (MI) (one). The arteries stented included left anterior descending (LAD) artery lesions (eight) and right coronary artery lesions (five). Two patients required urgent CABG, one due to failed stent deployment and one for inadequate control of vessel dissection. In seven of the stented patients the creatine kinase rose to greater than twice the upper limit or normal. Three patients had subacute thrombotic occlusion at seven to 19 days post stent deployment, managed with intravenous thrombolysis or repeat PTCA. At seven months follow-up, 11 patients were free of angina, two patients had Canadian Heart Association class II angina and there were no deaths. Eleven patients had repeat angiography at mean six months post stent. Five patients had evidence of restenosis managed with repeat PTCA in four and CABG in one. Conclusions: The GR stent is an effective alternative to urgent CABG in the treatment of refractory abrupt vessel closure complicating PTCA.  相似文献

Objectives. The objective of this study was to identify predictors of major adverse cardiac events after successful coronary angioplasty.Background. The acute complications of angioplasty are related to baseline clinical and angiographic variables, and early complications adversely affect long-term outcome. However, the predictors of enduring success after uncomplicated angioplasty are less well defined.Methods. Of 4,098 patients undergoing angioplasty in the Hirulog Angioplasty Study, 3,899 (95%) had a successful procedure without in-hospital death, emergent bypass surgery or clinical evidence of myocardial infarction. Baseline and procedural variables for these 3,899 patients were examined.Results. Major adverse cardiac events occurred in 22% of the patients with initially successful procedures at 6 months: death in 1%, myocardial infarction in 2% and repeat revascularization in 21%. Univariable predictors of increased events included successful salvage from abrupt vessel closure (p < 0.001), emergency stenting (p < 0.001), multilesion angioplasty (p < 0.001), diabetes (p = 0.02), target lesion in the left anterior descending artery (p = 0.02), unstable angina (p = 0.03) and smaller final luminal diameter (p = 0.04). There was a trend toward increased events among patients with prior angioplasty (p = 0.08), but asymptomatic elevation of the creatine kinase was not predictive (p = 0.5). In a multivariable model, abrupt vessel closure was the strongest independent predictor of major adverse cardiac events at 6 months (p < 0.001; odds ratio [95% confidence interval] = 3.6 [2.5 to 5.1]), while multivessel angioplasty, target lesion in the left anterior descending artery and diabetes also remained independent predictors (all p ≤ 0.02).Conclusions. This analysis suggests that “uncomplicated” abrupt vessel closure is a powerful predictor of adverse clinical outcome following successful angioplasty. Improved techniques to reduce abrupt closure during angioplasty are thus urgently needed, and patients who experience “uncomplicated” closure require closer surveillance during follow-up.  相似文献

OBJECTIVE--To determine the usefulness of the Stack autopersion dilatation catheter in patients with acute recurrent vessel closure during coronary angioplasty. DESIGN--Prospective data collection. SETTING--University hospital. PATIENTS--In 37 of 1003 consecutive patients undergoing angioplasty between November 1989 and December 1990 acute vessel closure developed that could not be redilated by a conventional balloon catheter. 13 (35%) of these 37 patients were sent immediately for emergency bypass surgery. INTERVENTION--In the remaining 24 patients an attempt was made to reopen the vessel with a Stack catheter. MAIN OUTCOME MEASURE--Successful reopening of the vessel. All successfully treated patients were followed for at least six months to detect recurrent ischaemia. RESULTS--In 16 patients (67%) the Stack procedure was successful. Of the eight patients in whom reopening of the occluded vessel was not achieved, seven were sent for bypass surgery and one was successfully treated by emergency stent implantation. The 16 patients successfully treated with the Stack autoperfusion system were followed up for a mean (SD) of 6.7 (2.6) (range 2 to 11) months. Ten patients remained symptom free but early clinical restenosis developed in four (25%). Overall, only three (19%) of 16 patients experienced recurrence of severe (class III-IV) symptoms and required further mechanical revascularisation. CONCLUSION--These data support the use of the Stack autoperfusion catheter system in selected patients with acute vessel closure not responsive to attempted redilatation with conventional balloon catheters. The short-term outcome seen in this series of patients who were successfully treated with this coronary autoperfusion system is encouraging.  相似文献

We report the implantation of a balloon-expandable stent in a patient with acute vessel closure in the state of evolving myocardial infarction following 48 hr after unsatisfactory coronary angioplasty. The stent was implanted after successful recanalization of an occluded left anterior descending artery, with repeated unsatisfactory results of balloon angioplasty. Adjunct thrombolytic therapy was contraindicated. No residual stenosis was documented in immediate control angiograms, or after 24 hr, 3 weeks, and 4 months.  相似文献

Dissections after coronary balloon angioplasty are risk factors for acute or subacute vessel closure. Intracoronary stenting was developed to avoid this complication by wrapping the intimal and medial flaps against the vessel wall, which reduces the risk of acute thrombosis. A total of 17 stents were implanted into the coronary arteries of 10 patients with angiographically documented dissections after balloon angioplasty, who presented angina and ischemic ECG changes. Mean minimal stenosis diameter was 1.11 +/- 0.35 mm (65.1%) before, and 1.48 +/- 0.56 mm (53.9%, n.s.) and 2.45 +/- 0.62 mm (23.5%, p less than 0.005), respectively, after balloon angioplasty with and without taking the dissection membrane into account. All patients reported an immediate recovery of their anginal complaints after stent delivery, and ischemic ECG changes disappeared. The mean residual stenosis after coronary stenting was measured as 3.33 +/- 0.23 mm (0%, p less than 0.001). The immediate control angiograms and these after 24 h depicted smooth vessel walls without any irregulations at the site of implantation, and the mean residual stenosis diameter remained unchanged after 24 h (3.3 +/- 0.23 mm; 1.2%, p less than 0.001). No patient suffered acute myocardial infarction, one patient underwent coronary bypass surgery because of persisting episodes of angina pectoris. Thus, intracoronary stenting seems to be a secure and effective method to handle dissections after balloon angioplasty, and perhaps reduces the rate of acute complications and the need for emergency bypass surgery after coronary balloon angioplasty.  相似文献

Small vessel disease is a common cause of cerebrovascular disease. It is responsible for ischemic and hemorrhagic strokes, cognitive decline, and asymptomatic disease. Millions of Americans are affected by silent strokes and white matter abnormalities. Lacunar stroke is the most common manifestation. Despite its importance, small vessel strokes remain understudied. There is a need for research focused on this prevalent stroke subtype to define optimal interventions to prevent stroke recurrence and cognitive impairment.  相似文献

目的:观察冠状动脉小血管病变患者的临床特点、支架植入情况及预后效果。方法:回顾分析冠状动脉小血管严重狭窄患者的临床及支架植入资料,并与同期进行的冠脉大血管支架患者进行比较,比较二者的临床特点、手术方法及预后的异同。结果:冠脉小血管支架组植入成功率97.14%,大血管支架组植入成功率98.95%(P〉0.05);两组在年龄、性别及高血压患病率方面组间比较差异无统计学意义(P〉0.05),2型糖尿病患病率在小血管支架组高于大血管支架组(P〈0.05);植入方法上,使用球囊预扩的比例两组比较差异无统计学意义(P〉0.05);支架释放后球囊后扩张比例小血管支架组低于大血管支架组(P〈0.05);术中并发症及随访期预后差异元统计学意义(P〉0.05)。结论:冠状动脉小血管支架植入的成功率、预后与大血管支架组无显著差异,可安全地用于冠脉小血管病变的治疗。  相似文献
