血管内栓塞治疗颈内动脉海绵窦瘘   总被引:4,自引:0,他引:4  
目的 探讨颈内动脉海绵窦瘘 (CCF)血管内栓塞治疗的效果。方法  5例均经DSA全脑血管造影明确瘘口部位 ,采用可脱性球囊或电致血凝性可脱性铂金弹簧圈 (GDC)进行栓塞。结果  5例中 ,1例球囊栓塞成功并保持颈内动脉通畅 ;2例表现为假性动脉瘤 ,用球囊闭塞患侧颈内动脉 ;1例因瘘口太小 ,用GDC填塞海绵窦 ;另 1例 2次试放球囊入海绵窦均破裂失败 ,其中 1枚瘪陷球囊滞留于海绵窦 ,患者不愿闭塞患侧颈内动脉未予进一步治疗 ,但患者治疗术后 1周临床症状和体征消失。结论 颈内动脉海绵窦瘘全脑血管造影可明确瘘口部位、大小 ,首选治疗方法是血管内栓塞治疗。  相似文献

目的总结23例颈动脉海绵窦瘘﹙CCF﹚的临床诊治经验并探讨治疗策略。方法回顾性分析23例各种类型CCF的诊治体会,患者:Barrow分型A型14例、B型1例、C型1例、D型7例。结果20例经动脉入路,2例静脉入路,1例放弃栓塞治疗。9例采用单纯球囊行瘘口栓塞,5例直接行患侧颈内动脉和瘘口闭塞;5例采用NBCA胶栓塞瘘口;1例采用明胶海绵颗粒部分栓塞联合颈动脉压迫治疗;2例采用弹簧圈海绵窦内栓塞;1例单纯采用颈动脉压迫治疗。21例痊愈,2例好转。结论CCF具有复杂性、难治性特点,血管内治疗应作为主要治疗手段。  相似文献

目的:探讨颈内动脉—海绵窦瘘血管内栓塞治疗方法。方法:使用可脱性球囊及微弹簧圈进行血管内栓塞治疗的颈内动脉—海绵窦瘘病例10例。结果:10例病例中9例栓塞成功,均保持颈内动脉通畅,随访未见瘘口再通,另1例因瘘口过小,球囊无法进入窦内而改用保守治疗。结论:血管内栓塞治疗开辟了治疗颈内动脉—海绵窦瘘的新途径,微弹簧圈是一种较可脱性球囊更为理想的栓塞材料。  相似文献

目的:总结应用多种血管内技术治疗外伤性颈内动脉海绵窦瘘(TCCF)的经验体会。方法:自2003年11月~2007年11月收治TCCF 16例。14例患者单纯应用可脱球囊治疗,1例应用可脱弹簧圈结合NBCA栓塞治疗,1例患者应用可脱球囊治疗失败后应用了覆膜支架治疗。结果:14例应用球囊成功栓塞瘘口,另2例分别应用可脱弹簧圈结合NBCA和覆膜支架最终治疗成功。14例闭塞瘘口并保留了颈内动脉,2例闭塞瘘口的同时闭塞了颈内动脉。无死亡以及手术相关性并发症。随访6~24个月(平均18个月),无1例复发。结论:多种血管内技术可以安全、有效地治疗外伤性颈内动脉海绵窦瘘。首选球囊闭塞瘘口,在球囊闭塞瘘口失败时可考虑应用可脱弹簧圈或覆膜支架。  相似文献

目的 探讨多种血管腔内手段治疗颅底动脉损伤的临床价值.方法 2004年10月至2007年5月间我院收治各类型颅底动脉损伤患者共53例,均有头颈部外伤史.主要症状为搏动性突眼和颅内血管杂音(39例)、声嘶或吞咽不适(9例)、鼻出血(5例)等;DSA检查证实颈动脉海绵窦瘘(carotid cavemous fistulae,CCF)39例,颈内动脉假性动脉瘤14例;针对上述53例患者不同的病变特点采用不同的血管腔内治疗,并通过电话或门诊随访.结果 对53例患者56支颈内动脉进行了腔内介入治疗,采用单纯可脱球囊栓塞治疗CCF'33例34支血管,可脱球囊联合弹簧圈栓塞3例,植入覆膜支架封堵CCF3例;采用单纯可脱球囊闭塞颈内动脉治疗假性动脉瘤8例,可脱球囊联合弹簧圈孤立假性动脉瘤2例,植入覆膜支架腔内隔绝治疗4例;平均随访时间9.5个月(2~25个月),85%(45/53)患者主诉症状于6个月内消失,12个月随访15%(8/53)患者遗留眼球运动受限或视力障碍.复查显示存在假性动脉瘤6例,残瘘3例,其中2例因海绵窦区的硬脑膜动静脉瘘而分别于术后第2、3个月行再次介入治疗.结论 对各型颅底动脉损伤,血管腔内介入治疗创伤小且安全有效.尽管存在缺陷,可脱球囊仍是治疗CCF和颈动脉假性动脉瘤的首选方法,在特殊情况下必需联合应用弹簧圈栓塞和覆膜支架植入等多种治疗手段.  相似文献

外伤性颈内动脉海绵窦瘘的介入治疗(附60例报告)   总被引:1,自引:0,他引:1  
目的分析60例外伤性颈内动脉海绵窦瘘用血管内栓塞治疗的方法、结果和技术特点。方法47例用可脱球囊栓塞;12例用弹簧圈栓塞;1例用IBCA胶栓塞。59例经股动脉穿刺插管至颈内动脉进入海绵窦,1例经扩张的眼上静脉插管至海绵窦内。结果①47例球囊栓塞中,35例保持了颈内动脉通畅,占74.5%;12例闭塞了颈内动脉。②12例弹簧圈栓塞中,4例保持了颈内动脉通畅,占33%;8例闭塞了颈内动脉。③1例经眼上静脉插管用IBCA胶栓塞成功。结论介入放射血管内栓塞治疗颈内动脉海绵窦瘘其方法简便、安全可靠、效果良好,特别是用可脱球囊栓塞,保持颈内动脉通畅率高。  相似文献

目的:研究外伤性颈内动脉海绵窦瘘的栓塞治疗。材料与方法:本文对5例外伤性颈内动脉海绵窦瘘患者施行了血管内介入栓塞术。所有患者均于手术前行Matas试验。结果:4例患者经可脱球囊栓塞术治疗,颈内动脉海绵瘘消失,颈内动脉保持通畅,颈内动脉海绵窦瘘的临床综合征于治疗后消失。1例由于瘘口较大,球囊不能完整闭塞瘘口,因该患者健侧颈内动脉代偿供应患侧的能力差,故无法栓塞患侧颈内动脉,栓塞术后该患者临床症状及体征有所改善但未能完全消除。结论:栓塞治疗对于外伤性预内动脉海绵窦瘘是一种有效治疗方法。  相似文献

目的 总结血管内栓塞治疗颅内动脉瘤经验。方法 股动脉Seldinger穿刺,先后应用MagicBD,Tracker-16,Tracker-10和Cordis 3F/2.5F微导管,送入球囊、不同长度的国产、进口游离微弹簧圈,配用MDS、GDC系统送入不同长度微弹簧圈、治疗颅内动脉海绵窦段动脉瘤5例、后交通动脉瘤1例、颈内动脉C2段动脉瘤3例、椎动脉颅内段动脉瘤3例、大脑后动脉瘤P2段动脉瘤1例,治疗颅内动脉C3-4段假性动脉瘤5例。结果 本组无1例死亡。球囊闭塞颈内动脉海绵窦段动脉瘤5例均痊愈,无并发症;微弹簧圈栓塞治疗动脉瘤栓塞达100%者9例、95%者2例,其中1例因血管痉挛遗留轻瘫,1例因MDS到位后不能顺利解脱导致部分脱垂于颈内动脉和大脑中动脉2cm,遗留轻瘫;5例假性动脉瘤完全治愈;闭塞载瘤动脉及动脉瘤的2例椎动脉完全治愈。结论 颈内动脉海绵窦段动脉瘤只要前、后交通动脉侧支循环代偿良好,可脱球囊闭塞载瘤动脉是安全有效的。微弹簧圈栓塞治疗颅内动脉瘤是一种有发展前途的治疗方法。  相似文献

目的 探讨颈内动脉海绵窦瘘的血管内栓塞治疗。方法 对 18例外伤性颈内动脉海绵窦瘘患者 ,全部经皮股动脉穿刺行全脑DSA检查确诊 ,用可脱球囊进行栓塞治疗。结果  18例患者全部栓塞成功 ,其中 15例同时保持颈内动脉通畅 ,1例CCF瘘口较大 ,2例球囊进入瘘口困难 ,改行闭塞瘘口的同时栓塞同侧颈内动脉。术后除 1例患者眼睛失明未恢复外 ,其余病例临床症状及体征完全消失。结论 血管内栓塞技术是目前治疗CCF的最好方法 ,具有损伤小 ,安全性高 ,疗效可靠等特点  相似文献

Transarterial platinum coil embolization of carotid-cavernous fistulas   总被引:4,自引:0,他引:4  
Of the 227 embolization procedures performed by our neurointerventional section for symptomatic carotid-cavernous fistulas over the past 10 years, five involved placement of platinum coils in the cavernous sinus from a transarterial route. In four patients, prior transarterial balloon procedures had failed to produce fistula closure. In the fifth patient, with Ehlers-Danlos syndrome, a prior transvenous embolization attempt was unsuccessful. In three patients, complete closure of the carotid-cavernous fistula was achieved with preservation of the parent artery. In one patient, the earliest treated, a portion of a platinum coil projected through the fistula into the parent artery. To eliminate the risk of clot formation and distal embolization, internal carotid occlusion was performed and tolerated without deficits. In the last patient, closure of the anterior drainage was achieved, but complicated by distal migration of the platinum coils with transient aggravation of ocular symptoms. Attempts to occlude the remaining cortical drainage were unsuccessful with platinum coils; therefore, a balloon was used to obliterate the small remaining fistula. Transarterial platinum coil embolization is an alternative treatment for symptomatic carotid-cavernous fistulas that cannot be closed successfully by other embolization techniques. The development of shorter, more thrombogenic, detachable or retrievable coils may make this technique more promising in the future.  相似文献

Four patients underwent transarterial embolization of a carotid-cavernous fistula with Guglielmi detachable coils; in three cases as the initial form of treatment and in one case after treatment via transarterial balloon embolization failed. The fistulas were 2 to 3 mm in diameter on pretreatment angiograms. Complete obliteration was achieved in two patients; in the other two, minimal residual flow remained immediately after embolization but disappeared by follow-up angiography. One to four coils were used to occlude the fistulas. The internal carotid artery remained patent in all patients, and there were no complications.  相似文献

目的 研究微导管可脱球囊技术治疗海绵窦瘘的临床意义。方法 :2 6例外伤性颈内动脉海绵窦瘘 (TCCF)患者和 1例大脑前动脉 (A1段 )动脉瘤破裂合并海绵窦瘘患者采用微导管可脱球囊技术进行栓塞治疗 ,以经颅多普勒 (TCD)进行复查 ,所有患者均随访 6月以上。结果 :2 2例CCF患者均成功栓塞瘘口并保持颈内动脉通畅 ,其中 2 0例为一次栓塞成功 ,2例因球囊过早泄漏而行 2次栓塞治疗。对 1例A1段动脉瘤破裂合并海绵窦瘘患者成功施行了双球囊动脉瘤颈孤立术。 4例行颈内动脉闭塞术。本组病例未发生严重并发症。结论 :微导管可脱球囊技术是对海绵窦瘘的一种微创、安全、有效的治疗方法。  相似文献

Objective

To summarize our preliminary experiences regarding HydroCoil occlusion for the treatment of carotid-cavernous fistula (CCF), and to evaluate the effectiveness of this treatment.

Methods

From January 2006 to June 2007, 15 patients with traumatic CCF who were treated using HydroCoil occlusion were included in this study. All the patients presented with symptoms such as intracranial pulsatile noise, bulbar conjunctival hyperemia, and pulsatile exophthalmia. Cerebral angiography revealed that the fistula was located in the right internal carotid-cavernous segment in 6 cases, in the left internal carotid-cavernous segment in 8 cases, and on both sides in 1 case.

Results

After CCF procedure, in 15 patients, the fistula was no longer visualized, the internal carotid artery on the affected side remained patent, and intracranial noise disappeared immediately. The preoperative symptoms exophthalmia, bulbar conjunctival hyperemia, etc., returned to normal 1 week after the operation. Vision recovered to varying extents. In the 9 patients who underwent 1-3 months of follow-up cerebral angiography, CCF recurrences and neurological complications were not observed.

Conclusion

Intravascular occlusion has been widely used for CCF treatment, and detachable balloon embolization remains the preferred treatment. In the case of failure of detachable balloon embolization because the patient cannot tolerate the procedure or internal carotid artery occlusion on the affected side is contraindicated, HydroCoil occlusion is a safe and effective though expensive alternative and a stable method of maintaining high carotid artery patency.  相似文献

目的 探讨载瘤动脉闭塞、弹簧圈栓塞以及支架技术治疗颅内创伤性颈内动脉假性动脉瘤(pseudoaneurysm,PSA)的适应证及其疗效.方法 本组6例患者,3例海绵窦段PSA合并颈内动脉海绵窦痿(carotid-cavernous fistula,CCF),2例单纯颈内动脉PSA,1例CCF球囊栓塞后复查显示PSA.球囊闭塞CCF及创伤性颈内动脉PSA 1例,弹簧圈栓塞3例,颈内动脉球囊闭塞2例.根据6例患者的临床表现、影像学资料、治疗方法选择、临床疗效、随访资料以及文献,分析三种治疗方式的适应证.结果 本组患者无手术相关并发症发生,无脑缺血并发症.术后无鼻出血发生,3例眼球突出回复,1例随访半年视力改善,3例颅内杂音消失,3例瞳孔缩小,复查数字减影血管造影(DSA)未见PSA复发.结论 血管内治疗是治疗颈内动脉PSA的首选,合并CCF时可行球囊闭塞或载瘤动脉闭塞.对单纯窄颈者予以弹簧圈栓塞,对宽颈的采用支架技术结合弹簧圈栓塞,而位于颈内动脉岩段可选择带膜支架.
Abstract:
Objective To investigate the indication and result of parental artery occlusion, embolization with coils, stents in treatment of the traumatic carotid artery pseudoaneurysm. Methods There were six patients with traumatic carotid artery pseudoaneurysm including three patients of cavernous pseudoaneurysm combined with carotid-cavernous fistula (CCF), two with simple traumatic carotid artery pseudoaneurysm and one with traumatic carotid artery pseudoaneurysm that was found after CCF embolization with detachable balloon. The treatment included balloon occlusion for CCF and traumatic carotid pseudoaneurysm in one patient, coil embolization in three and intenal carotid artery balloon occlusion in two. The Clinical manifestations, imaging data, choice of treatment, clinical efficacy, follow-up data and literatures were analyzed to discuss the indications for three treatments. Results There was no cerebral ischemia or surgically-related complication. No epistaxis occurred. The eyeball protrusion restoration was found in three patients and intracranial bruit vanishing in three. Vision was improved one patient after half a year follow-up. The pupils shrank in three patients during follow - up. Digital subtraction angiography (DSA) showed no recurrence of pseudoaneurysm. Conclusions Endovascular treatment is the preferred choice of treatment for traumatic carotid artery pseudoaneurysm. The occlusion or parent artery balloon occlusion can be used when the pseudoaneurysm is combined with CCF. Coil embolization can be used for shoes with narrow neck, stent technology combined with coil embolization for those with wide neck and the covered stent for the rock section of the internal carotid artery.  相似文献

探讨血管内治疗对直接型颈动脉海绵窦瘘的价值。材料和方法:38例患者在治疗前均行脑血管造影检查,所有病例均采用经动脉途径,用可脱球囊导管进行栓塞治疗。结果:38例患者中有36例栓塞成功,其中34例患者瘘口完全闭塞。36例患者中有32例于栓塞后保留颈内动脉,4例行颈内动脉球囊闭塞术。术后36例患者的海绵窦综合症均有不同程度的好转。3例患者曾行经静脉途径可脱球囊栓塞治疗,但由于海绵窦内分隔的阻挡作用,球囊不能进入海绵窦及其瘘口附近而告失败。2例患者由于球囊早脱并移位于大脑中动脉分支血管内,导致患者发生失语和一侧肢体偏瘫。结论:血管内可脱球囊栓塞治疗直接型颈脉海绵窦瘘,其瘘口闭塞率高,死亡率低,是直接型颈动脉海绵窦瘘的首选治疗手段。  相似文献
