The aim of this study was to evaluate the accuracy of quantitative 1-day exercise-rest technetium-99m tetrofosmin tomography in the identification of patients with coronary artery disease (CAD) and in the detection of individual stenosed coronary vessels. Sixty-one patients with suspected CAD who underwent coronary angiography and 13 normal volunteers were studied. All patients were submitted to two i.v. injections of99mTc-tetrofosmin, one at peak exercise (370 MBq) and the other (1110 MBq) at rest 3 h after exercise (images 15–30 min after injection for both studies). All patients with CAD (0% luminal stenosis) (n=50) had an abnormal99mTc-tetrofosmin tomogram. Only one patient without significant coronary narrowing showed abnormal findings. Overall sensitivity, specificity and diagnostic accuracy in the detection of individual stenosed vessels were 77%, 93% and 85%, respectively. Sensitivity and diagnostic accuracy in the identification of individuals stenosed coronary vessels were significantly higher (P<0.05) in patients with single-vessel disease (n=21) than in those with multivessel disease (n=29). Sensitivity, specificity and accuracy for detecting individual diseased vessels were similar in patients without previous myocardial infarction (n=26) and in those with previous myocardial infarction (n=35). In myocardial territories related to non-infarcted areas (n=128), sensitivity and specificity in the detection of stenosed vessels were 70% and 95%, respectively. In infarcted areas (n=55), sensitivity and specificity in the detection of stenosed vessels were 85% (P=NS vs non-infarcted areas) and 75% (P<0.05 vs non-infarcted areas), respectively. Finally, sensitivity was significantly lower (P<0.05) in vascular territories supplied by vessels with moderate stenosis (50%–75%) than in those supplied by vessels with severe stenosis (>75%). The results of this study demonstrate that quantitative 1-day exercise-rest99mTc-tetrofosmin single-photon emission tomographic imaging is a suitable and accurate technique to identify patients with CAD and to detect individual stenosed coronary vessels.  相似文献

BACKGROUND AND AIM: Previous studies have documented the prognostic value of normal exercise Tl myocardial perfusion imaging in patients with angiographic coronary artery disease (CAD). However, data on exercise Tc-sestamibi myocardial single photon emission computed tomography (SPECT) are scant. Accordingly, the purpose of this study was to investigate the prognostic value of normal exercise Tc-sestamibi SPECT in patients with angiographic CAD. METHODS: We retrospectively investigated 90 consecutive patients who had a normal exercise Tc-sestamibi myocardial SPECT but angiographic CAD. A group of 69 consecutive patients with both normal exercise Tc-sestamibi myocardial SPECT and coronary arteries were included as control. RESULTS: During a mean follow-up of 50+/-19 months, a total of three hard cardiac events (non-fatal myocardial infarction) and seven soft cardiac events (late revascularization) were observed. The annual hard cardiac event rate between the two groups was not significantly different (0.6% vs. 0.3%, chi=0.47, P=NS), nevertheless the annual soft cardiac event rate was higher in patients with angiographic CAD (1.9% vs. 0, chi=5.74, P=0.02). Moreover, the annual hard cardiac events rate in patients with angiographic CAD who were treated medically was also not significantly different from that of the control group (0.8% vs. 0.3%, chi=0.77, P=NS). Among patients with angiographic CAD, the annual hard cardiac event rate was not statistically different between those treated medically and those who underwent revascularization (0.8% vs. 0, chi=0.53, P=NS). CONCLUSIONS: Our data demonstrate that normal exercise Tc-sestamibi myocardial SPECT despite angiographic CAD suggests a low rate of cardiac death or non-fatal myocardial infarction but a relatively high rate of late revascularization during an intermediate term of follow-up.  相似文献

Background  Earlier studies have suggested a modest accuracy of stress thallium 201 myocardial perfusion imaging (MPI) for the diagnosis of coronary artery disease (CAD) in women. The accuracy of stress MPI with technetium 99m tetrofosmin has not been studied in women. The aim of this study was to assess the accuracy of stress Tc-99m tetrofosmin MPI for the diagnosis and localization of CAD in women. Methods and Results  We studied 88 women who underwent exercise or dobutamine stress Tc-99m tetrofosmin tomography and coronary angiography within 3 months. Significant CAD was defined as a stenosis 50% or greater in diameter in at least 1 major epicardial coronary artery. Myocardial perfusion abnormalities were detected in 44 of 53 patients with significant CAD and in 7 of 35 patients without significant CAD (overall sensitivity, 83% [95% confidence interval (CI), 73%-93%]; specificity, 80% [95% CI, 67%-93%]; and accuracy, 82% [95% CI, 74%-90%]). The sensitivity was 72% (18/25) in patients with single-vessel CAD and 93% (26/28) in patients with multivessel CAD. Perfusion abnormalities were detected in 2 or more vascular distributions in 20 of 28 patients with multivessel CAD and in 4 of 60 patients without multivessel CAD (sensitivity for the identification of multivessel CAD, 71% [95% CI, 55%-88%]; specificity, 93% [95% CI, 86%-98%]; and accuracy, 86% [95% CI, 79%-93%]). The sensitivity, specificity, and accuracy were 82%, 84%, and 83%, respectively, for the diagnosis of CAD in the left anterior descending artery; 77%, 84%, and 81%, respectively, for CAD in the right coronary artery; and 74%, 80%, and 78%, respectively, for CAD in the left circumflex artery. Conclusion  Stress Tc-99m tetrofosmin MPI is an accurate noninvasive technique for the diagnosis and localization of CAD in women. Background

The diagnostic accuracy of exercise 99mTc-labeled sestamibi and intravenous dipyridamole 201Tl-labeled myocardial tomography is established. The accuracy of dipyridamole stress 99mTc-labeled sestamibi myocardial tomography for the detection of coronary artery disease has not been reported.

Methods and Results

Our purpose was to determine the diagnostic accuracy of same-day, rest-dipyridamole stress 99mTc-labeled sestamibi myocardial single-photon emission computed tomography (SPECT) compared with coronary angiography. Two hundred forty-four patients who were unable to exercise adequately underwent both dipyridamole 99mTc-labeled sestamibi SPECT and coronary angiography within 6 months. Dipyridamole was administered intravenously in a standard dose of 0.56 mg/kg for 4 minutes. Cardiac and noncardiac side effects were recorded. The presence of coronary stenoses of 50% or greater diameter reduction in each of the major coronary arteries was compared with imaging data in corresponding myocardial perfusion beds. The patient population was predominately (98.8%) male with a mean age of 63±9 years (range 33 to 83 years). The majority of patients had stable angina (88%). Eighty-four patients (35%) gave a prior history of myocardial infarction; 44 patients (18%) had a history of congestire heart failure. The principal limitation to exercise stress was peripheral vascular disease in 62 patients (26%). No serious side effects occurred during dipyridamole stress; 14% of patients had chest pain and 8% of patients had 1 mm or greater ST segment depression. Of the 204 patients with documented coronary stenoses, 43 (21%) had single-vessel disease and 161 (79%) had multivessel disease. The sensitivity was 93% (40/43 in patients with single-vessel disease) and 91% (146/161 in patients with multivessel disease). Overall sensitivity was 91%. The specificity was 28% (11/40) in this population with a high prestest probability of coronary artery disease and posttest referral for cardiac catheterization.

Conclusion

99mTc-labeled sestamibi myocardial tomography in conjunction with intravenous dipyridamole stress is a safe and sensitive method for the detection of coronary artery disease. The diagnostic accuracy of dipyridamole stress 99mTc-labeled sestamibi SPECT for the detection of coronary artery disease is similar to that reported for exercise stress 99mTc-labeled sestamibi tomography, making this a suitable alternative for the evaluation of patients who are unable to exercise adequately.  相似文献

目的 探讨ATP负荷心肌灌注断层显像 (ATP MPI)诊断冠心病 (CAD)的价值。方法 6 1例无心肌梗死病史的疑诊CAD患者 ,均在不同时间分别行静息、ATP负荷及运动负荷99Tcm 甲氧基异丁基异腈 (MIBI)心肌灌注断层显像 (一日法静息 ATP、隔日运动或一日法静息 运动、隔日ATP ,分别简称为ATP MPI和ST MPI) ,其中 4 6例行冠状动脉造影。结果 ATP MPI诊断CAD的灵敏度、特异性和准确性分别为 90 9% ,91 7%和 91 3% ,ST MPI分别为 86 4 % ,91 7%和 89 1% ,差异无显著性(P >0 0 5 )。ATP MPI检测冠状动脉血管狭窄的灵敏度、特异性和准确性分别为 84 6 % ,86 9%和86 2 % ,ST MPI则分别为 84 6 % ,84 8%和 84 8% ,差异无显著性 (P >0 0 5 )。ATP负荷试验时不良反应发生率明显高于运动负荷试验 (88 5 %vs 5 4 1% ,χ2 =17 6 6 9,P <0 0 1) ,但均轻微而短暂 ,恢复时间明显短于后者。结论 ATP MPI诊断CAD的价值与ST MPI相似。ATP负荷试验安全性与耐受性良好 ,尤其适用于老年、无法运动和高血压患者。  相似文献

Background

Myocardial perfusion imaging in conjunction with dipyridamole low-level exercise stress has proved its value in the evaluation of patients with coronary artery disease (CAD). Simultaneous wall motion analysis by two-dimensional (2D) echocardiography may provide additional information beyond that obtained by myocardial perfusion imaging alone. The purpose of this study was to compare 99mTc-labeled sestamibi single-photon emission computed tomography (SPECT) and 2D echocardiography for the evaluation of CAD according to a dipyridamole low-level bicycle exercise stress protocol.

Methods and Results

We studied 35 consecutive patients referred for the evaluation of chest pain who had undergone coronary arteriography. 99mTc-labeled sestamibi SPECT and 2D echocardiography agreed in 27 patients (80%) studied for overall detection of CAD. On a segmental basis, agreement was found between SPECT and 2D echocardiography in 124 (73%) of 170 segments (Cohen’s kappa=0.43). The accuracy of the combined assessment of myocardial perfusion and echocardiographic wall motion in detecting CAD was 86%, which was not different from the accuracy of SPECT alone (80%; difference not significant) but significantly higher than for 2D echocardiography alone (71%; p=0.03). In the detection of individual coronary artery stenoses, SPECT had a significantly higher accuracy for detecting left anterior descending coronary artery lesions than had 2D echocardiography (80% vs 60%; p<0.01); combining the two methods did not improve the accuracy (80%). The combined assessment slightly improved the accuracy for detecting left circumflex coronary artery stenoses from 71% to 83% (p=0.05).

Conclusion

The combined simultaneous assessment of myocardial perfusion by 99mTc-labeled sestamibi SPECT and wall motion by 2D echocardiography did not significantly improve overall accuracy over that obtained by 99mTc-labeled sestamibi SPECT alone. Therefore 99mTc-labeled sestamibi SPECT with dipyridamole low-level exercise stress appears the preferred imaging modality for the evaluation of patients with CAD.  相似文献

Tc-99m tetrofosmin is a lipophilic, cationic perfusion imaging agent that changes to Tl-201 in detecting coronary artery disease during exercise testing. The purpose of this study is to evaluate the usefulness of Tc-99m tetrofosmin dipyridamole stress imaging combined with low level exercise for the detection of coronary artery disease. We examined 42 patients and 10 normal volunteers who also underwent coronary angiography. A one-day protocol was used: in the stress study, 296 MBq of tetrofosmin was injected and in the rest study 888 MBq was injected. After intravenous administration of dipyridamole (0.142 mg/kg/min for 4 minutes), the patient was exercised on a bicycle ergometer for 3 min (25 Watts). Tetrofosmin was injected 2 minutes after dipyridamole infusion during the exercise. Single photon emission computed tomographic images were obtained 30 minutes after the tracer injection. Images were interpreted as abnormal in 36 of 42 patients with coronary artery disease, and normal in all of 10 normal volunteers. The overall sensitivity of detection of coronary artery disease was 83.3% and the normalcy rate was 100%. The diagnostic values for the detection of significant stenosis in the three major arteries were: LAD sensitivity 83%, specificity 92%; LCX sensitivity 47%, specificity 91%; RCA sensitivity 75%, specificity 83%. Of the 66 arteries with more than 50% stenosis, 48 arteries were correctly identified. Of the 36 with more than 70% stenosis, 31 were identified. Scintigraphic evidence of multivessel disease was found in only 9 patients (50%). A protocol of Tc-99m tetrofosmin SPECT combined with low level exercise after dipyridamole is therefore useful for the detection of the coronary artery disease.  相似文献

Background. Mental stress precipitates myocardial ischemia in a significant percentage of coronary artery disease (CAD) patients. Exercise or adenosine stresses produce different physiologic responses and cause myocardial ischemia via different mechanisms. Little is known about the comparative severity and location of myocardial ischemia provoked by these different stressors. In this study we sought to compare the within-individual ischemic responses to mental versus exercise or adenosine stress in a cohort of CAD patients. Methods and Results. All patients underwent mental stress and either exercise or adenosine testing within a 1-week period. Mental stress was induced via a public speaking task. Rest-stress myocardial perfusion imaging was used with all testing protocols. Participants were 187 patients (65 women [35%]) with a documented history of CAD and a mean age of 64 ± 9 years. Mental stress-induced myocardial ischemia (MSIMI) was less prevalent and frequently of less magnitude than exercise- or adenosine-induced ischemia. Ischemia induced by exercise or adenosine testing did not accurately predict the development or the location of MSIMI. The overall concordance between these stressors for provoking ischemia was weak (percent agreement, 71%; κ [± SE], 0.26 ± 0.07). In a minority of patients (11%) mental stress provoked ischemia in the absence of exercise- or adenosine-induced ischemia. Moreover, in patients who had myocardial ischemia during both stressors, there were significant within-individual differences in the coronary artery distribution of the ischemic regions. MSIMI was more likely to occur in a single-vessel distribution (86%) compared with exercise- or adenosine-induced ischemia (54%). The stressors had moderate agreement if the ischemic region was in the right coronary artery territory (percent agreement, 76%; κ, 0.52 ± 0.19) or the left anterior descending coronary artery (percent agreement, 76%; κ, 0.51 ± 0.19) and significantly lower agreement in the left circumflex territory (percent agreement, 62%; κ, 0.22 ± 0.18). Conclusions. Our findings indicate that mental and exercise or adenosine stresses provoke different myocardial ischemic responses. These observations suggest that exercise or adenosine testing may not adequately assess the likelihood of occurrence or severity of MSIMI and that different mechanisms are operative in each condition. Background

Because symptoms of myocardial ischemia in elderly patients are often atypical, accurate noninvasive assessment of the presence, extent, and severity of coronary artery disease (CAD) would be especially useful to avoid unnecessary catheterization or invasive intervention. This study assessed the safety and diagnostic efficacy of 99mTc-labeled sestamibi (99mTcsestamibi) exercise and pharmacologic myocardial perfusion single-photon emission computed tomography (SPECT) with adenosine or dipyridamole in patients age 80 years or older.

Methods and Results

Stress 99mTc-sestamibi SPECT was performed in 75 consecutive patients who were 80 years old or older (range: 80 to 91 years) with suspected CAD and who underwent coronary angiography within 6 months of the nuclear study. Of these patients, 51 underwent a pharmacologic (adenosine=42; dipyridamole=9) stress study and 24 underwent an exercise treadmill study. A normalcy rate was derived from an additional 36 patients (ages 83±3 years) who had undergone stress 99mTc-sestamibi SPECT and who had a relatively low likelihood (<20%) of CAD. No serious adverse events occurred during or after the exercise or the pharmacologic stress test. The overall sensitivity and specificity for detecting CAD with ≥70% stenosis was 95% (52/55) and 75% (15/20), whereas the corresponding results were 87% (55/63) and 83% (10/12) for detecting patients with ≥50% stenosis. The sensitivity and specificity for pharmacologic stress 99mTc-sestamibi SPECT were 95% (35/37) and 71% (10/14) for detecting ≥70% stenosis, and 86% (37/43) and 75% (6/8) for detecting CAD with ≥50% stenosis. The sensitivity and specificity of treadmill testing were 94% (17/18) and 83% (5/6) in detecting CAD with stenosis ≥70% and 90% (18/20) and 100% (4/4) in detecting CAD with stenosis ≥50%. The normalcy rate among the low likelihood patients was 83% (30/36). The accuracy of stress sestamibi testing was similar for patients with or without angina.

Conclusions

Our findings suggest that exercise or pharmacologic myocardial perfusion SPECT with 99mTc-sestamibi is safe and diagnostically accurate for CAD detection in very elderly patients, irrespective of symptoms. Pharmacologic myocardial perfusion SPECT with adenosine or dipyridamole appears to be a valuable alternative to treadmill stress in very elderly patients incapable of performing adequate exercise.  相似文献

BACKGROUND: Mechanistic differences between pharmacologic stressors may offer different clinical benefits. Therefore the effects of dobutamine and adenosine on absolute myocardial blood flow (MBF) and coronary flow reserve (CFR) were compared. METHODS AND RESULTS: We divided 36 patients (mean age, 61 +/- 8 years) with coronary artery disease into 2 groups based on stenosis severity as follows: greater than 50% but less than 75% (n = 16) and greater than 75% (n = 20). In addition, 18 normal volunteers (mean age, 46 +/- 7 years) served as control subjects. Groups of equal sizes received either dobutamine or adenosine. MBF at rest and peak MBF were measured by use of positron emission tomography in territories subtended by the stenosis (ischemic) and remote myocardium (remote), whereas left ventricular MBF was used in control subjects. CFR was calculated as peak MBF divided by MBF at rest. CFR was significantly greater with adenosine than with dobutamine stress in control subjects and remote CFR. Ischemic CFR was blunted to a similar degree with each stressor. Therefore adenosine achieved flow heterogeneity across all coronary stenosis severities greater than 50%. However, dobutamine achieved flow heterogeneity only in the presence of a severe coronary stenosis greater than 75% despite provoking a greater ischemic stimulus. CONCLUSION: Adenosine stress demonstrated a higher sensitivity and dobutamine demonstrated a higher specificity with quantitative perfusion imaging. Therefore adenosine is superior for diagnostic perfusion imaging, whereas dobutamine is better suited in combination with visual imaging and in the functional assessment of a known coronary stenosis.  相似文献

The aim of this study was to determine the diagnostic accuracy of technetium-99m tetrofosmin myocardial imaging for the localization of coronary artery stenoses of different degrees of severity. Stress-rest single-photon emission tomography (SPET) was performed on separate days in 80 patients (64 males, 16 females; mean age 61 years; 43 patients with previous myocardial infarction; 18 patients with pharmacological stress), within 6 months of coronary angiography. Scintigraphic images were blindly and independently evaluated by three observers. Coronary stenosis was defined as a >50% narrowing in luminal diameter; severe stenosis was defined as a proximal stenosis of >75% or a peripheral stenosis of >90%. Coronary angiography revealed normal coronary arteries or insignificant coronary stenosis in 13 patients and significant coronary stenoses in 67 patients. The sensitivity and specificity of 99mTc-tetrofosmin SPET in respect of severely stenosed vessels were, respectively, 80% and 65% for the left anterior descending artery (LAD), 100% and 46% for the right coronary artery (RCA) and 58 and 78% for the left circumflex artery (LCx) territories. Considering all the significantly stenosed vessels, a significant decrease in sensitivity was observed for LAD territories (to 59%, P=0.05), and a nonsignificant decrease for RCA (88%) and LCx (47%) territories while specificity values remained essentially unchanged. No significant changes in sensitivity or specificity were observed when regions with previous myocardial infarction were excluded. In conclusion, the sensitivity of 99mTc-tetrofosmin SPET for the localization of individual stenosed vessels is only moderate when all significant stenoses are considered, but the ability of this technique to predict the location of severe coronary artery stenoses seems satisfactory, with the exception of the low specificity in respect of RCA territories. BACKGROUND: Our objective was to assess the prognostic value of the scintigraphic extent of coronary artery disease on stress technetium 99m tetrofosmin single photon emission computed tomography in patients with previous myocardial infarction. METHODS AND RESULTS: We studied 383 patients (280 men and 103 women; mean age, 60 +/- 11 years) more than 3 months after an acute myocardial infarction by exercise bicycle or dobutamine (up to 40 mug . kg -1 . min -1 ) stress Tc-99m tetrofosmin myocardial perfusion tomography. Stress images were acquired 1 hour after stress, and rest images were acquired 24 hours after stress testing. An abnormal study was defined as one demonstrating a reversible or fixed perfusion abnormality. Myocardial segments were assigned to corresponding coronary arteries as follows: the apex, anterior wall, and anterior septum were assigned to the left anterior descending coronary artery; the posterolateral wall was assigned to the left circumflex artery; and the basal posterior septum and inferior wall were assigned to the right coronary artery. During a mean follow-up of 4.3 +/- 2.1 years, 48 cardiac events occurred (36 cardiac deaths and 12 nonfatal myocardial infarctions). Myocardial perfusion was normal in 51 patients, abnormal in a single-vessel distribution in 170 patients, and abnormal in a multivessel distribution in 162 patients. The annual cardiac event rates in these groups were 0.4%, 2.6%, and 4%, respectively. In a multivariate analysis model, independent predictors of cardiac events were diabetes mellitus (odds ratio [OR], 2.3; 95% confidence interval [CI], 1.4-4.5), history of congestive heart failure (OR, 2.7; 95% CI, 1.4-4), age (OR, 1.05; 95% CI, 1.01-1.08), and scintigraphic extent of coronary artery disease (OR, 4.2; 95% CI, 1.8-9.1). CONCLUSION: Stress Tc-99m tetrofosmin myocardial perfusion imaging provides independent prognostic information for the risk stratification of patients with previous myocardial infarction. The event rate is directly related to the scintigraphic extent of coronary artery disease. Patients with normal perfusion have an excellent event-free survival rate.  相似文献

PURPOSE: To compare the prognostic ability of the imaging agents 99mTc-sestamibi versus 99mTc-tetrofosmin to predict mortality outcomes in patients with documented coronary artery disease and undergoing vasodilator stress testing. MATERIALS AND METHODS: The study included 2147 consecutive patients who underwent rest and stress single photon emission computed tomographic (SPECT) examination with either 99mTc-sestamibi (n=1128) or 99mTc-tetrofosmin (n=1019). Information relating to all-cause death and cardiovascular death was collected over a 4-year study period. Unadjusted Kaplan-Meier estimates were compared for the two imaging agents. Cox proportional hazard models were examined to determine the incremental contribution of SPECT sum stress score (SSS) and the imaging agent after adjusting for clinical and demographic characteristics. Additionally, the interaction between SSS and agent was examined to determine if the effect of SSS on prognosis was different for the two agents. RESULTS: Vasodilator agents were used for stress testing in all patients who received 99mTc-tetrofosmin and 99mTc-sestamibi. Despite differences in patient risk factors Kaplan-Meier estimates were similar for the two groups of patients. Resulting P-values for differences between models for the end points of (1) death from any cause and (2) cardiovascular death showed that SSS combined with clinical index was significantly better than a model that adjusted for only baseline characteristics (P<0.0001 for both endpoints). The addition of imaging agent (99mTc-tetrofosmin or 99mTc-sestamibi) to the model containing both SSS and the clinical characteristics did not show further significant improvement (P=0.62, P=0.96 for death from any cause and cardiovascular death, respectively). CONCLUSION: The type of clinically available 99mTc-labelled myocardial perfusion agents did not affect interpretation of results for prognostic assessment.  相似文献

PURPOSE: To determine if there was any difference in the ability of physicians to predict prognosis with technetium 99m ((99m)Tc) sestamibi or (99m)Tc tetrofosmin in a large consecutive series of patients at high risk for coronary artery disease who underwent coronary angiography. MATERIALS AND METHODS: This study included 1,818 consecutive patients who underwent a rest and stress single photon emission computed tomographic (SPECT) examination with either (99m)Tc sestamibi (n = 915) or (99m)Tc tetrofosmin (n = 903) and cardiac catheterization. A clinical index was generated and consisted of clinical and demographic variables. Information concerning death, cardiovascular death, and nonfatal myocardial infarction was 93% complete during the 1.5-year study period. Cox proportional hazards models were generated to help determine the incremental contribution of SPECT sum stress score (SSS) and the imaging agent variable to the clinical index. RESULTS: Exercise was used for stress testing in 473 (52%) patients who received (99m)Tc tetrofosmin and 519 (57%) patients who received (99m)Tc sestamibi (P =.06). Cardiovascular death or myocardial infarction occurred in 130 patients. Resulting P values for chi(2) differences between models for the end points of (a) death from any cause, (b) cardiovascular death, and (c) cardiovascular death or myocardial infarction showed that SSS combined with clinical index was a significantly better model than adjusting for only baseline characteristics (P =.001, P <.001, P =.004, respectively). Incremental addition of either (99m)Tc tetrofosmin or (99m)Tc sestamibi to those models containing SSS and the clinical index did not show further significant improvement (P =.87, P =.88, and P =.26 for death from any cause, cardiovascular death, and cardiovascular death or myocardial infarction, respectively). CONCLUSION: This study shows that the type of clinically available (99m)Tc-labeled myocardial perfusion agents should not affect interpretation of results for risk stratification and prognostic assessment.  相似文献

BACKGROUND: Technetium-99m tetrofosmin single photon emission computed tomography (SPECT) imaging is increasingly used in conjunction with exercise and vasodilator stress test as a means of evaluating coronary artery disease (CAD). Dobutamine stress test is an alternative in patients with limited exercise capacity. This study assessed the accuracy of dobutamine-atropine stress tetrofosmin SPECT as a means of diagnosing and localizing CAD. METHODS AND RESULTS: We studied 124 patients (mean age, 57+/-12 years; 88 men) with limited exercise capacity and suspected CAD with dobutamine (as much as 40 microg/kg/min)-atropine (as much as 1 mg) Tc-99m tetrofosmin SPECT. Resting images were acquired 24 hours after the stress test. Significant CAD was defined as 50% or greater luminal diameter stenosis in 1 or more major coronary arteries. Myocardial perfusion abnormalities (fixed and/or reversible defects) were detected in 70 of 88 patients with CAD and in 10 of the 36 patients without CAD (sensitivity = 80%, CI, 72 to 87; specificity = 72%, CI, 64 to 80; accuracy = 77%, CI, 70 to 85). Sensitivity and accuracy rates were higher by using the criterion of any defect than by using the criterion of reversible defects only (80% vs 51%, P<.0001; 77% vs 60%, P<.01, respectively). The sensitivity rate was higher in patients with multivessel CAD than in patients with single-vessel CAD (88% vs 63%, P<.05). Patients with multivessel CAD had a larger stress perfusion defect score (4.5+/-3.1 vs. 2.7+/-2.5, P<.01) than patients with single-vessel CAD. CONCLUSION: Dobutamine stress Tc-99m tetrofosmin SPECT is a useful method for the diagnosis and localization of CAD in patients with limited exercise capacity. Optimal accuracy of the technique is achieved by using both fixed and reversible perfusion abnormalities for the diagnosis of CAD in patients without an earlier myocardial infarction.  相似文献

BACKGROUND: The aim of this study was to assess the incremental prognostic value of stress technetium 99m tetrofosmin imaging after myocardial revascularization. METHODS AND RESULTS: We studied 381 patients (aged 60 +/- 10 years, 270 men), 4.5 +/- 3.2 years after myocardial revascularization (coronary artery bypass grafting in 201 patients and percutaneous coronary intervention in 180 patients), who underwent exercise or dobutamine stress tetrofosmin single photon emission computed tomography. Events during a mean follow-up period of 3.5 +/- 1.4 years were cardiac death in 22 patients, nonfatal myocardial infarction in 11 patients (33 hard cardiac events), and late revascularization in 50 patients. There was no incidence of hard cardiac events in the 100 patients with normal perfusion. Hard cardiac events occurred in 19% of patients with reversible perfusion abnormalities and in 4% of patients without them (P <.01). The incidence of hard cardiac events was similar in patients with and without angina before stress testing (17/197 [8.6%] vs 16/184 [8.7%]). In a multivariate analysis model, predictors of cardiac death were stress rate pressure product and abnormal perfusion. Reversible perfusion abnormalities were independently associated with the composite endpoints of cardiac death, nonfatal myocardial infarction, and late revascularization. In an incremental multivariate analysis model, an abnormal scan was additive to clinical data in the prediction of hard cardiac events (model chi(2) = 17 vs 11, P <.01). CONCLUSION: Stress Tc-99m tetrofosmin myocardial perfusion imaging provides independent prognostic information for the prediction of cardiac events after myocardial revascularization. Symptoms are not predictive of outcome, and therefore asymptomatic patients should not be deferred from stress testing. A normal study identifies a very low-risk population in whom no further intervention is required.  相似文献

目的:评价冠状动脉CT血管成像(CCTA)和运动平板试验(TET)对冠心病的诊断价值。方法:以常规冠状动脉造影(CAG)为诊断冠心病(冠脉狭窄≥50%)的"金标准",对同期先后行TET、CCTA和CAG 3种检查的75例疑似冠心病患者进行回顾性分析,将其TET和CCTA的结果与CAG进行比较。结果:TET和CCTA诊断冠心病的敏感度分别为45.2%和90.5%,特异度为69.7%和93.9%,阳性预测值为65.5%和95.0%,阴性预测值为50.0%和88.6%,准确率为56.0%和92.0%,P<0.01。在冠状动脉血管水平CCTA对右冠状动脉、左主干、前降支、回旋支狭窄诊断的准确率分别为86.7%、100.0%、88.0%和76.0%。冠心病患者中TET诊断阳性率与病变血管支数呈正相关(r=0.440,P=0.004);冠心病患者TET诊断结果阳性与阴性仅与血管狭窄程度≥75%狭窄的节段数目有统计学差异(P=0.016)。结论:CCTA诊断冠心病较TET有更高的诊断准确性和较低的诊断假阳性和假阴性,对有症状的疑诊冠心病患者CCTA的诊断具有更重要作用。  相似文献

Background

Receiver operating characteristic (ROC) curve analysis provides detailed information about the performance of a diagnostic test.

Methods and Results

As part of a multicenter phase III trial, 160 patients with known or suspected coronary artery disease underwent planar stress-rest99mTc-labeled tetrofosmin and stress-redistribution201Tl imaging within 14 days of each other, to compare the diagnostic accuracy of these imaging modalities by ROC curve analysis. Coronary arteriography was used as the reference standard and greater than 70% stenosis was considered significant. ROC curve analysis showed improved detection of coronary artery disease with99mTc-labeled tetrofosmin compared with201Tl, with ROC curve areas of 0.765 versus 0.644, respectively (p=0.02).99mTc-labeled tetrofosmin also better identified single-vessel disease in those without previous myocardial infarction (ROC curve areas of 0.684 vs 0.510 for99mTc-labeled tetrofosmin and201Tl, respectively;p=0.04) and enhanced the detection of multivessel disease in those with previous myocardial infarction (ROC curve areas of 0.764 vs 0.638, respectively;p=0.02).

Conclusions

Detailed assessment of diagnostic performance by ROC curve analysis shows that, for planar imaging,99mTc-labeled tetrofosmin enhances the detection of coronary artery disease compared with201Tl.  相似文献

目的 探讨三磷酸腺苷介入心肌灌注断层图像(ATP-MPI)诊断冠心病(CAD)的临床价值。方法 ATP-MPI 278例,其中51例行冠状动脉造影(CAG),并与运动心肌灌注断层显像(ST-MPI,73例,均行CAG)对照。结果 ①不良反应:有11种不适症状(发生率1.4%-24.5%);Ⅰ-Ⅱ度Ⅱ型房屋传导阻滞和Ⅱ度Ⅱ型窦房传导阻滞各10例和2例。所有患者均未中途停药,仅作对症处理。②ATP-MPI诊断CAD的灵敏度和特异性分别为97.1%和82.4%,探查冠状动脉血管狭窄宾灵敏度和特异性分别为91.0%和94.7%。③对非心肌梗死患者,ATP-MPI诊断心肌缺血的灵敏度和特异性与ST-MPI相比差异无显著性。结论 ATP-MPI诊断CAD的准确性较高,诊断价值与ST-MPI相同;安全性好,尤其适合无法运动和高血压患者。  相似文献

This study compared the results of planar and single-photon emission computed tomographic (SPECT) imaging with tetrofosmin with those of201Tl and coronary angiography. In three normal volunteers the images were normal by both201Tl and tetrofosmin (planar and SPECT). In 23 patients with coronary artery disease, the images were abnormal in 20 patients by SPECT tetrofosmin, in 19 by planar tetrofosmin, in 20 by SPECT thallium, and in 18 by planar thallium (difference not significant). Both planar and SPECT images were divided into five segments per patient. There were 58 perfusion defects by SPECT tetrofosmin, 50 by planar tetrofosmin (difference not significant), 47 by SPECT thallium, and 42 by planar thallium (difference not significant). Perfusion defects were reversible in 47 segments (36%) by SPECT tetrofosmin, 35 (27%) by planar tetrofosmin, 31 (24%) by SPECT thallium (p<0.05 vs SPECT tetrofosmin), and 31 (24%) by planar thallium (difference not significant). Among the 23 patients with coronary artery disease, 19 underwent coronary angiography. In these patients there were 32 diseased coronary arteries. Perfusion defects were present in 21 territories (66%) by SPECT tetrofosmin, 19 (59%) by planar tetrofosmin, 20 (63%) by SPECT thallium, and 18 (56%) by planar thallium. There was agreement between thallium and tetrofosmin in 108 of 130 segments (κ statistics=0.69±0.06). The images, especially with SPECT, are better with tetrofosmin than with201Tl. Thus myocardial imaging with tetrofosmin provides results that are at least as good as those of201Tl. Slightly more abnormal segments and more reversible defects are detected by tetrofosmin than by thallium imaging, especially with SPECT imaging.
