Metastatic involvement of the laryngeal is very rare, with around 150 cases reported to the literature. In eight of these cases, the primary tumor was a colon adenocarcinoma. We report the case of a 80 year-old woman treated of a colloid adenocarcinoma of 7 years earlier, referred to us for chronic and progressive dyspnea. Endoscopic examination showed a subglottic spherical mass, which caused an important compromise of the respiratory airway. Tomographic studies revealed also a thyroid mass. The patient was treated with a tracheostomy, resection of the subglottic mass (with intraoperative diagnosis of "mucin producing tumor"), and total thyroidectomy. The final pathologic diagnosis of the subglottic mass was a metastasis of colloid adenocarcinoma of the colon. In the literature reviewed there are no previous reports of metastatic involvement of the larynx with this type of colon adenocarcinoma. We discuss the clinical and radiological findings, and therapeutic options for metastasis to the larynx, as well as pathological differential diagnosis.  相似文献

Secondary neoplasms of the larynx are rare and account for 0.09 to 0.4% of all laryngeal tumors. These tumors can occur by hematogenous or lymphogenous spread and may represent a diagnostic dilemma when the laryngeal localization is the only manifestation of disease. When multiple metastases are present, the appropriate treatment becomes even more controversial. To our knowledge only 148 cases of metastatic laryngeal neoplasms have been reported in the literature, with cutaneous melanomas and carcinomas from the kidney representing the most frequent primary sites of origin. Our research of the available literature has shown that only nine cases of metastases from colon adenocarcinomas have been described. In general, the overall prognosis of patients with a secondary laryngeal tumor is poor, since involvement of the larynx by a distant tumor commonly occurs in the terminal phase of the primary disease process. As such, the aim of treatment frequently is only to improve the quality of life of the patient. We report a case of colon adenocarcinoma metastatic to the larynx and discuss the problems related to its pathogenesis, diagnosis and treatment.  相似文献

Metastatic involvement of the larynx by distant malignancies is a rare event that can occur by hematogenous or lymphogenous spread through a retrograde or anterograde route. Only 143 cases of secondary laryngeal tumors have been reported in the literature. Metastases from cutaneous melanoma and renal carcinoma are the most common, whereas only sporadic cases stemming from lung and colon adenocarcinoma have been documented. The authors report a case of lung adenocarcinoma and two cases of colonic adenocarcinoma metastatic to the larynx and discuss the problems related to the pathogenesis, diagnosis, and management of secondary laryngeal neoplasms. We report a case of metastatic laryngeal tumor originating from a colon adenocarcinoma in an 81-year-old female. Only a tracheostomy was performed because the patient presented with multiple metastases in other regions. Lymphoepithelial carcinoma is a very rare tumour of the larynx, with an exhaustive review of the literature having disclosed only 33 documented cases. The relationship between lymphoepithelial carcinoma of the larynx and Epstein-Barr virus is still controversial. We describe one new case of this tumour involving the supraglottis. The patient was treated with supraglottic laryngectomy and left modified neck dissection. Three years and 4 months later, the right side of the neck was found positive for metastatic disease and a right modified neck dissection was performed. No evidence of disease was exhibited 4 years after the diagnosis of metastatic disease. The diagnostic problems and therapy associated with this rare tumour are discussed.  相似文献

Metastatic malignant melanoma of the larynx is considered to be extremely rare by most authors. This paper describes a 78-year-old patient, previously treated for cutaneous malignant melanoma and intestinal fibrosarcoma, who presented with stridor due to a metastatic melanoma in the larynx. It was a pedunculated lesion and surgical excision of the lesion was accomplished with a tonsillar snare. This paper further discusses the evolving history, diagnosis and treatment of this metastatic tumour, and reviews the literature regarding previously reported cases.  相似文献

Leiomyosarcoma of the larynx is an extremely rare neoplasm; only about 50 cases have been reported in the English-language literature. We report a new case of laryngeal leiomyosarcoma in a 65-year-old man. The metastatic work-up was negative, and the patient underwent total laryngectomy. He remained disease-free 12 months postoperatively.  相似文献

Considering clinical evidence,<2% of laryngeal malignancies are distant metastases. Cutaneous melanomas are the preponderant primaries metastasizing to the larynx, followed by renal cell carcinomas, breast and lung carcinomas. Colonic adenocarcinoma metastases to the larynx are extremely rare: only 13 cases have been described since 1972. We present the first case of sigmoid colon adenocarcinoma metastasis to the larynx in a patient who developed peristomal malignancy recurrence after total laryngectomy. Patients with metastasis of colonic adenocarcinoma to the larynx usually present with disseminated disease: in view of the poor chances of long-term survival of these patients conservative palliative laryngeal treatments should be carried out to improve the quality of life. Curative treatment should be considered only in isolated laryngeal metastasis.  相似文献

Gingival metastases from non-oral malignancies are very uncommon. The case is presented of a man with rectal adenocarcinoma, whose first clinical evidence of metastatic disease was a gingival metastasis. Although the prognosis of patients with gingival metastases is felt to be poor, prompt initiation of appropriate therapy may afford significant palliation. A 20-year literature review of uncommon metastatic sites for rectal adenocarcinoma and non-oral malignancies which can metastnsize to gingiva is briefly presented. Gingival metastasis should be included in the differential diagnosis of a gingival lesion in a patient with prior or current malignancy.  相似文献

Metastases to the larynx from distant primaries are very rare. Usually the secondary lesion comes from an hypernephroma or malignant melanoma. A case of metastatic laryngeal tumour from a colonic adenocarcinoma, occurring in a 59-year-old woman, is presented and the routes for laryngeal spread are discussed. A case of metastatic adenocarcinoma from the prostate gland to the sphenoid sinus and diagnosed with the aid of immunoperoxidase staining is presented. A concurrent review of the literature uncovered only 17 previously reported cases of carcinoma metastatic to the sphenoid sinus. Primary adenocarcinoma of the temporal bone is a rare disorder. Fewer than 40 cases have been reported in the English literature, most of these being single case reports. Exclusion of metastatic adenocarcinoma is imperative. Adenocarcinomas must be differentiated from benign adenomas. Adenocarcinomas are best managed by aggressive surgical resection, with postoperative irradiation used in cases of incomplete resection or high-grade tumors.  相似文献

Distant metastases to the larynx are rare. We describe the case of a 46-year-old man who was referred to our head and neck surgery clinic with a 6-week history of sore throat and otalgia. He was found to have a laryngeal lesion that was consistent with a primary myxoid liposarcoma that had been extirpated from a lower extremity earlier To the best of our knowledge, no case of myxoid liposarcoma metastatic to the larynx has been previously reported in the English-language literature.  相似文献

A case is described of a 57-year-old woman with jaw metastasis from rectal adenocarcinoma who underwent colectomy and ovariectomy for moderately differentiated adenocarcinoma of the large intestine and peritoneal carcinosis. This patient subsequently underwent several cycles of chemoantiblastic therapy although, approximately six months after the initial surgery, a tumefaction of the gingival mucosa was found in the lower right premolar area. Radiography showed this neoformation to be an area of mandibular osteolysis. A biopsy, performed at the E.N.T Clinic of the IRCCS Oncological Hospital in Bari, Italy, revealed a metastatic lesion from rectal adenocarcinoma. This led to radiation therapy vs. the external fascia of the mandibular lesion. Then, given that further cerebral and hepatic metastases were found, palliative treatment was administered until the patient's death in June 2000. A review of the international literature shows how unusual it is to find secondary metastases from rectal adenocarcinoma in the mandibular region (only 23 cases have been published in the last forty years). For nearly all the authors examined, the treatment of choice for such lesions was radiation therapy associated with chemoantiblastic therapy. Despite such treatment, the literature bears significant agreement as to the poor, short-term prognosis.  相似文献

A case report of a patient with adenocarcinoma of the breast with metastasis to the nasopharynx is described. The patient presented initially with pulmonary metastasis followed later by metastasis to the left jugulo-digastric lymph nodes. A prominent but asymptomatic nasopharyngeal mass was concomitantly discovered on head and neck examination. Three months later, symptoms of panhypopituitarism developed. Invasion of the base of the skull and pituitary were documented. Patients with adenocarcinoma of the breast and high cervical node metastasis should have a thorough otolaryngologic and head and neck evaluation. Metastatic carcinoma to the nasopharynx is an extremely rare occurrence. Only two cases of bronchogenic carcinoma of the lung and two cases of hypernephroma metastatic to the nasopharynx have been reported in the literature (Bernstein et al., 1966). We present what we believe to be the first case of metastatic adenocarcinoma of the breast to the nasopharynx.  相似文献

Benign cartilaginous tumours of the larynx and trachea are rare. The most common is chondroma, and only 6 cases of osteochondroma of the larynx have been reported in the world literature. We present a case of 74 year's old woman with osteochondroma of the right arytenoid cartilage. This is followed by a review of the literature, diagnostic evaluation and surgical management.  相似文献
