Fusion is a developmental anomaly characterized by the union of two adjacent teeth. In this article we report a rare case of bilateral fusion of permanent mandibular second molars with supernumerary teeth. The rarity with which this entity appears, along with its complex characteristics, often make it difficult to treat. The endodontic management of one tooth is described, as well as the successful treatment of a periradicular lesion.  相似文献

AIM: To describe combined endodontic and orthodontic treatment of a maxillary lateral incisor fused with a supernumerary. SUMMARY: A rare case is presented in which combined endodontic and orthodontic treatment was performed on a cross-bite fused tooth. Clinical and radiographic examination showed the maxillary lateral incisor fused with a supernumerary and an impacted canine. The fused tooth required nonsurgical and surgical endodontic treatment for functional and aesthetic reasons. The root canals were dressed with calcium hydroxide for 2 months before they were obturated with thermoplasticized injectable gutta-percha. Then, the distal part of the fused tooth was removed and the mesial part of the tooth was replanted and fixed. Three months after the completion of orthodontic therapy, the impacted canine erupted between the remaining tooth and the first premolar. Recall examination, 3 years after completion of root canal treatment, showed clinical and radiographic evidence of healing. KEY LEARNING POINTS: Fusion has been described as a development anomaly characterized by the union of two adjacent teeth. Proper combined endodontic and orthodontic treatment resulted in maintaining one tooth half and solving the aesthetic and functional problem of a fused tooth.  相似文献

Supernumerary premolars are "extra" teeth morphologically belonging to the premolar group. Hyperdontia affecting premolars can be single (when only one supernumerary tooth is found), multiple (if several teeth are involved) or related to a syndrome such as cleidocranial dysplasia. Supernumerary premolars are the third most frequent group of supernumerary teeth, following mesiodens and fourth molars; the prevalence among the general population ranges from 0.09-0.64%. The present study describes 10 cases of supernumerary premolars treated in our Service of Oral and Maxillofacial Surgery of the Dental Clinic of the University of Barcelona. A late onset was demonstrated in one case, contrasting the present pantomograph with another obtained 5 years before. In only one case did the presence of a supernumerary premolar alter normal tooth eruption. Two supernumerary premolars were affected by follicular cysts.  相似文献

Dens invaginatus (dens in dente) is a common dental anomaly with a reported prevalence of between 0.04% and 10%. It typically affects permanent maxillary lateral incisors, central incisors and premolars. These developmental lesions are less common in mandibular teeth and are extremely rare in canines and molars. This report describes a rare case of dens invaginatus (Oehlers type II) in a permanent mandibular canine. The tooth was mature with a closed apex and showed apical pathosis. The tooth was treated endodontically using a non-surgical technique with hand endodontic files, and then followed up after a period of 8 months. A follow-up radiograph showed some healing of the lesion.  相似文献

Concrescence represents a rare developmental anomaly in which two fully formed teeth are joined along the root surfaces by cementum. Maxillary molars are the teeth most frequently involved, especially a third molar and a supernumerary tooth. Very few cases have been reported about the concrescence of a third molar and a supernumerary tooth. According to our current knowledge, this case report is the first in the literature in which concrescence is observed between a third molar and a supernumerary fourth molar in the mandible.  相似文献

OBJECTIVE: The aim of this study was to compare tooth size measurements between patients with supernumerary teeth and a control group. METHOD: The supernumerary group consisted of 56 subjects (21 females and 35 males) and the control group of 40 subjects (20 females and 20 males). All available permanent teeth on the dental casts were imaged and measured from both buccal and occlusal views using an image analysis system. Mesio-distal, bucco-lingual or occluso-gingival dimensions, area and perimeter were measured from each view. RESULTS: Supernumerary tooth patients tended to have larger tooth size measurements for almost all variables than controls. Significant differences (p<0.05) were found for the mesio-distal dimension of the upper and lower incisors and the bucco-lingual dimensions of the lower first premolars. CONCLUSIONS: The results are compatible with supernumerary teeth being a complex dental anomaly with a multifactorial aetiology in which both genetic and environmental factors are important. There is some evidence of a local effect with greater differences in tooth dimension adjacent to the site of the supernumeraries.  相似文献

The occurrence of supernumerary teeth is a relatively uncommon dental anomaly. The aetiology is not clear. Supernumerary teeth have frequently been observed as solitary teeth and impacted in the maxillary arch. This case report describes the rare presence of bilateral maxillary fourth molars and a supernumerary tooth in the maxillary canine region. On the left side, the third molar was extracted first, allowing the fourth molar to move into a more favourable position for later extraction. Two-year postoperative radiography confirmed that the supernumerary tooth had migrated occlusally and mesially permitting a safer extraction procedure.  相似文献

Fusion is a rare occurrence and its definitive diagnosis is of prime importance for successful root canal treatment. This case report discusses the endodontic and esthetic management of fused maxillary lateral incisor with a supernumerary tooth that presented with spacing between the fused tooth and the adjacent central incisor. Nonsurgical endodontic treatment was performed on the fused teeth followed by postendodontic esthetic restoration. The patient remained symptom-free and there was a reduction in the size of periapical radiolucency after 1 year.  相似文献

Careful management of fused teeth is essential as abnormal morphology can predispose a tooth to caries and periodontal disease. In this paper, a rare case of successful endodontic management of unilateral mandibular third molar fused to a distomolar is reported. Caries was removed from the tooth complex under local anaesthesia. The pulp chambers of the third molar and supernumerary tooth were accessed and the root canals were prepared using rotary instrumentation and copious irrigation with 2.5% sodium hypochlorite. Obturation using the lateral condensation technique with gutta-percha and AH26 sealer was subsequently performed. A 1-year recall showed a good treatment result.  相似文献

We re-examined tooth variation in specimens of the Siberian mole, Talpa altaica, from the collection of the Siberian Zoological Museum and discuss the mechanisms of dental evolution. The number of teeth counted in 1789 specimens ranged from 34 to 47, and supernumerary, absent, and connate teeth were observed. The most frequent tooth anomaly was an absent tooth in the premolar region (200 maxillary first premolars and 190 mandibular third premolars), which does not support Fujita and Kirino's terminal reduction hypothesis in the mandible [Fujita T, Kirino T. Ha No Kaibougaku. 21st ed. Tokyo: Kanehara Publishers Inc.; 1976 (in Japanese)]. Supernumerary teeth were found in premolar rows and in the incisor and molar regions. An maxillary fourth molar, positioned distal to the normal third molar, was thought to result from a genetically programmed atavistic event during the natal stages. Connate teeth were observed only in the premolar rows and were thought to have developed with the fusion of two independent tooth germs. Connate premolars appeared to result from an expression of an incomplete division of tooth germ at an early developmental stage or a reunion of independent tooth germs, based on the morphological similarity of the normal and supernumerary premolars. These extraordinarily frequent tooth anomalies of T. altaica are of much interest both in terms of tooth development and classification.  相似文献

Mesiodens is a midline supernumerary tooth commonly seen in the maxillary arch and the talon cusp is a rare dental developmental anomaly seen on the lingual surface of anterior teeth. This paper presents a rare clinical case of development of talon cusp in a mesiodens with multiple lobes, which interfered with both occlusion and appearance of an 11-year-old patient. During clinical interview, the patient reported difficulty on mastication. Clinical and radiographic examination revealed that a supernumerary tooth with completely formed root was causing an occlusal interference. The supernumerary tooth was diagnosed as multi-lobed mesiodens associated with a palatal talon cusp. The treatment plan consisted in the extraction of the supernumerary tooth followed by orthodontic treatment for diastema closure and tooth alignment.  相似文献

The term fusion is used to define a developmental anomaly characterised by the union of two adjacent teeth. In the case reported here, clinical and radiographic examinations suggested a unilateral fusion between the mandibular left permanent incisor and a super-numerary tooth. Radiographs showed that the fused teeth had two distinct pulp chambers and canals. A diagnosis of chronic periapical abscess of the supernumerary tooth was made. Before root canal therapy, a periodontal surgical procedure was performed to section the central incisor and its fused supernumerary. Also, odontoplasty was performed on the roots, to establish an anatomy consistent with a normal central incisor. Later, the chronic apical abscess on the supernumerary tooth was instrumented chemo-mechanically, root canal filling was performed and an anterior composite resin restoration was placed. The patient was evaluated for one year after root canal therapy. The tooth was asymptomatic, not exhibiting any pathological root resorption or alveolar resorption, and the anterior composite restoration was intact. Instead of extracting the supernumerary tooth, the application of endodontic, periodontal, and restorative procedures proved to be an alternative treatment.  相似文献

目的 应用曲面体层技术探讨少牙多牙症(CHH)的发生率和临床特征。 方法 收集2019年1月—2021年5月就诊的41 648例儿童口腔科患者的曲面体层片,纳入CHH患者145例,观察记录CHH的发生情况。应用SPSS 24.0软件统计分析所得的数据。 结果 CHH的发生率为0.35%(145/41 648),男性(102例)多于女性(43例),性别间差异有统计学意义(P<0.001)。恒牙先天缺失特征:缺失1~2颗为主;最好发下颌侧切牙和下颌第二前磨牙;下颌恒牙先天缺失多于上颌恒牙先天缺失,二者差异有统计学意义(P<0.001);左侧先天缺失与右侧先天缺失差异无统计学意义(P=0.84)。多生牙特征:数目1~2颗;多见于上颌前牙区;多为圆锥形;垂直倒置生长和垂直正位生长为主。 结论 CHH是一种少见的混合牙齿数目异常,男性多于女性,恒牙先天缺失和多生牙的特征与单独发生的恒牙先天缺失/多生牙的特征相似,建议早期诊断和多学科治疗。  相似文献

Multiple supernumerary teeth are usually associated with various syndromes, but they are relatively uncommon as an isolated dental anomaly. The presence of multiple supplemental premolars in all four quadrants and especially in the maxilla is rare. This report presents a unique case of multiple supplemental premolars in all four quadrants that were distributed unevenly (two erupted; five unerupted). A series of radiographs taken over four years demonstrated further sequential developing bilateral supplemental premolars in the maxilla, where they are considered the rarest. Appropriate management and the importance of regular radiographic monitoring of such cases are briefly reviewed.  相似文献

Fusion is developmental anomaly of the dental hard tissue and is defined as the joining of 2 developing tooth germs resulting in a single large tooth structure. Different treatment methods can be used according to the requirements of the situation. The purpose of this case report was to present the multidisciplinary treatment of a patient who had a permanent maxillary left central tooth fused with a supernumerary incisor and a macrodont permanent maxillary right lateral incisor. In the radiographic evaluation, it was determined that the fused tooth had 2 separate roots. The supernumerary tooth was extracted after hemisection, and endodontic treatment was performed on the remaining portion. The distal side of the macrodont lateral incisor was recontoured, and the right central incisor was reshaped with a strip crown to provide aesthetic appearance and to gain space for the alignment of teeth. Following the restoration of the incisors, orthodontic treatment was provided.  相似文献

Fusion is a developmental anomaly of dental hard tissues. Most cases occur in the anterior region of both the primary and permanent dentitions. Occasionally, fusion of permanent and supernumerary teeth may occur in the maxillary anterior region; however, a supernumerary tooth fused with a premolar is a rare case. Developmental lateral periodontal cyst is also an uncommon developmental odontogenic cyst with no clinical symptoms, and the lesion is often discovered on routine radiographic examination. In the present case, we report the uncommon fusion of teeth and lateral periodontal cyst in a Chinese girl. Using cone beam computed tomography we are able to acquire a better understanding of the complicated root canal morphology of the fused tooth and successfully manage the lateral periodontal cyst.  相似文献

Abnormalities in number of teeth are occasionally noted in clinical cases. Many theories have been proposed as regards the causes of the occurrence of supernumerary teeth, including atavism theory, mechanical tooth germ separation theory, tissue induction theory, and dental laminar morphological disturbance theory. However, none of these theories alone offers a sufficient explanation for this phenomenon. The incidence of supernumerary permanent teeth is approximately 1-3%. These are the maxillary anterior teeth, the maxillary molars, and the maxillo-mandibular premolars in terms of descending order of site of occurrence. On the other hand, incidence in the mandibular anterior tooth area, of which there have been few detailed reports, is about 0.01%, a markedly low value. In this paper, we report two rare cases of supernumerary teeth in the mandibular incisor area. We discuss their etiology and orthodontic treatment, and detail a differential diagnosis between the normal and supernumerary teeth. We found that it was difficult to establish a clear etiology and differentiation between the normal and supernumerary teeth.  相似文献

Supernumerary teeth are among the most common dental anomalies affecting the primary and permanent dentition. They are usually found in the anterior maxilla and occur infrequently in the primary dentition. The purpose of this paper was to report a case diagnosed with primary supernumerary tooth in the primary second molar region. The crown and root shape of the primary supernumerary tooth resembled that of the primary first molar. On radiographic examination, the primary supernumerary tooth was followed by a permanent supernumerary successor with an unusually big crown. The primary supernumerary molar was immediately extracted to avoid interference with the development of the second premolar's tooth bud. This dental anomaly is rarely observed, as only primary supernumerary teeth in the anterior region have been reported in the dental literature.  相似文献

An 8-and-a-half-year-old girl with supernumerary teeth of tuberculate and odontoma type is described. Treatment of the patient is carried out on conventional lines with a combination of surgical and orthodontic methods. The upper tuberculate type supernumerary teeth were extracted and, after surgical exposure, the upper permanent first incisors were aligned with removable appliances. After secondary dentition was completed, the lower odontoma type supernumerary tooth was removed surgically, and also the maxillary and mandibular first premolars were extracted because of severe crowding, and fixed orthodontic appliances were used to align the permanent dentition. Early diagnosis and treatment of this anomaly is necessary to avoid more serious consequences and to prevent severe orthodontic disturbances.  相似文献
