目的:探讨利用Nd∶YAG激光提高视网膜-脉络膜静脉吻合术的成功率。方法:灰色家兔20只(40只眼)用氩激光建立视网膜分支静脉阻塞(BRVO)模型,模型成功家兔随机分为实验组10只(20只眼),用氩激光随后用Nd∶YAG激光;对照组10只(20只眼),单独用氩激光诱发形成视网膜和脉络膜静脉间的吻合。结果:通过眼底荧光血管造影(FFA)检查和彩色眼底照像,实验组6只眼成功建立视网膜脉络膜吻合,检眼镜下激
光击射部位静脉中断呈漏斗状陷入脉络膜;FFA显示远端及近端静脉内血流汇入吻合处,吻合形成时间为4~6周,对照组未形成吻合。观察2~6个月由激光击射引起的视网膜或玻璃体出血均在8周内吸收,未见其他严重并发症。结论:用Nd∶YAG激光可提高诱导视网膜脉络膜静脉吻合的成功率。 相似文献

This experiment was done on 50 left side carotid communal arteries of rats. The arteries were cut and then anastomosed by welding using CO2 laser which produced by a self design laser microsurgical machine which was designed by us, while the right side arteries as control were anastomosed by conventional suturing with 11/0 nylon thread. The results showed that the patency rate was 100% in both groups either immediately or 1 hour, 3 days, and 10 days and 2 months after operation. The operative time of the laser group is about 5-6 min shorter than the sutured group which took 10-15 min. The anastomotic quality was examined by scanning electronmicroscopy. The results showed: On the third day the endothelium growth rate was 18% and on the 10th day was 98% in the laser group which was very close to the sutured group. 相似文献

目的观察小儿腹腔镜下肝门空肠吻合术中,动脉血CO2分压(PaCO2)与呼气末CO2分压(PETCO2)差值(Pa-ETCO2)的变化及其临床意义。方法胆总管囊肿患儿22例,均于腹腔镜下行肝门空肠吻合术。手术经历气腹-非气腹-再气腹过程,术中机械通气PCV模式,维持PETCO230~40mmHg。于气腹前5min,首次气腹后10、60min,气腹撤销后10、30、60min,再次气腹后10、60、90min以及再次气腹撤销后10min行动脉血气分析,计算每个时点的Pa-ETCO2。结果两次气腹后各时点及两次气腹撤销后10min,患儿的PETCO2与PaCO2较基础时点显著增高(P<0.01);在两次气腹后60min以及两次气腹撤销后10min,Pa-ETCO2较气腹前显著增大(P<0.05)。结论胆总管囊肿患儿腹腔镜下行肝门空肠吻合术中,在两次气腹后60min及两次气腹撤销后10min,PETCO2不能正确反映PaCO2,需进行血气分析测定该时点的PaCO2,以确保其处于正常范围。 相似文献

目的研究兔子颈总动脉纵向拉伸以及吻合情况,为临床修复颈总动脉长短缺损提供实验依据。方法新西兰大白兔6只,均游离出长60mm颈总动脉,分3组(各2只)分别切除18、21和24mm,纵向拉伸后进行端端吻合,相当于23.8%、28.2%及33.3%的纵向伸长率。15d后对动脉血管进行病理学检查。结果切除18和21mm组颈总动脉吻合成功,切除21mm组吻合难度较大,切除24mm组吻合失败。吻合成功血管术后15d病理检查显示,动脉的吻合口平滑,血管内径无狭窄,血管存在水肿和纤维增生;血管内膜处于极度的拉伸状态。结论颈总动脉可被拉伸延长后进行端端吻合,伸长率可达28.2%。长段游离后拉伸动脉是有效的修复创伤性动脉长段缺损的方法之一。 相似文献

Objective: Many methods have been used in an attempt to seal the epineurium and to prevent axonal outgrowth.In this study, the rat sciatic nerves were repaired with CO2 laser, the nerve regeneration and the morphology of spinal anterior horn neurons were investigated. Methods: Seventy-two male Sprague-Dawley rats were randomly divided into 6 groups of 12 rats. The animals were designed to observe the electrophysiology, the histopathology and the morphology of spinal anterior horn neurons. One of the rat sciatic nerve anastomosed with CO2 laser, the contralateral nerve was reconstructed by microsuture technique. At 2, 4, 6, 8 weeks postoperatively, neuromuscular functions, the regeneration of axons and neurons were evaluated by the electro-physiological and histopathological studies. The rats were killed at 4, 6 weeks postoperatively. Results: The recovery of toe spread and myodynamia in laser groups was better than that in suture groups (P<0.05). The latency of foot withdraw caused by radiate heat and neuromuscular conduction velocity in laser groups were faster than that in suture groups (P<0.05). The density of nerve fibers, percentage of axons passing through anastomotic area and numbers of neurons were better in laser groups than in suture groups. At 8 weeks postoperatively, the first grade dendrites of anterior horn neurons grew well. Their diameter, length, volume and total volume were much higher than that in control group. (P<0.05, P<0.01). Conclusion: CO2 laser repairing was effective in promoting the regeneration and the recovery of sciatic nerves in its earlypost-trauma stage. In addition, laser repairing was found to reduce regenerating axons misdirection and forming neuroma. 相似文献

目的评估单层缝合在胃肠道吻合中的可行性及安全性。方法将24只新西兰大白兔按照年龄、性别、体重随机平均分成A、B两组。其中A组行单层间断缝合,B组行双层缝合(胃肠的端端吻合),并于术后2、4、7d作吻合口的内径,压力测试实验及光镜、电镜、免疫组化切片观察新生肉芽组织的生成情况。结果A、B两组之间的吻合口漏与狭窄的发生率无统计学意义;而两组之间的吻合口的耐压、壁厚、内径均有显著的统计学差异(P〈0.05)。两种吻合术均无吻合口大出血。结论单层缝合与双层缝合的安全性的差别无统计学意义,但是单层缝合组的肉芽组织生成早,愈合快,并且节约了手术时间及手术费用,故为一种安全、可靠的缝合方法。 相似文献

目的 介绍齿科激光焊接技术在正畸临床治疗中的应用范围及方法.方法 激光焊接是利用激光能量来焊接2个金属成型品的一种技术.随着激光焊接在传统牙科合金焊接领域的不断强化,在正畸矫治器制作工艺上也逐步得到了良好的运用.在正畸矫治器制作中,除异种合金不宜与钛合金配对焊接外,其他如镍铬合金、钴铬合金的同种、异种金属焊接,以及钛合金的同种焊接均可取得较好的效果.结果 与传统银焊相比,焊接方法简单,无需包埋.速度快、耐腐蚀、焊点小、美观.焊接变形小、强度高、失败率低,可进行补救性焊接处理.结论 激光焊接可以有效的应用于口腔正畸矫治器的制作和修补中,是一种有推广前途的矫治器制备工艺. 相似文献

目的探讨失神经支配的咬肌瓣获得面神经再支配后的生理和结构特性变化。方法取24只新西兰大白兔,平均分为两组:A组行面神经———咬肌神经端端移植吻合;B组行面神经咬肌内植入。术后分别在第1、3、6个月进行大体观察及肌湿重测定、电生理检查、酶组织化学检查及超微结构观察。结果失神经咬肌可重获面神经再支配,并在面神经支配下行使收缩功能,肌纤维的组化型别发生改变,从而使咬肌的肌纤维类似于表情肌。两种方法在术后3个月时肌肉传导速度的恢复率有显著差异(P<0.01);术后6个月时两者无显著差异(P>0.05)。结论两种方法结果相似,均能使移植肌重获神经支配,恢复收缩功能。一期手术时,神经肌内植入法与吻合法无显著差异(P>0.05),但该方法简单、损伤小,对临床手术治疗面瘫有一定的借鉴作用。 相似文献

目的:观察6-氨基己酸(EACA)治疗内毒素血症的疗效.方法:实验1:将10只大白免随机分为两组,所有动物静脉滴注细菌内毒素的主要成分脂多糖(Lipopolysaccharide,LPS)制造内毒素发热反应,试验组静注EACA,对照组肌注非那根联合静注地塞米松.实验2:将9只大白兔随机分成3组,三组动物都经麻醉,颈动脉插管,应用MedLab生物信号采集处理系统记录动物的平均动脉压,三组动物都缓慢静注LPS,然后分别应用非那根联合地塞米松、EACA及生理盐水.结果:实验1中,两种方法都能治疗输液发热反应,控制发热反应所需的时间,对照组为(30.04士3.13)min;治疗组只要(2.36±1.12)min(P<0.001).实验2中,两组药物都有抗LPS而引起动物血压下降作用,EACA起效较快,在治疗后10 min及30 min可以观察到差异的显著性(P<0.05),地塞米松组起效稍晚,随着时间推移,该组效果明显好转.结论:在治疗LPS引起的急性中毒的时候,EACA是一种可供选择的好药物. 相似文献

目的评估浆肌层吻合法在结肠吻合中的安全性和有效性。方法选取2017年8月至2018年8月粤北人民医院收治的行结肠吻合的结肠癌患者48例,按随机数字表法分为浆肌层吻合组(n=24)和二层吻合组(n=24),浆肌层吻合组行Layer to layer缝合法,二层吻合组行Albert-gambee缝合法,观察并比较两组患者围术期指标及并发症发生情况。结果浆肌层吻合组与二层吻合组吻合时出血量、吻合口漏发生率、首次肛门排气时间、首次排便时间及术后住院时间分别为(19.25±3.75)mL和(19.29±4.57)mL、8.3%和4.7%、(4.00±1.18)d和(3.79±1.18)d、(7.13±1.39)d和(7.63±1.38)d、(11.29±1.94)d和(11.71±2.26)d,两组相比,差异无统计学意义(P0.05);浆肌层吻合组的吻合时间为(11.83±1.93)min,较二层吻合组的(21.29±1.94)min明显缩短,差异有统计学意义(P0.05)。结论结肠吻合中运用浆肌层吻合法安全可靠。 相似文献

目的评价CO2激光联合半导体激光治疗跖疣的临床疗效。方法70例跖疣患者被随机分为两组,治疗组(36例)给予CO2激光联合半导体激光治疗,对照组(34例)单用CO2激光治疗,分别观察两组患者的临床疗效。结果治疗组有效率为91.7%,对照组有效率为82.3%,两组比较差异无统计学意义(P>0.05);治疗组创面愈合时间为(14.80±3.84)d,对照组为(17.06±3.79)d,两组比较差异有统计学意义(P<0.01)。结论CO2激光联合半导体激光可缩短创面愈合时间,是治疗跖疣的有效方法之一。 相似文献

Background The biofragmentable anastomosis ring (BAR) is a simple alternative device to create intestinal anastomosis. Our study was designed to evaluate the clinical value of BAR in intestinal anastomosis. Methods A total of 167 patients performed intestinal anastomosis from January 2002 to February 2006 were randomized to BAR group (n=82) and manual suture group (n=-85) as control. They were equally allocated to the two groups regarding sex, age, site of anastomosis, emergent or elective surgery and contaminant diseases. The results of postoperative complications and recovery were recorded in each group. Results Eighty-seven intraperitoneal BAR anastomoses were completed in 82 patients. Two and one postoperative deaths were recorded in BAR and suture group, respectively, no deaths were directly related to anastomotic technique. In suture group, anastomotic leakage and early bleeding both occurred in two patients respectively, no anastomotic bleeding occurred in BAR group, one patient in BAR group developed enterocutaneous fistulae. Perioperative bleeding, operation time and length of hospitalization were similar in two groups (P 〉0.05). Time for return of bowel function was significantly shortened in BAR group than that in suture group (P 〈0.05). Conclusion The BAR appears to be a standard, easy, safe and effective alternative either in elective or emergent intraperitoneal intestinal anastomotic surgery. 相似文献

目的 研究点阵CO2激光治疗对皮肤胶原重建和部分生理指标的影响.方法 点阵CO2激光照射小鼠背部皮肤,于照射后1 h,3、7、14 d,4、8周用MPA皮肤多功能测试系统测定其皮肤回声值、皮肤血红蛋白含量、经皮失水量,并于照射后4、8周取皮肤做病理切片测量真皮厚度及酶联免疫吸附试验检测Ⅰ型胶原含量.结果 激光照射后短期内对皮肤血红蛋白含量、经皮失水量有一定程度影响,1周内恢复.照射后4、8周真皮层增厚,Ⅰ型胶原含量较对照侧明显增加,皮肤弹性改善.结论 点阵CO2激光嫩肤是一种安全有效的嫩肤方法. 相似文献

目的 比较右半结肠切除术中生物可降解吻合环(BAR)、吻合器两种方法的吻合效果.方法 回顾性分析我院87例右半结肠切除术,其中49例用吻合器吻合,38例用BAR吻合,比较二者术后吻合口瘘、吻合口狭窄、腹腔感染、术后肠梗阻、消化道出血、切口感染等的发生率及肠功能恢复时间、材料费用等.结果 两种方法均无吻合口瘘、吻合口狭窄、腹腔感染、术后肠梗阻等发生.吻合器法中1例出现下消化道出血、3例切口感染,BAR法中2例切口感染,差异均无统计学意义.吻合器法肠功能恢复时间平均为3.5d,BAR法肠功能恢复时间平均为3.1d,差异无统计学意义,但BAR法肠功能恢复时间明显早于吻合器法.吻合器法费用(管状吻合器加闭合器)平均约5 231元人民币,BAR约3 427元人民币.结论 生物可降解吻合环在右半结肠切除术中的应用与管状吻合器同样安全可靠;比吻合器更有利于患者肠功能的恢复、更经济实用. 相似文献

目的:探讨可降解吻合环应用于犬的肠切除、肠吻合的可行性及其临床应用价值。方法:用20只本地杂种犬行肠切除、肠吻合术,将其随机分为两组:单一缝合组(对照组,n=10),应用可降解吻合环吻合组(实验组,n=10)。观察两组手术前后肠道内径的变化,记录缝合时间和总手术时间,观察术后1~10d肠瘘的发生率、支架降解情况,术后12周取吻合口处部分肠管组织做病理学检查。结果:实验组术后胆漏的发生率(1/10)与对照组(0/10)比较,差异无统计学意义(P>0.05);对照组的缝合时间(10.20±2.88)min多于实验组(2.02±1.03)min。术后第12周肠道吻合口病理学显示,实验组肠壁各层较好,炎性反应轻,无明显肉芽肿形成,浆膜层无纤维瘢痕,明显优于对照;对照组与实验组术后吻合口内径变化比较,差异无统计学意义(P>0.05)。结论:可降解吻合环应用于肠切除、肠吻合在动物实验中是安全有效的,且优于线缝合组。 相似文献
