OBJECTIVE To explore a mcthod of trcating migraine attack. BACKROUND Migraine is the most common disorder which causcs .severe headache, nausea and vomiting in attack continuing several hours. There is little method to cease migraine attack quickly. METHOD The patients lied down and kept the head backward. Lidocain 2% 1 to 2 ml was slowly instilled into the nostril ipsilatcral to thc headache. RESULTS Pain relief was achievcd in minutes. The headache disappeared in 19 cases and slight headache rcmained in 2 cases mong 21 cascs. TCD (transcranial dopple ) was tested in 6 cases before thc treatment and 5 minutes after thc treatment. The results showed that CBF (cerebral blood flow) velocity diminished before thc treatment and increased to normal value after the treatment in 5 cases, and it increased beforc thc treatment in another case and diminished after the treatment. DISSCUSION Thc migraine attacks are associated with dysfunction of the vascular constriction and vasodilatation. It was reported that Lidocain had diplex effect that induce the vascular constriction and also induce vasodilatation on the bases of regulation of the autonomic nerves. Medications to abort the attack dramatically by the transnasal route.. CONCLUSION This method is casy to use, painless and effective.  相似文献

Objective To elucidate the clinical significance of diffusion tensor imaging (DTI) mainly used for protecting neurological functions in patients with gliomas. Methods Thirty - eight patients with gliomas were recruited to undergo DTI test and assess the status of neurological functions before and after surgery. According to the relationships between gliomas and adjacent white matter(WM) tracts, DTI can be classfied as the following types : type Ⅰ - displacement; type Ⅱ - Infiltration; type Ⅲ - Disruption. The neurological functions before surgery were compared in three groups,as well as the change of neurological functions after surgery was also compared in three groups. Results There were 14 cases in type Ⅰ , 16 cases in type Ⅱ, and 8 cases in type Ⅲ. There were significant difference between DTI types and preoperative neurological functions, as well as between DTI types and the change of postoperative neurological functions ( P < 0. 05 ). Conclusions DTI allows to visualizate the relationships of gliomas and adjacent WM tracts, which represent the different growth patterns of gliomas. 目的 探讨游离中鼻甲粘膜瓣在经鼻蝶入路神经内镜手术治疗垂体腺瘤鞍底重建中的方法及应用价值。方法 回顾性分析2017年6月至 2018年6月经鼻蝶入路神经内镜手术治疗的45例直径1~2.5 cm的垂体腺瘤的临床资料。经单鼻孔手术25例(单鼻孔组),双鼻孔手术20例(双鼻孔组)。术中均采用游离中鼻甲粘膜瓣重建鞍底。结果 肿瘤均全切除。术后随访 6个月,两组均未出现脑脊液漏和颅内感染,均无鼻腔干燥、异味等鼻部并发症;单鼻孔组术后嗅觉障碍发生率(16.0%,4/25)明显低于双鼻孔组(90.00%,18/20;P<0.05)。结论 对于直径1~2.5 cm的垂体腺瘤,经鼻蝶入路神经内镜手术中鞍底缺损直径约2 cm,采用游离中鼻甲粘膜瓣进行鞍底重建,可有效预防脑脊液漏、颅内感染,单鼻孔组术后嗅觉障碍发生率较双鼻孔组低。  相似文献

OBJECTIVE: To evaluate the efficacy of a revised technique of percutaneous radiofrequency neurotomy for third occipital headache. METHODS: The revisions included using a large gauge electrode, ensuring minimum separation between the three electrode placements, and holding the electrode in place by hand. The revised technique was used to treat 51 nerves in 49 patients diagnosed as suffering from third occipital headache on the basis of controlled diagnostic blocks of the third occipital nerve. The criteria for successful outcome were complete relief of pain for at least 90 days associated with restoration of normal activities of daily living, and no use of drug treatment for the headache. RESULTS: Of the 49 patients, 43 (88%) achieved a successful outcome. The median duration of relief in these patients was 297 days, with eight patients continuing to have ongoing relief. Fourteen patients underwent a repeat neurotomy to reinstate relief, with 12 (86%) achieving a successful outcome. The median duration of relief in these patients was 217 days, with six patients having ongoing relief. Side effects of the procedure were consistent with coagulation of the third occipital nerve and consisted of slight ataxia, numbness, and temporary dysaesthesia. No side effects required intervention, and they were tolerated by the patients in exchange for the relief of headache. CONCLUSIONS: Use of the revised procedure greatly improved the rather low success rate previously encountered with third occipital neurotomy. Although the relief of headache is limited in duration, it is profound and can be reinstated by repeat neurotomy. No other form of treatment has been validated for this common form of headache.  相似文献

OBJECTIVE: To evaluate the efficacy and tolerability of oral zolmitriptan 5 mg and 10 mg and placebo in cluster headache. METHODS: A multicenter, double-blind, randomized, three-period, crossover, outpatient study. Adult patients received placebo and zolmitriptan 5 mg and 10 mg orally for the acute treatment of episodic or chronic cluster headache. Headache intensity was rated by a five-point scale: none, mild, moderate, severe, or very severe. Patients only treated moderate to very severe headaches. The primary efficacy measure was headache response (two-point or greater reduction from baseline in the cluster headache rating scale) at 30 minutes. Secondary efficacy measures included proportion of patients with initial headache relief within 15 and 30 minutes, mild or no pain at 30 minutes, meaningful headache relief, and use of escape medication. RESULTS: A total of 124 patients took at least one dose of study medication, with 73% having episodic and 27% chronic cluster headache. For the primary endpoint, there was a treatment-by-cluster-headache-type interaction (p = 0.0453). Therefore, results are presented separately for chronic and episodic cluster headache. In patients with episodic cluster headache, the difference between zolmitriptan 10 mg and placebo at 30 minutes reached significance (47% versus 29%; p = 0.02). Expanding cysts of the septum pellucidum are rare and frequently manifest as intermittent headaches. Although the technique of endoscopic fenestration has been used since 1999, only a limited number of cases have been reported. We have added the use of a navigator system to guide keyhole creation and endoscopic access. To provide experience in navigator endoscopic treatment of symptomatic cyst of septum pellucidum and long-term follow-up of the surgical result. Under the guidance of the navigator system, a burr hole was made and rigid endoscope was inserted into the lateral ventricle through a working sheath. With direct visualization, only one side of the lateral wall of the cyst was fenestrated. And a grasping basket was used to further dilate the perforated hole. Patient A, a 14-year-old male adolescent, had an acute onset of severe headache with increased intracranial pressure. Patient B was a 37-year-old woman with a diagnosis of medically intractable migraine. Both patients experienced dramatic symptomatic relief after surgery at 4.5- and 2-year follow-up exams, respectively. The technique of navigator-assisted endoscopic fenestration in the treatment of a symptomatic cyst of the septum pellucidum might be a safe and effective method. Intranasal and sinus disease may result in disabling head and facial pain and serious complications. Awareness of the symptoms of acute and chronic sinusitis and their various presentations will enhance diagnostic accuracy and improve patient outcome. It is important to remember that the nasal mucosa is under autonomic control. Sinus symptomatology, whether from anatomic abnormality or chronic inflammation, may occur with and will typically be exacerbated by increased parasympathetic outflow or reduced sympathetic tone. Thus, the possibility of underlying intranasal or sinus disease must be considered in patients with atypical migraine or vascular instability headaches. A new appreciation of the impact of sinusitis on facial and head pain syndromes is developing among otolaryngologists because of the improved diagnostic capabilities afforded by combined intranasal endoscopy and modified CT. In the past, patients with nasal complaints and facial pain who had normal plain radiographs were often passed off as "chronic nasal complainers" and given decongestants or the recommendation to see a psychiatrist. On the other hand, patients with symptoms and radiologic abnormalities often underwent radical surgery aimed at the maxillary or frontal sinus, sometimes with persistence or worsening of their complaints. Now, however, underlying causes for these problems can often be found in the ostiomeatal complex and corrected with minimally invasive surgery. In general, the major sinuses appear to be more sensitive to pain before the development of chronic mucosal changes. Minor disease in critical locations within the ostiomeatal complex may therefore give rise to greater symptomatology than diffuse disease in less critical sites. Although head CT may be a routine part of the neurologist's examination in patients with headache or facial pain, routine CT techniques are inadequate to evaluate fully the ostiomeatal complex and sinuses and must be modified to rule out adequately pain of sinus origin. Nasal endoscopic examination provides a noninvasive examination by which patients may be selected for sinus CT and is a necessary part of a complete evaluation. Close cooperation between the neurologist and otolaryngologist-head and neck surgeon is therefore essential for the accurate diagnosis of this often puzzling group of patients.  相似文献

目的提高海马及脑岛区胶质瘤的手术治疗效果,分析影响预后的因素,提高治疗质量。方法结合颞叶内侧面及脑岛的显微解剖特点,回顾性分析了近年来我科经外侧裂显微外科治疗的海马及脑岛区22例胶质瘤的临床资料,包括临床表现、诊断和手术方式的选择及治疗结果。结果22例胶质瘤中,肿瘤位于海马14例,脑岛8例。术后病理按WHO分级分类,提示星形细胞的肿瘤12例,间变型星形细胞瘤6例,少突胶质细胞的肿瘤4例。在显微镜下行肿瘤全切除+海马杏仁核切除14例,次全切除8例。术后症状较术前明显改善的有14例,不变6例,加重2例,无手术死亡。结论术前充分了解肿瘤的解剖位置和周围结构的毗邻关系,充分解剖外侧裂,利用脑组织的自然解剖间隙,采用显微外科技术切除肿瘤是手术的关键,肿瘤的性质、级别,患者年龄、全身状况是影响肿瘤切除及其预后的重要因素。  相似文献

Metoclopramide is a well-known anti-emetic drug with central and peripheral pharmacological effects. Some authors have reported metoclopramide as an adjunct therapy to other analgesics in patients with migraine attacks. Treatment of migraine headache using a mix of metoclopramide and an NSAID has been patented (European Patent EP1014961) as well as a short series showing great efficacy and tolerability of metoclopramide in patients wtih migraine attacks. We decided to conduct an open, single-blind, parallel control study in the emergency department to evaluate the efficacy and tolerability of metoclopramide in patients with severe migraine attacks. 93 consecutive patients with severe migraine attacks were randomized into two groups (groups A and B). Patients in group A received 10mg of intravenous metoclopramide and patients in group B received 1 g of intravenous paracetamol. Patients were evaluated 5 minutes before (baseline), 15, 30, 60 and 120 minutes after drug delivery, and before being discharged from the emergency department They were then contacted by phone 48 hours after being discharged from the hospital (phone questionnaire). Patients treated with either metoclopramide or paracetamol showed a significant reduction in the intensity of pain at the 120 minute time point, with an 86% and 82% improvement respectively. However, patients treated with metoclopramide showed a more rapid improvement at the 15 and 30 minute evaluations. Patients with severe migraine attacks treated with metoclopramide as monotherapy showed a significant improvement in terms of pain relief and a faster improvement in pain intensity compared to those treated with paracetamol. Metoclopramide and other dopamine antagonistic drugs should be considered a therapeutic option in severe migraine headache attacks.  相似文献

目的 探讨影响微血管减压术治疗三叉神经痛手术疗效的因素。方法 分析 6 2例经微血管减压术治疗的三叉神经痛患者的临床特征、术中所见和术后疗效。血管对神经根的压迫程度分为单纯接触、接触和移位、单纯粘连、粘连和移位、萎缩五种。手术疗效包括术后疼痛立即缓解、延迟缓解、明显减轻和无效。结果  6 2例患者起病时均表现为典型三叉神经痛 ,但在术前 17例已经转变为不典型。术中发现压迫血管与三叉神经根之间单纯接触 14例、接触和移位 7例、单纯粘连 15例、粘连和移位 18例、萎缩 8例。术后平均随访 14个月 ,疼痛在术后立即缓解 32例 (5 1 6 % ) ,延迟缓解 17例 (2 7 4 % ) ,明显减轻 11例 (17 7% ) ,无效 2例。结论 病程短、症状典型、以动脉压迫为主且能够充分减压的患者 ,术后多能获得好的疗效。相反 ,以静脉压迫为主 ,病程长及症状不典型的患者 ,术后疗效多不理想  相似文献

目的探讨经皮脊柱内镜完全可视化椎间孔成形技术治疗腰椎间盘突出症的短期疗效。方法回顾性分析2016年6月至2018年12月复旦大学附属闵行医院神经外科(18例)和复旦大学附属中山医院神经外科(14例)采用经皮脊柱内镜完全可视化椎间孔成形技术治疗的32例腰椎间盘突出症患者的临床资料。术后复查腰椎MRI,并行疼痛视觉模拟评分(VAS)、日本骨科协会(JOA)腰椎评分及Oswestry功能障碍指数(ODI)评估疗效。对所有患者行门诊随访,随访内容包括VAS、JOA评分、ODI评估及改良MacNab疗效评估。结果32例患者均顺利完成手术,术后均未出现感染、椎管内血肿及神经功能障碍。术后复查腰椎MRI显示突出的髓核摘除基本满意。与术前比较,32例患者术后2 d的VAS评分[分别为(2.21±1.18)分、(6.25±1.46)分]和ODI[(30.28±7.42)%、(68.63±11.04)%]均下降(均P<0.001),JOA评分升高[分别为(16.66±1.58)分、(12.43±1.85)分,P<0.001]。32例患者的随访时间为(15±5)个月(10~20个月)。随着随访时间的推移,32例患者的疼痛VAS评分呈下降趋势、JOA评分呈上升趋势、ODI呈下降趋势(F值分别为187.43、72.54、564.38,均P<0.001)。术后12个月随访时,32例患者的改良MacNab疗效评定结果为,优20例、良9例、可2例、差1例;优良率为90.6%(29/32)。结论经皮脊柱内镜完全可视化椎间孔成形技术治疗腰椎间盘突出症可改善疼痛和神经功能,帮助患者恢复生活和工作,短期疗效较好,远期效果需进一步随访观察。  相似文献

The falcine sinus is an abnormal anatomic structure located in the falx cerebri that is closed after birth and is rarely observed. We describe two cases of persistent falcine sinus. A 60-year-old woman presented with headache. An 11-year-old girl presented with intermittent headache and a palpable scalp mass in the middle of the high parietal area. The straight sinuses were absent in both patients. In both patients, drainage of the galenic system took place through a sinus within the falx, also known as a falcine sinus. Suspicious dysplastic tentorium cerebelli was observed in one patient. It can be concluded that a mesenchymal disorder can be the primary cause for a persistent falcine sinus.  相似文献
