目的:研究支架式可摘局部义齿对牙周炎伴牙列缺损的修复效果。方法:对10例完成牙周治疗的患者采用支架式可摘局部义齿修复,分别记录义齿戴入时,半年,1年和2年后的菌斑指数(PI),龈沟出血指数(SBI)和牙周袋深度(PD)。结果:2年后PI和SBI指数增加,但1年后和2年后PD指数均有统计学意义的下降。结论:支架式可摘局部义齿对修复牙周炎伴牙列缺损具有一定稳定基牙,利于牙周恢复的效果。  相似文献

The segmented framework removable partial denture is made of two cast metal individual segments joined with a resin major connector. The small individual framework segments can be made to fit well and the design permits ready correction of the tissue surface of the major connector through relining or an altered cast procedure. In process of fabrication of a fixed partial denture, dies are trimmed to expose margins of the preparations. The need for the soft tissue cast is quite evident as the soft tissue emergence profile that surrounds the prepared tooth is destroyed in the process of fabrication. This article describes a modified technique to fabricate the soft tissue cast for the conventional fixed partial denture. The impression made with the polyvinylsiloxane was first poured to prepare the die cast. After retrieval of the cast, the same impression was poured second time with the resin based resilient material to cover the facial and proximal gingival areas. The remaining portion of the impression was poured with the gypsum material. This technique does not require additional clinical appointment, second impression procedure, technique sensitive manipulations with impression, or cumbersome laboratory procedures. The simplicity of this technique facilitates and justifies its routine use in fabrication of the fixed partial denture.  相似文献

When considering prosthetic replacement of absent teeth, a treatment plan is necessary. In evaluating whether replacement of teeth is sensible, the causes and consequences of tooth loss must be considered. This concerns information about problems having to do with the patient, potential complications, purely prosthodontic problems, and specific problems. The patient-related problems require collecting data through patient history as well as a clinical and radiologic examination. Complications are risk-factors which negatively influence the prognosis of general health or of a treatment. Potential complications should be eliminated as much as possible through a preliminary treatment. Purely prosthodontic problems are conditions in the area of absence of several teeth or parts of them, and in the area of occlusal and mandibular stability and articulation. Specific problems may be related to inadequate interocclusal space and the quality of the abutment teeth. A cast metal frame removable partial denture may be a relatively inexpensive and minimally treatment alternative for an expensive and complicated treatment with 1 or more fixed partial dentures.  相似文献

A unilateral complete dentition with tight intercuspal contacts complicates the design of the interocclusal retentive elements of a cast removable partial denture. Use of an Adams clasp, which is connected to a predesigned retentive mesh in the metal framework, is suggested as a conservative and economical treatment option in this situation. A technique is described that allows a removable partial denture with a broken clasp or a removable partial denture in which an abutment has been extracted to be restored by the reattachment of a new cast clasp component or a complete surveyed clasp assembly. The technique is applicable to most clasp designs and can include attachment to the acrylic denture base or the metal major connector. The technique is distinguished from other repair techniques by (1) providing a cast clasp replacement, (2) allowing the patient to retain the prosthesis during the repair, (3) generating a precisely formed surveyed clasp assembly by the laboratory, and (4) including an efficient transfer mechanism for the precision clasp created in the laboratory to be attached to the removable partial denture in the dental office. Digital workflows for removable partial dentures have enabled new machinable framework materials, including polyetheretherketone (PEEK). In addition, all denture components can be designed and manufactured digitally. Nonetheless, physical casts are still required for assembly of the components. The adoption of a cast-free fabrication procedure may enhance benefits, including a reduced number of procedures, faster processing, and reduced material waste. The purpose of this technical report was to demonstrate a workflow for the fabrication of a removable partial denture with a PEEK framework, milled teeth, and milled flanges, which uses intraoral scanning and no physical casts.  相似文献

Our aim is to report an application of a prefabricated light-polymerizing plastic pattern to construction of removable partial denture framework without the use of a refractory cast. A plastic pattern for the lingual bar was adapted on the master cast of a mandibular Kennedy class I partially edentulous patient. The pattern was polymerized in a light chamber. Cobalt-chromium wires were employed to minimize the potential distortion of the plastic framework. The framework was carefully removed from the master cast and invested with phosphate-bonded investment for the subsequent casting procedures. A retentive clasp was constructed using 19-gauge wrought wire and was welded to the framework by means of laser welding machine. An excellent fit of the framework in the patient's mouth was observed in the try-in and the insertion of the denture. The result suggests that this method minimizes laboratory cost and time for partial denture construction.  相似文献

A step-by-step procedure has been presented for fabrication of a cast crown to fit an existing partial denture clasp. The procedure can be accomplished in two appointments, and the patient need not be subjected to the inconvenience of being without the partial denture between office visits. It is an accurate and easy technique which gives uniform results and eliminates the need for remaking an adequate removable partial denture.  相似文献
