Purpose: To determine the benefits of submillimeter coronary computed tomography (CT) angiography in vitro.

Material and Methods: The coronary arteries of three domestic pigs were filled with contrast agent and depicted with clinically applicable CT angiography protocols with a slice thickness of 0.63 to 2.5 mm.

Results: With 2.5 mm slices, only the third-degree coronary artery branches could be discerned. With 1.25 mm slices, some fourth-degree branches of the right coronary artery could be discerned upon maximum intensity projections. With 0.63 mm slices, fourth-degree coronary artery branches could be discerned in all locations.

Conclusion: The introduction of 16-row detector CT with submillimeter z-axis resolution translates into an order-of-magnitude benefit for the depiction of coronary artery branches.  相似文献

Background: A matter of substantial concern regarding all needle biopsy techniques is seeding along the biopsy needle tract.

Purpose: To assess cell seeding along the needle tract of vacuum-assisted breast biopsy (VABB).

Material and Methods: The study included 21 patients with ductal carcinoma in situ (DCIS) and 10 patients with invasive ductal carcinoma (IDC) diagnosed by VABB for nonpalpable mammographic lesions. VABB (11G, on a Fischer table) was performed, and the duration of the procedure was measured. After surgery, the whole needle tract was embedded in paraffin blocks, stained with hematoxylin-eosin, and examined by a pathologist.

Results: Cases with dissemination of cancer cells in the needle tract were not observed (one-sided 97.5% CI 0-10.0%). In 2/31 (6.5%) cases (95% CI 0.8-21.4%), benign epithelial cell displacement was observed, and the duration of VABB was significantly longer in these two cases (52.5±3.5 min vs. 42.0±4.4 min for cases without benign cell displacement; P = 0.018, Mann-Whitney-Wilcoxon test for independent samples).

Conclusion: No displacement of malignant cells within the 11G needle tract was documented. Benign cell displacement was associated with longer VABB duration. The phenomenon of tumor cell dissemination along the needle tract is of questionable clinical significance when the treatment guidelines are followed.  相似文献

Purpose: To determine the intra- and interobserver reliability in grading disc and muscle parameters using low-field magnetic resonance imaging (MRI).

Material and Methods: MRI scans of 100 subjects representative of the general population were evaluated blindly by two radiologists. Criteria for grading lumbar discs were based on the spinal nomenclature of the Combined Task Force and the literature. Consensus in rating was achieved by evaluating 50 MRI examinations in tandem. The remaining 50 examinations were evaluated independently by the observers to determine interobserver agreement and re-evaluated by one of the observers to determine intra-observer agreement.

Results: Intra- and interobserver agreement was substantial when grading changes in the lumbar discs. Interobserver agreement was fair to moderate in grading the lumbar muscles, whereas intra-observer agreement was almost perfect.

Conclusion: Convincing reliability was found in the evaluation of disc- and muscle-related MRI variables.  相似文献

Purpose: To present MRI findings in two cases of cerebral hydatid disease with an emphasis on diffusion-weighted imaging (DWI) findings of Echinococcus granulosus (EG) versus Echinococcus alveolaris (EA).

Results: EG lesions were isointense with cerebrospinal fluid in all sequences including DWI. On DWI, EA lesions remained hypointense on b=1000 s/mm2 diffusion-weighted images. Apparent diffusion coefficient (ADC) values of EG and EA lesions were completely different from each other, 2.88±0.24×10-3 s/mm2 and 1.33±0.15×10-3 s/mm2, respectively.

Conclusion: The ADC values could not be used to discriminate from other differential diagnoses.  相似文献

Purpose: To assess the success rate and complication rate of a CT-guided pulmonary nodule-marker system before thoracoscopic resection.

Material and Methods: In 24 patients (15 M, 9 F; age range, 18-71 years) a total of 25 pulmonary nodules (in 1 patient 2 lesions simultaneously) were marked with a special wire under CT-guidance and then thoracoscopically resected. We evaluated lesion size, lesion distance to the pleura, the time of intervention, complications, and thoracoscopic success rate.

Results: Mean lesion size was 7 mm (range 4-15 mm) and mean lesional distance to the pleura was 13 mm (range 2-31 mm). The pulmonary nodule-marker system was positioned successfully in all 25 pulmonary nodules within 5-11 min (mean 7.5 min). Minimal pneumothoraces were observed in five patients with no requirements of chest drains. In addition, no bleeding complications or hematothorax were observed. All 25 pulmonary nodules could be resected thoracoscopically. However, in one patient (4%), the guide-wire dislocated during thoracoscopy, but the lesion could be successfully resected during thoracoscopy.

Conclusion: The CT-guided placement of the pulmonary nodule-marker system used here offers a safe and accurate guide for the localization of small pulmonary nodules during thoracoscopic resection.  相似文献

Background: Multidetector computed tomography (MDCT) enterography combines neutral enteric contrast with intravenously administered contrast material. The optimal intravenous (IV) contrast material protocol has still not been established.

Purpose: To determine the optimal delay time to image patients with small-bowel Crohn's disease during MDCT enterography.

Material and Methods: After oral administration of 1350 ml of neutral contrast medium, 26 patients with small-bowel Crohn's disease underwent MDCT enterography; scans were obtained 40 s (enteric phase) and 70 s (parenchymal phase) after IV administration of 100 ml of iodinated contrast material. Three radiologists, blinded to clinical and pathological findings, independently and retrospectively evaluated each scan in two separate reading sessions for the presence or absence of CT features of Crohn's disease activity. The interobserver agreement was evaluated, and the efficacy of each phase in detecting active disease in the terminal ileum for each reader was determined. The gold standard was pathology (n = 13), endoscopy (n = 3), and clinical evaluation (n = 10).

Results: No statistically significant difference was present between the enteric and the parenchymal phase for each reader in each segment regarding the presence or absence of CT features of Crohn's disease. The interobserver agreement for the presence of five main features of active Crohn's disease in the terminal ileum ranged from poor to excellent. The sensitivity, specificity, negative predictive value, positive predictive value, and accuracy for active Crohn's disease in the terminal ileum ranged from 40 to 90%, 88 to 100%, 70 to 94%, 44 to 100%, and 69 to 96%, respectively. There was no statistical difference between the two phases for each reader.

Conclusion: MDCT enterography in patients with suspected active Crohn's disease can be obtained at either 40 s or 70 s after IV contrast material.  相似文献

Purpose: To establish the reproducibility of a standardized region of interest (ROI) drawing procedure in delayed gadolinium-enhanced magnetic resonance imaging (MRI) of cartilage (dGEMRIC).

Material and Methods: A large ROI in lateral and medial femoral weight-bearing cartilage was drawn in images of 12 healthy male volunteers by 6 investigators with different skills in MRI. The procedure was done twice, with a 1-week interval. Calculated T1-values were evaluated for intra- and interobserver variability.

Results: The mean interobserver variability for both compartments ranged between 1.3% and 2.3% for the 6 different investigators without correlation to their experience in MRI. Post-contrast intra-observer variability was low in both the lateral and the medial femoral cartilage, 2.6% and 1.5%, respectively. The larger variability in lateral than in medial cartilage was related to slightly longer and thinner ROIs.

Conclusion: Intra-observer variability and interobserver variability are both low when a large standardized ROI is used in dGEMRIC. The experience of the investigator does not affect the variability, which further supports a clinical applicability of the method.  相似文献

Background: The rotation ratios method describes rotations between pairs of sequential pelvic radiographs. The method seems promising but has not been validated.

Purpose: To validate the accuracy of the rotation ratios method.

Material and Methods: Known pelvic rotations between 165 radiographs obtained from five skeletal pelvises in an experimental material were compared with the corresponding calculated rotations to describe the accuracy of the method. The results from a clinical material of 262 pelvic radiographs from 46 patients defined the ranges of rotational differences compared. Repeated analyses, both on the experimental and the clinical material, were performed using the selected reference points to describe the robustness and the repeatability of the method.

Results: The reference points were easy to identify and barely influenced by pelvic rotations. The mean differences between calculated and real pelvic rotations were 0.0° (SD 0.6) for vertical rotations and 0.1° (SD 0.7) for transversal rotations in the experimental material. The intra- and interobserver repeatability of the method was good.

Conclusion: The accuracy of the method was reasonably high, and the method may prove to be clinically useful.  相似文献

Purpose: To describe the findings of hyperperfusion on perfusion computed tomography (CT) in four patients following revascularization for acute stroke.

Material and Methods: In 2002-2003, among a series of 6 patients presenting with an acute stroke and treated with intra-arterial thrombolysis, we observed the presence of hyperperfusion in 3 patients on the follow-up CT perfusion. We included an additional patient who was treated with intravenous thrombolysis and who had hyperperfusion on the follow-up CT perfusion. We retrospectively analyzed their CT perfusion maps. Cerebral blood volume (CBV) and cerebral blood flow (CBF) maps were compared between the affected territory and the normal contralateral hemisphere.

Results: In the four patients, the mean CBV and CBF were 3.6±2.0 ml/100 g and 39±25 ml/100 g/min in the affected territory compared to the normal side (mean CBV = 2.7±2.1 ml/100 g, mean CBF = 27±23 ml/100 g/min). There was no intracranial hemorrhage in the hyperperfused territories. At follow-up CT, some hyperperfused brain areas progressed to infarction, while others retained normal white to gray matter differentiation.

Conclusion: CT perfusion can demonstrate hyperperfusion, which can be seen in an ischemic brain territory following recanalization.  相似文献

Purpose: To investigate whether three stereotactic core needle biopsies (S-CNB) in non-palpable breast lesion are enough for accurate preoperative diagnosis.

Material and Methods: Between September 1994 and December 2000, 523 patients with mammographically detected breast lesions and who proceeded to surgery were preoperatively stereotactically biopsied with an automated 14-gauge biopsy device. Three samples were taken from each lesion irrespective of whether the lesion presented as “microcalcifications only”, “microcalcifications and a mass”, or a “mass, architectural distorsion, or stellate lesion without microcalcifications”. The histopathology was divided into three subgroups: diagnostic, atypia (ranging from atypical cells to probably cancer), and non-diagnostic material.

Results: Post-surgical histopathology diagnosed 454 (87%) malignant tumors and 69 (13%) benign lesions. Three S-CNB correctly diagnosed the malignant tumors in 84% in the subgroup “microcalcifications only”. In the category “microcalcifications and a mass”, the diagnostic accuracy was 97% and in the subgroup “mass, architectural distorsion, or stellate lesion without microcalcifications” 3 S-CNB resulted in 93% correct diagnostic material. In 19 of the 454 patients (4%) 1, 2 or all 3 preoperative S-CNB showed atypia. In 20 patients (4%), all 3 S-CNB were non-diagnostic. Thirteen of these 20 patients had “microcalcifications only” and 7 had a mass without microcalcifications.

Conclusion: Three S-CNB were enough for correct diagnosis in “masses, architectural distorsions, or stellate lesions without microcalcifications” and in “microcalcifications and a mass”, but were not sufficient in “microcalcifications only”.  相似文献

Purpose: To validate a clinically useful method for measuring acetabular cup wear using computed tomography (CT).

Material and Methods: Eight uncemented acetabular cups were scanned twice ex vivo using CT. The linear penetration depth of the femoral component head into the cup and the thickness of the remaining polyethylene liner were measured in the CT volumes using dedicated software. Two independent examiners twice assessed each volume. The CT measurements were compared to direct measurements using a coordinate measuring device and micrometer measurements.

Results: Accuracy of wear measurements expressed as penetration depth was ±0.6 and ±1.0 mm for the two examiners, respectively, with no significant differences between examiners, trials, and CT scans. Accuracy of measurements of remaining polyethylene was ±1.3 and ±1.0 mm, respectively, for the two examiners. Systematic differences between examiners were found, but no significant differences between trials and CT scans. These differences were due to different interpretations of metal artifacts in the volumes.

Conclusion: The proposed CT method for evaluating wear as head penetration depth allows for reliable wear detection at a clinically relevant level. Measurements of remaining polyethylene on CT volumes are not as reliable as wear measurements owing to metal artifacts.  相似文献

Purpose: To establish whether information would be lost if slice reconstruction thickness was increased from 3 to 5 mm, and whether this altered how difficult it was to interpret the examinations.

Material and Methods: Twenty-three consecutive patients referred with suspected or known urinary stones were included. All examinations were performed without intravenous contrast media. The original series, with effective mAs 50, were reconstructed with slice thickness 3 and 5 mm, respectively. All demographic and examination data were removed and the series reviewed in PACS by two independent radiologists. Objective findings, i.e. number and size of stones, signs of obstruction, and evaluation of interpretation difficulty, were registered.

Results: Identical findings were registered in 18 of the series of 3 mm (n = 23) and 19 of the series of 5 mm (n = 23). In two series reconstructed with 3 mm slice thickness and in one series with 5 mm slice thickness, the observers disagreed on the presence of urinary stones. Main reasons for interpretation difficulties were given as “lack of intra-abdominal fat” and “many phleboliths in the pelvic region”, but never “disturbing noise”.

Conclusion: To determine the presence and size of urinary stones at low-dose computed tomography, 5 mm reconstruction algorithm seems equal to 3 mm. Patient-related factors influence the interpretation more than image quality.  相似文献

3D imaging of the whole spine at 3T compared to 1.5T: initial experiences   总被引:2,自引:0,他引:2  
Purpose: To describe our first experiences with a recently introduced 3T system for T2-weighted isotropic 3D whole-spine imaging.

Material and Methods: Magnetic resonance imaging of the whole spine was performed by implementing an isotropic 3D fast spin-echo sequence with variable flip-angle refocusing pulses at 3T and 1.5T. Signal-to-noise ratio (SNR) was compared on both systems in eight subjects.

Results: Mean values for SNR were significantly higher at 3T (346±214) for 1.0 mm voxel size compared to 1.5T (202±41), but showed considerably higher variability at 3T. At 3T, measurements with 0.9 mm voxel size were possible with similar SNR as with 1.0 mm voxel size at 1.5T.

Conclusion: High-resolution 3D imaging of the whole spine is feasible at 3T with an increased SNR compared to 1.5T. Signal gain at 3T can be used to further increase spatial resolution. Pronounced interpatient variability of SNR at 3T may be a result of inhomogeneous RF deposition due to dielectric effects.  相似文献

Purpose: 
Validation of a non-invasive CT method for detection of acetabular cup migration after total hip arthroplasty in a phantom study. Material and Methods: 
26 CT examinations were obtained of a pelvic model while altering the position of the acetabular cup. Using a previously described program for volume merging, the pelvi in different examinations were fused and the 3D alterations of the position of the acetabular cup were evaluated visually and numerically and correlated to direct measurements on the model. Results: 
Visually, two independent examiners differentiated between 0, 1 and 2 to 3 mm migration with 100% specificity and sensitivity. Numerically, the mean error over all cases between model and CT measurements was 0.04 mm (SD ± 0.33). The mean absolute error between model and CT data was 0.26 mm (SD ± 0.19). Intra- and interobserver 95% accuracy and repeatability limits were below 0.5/0.7 mm, respectively. No significant interobserver difference occurred. The data were normally distributed and not dependent on observer. Conclusion: 
The accuracy of this non-invasive method out-performs routine plain radiography. The method gives both visual and numerical correlates to migration and can be used in clinical practice.  相似文献

Background: Attempts to retrieve absolute values of cerebral blood flow (CBF) by dynamic susceptibility contrast magnetic resonance imaging (DSC-MRI) have typically resulted in overestimations.

Purpose: To improve DSC-MRI CBF estimates by calibrating the DSC-MRI-based cerebral blood volume (CBV) with a corresponding T1-weighted (T1W) steady-state (ss) CBV estimate.

Material and Methods: 17 volunteers were investigated by DSC-MRI and 133Xe SPECT. Steady-state CBV calculation, assuming no water exchange, was accomplished using signal values from blood and tissue, before and after contrast agent, obtained by T1W spin-echo imaging. Using steady-state and DSC-MRI CBV estimates, a calibration factor K = CBV(ss)/CBV(DSC) was obtained for each individual. Average whole-brain CBF(DSC) was calculated, and the corrected MRI-based CBF estimate was given by CBF(ss) = K×CBF(DSC).

Results: Average whole-brain SPECT CBF was 40.1±6.9 ml/min·100 g, while the corresponding uncorrected DSC-MRI-based value was 69.2±13.8 ml/min·100 g. After correction with the calibration factor, a CBF(ss) of 42.7±14.0 ml/min·100 g was obtained. The linear fit to CBF(ss)-versus-CBF(SPECT) data was close to proportionality (R = 0.52).

Conclusion: Calibration by steady-state CBV reduced the population average CBF to a reasonable level, and a modest linear correlation with the reference 133Xe SPECT technique was observed. Possible explanations for the limited accuracy are, for example, large-vessel partial-volume effects, low post-contrast signal enhancement in T1W images, and water-exchange effects.  相似文献

Purpose: To review magnetic resonance (MR) diffusion-weighted imaging (DWI) and spectroscopy findings in patients with focal cerebral tuberculosis and to assess whether these techniques can adequately characterize focal cerebral tubercular lesions.

Material and Methods: Sixteen patients with single or multiple lesions were evaluated on a 1.5T MR system. DWI was performed with three 'b' values of 50, 500, and 1000 s/mm2 and the apparent diffusion coefficient maps were calculated. MR spectroscopy was performed using the point-resolved single-voxel technique with 2 echo time values of 135 ms and 270 ms. The signal intensities of the tubercular lesions on diffusion images and the apparent diffusion coefficients (ADCs) of their centers, along with MR spectroscopy findings, were analyzed in relation to their T2-weighted MR appearances.

Results: DWI identified 17 of the 20 lesions evaluated. Increased signal intensity was seen in 9 of the 17 lesions. The ADCs of the lesions ranged from 0.406 to 2.64×10-3 mm2/s (mean±SD: 1.038±0.609 mm2/s). Most of the lesions with hyperintense centers on T2-weighted images were of increased intensity on diffusion images, while those with hypointense centers on T2-weighted images were of decreased signal intensity on diffusion images. However, no statistical difference in the ADCs was found between lesions with increased and those with decreased signal intensity centers on T2-weighted images. MR spectroscopy revealed a lipid peak at 0.9-1.3 ppm in all of the 14 lesions evaluated. An increase in normalized choline:creatine ratio was found in all the lesions in which the spectra were obtained with the voxel, including a variable portion of the lesion wall.

Conclusion: DWI and MR spectroscopy help in determining the nature of cerebral tubercular lesions; however, since the findings are varied, they do not help in specific characterization.  相似文献

Purpose: To retrospectively register the number and type of complications following ultrasound-guided thoracocentesis, and to evaluate the need for routine chest X-ray after the procedure.

Material and Methods: Complications were retrospectively registered from the radiological and clinical reports of 371 consecutive patients who had undergone thoracocentesis with a total of 711 procedures.

Results: The mean volume evacuated was 823 ml (range 0-3600 ml). Twenty (2.8%) pneumothoraces were found after 711 thoracocenteses, but in no case was chest tube drainage necessary. Hemoptysis occurred in one patient.

Conclusion: Ultrasound-guided thoracocentesis was found to be a safe procedure. Based on these results, no reason was found to introduce an upper limit of the amount of fluid drained in one session. Routine follow-up chest X-ray is not justified in the absence of clinical symptoms.  相似文献

Background: Recent concepts about cerebrospinal fluid (CSF) circulation in communicating hydrocephalus (CoHy), which is also termed “restricted arterial pulsation hydrocephalus,” suggest reduced arterial pulsations of subarachnoid vessels with a smaller amount of CSF shifted in subarachnoid spaces during the early systole. The postulated restriction of subarachnoid arterial pulsations in CoHy should induce a smaller motion artifact and reduced local stream effects in CSF in magnetic resonance (MR) diffusion-weighted imaging (DWI).

Purpose: To investigate the maximum diffusivity in CSF in patients with and without CoHy using DWI.

Material and Methods: 12 patients without CSF circulation disturbances and six cases with proven CoHy were assessed. Diffusion was measured in six noncollinear directions without triggering the arterial pulse wave (scan time 6:45 min, voxel size 2×2×2 mm). Due to expected artifacts, the calculated maximum diffusivity was called apparent diffusivity. Regional high and low apparent diffusivity was assessed in CSF spaces on newly created 3D CSF motion maps.

Results: Patients with regular CSF circulation exhibited high apparent diffusivity in CSF in basal subarachnoid spaces, whereas apparent diffusivity was low there in patients with CoHy.

Conclusion: DWI opens a feasible approach to study CSF motion in the neurocranium. Restricted arterial pulsations seem to be involved in CoHy.  相似文献

Background: A longstanding hypothesis that correlates fluid dynamic forces and atherosclerotic disease has led to numerous analytical, numerical, and experimental studies over the years because it is very difficult to measure the hemodynamic variables of blood in vivo.

Purpose: To investigate the technique of visualization and quantitation of hemodynamic variables at carotid artery bifurcation in vivo by combining computational fluid dynamics (CFD) and vascular imaging.

Material and Methods: Twenty-six healthy volunteers underwent magnetic resonance (MR) angiography of the bilateral carotid artery by a 3.0T whole-body scanner. Hemodynamic variables at these carotid bifurcations were calculated and visualized by combining vascular imaging post-processing and CFD.

Results: The average velocity of the carotid bifurcation in the systolic phase and the diastolic phase was 0.46±0.24 m/s and 0.23±0.05 m/s, respectively. Eddy current and back flows were observed at bifurcation and the lateral part of the proximal internal carotid arteries (ICA) and external carotid arteries (ECA), and the shapes of them changed with phases of the cardiac cycle, which were significant at the middle of the systolic phase and faded out quickly downstream of the ICA and ECA. The average range of wall shear stress (WSS) at the bifurcation was 4.36±1.32 Pa, and the maximum WSS was 18.02±4.11 Pa. The WSS map revealed a large region of low WSS at the carotid bulb and extended to the outer wall in the proximal end of the ICA (the lowest value was below 0.5 Pa), and there was also a small region of low WSS at the outer wall in the proximal end of the ECA.

Conclusion: CFD combined with vascular imaging can calculate and visualize hemodynamic variables at carotid bifurcation in vivo individually.  相似文献
