OBJECTIVE: To describe the MR imaging features of tuberculous osteomyelitis. DESIGN AND PATIENTS: MR imaging features of 11 patients (14-65 years) with proven extra-spinal tuberculous osteomyelitis were reviewed. Osseous and adjacent soft-tissue changes were analyzed. RESULTS: On the basis of the signal intensity characteristics compared with the normal marrow fat, two kinds of lesions were observed: (a) predominantly intermediate to low signal intensity lesions on T2-weighted images with low signal intensity on T1-weighted images, and (b) lesions which had a discrete peripheral zone of marginally higher signal intensity than the center on T1-weighted images and surrounding edema and lower signal intensity than the fatty bone marrow with variable signal intensity on T2-weighted images. Soft-tissue abscesses and marrow edema were each noted in eight cases. Soft-tissue edema was noted in most cases. CONCLUSION: An osseous lesion with intermediate to low signal intensity on T2-weighted images and associated soft-tissue abscess may be suggestive of tuberculous osteomyelitis. Lesions with a rim of mildly increased signal intensity on T1-weighted images, a non-specific indicator of an infective process, may also be seen.  相似文献

Characteristic MR findings of cervical pregnancy   总被引:2,自引:0,他引:2  
The aim of this study was to define the characteristic MR findings of cervical pregnancy. Twelve patients with cervical pregnancy underwent MRI because of difficulty in the diagnosis by ultrasonography, human chorionic gonadotropin assessment, and other clinical evaluations. The assessment of MRI included size and location of the lesion, margin, MR signal intensity, rim of low-signal intensity, enhancement pattern, appearance of enhancing solid component, parametrial change, endometrial change, pelvic fluid collection, and ovarian change. All cases showed ill-marginated mass with very heterogeneous signal intensity on T2-weighted images, irregular internal high-signal intensities on T1-weighted images, a partial or circumferential rim of low-signal intensity, dense irregular peripheral enhancement and enhancing papillary solid components with accompanying tubular signal voids, and variably increased parametrial vascularities. This heterogeneous hemorrhagic mass with densely enhancing papillary solid components may be the typical MR finding for cervical pregnancy. J. Magn. Reson. Imaging 2001;13:918-922.  相似文献

Signal-intensity characteristics of magnetic resonance (MR) images were assessed in five patients and in 10 rabbits with tuberculous arthritis. MR imaging findings were compared with histologic findings in the animal study. In both of clinical and experimental cases, tuberculous lesions showed both intermediate and high signal intensity on T2-weighted images, while they showed heterogeneously intermediate signal intensity on T1-weighted images. As T2-weighted images were compared with pathologic specimens, intermediate signal intensity corresponded to caseous necrosis, whereas high signal intensity related to granulomas or effusion. Postcontrast T1-weighted images showed enhancement at the peripheries rather than the centers of tuberculous lesions. These results indicate that tuberculous arthritis should be included in the differential diagnosis when intra-articular lesions with low or intermediate signal intensity are found on T2-weighted images.  相似文献

Five patients with a palpable mass at presentation underwent magnetic resonance (MR) imaging. The final diagnosis was myositis ossificans (MO). MR imaging features, particularly after injection of gadopentetate dimeglumine, mimicked those of an inflammatory mass or neoplasm. The lesions were excised in three patients, and the Images were correlated with histologic findings. Three different appearances were noted on MR images, corresponding to the stages of maturation of MO. Two cases Involved early-stage lesions, and Tl-weighted MR images showed a mass with homogeneous intermediate signal intensity. Both lesions showed rim enhancement after contrast agent injection and high signal intensity on T2-weighted images. Pathologic specimens demonstrated stroma with masses of spindle cells in which osteoid production was interspersed. The enhanced rim of the lesion mimicked the expected MR appearance of an abscess or necrotic tumor. Areas of enhancement in adjacent muscle were also seen on postcontrast T1-weighted images. Intermediate-stage MO was present in one case; there was evidence of a thin rim of calcification on plain radiographs and fatty changes in the lesion on T1-weighted Images, corresponding with histologic findings. One case of a mature lesion showed a considerable degree of peripheral calcification both on MR images and at histology. MR imaging is nonspecific in the diagnosis of early-stage MO.  相似文献

Magnetic resonance imaging of pyomyositis in 43 cases   总被引:4,自引:0,他引:4  
PURPOSE: To describe the magnetic resonance imaging (MRI) findings in pyomyositis. METHODS AND MATERIALS: Forty-three patients with proven muscle infection (30 males, 13 females) ranging in age from 14 to 86 years (mean 42 years) were studied with MRI. The initial clinical diagnose were soft tissue infection (n=27), neoplasm (n=12), thrombophlebitis (n=3), and lymphedema (n=1). Spin-echo T1- and T2-weighted images were obtained in all cases and STIR sequence in 6. Spin-echo T1-weighted images after Gd-DTPA injection were obtained in 16 cases. The signal intensity findings, the extent of the abnormalities in the soft tissue (muscle, fascial and subcutaneous involvement), the presence of fluid collections, and the involvement of neighbouring bone and joint were reviewed retrospectively. RESULTS: A hyperintense signal on T2-weighted and STIR images were detected in all patients. Fluid collections were seen in 21 cases as localized areas of hypointensity on the T1-weighted images, and highly hyperintense areas on the T2-weighted images. In four patients a rim of high signal intensity was seen around the fluid collection on the T1-weighted images. On contrast-enhanced T1-weighted images there was diffuse enhancement in the patients without fluid collections that was heterogeneous in seven and homogeneous in two. After Gd-DTPA all fluid collections showed a central area without enhancement and a well-defined enhancing peripheral rim. Involvement of adjacent structures included subcutaneous tissue (n=25), bone marrow (n=14), fascial planes (n=15) and joints (n=11). CONCLUSION: MRI is useful in the assessment of pyomyositis and in determining the location and extension. A hyperintense rim on unenhanced T1-weighted images and peripheral enhancement after Gd-DTPA are useful for identifying the number, size, and location of soft-tissue abscesses.  相似文献

OBJECTIVE: The objective of our study was to define the MRI features of tuberculous infection of the wrist. MATERIALS AND METHODS: We present the MRI findings of eight patients with tuberculous infection of the wrist. Spin-echo T1-weighted, gradient-echo T2(*)-weighted, and fast spin-echo T2-weighted sequences were performed for all patients. Gadolinium-enhanced MR images were obtained in seven patients. All images were evaluated for the characteristics of tuberculous infection of the wrist, including the presence of synovial thickening around the joints and tendons, signal intensity of the thickened tenosynovium and synovium on the T2-weighted images, synovial fluid collection in the tendon sheath, small low-signal and nonenhanced foci in the synovial fluid, bone erosion, osteomyelitis, and encasement of the median nerve. RESULTS: The tuberculous infection involved the right (n = 6) and left (n = 2) wrists. All patients had synovial thickening around the flexor and extensor tendons with synovial fluid collection in the tendon sheath. The thickened tenosynovium and synovium revealed low signal intensity on T1-weighted images, intermediate to low signal intensity on T2- and T2(*)-weighted images, and enhancement on contrast-enhanced MR images. The synovial fluid showed intermediate to low signal on T1-weighted images and homogeneous or heterogeneous high signal intensity on T2- and T2(*)-weighted images. Multiple small foci of low signal intensity and nonenhancement scattered in the synovial fluid were present in seven patients. Bone erosion occurred in seven patients, osteomyelitis was seen in six patients, and encasement of the median nerve was found in three patients. CONCLUSION: Characteristic MRI findings of tuberculous infection of the wrist include synovial thickening around the flexor and extensor tendons and synovial fluid collection that contains small low-signal and nonenhanced foci in the tendon sheath. Bone erosion, osteomyelitis, and median nerve encasement are also frequently present. These characteristic manifestations are helpful in diagnosing this disease entity.  相似文献

The magnetic resonance (MR) imaging features of Brodie abscess have not yet been fully evaluated. Ten patients with Brodie abscess, eight of long bone and two of vertebra, were studied with MR imaging. Long bone abscess had a characteristic “target” appearance with four layers: (a) a center with low signal intensity on T1-weighted images and high signal intensity on T2-weighted and STIR (short-inversion-time inversion recovery) images, (b) an inner ring isointense to muscle on T1-weighted images and with high signal intensity on T2-weighted and STIR images, (c) an outer ring hypoin-tense on all images, and (d) a peripheral halo hypointense on T1-weighted images. In six of eight cases, a soft-tissue mass was found. The two vertebral abscesses had a less specific appearance, with low signal intensity on T1-weighted images and high signal intensity on T2-weighted and STIR images. Only the peripheral halo was clearly identified in both cases.  相似文献

The purpose of this study was to describe the magnetic resonance (MR) imaging features of biliary hamartomas on T1- and T2-weighted and gadolinium-enhanced sequences, and to correlate these findings with histopathology. MR imaging findings in four patients with pathologically proved biliary hamartomas are described. In all patients, MR imaging sequences, including T1- and T2-weighted and early and late gadolinium-enhanced images, were retrospectively evaluated for the size, morphology, signal intensity, and enhancement pattern of the lesions. Correlation was made between the MR imaging findings and histopathology. Biliary hamartomas ranged in diameter from 0.5 to 1.5 cm. Lesions were solitary in one patient and numerous in three patients. In all patients, the lesions were low signal on T1-weighted images and high signal and well-defined on T2-weighted images and demonstrated thin rim enhancement on early post-gadolinium images that persisted on late post-gadolinium images. No appreciable central enhancement of the lesions was observed. At histopathology, the lesions were composed of cystic spaces and fibrous stroma. Lesions showed compressed liver parenchyma surrounding the lesions (three cases) and inflammatory cell infiltrate (one case), which correlated with the rim enhancement on the gadolinium-enhanced MR images. Most of the biliary hamartomas in our small series were less than 1 cm in diameter and of high signal intensity on T2-weighted images, and had a thin rim of enhancement on early and late post-gadolinium images. The imaging features were explainable by the underlying histopathology. In patients with known malignancy, caution should be exercised not to misinterpret these lesions as metastases due to the presence of thin rim enhancement. J. Magn. PURPOSE: The aim of this study was to describe the MR findings in extraspinal musculoskeletal tuberculosis (EMT). METHOD: A retrospective review was conducted of the MR findings of 18 patients with microbiologically and/or pathologically proven EMT. All MR studies were performed using T1-and T2-weighted spin echo sequences. T1-weighted spin echo sequences after Gd-DTPA injection were obtained for 12 patients. The MR images were evaluated for abnormalities in joints, bones, and soft tissues, and the results were grouped by anatomic localization, frequency distribution of structures affected, and morphologic patterns of involvement. RESULTS: Isolated soft tissue tuberculosis was found in 10 (55.5%) patients and involvement of more than one structure in 8 (44.4%). Pyomyositis (n = 6) and arthritis with involvement of adjacent soft tissues (n = 7) were the most common forms of presentation. One patient presented with isolated fascial superficial tissue involvement in one leg. Isolated pyomyositis involving one (n = 3) or two (n = 3) muscles was homogeneous in six cases and showed intermediate (n = 6), low (n = 2), or high (n = 1) signal intensity on T1-weighted images and a high and very hyperintense signal on T2-weighted images. The tenosynovitis synovial fluid was homogeneous (n = 1) or heterogeneous with multiple tiny hypointense nodules (n = 1) on T2-weighted images. The subdeltoid bursitis fluid was characterized by homogeneous low signal intensity with a hyperintense rim (n = 2) on T1-weighted images and homogeneous (n = 1) or heterogeneous hyperintense signals with areas of low signal intensity (n = 1) on T2-weighted images. In tuberculous arthritis, the synovial joint fluid (n = 7) showed heterogeneous (n = 4) or homogeneous (n = 3) low signal intensity on T1-weighted images and high or very high signal intensity on T2-weighted images. Where involved, the adjacent muscle(s) (n = 8) were usually hypointense on T1-weighted images and very hyperintense on T2-weighted images. Associated cellulitis was found in arthritis with involvement of neighboring soft tissues (n = 5), pyomyositis (n = 2), and tenosynovitis (n = 1). The images obtained after Gd-DTPA showed peripheral (n = 10) or heterogeneous (n = 1) enhancement or no enhancement (n = 1). CONCLUSION: The MR findings for EMT are variable. Although diagnosis is dependent largely on prior presumption and clinical context, MRI provides valuable guidelines in defining the extent of the lesions to select the appropriate treatment and for follow-up of abnormalities.  相似文献

Spinal epidural abscess: evaluation with contrast-enhanced MR imaging.   总被引:1,自引:0,他引:1  
Seven patients with spinal epidural abscess were evaluated with MR imaging. T1-weighted images were obtained before and after administration of gadopentetate dimeglumine, and contrast-enhanced images were compared with available T2-weighted images and unenhanced T1-weighted images. In all seven cases, the epidural infection was iso- to hypointense compared with the spinal cord on unenhanced T1-weighted images, and increased in intensity on proton-density- and T2-weighted images. Three patterns of enhancement were observed after contrast administration. In three patients the infection enhanced homogeneously, likely representing thickened, inflammed tissue with microabscesses and granulomatous material. In one patient, peripheral enhancement surrounded a central focus of low signal intensity, representing necrotic abscess. In two patients, a combination of both patterns was observed. One abscess infiltrated the posterior thoracic epidural fat, producing decreased signal within the high-signal fat on T1-weighted images. Enhanced T1-weighted images were equivalent to unenhanced T2-weighted images in detecting the extent of epidural involvement in three cases. In two cases, enhanced T1-weighted images were superior to T2-weighted images in differentiating the infectious component from surrounding CSF. In one case, contrast administration produced no discernible enhancement. Enhanced images also provided important information regarding the composition of the abscess (liquid versus solid). Contrast-enhanced MR images are valuable in the characterization of spinal epidural abscesses.  相似文献

PURPOSE: To report the Magnetic Resonance Imaging (MRI) features of acute and chronic spontaneous spondylodiscitis as well as any typical patterns which can be useful for the differential diagnosis between pyogenic and tuberculous forms. MATERIAL AND METHODS: Eleven patients affected with spontaneous spondylodiscitis were selected for the study; they were 7 men and 4 women ranging in age 33-87 years (mean: 64). We excluded the patients with iatrogenic spondylodiscitis. MR images were acquired with a superconductive magnet at 1.5, with the following sequences: sagittal PD and T2-weighted TSE, sagittal T1-weighted SE, axial PD and T2-weighted TSE for the lumbar spine, axial T2-weighted GRE for the cervical and dorsal spine and axial and sagittal T1-weighted SE after contrast agent (gadolinium DTPA) injection. MR images were reviewed by three experienced radiologists and morphological and signal intensity changes of vertebral body and disk were recorded on a standard form. In 9 patients it was possible to compare MR to CT findings. RESULTS: At the time of our observation all patients reported pain at the spine level, associated with fever and weight loss in 50% of cases and with increased values of the inflammatory markers. Three patients had infectious diseases in other organs and 2 were diabetics. Biopsy was performed in two cases only and demonstrated Staphylococcus aureus in one and Mycobacterium tuberculosis in the other patient. MRI allowed the correct diagnosis to be made in all cases, demonstrating the pathological involvement of the paravertebral structures and into the spinal canal earlier and more accurately than CT. A common finding in pyogenic and tuberculous spondylodiscitis was the low signal of the subcortical bone marrow on T1-weighted sagittal images, which enhanced after Gd-DTPA administration and became intermediate or high on T2-weighted images. Moreover, the steady high signal intensity of the disk on T2-weighted images and its contrast enhancement on T1-weighted images is typical for an acute inflammatory process. CONCLUSIONS: Based on our personal experience and literature data, we believe MRI to be the most sensitive technique for the diagnosis of spondylodiscitis in the acute phase, whereas it is comparable to CT in the chronic stage of the disease. At present MRI does not allow to differentiate pyogenic from tuberculous forms.  相似文献

目的:探讨前列腺脓肿的MRI诊断价值。方法:回顾性分析4例临床证实的前列腺脓肿的MRI表现。结果:单个脓肿2例,多发脓肿2例。脓肿T1WI呈稍低信号或低信号,T2WI呈高信号或较高信号。增强扫描脓肿壁薄环状明显强化2例,厚环状中等度强化2例,内见强化分隔,脓腔不强化。伴精囊腺肿大3例,精囊腺脓肿1例。向前列腺周围组织蔓延2例。结论:前列腺脓肿MRI表现具有特征性,容易发现小脓肿,并可以确定脓肿蔓延的范围。  相似文献

目的:回顾分析单纯疱疹病毒性(HSV)脑炎的MR成像表现,为临床早期诊断和治疗提供依据。材料和方法:10例(男4例,女6例;平均年龄39.5岁)经脑脊液PCR证实的HSV脑炎患者,采用高磁场(1.0T)磁共振扫描仪,以自旋回波序列行横断/冠状面T1加权、质子加权、T2加权成像和注造影剂后T1加权成像。结果:10例中,8例MR像存在异常信号(广泛、多发、双侧者6例,局灶改变2例)。T1加权像呈均质低信号5例,该5例在T2加权像呈均质高信号,提示病理上病灶区坏死,伴周围水肿;3例T1加权像呈片状低信号内伴脑回状高信号,T2加权像呈不均质信号,病理上代表出血;增强后T1加权像呈脑回状强化3例,1例呈结节状改变。结论:以颞叶、岛叶、额底分布为主(尤其为扣带回受累)的多发病灶、伴脑回状出血和强化是HSV脑炎的特征性MR成像表现。不典型表现须与脑瘤、脑梗塞和其它类型病毒性脑炎鉴别诊断。  相似文献

Four cases of brain abscess were examined on 1.5T MR and their findings were evaluated retrospectively. Preoperative cases had relatively characteristic findings, such as 1) peripheral edema producing hypointensity on T1-weighted images (T1WI) and hyperintensity on T2-weighted images (T2WI); 2) central necrosis with abscess fluid hypointense relative to white matter on T1WI and hyperintense on T2WI; 3) abscess rim iso- to hypointense on T2WI. Gd-DTPA enhanced T1WI carried further information; comparison of iso- to hypointense rim on T2WI (abscess rim) and enhanced area by Gd-DTPA on T1WI suggested that the former represented zone of macrophage infiltration.  相似文献

OBJECTIVE: To characterize MR imaging features of pure mucinous carcinoma of the breast. MATERIALS AND METHODS: MR images obtained from 16 women (age range, 29-81; mean age, 57 years) with pure mucinous carcinoma of the breast determined at surgery were reviewed. The MR findings used were shape, margin, internal mass enhancement, kinetic curve pattern on dynamic study, signal intensity on short time inversion recovery (STIR) T2-weighted images, and non-mass-like enhancement around the main tumor. Non-mass-like enhancement was compared with the presence of extensive intraductal component (EIC) on histopathological findings. RESULTS: Eleven tumors (69%) had lobular contour, and nine tumors (56%) had smooth margin. Eight tumors (50%) showed rim enhancement and six tumors (38%) showed heterogeneous enhancement. Fourteen tumors (88%) showed a persistent enhancing pattern on kinetic curve. Fifteen tumors exhibited homogenous strongly high signal intensity on STIR T2-weighted images. In six cases with EIC, five cases had non-mass-like enhancement around the main mass. CONCLUSIONS: MR findings such as lobular shape, rim or heterogeneous enhancement, persistent pattern on kinetic curve, and homogeneous strongly high signal intensity on STIR T2-weighted images may be useful in diagnosing pure mucinous carcinoma. Moreover, linear-ductal enhancement around main mass may indicate presence of EIC.  相似文献

The purpose of this study was to determine the magnetic resonance imaging (MRI) features of pyogenic hepatic abscesses on T1-weighted, T2-weighted, and serial gadolinium (Gd)-enhanced T1-weighted spoiled gradient-echo (SGE) images including images acquired in the immediate, intermediate, and late phases of enhancement. The MRI studies of 20 patients with pyogenic liver abscesses were retrospectively reviewed. All patients were examined on 1.5 (n = 19) and 1.0 (n = 1) T MR scanners. MR studies included T1-weighted, T2-weighted, and serial Gd-enhanced SGE images. The following determinations were made: signal intensity of the abscess cavity and perilesional liver tissue, and the presence of internal septations, layering material, or air in the abscess cavity. The pattern of enhancement of the abscess wall, internal septae and peri-abscess liver were evaluated on serial Gd-enhanced SGE images. A total of 53 abscesses were observed in the 20 patients. Fortyeight abscesses were hypointense on T1-weighted and hyperintense on T2-weighted images. Internal septations were present in four abscesses. Lower signal intensity material was observed in a dependent location on T2-weighted images in one abscess. Signal void foci of air located on the nondependent surface was observed in two abscesses. Two other abscesses contained signal void air that occupied the entire abscess cavity, observed on all imaging sequences. On serial gadolinium-enhanced images, all abscesses revealed early enhancement of the wall, which persisted with negligible change in degree of enhancement or thickness on delayed images. Abscess walls ranged in thickness from 2 to 5 mm. Internal septations ranged in thickness from 2 to 3 mm. Abscess walls and septations were relatively uniform in thickness with no evidence of focal nodularity. Periabscess liver tissue was mildly hypointense on T1-weighted and mildly hyperintense on T2-weighted images in 20 lesions, which were either circumferential (n = 12) or wedge-shaped (n = 8). All these regions enhanced more than the remainder of the liver on immediate post-gadolinium images and remained relatively hyperintense on late phase images. Periabscess liver parenchyma was isointense on both T1- and T2-weighted images in 18 lesions, and in these lesions wedge-shaped subsegmental (n = 6) or segmental (n = 12) enhancement was observed on immediate gadolinium-enhanced images, which faded to isointensity on intermediate phase images. No perilesional signal changes and enhancement difference was observed in 15 lesions. Characteristic features of abscesses include: intense mural enhancement on early gadolinium-enhanced images, which persists with negligible change in thickness and intensity on later post-gadolinium images, and the presence of periabscess increased enhancement on immediate post-gadolinium images. These MRI features may help to distinguish abscesses from other focal liver lesions during differential diagnosis.  相似文献

目的探讨血管瘤和血管畸形MRI的鉴别诊断。方法回顾性分析61例手术证实的血管瘤和血管畸形MRI表现并与病理进行对照。结果29例血管瘤中,在T1WI上,20例为等信号(等于肌肉信号),9例为高等混杂信号;在T2WI上,24例呈较高信号(接近脂肪信号),5例呈高信号(高于脂肪信号),另见25例条状低信号分隔,其中4例增强呈分块状强化。32例血管畸形中,T1WI图,16例为等信号,16例为高等混杂信号;T2WI图,3例呈较高信号,29例呈高信号。其中15例增强呈不均匀强化。结论MRI对血管瘤和血管畸形的鉴别诊断有重要价值。  相似文献

Eight patients with intracranial tuberculomas were studied with CT and magnetic resonance (MR) imaging. Large, ring enhancing, solid lesions on CT showed low intensity on T2-weighted images and intermediate intensity on T1-weighted images. Small lesions, with ring enhancement on CT, showed central bright signal on T2-weighted images with a peripheral low intensity rim surrounded by high intensity edema. The MR imaging features of the tuberculomas were found to be distinct from those of abscesses, metastases, and gliomas.  相似文献

PURPOSE: Evaluating the MR findings of renal cortical necrosis was the purpose of this study. METHOD: Eight series of T1-/T2-weighted (n = 8) and contrast-enhanced T1-weighted (n = 4) MR images in six patients with renal cortical necrosis diagnosed by renal biopsy (n = 4) or on clinical grounds (n = 2) were reviewed. In those who had follow-up MRI (n = 2) or comparable CT (n = 3), interval changes of MR findings and comparison with CT images were done. RESULTS: Swollen kidney with dark signal intensity rim involving the inner cortex and column of Bertin was noted on T2-and T1-weighted images. It was more conspicuous on T2-weighted images. The lesion did not enhance and was differentiated from uninvolved renal parenchyma. In the follow-up MRI, thickened dark signal intensity was more prominent and proved to be calcification or fibrosis. CONCLUSION: MRI, especially T2-weighted and contrast-enhanced T1-weighted imaging, was helpful in evaluating renal cortical necrosis.  相似文献

 The MR findings in transient osteoporosis of the knee have been described as showing a diffuse area of decreased signal intensity (relative to normal bone marrow) on T1-weighted images and increased signal intensity on T2-weighted images. We report a case of transient osteoporosis, in which MRI showed a crescentic area of abnormal signal intensity in the posterior portion of the lateral femoral condyle, which was bordered by a rim of low signal intensity, best seen on the T2-weighted images. This abnormality was shown to resolve on follow-up MR scans.  相似文献
