Primary tracheal rhabdomyosarcoma. Case report

A case of primary tracheal rhabdomyosarcoma is presented--the second in the English-language literature. The treatment policy is discussed on the basis of this case and the literature is reviewed.     

Near fatal systemic toxicity from local injection of podophyllin for pedal verrucae treatment.

An extremely rare case of systemic podophyllin toxicity from local injection is presented with a review of the literature. There is only one other known similar case reported in the medical literature world wide. Clinical and pathologic findings will be discussed.     

Mediastinal seminoma. A case report and review of the literature

A case of mediastinal seminoma presenting with hemoptysis and superior vena cava obstruction is reported. In the literature, only 1 case of mediastinal seminoma presenting with hemoptysis is reported. The relevant literature is reviewed.     

Aorto-oesophageal fistula complicating tuberculous aortitis. A case report.

A case of massive upper gastro-intestinal haemorrhage in a Black woman owing to tuberculous aorto-oesophageal fistula is reported. The literature is reviewed and the clinical presentation discussed. This possibly represents only the second case of aorto-oesophageal fistula complicating postprimary tuberculosis to appear in medical literature.     

Lymphedema of the face

Lymphedema of the extremities is well documented in the literature. We present a case of lymphedema involving the face of a 27-year-old man and discuss the clinical presentation, microscopic findings, and surgical treatment. To our knowledge, this is the first such case report in the literature.     

Clinical aspects, radiology and therapy of angiosarcoma of the breast gland

A case of angiosarcoma of the breast is described. The problems of clinical appearance, roentgenology and therapy are discussed. The case described is similar to those seen in the literature, of which 44 cases are known in the literature. Problems of diagnosis even during surgery are evident.     

Iatrogenic Reno-Jejunal Fistula

The development of a reno-jejunal fistula following a Roux-en-Y cystojejunostomy for an incorrectly diagnosed pancreatic pseudocyst is described. Up to now, this is the second case of iatrogenic reno-jejunal fistula reported in literature.Reno-jejunal fistulas are exceedingly rare and are usually consequences of urologic pathologies of infectious origin. Reno-jejunal fistulas of iatrogenic origin are even rarer, only one case being referred in literature [1]. Herein the second case is reported.     

Transverse colon volvulus in a 15 year old boy and the review of the literature

We report a rare case of transverse colon volvulus in a fifteen year old boy with a review of the literature. This brings the total number of pediatric cases reported in the English literature to fifteen. This case is unusual in that no aetiological factor has been found, in contrast to the majority of the pediatric cases. Diagnosis can be challenging and the effective management remains controversial. The various radiological imaging modalities are presented. The epidemiology, aetiology, diagnosis and management of transverse colon volvulus are discussed. It is important to highlight this case and those in the literature, as many surgeons may never have seen a single case of transverse colon volvulus. It may therefore not be considered in the differential diagnosis of recurrent intermittent abdominal pain or acute intestinal obstruction.     

Aspergillus discitis. Report of two cases and review of the literature

Aspergillus discitis is a rare disease that usually occurs in immunocompromised hosts. In the one case reported in the orthopedic literature surgical debridement was required. Two additional cases, which occurred in intravenous drug abusers, are reported here. One case is unique in the orthopedic literature in that eradication of the disease occurred with chemotherapy alone.     

Pyeloduodenal fistula: a case report and review of the literature.

A case of the rare condition of pyeloduodenal fistula is presented--the thirty-second case so far recorded in the literature. The authors consider that this instance shows some of the features typical of the cases so far reported. The literature is reviewed and the aetiology, presentation, diagnostic methods and treatment are briefly discussed.     

Family cases of gastroschisis

The etiology of gastroschisis is still unclear. To the authors' knowledge, there are only 14 cases of familial gastroschisis in the literature. The authors add the second case of mother-and-son occurrence and a case of siblings occurrence, thereby updating the current literature for family gastroschisis.     

A fatal case of MRSA septicaemia and gallbladder empyema

Methicillin-resistant Staphylococcus aureus (MRSA) has become a major problem worldwide and is associated with significant morbidity and mortality. It is a particular issue in surgical patients especially vascular surgery involving prosthetic implants. We present an unusual case of MRSA gallbladder empyema and septicaemia in a previous vascular surgical patient. Teranishi et al reported a case of acute cholecystitis caused by MRSA in the Japanese literature but as far as we are aware this is the first case reported in the English literature.     

Lymphangioma of the stomach--report of a case and review of the literature

A rare case of a 66 year old man with lymphangioma of the stomach is presented herein. Upper gastrointestinal X-ray series revealed a slow growing submucosal tumor in the anterior wall of the antrum of the stomach which was subsequently found to be lymphangioma after a partial gastrectomy. To our knowledge, this is only the thirteenth case of lymphangioma of the stomach to have been reported in the English literature. A brief review of the English and Japanese literature is presented following the case report.     

Bilateral rupture of extensor pollicis longus. A case report

Bilateral rupture of the extensor pollicis longus tendon is a rare entity. Most case studies in the literature have been reported in patients with an underlying systemic condition such as rheumatoid arthritis or following an episode of trauma. An interesting and unusual case is presented and theories concerning etiology, various operative techniques, and recent cases in the literature are discussed.

Visual loss associated with fibrous dysplasia of the anterior skull base. Case report and review of the literature

The authors present a case of visual loss associated with fibrous dysplasia of the anterior skull base and the surgical management of this case. Preoperative computerized tomography scanning in this patient demonstrated a patent optic foramen and a rapidly growing cystic mass within the orbit, which was responsible for the patient's visual loss. A literature review revealed that this case is typical, in that cystic mass lesions of various types are frequently responsible for visual loss associated with fibrous dysplasia. The authors did not find significant evidence in the literature to support the notion that visual loss associated with fibrous dysplasia is the result of progressive optic canal stenosis, thus raising questions about the value of prophylactic optic canal decompression. Instead, as demonstrated by this case and those uncovered in the literature review, most instances of visual loss result from the rapid growth of mass lesions of cystic fibrous dysplasia, mucoceles, or hemorrhage. Findings of the literature review and the present case of fibrous dysplasia of the anterior skull base support a role for extensive surgical resection in these cases and indicate a need for additional prospective analysis of a larger number of patients with this disease.     

气肿性肾盂肾炎致突发呼吸心跳骤停1例及文献综述

目的 报告气肿性肾盂肾炎致突发呼吸心跳骤停1例及进行文献回顾总结。方法 报告1例气肿性肾盂肾炎导致突发呼吸心跳骤停患者的临床资料并总结既往文献报道。结果 本例患者由于延误就诊，病情危重，拟急诊行患肾切除术，在进入手术室后发生呼吸心跳骤停，抢救无效而死亡。结论 气肿性肾盂肾炎广泛气肿破坏肾实质需行肾切除术，延误治疗可导致突发呼吸心跳骤停，需密切监护及预防。     

Fracture-dislocations of all five carpometacarpal joints: a case report

Injuries to all five carpometacarpal joints are rare. Only four cases have been reported in the English literature. A case of fracture-dislocations of all five carpometacarpal joints is presented. An excellent result has been obtained 2 years after open reduction and internal fixation. This case is the only one in the English literature so treated and with prolonged follow-up.     

A case of testicular rhabdomyosarcoma

Pure testicular rhabdomyosarcoma is a very rare tumor and 7 cases have been reported in literature. A 20-year-old male patient presented with a painless right testicular swelling who underwent inguinal orchiectomy with suspicion of testicular malignancy. The case was regarded as an embryonal rhabdomyosarcoma according to histopathologic and immunohistochemical findings. The case is presented and the relevant literature is reviewed and discussed.     

Hemorrhagic necrosis of pheochromocytoma associated with phentolamine administration.

A case of ruptured pheochromocytoma is presented, the pathophysiology discussed, and the literature reviewed. Evidence is presented that the use of alpha-adrenergic blockade in general, and phentolamine in particular, may predispose to this complication. Twelve cases of massive hemorrhagic necrosis with or without rupture were found in the literature, including the present case. Six had no operation; one survived. Six had immediate operation; 4 survived. An additional case of hemorrhage into a small pheochromocytoma following phentolamine is presented. This tumor was neither ruptured nor massively necrotic, but the case supports the hypothesis that alpha-adrenergic blockade may cause hemorrhage within the pheochromocytoma.
