SUMMARY

Preliminary analyses of data from one third of the Keokuk County Rural Health Study cohort suggest that risk factors for injury among rural populations are not uniformly distributed and that the pattern varies with the risk factor. For some, such as expo-sure to all-terrain vehicles, the occupation of farmer determines the degree of exposure to the risk factor. Similarly, farmers seem less likely than other rural people to wear their seat belts. Although farm-ers are not more likely than rural nonfarmers to have firearms in their homes, they are more likely to have used them in the last year. How-ever, for other injury risk factors such as alcohol consumption, there do not appear to be differences among farmers, rural nonfarmers, and townspeople. These early results suggest that the Keokuk County Rural Health Study will yield important information for tar-geting specific rural injury prevention interventions.  相似文献

ABSTRACT

Background. Studies report low prevalence of several health problems among farmers. Health status and psychosocial factors are generally assumed to impact on retirement age and the ability to stay in work. Objectives. To study retirement in a male rural population and to test the hypothesis that farmers retire later than nonfarmers thanks to better health status and advantageous psychosocial factors. Method. The study design was a longitudinal cohort study on farmers and rural nonfarmers with data assessment at two surveys 12 years apart. Analyses were performed with multiple logistic regression models. Results. Among subjects 64 years or younger at survey 2, 93% of the farmers were still working (full or part time) as compared with 76% of the employed nonfarmers. Among those 65 years or older, 64% of the farmers, 33% of nonfarming entrepreneurs, and 6% of employed nonfarmers were still occupationally active. Significant differences in reported diseases and psychosocial factors were found between farmers and nonfarmers, but in the fully adjusted model these variables did not modify the low odds ratios of retirement found for farmers. The farmers adjusted risk (OR) for premature retirement (64 or younger) was 0.30 (95% CI 0.16–0.55) and for standard retirement (65 or older) 0.01 (95% CI 0.00–0.05). Conclusions. Farmers continue to work full or part time around retirement age to a much larger extent than employees. The employment status of farmers may only partly explain this. Health status and psychosocial factors did not impact on the results.  相似文献

BACKGROUND: In 2005, 40% of motor-vehicle occupant deaths in the group aged 16-19 years involved passengers. Although seatbelts can reduce crash mortality by 50% or more, little is known about the differences in driver-versus-passenger seatbelt use among teens. METHODS: In 2007, data from the 2001 and 2003 Youth Risk Behavior Surveys were analyzed for 12,731 black, white, and Hispanic high school students aged >/=16 years reporting seatbelt use as both drivers and passengers. Seatbelt use was compared for driver- and passenger-seat positions, and stratified by age, gender, race/ethnicity, school grades, and histories of either drinking and driving or riding with a drinking driver. RESULTS: Overall, 59% of students always used seatbelts when driving, but only 42% always buckled up as passengers. Across all covariate strata, passenger seatbelt use was significantly less prevalent than driver seatbelt use (p<0.001). A concordance analysis showed that only 38% of students always wore seatbelts both when driving and while riding as a passenger. Multivariate analyses indicated that, regardless of seat position, seatbelt use was lower for young men, blacks, students with poor grades, and students who reported either drinking and driving or riding with a drinking driver. CONCLUSIONS: U.S. high school students aged >/=16 years are significantly less likely to wear seatbelts as passengers than as drivers. Interventions designed to promote seatbelt use among teens need to address this disparity.  相似文献

Less than one-fifth of the U.S. population consistently wears automobile seatbelts. Automatic seatbelts or air bags will be required in all new cars, unless States whose total population equals two-thirds of the nation's adopt mandatory seatbelt use laws by 1989, according to the U.S. Department of Transportation. In July 1984, New York State passed the first mandatory seatbelt law in the nation, followed by New Jersey, Illinois, Missouri, Michigan, and New Mexico. A telephone survey of 2,982 randomly selected drivers examined belt use and support for mandatory belt use laws in the six New England States. Drivers with little education and low income, younger drivers, and drivers who drove after heavy drinking or marijuana use, or both, were least likely to wear seatbelts. Beliefs that seatbelts are not effective in reducing injury risk and are uncomfortable were more common among those not wearing belts. Sixty percent of drivers favored a mandatory seatbelt use law. The most important predictors of opposition to a belt use law were beliefs that seatbelts are ineffective, inconvenient, and uncomfortable. Opposition was also more likely among persons who reported that they drove after marijuana use, or heavy drinking, or both, drove more miles per year, exhibited a low frequency of seatbelt use, and perceived a low probability of personal crash involvement.  相似文献

SUMMARY

The Keokuk County Rural Health Study is a population-based, prospective study of environmental exposures and health status of a large randomly-selected sample of residents from each of three strata in one rural Iowa county. The study focuses on injury rates and respiratory disease; in addition, it monitors health care de-livery; geriatric, reproductive, and mental health; and other health outcomes. Injury and disease prevalence are investigated in relation to occupational, agricultural, and other environmental exposures. This paper describes the sampling method, the protocol, and the demographic profile of adult subjects of the first 400 households en-rolled in the study. Farmers were slightly younger than rural non-farmers and townspeople and more farmers had lived in the county all of their lives. Females were more highly educated than were males, and less than half as many female farmers had lived in the county all their lives than had male farmers. Net household income figures were complex and are shown in a graph in Table 1.  相似文献

OBJECTIVES: The objective of the study was to determine whether swine farmers continuously exposed to the farming environment react differently to acute exposure than previously unexposed nonfarmers. METHODS: Nine healthy nonfarmers, not previously exposed to a farming environment, and eight swine farmers were exposed in a swine confinement building for 3 hours while weighing pigs. Lung function measurements, methacholine challenge tests, and nasal lavages were performed before and after the exposure. Blood samples were drawn repeatedly during the exposure day. Differential cell counts and cytokine levels were analyzed in the nasal lavage fluid and blood. RESULTS: The exposure levels were the same in both groups. Bronchial responsiveness to methacholine increased by a median of 4.0 (25th-75th percentiles 2.2-10.1 among the nonfarmers) and 0.7 (25th-75th percentiles 0.01-3.5 among the farmers) doubled concentration steps. The median serum levels of interleukin-6 increased from 3.8 (25th-75th percentiles <3-5.8) ng/l to 23.7 (25th-75th percentiles 11.6-41.6) ng/l among the nonfarmers and from <3 to 3.8 (25th-75th percentiles 3.1-11.6) ng/l among the swine farmers after the exposure. Swine dust exposure induced a ninefold increase in the total cell counts in the nasal lavage fluid of the nonfarmers, but no significant increase among the swine farmers. CONCLUSIONS: The exposure altered lung function and bronchial responsiveness, as well as cell number and cytokines in blood and nasal lavage fluid in previously unexposed nonfarming subjects, whereas only minor alterations were found in the farmers. This finding suggests possible adaptation mechanisms in chronically exposed swine farmers.  相似文献

Abstract: Young (18–22 years) and middle–aged (45–49 years) women living in urban and rural areas of New South Wales completed a brief food frequency questionnaire as part of a wider health survey. Urban women in both age groups consumed meat less frequently than women in rural areas, and women in the less populated rural areas were more likely to eat green and yellow vegetables and least likely to eat dried beans. There were few other geographic differences in food habits. Middle–aged women consumed reduced–fat milk, fruit, vegetables, fish, biscuits and cakes significandy more frequently, and rice, pasta, full–cream milk, fried food and take–away food less frequently than younger women. Smokers in both age groups consumed fresh fruit, vegetables and breakfast cereals significantly less frequendy than non–smokers, and women with low levels of habitual physical activity consumed fresh fruit and cereals less frequendy than more active women. The findings suggest that strategies aimed at changing eating behaviours should be age–group–specific and targeted specifically to smokers and less active women.  相似文献

PurposeFarm youth continue to experience high rates of injury and deaths as a result of agricultural activities. Farm machinery, especially tractors, is the most common cause of casualties to youth. A Roll-Over Protection Structure (ROPS) along with a fastened seatbelt can prevent almost all injuries and fatalities from tractor overturns. Despite this knowledge, the use of seatbelts by farmers on ROPS tractors remains low. This study treats farm safety as a family issue and builds on the central role of parents as teachers and role models of farm safety for youth.MethodsThis research study used a longitudinal, repeated-measures, randomized-control design in which youth 10–19 years of age were randomly assigned to either of two intervention groups (parent-led group and staff-led group) or the control group.ResultsFathers in the parent-led group were less likely to operate ROPS tractors without a seatbelt compared with other groups. They were more likely to have communicated with youth about the importance of wearing seatbelts on ROPS tractors. Consequently, youth in the parent-led group were less likely to operate a ROPS tractor without a seatbelt than the control group at post-test.ConclusionsThis randomized control trial supports the effectiveness of a home-based, father-led farm safety intervention as a promising strategy for reducing youth as well as father-unsafe behaviors (related to tractor seatbelts) on the farm. This intervention appealed to fathers' strong motivation to practice tractor safety for the sake of their youth. Involving fathers helped change both father as well as youth unsafe tractor-seatbelt behaviors.  相似文献

BACKGROUND: Farmers are at increased risk of contracting tetanus. However, no difference in immunity to tetanus has been reported between rural and urban dwellers in large epidemiological studies. We hypothesized that tetanus antibody concentrations would be lower in farmers than in nonfarmers within the rural population. METHODS: We recruited 102 adult subjects attending an agribusiness trade show who identified themselves as farmers in Wisconsin. The nonfarmer group (n = 120) was composed of adults attending the agribusiness show who were not engaged in farming or were participating in another research study. Concentrations of antibody to tetanus toxin (antiTT) in sera were measured by an enzyme-linked immunosorbent assay (IBL, Hamburg). AntiTT levels of >0.15 IU/mL were considered protective. RESULTS: The antiTT concentrations for the farmer population (median = 2.74 IU/mL) were much higher than those for the nonfarmer group (median = 1.82 IU/mL) (P<.008). As in other studies, being male, being younger, and having a history of military service were positively correlated with protective antiTT concentrations. However, only farming, age, and the farming-sex interaction term were significantly associated with antiTT concentrations in the multiple-regression model. CONCLUSIONS: The farmers we studied had high antiTT concentrations and a high tetanus seroprevalence rate. Occupation may be an important consideration in the development of immunization policies. A broader seroepidemiological study of farmers must be undertaken before any such considerations can be made.  相似文献

Objectives: To determine whether overweight and obese individuals have higher reported fall and fall injury risk than individuals of healthy weight, and to examine the influence of BMI on health, quality of life and lifestyle characteristics of fallers. Methods: A representative sample of community‐based individuals aged 65 years and older in New South Wales was surveyed regarding their history of falls, height, weight, lifestyle and general health within a 12‐month period. Results: Obese individuals had a 31% higher risk of having fallen, but no higher risk of a fall‐related injury compared to healthy‐weight individuals. Obese fallers also had a 57% higher risk of believing nothing could be done to prevent falls; a 41% higher risk of using four or more medications; a 30% higher risk of experiencing moderate or extreme pain or discomfort; were 26% less likely have walked for two or more hours in the last week; and were less likely to think they were doing enough physical activity. Conclusions: Older obese individuals have an increased risk of falls and obese fallers have a higher prevalence of pain and inactivity than fallers of a healthy weight. Implications: A decrease in sedentary lifestyle and regular weight‐bearing exercise may reduce fall risk in older obese individuals.  相似文献

  目的  了解西藏地区城乡居民心血管病高危人群现况及其影响因素,为制定西藏心血管疾病的预防控制措施提供依据。  方法  2016年5月-2019年10月采用随机整群抽样方法在西藏7个项目市(区)抽取41 476名35~75岁城乡居民进行问卷调查、体格测量和实验室检测。  结果  在符合条件的41 476名城乡居民中筛出高危者9 185人,高危率为22.15%,其中有1个危险因素者7 882人(85.81%),2个危险因素者1 457人(15.86%),≥3个危险因素146人(1.59%)。心血管病史型检出率为0.89%,血压高型检出率为18.62%,血脂异常型检出率为3.74%,WHO评估10年患病风险≥20%的检出率为2.17%。≥40岁、职业为农民和居住在农村的居民更易成为心血管病高危人群(均有P<0.05),女性和婚姻状况为在婚的西藏城乡居民更不易成为心血管病高危人群。  结论  西藏35~75岁心血管病高危人群中男性高危率高于女性,高危率随着年龄的增长而逐渐升高,西藏城乡居民心血管病高危因素以血压高为主,男性、非在婚、年龄较大者、农民等是该地区心血管病综合防治的关键人群。  相似文献

ABSTRACT: Farm injury is a major problem for agriculture in Australia. Agriculture is subject to rates of work-related deaths that are among the highest in Australia, while on-farm injuries result in between 20 and 60 presentations to rural hospital emergency departments per 100 farms per annum. Prevention of farm injury has been constrained by: (i) lack of relevant information regarding injury causation; (ii) lack of appropriate education and training in occupational health and safety; (iii) limited applicability of previous occupational health and safety legislation; and (iv)lack of the necessary management tools for farmers to manage risk Farmsafe Australia has developed a risk management approach to assist the farming community to reduce risk of injury and death and, to reduce the costs associated with the current high injury and death rate. The Farmsafe network is maintained to varying degrees at local, State and national levels. Farmsafe organisations are comprised of members from relevant agencies and other individuals with an interest and commitment to improved safety in agriculture; namely, farmers, women, government departments and unions. Moves have been made to develop joint farm injury prevention programs between health service providers and those whose role is primarily in prevention. The challenge remains, however, to develop more effective and sustainable partnerships between these groups to minimise the impact of farm injury for those who are injured and for rural communities.  相似文献

PURPOSE: This study aimed to explore perceptions of TB, and health care seeking pathways, among poor rural communities in Inner Mongolia. METHODOLOGY: Twenty focus group discussions (FGDs) were held and 105 farmers were included. Six hundred and fourteen randomly selected respondents were surveyed through interview questionnaire, in three poor rural counties with a high TB prevalence. MAIN FINDINGS: A substantial proportion of community members were unclear or misinformed as to how TB was transmitted. Sixty percent of respondents identified prolonged cough as a main symptom of TB, while only 40% perceived TB to be caused by 'close interaction with TB patient'. In addition, 70% could not afford TB treatment and fell into debt as a result of having to seek medical care. Social stigma associated with TB influenced marriage prospects and impeded important social interactions within the community. Respondents' perceptions of TB were associated with their socio-economic status. Women, young people, low-income groups and those with less education tended to be less knowledgeable about TB. All farmers in the study reported only seeking health care after they failed to treat themselves; and most of them then sought care from less qualified village level health care providers. Less educated people, low-income groups and old people were identified as less likely to seek care, or more likely to seek care at village level where it is cheaper. Both financial and structural barriers were found to stop farmers seeking health care. CONCLUSIONS AND POLICY IMPLICATIONS: Perceptions of TB and social stigma associated with the disease, together with socio-economic factors, shape the health seeking behaviour of poor farmers. Accessibility and affordability of TB health care issues should be dealt with through a multi-pronged approach, including health promotion in addition to expansion of the DOTS strategy and rural health insurance schemes.  相似文献

PURPOSE: We sought to identify child and parent risk factors for nonfatal unintentional injury among rural children.METHODS: Design: cross-sectional health interview survey. Setting: a completely rural midwestern county not adjacent to a metropolitan area. Participants: stratified, random sample of farm, town, and nonfarm, nontown households, including parents of all children aged 0-17 years. Outcome measures: parent-reported injury episodes that occurred in the past 12 months in children that required professional care, restricted normal activities for at least four hours, or resulted in blacking out or losing awareness or memory. Potential risk factors included nine child factors and six parent factors.RESULTS: Of the 621 children in participating households, 137 or 22.1% were injured. Children on a sports team were 1.88 times (95% confidence interval: 1.07 to 3.31) more likely than other children to be injured. Track team members were more likely than non-members to have an injury (54.5% vs. 23.0%, chi square test, p = 0.001), and volleyball team members were more likely than non-members to have an injury (50.0% vs. 29.0%, chi square test, p = 0.009). Unrelated to injury status were the child's gender, rural stratum, emotional health, behavioral problems, impairment or medications. Also unrelated to child injury were the parent's marital status, alcohol dependence, binge drinking, antisocial personality, depression symptoms or fear of child abuse by spouse.CONCLUSIONS: Our findings illustrate the difficulty of discovering clues with potential to aid design of effective childhood unintentional injury prevention policies, programs, and practices. The instruments and processes in our study were of acceptable quality, therefore we failed to asses the true risk factors for childhood injury or our sample size was too small to detect associations with the factors that were included.  相似文献

CONTEXT: Although farmers are at risk for injuries from contact with large livestock, few previous studies have examined risk factors for animal-related injuries. PURPOSE: This case-control study, nested in the Agricultural Health Study, aimed to assess risk factors for animal-related injury among Iowa large-livestock farmers. METHODS: A screener questionnaire sent to 6999 farmers identified 116 farmers with large livestock who had an animal-related injury requiring medical advice/treatment in the previous year. Several possible risk factors for injury were assessed by comparing these farmers with 342 farmers who had livestock but were not injured in the previous year. FINDINGS: A multiple logistic regression analysis showed significant associations between animal-related injury and the use of a hearing aid (odds ratio [OR] = 5.4 [95% CI, 1.6 to 18.0]), doctor-diagnosed arthritis or rheumatism (OR = 3.0 [95% CI, 1.7 to 5.2]), education beyond high school (OR = 1.8 [95% CI, 1.1 to 2.8]), and a younger age. Farmers engaged in off-farm work were less likely to sustain animal-related injuries (OR = 0.4 [95% CI, 0.2 to 0.8]). CONCLUSIONS: This is the first study to show associations between animal-related injury and a younger age, hearing difficulties, and doctor-diagnosed arthritis. Hearing loss and arthritis, which are more common among farmers than among other workers, may be particularly important risk factors to address in future preventive studies.  相似文献

The Keokuk County Rural Health Study (KCRHS) was designed as a 20-year, prospective cohort study focusing on chronic disease and injury in an agricultural southeastern Iowa county. The goals of the KCRHS are to prospectively describe, measure, and analyze prevalent rural and agriculturally related adverse health outcomes and their respective risk factors and to provide the basis for future community-based intervention programs to reduce disease and injury incidence. Methods of data collection included in-person interviews, medical screenings, and environmental assessments of homes and farms. All households studied were rural; comparisons were made among farm, rural nonfarm, and town households, between men and women, and between smokers and nonsmokers. The present paper reports selected adult baseline data from Round 1 of this study. Residents of farm households were somewhat younger and better educated than residents of rural nonfarm and town households; smoked less; were more likely to have ridden an all-terrain vehicle; and were more likely to report firearms in the home. Eighty-nine percent of the men and 66% of the women engaged in farming or did so in the past. Men more often reported hearing loss, were more often overweight and obese, more often reported an injury, less often reported asthma, and less often saw a medical practitioner. Women reported poorer emotional health and higher rates of depression symptoms. The KCRHS has identified several modifiable health outcomes and risk factors as candidates for further analysis and targets for community-based prevention and intervention programs.  相似文献

目的估计山西省某农村既往有偿献血地区18~59岁村民丙型肝炎病毒(HCV)感染的流行率并分析HCV感染的可能危险因素。方法选择山西省某县12个村庄随机抽取660名18~59岁村民开展以社区为基础的横断面研究。采用标准化问卷收集研究对象的人口学、医疗史、危险行为等信息,采集7ml静脉血用于检测HCV感染。结果该既往有偿献血地区18~59岁村民总的HCV感染率为8.2%,献血人群HCV感染率为27.7%。多因素logistic回归分析提示,曾经献过血(调整OR=14.28,95%CI:6.83~29.87)和输过血(调整OR=8.66,95%CI:2.31~32.47)是该地村民感染HCV的主要危险因素。对献血亚人群分析发现,曾经献过血浆(调整OR=8.56,95%CI:2.87~25.54)是献血者感染HCV的最主要危险因素;1994年及以后停止献血的村民感染HCV的危险性较1994年之前停止献血的低(调整OR=0.32,95%CI:0.11~0.93);女性献血者感染HCV的危险性较男性为低(调整OR=0.28,95%CI:0.10~0.83)。结论该献血地区HCV流行水平较高,既往有偿献血(浆)是该地区HCV流行的最主要原因。应采取有效措施预防该地区居民HCV的二代传播及为阳性患者提供治疗刻不容缓。  相似文献

OBJECTIVE: To compare the risk of death in a cohort of Western Australian released prisoners with the risk experienced by the general population of Western Australia. METHODS: A cohort study of prisoners in Western Australia whose last date of release ranged from 1 January 1994 to 1 January 1999. Overall mortality and cause of death were determined by data linkage to the Registrar General's record of deaths. RESULTS: Aboriginal prisoners had a significantly lower survival rate after release than non-Aboriginal prisoners (p < 0.0001). When compared with their peers in the Western Australian community, both Aboriginal and non-Aboriginal prisoners were found to have an increased relative risk of death. Female non-Aboriginal released prisoners aged between 20 and 40 years were 17.8 (95% CI 8.1-27.5) times more likely to die than other female non-Aboriginals in Western Australia in the same age range. Male non-Aboriginal prisoners aged 20-40 years were 6.3 (95% CI 5.2-7.4) times more likely to die than their counterparts in the WA community. Female Aboriginal released prisoners were 3.4 (95% CI 1.2-5.6) times more likely to die than their peers, while male Aboriginal released prisoners were 2.9 (95% CI 2.2-3.5) times more likely to die. In their first six months after release, female non-Aboriginal prisoners aged 20 to 40 years were 69.1 (95% CI 17.9-120.3) times more likely to die than their counterparts in the WA community. The main causes of excess death were related to drug and alcohol abuse. CONCLUSION: All prisoners were at greater than expected relative risk of death after release from prison, with female non-Aboriginal prisoners at particularly high relative risk.  相似文献

Farmers are exposed to noise that is potentially hazardous to hearing. We measured the hearing of 161 male farmers and 75 male nonfarmers at the 1979 Missouri Farmers Association Agri-Fair and compared it with the hearing of 129 office workers from central Missouri. Fixed-level screening tests were conducted in both ears at three stimulus frequencies: 1000 and 2000 hertz at 20 decibels hearing level and 4000 hertz at 25 decibels hearing level. Audiometers were calibrated in accordance with the ANSI-1969 standard. The results show that farmers are at risk for hearing loss at 2000 and 4000 hertz when compared with office workers. The prevalence of hearing loss was greater for farmers at both frequencies in every decade age group from 25 to 64 years. Using screening failure at 2000 and 4000 hertz in both ears as a criterion for a loss that would affect communication ability, we found that the failure rate was 16.8 percent for farmers and 6.2 percent for office workers. As other investigators have found, the prevalence of high-frequency hearing loss in male nonfarmers who associate with farmers was nearly as great as for farmers.  相似文献

ABSTRACT:  Context: Rates of preterm birth (PTB) and low birth weight (LBW) vary by region, with disparities particularly evident in the Appalachian region of the South. Community conditions related to rurality likely contribute to adverse birth outcomes in this region. Purpose: This study examined associations between rurality and related community conditions, and newborn outcomes in southern Appalachia, and explored whether pregnancy smoking explained such associations. Methods: Data for all births in a southern Appalachian county over a 2-year period were extracted from hospital records. Findings: Data were available for 4,144 births, with 45 different counties of residence. Babies born to women from completely rural counties, on average, weighed 700 g less, were 1.5 inches shorter, and were born over 3 weeks earlier than less rural infants. In addition, these babies were 4.5 times more likely to be LBW, 4 times more likely to be PTB, and 5 times more likely to be admitted to the neonatal intensive care unit (NICU). Effects were also found for per capita income, poverty rate, and unemployment rate, all of which were associated with rurality. Some, but not all of the association was explained by elevated rates of pregnancy smoking. Conclusions: Babies born to women residing in rural and economically depressed counties in southern Appalachia are at substantially increased risk for poor birth outcomes. Improving these outcomes in the rural South will likely require addressing access to health services and information, health care provider retention, transportation services, employment opportunities, and availability of public health services including smoking cessation assistance.  相似文献
