We report very rare case of benign mestasizing leiomyoma (BML). A 42-year-old female was referred to our hospital presenting with abnormal shadows revealed by a chest X-ray film. Chest computed tomography (CT) scan revealed multiple well-defined nodules in bilateral lung field, suggesting metastatic lung tumor. Further examination could not detect any suspicious primary lesion. Several tumor markers were all within normal limits. Surgical resection was performed to establish diagnosis and treatment. Pathological diagnosis was BML from the uterus myoma which had been resected 5 years before.  相似文献

A 35-year-old female was admitted for biopsy of abnormal shadow on chest X-ray. She was operated on for partial wedge resection of the right lower lobe by video-assisted thoracoscopy without complication. Pathological findings in operation showed inflammatory benign tissues and suggested pulmonary dirofilariasis. Serologic examination was negative after operation, however histological diagnosis supported pulmonary dirofilariasis because a pulmonary artery embolism of calcified tissues consisted of non-human cells. There may be many cases due to old infections like this one, so it is important to consider it for diagnosis and perform more positive surgical procedures. We review a case of pulmonary dirofilariasis safely resected by video-assisted thoracoscopy for diagnosis by exclusion.  相似文献

A 32-year-old man was referred to our hospital because of a mediastinal tumor with mild dysphagia. Computed tomography (CT) showed that the tumor was located in the neck and the posterior mediastinum. An esophagogram demonstrated severe distortion of the esophageal lumen and an endoscopic examination disclosed a submucosal bulge covered by normal mucosa with no erosion and no ulceration. A CT-guided needle biopsy for neck lesion of the tumor was performed and the histopathological diagnosis was an esophageal leiomyoma. Complete enucleation via a left anteroaxillary thoracotomy for the tumor was performed without perforation of esophageal mucosa. The resected tumor was 12 cm in size. The final diagnosis of the resected tumor located in the cervical, upper, middle and intramural esophagus was a rare giant esophageal leiomyoma. Postoperative course was uneventful. We suggested that enucleation with an operative procedure selected carefully for a giant esophageal leiomyoma was attempted for preservation of esophageal function.  相似文献

A case, 53-year-old female, of benign metastasizing leiomyoma was reported. The patient had undergone a hysterectomy for uterine myoma at the age of 40. The computed tomography (CT) scan film on health screening revealed 4 nodular lesions on right lung field and 2 nodular lesions on left lung field. Two tumors of the left side were resected under thoracoscopic surgery. Pathologically 2 lesions were consisted of benign spindle cells and diagnosed as benign metastasizing leiomyoma. Postoperative course was uneventful. Right lung nodules have been meticulously followed up.  相似文献

A 63-year-old female presented with an abnormal shadow on a chest X-ray. A serial chest computed tomography (CT) showed ground-glass attenuation, which measured 2 cm on S1+2 of the left lung. When bronchofiberscopy was performed to make a diagnosis, a tumor with a smooth surface was revealed which obstructed the right middle bronchus. Leiomyoma was thus diagnosed. At first, a wide wedge resection of left lung tumor was performed. Secondly, a bronchus tumor was successively removed using a high frequency snare and a laser by a bronchofiberscopy. Her postoperative course was uneventful. Leiomyoma of the bronchus is rare benign tumor. This report describes the performance of a resection using bronchofiberscopy with good results.  相似文献

A 53-year-old man came to our hospital complaining of painful induration in the left scrotum. It was difficult to determine by manual palpation whether the induration was derived from the testis or paratesticular tissues including epididymis. Computerized tomographic scanning and ultrasonography revealed a left paratesticular mass, 3 cm in diameter, with unclear margin of the atrophic testis, which was suspected as originating from the epididymis. Although tumor extirpation was intended, he underwent inguinal orchiectomy due to strong adhesion of the mass to the ipsilateral atrophic testis. The tumor was opalescent, gum elastic, and solid, measuring 5 x 3 x 2 cm in size. Histopathological examination revealed a primary leiomyoma of the epididymis. The details of this rare case of primary epididymal leiomyoma that originated from the epididymal tail are reported herein.  相似文献

A 61-year-old woman was referred to our hospital due to a liver tumor discovered at a medical checkup. A hypervascular tumor located in liver segment I, measuring 45 mm in diameter, was visualized on ultrasonography. The tumor showed strong uniform enhancement from an early phase on computed tomography and angiography. Right hepatectomy with total caudate lobe resection was performed. Pathological examination revealed the tumor to be a primary leiomyoma of the liver, positive for smooth muscle actin and desmin on immunohistochemical examination. Primary liver leiomyoma is very rare and only 18 cases have been reported to date. The clinical features of these cases from the literature are also discussed.  相似文献

A case of leiomyoma of the testis is reported. The origin of the intratesticular leiomyoma is controversial, but recently it is thought to arise from the contractile cells in the tunica propria of the seminiferous tubules. A 32-year-old builder suffered from a chest trauma in a construction area. A ratchet penetrated his right chest wall posterolaterally, and he was transferred to our hospital. A chest X-ray and a computed tomography (CT) revealed that the tip of the instrument remained in the thoracic cavity and there were no other obvious damages including pneumothorax, hemothorax, and liver damage. We thought the simple removal of the instrument from his body was dangerous so he was transferred to the operating room and his chest was opened under general anesthesia. However, it was difficult to confirm the inside of penetrating point from the thoracotomy wound; therefore we used a thoracoscope. The thoracoscopy disclosed a penetration in the lung and a contusion on the diaphragm. The application of thoracoscopy in the operation of thoracic trauma is very promising in exploration of the thoracic cavity, especially in the area which is out of vision from the open thoracotomy.  相似文献

A 46-year-old female with benign metastasizing leiomyoma was reported. The patient had undergone a lobectomy of thyroid for adenomatous goiter at the age of 30, and a myomectomy for uterine myoma and a modified radical masmectomy for breast cancer at the age of 32. The chest X-ray on health screening revealed multiple nodules in the bilateral lung. Computed tomography (CT) revealed 6 nodules in the right side and 3 nodules in the left side of the lung. The tumors of the right lung were resected under thoracoscopic surgery. Pathologically these lesions were diagnosed as benign metastasizing leiomyoma. A 55-year-old female felt a transient chest pain soon after a fish meal. Sixteen days later she presented a local clinic with fever of 38°C. After chest computed tomography( CT) and upper gastrointestinal endoscopy showed fish bone embedded in the wall of the middle portion of the esophagus, she referred to our hospital 22 days after the meal. Thirty days after the meal ,endoscopic removal of the fish bone was challenged bygastroentenologists in vain. Chest CT after the attempt showed migration of the fish bone to the lung adjacent to the right inferior pulmonary vein. The fish bone which stuck out from the lung after division of the pulmonary ligament was removed successfully under thoracoscopic surgery. The patient is well 26 months after the surgery.  相似文献

Primary epididymal leiomyoma is relatively rare. We experienced a case of bilateral simultaneous primary epididymal leiomyoma. A 61-year-old man visited our clinic, with the chief complaint of bilateral painless nodules in the scrotum. Each nodule was palpated at the tail of each epididymis (the diameters of these nodules were 12 mm on left and 5 mm on right). We followed the patient for 18 months. Since the nodules were increasing in size, the nodules were surgically removed. Histological examination revealed primary leiomyomas of the epididymis.  相似文献
