An Unusual Complication of Facial Sclerotherapy

The treatment of facial veins can be achieved in various ways, with each method having its particular side effects and benefits. Sclerotherapy is a relatively simple and easy method to treat these vessels, generally with an acceptable side effect profile and minimal material requirement and cost. Facial veins occur in many individuals and may relate to estrogen levels, disease, or oral contraceptive pills. They also occur in a number of hereditary conditions such as poikiloderma and other essential telangiectasiae. The case history described outlines an unusual consequence of sclerotherapy to the face that may relate to the anatomy of the vessel, the sclerosant, neurovascular considerations, or the personal medical status of the patient. In this instance, the resolution was complete with treatment success, but this event was temporarily distressing to all involved, and to determine etiology may be of benefit in preventing further incidents.     

An Unusual Complication of Vesical Stones

The author reports a complicated recurrence of genital tract fistula. The initial vesicovaginal fistula resulted from obstructed labor, which was subsequently surgically managed without success. The patient had a recurrent vesicovaginal fistula that was formed by the erosion of vesical stones through the anterior endopelvic fascia into the vagina. The stones eventually eroded into the rectum, resulting in the formation of a complex vesicovaginal and rectovaginal fistula.     

An Unusual Complication after Roux-en-Y Gastric Bypass: Torsion and Infarction of the Divided Omentum

A 41-year-old male was admitted from the emergency department with severe epigastric and right hypochondric abdominal pain. The patient had a laparoscopic Roux-en-Y gastric bypass operation 12 months previously. An abdominal computed tomography scan showed signs of omental infarction. A laparoscopy was performed and showed a torsion and infarction of the divided greater omentum. Resection of the ischemic omentum was performed.     

An Unusual Complication of Gastric Bypass: Perforated Duodenal Ulcer

Peptic ulcer in the excluded segment of a gastric bypass has been reported in the literature in only 17 cases.We report a 54-year-old woman with a perforated duodenal ulcer, who had undergone laparoscopic Roux-en-Y gastric bypass surgery for morbid obesity 15 months previously. She was successfully treated by a laparoscopic repair of the perforated duodenal ulcer.
