Exerc., Vol. 33, No. 3, 2001, pp. 442-448. Background: Various abnormalities in skeletal muscle have been demonstrated by biopsy in patients with chronic heart failure (CHF). In mammalian muscles, high-energy metabolite composition at rest (HEMC) provides data on important metabolic characteristics; however, the significance of HEMC has not been clarified in patients with CHF. Therefore, we investigated HEMC in normal subjects and patients with CHF and examined its relation to muscle metabolic capacity and exercise tolerance. Methods: High-energy metabolites (phosphocreatine (PCr), inorganic phosphate (Pi), and ATP) in resting calf muscle were measured by 31P-magnetic resonance spectroscopy (31P-MRS), and ratios of Pi to PCr, Pi to ATP, and PCr to ATP were calculated in 34 patients with CHF and 13 age- and size-matched normal subjects. Muscle metabolism was evaluated during local exercise of unilateral plantar flexion by 31P-MRS. Metabolic capacity was estimated by the rate of PCr breakdown in relation to the workload. Systemic exercise capacity was evaluated by a bicycle ergometer. Results: The ratio of PCr to ATP was significantly increased in patients with CHF compared with controls (3.06 +/- 0.43 vs 2.72 +/- 0.36, P < 0.05) and was significantly correlated with metabolic capacity (r = -0.37, P < 0.01) and with peak oxygen uptake (r = -0.45, P < 0.01). There was a significant correlation between metabolic capacity and peak oxygen uptake (r = 0.53, P < 0.001). Conclusion: HEMC was altered in patients with CHF, and this change was related to metabolic capacity and exercise capacity. These findings provide new insight into the mechanism of impaired muscle metabolism in CHF.  相似文献

PURPOSE: Our purpose was to study effects of creatine (Cr) supplementation on muscle metabolites noninvasively by means of magnetic resonance spectroscopy (MRS) before and after supplementation with Cr or placebo. METHODS: 1H-MRS was used in a comprehensive, double-blind, cross-over study in 10 volunteers to measure Cr in m. tibialis anterior and m. rectus femoris at rest. PCr/ATP was observed in m. quadriceps femoris by 31P-MRS at rest and after exercise. RESULTS: A significant increase in total Cr was observed with Cr intake in m. tibialis anterior (+9.6 +/- 1.7%, P = 0.001) and in m. rectus femoris (+18.0 +/- 1.8%, P < 0.001). PCr/ATP showed a significant increase (+23.9 +/- 2.3%, P < 0.001) in m. quadriceps femoris at rest with Cr supplementation. Post-Cr supplementation recovery rates from exercise were significantly lower (k = 0.029 s(-1), P < 0.01) compared with postplacebo consumption (k = 0.034 s(-1)) and presupplementation (k = 0.037 s(-1)). However, higher levels of PCr/ATP at rest compensate for this reduction of the recovery rate after Cr supplementation. The increase of PCr/ATP determined by 31P-MRS correlates with the increase of Cr observed by 1H-MRS (r = 0.824, P < 0.001). CONCLUSION: Noninvasive observation of Cr and PCr after Cr supplementation shows an increase in a muscle specific manner. Higher preexercise levels of PCr/ATP at rest compensate for significantly slower recovery rates of PCr/ATP after Cr supplementation.  相似文献

目的用磁共振动态磷谱技术(31P-MRS)无创性在体评价骨骼肌的功能。资料与方法对10名成年人和6名青少年受试者骨骼肌进行动态31P-MRS分析,对磷酸单酯(PME)、无机磷(Pi)、磷酸二酯(PDE)、磷酸肌酸(CP)、γ-ATP、α-ATP和β-ATP进行半绝对定量分析,同时计算二磷酸腺苷(ADP)和线粒体内氧化磷酸化潜能(PP)以及细胞内镁离子的浓度。分析在静息期、运动期和恢复期两组受试者能量代谢特点及肌肉做功效率。结果肌肉运动时CP和PP含量明显降低,Pi、Pi/CP和ADP升高,而ATP维持恒定。青少年肌细胞内ATP明显高于成人,而CP和PP含量相似,动态变化趋势相同。成人做功较多,但肌肉的效率与青少年组相同。结论动态磷谱技术可以无创性定量评价骨骼肌不同运动状态能量代谢特点,为肌肉的功能影像学提供客观信息。  相似文献

Electrical stimulation (ES) and voluntary contraction (VC) were compared in the quadriceps muscle of ten male volunteers. In both modes, a workload corresponding to 20% of maximal voluntary contraction was applied during 64 isometric contraction (5.5 s)-relaxation (5.5 s) cycles. The protocols were performed in a 1.5 T whole-body magnet. The Pi/PCr ratio and the intracellular pH (pHi) were monitored by 31P NMR spectroscopy during baseline, exercise and recovery periods, in a superficial region of the vastus medialis. During baseline, the Pi/PCr ratio (0.12 vs. 0.10) and the pHi (7.01 vs. 7.00) were comparable in both conditions. During exercise, the Pi/PCr ratio was higher (0.36 vs. 0.14) and the pHi was lower (6.85 vs. 7.07) during ES than during VC. For the same external work production, these results reflect a different metabolic solicitation in the ES quadriceps than in the VC ones.  相似文献

OBJECTIVE: To compare peak oxygen consumption (VO2peak) and skeletal muscle oxidative metabolism between nine African-American and nine Caucasian men. METHODS: Subjects performed arm ergometry to exhaustion. On a separate occasion 31phosphorous-nuclear magnetic resonance spectroscopy (31P-NMRS) was used to determine the concentrations of phosphorous (Pi), phosphocreatine (PCr), and the intracellular pH of the flexor carpi radialis before and during 4 min of steady-state, wrist flexion exercise performed at 28% (15 W) of each subject's peak voluntary contraction. RESULTS: The Pi/PCr ratio was used as an indirect measure of skeletal muscle oxidative metabolism. VO2peak was lower in the African-Americans compared with the Caucasians (means +/- SD, 19.4 +/- 3.4 vs 23.3 +/- 4.0 mL x kg(-1) x min(-1)) (P < 0.05). No significant between group difference was noted in the Pi/PCr ratio at rest (0.10 +/- 0.02 both groups). However, resting pH was lower in the African-Americans (6.99 +/- 0.04 vs 7.03 +/- 0.05) (P < 0.05). Exercise caused an increase in the Pi/PCr ratio in the African-Americans (1.06 +/- 0.11), which was higher than the increase observed in the Caucasians (0.50 +/- 0.14) (P < 0.05). pH levels decreased to a lower level during exercise in the African-Americans (6.89 +/- 0.04) than in the Caucasians (6.98 +/- 0.05) (P < 0.05). CONCLUSIONS: This select group of African-American men achieved a lower VO2peak than the Caucasian men. Variations in skeletal muscle oxidative metabolic components may explain this difference.  相似文献

目的:应用磁共振成像(MRI)及磷频谱(31P-MRS)评价脂质沉积性肌病(LSM)的影像表现,探讨其临床应用价值.方法:对经病理证实的6例LSM和17例健康对照者进行MRI及31P-MRS检查.结果:6例LSM患者大腿后组肌群和内侧肌群T1 WI及T2 WI上均表现为多发小斑点状高信号,T2 WI抑脂后信号减低;LSM组PCr(磷酸肌酸)、PCr/ATP(三磷酸腺苷)和磷酸化能力(PP)较对照组明显降低(P<0.05),二磷酸腺苷(ADP)、Pi(无机磷)、Pi/ATP、Pi/PCr较对照组明显增高(P<0.05),PH与对照组无明显差别(P=0.6395).结论:MRI联合31P-MRS检查有利于LSM的诊断、鉴别诊断.  相似文献

PURPOSE: In the present study we investigated whether a high volume of cycling training would influence the metabolic changes associated with a succession of three exhaustive cycling exercises. METHODS: Seven professional road cyclists (VO2max: 74.3 +/- 3.7 mL.min.kg; maximal power tolerated: 475 +/- 18 W; training: 22 +/- 3 h.wk) and seven sport sciences students (VO2max: 54.2 +/- 5.3 mL.min.kg; maximal power tolerated: 341 +/- 26 W; training: 6 +/- 2 h.wk) performed three different exhaustive cycling exercise bouts (progressive, constant load, and sprint) on an electrically braked cycloergometer positioned near the magnetic resonance scanner. Less than 45 s after the completion of each exercise bout, recovery kinetics of high-energy phosphorylated compounds and pH were measured using P-MR spectroscopy. RESULTS: Resting values for phosphomonoesters (PME) and phosphodiesters (PDE) were significantly elevated in the cyclist group (PME/ATP: 0.82 +/- 0.11 vs 0.58 +/- 0.19; PDE/ATP: 0.27 +/- 0.03 vs 0.21 +/- 0.05). Phosphocreatine (PCr) consumption and inorganic phosphate (Pi) accumulation measured at end of exercise bouts 1 (PCr: 6.5 +/- 3.2 vs 10.4 +/- 1.6 mM; Pi: 1.6 +/- 0.7 vs 6.8 +/- 3.4 mM) and 3 (PCr: 5.6 +/- 2.4 vs 9.3 +/- 3.9 mM; Pi: 1.5 +/- 0.5 vs 7.7 +/- 3.3 mM) were reduced in cyclists compared with controls. During the recovery period after each exercise bout, the pH-recovery rate was larger in professional road cyclists, whereas the PCr-recovery kinetics were significantly faster for cyclists only for bout 3. DISCUSSION: Whereas the PDE and PME elevation at rest in professional cyclists may indicate fiber-type changes and an imbalance between glycogenolytic and glycolytic activity, the lower PCr consumption during exercise and the faster pH-recovery kinetic clearly suggest an improved mitochondrial function.  相似文献

In the conclusion of this series of reports, the application of 31P/2H NMR to investigate the pathophysiology of sepsis in rat hindlimb muscle is demonstrated. Sepsis decreased muscle [PCr] by 18%, 18 +/- 4 SD vs 22 +/- 4 SD mmol/kg tissue wet wt (P = 0.01) in control rats but [ATP] was unchanged, 6 mmol/kg tissue wet wt (P = 0.2). The derived free cytosolic [ADP] in the two groups was similar, [ADP]septic = 0.023 +/- 0.004 SD and [ADP]control = 0.021 +/- 0.003 SD mmol/kg tissue wet wt, and not statistically different (P = 0.14). Likewise [Pi] in the septic and control groups was not statistically different, [Pi]septic = 1.1 +/- 0.5 SD and [Pi]control = 1.2 +/- 0.4 SD mmol/kg tissue wet wt (P = 0.2). Septic rats presented the symptom of respiratory alkalosis evidenced by elevated blood pH. Sepsis decreased muscle blood flow by 33%, P = 0.003, but examination of individual subjects did not demonstrate a correlation with the reduction in [PCr]. Thus, a metabolic energy deficit caused by cellular ischemia/hypoxia is not a likely cause of cellular abnormality in rat hindlimb muscle during sepsis.  相似文献

PURPOSE: The purpose of this study was to measure noninvasively the absolute concentrations of muscle adenosine triphosphate [ATP], phosphocreatine [PCr], inorganic phosphate (Pi), and glycogen [Gly] of elite soccer players. METHODS: Magnetic resonance spectroscopy (31P- and 13C-MRS) was used to measure the concentrations of metabolites in the calf muscles of 18 young male players [age = 17.5 +/- 1.0 (SD) yr]. RESULTS: Average muscle [PCr] and [ATP] were 17.8 +/- 3.3 and 6.0 +/- 1.2 mmol x (kg wet weight)(-1), respectively. The ratios of Pi/PCr and PCr/ATP were 0.15 +/- 0.05 and 3.00 +/- 0.26, respectively. The muscle [Gly] was 144 +/- 54 mmol x (kg wet weight)(-1). There was a high correlation (r = 0.93, P < 0.0001) between muscle ATP and PCr concentrations, but there was no correlation between [Gly] and [PCr] or [ATP]. The concentrations of the different metabolites determined in the present study with noninvasive MRS methods were within the ranges of values reported in human muscle from biochemical analysis of muscle biopsies. CONCLUSION: MRS methods can be utilized to assess noninvasively the muscle energetic status of elite soccer players during a soccer season. The high correlation between ATP and PCr might be indicative of fiber type differences in the content of these two metabolites.  相似文献

Muscle data from phosphorus-31 magnetic resonance (MR) spectroscopy and hydrogen-1 MR imaging and popliteal artery data from duplex Doppler ultrasound were compared during an exercise test of the anterior compartment of the leg, in nine healthy volunteers. Significant variations (mean +/- standard deviation) were observed at the end of exercise versus rest in intracellular pH (pHi) (6.32 +/- 0.02 vs 7.02 +/- 0.04, P < .001), T2 (38.2 msec +/- 2.3 vs 29.5 msec +/- 1.1, P < .001), and popliteal output (652 mL/min +/- 232 vs 149 mL/min +/- 65, P < .001). These variables showed the following significant correlations at the end of exercise: T2 and pHi (r = -.784, P < .01), T2 and popliteal output (r = .737, P < .03), and pHi and popliteal output (r = -.902, P < .001). However, during recovery, the T2 curve was significantly different from those of pHi and popliteal output. This suggests that even if circulatory conditions play a role in the maximum T2 variation during exercise, they do not directly explain T2 changes. Furthermore, the correlations involving pHi suggest the role of the metabolism of exercising muscle in transcapillary fluid movement.  相似文献

PURPOSE: The effects of aging on muscle metabolism and oxygenation have not yet been elucidated. We evaluated the effects of aging on energy metabolism and oxygenation in sedentary healthy subjects by simultaneously measuring 31P-magnetic resonance spectroscopy (MRS) and near-infrared spectroscopy (NIRS). METHODS: Nine young (28.1 +/- 5.0 yr) and nine older (61.4 +/- 4.6 yr) healthy subjects were studied. The 31P-MR spectrum was obtained every 15 s during and after hand gripping exercise. Intracellular pH (pHi) and PCr/(PCr+Pi) [PCr: phosphocreatine, Pi: inorganic phosphate] were calculated as an index of energy metabolism. The time constant of the PCr/(PCr+Pi) recovery (tau PCr) was calculated. With NIRS, we evaluated the recovery rates of oxygenated (RHbO2) and deoxygenated hemoglobin (RHb) during the initial 10 s of recovery. RESULTS: The PCr/(PCr+Pi) and pHi at rest and at completion of the exercise and tau PCr did not differ between young and older subjects. However, RHbO2 and RHb were significantly slower in older subjects than in young subjects. CONCLUSIONS: The results suggest that muscle energy metabolism in the forearm muscle was not affected by aging. The slower RHbO2 and RHb in older subjects suggested impaired O2 supply, which was probably due to impaired peripheral circulation caused by the process of aging.  相似文献

This study evaluates the effects of a 6 months' training period on the bioenergetics of the calf muscle of elite athletes by means of phosphorus-31 Nuclear Magnetic Resonance Spectroscopy (31P-NMR). Four downhill skiers, belonging to the Spanish National Team, performed a standardized exercise protocol using their right leg inside a wide-bore 2.35 Tesla magnet. The inorganic phosphate/phosphocreatine (Pi/PC) ratio and intracellular pH (pHi) were measured at steady-state during an exercise protocol composed of 5 work levels between 20% and 80% of the maximum voluntary contraction (MVC), before and after the training period. The measured values, which were markedly scattered at the beginning, regrouped after training. This was caused by a shift towards lower Pi/PC ratios and by a lower pHi acidification in three of the four subjects. This result suggests that 31P-NMR is a good tool to evaluate changes in the muscle aerobic capacity of athletes induced by training.  相似文献

PURPOSE: To investigate the effects of cooling human skeletal muscle after strenuous exercise using 31P MR spectroscopy and MR imaging. METHODS: 14 male subjects (mean age +/- SD, 23.8 +/- 2.3 yr) were randomly assigned to the normal (N = 7) or the cooling group (N = 7). All subjects performed the ankle plantar flexion exercise (12 repetitions, 5 sets). Localized 31P-spectra were collected from the medial gastrocnemius before and after exercise (immediately, 30, 60 min, 24, 48, 96, and 168 h) to determine the ratio of inorganic phosphate to phosphocreatine (Pi/PCr) and intracellular pH. Transaxial T2-weighted MR images of the medial gastrocnemius were obtained to calculate T2 relaxation time (T2), indicative of intramuscular water level, before and after exercise (24, 48, 96, and 168 h). In addition, the muscle soreness level was assessed at the same time as 31P-spectra measurements. Fifteen-minute cold-water immersion was administered to the cooling group after exercise and initial postexercise measurements. RESULTS: The control group showed significantly increased T2 from rest at 48 h after exercise (P < 0.05), but the cooling group showed no significant change in T2 throughout this study. Both groups showed a significantly decreased intracellular pH immediately after exercise (P < 0.05). After that, the cooling group showed a significantly greater value than the value at rest or the control group at 60 min after exercise (P < 0.05). For the Pi/PCr, no significant change was observed in both groups throughout this study. The muscle soreness level significantly increased immediately and at 24-48 h after exercise in both groups (P < 0.05). CONCLUSION: The findings of this study suggest that cooling causes an increase in intracellular pH and prevents the delayed muscle edema.  相似文献

Effect of exogenous creatine supplementation on muscle PCr metabolism   总被引:5,自引:0,他引:5  
31P NMR was used to assess the influence of two weeks creatine supplementation (21g x d(-1)) on resting muscle PCr concentration, on the rate of PCr repletion (R(depl)), and on the half-time of PCr repletion (t 1/2). Body mass (BM) and volume of body water compartments were also estimated by impedance spectroscopy. Fourteen healthy male subjects (20.8+/-1.9 y) participated in this double-blind study. PCr was measured using a surface coil placed under the calf muscle, at rest and during two exercise bout the duration of which was 1 min. They were interspaced by a recovery of 10 min. The exercises comprised of 50 plantar flexions-extensions against weights corresponding to 40% and 70% of maximal voluntary contraction (MVC), respectively. Creatine supplementation increased resting muscle PCr content by approximately 20% (P= 0.002). R(depl) was also increased by approximately 15% (P< 0.001) and approximately 10% (P = 0.026) during 40% and 70% MVC exercises, respectively. No change was observed in R(repl) and t1/2. BM and body water compartments were not influenced. These results indicate that during a standardized exercise more ATP is synthesized by the CK reaction when the pre-exercise level in PCr is higher, giving some support to the positive effects recorded on muscle performance.  相似文献

PURPOSE: To determine the clinical feasibility of rapid-sequence phosphorus-31 magnetic resonance spectroscopy (31P-MRS) of the heart with cardiac patients using a 1.5T clinical MR system. MATERIAL AND METHODS: Twenty cardiac patients, i.e. dilated cardiomyopathy (DCM) 13 cases, hypertrophic cardiomyopathy (HCM) 3 cases, hypertensive heart diseases (HHD) 3 cases, and aortic regurgitation (AR) 1 case were examined using rapid cardiac 31P-MRS. Complete three-dimensional localization was performed using a two-dimensional phosphorus chemical-shift imaging sequence in combination with 30-mm axial slice-selective excitation. The rapid-sequence 31P-MRS procedure was phase encoded in arrays of 8 x 8 steps with an average of 4 acquisitions. The total examination time, including proton imaging and shimming, for the rapid cardiac 31P-MRS procedure, ranged from 10 to 15 min, depending on the heart rate. Student's t test was used to compare creatine phosphate (PCr)/adenosine triphosphate (ATP) ratios from the cardiac patients with those of the control subjects (n = 13). RESULTS: The myocardial PCr/ATP ratio obtained by rapid 31P-MRS was significantly lower (P < 0.001) in DCM patients (1.82 +/- 0.33, mean +/- SD), and in patients with global myocardial dysfunction (combined data for 20 patients: 1.89 +/- 0.32) than in normal volunteers (2.96 +/- 0.59). These results are similar to previous studies. CONCLUSION: Rapid-sequence 31P-MRS may be a valid diagnostic tool for patients with cardiac disease.  相似文献

Physiological alterations following unaccustomed eccentric exercise in an isokinetic dynamometer of the right m. quadriceps until exhaustion were studied, in order to create a model in which the physiological responses to physiotherapy could be measured. In experiment I (exp. I), seven selected parameters were measured bilaterally in 7 healthy subjects at day 0 as a control value. Then after a standardized bout of eccentric exercise the same parameters were measured daily for the following 7 d (test values). The measured parameters were: the ratio of phosphocreatine to inorganic phosphate (PCr/Pi), the ratio of inorganic phosphate to adenosintriphosphate (Pi/ATP), the ratio of phosphocreatine to adenosintriphosphate (PCr/ATP) (all three ratios measured with 31P-nuclear magnetic resonance spectroscopy), dynamic muscle strength, plasma creatine kinase (CK), degree of pain and “muscle” blood flow rate (133Xenon washout technique). This was repeated in experiment II (exp. II) 6–12 months later in order to study reproducbility. In experiment III (exp. III), the normal fluctuations over 8 d of the seven parameters were measured, without intervention with eccentric exercise in 6 other subjects. All subjects experienced pain, reaching a maximum 48 h after eccentric exercise in both exp. I and II. A systematic effect over time for CK (increasing 278% resp. 308%), muscle strength (decreasing more than 10%), PCr/Pi (decreasing 31% resp. 43%) and Pi/ATP (increasing 55% resp. 99%) was found in both exp. I and II (P<0.05), but not in exp. III. No significant difference was observed between exp. I and II for CK, blood-flow rate, concentric muscle strength, PCr/Pi, Pi/ATP and PCr/ATP. It is concluded that pathophysiological alterations in m. quadriceps following eccentric exercise can be induced and can be reproduced after an interval of 6 months. Thus, this model can be used to study the effects of physiotherapy.  相似文献

In vivo 31P NMR spectroscopy was used to determine the ratios of creatine phosphate (PCr) to adenosine triphosphate (ATP) and inorganic phosphate (Pi) in leg and arm muscles of four sprinters, one marathon runner, and two sedentary subjects. Both ratios were definitely higher in the sprinters indicating that, since muscle ATP and Pi concentrations are constant, the PCr muscle content of these athletes is higher than usual. Sprinters are known to have higher percentages of fast-twitch fibers, which are richer in PCr than slow-twitch fibers. It is concluded that measurements of muscle ATP, PCr, and Pi through in vivo NMR spectroscopy could be used to determine muscle fiber composition.  相似文献

In this study 31P spectral changes were closely monitored following the initial administration of cytotoxic drugs and related to five parameters of patient response. Pre- and postchemotherapy 31P MRS examinations were performed on 16 patients with large, malignant tumors. These included four tumor types: (i) lymphoma (n = 7), (ii) breast carcinoma (n = 4), (iii) musculoskeletal tumors (n = 4), and (iv) adenocarcinoma (n = 1). A mean of 5 spectra/patient (range 2-10) was performed following the initial chemotherapy. The spectral trends exhibited by 14 of 16 patients reached "points of maximum change," after which they began to revert toward prechemotherapy values. In 2 of 16 patients that did not respond to the initial chemotherapy regimen, no spectral trends were observed. The degree of change of certain spectral parameters, namely, decreases in PME, PME/PDE, PME/PCr, PME/NTP, PDE/PCr, and tumor pH, as well as increases in the ratios Pi/PME and Pi/PDE, were associated with good patient response and separated responders from nonresponders. 目的:运用3T高场强磁共振动态磷谱技术(^31P-MRS)在体评价骨骼肌线粒体能量代谢情况。方法:对20名正常人受试者骨骼肌进行动态31P-MRS采集,后期利用Matlab软件对无机磷(Pi)、磷酸肌酸(PCr)、三磷酸腺苷(ATP)等化合物的峰下面积进行定量分析,分别计算在静止期,运动末期及数个恢复期骨骼肌内高能磷酸化合物的含量,同时计算二磷酸腺苷(ADP)和细胞内PH值,评价磁共振动态磷谱技术对研究骨骼肌线粒体功能的价值。结果:肌肉运动时PCr含量明显降低,Pi、Pi/PCr和ADP升高,恢复期各含磷化合物含量逐渐恢复至静息水平。结论:3T高场强动态磷谱技术可以无创性定量评价骨骼肌线粒体功能,为肌肉的功能影像学提供客观证据,为以后客观研究肌肉相关疾病提供了理想的方法。  相似文献
