Hyperprolactinemia is common in patients with renal failure. Because radiographic contrast material given during a computed tomographic (CT) scan of the sella as part of the evaluation for prolactinoma worsens renal insufficiency, we attempted to define the point at which hyperprolactinemia becomes an expected finding in patients with renal insufficiency in this study. Of 59 patients with serum creatinine levels of 1.5 to 12 mg/dL, 16 (27.1%) were hyperprolactinemic. Of these 16, nine were not taking medications known to raise prolactin levels and their prolactin levels were less than 100 ng/mL. In the eight patients taking medications prolactin levels were much higher. In one patient the prolactin level fell from 2,210 to 100 ng/mL when methyldopa was discontinued. In patients with chronic renal failure prolactin levels were similar regardless of the method of dialysis. We conclude that in the absence of medications known to affect prolactin secretion, hyperprolactinemia occurs infrequently (18.3%) and, when it occurs, is mild (less than 100 ng/mL). Marked hyperprolactinemia may occur in patients taking such medications. These should be stopped and the prolactin level rechecked before a CT scan is performed.  相似文献

OBJECTIVE: Coronary artery bypass grafting (CABG) under a beating heart is reported to be less invasive and promise earlier recovery. This study was performed to evaluate the efficacy of off-pump CABG in patients with end-stage renal failure. METHODS: Isolated CABG was performed on 40 hemodialysis patients at Shin-Tokyo Hospital Group between September 1, 1993, and December 31, 2000. Among them, off-pump CABG was performed in 16 and on-pump CABG in 24. Their preoperative, perioperative, and follow-up data were retrospectively collected. RESULTS: Patient's demographics and coronary risk factors were similar in off-pump and on-pump groups. The mean number of bypass grafts was 1.9 +/- 1.1 in the off-pump group and 2.8 +/- 1.1 in the on-pump group (P < 0.05). Blood transfusion was significantly less frequent in the off-pump group than in on-pump group. Postoperative complications were more frequently observed in the on-pump group (7.1% off-pump vs 25.0% on-pump). There were two hospital deaths in the on-pump group and none in the off-pump group. Postoperative intubation time, ICU stay, and hospital stay were significantly shorter in the off-pump group than in the on-pump group. Although follow-up period was short (1.1 +/- 0.7 years), no cardiac events occurred in the off-pump group. CONCLUSIONS: Off-pump CABG for hemodialysis patients is safe and useful and it enables early recovery. Postoperative cardiac events were controlled effectively during the short period of follow-up.  相似文献

Background: Erectile dysfunction (ED) is a disorder that is frequently observed in people with chronic kidney disease who undergo hemodialysis (HD). In the context of evidence-based medicine, we aimed to investigate the effect of low-dose tadalafil on sexual function in patients undergoing HD.

Methods: The medical records of 30 males (aged 29–65?years) with end-stage renal disease (ESRD) on a HD program, and who had received 5?mg tadalafil twice weekly, were retrospectively evaluated. Changes in erectile and ejaculatory function were evaluated using the International Erectile Function Index questionnaire, the Erection Hardness Scale (EHS), and the Male Sexual Health Questionnaire (MSHQ).

Results: The mean age of the patients was 47.6?±?10.1?years, their mean body mass index was 24.3?±?4.2?kg/m2, their mean hemoglobin was 11.9?±?0.9?g/dL, and their mean creatinine clearance was 5.8?±?1.1?mL/min. At the third month of treatment, 36.6% of the patients had no ED, 40% had mild ED, 10% had mild-to-moderate ED, and 13.3% had moderate ED. The mean MSHQ scores (p?p?=?.001) were significantly improved. There was no significant difference between Beck's Depression Inventory scores (p?>?.05), but Hamilton anxiety rate scores decreased significantly (p?=?.001). The quality-of-life score improved throughout the study period (p?Conclusions: Tadalafil therapy is an effective therapeutic option in patients with ESRD who undergo HD, not only for the treatment of ED, but also for ejaculatory function, with acceptable adverse effects.  相似文献

目的探讨血管介入治疗对移植肾动脉狭窄(transplanted renal arterial stenosis,TRAS)的疗效。方法回顾性分析本中心513例肾移植患者中并发TRAS的患者资料,比较血管介入治疗前、后的1周、1个月、3个月、6个月,患者的肌酐、血压及移植肾血流动力学指标的变化。结果513例肾移植患者中有9例并发TRAS,发生率为1.75%,9例患者均行血管介入治疗,其中8例患者单纯行球囊扩张术,1例患者行球囊扩张后置入血管支架。8例球囊扩张术的患者有3例术后2个月内再次出现狭窄,二次狭窄率为33.3%,再次给予球囊扩张均成功。所有患者随访6个月,1例患者血管介入治疗后4个月因肺部感染死亡。治疗前患者血肌酐为(142.3±59.6)μmol/L,治疗后1周、1个月、3个月、6个月分别为(133.5±57.2)μmol/L、(131.8±35.6)μmol/L、(127.0±29.9)μmol/L、(125.7±37.1)μmol/L,与术前相比,无统计学差异,但有下降趋势。治疗前患者收缩压为(149.7±19.3)mmHg(1 mmHg=0.133 kPa),治疗后1周、1个月、3个月、6个月分别为(131.3±4.1)mmHg、(136.2±7.9)mmHg、(128.5±6.6)mmHg、(127.1±3.6)mmHg,与术前相比,收缩压明显降低,具有统计学差异。治疗前患者移植肾主动脉峰值流速(peak systolic velocity,PSV)为(297.2±105.3)cm/s,治疗后1周、1个月、3个月、6个月分别(171±56.3)cm/s、(185.8±64.8)cm/s、(197.5±69.1)cm/s、(178.8±75.4)cm/s,与术前相比,均具有统计学差异。治疗后1周、1个月、3个月、6个月的叶间PSV、叶间动脉阻力指数,与术前相比,无统计学差异。结论血管介入治疗的疗效确切,能有效改善TRAS患者移植肾功能。  相似文献

目的探讨同种异体肾移植术后移植肾动脉狭窄(TRAS)介入治疗的临床疗效。方法介入治疗同种异体肾移植术后TRAS患者22例,其中单纯球囊扩张治疗10例(球囊组),内支架治疗12例(支架组)。对所有患者术前及术后2年内血压、肾功能以及生活质量进行评估,并与同期接受单纯药物治疗的6例TRAS患者(药物组)进行比较。结果球囊组技术成功率90.00%,支架组技术成功率100%。术后在血压及肾功能改善方面,球囊组和支架组均取得显著疗效,两组近期疗效未见明显差异,药物组疗效不满意。术后随访6个月~2年,支架组再狭窄率16.67%,除1例不明原因治疗无效外,其余11例均能比较健康的生活和工作。球囊组手术失败1例,再狭窄率达40.00%。结论内支架植入术可作为TRAS特别是I型及Ⅱ型狭窄的首选治疗方法。  相似文献

Background: Ischemic heart disease is the major cause of death inpatients with end-stage renal disease. The high prevalence of coronary artery disease results in a rising number of dialysis patients requiring myocardial revascularisation. Objective: The objective of this study was to compare the outcomes of recurrent angina, myocardial infarction, rate of reinterventions and cardiovascular death following percutaneous coronary angioplasty (PTCA) or coronary artery bypass grafting (CABG) inpatients with end-stage renal disease. Patients and methods: In a retrospective investigation 40 patients with chronic renal failure undergoing primarily PTCA and 65 patients undergoing CABG were included. Both groups were comparable for gender, duration on dialysis and the number of cardiovascular risk factors per patient. Patients undergoing PTCA were younger (53 ± 12 years vs. 57 ± 8 years; p < 0.05) and more often diabetics (30% vs. 14%; p < 0.05). Results: Most patients in both groups had a multi-vessel disease (95% in the CABG group vs. 74% in the PTCA group), in the CABG group there were significantly more patients with a triple-vessel disease (62% with vs. 40%in the PTCA group; p < 0.01), PTCA was primarily successful in 95% of the patients while complete revascularization was achieved in 88% of patients undergoing CABG. The perioperative mortality after CABG was 4.8% as compared to none after interventional revascularisation. The cumulative freedom of angina after 6, 12 and 24 months after intervention was significantly lower after PTCA (54%, 40%, 29%) than after bypass grafting (97%, 94%, 90%, p < 0.001). The frequency of reinterventions following PTCA was significantly higher compared to patients following CABG (p < 0.001). After PTCA 15 patients needed further revascularisations, 8 of them underwent CABG, whereas after CABG only two patients required additional myocardial revascularisation. There was no significant difference in the overall mortality between both groups; the survival rate after 12 and 24 months was 95% and 82% after PTCA and 93% and 86% after CABG, respectively. Condition: Although patients receiving CABG had a more severe coronary artery disease the overall mortality was comparable and clinical and functional outcome was improved compared to patients after coronary angioplasty. Objective: To determine the impact of mitral insufficiency on survival after reoperative coronary artery bypass grafting (CABG) in ischemic cardiomyopathy patients. Methods: We retrospectively studied 891 (Initial 708, Redo 183) consecutive CABG patients (1993–2002) with ejection fraction (EF) 35% or less. Patient characteristics: mean age=67.0±10.5 yrs, men=77.1%, EF=26.4±7.4%, mean CCS=3.47±0.73, mean NYHA=3.50±0.68. There were 180 (Initial 141, Redo 39) patients with 3/4+mitral regurgitation (MR). Late survival statistics of cohorts were compiled using National Death Index. Results: At a mean follow-up period of 3 years, reoperative CABG with MR (Redo/MR+) survival was 41.7±9.2% (n=39), which was worse than reoperative CABG without MR (Redo/MR−) survival of 71.8±4.1% (n=144, P=0.0003), initial CABG with MR (Initial/MR+) survival of 68.5±4.2% (n=141, P=0.014) and initial CABG without MR (Initial/MR−) survival of 76.2±2.0% (n=567, P<0.0001). By multivariate analysis, congestive heart failure (P=0.029), 3/4+MR (P=0.044) were independent predictors of Redo late mortality. In contrast, renal failure on dialysis, stroke, no angina, age >65 yrs, absence of hypercholesterolemia, EF<26% but not 3/4+MR were independent predictors of Initial late mortality. In subset analysis, adverse impact of 3/4+MR on late survival was greatest in Redo with EF<26%. The 3- and 5-yr late survival were only 44.4 and 26.8% (P=0.041). Concomitant mitral valve repair (MVrep) was performed in 100 (Initial 75, Redo 25) patients. MVrep in Initial/MR+patients achieved similar late survival as Initial/MR− patients. MVrep did not produce the same late survival benefit in Redo/MR+ patients. Conclusions: (1) Mitral insufficiency has a greater survival impact on redo than initial CABG patients with ischemic cardiomyopathy. (2) Mitral insufficiency and congestive heart failure are the primary independent predictors of late survival following redo CABG. (3) Mitral repair has less neutralizing effect on late survival in redo than initial CABG patients.  相似文献

ObjectiveA high coronary artery calcification score (CACS) may be associated with high mortality in patients undergoing hemodialysis (HD). Recently, effects of iron on vascular smooth muscle cell calcification have been described. We aimed to investigate the relationships between iron, CACS, and mortality in HD patients.MethodsWe studied 173 consecutive patients who were undergoing maintenance HD. Laboratory data and Agatston’s CACS were obtained at baseline for two groups of patients: those with CACS ≥400 (n = 109) and those with CACS <400 (n = 64). Logistic regression analyses for CACS ≥400 and Cox proportional hazard analyses for mortality were conducted.ResultsThe median (interquartile range) age and duration of dialysis of the participants were 67 (60–75) years and 73 (37–138) months, respectively. Serum iron (Fe) and transferrin saturation (TSAT) levels were significantly lower in participants with CACS ≥400 than in those with CACS <400, although the serum ferritin concentration did not differ between the groups. TSAT ≥21% was significantly associated with CACS ≥400 (odds ratio 0.46, p<0.05). TSAT ≥17%, Fe ≥63 µg/dL, and ferritin ≥200 ng/mL appear to protect against 5-year all-cause mortality in HD patients, independent of conventional risk factors of all-cause mortality (p < 0.05).ConclusionWe have identified associations between iron, CACS, and mortality in HD patients. Lower TSAT was found to be an independent predictor of CACS ≥400, and iron deficiency (low TSAT, iron, or ferritin) was a significant predictor of 5-year all-cause mortality in HD patients.  相似文献

One hundred radiographs of the left hand and wrist from 40 children with chronic renal insufficiency or end-stage renal disease were examined to determine which method of bone age estimation provided the most useful information in these children. The Tanner and Whitehouse method showed better repeatability than the Greulich and Pyle atlas or the Buckler handbook when a sample of the radiographs were assessed twice by the same observer. The Tanner and Witehouse 20 (TW20) bone age showed less inter-observer bias than the radius, ulna and short bone age or the carpal bone age when three observers independently assessed the same sample of radiographs. TW20 was the most useful method of bone age assessment in this study of British children. An unexpected finding was that the carpal bones were significantly more retarded than the radius, ulna and short bones. Separate assessment of the carpal bone age may provide extra information of clinical relevance.  相似文献

BACKGROUND: Patients with end-stage renal disease have an increased risk of developing coronary artery disease (CAD). The cardiovascular mortality of dialysis patients is 10-15 times higher compared with the general population. The aim of our study was to evaluate the morphological progression of coronary arteriosclerosis in this cardiovascular high-risk group by visual assessment and quantitative coronary angiography. Methods and results. In 26 patients with chronic renal failure (age, 47+/-11 years; 15 male; duration of dialysis, 23+/-25 months) the severity of CAD and degree of coronary stenoses were assessed in two coronary angiograms after a mean follow-up interval of 30+/-15 months (12-60). Baseline angiography revealed CAD in 13/22 patients (59%). The second angiography was performed as screening procedure prior to renal transplantation (n=20) and/or as follow-up angiography after coronary angioplasty (n=10). Visual assessment showed a progression defined by the development of haemodynamically relevant stenosis of >50% luminal diameter in 13 patients. Quantitative angiographic evaluation was performed in a total of 45 segments showing >25% narrowing at the second angiogram. A progression (>15% luminal reduction) was found in 17 of 45 segments, a new lesion (initial luminal diameter <20%) was detected in nine segments, resulting in progression or new lesion in 16 patients (62%). Patients with or without progression did not differ in age, duration of dialysis treatment, number of cardiovascular risk factors, or serum total cholesterol and fibrinogen levels. After percutaneous transluminal coronary angioplasty (PTCA) a restenosis was seen in seven of 16 primarily successfully dilated segments. After the second angiography, myocardial revascularization was performed in eight patients (1 PTCA, 7 coronary artery bypass graft). CONCLUSIONS: Patients with end-stage renal disease have a high prevalence of CAD. In line with the clinical course, CAD patients on maintenance dialysis undergo rapid angiographic progression of CAD, which results in a high rate of subsequent myocardial revascularizations.  相似文献

BACKGROUND: Renal atherosclerosis is associated with increased cardiovascular mortality. This study aimed to determine the prevalence and predictors of renal artery stenosis (RAS) in patients with coronary artery disease (CAD) and supraaortic arteries (SA) stenosis. METHODS: Renal angiography was performed in 1193 (807 men) consecutive patients referred for coronary or SA angiography. Group I included 296 (136 men, 60.1 +/- 9.5 years) patients with no significant (< 50%) lesion in coronary arteries or SA; group II included 706 (526 men, 62.2 +/- 9.7 years) patients with stenosis > or = 50% within single arterial territory (coronary arteries or SA) and group III included 191 (145 men, 64.9 +/- 8.5 years) patients with stenosis > or = 50% in both territories. RESULTS: Some RAS was found in 55 (18.6%) patients in group I, 250 (35.4%) patients in group II and 115 (60.2%) patients in group III (P < 0.001). The proportion of patients with RAS > or = 50% in groups I, II and III was 3.3, 6.2 and 18.3%, respectively (P < 0.001). RAS prevalence increased with the number of stenosed coronary arteries (38.4% in 1-vessel, 42.1% in 2-vessel, 48.5% in 3-vessel CAD, P < 0.001). Independent predictors of RAS > or = 50% identified by logistic regression analysis were SA stenosis [relative risk (RR) = 3.28, P < 0.001], 2-3-vessel-CAD (RR = 2.04, P = 0.002), creatinine level > or = 1.07 mg/dl (RR = 2.95, P < 0.001), hypertension (RR = 2.97, P = 0.012) and body mass index < 25 kg/m(2) (RR = 1.42, P = 0.169). A calculated score for RAS > or = 50% prediction (based on the regression model) was reliable (coefficient of determination, R = 0.978) and showed a sensitivity of 77.5% and a specificity of 63.9%. CONCLUSIONS: RAS prevalence and severity increases with the number of arterial territories involved and CAD severity. The following independent predictors of RAS > or = 50% were identified: SA involvement, 2-3-vessel-CAD, serum creatinine level and hypertension.  相似文献

目的 探讨冠心病(CAD)合并甲状腺功能低下(甲低)病人的冠状动脉旁路移植术(CABG)围术期外科处理的临床效果.方法 2002年9月至2009年6月,1347例CABG中21例(A组)合并甲低需要甲状腺激素替代治疗,男6例,女15例;平均(60.4±14.2)岁.体外循环下手术4例(包括心脏停跳手术1例),非体外循环下CABG 17例.术前均口服左旋甲状腺素,FT3、FT4、TSH、TT3、TT4明显改善后手术.同期对照20例甲状腺功能正常CABG者(B组),其中4例体外循环下CABG.观察两组术前、术中、术后甲状腺功能指标以及近端吻合时血流动力学指标.结果 围术期应用放射免疫法甲状腺激素水平检测,非体外循环下手术者,A组17例FT3术前及术中水平为[(1.39±0.36)pg/ml对(1.29±0.32)pg/ml]、B组16例为[(2.28±0.36)pg/ml对(2.19±0.34)pg/ml];体外循环下手术者,A组4例FT3术前及术中水平为[(1.53±0.51)pg/ml对(0.85±0.40)pg/ml]、B组4例为[(2.08±0.24)pg/ml对(1.96±0.26)pg/ml].A、B两组术中心排指数[(2.7±1.4)L·min-1·m-2对(2.8±1.5)L·min-1·m-2,P=0.53].A组1例重度甲低病人体外循环下心脏停跳手术后因心脏复跳困难死亡,20例生存者均为心脏不停跳方式手术者,其中17例为非体外循环手术,术后随访2~30个月均有心功能改善,射血分数(EF)由术前0.48±0.17增加至术后0.55±0.21.B组均生存.两组间术中血流动力学、手术预后、住院时间[(12.2±4.7)天对(10.1±3.9)天]、呼吸机辅助[(17.6±9.1)h对(15.1±13.7)h],差异无统计学意义.结论 冠心病合并甲低病人,术前准备充分,采用心脏不停跳手术方式较安全,非体外循环下手术对病人FT3激素水平影响较小;围术期甲状腺素治疗是关键;重度甲低病人体外循环下手术风险大.  相似文献

目的 探讨冠心病患骨密度变化。方法 研究了65例冠心病患脚跟骨骨密度,以80例年龄健康人作对照。结果 发现冠心病患较同年龄组健康对照骨密度降低(P〈0.01),合并骨质疏松率男性为31.9%,女性为39.4%。冠心病患随着病程增加骨密度降于显负相关(P〈0.01),合并骨质疏松的冠心病患心脏的收缩功能较不合并骨质疏松患差,且其心律失常发生率较不合并骨质疏松患高(P〈0.01)。结论  相似文献

目的探讨以双冠状动脉导丝法建立先天性心脏病(简称先心病)介入治疗复杂路径轨道的价值。方法回顾性分析12例应用双冠状动脉导丝法建立介入治疗轨道的先心病患儿的临床资料,其中肺动脉闭锁伴室间隔缺损(PA/VSD)矫治术后肺动脉狭窄5例,法洛四联症(TOF)矫治术后肺动脉狭窄1例,大动脉转位(TGA)术后肺动脉狭窄1例,粗大体肺侧支(MAPCAs)3例,复杂型肺动静脉瘘1例及左冠状动脉回旋支-右心房瘘1例;分析先心病介入治疗复杂路径特点。结果采用双冠状动脉导丝均成功建立输送轨道。对PA/VSD矫治术后肺动脉狭窄、TOF术后肺动脉狭窄、TGA术后肺动脉狭窄患儿成功进行球囊扩张,对MAPCAs、复杂型肺动静脉瘘及左冠状动脉回旋支-右心房瘘患儿均封堵成功,未出现瓣膜损伤、血管损伤、心包填塞及死亡等严重并发症。结论以双冠状动脉导丝法建立先心病介入治疗复杂路径轨道方法安全、有效,可调控性强,能提供足够支撑力通过纡曲段,有利于提高介入治疗成功率,值得临床推广应用。  相似文献

Aim: To determine the incidence of coronary heart disease (CHD) in patients (pts) over 65 years (y) and its relation to common risk factors.Methods: We retrospectively studied 128 hemodialysis (HD) pts (80 M and 48 F), mean age 73 ± 6.5 years, mean time on HD 44.4 ± 26.4 months and BMI 25.4 ± 3 kg/m2. They were evaluated for: age, sex, smoking, diabetes, hypertension, left ventricular hypertrophy, secondary hyperparathyroidism (SHP), inflammation, as evidence by elevated level of hsCRP, hyperhomocysteinemia (HOC), time on HD, fluid overload and adequacy of HD. Forty-eight pts (37%) had CAD diagnosed by coronary angiography in 22 (46%) and 201TL-chloride dipyridamole stress test in 26 (54%).Results: There was a statistically significant correlation between CAD and increasing age (p < 0.0001). The relative risk was significantly increased concerning: (1) male over female pts (RR: 1.95, p < 0.01), (2) diabetic vs. non diabetic pts (RR: 2.09, p < 0.001), (3) patients with SHP over pts with iPTH values < 250 pg/ml (RR: 2.16, p < 0.001), (4) hypertensive vs. non hypertensive pts (RR: 2.26, p = 0.002), (5) smokers vs. non smokers (RR: 1.69, p < 0.05), (6) pts with HOC over pts with normal homocysteine values (RR: 2.09, p < 0.05), (7) pts with increased CRP levels over pts with normal CRP levels (RR: 1.8, p < 0.01), (8) pts undergoing HD for 36 vs. 12 months (RR: 1.71, p = 0.03), (9) between pts with inadequate or adequate HD (RR: 1.73, p = 0.02). No significant correlation existed between CAD incidence and the other risk factors.Conclusions: Coronary heart disease incidence in elderly HD patients increases with age, male sex, diabetes, SHP, hypertension, increased CRP levels, HOC, smoking, time on HD and inadequacy of HD.  相似文献

目的探讨冠心病患者冠状动脉病变的严重程度与冠心病危险因素的关系。方法选择冠心病患者116例为冠心病组,选择冠状动脉样硬化狭窄程度〈50%的患者66例为对照组。分析冠状动脉狭窄程度与冠心病危险因素的关系。结果多因素分析结果显示,LVEF、Ccr与冠状动脉粥样硬化程度呈负相关,而hs-CRP与冠状动脉粥样硬化程度呈正相关(P〈0.05或P〈0.01)。结论冠状动脉狭窄程度与LVEF、Ccr呈负相关,与hs-CRP呈正相关。  相似文献

PurposeTo assess the short-term outcomes in non-dialysis-dependent patients with stage 2 and stage 3 chronic kidney disease (CKD), undergoing off-pump coronary artery bypass (OPCAB). To examine whether there was a difference in mortality between stage 2 and stage 3 CKD patients and whether mortality in diabetics was different compared to non-diabetics. Outcomes would be judged on the basis of possible cardiovascular, pulmonary, infective, neurological and renal complications, duration of stay at the intensive therapy unit (ITU), and overall duration of stay at the hospital. A comparative study of outcomes between stage 2 and stage 3 CKD would be undertaken. Also, given the impact of diabetes mellitus in this patient population, a comparative study of outcomes would be made between diabetics and non-diabetics.MethodsThree hundred fifteen consecutive patients undergoing OPCAB were included in this observational prospective study. Of them, 201 (64%) had stage 3 CKD and 114 (36%) had stage 2 CKD. Nearly half of the study group (49.52%) were diabetics. Data was collected from patients’ files, patient observation charts at the ITU, and patient interviews. Continuous variables were expressed as mean ± standard deviation or median (Q1, Q3) as appropriate and qualitative variables presented with the frequency and corresponding percentage. Comparison between diabetic and non-diabetic patients was performed by the Student’s t test or chi-square test as appropriate. And when assumptions of parametric test failed, then an appropriate non-parametric test was performed. Repeated measures ANOVA (analysis of variance) was used to see the trend of estimated glomerular filtration rate (eGFR) values. Statistical analysis was done by using SPSS version 19.0 (SPSS Inc., Chicago, IL, USA). All p values <0.05 were considered statistically significant.ResultsThe eGFR was measured pre-operatively, on the day following OPCAB and at the time of discharge and the mean eGFR, remained more or less the same throughout (mean of 55.86 ml/min/1.73 m2 pre-operatively, 58.39 ml/min/1.73 m2 on the day following OPCAB and 58.39 ml/min/1.73 m2 at discharge). One patient with stage 2 CKD (0.9%) required hemodialysis post-operatively while 3 patients (1.5%) required the same in the stage 3 CKD group, which was not statistically significant. Nineteen patients (6.03%) required re-intubation for hypoxia in the post-operative period. Post-operative myocardial infarction developed in 12 patients (3.81%). Twenty patients (6.35%) patients needed an intra-aortic balloon pump (IABP) insertion in the peri-operative period owing to hemodynamic compromise. Fifteen patients (4.76%) were re-explored for bleeding following surgery and fifteen patients (4.76%) had a deep sternal wound infection in the post-operative period. New-onset atrial fibrillation was found to be present in 42 stage 3 CKD patients (11%) while it occurred in 21 stage 2 CKD patients (9.6%) (p value = 0.014), which was statistically significant. The mean duration of ITU stay was 84 ± 6.22 h in the stage 2 CKD group and 92.9 ± 8.18 h in the stage 3 CKD group (p value = 0.01), which was statistically significant. Mean duration of ITU stay was 94 ± 10.12 h in the diabetic group while it was 86.7 ± 11.08 h in the non-diabetic group (p value = 0.008) which was statistically significant. Duration of post-operative hospital stay was a mean of 8 ±0.08 days in the diabetic group whereas it was 7 ± 0.04 days in the non-diabetic group (p value = 0.012), which was statistically significant. Surgical mortality was 6 out of 315 patients (1.9%).ConclusionOPCAB is a safe and effective revascularization strategy in patients with stage 2 and stage 3 CKD. Short-term outcomes of OPCAB have been good in the patient population in this study, in terms of both surgical morbidity and mortality. Surgical mortality was 1.9%. New-onset atrial fibrillation was found in eleven patients (9.6%) in the stage 2 CKD group and 42 patients (21%) in the stage 3 CKD group (p value =0.014) which was statistically significant. The results of this study reflect the reno-protective nature of OPCAB. The duration of ITU stay and the post-operative duration of stay at the hospital were found to be significantly more in diabetics than in non-diabetics.  相似文献

Oxalate removal by hemodialysis in end-stage renal disease   总被引:1,自引:0,他引:1  
Because of mounting evidence of precipitation of calcium oxalate in the soft tissues of patients with end-stage renal disease (ESRD) on maintenance hemodialysis, the plasma oxalate concentrations and calculated dialysis removal of oxalate were studied in seven patients without evidence of either primary or absorption hyperoxaluria prior to ESRD. A reversed-phase high-pressure liquid chromatographic method was developed to quantitate serum oxalate. Mean value +/- SE in four healthy controls was 28 +/- 5 mumol/L, and in the seven patients it was 187 +/- 15 mumol/L predialysis and 89 +/- 11 mumol/L postdialysis. Oxalate deposition in the soft tissues of ESRD patients is the consequence of sustained hyperoxalemia. Oxalate removal by dialysis was calculated from the four-hour oxalate clearance. Since the ionic radii of phosphate and oxalate are similar, total oxalate clearance was calculated midpoint of dialysis. Mean oxalate removal/dialysis was 3.01 +/- 0.283 mmol. On a daily basis this value was 1.645 +/- 0.155 mmol, which is about threefold the normal oxalate excretion rate. It is not significantly different from the excretion rate in absorption oxalurias but is less than that in primary hyperoxaluria. Therefore, it is concluded that hyperoxalemia in ESRD results from loss of renal excretion, failure of hemodialysis to remove enough oxalate to maintain a normal serum concentration, and increased intestinal absorption of oxalate and/or increased endogenous production.  相似文献

目的 分析估测肾小球滤过率(eGFR)<60 ml/(min·1.73 m~2)的患者冠状动脉旁路移植术后的长期随访结果.方法 回顾性分析1999年1月至2003年9月3371例冠状动脉旁路移植术患者的临床资料,用 Cockcroft-Gault公式计算eGFR,根据eGFR将患者分为肾功能不全组[eGFR<60 ml/(min·1.73 m~2),n=649]肾功能正常组[eGFR>=60 ml/(min·1.73 m~2),n=2722],比较两组患者的近远期随访结果.结果 肾功能不全组的住院病死率和随访4年病死率分别为2.77%和6.81%,明显高于肾功能正常组.肾功能不全组的其他围手术期并发症及远期不良事件发生率也明 显高于肾功能正常组.多因素 COX 回归分析结果显示,eGFR<60 ml/(rain·1.73 m~2)是冠状动脉旁 路移植术后远期死亡的独立危险因素(HR=1.948,95% CI:1.357-2.797,P<0.01).结论 eGFR <60 ml/(min·1.73 m~2)是冠状动脉旁路移植术的独立危险因素.  相似文献

本文报道我院7例尿毒症合并冠心病患者肾移植的经验。7例中有6例经冠脉造影证实,其中3例狭窄严重(>70%)或临床症状明显者先行经皮冠状动脉成形术或冠状动脉旁道移植术,1例合并高度房室传导阻滞而安装体内永久性起搏器。6例患者移植肾分别存活大于8、10、11、17、34和3个月。我们认为对冠状动脉严重狭窄(>70%)者应先行冠状动脉成形术/旁道移植术;而对冠状动脉狭窄较轻(<70%)或隐性冠心病患者可在充分术前准备条件下行肾移植。  相似文献
