Purpose

To evaluate potential benefits of using multiplanar reconstruction (MPR) in computer-aided detection (CAD) of lung nodules on multidetector computed tomography (MDCT).

Materials and methods

MDCT datasets of 60 patients with suspected lung nodules were retrospectively collected. Using “second-read” CAD, two radiologists (Readers 1 and 2) independently interpreted these datasets for the detection of non-calcified nodules (≥4 mm) with concomitant confidence rating. They did this task twice, first without MPR (using only axial images), and then 4 weeks later with MPR (using also coronal and sagittal MPR images), where the total reading time per dataset, including the time taken to assess the detection results of CAD software (CAD assessment time), was recorded. The total reading time and CAD assessment time without MPR and those with MPR were statistically compared for each reader. The radiologists’ performance for detecting nodules without MPR and the performance with MPR were compared using jackknife free-response receiver operating characteristic (JAFROC) analysis.

Results

Compared to the CAD assessment time without MPR (mean, 69 s and 57 s for Readers 1 and 2), the CAD assessment time with MPR (mean, 46 s and 45 s for Readers 1 and 2) was significantly reduced (P < 0.001). For Reader 1, the total reading time was also significantly shorter in the case with MPR. There was no significant difference between the detection performances without MPR and with MPR.

Conclusion

The use of MPR has the potential to improve the workflow in CAD of lung nodules on MDCT.  相似文献

Objectives

To find the best pairing of first and second reader at highest sensitivity for detecting lung nodules with CT at various dose levels.

Materials and methods

An anthropomorphic lung phantom and artificial lung nodules were used to simulate screening CT-examination at standard dose (100 mAs, 120 kVp) and 8 different low dose levels, using 120, 100 and 80 kVp combined with 100, 50 and 25 mAs. At each dose level 40 phantoms were randomly filled with 75 solid and 25 ground glass nodules (5–12 mm). Two radiologists and 3 different computer aided detection softwares (CAD) were paired to find the highest sensitivity.

Results

Sensitivities at standard dose were 92%, 90%, 84%, 79% and 73% for reader 1, 2, CAD1, CAD2, CAD3, respectively. Combined sensitivity for human readers 1 and 2 improved to 97%, (p1 = 0.063, p2 = 0.016). Highest sensitivities – between 97% and 99.0% – were achieved by combining any radiologist with any CAD at any dose level. Combining any two CADs, sensitivities between 85% and 88% were significantly lower than for radiologists combined with CAD (p < 0.03).

Conclusions

Combination of a human observer with any of the tested CAD systems provide optimal sensitivity for lung nodule detection even at reduced dose at 25 mAs/80 kVp.  相似文献

Objectives

To assess the impact of shifting from a standard double reading plus arbitration protocol to a single reading by experienced radiologists assisted by computer-aided detection (CAD) in a breast cancer screening program.

Methods

This was a prospective study approved by the ethics committee. Data from 21,321 consecutive screening mammograms in incident rounds (2010–2012) were read following a single reading plus CAD protocol and compared with data from 47,462 consecutive screening mammograms in incident rounds (2004–2010) that were interpreted following a double reading plus arbitration protocol. For the single reading, radiologists were selected on the basis of the appraisement of their previous performance.

Results

Period 2010–2012 vs. period 2004–2010: Cancer detection rate (CDR): 6.1‰ (95% confidence interval: 5.1–7.2) vs. 5.25‰; Recall rate (RR): 7.02% (95% confidence interval: 6.7–7.4) vs. 7.24% (selected readers before arbitration) and vs. 3.94 (all readers after arbitration); Predictive positive value of recall: 8.69% vs. 13.32%. Average size of invasive cancers: 14.6 ± 9.5 mm vs. 14.3 ± 9.5 mm. Stage: 0 (22.3/26.1%); I (59.2/50.8%); II (19.2/17.1%); III (3.1/3.3%); IV (0/1.9%). Specialized breast radiologists performed better than general radiologists.

Conclusions

The cancer detection rate of the screening program improved using a single reading protocol by experienced radiologists assisted by CAD, at the cost of a moderate increase of the recall rate mainly related to the lack of arbitration.  相似文献

Objective

Observer performance tests were conducted to compare the effect of presenting digital luminescence radiography (DLR) monitor images in positive mode (“bones black”) or negative mode (“bones white”) in the detectability of subtle lung nodules.

Methods

Five radiologists independently reviewed digital radiographs of 55 patients with either (a) a single, small (6–12 mm), non- calcified peripheral nodule confirmed by chest CT (n = 47) or (b) normal finding (n = 8) confirmed by chest CT, respectively. Mean size of pulmonary nodules was 8.3 mm (range 6–12 mm, median 7 mm). Radiographs were displayed either in standard mode (bones white) or inverse intensity (bones black). A total of 550 observations resulted. For the evaluation ROC analysis was performed using a specialized computer algorithm.

Results

The standard presentation mode showed a sensitivity of 61.7% and a specificity of 72.5%, the inverse presentation mode a sensitivity of 68.1% and a specificity of 75.0%. ROC- analysis showed that the difference between the standard mode (Az- value 0.694) and the inverse mode (Az- value 0.810) was significant in favour of the inverse presentation mode (p = 0.001). This finding was especially observed in more experienced radiologist (Az- values 0.716 vs. 0.909, P < 0.001).

Conclusion

Our study demonstrates a significant advantage of the inverse mode in the detection of small pulmonary nodules compared with the commonly used negative mode when interpreted by more experienced radiologists.

Advance in knowledge

Inverse intensity images (“bones black”) may have some advantages in the detection of small pulmonary nodules in experienced readers when presented on a stand-alone display.  相似文献

Objective

To investigate the potential of MRI for lung nodule detection in a high-risk population in comparison to low-dose CT.

Methods

49 participants (31 men, 18 women, 51–71 years) of the German Lung Cancer Screening and Intervention Trial (LUSI) with a cancer-suspicious lung lesion in CT were examined with non-contrast-enhanced MRI of the lung at 1.5 T. Data were pseudonymized and presented at random order together with 30 datasets (23 in men, 7 in women, 18–64 years) from healthy volunteers. Two radiologists read the data for the presence of nodules. Sensitivity and specificity were calculated. Gold standard was either histology or long-term follow-up. Contrast-to-Noise-Ratio (CNR) was measured for all detected lesions in all MRI sequences.

Results

Average maximum diameter of the lesions was 15 mm. Overall sensitivity and specificity of MRI were 48% (26/54) and 88% (29/33) compared to low-dose CT. Sensitivity of MRI was significantly higher for malignant nodules (78% (12.5/16)) than for benign ones (36% (13.5/38); P = 0.007). There was no statistically significant difference in sensitivity between nodules (benign and malignant) larger or smaller than 10 mm (P = 0.7). Inter observer agreement was 84% (κ = 0.65). Lesion-to-background CNR of T2-weighted single-shot turbo-spin-echo was significantly higher for malignant nodules (89 ± 27) than for benign ones (56 ± 23; P = 0.002).

Conclusion

The sensitivity of MRI for detection of malignant pulmonary nodules in a high-risk population is 78%. Due to its inherent soft tissue contrast, MRI is more sensitive to malignant nodules than to benign ones. MRI may therefore represent a useful test for early detection of lung cancer.  相似文献

Objective

To compare the detectability of simulated pulmonary nodules on chest radiographs between an irradiation side sampling indirect flat-panel detector (ISS-FPD) and computed radiography (CR).

Materials and methods

This study was an observer performance study. Simulated pulmonary nodules of 8 mm in diameter were superimposed on an anthropomorphic chest phantom. Chest radiographs were acquired under 2 exposure levels (4 and 3.2 mAs) with the ISS-FPD and the CR. Six thoracic radiologists evaluated all 40 images (10 patterns × 2 different exposure doses × 2 different systems) for the presence or absence of a lesion over each of 12 defined areas on a 3-megapixel monochrome liquid-crystal display. Receiver operating characteristic (ROC) curves were obtained for observation in predefined 480 areas. A jackknife method was used for statistical analysis. Differences with a P value of <0.05 were considered significant.

Results

The analysis of the observer detection of simulated pulmonary nodules showed larger areas under the ROC curve (AUC) by the ISS-FPD than by the CR. There was a statistically significant difference between the two systems at 3.2 mAs (P = 0.0330).

Conclusion

The ISS-FPD was superior to the CR for the detection of simulated pulmonary nodules at 3.2 mAs.  相似文献

Objectives

To assess the effect of bone suppression imaging on observer performance in detecting lung nodules in chest radiographs.

Materials and methods

Posteroanterior (PA) and lateral digital chest radiographs of 111 (average age 65) patients with a CT proven solitary nodule (median diameter 15 mm), and 189 (average age 63) controls were read by 5 radiologists and 3 residents. Conspicuity of nodules on the radiographs was classified in obvious (n = 32), moderate (n = 32), subtle (n = 29) and very subtle (n = 18). Observers read the PA and lateral chest radiographs without and with an additional PA bone suppressed image (BSI) (ClearRead Bone Suppression 2.4, Riverain Technologies, Ohio) within one reading session. Multi reader multi case (MRMC) receiver operating characteristics (ROC) were used for statistical analysis.

Results

ROC analysis showed improved detection with use of BSI compared to chest radiographs alone (AUC = 0.883 versus 0.855; p = 0.004). Performance also increased at high specificities exceeding 80% (pAUC = 0.136 versus 0.124; p = 0.0007). Operating at a specificity of 90%, sensitivity increased with BSI from 66% to 71% (p = 0.0004). Increase of detection performance was highest for nodules with moderate and subtle conspicuity (p = 0.02; p = 0.03).

Conclusion

Bone suppressed images improve radiologists’ detection performance for pulmonary nodules, especially for those of moderate and subtle conspicuity.  相似文献

Purpose

To determine the frequency and significance of pulmonary nodules detected on thin-section CT in patients with extrapulmonary malignant neoplasms.

Materials and methods

The institutional review board approved this study. This study retrospectively evaluated 308 patients with extrapulmonary carcinomas or sarcomas and had undergone thin-section chest CT (2 mm slice thickness) for staging. Three radiologists identified non-calcified nodules and evaluated the size, the growth and the distance from the nearest pleural surface. The characteristics of the nodules were defined based on the results of either a diagnostic biopsy or nodule growth.

Results

One or more non-calcified pulmonary nodules were detected in 75% of the patients (233/308). One hundred and thirty-seven of these patients had nodules that met the criteria of either benign or malignant nodules. Nodules smaller than 10 mm were more likely to be benign, whereas those 10 mm or greater were more likely to be malignant (22/26, 85%; P < .0001). Most nodules less than 10 mm from the pleura were benign (91%), whereas approximately half of the nodules 10 mm or more away from the pleura were malignant (20/43, 47%; P < .0001). Patients with melanoma, sarcoma, or testicular carcinoma were more likely to have malignant nodules. A multivariable analysis demonstrated the nodule size (P < .0001) and distance from the pleura were predictive of malignancy.

Conclusion

The nodule detection rate on thin-section CT in patients with extrapulmonary malignancy is high. Most of the nodules smaller than 10 mm or less than 10 mm from the pleura are benign.  相似文献

Purpose

To assess the ability of low-dose CT to detect and characterize the most common CT patterns of pulmonary disease.

Methods and materials

Sixty patients with nodules, consolidations or interstitial disease were scanned using a low-dose (128 mm × 0.6 mm, 40 reference mAs, 120 kVp) and standard-dose CT protocol (150 reference mAs, 120 kVp). Two radiologists with 3 and 10 years of thoracic imaging experience searched both exams in consensus for the most commonly observed CT patterns according to the Fleischner Society criteria, which consisted of 46 different subgroups of ground-glass opacities, nodules, interstitial and airspace diseases. The standard of reference was established by consensus of a panel of two experienced chest radiologists (9 and 12 years of experience).

Results

The lung segments (1080) showed 813 nodules, 596 ground-glass opacities, 74 airspace and 575 interstitial diseases and 64 normal segments. In particular, air-space disease and nodules were unaffected by the increase in noise. However, the sensitivity to detect ground-glass opacities, ground-glass nodules and interstitial opacities decreased significantly, from 89% to 77%, 86% to 68% and 91% to 71%, respectively (all p-values < 0.00001). Using iterative reconstruction instead of the applied filtered back projection sensitivity for ground-glass nodules rose to the sensitivity of standard-dose CT in an additional phantom study.

Conclusion

A low-dose CT of 40 mAs/120 kVp is feasible for detecting solid nodules, airspace, airways and pleural disease. For diagnosing pathologies consisting of ground-glass opacities or interstitial opacities, higher tube current or iterative reconstruction is required.  相似文献

Background

Carcinoma of the gall bladder is the fifth most common malignancy of the gastrointestinal tract after colorectal, pancreatic, gastric and esophageal carcinomas.

Aim of the work

To study the role of multidetector computed tomography in the diagnosis and staging of gall bladder carcinoma.

Subjects and methods

This study included twenty-five patients presented to us during the period from June 2011 till May 2012 and were diagnosed radiologically by multidetector computed tomography as carcinoma of the gall bladder and the diagnosis was confirmed histopathologically. All the patients were subjected to ultrasonography followed by multidetector computed tomographic imaging using triphasic examination technique.

Results

Among the twenty-five patients included in this study there were fifteen females and ten males. Their ages ranged from 50 to 70 years old (average 60 years). All the patients were radiologically diagnosed as gall bladder carcinoma based on the multidetector computed tomographic findings. Infiltrating masses were detected in fifteen patients (60%), Intraluminal polypoidal masses were detected in nine patients (36%) while one patient (4%) presented with mural thickening of the gall bladder wall.

Conclusion

We conclude that multidetector CT is the diagnostic tool of choice in the detection and staging of gall bladder carcinoma.  相似文献

Objectives

To assess the feasibility of an objective approach for the evaluation of low-contrast detectability in multidetector computed-tomography (MDCT) by combining a virtual phantom containing simulated lesions with an image quality metric.

Materials and methods

A low-contrast phantom containing hypodense spheric lesions (−20 HU) was scanned on a 64-slice MDCT scanner at 4 different dose levels (25, 50, 100, 200 mAs). In addition, virtual round hypodense low-contrast lesions (20 HU object contrast) based on real CT data were inserted into the lesion-free section of the datasets. The sliding-thin-slab algorithm was applied to the image data with an increasing slice-thickness from 1 to 15 slices. For each dataset containing simulated lesions a lesion-free counterpart was reconstructed and post-processed in the same manner. The low-contrast performance of all datasets containing virtual lesions was determined using a full-reference image quality metric (modified multiscale structural similarity index, MS-SSIM*). The results were validated against a reader-study of the real lesions.

Results

For all dose levels and lesion sizes there was no statistically significant difference between the low-contrast performance as determined by the image quality metric when compared to the reader study (p < 0.05). The intraclass correlation coefficient was 0.72, 0.82, 0.90 and 0.84 for lesion diameters of 4 mm, 5 mm, 8 mm and 10 mm, respectively. The use of the sliding-thin-slab algorithm improves lesion detectability by a factor ranging from 1.15 to 2.69 when compared with the original axial slice (0.625 mm).

Conclusion

The combination of a virtual phantom and a full-reference image quality metric enables a systematic, automated and objective evaluation of low-contrast detectability in MDCT datasets and correlates well with the judgment of human readers.  相似文献

Purpose

To investigate the ultrasonographic (US) characteristics for nonpalpable thyroid nodules 1–2 cm in diameter and to evaluate the guideline for fine needle aspiration (FNA) in terms of US findings.

Materials and methods

Between June 2005 and November 2006, FNA was performed in 919 thyroid nodules by radiologists. Of these nodules, 51 malignant nodules and 72 benign nodules were finally included. All 123 nodules were analyzed by 3 radiologists if there were the following US characteristics: marked hypoechogenecity, hypoechogenecity, isoechogenecity, hyperechogenecity, microcalcification, coarse calcification, rim calcification, spiculated margin, taller-than-wide shape, irregular shape, hypoechoic rim and honeycomb appearance. The maximum diameters of nodules and thickness of hypoechoic rim were measured. US characteristics relevant as predictors were identified using a Chi-square or Fisher's exact test and odds ratio. We compared the diagnostic efficacy of 3 US criteria for FNA indication.

Results

Microcalcification, taller-than-wide shape, marked hypoechogenecity, hypoechogenecity, coarse calcification, irregular shape and spiculated margin were significant characteristics of malignant nodules. Isoechogenecity, hypoechoic rim and honeycomb appearance was significant characteristics of benign nodules. Rim or arc calcification, hyperechogenecity and thickness of hypoechoic rim were insignificant. Among 3 US criteria for FNA indication, NFI showed the highest diagnostic efficacy, 98.0% in sensitivity, 75.0% in specificity and 150.0 in odds ratio.

Conclusion

As for nonpalpable thyroid nodules 1–2 cm in diameter, US characteristics are useful for differentiating between malignant and benign nodules. Malignant US characteristics and honeycombing appearance have significant value in selecting nodules for biopsy and reducing the frequency of the FNA procedure.  相似文献

Objectives

To evaluate the feasibility of diagnosing recurrence of HCC after TACE color-coded iodine CT (CICT) based on arterial phase scans obtained by a dual-energy CT (DECT) scanner.

Methods

A CICT scan was acquired from an iodine map after applying material decomposition of the liver tissue and setting a threshold attenuation level for viable tumors. Two radiologists reviewed both conventional and CICT sets in 31 patients who had a history of TACE for HCC. The performances in detecting local tumor progression (LTP) were evaluated by alternative free-response receiver operating characteristics. The rate of uncertain diagnosis and interobserver agreement of the diagnosis were explored. Additionally, the reading time and radiation dose were also investigated.

Results

The mean figures of merit of the conventional and CICT sets for LTP were 0.818 and 0.847, respectively (p = 0.459). The rate of uncertain diagnosis was significantly decreased in CICT sets (34.5% vs. 0%), and interobserver agreement was improved (k = 0.527 vs. 0.718). On the CICT set, mean reading time was reduced by 49 s and mean radiation dose was also decreased by 18.3% when replacing the non-contrast CT with CICT.

Conclusions

CICT is comparable to conventional liver CT protocol in demonstrating viable HCCs, while it allows a reduction in radiation dose.  相似文献

Purpose

To evaluate the influence of CAD for the evaluation of CT colonography (CTC) datasets by inexperienced readers during the attendance of a dedicated hands-on training course.

Method and materials

Twenty-seven radiologists inexperienced in CTC (11 with no CTC training at all, 16 having previously reviewed no more than 10 CTC cases overall) attended a hands-on training course based on direct teaching on fifteen workstations (four Advantage Windows 4.4 with Colon VCAR software, GE; six CADCOLON, Im3D; five ColonScreen (Toshiba/Voxar) with ColonCAD™ API, Medicsight). During the course, readers were instructed to analyze 26 CTC cases including 38 colonic lesions obtained through low-dose MDCT acquisitions, consisting of 12 polyps sized less than 6 mm, 9 polyps sized between 6 and 10 mm, 12 polyps sized between 11 mm and 30 mm, and 5 colonic masses sized >3 cm. CTC images were reviewed by each reader both in 2D and 3D mode, respectively by direct evaluation of native axial images and MPR reconstructions, and virtual endoscopy or dissected views. Each reader had 15 min time for assessing each dataset without CAD, after which results were compared with those provided by CAD software. Global rater sensitivity for each lesion size before and after CAD usage was compared by means of two-tailed Student's t test, while sensitivity of each single reader before and after CAD usage was assessed with the McNemar test.

Results

For lesions sized <6 mm, global rater sensitivity was 0.1852 ± 0.1656 (mean ± SD) before CAD-assisted reading and 0.2345 ± 0.1761 after CAD (p = 0.0018). For lesions sized 6-9 mm, sensitivity was 0.2870 ± 0.1016 before CAD-assisted reading and 0.3117 ± 0.1099 after CAD (p = 0.0027). For lesions sized 10-30 mm, sensitivity was 0.5308 ± 0.2120 before CAD-assisted reading and 0.5637 ± 0.2133 after CAD (p = 0.0086), while for lesions sized >30 mm, sensitivity before CAD-assisted reading was 0.3556 ± 0.3105 and did not change after CAD usage (p = 1). Sensitivity of each single rater did not significantly differ before and after CAD for any lesion size category (McNemar test, p > 0.05). Specificity was not significantly different before and after CAD for any lesion size (>96% for all size categories).

Conclusion

CAD usage led to increased overall sensitivity of inexperienced readers for all polyps sizes, except for lesions >30 mm, but sensitivity of individual raters was not significantly higher compared with CAD-unassisted reading.  相似文献

Objectives

To investigate the imaging findings of immunoglobulin G4 (IgG4)-related kidney disease (IgG4-KD) on magnetic resonance imaging (MRI) including diffusion-weighted imaging (DWI) and to evaluate the usefulness of DWI in lesion detection.

Methods

This retrospective cohort study included 31 patients with IgG4-KD who underwent MRI covering both kidneys. Two radiologists reviewed in consensus the MR images to determine the distribution pattern (location, laterality, and multiplicity) and the visually assessed signal intensity (hypointense, isointense or hyperintense) of the renal lesions compared to the normal renal parenchyma on each sequence. Per-patient sensitivity for detecting IgG4-KD and the number of detectable lesions were compared in T2-weighted images, DWI, and dynamic contrast-enhanced images.

Results

IgG4-KD typically manifested as bilateral (83.9%), multiple (93.5%), and renal parenchymal (87.1%) nodules appearing isointense (93.5%) on T1-weighted images, hypointense (77.4%) on T2-weighted images, hyperintense (100%) on DWI (b = 1000), and hypointense (83.3%) in the arterial phase and with a progressive enhancement pattern on dynamic contrast-enhanced images. The sensitivity of DWI for detecting IgG4-KD was significantly higher than that of T2-weighted images (100% vs. 77.4%, P = 0.034). The median number of detectable lesions was significantly greater in DWI (= 9) than in T2-weighted images (= 2) and dynamic contrast-enhanced images (= 5) (P ≤ 0.008).

Conclusions

The characteristic MRI findings of IgG4-KD were bilateral, multiple, renal parenchymal nodules with T2 hypointensity, diffusion restriction, and a progressive enhancement pattern. As DWI was useful in the detection of IgG4-KD, adding DWI to conventional MRI for patients suspected of having IgG4-KD may enhance the diagnosis.  相似文献

Purpose

To evaluate the clinical utility and limitations of a computer software program for detecting tumor feeders of hepatocellular carcinoma (HCC) during transarterial chemoembolization (TACE).

Materials and methods

Forty-six patients with 59 HCC nodules underwent nonselective digital subtraction angiography (DSA) and C-arm computed tomography (CT) in the same hepatic artery. C-arm CT data sets were analyzed using the software to identify potential tumor feeders during each TACE session. For DSA analysis, 3 radiologists were independently assigned to identify tumor feeders using the DSA images in a separate session. The sensitivity of the 2 techniques in detecting tumor feeders was compared, with TACE findings as the reference standard. Factors affecting the failure of the software to detect tumor feeders were assessed by univariate and multivariate analyses.

Results

We detected 65 tumor feeders supplying 59 HCC nodules during TACE sessions. The sensitivity of the software to detect tumor feeders was significantly higher than that of the manual assessment using DSA (87.7% vs. 71.8%, P < 0.001). Multivariate analysis showed that a tumor feeder diameter of <1.0 mm (hazard ratio [HR], 56.3; P = 0.003) and lipiodol accumulation adjacent to the tumor (HR, 11.4; P = 0.044) were the significant predictors for failure to detect tumor feeders.

Conclusion

The software analysis was superior to manual assessment with DSA in detecting tumor feeders during TACE for HCC. However, the capability of the software to detect tumor feeders was limited by vessel caliber and by prior lipiodol accumulation to the tumor.  相似文献

Purpose

The purpose of this study is to evaluate the usefulness of a novel computerized method to select automatically the similar chest radiograph for image subtraction in the patients who have no previous chest radiographs and to assist the radiologists’ interpretation by presenting the “similar subtraction image” from different patients.

Materials and methods

Institutional review board approval was obtained, and the requirement for informed patient consent was waived. A large database of approximately 15,000 normal chest radiographs was used for searching similar images of different patients. One hundred images of candidates were selected according to two clinical parameters and similarity of the lung field in the target image. We used the correlation value of chest region in the 100 images for searching the most similar image. The similar subtraction images were obtained by subtracting the similar image selected from the target image. Thirty cases with lung nodules and 30 cases without lung nodules were used for an observer performance test. Four attending radiologists and four radiology residents participated in this observer performance test.

Results

The AUC for all radiologists increased significantly from 0.925 to 0.974 with the CAD (P = .004). When the computer output images were available, the average AUC for the residents was more improved (0.960 vs. 0.890) than for the attending radiologists (0.987 vs. 0.960).

Conclusion

The novel computerized method for lung nodule detection using similar subtraction images from different patients would be useful to detect lung nodules on digital chest radiographs, especially for less experienced readers.  相似文献

Background

To assess the added value of 80 kVp images to weighted average 120 kVp images for detecting hepatocellular carcinomas (HCCs) using dual-source, dual-energy MDCT.

Materials and methods

Forty-one HCCs in 42 patients who underwent liver transplantation (LT) were included. All patients underwent quadruple-phase CT using a 64-row dual-source, dual-energy MDCT with 80 kVp and 140 kVp. For 120 kVp, a linear blending ratio of 0.3 was chosen. Interval reviews for both simulated 120 kVp images without and with pure 80 kVp data were performed independently by two radiologists. They detected HCCs using a 4-point confidence scale. Tumor-to-liver contrast-to-noise ratio (CNR) was calculated and compared between the 80 kVp and simulated 120 kVp images. The additional diagnostic value of 80 kVp images was evaluated by jackknife alternative free-response receiver-operating characteristic (JAFROC) analysis.

Results

There were 41 HCCs on pathology and 37 of the 41 HCCs were depicted on CT scan. The mean CNR of the 37 HCCs in late arterial and portal-phase images was significantly better in the 80 kVp images than in 120 kVp images. The average JAFROC figure of merit, however, was not significantly improved when 80 kVp was added. Furthermore, the number of false-positives was significantly increased in reader 1 when adding 80 kVp data.

Conclusion

The addition of 80 kVp CT images to simulated 120 kVp images did not significantly improve the detection of HCCs despite of the significantly better CNR of 80 kVp images.  相似文献

Purpose

To assess the efficacy of the developed guideline on reading CT images of malignant pleural mesothelioma for improving radiologists’ reading proficiency.

Materials and Methods

Three radiologists independently read the CT films of 22 cases including definite mesothelioma and non-mesothelioma cases at two times before and after studying the malignant pleural mesothelioma CT Guideline. The sensitivity and specificity for mesothelioma were calculated and compared between the 1st and 2nd trials. The kappa statistics was examined for agreement with experts for mesothelioma probability and for mesothelioma features recorded by three radiologists.

Results

After studying the mesothelioma CT Guideline, the sensitivity for mesothelioma shown by the three radiologists at the 2nd trial was 100%, 100% and 80%, which were higher than 80%, 85% and 60% at the 1st trial, respectively. The average kappa for agreement between radiologists and experts on dichotomized mesothelioma probability were 0.69 (good) at the 2nd trial vs. 0.38 (fair) at the 1st trial. The average kappa for the agreement with experts for each of 7 features by three radiologists were 0.52–0.80 at the 2nd trial, which were significantly higher than 0.34–0.58 at the 1st trial (Wilcoxon Signed Rank Test: P < 0.01), and as to five features “unilateral pleural effusion”, “nodular pleural thickening”, “tumoral encasement of lung”, “mediastinal pleural thickening”, and “diminished lung”, they achieved good agreement with average kappa of 0.61–0.80.

Conclusion

The developed mesothelioma CT Guideline was suggested to have substantial effect in improving the radiologists’ proficiency for reading CT images of mesothelioma, and may contribute to accurate diagnosis of mesothelioma.  相似文献

Objectives

To retrospectively investigate the added value of kinetic features measured by computer-aided diagnosis (CAD) for differentiating benign and malignant contralateral breast lesions detected by preoperative MRI in breast cancer patients.

Methods

This study was approved by our institutional review board, and the requirement for informed consent was waived. Fifty-two breast MR images and their CAD kinetic features were obtained for 52 consecutive breast cancer patients with contralateral breast lesions detected by preoperative MRI and confirmed by excision (23 cancers and 29 benign lesions). Three experienced radiologists independently reviewed the MR images without CAD information and assessed probabilities of malignancy. Four weeks later, these probabilities were reanalyzed using stored CAD data. Diagnostic performances and detection rates of delayed washout components were compared between interpretations without and with CAD for each reader.

Results

Use of MR CAD increased detection of washout component by 2.4- to 3.7-fold than visual assessment for enhancing contralateral lesions, which increased sensitivity (91% vs. 87% in reader 1; 96% vs. 74% in reader 2; 91% vs. 70% in reader 3) and decreased specificity, but statistical significance was only found for decreased specificity in one reader (52% vs. 28%, P = 0.039), and overall performance (areas under ROC curves 0.672 vs. 0.616 in reader 1; 0.624 vs. 0.603 in reader 2; 0.706 vs. 0.590 in reader 3) remained unimproved.

Conclusion

Addition of MR CAD increased sensitivity and decreased specificity than radiologist's assessment alone for differentiating benign and malignant contralateral lesions in breast cancer patients and overall performance remained unimproved.  相似文献
