The hypothesis that manual work and exposure to vibration are antecedents to the development of osteoarthrosis was assessed employing a cross sectional study design. The frequency of osteoarthrosis in the acromioclavicular joint was studied in three groups of workers in the construction industry. Two groups were manual workers (54 bricklayers and 55 rock blasters); the third group consisted of 98 foremen. The radiographic appearance of the right and left acromioclavicular joints was classified into one of five grades of osteoarthrosis. A protocol was developed to assess exposure on the basis of job title, years of manual work, total weight lifted during working life, and total hours of exposure to vibrating tools. Odds ratios for job titles (manual worker v foreman) and for years of manual work as indicators of exposure were of similar magnitude of around 2.5. Construction workers who had lifted more than 709 tonnes had an increased risk of developing severe osteoarthrosis of the right acromioclavicular joint, odds ratio: 2.62 (95% confidence interval (95% CI), 1.13-6.06). The odds ratio for the left side was 7.67 (95% CI, 2.76-21.34). In the analysis of vibration exposure, workers who had been highly exposed to vibration had an odds ratio of 1.99 (95% CI, 1.00-3.92) on the right side and 2.20 (95% CI, 1.07-4.56) on the left. This effect almost disappeared after simultaneous adjustment for manual work. Occupational and ergonomic factors, such as the sum of lifted tonnes during working life, job title, and the sum of years of manual work seem to be risk factors for osteoarthrosis of the acromioclavicular joint, whereas vibration alone was a weaker risk factor.  相似文献

Possible connections between carpal tunnel syndrome (CTS) and exposure to vibrating handheld tools, repetitive wrist movements, and heavy manual work were examined in a case-referent study. The cases were 38 men operated on for CTS between 1974 and 1980. For each case, two referents were drawn from among other surgical cases (hospital referents) and two further referents from the population register and telephone directory, respectively (population referents). Thirty four of 38 cases (89%) and 143 of 152 referents (94%) were interviewed by telephone. An increased prevalence of obesity, rheumatoid disease, diabetes, or thyroid disease was observed among the cases but most did not suffer from any of these disorders. CTS was significantly correlated with exposure to vibration from handheld tools and to repetitive wrist movements but showed a weaker correlation with work producing a heavy load on the wrist. A cause-effect relation between CTS and exposures to handheld vibrating tools and to work causing repetitive movements of the wrist seems probable. Some differences between hospital and population referents indicate that a case-referent study of this type could be biased by inappropriate selection of referents.  相似文献

目的通过对某石化厂单纯接触噪声及接触噪声同时伴有高温、硫化氢混杂因素工人的听力测试结果进行分析,了解噪声与其他危害因素联合作用对工人听力的影响,为噪声性听力损失的防治工作提供参考。方法根据某石化厂2007年242例接触噪声及相关有害因素的工人听力检查结果,按工作中接触的职业危害因素差异分成3组:单纯噪声组,噪声合并高温组,噪声合并硫化氢组。测定计算每组各工人的语频和高频听力损失值,对各组的听力损失值进行统计分析。结果3组工人均不同程度地出现噪声性听力损伤。三组工人的听力损失程度存在区别(P0.05),尤其在语言频率的损失上表现明显。噪声合并硫化氢组工人在语言频率的听力损伤程度最严重,噪声合并高温组的工人次之,单纯噪声组工人在语言频率的听力损伤程度最小。结论噪声联合高温或硫化氢作用的听损率和损失程度均较单纯性噪声高,提示噪声联合高温、硫化氢作用于人耳存在联合加强作用。应对噪声作业场所环境温度和硫化氢浓度加以控制,以减少噪声与其他有害因素联合作用对工人听力的影响。  相似文献

Summary Elbow joint disorders were studied in relation to vibration exposure and age in 74 male stone quarry workers who operated mainly chipping hammers and sometimes rock drills. They were examined for range of active motion in elbow extension and flexion, and by means of radiographs of the elbow joint. Effects of age and vibratory tool operation on the elbow joint were statistically estimated using multiple regression analysis. In the analysis of all subjects, including those aged over 60 years, age was significantly related to the range of motion in extension and to radiographic changes in both elbows, and the duration of vibratory tool operation was associated with the range of right elbow flexion. Among subjects under the age of 60 years, duration of vibratory tool operation showed a significant dose-effect relationship to the range of flexion and radiographic changes in the right elbow, but there was no significant relationship with age. The present results suggest that the operation of chipping hammers and rock drills contributes to elbow joint disordes or osteoarthrosis, even when the effect of age is taken into account. Besides vibration exposure, it may be necessary to consider various loads on the elbow joint such as firmly grasping and pressing the tool against stones with the arm bent at about 90°, and carrying stones.  相似文献

Plain chest radiographs from a one in six random sample of the workforce of the asbestos industry at Wittenoom, Western Australia between 1943 and 1966 have been classified for degree of profusion and pleural thickening by two independent observers according to the 1980 UICC-ILO Classification of Radiographs for the pneumoconioses to clarify the effect of degree of radiological abnormality on survival. A total of 1106 subjects were selected. Each subject's age, cumulative exposure to crocidolite, and time since first exposure were determined from employment records, the results of a survey of airborne concentrations of fibres > 5 mu in length conducted in 1966, and an exposure rating by an industrial hygienist and an ex-manager of the mine and mill at Wittenoom. By the end of 1986 193 subjects had died. Conditional logistic regression was used to model the relative risk of death in five separate case-control analyses in which the outcomes were deaths from: (1) all causes, (2) malignant mesothelioma, (3) lung cancer, (4) asbestosis, and (5) other causes excluding cancer and asbestosis. Up to 20 controls per case were randomly chosen from all men of the same age who were not known to have died before the date of death of the index case. After adjustment for exposure and time since first exposure, there were significant and independent effects of radiographic profusion and pleural thickening on all cause mortality. The effect of profusion was largely a result of the effect on mortality from malignant mesothelioma and asbestosis but not lung cancer. The effect of pleural thickening was greatest on mortality from other causes, mainly ischaemic heart disease. This study has shown that degree of radiographic abnormality has an independent effect on mortality from malignant mesothelioma, asbestosis, and all causes even after allowing for the effects of age, degree of exposure, and time since first exposure.  相似文献

Hand-arm vibration syndrome has been reported in the literature to occur following exposure to vibration from the use of many tools, but to date there have been no case reports of its occurrence in workers who have used high-pressure hoses, alone or with other tools. To remedy this, the case histories of nine subjects (two without mixed exposure) examined in the UK and Canada are presented, together with their severity classified according to the Stockholm scales. Attention is drawn to the need to use multiple diagnostic tests to establish the diagnosis and the need to implement vibration isolation and damping methodologies, as and when feasible, with respect to hose nozzles in order to minimize the hazard. The ultimate goal for tool manufacturers, hygienists and engineers should be to reduce workplace vibration levels to meet national and international guidelines and legislation, including UK Health & Safety Executive guidelines and European Economic Community directives. The respective risk levels are presented, together with vibration measurements on hoses used by some of the cases.  相似文献

In this study we quantify economic benefits from projected improvements in worker productivity resulting from the reduction in children's exposure to lead in the United States since 1976. We calculated the decline in blood lead levels (BLLs) from 1976 to 1999 on the basis of nationally representative National Health and Nutrition Examination Survey (NHANES) data collected during 1976 through 1980, 1991 through 1994, and 1999. The decline in mean BLL in 1- to 5-year-old U.S. children from 1976-1980 to 1991-1994 was 12.3 microg/dL, and the estimated decline from 1976 to 1999 was 15.1 microg/dL. We assumed the change in cognitive ability resulting from declines in BLLs, on the basis of published meta-analyses, to be between 0.185 and 0.323 IQ points for each 1 g/dL blood lead concentration. These calculations imply that, because of falling BLLs, U.S. preschool-aged children in the late 1990s had IQs that were, on average, 2.2-4.7 points higher than they would have been if they had the blood lead distribution observed among U.S. preschool-aged children in the late 1970s. We estimated that each IQ point raises worker productivity 1.76-2.38%. With discounted lifetime earnings of $723,300 for each 2-year-old in 2000 dollars, the estimated economic benefit for each year's cohort of 3.8 million 2-year-old children ranges from $110 billion to $319 billion.  相似文献

Carbon monoxide (CO) is an odorless, colorless gas produced from the incomplete combustion of carbon-based fuels such as gasoline or wood. In the United States, CO poisoning causes approximately 500 unintentional deaths each year. Although CO poisonings often have been reported to occur in enclosed and semi-enclosed environments, they can also occur in open-air environments. This report describes two related cases of CO poisoning that occurred in children who were participating in recreational activities on a ski boat. Recreational boaters should be aware of the dangers of open-air CO poisoning, and engineering solutions are needed to reduce the amount of CO in boat exhaust.  相似文献

Exposure to zinc chloride aerosol as a result of detonation of a smoke bomb in an airport disaster drill resulted in participants experiencing upper respiratory tract symptoms which corresponded in frequency of occurrence and intensity with proximity to the site and duration of exposure. Fortunately the exposure was not intense and all symptoms appeared to be transient and receded over several days without any apparent permanent disability. Forced expiratory maneuvers carried out on a substantial fraction of the participants revealed no evidence of lower-airway abnormalities. These findings are consistent with what is known about the size, solubility, and hygroscopic nature of the aerosol. Much of the literature on this subject relates to accidental exposures in enclosed spaces in which exposure is far more intense than occurs in the open air. Alternative methods for generating smoke should be considered in the future when there is potential for intense human exposure.  相似文献

Manual flax processing originated in Egypt in 2 000 BC. In the present study a representative sample of the workers involved in this trade, where flax is processed in small workshops or homes, was examined, and their dust exposure was evaluated. The study showed that workers handling and processing flax are exposed to high concentrations of dust; the levels of dust at hackling and combing are considerably higher than at batting and spinning. Byssinosis prevailed in 22-9% of the examined workers, and 18-4% of them had their forced expiratory volume in one second reduced by more than 10% at the end of the first morning work period (4 hours) of the week. Both the rates and the grades of these syndromes increased with duration of exposure. Smoking appeared to be one of the important contributory factors in the production of byssinosis. The relationship between dust concentration and prevalence of byssinosis seems to be curvilinear.  相似文献

Medical publications contain numerous references to obstacles or disincentives confronting patients who wish to return to work after illness or injury. Current developments of importance to this aspect of medical practice and four recent studies, conducted in Edinburgh, that may contribute to the continuing debate about how best to deal with such problems are reviewed. The studies concern measurement of rehabilitation status, identification of potential to benefit from rehabilitation, medical contributions to occupational assessment of accident victims, and patients' patterns of involvement with medical and vocational rehabilitation services. It is concluded that coordination between services is still a major problem and is likely to remain so if more effective bridging between medical and vocational aspects of rehabilitation is not achieved.  相似文献

Summary Serum creatine kinase (CK) variations during work and subjective assessments of postural discomfort were analysed in 30 workers assigned to three different tasks in a rolling mill. After four days of work, serum CK levels were significantly increased above control levels. No difference was found between the tasks studied. Inter-individual variability in CK response was large: a marked CK increase was only seen in 21 workers, while 5 workers showed a marked CK decrease. The postural discomfort sensations increased with work in each group, with their location in the body being related to the characteristics of each task or workplace. No relationship was found between the postural discomfort scores and CK changes. Results showed that determination of an individual baseline CK level was difficult to achieve in an occupational setting. It is concluded that CK variations cannot reliably be used in the field for detection of individuals exposed to excessive postural constraints. Their use as a screening tool in groups of workers assigned to similar tasks deserves further study.  相似文献
