The purpose of this study was to examine the link between work-related stress and early atherosclerosis as measured by common carotid artery intima-media thickness and focal lesions in the common carotid artery and bifurcation. Four hundred sixty-seven members of an occupational cohort (total N = 573) were examined via questionnaires and B-mode ultrasound. We used multiple linear and logistic models to regress lesion risk and intima-media thickness on work-related stress scores from a questionnaire administered at an 18-month follow-up examination. In an age-adjusted model, the prevalence of carotid lesions among men scoring in the highest stress quintile was 36% compared with 21% among men in the lowest quintile. We also observed an increase in intima-media thickness in the highest quintile relative to the lowest (difference = 0.048 +/- 0.025 mm) among men. Among women, stress was not related to the prevalence of lesions or intima-media thickness. These findings suggest that men with greater work-related stress are at increased risk for atherosclerotic disease. Women in this age group may be protected from such effects, or current work-place questionnaires may not accurately assess stress in women. 相似文献

Occupational stress and burnout have been studied extensively in the human services. It has been suggested that healthcare professionals in particular are at risk of stress owing to the caring nature of their work. Articles related to occupational therapy and work-related stress were reviewed in regard to practice in Australia, Canada, the United Kingdom, the United States and Sweden. Although the empirical literature is relatively weak for occupational therapy, it has been argued that occupational therapists in health care share risk factors with other healthcare professionals. These risk factors include repeated exposure to distress and difficult behaviour, prolonged interventions and uncertain outcome. Issues such as professional status, staffing issues and the nature of the profession have been identified as additional risk factors for occupational therapists. However, empirical studies that enable burnout rates of occupational therapists to be compared with those of related occupational groups suggest that this may not be the case. Occupational therapists may in fact be protected from some stress and burnout factors. Further research is recommended to clarify the nature of stress experienced by occupational therapists and to identify both risk and protective factors characteristic of the profession. 相似文献

BACKGROUND. As a previously unresearched occupational group, the overall aim of this research was to establish the prevalence of stress and associated outcomes in Her Majesty's Coastguard (HMCG). MATERIAL AND METHODS. Data were collected from 282 coastguards by paper questionnaire and compared with general UK working population data from the Bristol Stress and Health at Work Study (SHAW) and the Psychosocial Working Conditions Survey (PWC), 2009. RESULTS. The level of high stress reported in HMCG (11%) was significantly lower than the comparison data (17%). The level of depression found was significantly higher. CONCLUSIONS. HMCG had lower levels of stress than the general UK working population, due, in part to high levels of social support. Data suggests HMCG worthy of study for both negative effects of stress and stress reduction elsewhere. 相似文献

A variety of occupational health hazards can cause employees to experience feelings of victimization and to develop work-related post-traumatic stress responses. The author presents assessment guidelines developed in work with people suffering from toxic exposures to help identify factors that can place employees at high risk for such stress responses. These guidelines can be adapted to evaluate employee reactions to any type of occupational injury. Clinicians also need to move beyond individual treatment approaches to address workplace and policy interventions. 相似文献

We investigated whether work-related psychologic stress--defined as work characterized by both high psychologic demands and limited control over the response to these demands--increases a woman's risk of delivering a preterm, low birthweight infant. We studied 786 employed pregnant women included in the National Longitudinal Survey of Labor Market Experience, Youth Cohort (NLSY), a nationally representative sample of 12,686 young adults. Data concerning work status, job title, and other factors affecting pregnancy outcome were obtained from the NLSY. Assessment of job experience was based on job title, using an established catalogue of occupation characteristics. After accounting for the physical exertion entailed in a job, occupational psychologic stress as measured by job title was not associated with preterm, low birthweight delivery for the sample as a whole (Relative risk = 1.16, 95% confidence interval .45, 2.95). For those women who did not want to remain in the work force, work-related stress increased their risk of experiencing this outcome (RR = 8.1, 95% CI 1.5, 50.2). Personal motivation toward work, as well as the physical effort of work, should be considered in evaluating the impact of a job's psychologic characteristics on pregnancy outcome. 相似文献

Post-traumatic stress disorder (PTSD) may arise after events involving a risk to physical integrity or to life, one’s own or that of others. It is characterized by intrusive symptoms, avoidance behaviors, and hyper-excitability. Outside certain categories (e.g., military and police), the syndrome is rarely described in the occupational setting. We report here five unusual cases of work-related PTSD, diagnosed with an interdisciplinary protocol (occupational health visit, psychiatric interview, psychological counselling and testing): (1) a 51-yr-old woman who had undergone three armed robbery attempts while working in a peripheral post office; (2) a 53-yr-old maintenance workman who had suffered serious burns on the job; (3) a 33-yr-old beauty center receptionist after sexual harassment and stalking by her male employer; (4) a 57-yr-old male psychiatrist assaulted by a psychotic outpatient; (5) a 40-yr-old woman, sales manager in a shoe store, after physical aggression by a thief. All patients required psychiatric help and pharmacological treatment, with difficulty of varying degrees in resuming work. We conclude that PTSD can develop even in professional categories generally considered to be at low risk. In such cases, a correct interdisciplinary diagnostic approach is fundamental for addressing therapy and for medico-legal actions. 相似文献

People working at sea experience stress connected with their specific work conditions, high job responsibility and psychosocial factors, which generate health and psychological problems. The self-evaluation of mental strain related to the work at sea was arranged for Maritime Academy students and for merchant marine officers. Two study groups were examined: 30 students of the Mechanical Faculty of the Maritime Academy (MA) in Gdynia, of the mean age 22.7 years, and 30 seamen, i.e. deck officers and ship's engineers, of the mean age 47.2 years, with a minimum 15-year period of service at sea. For the assessment of work-related stress level, the "Work-Related Stress--Subjective Work Evaluation Questionnaire" was used. Comparing to the officers with a long period of service at sea, the students were more vulnerable to stress at work, and this was attributed to: social relations, physical onerousness, the lack of control, and the lack of support. The programme of psychological training, aimed at supporting the personal competence, was proposed as a preventive intervention. 相似文献

Work-related violence is a major public health problem; however, there is a serious deficiency in the knowledge of risk factors for this problem. The purpose of this case-control study was to identify risk factors for work-related assault injuries among nurses. We used unconditional logistic regression to model the dependence of work-related assault injuries on each exposure of interest and the respective confounders. We found a decreased rate for the presence of security personnel (RR = 0.40; 95% CI = 0.19-0.82). We found increased rates for the following factors: the perception that administrators considered assault to be part of the job (RR = 8.14; 95% CI = 3.76-17.60); having received assault prevention training in the current workplace (RR = 4.64; 95% CI = 2.33-9.23); a high (>5) vs. low (<2) patient/personnel ratio (RR = 2.54; 95% CI = 1.13-5.70); working predominantly with patients with mental illness (RR = 3.5; 95% CI = 1.41-8.85); and working with patients who had more than 1- to 4-week and more than 4-week lengths of stay in the institution vs. <1 day (RR = 8.85; 95% CI = 1.58-49.52 and 4.25; 95% CI = 1.17-15.39, respectively). 相似文献

Analysis of investigation records of the United States Occupational Safety and Health Administration (OSHA) concerning work-related deaths in Standard Industrial Classification (SIC) 1721, construction painting, showed a higher risk of fatal injury than expected from cohort studies including injuries on and off the job. Work-related death rates were 2.3 x 10(-4)/year (ie, three to five times that of general industry). Of the 129 deaths investigated, the largest category was falls (N = 65), followed by electrocution (N = 40) and asphyxiation from solvents or oxygen deficiency (N = 6). Eighteen deaths had other causes. The average OSHA fine for the employer was USD 607.00/fatality. Only 31% of the deaths occurred at firms covered by a union contract. Risk of fatal injury was the highest for small firms with fewer than 10 employees. Cohort mortality studies based on records from unions or large employers probably exclude many small firms and so underestimate the risk of fatal injury to painters. 相似文献

Studies of worker heat stress and strain in aluminum smelters have found that heat exposure likely to exceed the American Conference of Governmental Industrial Hygienists' threshold limit value (TLV) and that the dose-response relationship between heat stress and strain was weak. A heat stress model based on climatic data and a task analysis indicated exposures to heat stress in excess of the TLV during the July/August study period. To study the impact of working above the TLV, heat strain data (i.e., oral temperature, recovery heart rate, average heart rate) were collected. Recovery heart rates indicated high strain most of the time, and oral temperatures after peak demands were above the no-strain threshold of 37.5 degrees C about a quarter of the time, indicating that heat stress had an effect. About 95% of the readings were below 38.0 degrees C, the acute oral temperature threshold for a safe exposure. Average heart rates over 6- and 12-hour intervals were generally below acceptable limits of 120 and 110 bpm, respectively. Oral temperature and average heart rates indicated good control of heat stress exposures. Because recovery heart rates were high, some employees were working near their individual limits. The dose-response relationship for recovery heart rate and oral temperature were examined against the level of heat stress above the TLV. There was no relationship between oral temperature and heat stress level. There was an apparent trend toward higher recovery heart rates with heat stress. The lack of a dose-response relationship may be explained by brief periods of very high wet bulb globe temperatures that drove the time-weighted average up out of proportion to the physiological response. 相似文献
