PURPOSE: To evaluate the effect of platelet-rich plasma (PRP) on autogenous bone graft remodeling during sinus augmentation in a rabbit model. MATERIALS: Twelve New Zealand White rabbits were divided randomly into 3 groups based on their time of sacrifice (2, 4, and 8 weeks). All animals underwent a general anesthetic and harvesting of an autogenous bone graft from the right iliac crest with subsequent bilateral maxillary sinus augmentation. PRP was prepared via standard approved technique by acquiring 21 cc of autogenous blood and performing differential centrifugation to obtain PRP. One cc of PRP was produced that was mixed with bovine topical thrombin and calcium chloride. The left maxillary sinus received only autogenous bone, while the right maxillary sinus received a mixture of PRP mixed with autogenous bone, thus each animal acted as its own control. Equal volumes of bone were inserted in each maxillary sinus. Animals were sacrificed at 2, 4, and 8 weeks and all specimens were harvested for peripheral quantitative computed tomography (pQ-CT), static, and dynamic and histomorphometric analysis. RESULTS: Student t tests were performed comparing bone density via pQ-CT analysis, histomorphometric parameters of total bone area, and bone apposition rate. PRP had no statistically significant effect on bone graft healing in maxillary sinus augmentation when compared using standard pQ-CT, static, and dynamic histologic criteria. CONCLUSION: This study fails to find a direct stimulatory effect of PRP on healing of autogenous bone grafts using pQ-CT, static, and dynamic histomorphometric analyses.  相似文献

Background: This meta‐analysis investigates the effects of platelet‐rich plasma (PRP) on the sinus bone graft. Methods: PubMed, the Cochrane Library, and EMBASE were searched in January 2010. Results: Of 61 articles searched, eight controlled clinical trials, which included a total of 352 sinus bone graft cases in 191 patients, were included in the final analysis. In the fixed‐effects meta‐analysis, the implant survival was not significantly different between two groups in the patient‐based data of four studies (relative risk [RR]: 1.02; 95% confidence interval [CI]: 0.97 to 1.08) and in the implant‐based data of three studies (RR: 1.02; 95% CI: 0.99 to 1.04). The bone formation was significantly greater in the intervention group in the random‐effects model (standardized mean difference [SMD]: 1.30; 95% CI: 0.21 to 2.39) in five studies with significant heterogeneity (I2 = 76.2%). The bone‐to‐implant contact was not significantly different between two groups in the random‐effects model (SMD: 1.02; 95% CI: ?1.65 to 3.70) in two studies with significant heterogeneity (I2 = 82.4%). Conclusion: The present study indicates that there was sufficient evidence to support the use of PRP for bone formation on a sinus bone graft, whereas there was no significant effect on the implant survival and bone‐to‐implant contact.  相似文献

Recently, platelet-rich plasma (PRP) has been proven to be an effective regeneration adjunct when combined with autogenous bone in the reconstruction of mandibular defects. However, little is known about the effect of PRP when combined with a bone allograft in the maxillary sinus. The purpose of this study was to quantitatively evaluate the ability of PRP to enhance bone regeneration in the maxillary sinus of sheep when combined with demineralized freeze-dried bone allograft (DFDBA) and cortical cancellous freeze-dried bone allograft (CCFDBA). Ten sheep were selected for bilateral sinus augmentation DFDBA + CCFDBA + PRP (test) and DFDBA + CCFDBA (control). Five were sacrificed at 3 months and the other 5 at 6 months. Hematology tests were performed for platelet count, and histology slides were obtained for histomorphometric analysis taking 2 measures of interest: total area (square millimeters) and percentage of bone fill. Student t tests showed no significant difference between test and control groups for total area (P > .25) and percentage of bone fill (P > .80) at either 3 or 6 months. The control group showed no statistical difference for total area (P < .095) and percentage of bone fill (P <.60) between 3- and 6-month healing times. The test group, however, showed a significant increase in total area (P <.025) but not in percentage of bone fill (P <.40) for the 2 healing periods. When the treatments were compared for interactions within the animal model, no clear tendency was evident for the test group to perform in relation to the control group regarding total area (r = .766, P < .01). A moderate tendency existed between the percentages of bone filled (r = .824, P < .005). Platelet-rich plasma showed higher platelet count than did the whole blood (2 to 5 times). However, no correlation was found between the log ratio and the bone measures. Within the limitations of this study, PRP failed to enhance or accelerate bone regeneration in the maxillary sinus of sheep when combined with bone allograft.  相似文献

Objectives: Autologous, allogenic, and alloplastic materials for sinus augmentation have specific drawbacks, which has stimulated an ongoing search for new materials and tissue‐engineering constructs. We investigated whether mesenchymal stem cells (MSCs) and platelet‐rich plasma (PRP) seeded on a fluorohydroxyapatite (FH) scaffold can improve bone formation and bone‐to‐implant contact (BIC) in maxillary sinus grafting. Material and Methods: Bilateral sinus augmentation procedures were performed in eight minipigs. MSCs, PRP, and FH scaffold (test site) or FH alone (control site) were grafted in each maxillary sinus. Distal to the osteotomy, one dental implant per sinus was placed in the grafting material through the facial sinus wall. The animals were killed 3 months after grafting, and block sections of the implant sites were harvested and prepared for histomorphometric analysis. Results: After 12 weeks, a significant increase in bone formation occurred in the test sites compared with the control sites (42.51%versus 18.98%; p=0.001). In addition, BIC was significantly greater in the test sites compared with the control sites in the regenerated area (23.71%versus 6.63%; p=0.028). Conclusions: These findings show that sinus augmentation with MSCs–PRP, combined with FH may enhance bone formation and osseointegration of dental implants compared with FH alone in minipigs.  相似文献

Although the lateral wall sinus lift is a predictable clinical procedure to increase vertical bone height resulting in implant success rates comparable to that of native bone, the issue of extended healing periods remains troublesome. Clinicians and researchers have investigated several methods, including addition of growth factors and peptides, to reduce this healing time and enhance bone formation within the subantral environment. Platelet-rich plasma (PRP) is an autologous blood product containing high concentrations of several growth factors and adhesive glycoproteins. The incorporation of PRP into the sinus graft has been proposed as a method to shorten healing time, enhance wound healing, and improve bone quality. This article reviewed pertinent literature assessing the effect of PRP on sinus augmentation. Currently, the literature is conflicting with respect to the adjunctive use of PRP in sinus augmentation. Factors that may contribute to this variability include variable/inappropriate study design, underpowered studies, differing platelet yields, and differing graft materials used. In addition, methods of quantifying bone regeneration and wound healing differ between studies. At present, because of limited scientific evidence, the adjunctive use of PRP in sinus augmentation cannot be recommended. Further prospective clinical trials are urgently needed.  相似文献

Introduction: Sinus grafting is a technique oriented to facilitate implant placement in posterior atrophic maxillae. Several modifications of the original technique and a wide variety of materials have been proposed; most of them associated with implant survival rates. However, the quality of the bone obtained after the application of certain grafting materials has not been fully elucidated yet. The aims of this multicenter study were to analyse histomorphometrical samples obtained 6 months after sinus grafting using a composite graft consisting of anorganic bovine bone (ABB)+ autologous bone (AB), and to compare these samples with maxillary pristine bone biopsies. Material and methods: Ninety maxillary sinus augmentations were performed for delayed implant placement (N=90) in 45 consecutive patients (test group). Bone cores were harvested 6 months after grafting for histomorphometric and ultrastructural study. Control pristine bone biopsies were taken from the posterior maxilla of 10 patients (control). Bone radiographic changes were assessed up to 24 months after implant loading. Results: The total mean values after analysis of test cores revealed a proportion of 46.08±16.6% of vital bone, 42.27±15.1% of non‐mineralized connective tissue, and 37.02±25.1% of the remaining ABB particles. Significant bone remodeling activities were noticed in sinus grafting samples when compared with pristine bone. A statistically significant difference was observed in the number of osteoid lines between two groups, with higher values in the test one (15.1±11.48% vs. 2.5±2.2%, P=0.0005). Ultrastructural study showed that vital trabecular bone was in intimal contact with ABB particles. Radiographic analysis revealed that the higher the proportion of remaining ABB, the lower the total vertical resorption of the graft. Conclusion: Sinus grafting constitutes an excellent model for the study of de novo bone formation patterns and graft consolidation, when a combination of different bone substitutes is applied. The demands of current periodontal practice require that clinicians use varied approaches to resolve poor esthetic appearances. In particular, patients are becoming less and less accepting of exposed roots and lost interdental papillae, especially in the maxillary anterior sextant. This report describes a surgical technique that combines the advantages of a root-coverage procedure using subepithelial connective tissue grafts and an incision design that preserves interdental papillae, coupled with the use of enamel matrix derivative (Emdogain) to maximize the regenerative potential.  相似文献

Platelet-rich plasma is an autologous product that is derived from whole blood through the process of gradient density centrifugation. The proposed value of this product in dental implantology and in bone augmentation procedures lies in the ability to incorporate high concentrations of the growth factors PDGF TGF-beta1, TGF-beta2, and IGF as well as fibrin, into the graft mixture. Research has shown an increased bone maturation rate and improved bone density when this product, or its recombinant growth factors, is added to small bony defects or to larger defects that use autogenous bone as the grafting material. This study tested the efficacy of platelet-rich plasma in three bilateral sinus graft cases with grafts of anorganic bovine bone that contained minimal or no autogenous bone. Histomorphometric analysis indicated that the addition of platelet-rich plasma to the grafts did not make a significant difference either in vital bone production or in interfacial bone contact on the test implants.  相似文献

Subantral sinus augmentation is often necessary to permit placement of endosseous implants. Recent efforts to improve wound healing have focused on autogenous sources of bioactive mediators, such as platelet-rich plasma (PRP), which offer the potential to enhance the biological activity of bone replacement grafts. The purpose of this randomized, single-blinded, controlled study was to compare bone formation after subantral maxillary sinus augmentation with freeze-dried bone allograft (FDBA) plus PRP versus FDBA plus resorbable membrane. Ten patients underwent bilateral maxillary subantral sinus augmentation, with sites within subjects randomized to receive FDBA plus PRP or FDBA plus membrane. Core biopsy specimens were obtained 4.5 to 6 months after the grafting procedure at time of implant placement. Histomorphometric analysis revealed a significantly higher percentage of vital tissue in sinuses after treatment with FDBA and PRP (78.8 +/- 8.3) than with FDBA and membrane (63.0 +/- 15.7). Objective: The combination of anorganic bovine bone (ABB) with platelet‐rich plasma (PRP) has been widely used in bone regeneration procedures although its benefits are still unclear. The purpose of this study was to evaluate whether or not PRP improves the efficacy of ABB in sinus floor augmentation. In addition, we have investigated the effect of residual bone height and tobacco on implant survival in sinus augmentation procedures. Patient and Methods: Eighty‐seven patients recruited for this study underwent 144 sinus floor augmentation procedures using ABB alone or ABB plus PRP (ABB+PRP) in a randomized clinical trial. A total of 286 implants were placed in the augmented bone, and their evolution was followed up for a period of 24 months. In order to investigate on a histological level and any adjunctive effects, we performed an ancillary study in five edentulous patients with a symmetrical severely resorbed maxilla. In these patients, a bilateral sinus augmentation was randomly performed using ABB or ABB+PRP in a split‐mouth design, and after 6 months, bone biopsies were taken from the implant sites for histological and histomorphometric analysis. Results: Overall, 96.2% of ABB and 98.6% of ABB+PRP implant success were obtained during the monitoring period and differences were not found between sites grafted with and without PRP in the 87 patients studied. Densitometry assessments and graft resorption were similar in both experimental groups. However, the histological and histomorphometrical analysis in the five edentulous patients revealed that bone augmentation was significantly higher in sites treated with ABB+PRP (p0.05). Another outcome from our study is that the lack of initial bone support (p0.05) and smoking (p=0.05) appeared to have a negative effect on the treatment success, which was accentuated when both circumstances coincided. Conclusions: PRP is not a determining factor for implant survival in sinus lifting procedures. However, this study revealed that PRP can improve the osteoconductive properties of ABB by increasing the volume of new bone formed. Moreover, in sinus augmentation procedures the implant's survival rate appears to be more influenced by the residual bone height or by tobacco than by the type of bone graft.  相似文献

Sinus bone grafting with autogenous bone is routinely performed to allow placement of endosseous dental implants. Although numerous maxillary sinuses have been successfully grafted, some complications of this procedure have been reported. These include maxillary sinusitis, resorption, infection and possible failure of grafts, loss of implants after 1-stage surgery, and oroantral fistulae. Only one case of postoperative maxillary cyst developing in the graft has been reported in the literature. Here, the authors report a similar case that necessitated grafting of the cyst with autologous iliac bone.  相似文献

Platelet-rich plasma (PRP) has been introduced to the field of oral and maxillofacial surgery for a decade, but its beneficial effects on maxillary sinus augmentation remain unclear. The aim of this study was to evaluate the short- and long-term effects of PRP on osseointegration following single-stage sinus augmentation in a randomized prospective animal study. The maxillary premolars of 24 minipigs were extracted bilaterally and allowed to heal for 2 months. Consecutively all animals underwent bilateral sinus floor elevation using autogenous bone, Biogran as well as a combination of the materials with PRP. Three dental implants (Ankylos, Dentsply Co., Mannheim, Germany) were installed in each sinus simultaneously. Four animals were sacrificed at each period of observation (1, 2, 8 and 12 months). Microradiographic images of the specimens were made for quantitative evaluation of the bone-implant contact (BIC) and light microscopic images were made for qualitative analysis. An increment of the BIC during the observation time could be seen over the observation time in all groups. Autogenous bone exhibited a level of BIC from 25.1 +/- 9.96% at 1 month to 55.1 +/- 13.10% at 12 months; on adding PRP, the BIC ranged from 28.4 +/- 4.64% to 52.5 +/- 17.06%. Biogran with and without PRP led to BIC levels from 16.3 +/- 4.64% to 37.6 +/- 16.40% and 21.7 +/- 4.33% to 46.6 +/- 19.37%, respectively. The results of this study did not show a significantly positive effect of PRP on the BIC following sinus augmentation in both groups.  相似文献

This study reports the successful use of osseointegrated implants to replace posterior teeth combined with the autogenous bone graft technique in the sinus. Preliminary data (1 to 6 years experience) of this one-stage procedure are presented. A total of 33 patients were treated--44 sinus grafts were augmented with bone from the anterior iliac crest. Reconstruction was completed with ceramic fixed partial dentures, and there was a follow-up of 3 to 80 months (mean 40.2 months) after loading. None of the 44 sinus grafts was lost. Of the 121 implants placed, eight failed, resulting in a failure rate of 6.6% and a cumulative failure rate of 6.8%. Of the 44 prostheses placed in the 44 sinuses, one failed, resulting in a prosthesis stability of 97.7%. Complications were encountered in three patients (three sinuses, eight implants); treatments were administered, symptoms subsided, and the implants integrated in two patients (two sinuses, six implants). One patient lost the two implants and the prosthesis, but the graft integrated. No other complications have since occurred. The results compare favorably with previous reports in terms of implant survival and stability. The implant survival rate approached that of implants placed in uncompromised maxillary bone.  相似文献

In this study, we evaluated the effect of platelet-rich plasma (PRP) on bone regeneration in an autogenous bone graft in a canine model. The mandibular premolar teeth had been bilaterally extracted previously, and the ridges had been allowed to heal for 3 months. After this period, continuity resection was performed on both sides of the mandible. One defect (the PRP group) was reconstructed with the original particulate bone mixed with PRP. As a control, the contralateral defect (non-PRP group) was reconstructed with the original particulate bone alone. Biopsies after 6 weeks showed lower levels of bone formation in the PRP group than in the non-PRP group, and fluorescence microscopy revealed a delay in the remodelling of grafts loaded with PRP. These findings suggest that the addition of PRP does not appear to enhance new bone formation in autogenous bone grafts.  相似文献
