Background and aims  Autogenous bone grafting has been used in reconstructing bone defects and in stimulating fracture healing, producing high healing rates in the treatment of infected tibial non-unions. A novel therapeutic alternative is now available known as “vitalised allograft”, a cancellous bone graft procured from femoral heads from living human donors and “vitalised” through the injection of autologous bone marrow. The aim of this study is to summarise the initial results of the fibula and tibia fusion using vitalised cancellous allograft in the treatment of infected tibial non-unions. Patients and methods  We initiated a follow-up of 15 prospective non-randomized patients who received a vitalised allograft in the treatment of infected tibial non-unions in order to produce bony union. The patients included 13 men and 2 women with an average age of 48 years. All patients received a multi-stage surgical approach. After establishing an infection-free environment, allogenic cancellous bone grafting was performed, intended as the final surgical procedure in fibula and tibia fusion. Our follow-up included a clinical and radiographic investigation of the calf in four planes. We analysed union-rate and time required for bony consolidation, as well as recurrent infections, re-fractures, potential graft-resorption, and time needed for graft and bone remodelling. Results  With an average follow-up of 17.1 months, infection control was obtained in 14 of 15 patients, producing an infection arrest rate of 93.3%. Radiographs indicated consolidation in 11 out of 15 cases, with a union rate of 73.3%. Bone union was achieved on average in 17.1 weeks. Conclusions  Fibula and tibia fusion with allogenic cancellous bone grafting, vitalised through autogenic bone marrow, could well become an innovative treatment option for infected tibial non-unions. We need, however, to analyse a higher number of cases over a longer follow-up period in order to assess more accurately recurrent infections and re-fractures.  相似文献

OBJECTIVE: Bone grafting plays an important role in reconstructing infected tibial nonunions. The effects of antibiotic-impregnated bone grafting in infection elimination and bone incorporation was reported in this retrospective study. METHODS: Ninety-six patients treated for infected tibial nonunions were evaluated. These patients were managed with local antibiotic bead therapy and staged antibiotic-impregnated autogenous cancellous bone graft or pure autogenous cancellous bone graft. Patients were randomized to antibiotic-impregnated bone grafting or bone grafting-only groups on the basis of whether the admission date was odd or even. Patients were divided into two groups (antibiotic-impregnated bone grafting group and pure cancellous bone grafting group), according to the procedure used in preparing the bone grafts. The antibiotic-impregnated bone grafting group included 37 men and 9 women whose average age was 36 years (range, 17 to 72 years). The average follow-up period was 4.8 years. By using the Cierny-Mader staging classification of chronic osteomyelitis, 32 of 46 patients (70%) were stage 4A, and 14 of 36 patients (30%) were stage 4B. The pure cancellous bone grafting group included 39 men and 11 women whose average age was 37 years (range, 18 to 72 years). The average follow-up period was 4.5 years (range, 4 to 6 years). Thirty-nine of 50 patients (78%) were stage 4A, and 11 of 50 patients (22%) were stage 4B. The bone defects in both groups ranged from 2 to 4 cm. RESULTS: Wound healing and bony union were achieved in the antibiotic-impregnated bone grafting group. Only two patients had recurrent infections. The infection arrest rate was 95.6%. However, 9 of 50 patients in the pure cancellous bone grafting group had recurrent infections. The infection arrest rate was 82%. The antibiotic-impregnated bone grafting group had significantly superior results (95.6% vs. 82% chi2 test, p < 0.05) in infection elimination than the pure cancellous bone grafting group. CONCLUSION: After 4 to 6 years of follow-up, our results suggest that the use of impregnating antibiotics have no adverse effects on autogenic cancellous bone graft incorporation and could help to eliminate infection effectively.  相似文献

BackgroudThe aim of this study was to evaluate results of osteoperiosteal decortication and autogenous cancellous bone graft combined with a bridge plating technique in atrophic and oligotrophic femoral and tibial diaphyseal nonunion.MethodsWe retrospectively reviewed 31 patients with atrophic or oligotrophic femoral and tibial diaphyseal nonunion treated with osteoperiosteal decortication and autogenous cancellous bone graft between January 2008 and December 2018. Patients with hypertrophic nonunion, infected nonunion, and nonunion treated with autogenous cancellous bone graft alone were excluded. The nonunion site was exposed by using the Judet technique of osteoperiosteal decortication. Nonunion with a lack of stability was stabilized with a new plate using a bridge plating technique or augmented by supplemental fixation with a plate. Nonunion with malalignment was stabilized with a new plate after deformity correction. Autogenous cancellous bone graft was harvested from the posterior iliac crest and placed within the area of decortication. A basic demographic survey was conducted, and the type of existing implants, mechanical stability of the implants, the type of implants used for stabilization, the operation time, the time to bone union, and postoperative complications were investigated.ResultsThe average follow-up period was 33.3 months (range, 8–108 months). The operation time was 207 minutes (range, 100–351 minutes). All but 1 nonunion (96.7%) were healed at an average of 4.2 months (range, 3–8 months). In 1 patient, bone union failed due to implant loosening with absorbed bone graft, and solid union was achieved by an additional surgery for stable fixation with a new plate, osteoperiosteal decortication, and autogenous cancellous bone graft. There were no other major complications such as neurovascular injuries, infection, loss of fixation, and malunion.ConclusionsOsteoperiosteal decortication and autogenous cancellous bone graft combined with stable fixation by bridge plating showed reliable outcomes in atrophic and oligotrophic diaphyseal nonunion. This treatment modality can be effective for treating atrophic and oligotrophic diaphyseal nonunion because it is very helpful stimulating bone union.  相似文献

BackgroundThe nonunion rate for all fractures is about 5–10%. The treatment of nonunion is based on the biologic and mechanical factors contributing to the cause of the nonunion. Debridement and bone grafting are the standard procedures used to treat nonunion of fractures.PurposeWe evaluated the results of endoscopically assisted allogeneic bone grafting performed to treat the nonunion of tibial and femoral fractures.MethodsBetween May 2006 and January 2011, eight patients (two men and six women) with tibial or femoral fracture nonunion were enrolled into our study. The average age of the patients was 35.4 years (range, 24–56 years). All patients underwent endoscopically assisted allogeneic bone graft implantation. We recorded the union status, clinical symptoms, and complications in all patients.ResultsThe average time from the fracture to surgery was 14.4 months (range, 9–22 months). The average follow-up period was 19.1 months (range, 9–28 months). Seven patients achieved bone union and only one patient required additional surgery. The average time between surgery and bone union was 6.4 months (range, 4–8 months). No major complications were reported.ConclusionEndoscopically assisted allogeneic bone grafting is a less invasive and effective treatment for atrophic nonunion of fractures.  相似文献

混合式单臂外固定架骨延长术治疗感染性骨不连   总被引:12,自引:1,他引:11  
目的评价混合式单臂外固定架骨延长术治疗长骨干骺端感染性骨不连的初步临床结果。方法2003年1月至2006年2月采用混合式单臂外固定架固定、局部清创和截骨延长法治疗感染性骨不连21例,男17例,女4例;年龄18~48岁,平均31.5岁。16例为开放骨折内固定术后感染,5例为闭合骨折内固定术后感染。胫骨近端12例,胫骨远端6例,股骨远端3例。12例行骨折端植骨,其中2例二次植骨。结果21例术后随访10~36个月,平均18个月。18例骨折获得初期愈合,3例骨折愈合时仍有局部窦道和渗液,2例骨折尚未完全愈合,1例行截肢术,20例感染得到控制。改良ASAMI骨评定结果为优良13例,中4例,差4例;功能评定结果为优良11例,中6例,差4例。平均骨延长5.6cm,平均愈合时间为11个月。15例发生钉道感染。结论对长骨干骺端感染性骨不连可使用混合式单臂外固定架骨延长术、骨折端开放换药的方法。该方法控制感染好,可自体修复骨缺损,供区畸形发生率低。但固定需采用HA涂层螺钉,严格控制延长速度,一般在1mm/d以内,分次进行延长,手术风险小。  相似文献

目的探讨附加钢板联合断端清理去皮质化植骨治疗髓内钉术后无菌性骨不连的临床疗效。方法回顾性分析2013年1月至2017年8月期间山东大学齐鲁医院急诊外科收治的40例股骨干骨折髓内钉治疗后无菌性骨不连的患者资料。所有患者均采用骨折断端清理、去皮质化、取自体髂骨植骨及附加单皮质锁定钢板治疗。术后随访观察切口愈合、疼痛、关节功能及骨折愈合情况等指标。结果所有患者术后均获得随访,平均随访时间为15(12~18)个月。治疗效果优良率100%(优37例,良3例)。所有患者下地负重行走时主观疼痛症状消失,患肢功能恢复良好,均无切口感染和内固定物疲劳断裂发生。所有患者疼痛视觉模拟评分由术前(6.0±1.1)分降至术后末次随访时(2.0±1.3)分(P<0.05)。末次随访时患者均获得骨性愈合,平均愈合时间8.5(5.5~14.5)个月。经X线或CT证实,均未发现螺钉及钢板松动断裂及感染等并发症。结论附加钢板联合断端清理去皮质化植骨治疗股骨干骨折髓内钉固定术后无菌性骨不连疗效确切、可靠,是一种有效的治疗股骨干无菌性骨不连的手术方式。  相似文献

目的分析骨搬移治疗胫骨感染性骨不连的临床疗效。方法2002年10月至2007年10月采用胫骨感染骨端清创、重建外固定支架加压固定及胫骨干骺端截骨骨搬移治疗胫骨感染性骨不连、纠正肢体短缩畸形45例。结果45例胫骨感染均得到一期控制,软组织缺损创面消灭,4例出现钉道感染;44例骨折愈合,1例骨折端再次形成骨不连;39例双下肢长度基本恢复一致,6例仍有1.5~3cm肢体短缩畸形存在,1例因截骨端提前愈合使肢体未能达到预期长度;术后截骨端骨延长2~9cm,平均延长6cm,无血管及神经损伤的症状出现;术后外固定支架固定6~12个月,平均8个月,所有病例延长区新骨组织形成良好。结论采用骨搬移治疗胫骨感染性骨不连,能达到一次手术兼顾控制骨端感染、消灭皮肤创面、骨折端不需植骨达到骨性愈合及均衡肢体长度的临床治疗作用,是治疗胫骨感染性骨不连的理想方法。  相似文献

目的 探讨应用自体骨植骨再固定的方法治疗大段同种异体骨移植后骨端不愈合的手术方法和疗效.方法 1994年1月至2006年12月北京积水潭医院所行大段同种异体骨移植患者176例中41例不愈合,22例进行了骨接触端的重新植骨治疗,其中15例临床资料完整.再植骨治疗时年龄15~34岁,平均为24岁.初始疾病为:骨肉瘤5例、骨巨细胞瘤4例、皮质旁骨肉瘤2例、血管内皮瘤2例、骨原始神经外胚瘤(PNET)2例.发病部位:股骨下端7例、肱骨中段3例、股骨中段2例、胫骨上端2例、肱骨上端1例.15例患者中,8例单纯不愈合行植骨术,另7例不愈合并发原内固定断裂行植骨和再固定手术.结果 随访时间18~148个月,平均47个月.15例中13例骨愈合,占86.7%.愈合时间5~20个月,平均13个月.其中8例无原内固定失败者均重新愈合,愈合时间平均为14个月.另7例原内固定失败者5例重新愈合,愈合时间平均为12个月,与上述8例比较愈合时间相似.无感染等并发症发生.2例仍不愈合,最终行人工假体置换术.13例骨愈合的患者MSTS评分平均25.1分,8例无原内固定失败者25.4分,5例原内固定失败者24.6分,评分基本相同.结论 应用自体骨植骨再固定的方法治疗大段异体骨移植后骨端不愈合手术简单,并发症少,愈合率高,再手术后功能影响小,较人工假体置换相比有明显优势,对于大段异体骨移植后骨端不愈合患者,应为首选治疗方法.  相似文献

Forty-three patients underwent microvascular free flap reconstruction of severe acute compound tibial fractures between 1980 and 1987. Thirty-three patients with follow-up of one to three years were retrospectively reviewed. Time to bone union took an average of 37 weeks; 30% developed infection necessitating bone removal, and 2 patients required amputation for infected nonunion. Infection rates were lower for the free muscle flaps than for the free skin flaps. No correlation was drawn with the type of fracture, but a delay in wound closure was associated with a higher infection rate. All adult patients required bone grafting, with the least infection seen with the free vascularized bone flaps. Immediate fresh bone grafting at the time of free flap reconstruction, compared with delayed bone grafting, showed no added risk of infection.  相似文献

目的:探讨抗生素骨水泥珠链植入结合外固定架固定治疗感染性骨折不愈合的疗效。,方法:回顾分析22例感染性骨折不愈合患者,男20例,女2例;年龄21±74岁,平均(34.7±11.6)岁。骨折部位:股骨粗隆间3例、股骨干6例、股骨髁上2例、胫骨干9例、肱骨干2例。治疗过程分为3个步骤:先取出内固定物,清创后植入抗生素骨水泥珠链,Ⅰ期闭合伤口;1周后再次清创,更换抗生素骨水泥珠链,行外固定架固定;3个月后取出抗生素骨水泥珠链,取髂骨植骨。结果:随访15~28个月,平均(19.98±4.16)个月。1例胫骨干骨折和1例股骨粗隆问骨折患者分别于植骨术后2、3个月感染复发,其余20例患者感染控制良好。22例患者骨折全部愈合,愈合时间为植骨术后8-24周,平均(15.09±4.13)周。结论:彻底清创、抗生素骨水泥珠链植入结合外固定架固定及Ⅱ期植骨是治疗感染性骨折不愈合简单而有效的方法、  相似文献

Twenty seven patients with humeral diaphyseal nonunion treated with interlocked nailing and autogenous bone grafting were retrospectively assessed. The mean age was 37 years (28 to 59 years). There were 16 women and 11 men. An average of 11 months (8 to 36 months) had elapsed between the initial trauma or treatment and presentation in our clinic. The nonunion site was in the distal third of the humerus in 18 patients (66%) and in the middle third in 9 (34%). Seventeen (73%) had hypertrophic and 10 (37%) atrophic nonunion. Patients were treated with locked intramedullary nailing and autogenous cancellous bone grafting. The mean follow-up was 42 months (28 to 62 months), and the mean time to union was 4.8 months (2.5 to 11 months). Three patients developed superficial infection. Union was achieved in all cases but one, in which there was multiple nerve injury. According to Steward and Hundley's scoring, results were good in 24 patients, fair in 2, and poor in one. Interlocked nailing and autologous bone grafting has a good rate of union provided nails of appropriate diameter are used and distal and proximal locking is performed correctly. We think that its low rate of infection, low risk of injury to the radial nerve, and low requirement for soft tissue dissection make it a suitable choice in the treatment of nonunions of the humeral diaphysis.  相似文献

The data presented in this article support the view that first saucerizing the infected bone and then grafting give good results. Immediate bone grafting, that is, bone grafting within a few days or weeks of saucerization, will fail when insufficient care has been employed in preparing the area for bone grafting. Using finely divided cancellous bone combined with a suitable fixator is important. Treatment of a nonunion can be successful without bone grafting if, after complete saucerization, stability is achieved by a brace, cast, or external fixator. Failure in all categories of patients is, in the main, due to failure to control the infection or inadequate stabilization. Posterolateral bone grafting may be an excellent alternative and was used in 20 per cent of the patients with tibial nonunion and a substantial loss of tibial shaft.  相似文献

We present our therapeutic strategy for the treatment of type IIIB open tibial fractures. It involves emergency internal stabilisation of the bone by locked intra-medullary nailing when appropriate and skin cover using either a pedicled or free muscle flap. Where there is bone loss, a cancellous iliac graft is performed at the same time. Eighteen cases of type IIIB open tibial fractures treated between 1986 and 1995 were analysed. There were 17 men and 1 woman; the average age was 35 years. Each of the 18 patients underwent wound debridement as a primary emergency procedure with no secondary reoperation. Bone fixation was performed by locked intra-medullary nailing (AO nail, How Medica) 6-10 h after trauma. A primary cancellous iliac bone graft was performed in three cases. Cover was applied immediately after nailing (muscular pedicle flaps in 12 cases, muscular free flaps in 6 cases). Local flap cover led to two failures: both these fractures were followed by postoperative complications. The 6 free muscle flaps were successful. The average time to bone union was 6.5 months (range: 3-18.5 months) according to clinical criteria and 9 months (range: 4-27 months) according to radiological criteria. Out of the 18 fractures, 13 were primarily united (72.2% of cases); 3 involved osteitis and 2 nonunion. Sixteen patients were examined again with a mean follow-up of 4.8 years (range: 1-11 years). Six moderate malunions occurred; none needed surgical reoperation. Ankle motion was normal in 7 cases and reduced to below 50% in 9 cases when compared with the healthy ankle. Thirteen patients resumed their previous professional activities. This surgical strategy reduces bone union time, the number of operations and the time spent in hospital; it improves functional results.  相似文献

目的分析胫骨骨折术后骨不连发生的原因与治疗效果。方法自1989年1月~2003年12月共收治胫骨骨折术后骨不连30例,其中22例采用有限接触钢板内固定 自体髂骨植骨术,4例采用外固定支架固定,4例采用带锁髓内钉内固定 植骨固定。结果术后平均随访14个月(8月~3年),未发生骨髓炎等并发症。经X线片检查,骨折均骨性愈合,其中6~12月愈合17例,12~18个月愈合11例,大于18个月愈合2例。结论骨折区段的血供、内固定材料、操作技术三大综合因素是造成骨不连的主要原因。自体髂骨植骨加可靠内固定是治疗的重要方法。  相似文献

The purpose of this study was to demonstrate the effectiveness of the Ilizarov method and circular external fixator in order to eradicate the infection and restore bone union, limb anatomy and functionality in cases with infected nonunion of the tibia following intramedullary nailing.During 7 years nine patients suffering from infected nonunion of the tibia after intramedullary nailing were treated in our department. The series comprised seven men and two women with an average age of 39.7 years (range 21-75 years). The patients had previously undergone an average of 4.8 operations (range 3-6 operations). Active purulent bone infection occurred in all nine patients. Bone defect was present in all patients with a mean size of 5 cm (range 2-12 cm). In three cases with bone defect less than 2 cm, monofocal compression osteosynthesis technique was used. In the rest cases where bone defect exceeded 2 cm, bifocal consecutive distraction-compression osteosynthesis technique was applied. Three patients required a local gastrocnemius flap. The mean follow-up period was 26.6 months (range 13-42 months). Results were evaluated using Paley's functional and radiological scoring system.Bone union was achieved in all nine patients without recurrence of infection during the follow-up period. Bone results were graded as excellent in five cases and good in the rest four cases. Functional results were graded as excellent in three cases, good in four and fare in two cases. Mean external fixation time was 187.4 days (range 89-412 days) and mean lengthening index was 32 days/cm (range 27-39 days/cm). Complications observed included eight grade II pin tract infections, axial deformity at the lengthening site in two cases and at the nonunion site in another two cases. Ankle joint stiffness was detected in five cases.The Ilizarov method may be an effective method in infected nonunions of the tibia following intramedullary nailing.  相似文献

OBJECTIVE: To investigate the use of the Ilizarov circular fixator and nail retention in treating diaphyseal nonunion following previous intramedullary nailing. DESIGN: Retrospectively reviewed, consecutive series. Mean duration of follow-up after achieving bone union: 19.2 months (range 6 to 33 months). SETTING: A tertiary referral center for nonunion surgery. PATIENTS: Nine patients (two femoral, three tibial, and four humeral nonunions) were included in the study. All patients were referred from other centers after failure to achieve union with intramedullary nailing. Patients who had nonunion with other fixation devices in situ, those with active infection and nonunion following nonoperative treatment, were excluded from the study. The patients had undergone an average of 2.4 operations (range 1 to 5 operations) before application of the Ilizarov fixator. All patients completed the study. INTERVENTION: The circular fixator was used to compress the nonunion site from without, retaining the intramedullary nail in each case. We excluded a patient who had his nonunion site explored followed by bone excision and transport. The mean duration of fixator treatment was 6.2 months (3 to 11 months). MAIN OUTCOME MEASUREMENTS: Clinical and x-ray evidence of bone union, infection, residual deformity, shortening, and assessment of functional outcome. RESULTS: Bone union was achieved in all nine patients using the circular fixator over the nail. The bone results were graded as six excellent, one good, and two fair. All patients reported a reduction in pain and satisfaction with their final outcome. CONCLUSIONS: There is a role for the use of the Ilizarov fixator with nail retention in resistant long bone diaphyseal nonunion in carefully selected patients. This method can achieve high union rates where other treatment methods have failed.  相似文献

目的 探讨经皮自体红骨髓植入治疗胫骨中下段骨折延迟愈合及不愈合的疗效.方法 对胫骨中下段骨折延迟愈合18例、骨折不愈合5例患者,将自体红骨髓植入至骨折断端,定期摄X线片复查.结果 23例均获得随访,时间4~16个月,患者骨折均愈合,愈合时间3~12个月,未见骨折畸形愈合、异位骨化及感染等并发症.结论 经皮自体红骨髓植入治疗胫骨中下段骨折延迟愈合及不愈合是一种有效、安全、简易、实用的方法.  相似文献

目的探讨髓外固定股骨近端接骨板(PFP)及锁定加压接骨板(LCP)辅助双植骨头钉孔道内松质骨、异体骨混合打压植骨及骨不连断端结构植骨治疗股骨转子下骨折髓内固定术后骨不连的手术技巧和临床疗效。方法回顾性分析2018年1月至2018年12月期间西安交通大学附属红会医院创伤骨科下肢病区收治且完整随访的21例股骨转子下骨折髓内固定术后骨不连患者资料。男15例,女6例;平均年龄为52.3岁(27~65岁)。骨不连类型:萎缩型18例,缺血型3例。患者骨不连手术与骨折初次手术的时间间隔平均为10.3个月(9~13个月),骨折手术均采用髓内固定(PFNA):闭合复位10例,切开复位11例(其中钢丝捆扎8例)。骨不连均采用髓外固定(PFP+LCP)辅助双植骨(头钉孔道内松质骨、异体骨混合打压植骨+骨不连断端结构植骨)治疗。术后12个月采用Harris评分评估髋关节功能。结果21例患者术后获平均11.4个月(10~12个月)随访。所有患者术后骨不连均获愈合,平均愈合时间为5.2个月(4~6个月)。1例患者术后出现皮下血肿,再次行清创术。术后12个月Harris髋关节评分平均为85.7分(84~92分)。结论PFP接骨板能够有效纠正内翻畸形并坚强固定,LCP接骨板+皮质骨结构植骨能够提供内侧力学支撑,松质骨+异体骨混合打压植骨能够有效增加头钉孔道内骨量,增加近端螺钉的把持力。该方法可以极大地提高骨折愈合率,减少并发症的发生,且能获得较好的髋关节功能。  相似文献
