依据卫生服务"城乡一体化"的发展目标和城市"社区一医院"二级网络的发展趋势,农村卫生系统虽保持"县一乡镇一村"三级网络模式,但分工更加明确,其中,乡镇卫生院主要承担农村"社区"的职能.对"城乡一体化"模式下乡镇卫生院的功能定位进行了界定,同时分析了现阶段乡镇卫生院运行过程中存在的主要问题,并提出相应的措施.     

医院绩效评价指标体系研究

通过采用文献法、Delphi专家咨询法,确定了医院绩效评价指标及其权重,并规定了指标的内涵以及评价方法,从而构建了医院绩效评价指标体系.该体系既可作为医院自我评价的参照系统,又可为卫生行政部门评价检查医院提供参考指标.     

公共卫生机构的绩效预算管理——以县疾病预防控制中心为例

从绩效预算管理的含义入手,分析在我国公共卫生机构实施绩效预算管理的重要意义,根据绩效预算管理逻辑框架,以县疾病预防控制中心为例提出在公共卫生机构实施绩效预算的方法,并提出在我国公共卫生机构实施绩效预算可能遇到的困难,为改革公共卫生财政资金管理机制提供依据。     

A Framework for Contracted Health IT Performance Assessments

Limited research exists which aids in structuring health IT contracts in an era of performance-based payments. We provide an assessment of common approaches to contracting and measuring of performance in practice. We conducted a review of existing literature and compliment this approach with a survey of healthcare professionals directly involved with health IT systems to further understand and classify current approaches. We identified architypes for structuring healthcare IT performance contracts to include: (1) internal operations, (2) external evaluation and (3) joint agreement for the delivery of value-based care.     

Random Output and Hospital Performance

Many countries are under pressure to reform health care financing and delivery. Hospital care is one part of the health system that is under scrutiny. Private management initiatives are a possible way to increase efficiency in health care delivery. This motivates the interest in developing methodologies to assess hospital performance, recognizing hospitals as a different sort of firm. We present a simple way to describe hospital production: hospital output as a change in the distribution of survival probabilities. This output definition allows us to separate hospital production from patients' characteristics. The notion of better performance has a precise meaning: (first-order) stochastic dominance of a distribution of survival probabilities over another distribution. As an illustration, we compare, for an important DRG, private and public management and find that private management performs better, mainly in the range of high-survival probabilities. The measured performance difference cannot be attributed to input prices or to economies of scale and/or scope. It reflects pure technological and organisational differences.     

灰色聚类在医院综合评价中的应用研究

本文应用灰色聚类方法对医院进行综合评价,并对标定聚类权进行了研究。要根据参加综合评价的指标作用情况不同,采用不同的计算方法计算标定聚类权,这是评价结果是否科学的关键问题。一般说来,不同类型指标在综合评价中作用大小是不一样的,为此,本文认为采用加权法计算标定聚类权,使判断结果更科学。

在公平与效率两方面对市场主导型、国家福利型、公共合同型和公私功能互补型4类医药卫生体制的代表国家进行分析与比较的基础上,对4类医药卫生体制的改革效果进行分析与比较,进而找出其对中国医药卫生体制改革的启示。
