Ovarian carcinoma in patients with endometriosis: MR imaging findings   总被引:3,自引:0,他引:3  
OBJECTIVE: Various types of malignancy can develop in patients with endometriosis. Enhancing mural nodules have been reported as an imaging characteristic of malignant transformations. We evaluated contrast-enhanced MR imaging to determine the optimum sequence to reveal mural nodules and other characteristics of malignant transformations. MATERIALS AND METHODS: We examined 10 patients with pathologically proven ovarian adenocarcinoma in endometriosis and 10 patients (the control group) with ovarian endometrioma suggestive of malignant transformation on the basis of sonographic findings. We analyzed the size and nature of the endometriomas in each patient. We compared four types of contrast-enhanced MR imaging to determine which sequence best revealed mural nodules. RESULTS: In the malignant and control groups, 80% of the cysts with findings suggestive of malignant transformation showed unilateral disease or larger endometrial cysts on the suggestive side than on the contralateral side. High signal intensity on T1-weighted images and low signal intensity on T2-weighted images relative to the myometrium were observed only in two of 10 malignant endometrial cysts and in all control cysts. All malignant endometriomas had small mural nodules with low signal intensity on T1-weighted contrast-enhanced images. Only three benign endometriomas had mural nodules and none of them enhanced. The enhancement of mural nodules was easily seen on dynamic subtraction imaging. CONCLUSION: On the basis of our findings, endometrial cysts with malignant transformation rarely show low signal intensity on T2-weighted images and usually have enhancing mural nodules. Because the enhancement of mural nodules is often difficult to evaluate on conventional T1-weighted images, dynamic subtraction imaging can be valuable.  相似文献

Magnetic resonance (MR) imaging for detection and characterization of ovarian masses was assessed in 33 patients with a total of 60 lesions. Lesions were characterized prospectively as benign or malignant by using T2-weighted MR images and unenhanced and gadolinium-enhanced T1-weighted MR images. MR imaging findings were compared with results of surgical laparotomy performed for staging of lesions. When malignancy was suspected, staging with MR imaging was performed. MR imaging demonstrated 57 of 60 (95%) surgically proved ovarian masses (34 of 36 were benign, 23 of 24 were malignant). Five significant primary criteria and four ancillary criteria for malignancy were established. For all MR pulse sequences combined, characterization of either type of lesion was correct in 84% of cases (48 of 57) when the five primary criteria were used and 95% (54 of 57) were correct when the four ancillary criteria were added. With gadolinium-enhanced images, correct characterization of malignant lesions increased from 56% to 78% with use of the five primary criteria and from 83% to 100% with use of both sets of criteria. Malignancies were correctly staged with MR imaging in 12 of 16 patients. Staging accuracy was 63% with unenhanced images and 75% with the addition of enhanced images.  相似文献

OBJECTIVE: The aim of our study was to evaluate the accuracy of MR imaging in the detection and characterization of adnexal mass lesions and to determine which imaging features are predictive of malignancy. SUBJECTS AND METHODS: We prospectively performed MR imaging in 104 patients (age range, 19-87 years; mean age, 50 years) with clinically or sonographically detected complex adnexal masses. We used a 1.5-T unit to perform T1-, T2-, and fat-suppressed T1-weighted sequences before and after IV injection of gadolinium. The adnexal lesions were examined for several features including size, shape, character (solid-cystic), vegetation, signal intensity, and enhancement. Secondary signs such as ascites, peritoneal disease, and lymphadenopathy were noted. We compared the imaging features with the surgical and pathologic findings. Multiple logistic regression analysis was performed on all MR imaging features. RESULTS: A total of 163 lesions--94 benign and 69 malignant lesions--were examined. On MR imaging, 95% (155/163) of the lesions were detected. The overall accuracy for the diagnosis of malignancy was 91%. On univariate analysis, the imaging features associated with malignancy were a solid-cystic lesion, irregularity, and vegetation on the wall and septum in a cystic lesion, the large size of the lesion, an early enhancement on dynamic contrast-enhanced MR images, and the presence of ascites, peritoneal disease, or adenopathy. On multiple logistic regression analysis, ascites and vegetation in a cystic lesion were the factors most significantly indicative of malignancy. CONCLUSION: MR imaging is highly accurate in the characterization of adnexal mass lesions, and the best predictors of malignancy are vegetation in a cystic lesion and ascites.  相似文献

The purpose of this study was to describe the magnetic resonance (MR) imaging features of biliary hamartomas on T1- and T2-weighted and gadolinium-enhanced sequences, and to correlate these findings with histopathology. MR imaging findings in four patients with pathologically proved biliary hamartomas are described. In all patients, MR imaging sequences, including T1- and T2-weighted and early and late gadolinium-enhanced images, were retrospectively evaluated for the size, morphology, signal intensity, and enhancement pattern of the lesions. Correlation was made between the MR imaging findings and histopathology. Biliary hamartomas ranged in diameter from 0.5 to 1.5 cm. Lesions were solitary in one patient and numerous in three patients. In all patients, the lesions were low signal on T1-weighted images and high signal and well-defined on T2-weighted images and demonstrated thin rim enhancement on early post-gadolinium images that persisted on late post-gadolinium images. No appreciable central enhancement of the lesions was observed. At histopathology, the lesions were composed of cystic spaces and fibrous stroma. Lesions showed compressed liver parenchyma surrounding the lesions (three cases) and inflammatory cell infiltrate (one case), which correlated with the rim enhancement on the gadolinium-enhanced MR images. Most of the biliary hamartomas in our small series were less than 1 cm in diameter and of high signal intensity on T2-weighted images, and had a thin rim of enhancement on early and late post-gadolinium images. The imaging features were explainable by the underlying histopathology. In patients with known malignancy, caution should be exercised not to misinterpret these lesions as metastases due to the presence of thin rim enhancement. J. Magn. PURPOSE: To retrospectively compare quantitative and qualitative methods of assessing magnetic resonance (MR) imaging contrast enhancement as the basis for diagnosing renal malignancy. MATERIALS AND METHODS: MR imaging was performed by using a gadolinium-enhanced breath-hold fat-suppressed three-dimensional T1-weighted gradient-echo sequence in 71 patients (48 men and 23 women; mean age, 62 years; age range, 26-87 years) with 93 renal lesions for which pathologic correlation was available. For quantitative measurements of enhancement, the relative increase in signal intensity values was measured by one investigator with manually defined regions of interest, and the threshold of an increase of 15% or greater was used to distinguish malignant from benign masses. For qualitative assessment, two investigators independently reviewed the subtracted images of all lesions and subjectively determined whether enhancement was present or absent. The sensitivity, specificity, and positive and negative predictive values for each method were calculated and compared. Mean (+/- standard deviation) and median values of relative enhancement were also calculated for benign and malignant lesions. RESULTS: At pathologic analysis, 74 (80%) of the 93 lesions were malignant, and 19 (20%)-including seven oncocytomas-were benign. For diagnosing malignancy based on enhancement alone, sensitivity and specificity, respectively, were 95% (70 of 74 lesions) and 53% (10 of 19 lesions) at quantitative analysis and 99% (73 of 74 lesions) and 58% (11 of 19 lesions) at qualitative analysis. All seven oncocytomas were considered to be malignant with both methods. When the oncocytomas were excluded, specificities increased to 83% (10 of 12 lesions) and 92% (11 of 12 lesions) for the quantitative and qualitative evaluations, respectively. Three of the four malignant lesions incorrectly characterized as benign at quantitative assessment were hyperintense on unenhanced MR images; all were diagnosed correctly at qualitative evaluation. CONCLUSION: Image subtraction enables accurate assessment of renal tumor enhancement, particularly in the setting of masses that are hyperintense on unenhanced MR images.  相似文献

The MR and CT appearances of two cases of splenic hamartoma are presented. Computed tomography showed a well demarcated low-density mass without calcification. Dense spreading enhancement was seen in one case on dynamic CT, and prolonged enhancement was noted in both. The masses were demonstrated as areas of isointensity on T1-weighted MR images and of high intensity on T2-weighted images. On gadolinium-enhanced T1-weighted images they were shown as areas of high intensity. Prolonged enhancement on postcontrast CT and MR imaging was a useful finding in differentiation of splenic hamartoma from malignant lesions of the spleen, especially from nodular lesions of malignant lymphoma.  相似文献

OBJECTIVE: Prior studies have shown that renal MR contrast enhancement improves the efficacy of mass and proximal vascular evaluation. This study assessed the usefulness of different sequences for characterization of masses that appeared suspicious on CT and for prediction of their potential for malignancy. SUBJECTS AND METHODS: In a prospective manner 32 patients (age range, 26-78 years: average age, 54 years), each with at least one suspicious mass on CT, were examined with MR imaging. The following sequences were performed: conventional spin-echo with and without fat saturation, fast spin-echo, and dynamic gadopentetate dimeglumine-enhanced infusion using a 1.5-T superconducting magnet. Results were analyzed and compared with pathologic results after resection. RESULTS: A total of 65 renal masses of average size 2.6 cm (range, 1-10 cm) were detected with dynamic MR imaging. Seventeen of the 65 masses were malignant. Of the 17 malignant masses, three did not enhance on dynamic MR imaging (because of hemorrhage). Sixteen of the 17 malignant masses were heterogeneous on T2-weighted images. Three enhancing masses contained fat and all were angiomyolipomas. Thirty-five of the 65 masses (four with hemorrhage) did not show enhancement, all of which were homogeneous on T2-weighted images and were proven to be cysts. Five masses resulted from infections and had heterogeneous T2 appearance. The remaining masses were three hematomas with hemorrhage, one column of Bertin, and one aneurysm. CONCLUSION: Renal masses that are interpreted as suspicious on CT may lack MR enhancement because of hemorrhage effects; heterogeneity of their T2 appearance is thus critical in differentiating malignancy from benign disease. Odds-ratio calculations give an adjusted estimate of a 3.36-fold increase (95% confidence interval, 1.8-6.27) in the likelihood of malignancy when masses are heterogeneous on T2-weighted images and a 29-fold increase (95% confidence interval, 3.67-241.8) for predicting malignancy when enhancement is present.  相似文献

The purpose of our study was to review the magnetic resonance (MR) imaging findings and pathological characteristics of benign and malignant cystic circumscribed breast masses. This article reviews the MR imaging findings of cystic circumscribed breast masses. Intracystic papillomas typically show a single mural nodule with a washout enhancement pattern. Intracystic papillary carcinomas show multiple mural nodules or grow as a solid mass that obliterates the cystic space with a washout enhancement pattern. Despite the MR imaging findings, however, many circumscribed breast masses are impossible to differentiate without performing a biopsy.  相似文献

PURPOSE: To compare computed tomography (CT) and magnetic resonance (MR) imaging in the evaluation of cystic renal masses by using the Bosniak classification system. MATERIALS AND METHODS: Images of 69 renal masses in 59 patients (38 men, 21 women; mean age, 60.4 years; range, 30-86 years), who had undergone both CT and MR imaging examinations within 1 year (average, 60.5 days; range, 0-356 days), were retrospectively analyzed by two radiologists in consensus. For each lesion, images were compared for thickness of wall and septa, number of septa, and presence of enhancement. Each mass was categorized (Bosniak classification) first on CT images and then on MR images, and results were compared. Pathologic correlation was available in 25 lesions. RESULTS: On CT images, there were 15 category I, 16 category II, 10 category IIF, 19 category III, and nine category IV lesions. Findings on CT and MR images were similar in 56 (81%) lesions; in 13 (19%) lesions, there were differences. In eight (12%) lesions, MR imaging depicted more septa than did CT, which resulted in an upgrade of the classification at MR imaging in two cases. In seven (10%) lesions, MR imaging depicted increased wall and/or septa thickness compared with CT, resulting in a classification upgrade in six cases. Three lesions had both increased numbers of septa and thickening of the wall and/or septa. In two (3%) lesions, enhancement characteristics at CT and MR imaging were different. One of these lesions also had an increased number of septa. Overall, MR imaging results led to a cyst classification upgrade of seven lesions, from category II to IIF (n = 2), IIF to III (n = 3), or III to IV (n = 2). Pathologic correlation in 25 lesions revealed 20 malignant and five benign lesions. CONCLUSION: CT and MR imaging findings were similar in the majority of cystic renal masses. In some cases, however, MR images may depict additional septa, thickening of the wall and/or septa, or enhancement, which may lead to an upgraded Bosniak cyst classification and can affect case management.  相似文献

The purpose of this study was to report the magnetic resonance imaging (MRI) features of multiple biliary hamartoma (MBH) and to correlate them with histopathology. MRI features of 11 patients with MBH proven by histology were retrospectively reviewed and correlated to histopathology. MBH presented as multiple, tiny, and uniformly distributed lesions in all cases. All were hypointense on T1-weighted images and hyperintense on T2-weighted images relative to the liver parenchyma. Mural nodules were identified in 10 of 11 (91%) cases. They were isosignal on T1-weighted images, intermediate signal on T2-weighted images. Gadolinium-enhanced images showed mural nodule enhancement in 9 of 10 patients (90%) or a peripheral rim-like enhancement of the whole lesion in one case (9%). MBH were present in all liver specimens. In the six patients examined at MR cholangiography, the lesions lacked communication with the biliary tree. At histopathology, the mural nodule corresponded to an endocystic polypoid projection made of conjunctive septa. Three (27%) patients had associated focal nodular hyperplasia, and 1 (9%) had concomitant cholangiocarcinoma. MRI features allowed diagnosis of MBH with accuracy. Their recognition, especially the mural nodule, may help avoid misdiagnosis.  相似文献

OBJECTIVE: The purpose of this study was to evaluate the sensitivity, specificity, and accuracy of MR imaging in the characterization of adrenal masses by correlating imaging findings with histopathologic results. In addition, adrenal tumors that were of an indeterminate nature on MR imaging were analyzed. SUBJECTS AND METHODS: For 114 patients with 134 adrenal masses, MR findings were compared with histologic results. In all patients, MR imaging was performed using T2-weighted fast spin-echo imaging and unenhanced and gadolinium-enhanced T1-weighted spin-echo imaging. Chemical-shift imaging was performed in 92 patients and dynamic gadolinium-enhanced studies in 108 patients. Chemical-shift images were analyzed quantitatively and qualitatively, and dynamic gadolinium-enhanced studies were qualitatively assessed. RESULTS: The sensitivity of MR imaging in differentiating between benign and malignant adrenal masses was 91%, the specificity was 94%, and the accuracy was 93%. The diagnosis at MR imaging differed from that at histology in 12 (9%) of 134 patients. Results of quantitative analyses of chemical-shift imaging techniques showed significant differences between adenomas and nonadenomas (-36.0% versus -3.7%; p < .001). Qualitative analysis provided a similar diagnostic confidence compared with quantitative analysis. Both chemical-shift and dynamic gadolinium-enhanced studies proved to be unreliable in characterizing borderline tumors (epithelial tumors with high malignant potential). Moreover, such imaging failed to allow correct diagnosis of adenomas in two patients. CONCLUSION: The characterization of an adrenal mass can be made with high sensitivity and specificity using MR imaging. The increased reliance on MR imaging seems to be based mainly on findings from chemical-shift and dynamic gadolinium-enhanced studies. The need to perform histologic sampling of incidentally discovered adrenal masses may be reduced to some problematic lesions, which will remain during the era of MR imaging.  相似文献

Magnetic resonance imaging findings are reported for 12 pathologically proven lesions of clear cell carcinoma (CCC) of the ovary in 11 women (mean age 50 years). T1- and T2-weighted MR images were obtained in all patients, and gadolinium-enhanced MR images were obtained in 9. The mean diameter of the tumors was 13 cm. Seven patients presented with stage-I tumors. All 12 lesions consisted of cystic masses with solid protrusions occurring in 10 and solid masses in 2. The cysts were unilocular in 9 lesions and multilocular in 1. In four lesions, the cysts displayed with high intensity on T1-weighted images. Round solid protrusions were identified in 8 lesions. In 5 lesions, the number of protrusions was only a few. The solid portions of 5 masses had slightly high-intensity regions on T1-weighted images. The number of patients with ascites was three. Magnetic resonance imaging of CCC usually shows a unilocular large cyst with solid protrusions, which are often round and few in number. The purpose of this study is to describe a subset of atypical hepatic hemangiomas that enhance rapidly and diffusely and to determine whether heavily T2-weighted images could distinguish between atypically enhancing liver hemangiomas and hypervascular malignancies. A retrospective search of MR records identified seven patients with liver hemangiomas that demonstrated diffuse early enhancement and 23 patients with biopsy-proven malignant liver lesions that were hypervascular on dynamic gadolinium-enhanced MR images. Quantitative analysis of signal intensity measurements was performed on the T2-weighted images, heavily T2-weighted (TE < 140), and dynamic gadolinium-enhanced images. Blinded reader comparison of the T2-weighted images and gadolinium-enhanced images was performed. Hypervascular hemangiomas enhanced to a greater degree than hypervascular malignant liver lesions on the early phase gadolinium-enhanced images. Perilesional parenchymal enhancement was demonstrated in five cases of rapidly enhancing hemangiomas. Signal intensity and contrast-to-noise ratios on the heavily T2-weighted images of the hemangiomas were significantly greater than that of the hypervascular malignant lesions (P < .05). Hemangiomas were differentiated from the hypervascular malignant liver lesions with high accuracy (97–100%) by three blinded readers based on the T2-weighted images. A subset of hemangiomas have atypical rapid diffuse enhancement on dynamic gadolinium-enhanced images. These atypical hemangiomas can be distinguished from hypervascular malignant liver lesions on T2-weighted MR images.  相似文献

Gadolinium-DOTA enhanced MR imaging of adnexal tumors   总被引:1,自引:0,他引:1  
We conducted a retrospective study to assess the potential of contrast enhanced magnetic resonance (MR) imaging in evaluating adnexal tumors. Sixty patients with a total of 77 pelvic lesions underwent MR imaging at 1.5 T and transabdominal ultrasound (US). Precontrast T1- and T2-weighted and Gd-DOTA enhanced T1-weighted MR images were obtained. Diagnoses were proved by surgery in 57 patients. Of the 77 lesions, 54 masses were of ovarian origin, including 12 malignant disorders. Ultrasound demonstrated the lesions in 92%, whereas the sensitivity of unenhanced T2-weighted and postcontrast MR images was 98 and 96%, respectively. Compared to the T2-weighted images, postcontrast MR imaging showed superior overall tumor delineation, assessment of intratumoral architecture, and definition of tumor origin. Contrast enhancement of tumors did not differ significantly between normal ovary and benign and malignant lesions. All modalities were unable to predict malignancy of complex lesions. Based on our study, US remains the screening modality of choice in the evaluation of adnexal tumors. Contrast enhanced MR imaging may be valuable for assessing complex lesions or when the origin of the mass cannot be determined by US. Using contrast enhanced T1-weighted images instead of T2-weighted images may lead to a significant reduction in acquisition time.  相似文献

OBJECTIVES: To compare retrospectively the imaging features of computed tomography (CT) and contrast-enhanced US (CEUS) imaging for the assessment of cystic renal masses using the Bosniak classification system. MATERIALS AND METHODS: The CT and CEUS images of 31 pathologically confirmed cystic renal masses in 31 patients were retrospectively analyzed for septa numbers, wall and/or septa thickness, enhancement degree, and for the presence of a solid component by consensus between two radiologists using the Bosniak classification. Diagnostic accuracies of CT and CEUS for malignant cystic tumor were calculated and compared using McNemar test. RESULTS: Diagnostic accuracies of CT and CEUS for malignant renal tumor were 74% and 90%, respectively, but there were not statistically different (P>0.05). CEUS and CT images showed same Bosniak classification in 23 (74%) lesions and there were differences in 8 (26%) lesions, all of which were upgraded by CEUS; one lesion from I to IV, two lesions from II to IV, two lesions from IIF to III, and three lesions from III to IV. CEUS images depicted more septa in 10 (32%) lesions, more thickened wall and/or septa in 4 (13%) lesions, and stronger enhancement in 19 (61%) lesions. Moreover, for six lesions, solid component was detected by CEUS but not by CT. CONCLUSION: CEUS might better visualize septa number, septa and/or wall thickness, solid component and the enhancement of some renal cystic masses than CT, resulting in upgrade of Bosniak classification and affecting their treatment plan.  相似文献

This study describes the occurrence of hemorrhage in renal cancer in patients with chronic renal insufficiency as shown on MR images. Thirteen consecutive patients with chronic renal insufficiency who had his tologically proven renal cancer and underwent MRI at 1.5 T were entered in the study. MR examinations included spoiled gradient echo (SGE) and T1-weighted fat-suppressed imaging pre- and postgadolinium administration. All renal cancers were well shown on MR images and were most clearly depicted on postgadolinium T1-weighted fat-suppressed images. Tumors in 12 of 13 patients had regions of high signal intensity on precontrast T1-weighted images. Histology demonstrated intratumoral hemorrhage in all 12 of these patients. Four hemorrhagic tumors were largely cystic on imaging studies. One of these cancers altered in appearance from largely cystic with extensive hemorrhage to largely solid with substantial enhancement after a 2.5-year interval. Renal cancers demonstrated minimal enhancement (11 patients) on early postgadolinium images and were minimally enhanced on delayed images in 10 of 13 tumors. Two renal cancers demonstrated intense enhancement. Renal cancers are well shown on MR images in patients with chronic renal insufficiency. Because of the common occurrence of hemorrhage into renal cancers in patients with renal insufficiency, caution should be exercised when evaluating hemorrhagic cystic lesions in these patients.  相似文献

目的 :探讨囊性肾恶性肿瘤的CT表现与临床病理基础 ,提高对囊性肾恶性肿瘤的认识。方法 :对经手术病理证实的 2 2 7例肾恶性肿瘤中 2 5例囊性肾恶性肿瘤的CT图像进行回顾分析。结果 :2 5例囊性肾恶性肿瘤中肾盂移行细胞癌 8例 ,肾细胞癌 16例 ,肾胚胎癌 1例。本组 2 5例囊性肾恶性肿瘤的壁均显示较厚 (厚于 3 .0mm) ,且厚薄不均 ,占 10 0 %。囊变区有不规则的分隔或囊内有实性成分 2 0例 ,占 80 %。增强后囊壁、囊内分隔及实性成分均有强化。囊变区内CT值较单纯性肾囊肿高 ,P <0 .0 1。结论 :囊性肾恶性肿瘤的CT表现有一定特征性 ,了解这些特征有利于正确诊断  相似文献

PURPOSE: To prospectively compare oral contrast-enhanced T2-weighted half-Fourier rapid acquisition with relaxation enhancement (RARE) magnetic resonance (MR) imaging with T1-weighted gadolinium-enhanced fast low-angle shot (FLASH) MR and standard examinations in the evaluation of Crohn disease. MATERIALS AND METHODS: Institutional review board approval and informed consent were obtained. Fifty-nine patients with Crohn disease underwent MR imaging after oral administration of a superparamagnetic contrast agent; RARE plain and fat-suppressed sequences and FLASH sequences were performed before and after intravenous injection of gadolinium chelate. References were endoscopic, small-bowel barium, computed tomographic, ultrasonographic, and clinical-biochemical scoring of disease activity. Two radiologists analyzed MR images for presence and extent of Crohn disease lesions, presence of strictures or other complications, and degree of local inflammation. MR findings were correlated with endoscopic, radiologic, and clinical data (kappa statistic and Spearman rank correlation test). RESULTS: T2-weighted MR was 95% accurate, 98% sensitive, and 78% specific for detection of ileal lesions. Agreement between T1- and T2-weighted images ranged from 0.77 for ileal lesions to 1.00 for colic lesions. T2-weighted MR enabled detection of 26 of 29 severe strictures, 17 of 24 enteroenteric fistulas, and all adhesions and abscesses; T1-weighted MR enabled detection of 20 of 29 severe strictures, 16 of 24 enteroenteric fistulas, and all adhesions and abscesses. Complications leading to surgery were found in 12 (20%) patients; these were assessed correctly with either T1- or T2-weighted images. T2-weighted signal intensities of the wall and mesentery correlated with biologic activity (P < .001, r of 0.774 and 0.712, respectively). Interobserver agreement was 0.642-1.00 for T2-weighted and 0.711-1.00 for T1-weighted images. CONCLUSION: T2-weighted MR can depict Crohn disease lesions and help assess mural and transmural inflammation with the same accuracy as gadolinium-enhanced T1-weighted MR. Combination of gadolinium-enhanced T1- and T2-weighted sequences is useful in the assessment of Crohn disease.  相似文献

The authors studied the ability to improve detection of splenic lesions during suspended respiration with dynamic gadolinium-enhanced T1-weighted spin-echo magnetic resonance (MR) imaging. In the first phase of the study, normal splenic contrast material enhancement patterns were assessed in 10 control patients without splenic lesions. A heterogeneous signal intensity pattern was observed in 11 patients with splenic lesions during bolus injection of gadopentetate dimeglumine, with conversion to homogeneous enhancement 1 minute later. Mean splenic enhancement was 321% during bolus injection, with a rapid return toward baseline signal intensity thereafter. In the second phase, evaluation of 18 splenic lesions detected with contrast-enhanced computed tomography in 11 patients revealed that dynamic gadolinium-enhanced MR pulse sequences significantly improved lesion conspicuity and detectability compared with conventional T1-and T2-weighted pulse sequences. Contrast-to-artifact ratio measurements were 0.5, 3.7, and 9.3 for conventional T1-weighted, T2-weighted, and dynamic gadolinium-enhanced MR images, respectively.  相似文献
