STUDY OBJECTIVES: Transesophageal echocardiography (TEE) has a high sensitivity for the diagnosis of infective endocarditis (IE), but the prospective role of TEE when added to a careful clinical examination has not been well-studied. DESIGN: We compared the results of TEE to a clinical evaluation by an infectious disease specialist in 43 consecutive patients in whom TEE was ordered to rule out IE. Prior to TEE, the patients were classified on clinical grounds as to their likelihood of IE using a modification of the von Reyn criteria. Changes in management occurring as a result of TEE also were evaluated. MEASUREMENTS AND RESULTS: TEE was positive in 11 patients, negative in 29, and indeterminate in 3. TEE was positive in 6 (46%) of 13 high probability patients, 2 (67%) of medium probability patients, and 3 (11%) of 27 low probability patients. A change in management based on TEE occurred in 4 (31%) patients with high probability, in no patients with medium probability, and in 1 (4%) patient with low probability. CONCLUSIONS: TEE confirms IE in patients with high probability of IE and often leads to a management change. However, TEE is unlikely to establish the diagnosis or change management in patients with low probability.  相似文献

Thirteen patients already scheduled for surgery for repair of prosthetic paravalvular regurgitation underwent intraoperative real time two-dimensional transesophageal echocardiography (2D TEE) and live/real time three-dimensional transesophageal echocardiography (3D TEE). In all patients, 3D TEE was able to provide more information regarding the location and size of the paravalvular defect as compared to 2D TEE. 3D TEE resulted in a more accurate localization of the defect and an estimation of the size of the defect that correlated much more closely with surgical findings when compared with 2D TEE. Our preliminary results demonstrate the superiority of 3D TEE over 2D TEE in the evaluation of paravalvular prosthetic regurgitation. 3D TEE not only provides an accurate assessment of the exact site of the leakage, but also gives a more accurate estimate of its size. This information could be valuable to surgeons who may encounter difficulty when localizing and estimating the size of paraprosthetic leaks while the heart is devoid of blood during surgery.  相似文献

目的:探讨经食道超声心动图评价成人先天性心脏病的价值。方法:我们用经食道超声心动图(TEE)诊断成人先天性心脏病173例,156例经心导管和/或手术证实。结果:在131例经手术和/或心导管证实的房间隔缺损(ASD)的病例,经胸超声心动图(TTE)显示ADS109例,TEE显示ASD128例,TTE诊断房间隔缺损的敏感性为83%,TEE诊断房间隔缺损的敏感性为98%,值得注意的是,10例静脉窦型ASD,TTE仅能显示3例,而TEE能观察到所有的10例,且所有的TEE检查的病例,能清晰地观察到与静脉窦型ASD相关的部分肺静脉异位引流,3例冠状静脉窦部位的小ASD,TTE显示2例,但TEE未能显示,结论:TEE探查ADS明显优于TTE,尤其是静脉窦型ASD,但靠近冠状静脉窦部位的小ASD,TEE并不比TTE检查优越,同时TEE对其他先天性心脏病异常结构的观察也提供大量的附加信息。  相似文献

Transesophageal echocardiography (TEE) is a semi-invasive examination that provides better images of the atrium than classical transthoracic echocardiography (TTE) due to the anatomical positioning of the captor and the high frequency Doppler apparatus used. We used TEE and TTE to evaluate the incidence of the cardiac origin of emboli in 46 patients with unexplained stroke or transient ischemic attack (TIA): 23 had documented heart disease (mean age 60 years) and 23 had no cardiac disease (mean age 43 years). Among those with existing heart disease, 4% of the anomalies certainly or probably responsible for the emboli in addition to the underlying cardiopathy were detected by TTE versus 37% by TEE (as compared to values reported in the literature: 25% by TTE and 51% by TEE). In particular, 4 abnormalities were better visualized by TEE: left atrial thrombus, especially those located in the auricle (5 TEE versus 1 TTE); spontaneous contrast showing the swirl of blood stagnating in the dilated left atria of patients with mitral valve disease seen in 7-39% of the TIA by TEE as compared to less than 1% by TTE (3 TEE versus 0 TTE); aneurysm of the interauricular septum (AIAS) observed in 5-16% of the TIA by TEE as opposed to 0-1% by TTE (4 TEE versus 1 TTE); patent foramen ovale (PFO) was noted more frequently following injection of a contrast medium when visualized by TEE (19-22%) than by TTE (6-8%) and can explain the passage of a paradoxical embolus (1 TEE versus 0 TTE). The incidences of left atrial thrombus, AIAS and PFO are well correlated with systemic emboli, especially in young adults having experienced an unexplained TIA without underlying cardiopathy. TEE is an easy-to-use and well tolerated technique for detecting the cardiac origin of emboli in unexplained stroke. Whether to opt for a medical or surgical treatment to avoid recurrences is discussed.  相似文献

Human studies have suggested that Doppler transesophageal echocardiography (TEE) can determine normal physiologic coronary blood flow (CBF) and alterations in CBF due to proximal flow-limiting stenoses. However, assessment of CBF by Doppler TEE has not been validated. To determine if true estimation of CBF could be obtained with Doppler TEE, seven mongrel dogs (weight range 28 kg-36 kg) were evaluated. Simultaneous CBF determinations by Doppler TEE and epicardial electromagnetic flow (EMF) and/or epicardial Doppler flow (EDF) probes were compared. Measurements were obtained at baseline and following varying degrees of proximal coronary occlusion, which produced reactive hyperemia. RESULTS: Consistent Doppler flow waveforms were obtainable by Doppler TEE in 34 different measurements during perturbations: Mean for TEE Flow (ml/min) = 85, EMF or EDF Flow (ml/min) = 53; Standard Deviation for TEE Flow (ml/min) = 45, EMF or EDF Flow (ml/min) = 38; Minimum for TEE Flow (ml/min) = 42, EMF or EDF Flow (ml/min) = 11; and Maximum for TEE Flow (ml/min) = 174, EMF or EDF Flow (ml/min) = 130. TEE Flow (ml/min) = 1.1 EMF/EDF flow + 26.3. There was a general trend towards overestimation of CBF by Doppler TEE. This study demonstrates that Doppler TEE is a promising method for obtaining measurements of CBF over the physiologic range.  相似文献

The most important role of TEE in aortic valve disease is in the diagnosis of endocarditis and its complications. Examination of the annulus and subvalvular region is essential in any patient with possible aortic valve endocarditis. Assessment of the severity of aortic stenosis is a useful application of TEE when other data are either inconsistent or unavailable. TEE can provide a diagnosis of the origin of acute severe aortic insufficiency; this information may play a critical role in surgical planning. The diagnosis of a variety of aortic valve diseases can be made when TEE is performed to find an embolic source or to rule out dissection. In the case of mass lesions, such as papillary fibroelastomas and Libman-Sacks vegetations, the results of TEE carry major therapeutic implications. TEE offers generally excellent quality images of the LVOT and images of the RVOT and pulmonic valve that are superior to transthoracic echocardiography. The major clinical usefulness of TEE stems from its ability to identify pulmonic valve mass lesions and the causes of left and right ventricular outflow obstruction. TEE is also an important adjunct in the surgical management of left ventricular outflow obstruction.  相似文献

BACKGROUND: The clinical implications of a negative multiplane transesophageal echocardiography (TEE) have not yet been reported. We aim to determine the negative predictive value (NPV) of a negative multiplane TEE in patients with suspected infective endocarditis (IE). METHODS AND RESULTS: We identified 83 consecutive patients with suspected IE and negative multiplane TEE from our echocardiographic database. Of 74 patients with a minimum of 1-month follow-up, only 1 patient developed "definite IE". Eight patients had "possible IE". The calculated NPV of multiplane TEE in IE was 98.6% if we only considered the case of "definite IE". If we assumed that all patients with "possible IE" had the disease, then the NPV of multiplane TEE was 87.8%. CONCLUSIONS: Multiplane TEE is a highly accurate diagnostic tool with excellent NPV in IE. However, in a highly suspicious clinical setting for IE, a repeat TEE is still recommended to assess evolving echocardiographic features.  相似文献

Before the introduction of transesophageal echocardiography (TEE) in the operating room, intraoperative echocardiography relied on epicardial imaging. The disadvantages of this approach included interference with the surgical procedure, limited windows, and potential distortion of cardiac structures. Consequently, multiplane TEE has now emerged as the intraoperative imaging method of choice. It provides high-resolution images of cardiac structures and excellent portraits of flow abnormalities. Intraoperative TEE does not interfere with the surgical field and procedure. TEE provides better imaging of the valves, atria, aorta, pulmonic vasculature, and pericardium, which are sometimes difficult to visualize by transthoracic echocardiography. TEE is especially beneficial in surgeries for valve replacement, valve repair, cardiac mass, aortic disease, congenital heart disease, and pericardial disease. Presurgical TEE provides information for surgical planning. TEE is helpful for the assessment of the immediate result of surgery and detection of complications that may need a prompt response. Thus, intraoperative TEE has a vital impact on management of cardiac surgery.  相似文献

Aortic Stenosis     
Noninvasive assessment of aortic valve area by echocardiography has become the standard of practice over the past few years. The advent of transesophageal echocardiography (TEE) has provided a new method for the assessment of aortic valve area (AVA) using planimetry by two-dimensional imaging. Clear visualization of the anatomy of the valve, as well as accuracy of AVA assessment, makes TEE an invaluable tool for the evaluation of aortic valve stenosis. TEE is especially helpful in clinical settings when there is a discrepancy between the AVA obtained by transthoracic echocardiography and cardiac catheterization. TEE is particularly helpful in the assessment of the aortic valve during intraoperative echocardiography. This review discusses the techniques, imaging planes, and details for assessing AVA by TEE. The role of TEE in AVA assessment is described, with specific clinical case examples cited.  相似文献

Transesophageal echocardiography (TEE) has provided an accurate new window for the evaluation of diseases of the thoracic aorta. Experience with TEE has led to an increased recognition of atherosclerosis of the thoracic aorta as a source of cerebral and systemic embolism. Certain features of aortic plaque morphology detected by TEE may prove to have prognostic and therapeutic significance. The intraoperative assessment of thoracic aortic atherosclerosis by TEE may guide modifications in surgical techniques and aortic manipulations that reduce the incidence of perioperative neurologic complications. TEE has also become a valuable tool for the diagnostic evaluation of patients with blunt chest trauma. The precise role of TEE in the management of these disorders is currently under investigation.  相似文献

Transesophageal echocardiography (TEE) is an excellent method for visualizing pathology of the mediastinum. We present a case in which TEE detected an unsuspected lymphoma posterior to the descending thoracic aorta in a man suspected of having endocarditis. This case illustrates the advantages of TEE over transthoracic echocardiography in examining the mediastinum and the importance of performing a complete TEE examination.  相似文献

The role of transoesophageal echocardiography (TEE) was evaluated in a consecutive series of 100 procedures performed in 86 patients (age 17–81, mean 56 years). All patients had prior transthoracic echocardiography (TTE). TEE was performed with a 5 MHz phased array transoesophageal transducer with pulsed wave Doppler and colour flow mapping capability. Forty-four per cent of patients received intravenous sedation and 36% received antibiotic prophylaxis. There were no complications of TEE. The TTE and TEE findings were compared. In patients referred for possible cardiac source of embolism, left atrial thrombi were detected in 8/27 TEE studies but in none of 27 TTE studies. In 12 patients with prosthetic valve dysfunction TEE distinguished prosthetic from periprosthetic regurgitation in 9/12 studies compared to 3/12 with TTE. In 11 patients with suspected aortic dissection TEE correctly detected dissection in all seven cases in which the diagnosis was subsequently confirmed, whereas TTE showed only equivocal findings in two cases. Vegetations were detected by TEE in 4/5 studies in patients with proven native valve endocarditis and by TTEin 2/5. No vegetations were detected by TTE or TEE in five studies in patients with proven prosthetic valve endocarditis. Compared with other investigations there were no false positive TEE studies and one possible false negative study. We conclude that TEE is a safeprocedure which often provides additional clinical information to transthoracic echocardiography.  相似文献

Transesophageal echocardiography (TEE) has emerged as a valuable complement to trans-thoracic echocardiography (TTE) and expanded our ability to visual the heart with ultrasound. TEE provides high resolution imaging with minimal attenuation from non cardiac structures, and allows detailed visualization of cardiac structures and flow disorders not well seen with TTE. While developments in transesophageal imaging over the last 3 decades have led to established clinical indications for TEE, recent advances in TEE including: 1) new applications of trans-thoracic methodologies in TEE; 2) innovations in TEE technology, and 3) advances in clinical care of cardiovascular disease, have all contributed to exciting new clinical applications of TEE. This review provides an overview of the advances in TEE technology and the current and emerging clinical applications of TEE.  相似文献

Objectives

Catheter-based mitral valve clip repair is an effective procedure in selected patients with mitral regurgitation. Two-dimensional (2D TEE) is the primary imaging mode for guidance of mitral-clip procedures. Recently, real-time three-dimensional transesophageal echocardiography (RT 3D TEE) has been used as an additional imaging mode. This review describes the use of 2D TEE and RT 3D TEE imaging during percutaneous edge-to-edge mitral valve repair (mitral-clip).

Results

Compared to 2D TEE, RT 3D TEE provides additional information in several steps of the procedure including alignment of catheter trajectory, clip positioning and orientation of clip arms.

Conclusions

RT 3D TEE is a powerful new imaging tool that may become the technique of choice for guidance of mitral-clip procedures.  相似文献

Although transesophageal echocardiography (TEE) is a well-established tool in delineating the cardiac and aortic sources of emboli in stroke patients, its reproducibility in detecting aortic atherosclerosis (AA) that may lead to stroke is not well documented. The aim of this study is to demonstrate the inter- and intra-observer agreement in diagnosing AA based on the TEE study. During the study period, 2 echocardiographers read 32 TEE studies performed on a group of stroke patients. The AA was estimated by measuring the largest atheroma thickness in millimeters (mm). The intra-observers and intraclass correlation coefficients (ICC) were 0.97 and 0.85, respectively, for the 2 TEE observers. The inter-rater ICC coefficient between TEE readers was 0.91. In conclusion, TEE detected AA and defined high proportions of the morphological characteristics of atheroma with good reproducibility among inter and intra-observers.  相似文献

The rapidly expanding role of transesophageal echocardiography (TEE) in the management and treatment of congenital heart disease includes guidance of interventional cardiac catheterization procedures and monitoring of results. TEE is particularly helpful for assessing device position during device closure of atrial septal defects (ASD), ventricular septal defects, and perivalvar defects, especially when multiple or complex defects are present. It is also useful during balloon dilation of certain congenital and postoperative obstructions. Stent placement for treatment of intracardiac obstructions is facilitated by TEE guidance. The risks associated with Brockenbrough transseptal puncture and ASD enlargement, particularly in the setting of unusual atrial anatomy, may be reduced by TEE guidance of the procedure. TEE monitoring of video assisted thoracoscopic closure of patent ductus arteriousus has contributed to the high success rate of this innovative procedure. TEE imaging assists in positioning of devices and catheters, reduces radiation exposure and contrast load, and provides immediate and continous assessment not only of results, but of potential complications of interventional procedures. As the applications of interventional catheterization in the field of pediatric cardiology increase so will the role of TEE.  相似文献

Background: Robotic mitral valve (MV) repair is a new surgical technique that uses small incisions. Previous studies have demonstrated the importance of intraoperative transesophageal echocardiography (TEE) for conventional MV surgery with the use of a median sternotomy incision. The aim of the present study was to delineate the utility of intraoperative TEE in robotic MV repair. Methods: Intraoperative TEE was performed in 22 consecutive patients undergoing robotic MV repair for severe degenerative mitral regurgitation (MR) over a period of 2 years. Before cardiopulmonary bypass (CPB), TEE was used to define the lesions of degenerative MR and the localization of the prolapsed leaflets, and to evaluate the severity of MR. During establishment of peripheral CPB, TEE was used to guide placement of the cannulae in the inferior vena cava (IVC), superior vena cava (SVC), and ascending aorta (AAO). After weaning from CPB, TEE was used to assess immediately the competency of the surgical repair. Results: Agreement between TEE and surgical findings was excellent: 92.3% (kappa, 0.873) for the lesions of degenerative MR, and 98.5% (kappa, 0.943) for the localization of the prolapsed leaflets. Under TEE guidance, all the cannulae (100%) in the SVC, IVC, and AAO were placed correctly. TEE demonstrated all the patients (100%) had successful robotic MV repairs. A 65-year-old man was consulted because of buckling of transesophageal echocardiography (TEE) probe in the esophagus. A forward-viewing endoscope was inserted to the esophagus alongside the TEE probe. TEE probe was pushed to the stomach while the retroflexed tip portion was pushed by the endoscope in order to prevent retroflexion. The TEE probe was advanced into the stomach by this method where the buckled part was unfolded and then withdrawn. Cardiologists performing TEE and the referred gastroenterologists could perform such a method of solution in case of buckling of TEE probe in the esophagus.  相似文献

目的探讨经食管超声心动图在二尖瓣成形术中的应用价值。方法术前18例二尖瓣关闭不全患者均常规经胸超声心动图检查,术中经食管超声心动图监测,并即刻评价二尖瓣成形术的效果。结果本组18例中16例一次性手术实施成功。1例术中监测发现反流2级后再次实施成形后成功,1例术中监测发现反流3级后改行二尖瓣置换术。结论经食管超声心动图在二尖瓣成形术中具有非常重要的临床应用价值。  相似文献

We reviewed our experience with transesophageal echocardiography (TEE) and color flow imaging in 157 consecutive patients with known or suspected heart disease to ascertain the impact of this technology on patient care. TEE was performed for diagnostic purposes (22/157), during interventional cardiac catheterizations (13/157), and during operative procedures (122/157). Diagnostic studies were performed after transthoracic echocardiography (TTE) in 21 of 22 patients. TEE was performed because TTE was inconclusive (15/21) or failed to provide sufficient detail of an abnormality (6/21). TEE detected an abnormality in 6 of 15 inconclusive TTEs. TEE was helpful during interventional cardiac catheterizations, particularly during umbrella closure of septal defects and in patients with complex venous and atrial anatomy undergoing transseptal puncture. TEE studies performed before cardiac operations significantly changed the diagnosis in only 5 of 122 (4%) patients, but the information changed the surgical approach in 4 of 5 of these patients. Postoperative TEE assessment more frequently changed care and resulted in further surgical management in 9 of 122 (7%) or a change in medical management in 6 of 122 (5%) patients. TEE was discontinued because of complications before studies were completed in only 4 of 157 (3%) patients. TEE and color flow imaging is a useful adjunct to care of children with known or suspected congenital heart disease.  相似文献
