PURPOSE: To examine the relationship among hypermetropia, axial length, and hypertension in a Chinese population. DESIGN: Population-based cross-sectional study. METHODS: This study included 1213 Chinese individuals aged 40 to 81 years. Hypertension was defined as systolic blood pressure > or =140 mm Hg, diastolic blood pressure > or =90 mm Hg, or self-report history of antihypertension treatment. Refraction was determined with an autorefractor and refined subjectively. A-mode ultrasound scanning was used to measure axial length. RESULTS: The crude odds ratio of hypertension was 2.5 (95% CI, 1.4-4.6) for moderate hypermetropia (> or =+2.00 spheric equivalent diopters) vs high myopia (< or =-6.00 diopters), and 1.4 (95% CI, 0.9-2.1) for highest vs lowest axial length quintile. These associations were no longer significant after adjustment for age, gender, education, housing type, and income. CONCLUSIONS: These data provide no evidence that hypermetropia or shorter axial length is associated independently with hypertension in Chinese individuals.  相似文献

AIM: To relate indices of education, occupation, and socioeconomic status to ocular dimensions and refraction in an adult population. METHODS: A population based, cross sectional survey of adult Chinese aged 40-81 years residing in the Tanjong Pagar district in Singapore. Ocular dimensions, including axial length, anterior chamber depth, lens thickness, and vitreous chamber depth, were measured using an A-mode ultrasound device. Corneal radius of curvature and refraction were determined with an autorefractor, with refraction further refined subjectively, and lens nuclear opacity was graded clinically using the modified Lens Opacity Classification System III score. Data on education, occupation, income, and housing type were obtained from a standardised interview. RESULTS: Biometric data were available on 951 phakic subjects. After controlling for age, sex, occupation, income and housing type, higher education was associated with longer axial lengths (0.60 mm; 95% confidence interval (CI): 0.34, 0.85, for every 10 years of education), longer vitreous chambers (0.53 mm; 95% CI: 0.30, 0.77), and more myopic refractions (-1.50 dioptres, 95% CI: -2.08, -0.92). Adjustment for axial length attenuated the refractive association of education (-0.68 dioptre, 95% CI: -1.14, -0.21). Similarly, near work related occupations (managers, professionals, and office workers) and higher income were independently associated with longer axial lengths, longer vitreous chambers, and more myopic refractions, and adjustment for axial length attenuated the refractive associations. CONCLUSIONS: Adults with greater education, near work related occupations, and higher income are more likely to have longer axial lengths and vitreous chambers, and more myopic refractions. The refractive associations of education, occupation, and income are largely explained by variations in axial length.  相似文献

PURPOSE. To describe the association of ocular dimensions and refraction with adult stature. METHODS. This was a population-based cross-sectional survey of adult Chinese aged 40 to 81 years residing in the Tanjong Pagar district in SINGAPORE: As part of the examination, ocular dimensions, including axial length, anterior chamber depth, lens thickness, and vitreous chamber depth, were measured using an A-mode ultrasound device. Corneal radius and refraction were determined with an autorefractor, with refraction further refined subjectively. Height (in meters) and weight (in kilograms) were measured using a standardized protocol, and body mass index (BMI) was calculated as weight divided by the square of the height (kilograms per square meter). RESULTS. Data on ocular biometry, refraction, height, and weight were available on 951 (55.4%) participants with phakic eyes. After controlling for age, sex, education, occupation, housing type, income, and weight, it was found that taller persons were more likely to have longer axial lengths (+0.23 mm longer axial length, for every 0.10 m difference in height), deeper anterior chambers (+0.07 mm), thinner lenses (-0.09 mm), longer vitreous chambers (+0.26 mm), and flatter corneas (+0.09 mm longer corneal radius), although refractions were similar. In contrast, heavier persons tended to have more hyperopic refractions (+0.22 D for every 10 kg difference in weight, +0.56 D for every 10 kg/m(2) difference in BMI) but similar ocular dimensions. CONCLUSIONS. Adult height is independently related to ocular dimensions, but does not appear to influence refraction. Thus, although taller persons are more likely to have longer globes, they also tend to have deeper anterior chambers, thinner lenses, and flatter corneas. Conversely, weight is independently related to refraction, although the exact biometric component responsible for this association is not apparent.  相似文献

AIM: To describe risk factors for nuclear, cortical, and posterior subcapsular (PSC) cataracts in Chinese Singaporeans. METHODS: A population based cross sectional study was carried out on ethnic Chinese men and women aged 40-81 years. A stratified, clustered, disproportionate (more weights to older people), random sampling procedure was used to initially select 2000 Chinese names of those aged 40-79 years from the 1996 electoral register in the Tanjong Pagar district in Singapore. Eligible subjects (n = 1717) were invited for a standardised ocular examination and interview at a centralised clinic, following which an abbreviated examination was conducted for non-respondents in their homes. Cataract was graded clinically using to the Lens Opacity Classification System (LOCS) III system. The main outcome measures were adjusted odds ratio for risk factors for specific cataract types (nuclear, cortical and PSC), any cataract and cataract surgery, examined in multiple logistic regression models. RESULTS: Out of the 1232 (71.8%) examined, 1206 (70.2%) provided lens data for this analysis. Increasing age was associated with all cataract types, any cataract, and cataract surgery. There was no significant sex difference in presence of any cataract, specific cataract types or cataract surgery. After controlling for age, sex, and other factors, diabetes was associated with cortical cataract (3.1; 95% CI: 1.6 to 6.1), PSC cataract (2.2; 95% CI 1.2 to 4.1), any cataract (2.0; 95% CI: 0.9 to 4.5), and cataract surgery (2.3; 95% CI: 1.3 to 4.1). Lower body mass index was associated with cortical cataract (1.8; 95% CI: 1.1 to 2.9; lowest versus highest quintile) and any cataract (2.3; 95% CI: 1.3 to 4.0). Current cigarette smoking was associated with nuclear cataract (1.7, 95% CI: 1.0 to 2.9; more than 10 cigarettes per day versus none). A non-professional occupation was associated with nuclear cataract (2.9; 95% CI: 1.5 to 5.8; for production or machine operators and 2.6; 95% CI: 1.2 to 5.5; for labourers or agricultural workers, both versus professionals). Lower education was associated with nuclear cataract (2.3; 95% CI: 1.0 to 5.2, none versus tertiary), while lower household income was associated with PSC cataract (4.7, 95% CI: 1.1 to 20.0; income S$4000). CONCLUSIONS: Age related cataracts are associated with a variety of risk factors among Chinese people in Singapore, similar to those reported in European, Indian, and African derived populations. These data support common aetiological mechanisms for age related cataracts, irrespective of ethnic origin.  相似文献

Refractive errors and incident cataracts: the Beaver Dam Eye Study   总被引:4,自引:0,他引:4  
PURPOSE: To describe the relation between refractive errors and incident age-related cataracts in a predominantly white US population. METHODS: All persons aged 43 to 84 years of age in Beaver Dam, Wisconsin, were invited for a baseline examination from 1988 through 1990 and a follow-up examination 5 years later from 1993 through 1995. At both examinations, participants had refraction and photographic assessment of cataract, according to a standardized protocol. Myopia was defined as a spherical equivalent of -1.0 diopters (D) or less, hyperopia as +1.0 D or more. The relations between refractive errors at baseline and cataract at baseline (prevalent cataract), 5-year incident cataract, and incident cataract surgery were analyzed by using generalized estimating equations. RESULTS: When age and gender were controlled for, myopia was related to prevalent nuclear cataract (odds ratio [OR], 1.67; 95% confidence interval [CI], 1.23-2.27), but not to cortical and posterior subcapsular cataracts. Myopia was not related to 5-year incident nuclear, cortical, and posterior subcapsular cataracts, but was related to incident cataract surgery (OR 1.89; CI 1.18-3.04). Hyperopia was related to incident nuclear (OR 1.56; CI 1.25-1.95) and possibly cortical (OR 1.25; CI 0.96-1.63) cataracts, but not to posterior subcapsular cataract or cataract surgery. After further adjustment for diabetes, smoking, and education, the association between myopia and incident cataract surgery was attenuated (OR 1.60; CI 0.96-2.64), but the associations between hyperopia and incident nuclear and cortical cataracts were unchanged. CONCLUSIONS: These data support the cross-sectional association between myopia and nuclear cataract seen in other population-based studies, but provide no evidence of a relationship between myopia and 5-year incident cataract. Hyperopia may be related weakly to incident nuclear and cortical cataract.  相似文献

BACKGROUND: Lower socioeconomic status (SES) is associated with higher morbidity and mortality in many countries. Present evidence suggests that glaucoma has similar risk factors to major chronic diseases such as cardiovascular disease. This study investigates the association between SES and intraocular pressure (IOP), an important risk factor for glaucoma. METHODS: The Tanjong Pagar Study was a population-based cross-sectional survey of Chinese people aged 40-79 years, who were randomly selected from the Singapore electoral register. Of the 2000 people selected, 1717 were considered eligible and 1090 were examined in clinic and included in the present study. IOP was measured using applanation tonometry. SES was assessed using a standardised questionnaire; education and income were used as the main explanatory variables. The effect of systolic blood pressure (SBP) was also examined. RESULTS: Participants with lower levels of education and income had higher mean IOP (both p<0.01). These associations remained after adjusting for age and central corneal thickness, a strong independent predictor. SBP was strongly associated with both SES and IOP (both p<0.01). Adjusting for SBP attenuated the association between SES and IOP. CONCLUSION: Participants with lower education and income have a higher mean IOP. This effect may be mediated, in part, by an association of education and income with SBP. This is the first study to suggest that there is a social gradient in the distribution of the only major modifiable risk factor for glaucoma. Increasing similarities exist between the causation models of chronic diseases and that of glaucoma.  相似文献

PURPOSE: To determine the prevalence and risk factors for pterygium in the Chinese population of Singapore. METHODS: A population-based survey was conducted in Singapore, an island located 1 degree north of the equator with a stable tropical climate. A disproportionate, stratified, clustered, random sampling procedure was used to select the names of 2000 Chinese people aged 40 to 79 years from the 1996 electoral register in the Tanjong Pagar district of Singapore. Selected subjects underwent a comprehensive interview and ocular examination. Pterygium was diagnosed and graded clinically as grade 1 (transparent), 2 (intermediate), and 3 (opaque). Risks factors associated with pterygium and grade 3 pterygium were evaluated with logistic regression models. RESULTS: From a total of 1717 eligible subjects, 1232 (71.8%) were examined. There were 120 people with either unilateral (n = 70) or bilateral (n = 50) pterygium, equivalent to an overall prevalence of 6.9% (95% confidence interval [CI], 5.2, 8.8) in the Chinese population aged 40 and older. The prevalence increased linearly with age (chi-square test of trend P <.001) and was higher among men than women (age-adjusted odds ratio [OR], 4.2; 95% CI, 2.5, 6.9). Men aged 70 and above had the highest overall prevalence of 25.4% (95% CI, 18.2, 19.4), but pterygium was not seen in women aged 40 to -49 years. In multivariate analysis, ptergyium was independently associated with increasing age (OR, 7.8; 95% CI, 3.2, 18.8 for persons 70 to 81 years, compared with 40 to 49 years), male sex (OR, 5.1; 95% CI, 2.9, 9.3) and certain occupations; factory workers, production workers and machine operators (OR, 3.1; 95% CI, 1.5, 6.3), as well as laborers and agricultural workers (OR, 3.3; 95% CI, 1.6, 7.0) had higher risks, compared with professionals and office workers. Grade 3 pterygium (n = 36) was also independently associated with male sex (OR, 11.6; 95% CI, 3.5, 38.6) and similar occupations but was not related to age. CONCLUSIONS: The prevalence of pterygium in Singapore is 7% among Chinese aged 40 years and older. Independent associations with increasing age, male sex, and occupations linked to outdoor work and other exposures suggest a multifactorial cause of this condition.  相似文献

Refractive errors and axial length among Alaskan Eskimos   总被引:1,自引:0,他引:1  
An epidemiological study of the prevalence of refractive errors was made of the Eskimo population of the Norton Sound and Bering Straits region of Alaska. It was possible to determine the subjective refraction in 83.7% of 1673 persons examined: 44.9% were emmetropic, 44.7% myopic and 10.4% hyperopic. Myopia was found more often in women (48.3%) than in men (41.3%), while emmetropia was found more often in men (49.3%) than in women (40.3%). Nine,four percent of the men were hyperopic and 11.4% of the women. The prevalence of myopia increased with age, with a maximum of 67.2% in the age group between 30 and 40 years. Subsequently, the number of myopes decreased rapidly. Little hyperopia was seen before the age of 50 years, it then increased rapidly, up to 71.5% for persons above the age of 80 years. Emmetropia, on the other hand, decreased with age. Ultrasound examination showed that an increasing axial length was associated with more myopia.  相似文献

PURPOSE: To describe the relationship of refractive errors to the 10-year incidence of age-related maculopathy (ARM) in a defined white population. METHODS: Persons aged 43 to 86 years of age in Beaver Dam, Wisconsin, were invited for a baseline examination from 1988 through 1990, and follow-up examinations 5 and 10 years later (n = 3684). Refraction was measured at baseline, with myopia defined as a spherical equivalent of -1.00 D or less, emmetropia as -0.75 to +0.75 D and hyperopia as +1.00 D or more. At each examination, signs of ARM were ascertained from grading stereoscopic color fundus photographs based on a standard protocol. The association between baseline refractive status and the 10-year incidence and progression of ARM was analyzed. RESULTS: The 10-year cumulative incidence for early ARM was 7.1%, 7.7%, and 11.7%, in eyes with myopia, emmetropia, and hyperopia, respectively. The corresponding 10-year cumulative incidence for late ARM was 0.3%, 0.8%, and 2.2%. When age was controlled for, there was no association between myopia and incident early (relative risk [RR] 1.0, 95% confidence interval [CI], 0.7-1.3) and late (RR 0.5, 95% CI, 0.2-1.5) ARM. Similarly, after controlling for age, hyperopia was not associated with incident early (RR 0.9, 95% CI, 0.7-1.1) or late (RR 1.2, 95% CI, 0.6-2.3) ARM. CONCLUSIONS: These prospective population-based data provide no evidence of an association between refractive errors and risk of ARM.  相似文献

The aim of this study is to determine and compare biometric and refractive characteristics of thalassemia major patients and normal individuals. In this cross-sectional study, 54 thalassemia major patients were selected randomly as case group, and 54 age- and sex-matched healthy subjects were regarded as control group. Refractive errors, corneal curvature and ocular components were measured by autokeratorefractometery and A-scan ultrasonography, respectively. Mean spherical equivalent was ?0.0093 ± 0.86 D in thalassemia patients and ?0.22 ± 1.33 D in the normal group. The prevalence of myopia, Hyperopia, and emmetropia among thalassemia patients was 16.7, 19.4, and 63.9 %, respectively. While in the control group, 26.9 % were myopic, 25 % were hyperopic, and 48.1 % were emmetropic. The prevalence of astigmatism in case group was 22.2 %, which was not significantly different from that in control group, (27.8 %, p = 0.346). Mean axial length in thalassemia patients was 22.89 ± 0.70 which was significantly lower than that in normal group (23.37 ± 0.91, p = 0.000). The flattest meridian of the cornea (R1) was significantly steeper in thalassemia patients (7.77 ± 0.24) in comparison to normal individuals (7.85 ± 0.28). Although thalassemic patients had significantly smaller axial length and vitreous chamber depth in comparison to normal group, which could be due to their abnormal physical growth, there was no significant difference between the mean of spherical equivalent among two groups. This can be due to their steeper corneal curvature that overcomes the refractive disadvantage of their shorter axial length.  相似文献

PURPOSE: To examine the relationship between intraocular pressure (IOP), anthropomorphic, demographic, socioeconomic, systemic, and ocular factors and glaucomatous optic neuropathy (GON) in Chinese people. METHODS: Chinese people (n = 2000), aged 40 to 79 years, were selected from the Singapore electoral register. Of the 1717 considered eligible for examination, 1232 participated, representing a response rate of 71.8%. IOP was estimated with Goldmann applanation tonometry. The drainage angle was assessed with static and dynamic gonioscopy. The optic nerve was examined at high magnification through a dilated pupil with a fundus contact lens or a +78-D lens. Static automated visual field testing was performed on subjects with suspected glaucoma. GON was diagnosed on the basis of structural and functional abnormalities of the optic nerve. RESULTS: The main independent determinants of higher IOP were higher systolic blood pressure (P < 0.001), quadrants of any peripheral anterior synechiae (PAS, P = 0.02) and width of the drainage angle (P = 0.049). A 100- micro m increase in corneal thickness was associated with an increase in mean IOP of 1.5 to 1.8 mm Hg (P < 0.001). Odds of GON increased 1.2 times per 1-mm Hg increase in screening IOP. A clear association between corneal thickness and GON was not identified. CONCLUSIONS: Clinical IOP estimates are related to systolic blood pressure and corneal thickness. Variation in IOP with angle width may suggest that trabecular compaction significantly contributes to causes of the increase in IOP, independent of angle-closure. GON is an IOP-related phenomenon among Chinese Singaporeans.  相似文献

PURPOSE: To examine the relationships of diabetes with anterior chamber depth, axial length, and refraction. DESIGN: Population-based cross-sectional study. METHODS: This analysis included 943 Singapore Chinese subjects ages 40 to 81 years from the Tanjong Pagar Survey. Diabetes was determined by self-report. A-mode ultrasound was used to measure axial dimensions. Refraction was performed with an autorefractor and refined subjectively. RESULTS: There were 140 (11.4%) participants with diabetes. After controlling for age and gender, participants with diabetes had shallower anterior chambers (2.78 vs 2.91 mm, P = .004) and thicker lenses (4.88 vs 4.75 mm, P = .003), but similar spherical equivalent refraction (-0.38 vs -0.47 diopters [D], P = .73), axial length (23.09 vs 23.19 mm, P = .40), and vitreous chamber depth (15.52 vs 15.59 mm, P = .53). CONCLUSIONS: In the Singapore Chinese population, persons with diabetes have shallower anterior chambers and thicker lenses than those without diabetes. Diabetes may be a risk factor for angle closure glaucoma (ACG).  相似文献

Many advances have been made in recent years in the treatment of patients with cataracts, particularly in the area of aphakic vision correction. In this study, 60 optometrists were surveyed about their role in the management of patients who have cataracts significantly affecting vision. It was found that over 90% of the optometrists surveyed attempted to educate their patients about cataracts, cataract surgery, and alternatives for aphakic vision correction. However, these optometrists felt least knowledgeable about intraocular lens implants, the most common form of aphakic vision correction used today. It is recommended that the optometrist keep abreast of advances in the fields of cataract management and aphakic vision correction to provide patients with state of the art optometric care.  相似文献

OBJECTIVE: To determine the prevalence rates and causes of low vision, blindness, and patient-assessed deficient visual function among Singaporean Chinese adults. DESIGN: Population-based cross-sectional survey. PARTICIPANTS: Singaporean Chinese adults 40 to 79 years old (n = 1152). METHODS: From an initial sampling frame of 40- to 79-year-old Chinese in the Tanjong Pagar district in Singapore, 2000 subjects were selected using a disproportionate, stratified, clustered, random-sampling method. Of 1717 eligible subjects, 1232 were examined (participation rate = 71.8%), and 80 adults who did not have visual acuity (VA) data were excluded from the analysis. MAIN OUTCOME MEASURES: Bilateral low vision was defined as best-corrected VA (BCVA) worse than 6/18 and 3/60 or better, and bilateral blindness as BCVA worse than 3/60 in the better eye or constriction of the visual field to within 10 degrees of fixation, in accordance with the World Health Organization criteria. Patient-assessed visual function was measured using a modified VF-14 questionnaire. RESULTS: The age- and gender-adjusted prevalence rates were 1.1% (95% confidence interval [CI], 0.6-1.8) for bilateral low vision and 0.5% (95% CI, 0.2-1.1) for bilateral blindness, and the mean visual function score was 98.6. The rates of bilateral low vision and blindness increased with age, whereas visual function scores decreased with age, even after adjusting for gender and education. Cataract accounted for 58.8% of bilateral low vision, 20.0% of bilateral blindness, and 52.0% of poor visual function (score<90). Glaucoma contributed to 60.0% of bilateral blindness. CONCLUSION: The age- and gender-adjusted rates of low vision and blindness were 1.1% and 0.5%, respectively. Glaucoma is a leading cause of blindness in Singaporean Chinese adults, in addition to well-recognized causes in the rest of Asia such as cataract.  相似文献

PURPOSE: To evaluate the relationship of reproductive exposures and incident age-related cataract and maculopathy in women. METHODS: This was a population-based cohort study including all adults 43 to 84 years of age living in Beaver Dam, Wisconsin (a representative midwestern community) who were identified during a census in 1987 to 1988. They were evaluated initially in 1988 to 1990 and at follow-up in 1993 to 1995. Evaluations included medical histories and both fundus and lens photography. All procedures were done according to protocols that were the same at both examinations. All photographs were graded by trained observers using defined grading schemes. The severities of age-related cataracts and maculopathy were determined by grading of photographs. Information on hormone exposures was ascertained from structured interviews. RESULTS: After adjusting for age, the only significant finding for lens end points was a trend indicating a possible protective effect of increasing number of live births and incident posterior subcapsular cataract. There were no significant associations of any reproductive exposure with lesions of early or late age-related maculopathy. CONCLUSIONS: In these population-based data, there is little evidence of association of hormone exposures with incident age-related eye disease in women 5 years later. Longer follow-up of this population, whose mean age is approaching that of heightened incidence, may disclose significant relationships.  相似文献
