Changes in central and coronary haemodynamics and myocardial oxygenation during anaesthesia induction, intubation, skin incision and sternotomy were studied in 24 patients undergoing a coronary artery bypass grafting operation under either high-dose fentanyl or enflurane anaesthesia. The anaesthesia induction caused no changes in the coronary haemodynamics in either group, in spite of a marked decrease in the coronary perfusion pressure during enflurane induction. In both groups a decrease in the coronary sinus blood flow and an increase in the coronary vascular resistance was observed after the intubation. At this stage, two patients in both groups had a low level of myocardial lactate extraction, indicating possible myocardial ischaemia; mean myocardial lactate extraction had decreased significantly from the awake level in both groups. A circulatory response to surgical stimulation was seen in both groups, although it could be somewhat better controlled during enflurane anaesthesia. In the enflurane patients the increase in myocardial oxygen demand was accompanied by increased coronary flow, while in the fentanyl group the increase in coronary flow was not in proportion to the increased oxygen demand, and an increase in myocardial oxygen extraction was also seen. During surgery, three fentanyl patients and one enflurane patient had a low level of myocardial lactate extraction.  相似文献

A randomized, prospective, comparative study was performed to evaluate induction characteristics, haemodynamic changes and recovery in 60 ASA I-II patients undergoing mainly gynaecological laparotomies with either propofol or thiopentone-enflurane anaesthesia. The propofol group (n = 30) received 2 mg.kg-1 propofol for induction of anaesthesia followed by propofol infusion. The thiopentone-enflurane group (n = 30) received thiopentone 4 mg.kg-1 for induction followed by enflurane (0.5-2 per cent). All patients received nitrous oxide (66 per cent] in oxygen begun one minute after tracheal intubation, and fentanyl (1.5 micrograms.kg-1) four minutes prior to induction. Other drugs administered during or after anaesthesia were similar among the groups. Haemodynamic measurements were similar between propofol and enflurane groups except after tracheal intubation when the mean arterial pressure was lower in the propofol group (P less than 0.05). The propofol group had significantly less (P less than 0.01) emesis in the recovery room than the enflurane group. The propofol group experienced significantly less (P less than 0.05) dizziness, depression/sadness and hunger than the enflurane group in the postoperative period as assessed with a visual analogue questionnaire. We conclude that propofol provided better outcome than enflurane in terms of these nonvital but annoying outcome measures after relatively long intra-abdominal operations.  相似文献

We have studied the effects of a 30-min infusion of propofol 6, 9, 12, 15, 18 and 21 mg kg-1 h-1 on cardiovascular haemodynamics, coronary circulation and myocardial metabolism in 12 mongrel dogs. Mean plasma concentrations of propofol after infusion of 6 and 21 mg kg-1 h-1 increased from 2.9 (SEM 0.3) to 11.5 (0.1) micrograms ml-1. Propofol produced a progressive decrease in arterial pressure. Heart rate tended to decrease at 15, 18 and 21 mg kg-1 h-1 and cardiac index decreased significantly at infusion rates > or = 9 mg kg-1 h-1. Systemic vascular resistance tended to increase except at 21 mg kg-1 h-1 and left ventricular systolic and diastolic function were depressed. Both coronary sinus blood flow and myocardial oxygen consumption decreased in parallel with a decrease in left ventricular minute work index without producing lactate. Propofol produced progressive decreases in coronary blood flow and myocardial oxygen consumption but did not exert adverse effects on the coronary circulation.   相似文献

This study was performed to compare the incidence of prebypass myocardial ischemia in patients receiving fentanyl and enflurane for anesthesia along with either pancuronium or vecuronium. Ninety-eight patients with normal left ventricular function were randomly allocated to receive either pancuronium 0.15 mg.kg-1 or vecuronium 0.15 mg.kg-1 in a double-blind manner after fentanyl 40 micrograms.kg-1 for induction of anesthesia for elective coronary artery bypass grafting (CABG). Premedication included diazepam 0.15 mg.kg-1 po, morphine 0.10 mg.kg-1, and scopolamine 0.005 mg.kg-1 im. Two lead Holter monitor recordings (leads V6 and V9) from the time of arrival in the operating suite to institution of cardiopulmonary bypass were analyzed for ischemia by a cardiologist blinded to the choice of muscle relaxant. Intraoperatively, heart rates greater than 90 beats.min-1 and systolic blood pressure +/- 20% of ward values were treated with propranolol, enflurane, or phenylephrine. Nitroglycerin was infused for ECG signs of ischemia or pulmonary hypertension. After induction of anesthesia the heart rate and cardiac index were consistently decreased in patients receiving vecuronium and also lower in these patients compared with those receiving pancuronium. Thirty-two per cent of patients receiving pancuronium received propranolol for heart rates greater than 90 beats.min-1 versus 7% of those who received vecuronium (P approximately 0.01). Eight patients developed 13 episodes of ischemia after administration of the muscle relaxant: four who received pancuronium (n = 44; 9%) and four receiving vecuronium (n = 54; 7%). Four episodes occurred at induction or tracheal intubation, two in each group. A prospective study has been undertaken to compare a new intravenous anaesthetic agent, propofol, to methohexitone in 40 ASA I or II patients aged between 18 and 50 years undergoing maxillo-facial surgery and divided into two groups. Intramuscular premedication was standardized for all patients. In group I, propofol 2 mg X kg-1 was injected over 1 min in a peripheral venous line with fentanyl 0.86 microgram X kg-1, followed by an infusion of propofol 5 mg X kg-1 X h-1 and fentanyl 3 micrograms X kg-1 X h-1. In group II, the fentanyl dosage was the same as in group I, whilst methohexitone 3 mg X kg-1 was given for induction and 4.5 mg X kg-1 X h-1 for maintenance of anaesthesia. The following were recorded during induction, maintenance and recovery; haemodynamic parameters using a non invasive method; respiratory parameters; quality of anaesthesia; side-effects. Statistical analysis was performed using the Student t test and qualitative analysis using the Schwartz comparison test at 2%. The following results were found: the quality of anaesthesia with propofol was superior to that of methohexitone during the three stages of anaesthesia. The duration of induction was similar in both groups, but the quality of induction (occurrence of more minor side-effects; p less than 0.05) and intubation was in favour of propofol (p less than 0.05). 目的 观察舒芬太尼在非体外循环冠状动脉搭桥手术应用中的安全性和有效性.方法 择期行OPCABG病人54例,被随机分成两组,舒芬太尼组和芬太尼组,每组27例,麻醉诱导应用丙泊酚1 mg/ks~2 ms/ks,同时分别静注舒芬太尼0.5 μg/ks~1μg/kg,或芬太尼4 μg/ks-8 μg/ks,同时吸入安氟醚维持麻醉,持续输注舒芬太尼0.08 μg·ks-1min-1,或芬太尼0.6 μg·ks-1·min-1.记录各组气管插管、切片、麻醉维持和拨除气管导管各时间的收缩压和舒张压.结果 在气管插管过程中,浅麻醉反应的病人数舒芬太尼组明显低于芬太尼组,在气管插管切皮,麻醉维持和拔管期间,芬太尼组收缩压、舒张压明显高于舒芬太尼组,术后清醒和拔管时间两组病人无统计学差异.结论 舒芬太尼的麻醉效果优于芬太尼,舒芬太尼能提供术中更稳定的血液动力学.  相似文献

The effects of propofol during low flow (1.6 l.min-1.m-2) hypothermic (27-29 degrees C) cardiopulmonary bypass on systemic haemodynamic and metabolic variables were studied in 20 patients in a parallel group comparison. Patients in the control group underwent two consecutive control periods, whereas those in the propofol group underwent a control period followed by a 'propofol' period during which they received an intravenous bolus of propofol 5 mg.kg-1 followed by an infusion at a rate of 20 mg.kg-1.h-1. Haemodynamic and metabolic variables were measured at the end of each period. The propofol group showed a significant reduction in both systemic vascular resistance (p less than 0.001) and systemic oxygen uptake (p less than 0.05). There was a small but significant fall in lactate in the control group (p less than 0.02); however, there was no significant change in the propofol group. These findings could indicate impaired tissue perfusion, although they more likely indicate systemic metabolic depression with washout of lactate from previously hypoperfused tissues.  相似文献

The effects of enflurane (end-tidal concentration 0.7%) on central and coronary hemodynamics and myocardial oxygenation were studied during steady state, high-dose fentanyl anesthesia in ten patients undergoing coronary artery bypass grafting operations. Compared with the response in ten patients receiving the same fentanyl anesthesia (100 micrograms/kg) without enflurane supplementation, enflurane caused a moderate reduction in mean arterial pressure, systemic vascular resistance, and left ventricular stroke work index. No patient showed signs of myocardial ischemia, and mean coronary sinus flow and calculated coronary resistance remained unchanged. Surgical stimulation induced no central or coronary hemodynamic responses in the enflurane-fentanyl group. No coronary hemodynamic changes occurred in the fentanyl group, but a marked increase in arterial pressure and systemic vascular resistance was seen. Myocardial oxygen extraction decreased in the enflurane supplemented group although it increased in the fentanyl group after surgical stimulation. Three fentanyl group patients and one enflurane-fentanyl group patient had a low myocardial lactate extraction as a sign of myocardial ischemia during surgery. We conclude that a 0.7% enflurane supplementation of 100 micrograms/kg fentanyl anesthesia does not endanger myocardial oxygenation and effectively prevents central and coronary hemodynamic responses to skin incision and sternotomy in patients undergoing coronary artery surgery.  相似文献

This study was designed to evaluate airway and intubating conditions without muscle relaxants after administration of fentanyl and propofol in 55 patients aged 20-60 years for elective surgery. Patients were randomly assigned to one of four groups to receive fentanyl 0, 2, 3, or 4 micrograms.kg-1, respectively. Three minutes after the administration of fentanyl, propofol (2 mg.kg-1) was given for induction of anesthesia. After the loss of consciousness, laryngoscopy and tracheal intubation, supplemented with topical anesthesia of lidocaine (2 mg.kg-1), were attempted. In control group, without administration of fentanyl, all patients were judged to provide poor intubating conditions. Increasing doses of fentanyl reduced the incidences of movement and persistent coughing on laryngoscopy and intubation in a dose-related manner. However, visualization of the vocal cord was significantly. (P < 0.05) more likely to be impossible in patients in 4 micrograms.kg-1 fentanyl group (40%) compared with patients in 2 micrograms.kg-1 fentanyl group (7%). There were no significant differences among groups receiving fentanyl with respect to vocal cord position. The vocal cords were closed in 26% of patients receiving fentanyl and propofol for intubation. Tracheal intubation without muscle relaxants is not recommended because of the potential unacceptable intubating conditions.  相似文献

OBJECTIVE: To determine the influence that bispectral index (BIS) monitoring of hypnosis might have on need for analgesia during surgery under total intravenous anesthesia provided by bolus administration of fentanyl. PATIENTS AND METHOD: Prospective, randomized and partially double-blind study of 40 patients undergoing major gynecological surgery under total intravenous anesthesia with propofol and fentanyl. In the BIS group (n = 20) propofol administration was adjusted to maintain BIS between 40 and 60. In the control group (n = 20) standard doses were given: 10 mg/kg-1/h-1 after anesthetic induction and for 5 minutes, 8 mg/kg-1/h-1 over the next 5 minutes and 6 mg/kg-1/h-1 throughout the rest of the operation. All patients received intravenous bolus administration of 150 or 75 microg of fentanyl to maintain analgesia whenever systolic blood pressure and heart rate increased 20% over baseline. We compared propofol and fentanyl requirements, intraoperative changes in BIS, and awakening from anesthesia. RESULTS: Patient and surgical characteristics were similar in both groups. BIS monitoring allowed propofol administration to be decreased a mean 24% during maintenance of anesthesia, and this in turn was associated with a significant increase in mean dose of fentanyl (415 microg versus 253 microg in the BIS and control groups, respectively; p = 0.01). Mean values of BIS were higher in the BIS group (46.4 versus 42.2; p = 0.04) and patients in the BIS group awoke sooner (in 7.7 min versus 11.1 min; p = 0.01) and tended to report less pain upon arrival at the postanesthetic recovery room, although the difference was not statistically significant. CONCLUSIONS: BIS monitoring of depth of hypnosis can influence requirements for fentanyl during total intravenous anesthesia by bolus dosing for maintenance of analgesia. This is probably due to changes in the administration of propofol made possible by BIS monitoring.  相似文献

So as to compare the anaesthesia obtained using propofol with that obtained using propanidid, 40 ASA I patients, aged between 18 and 50 years, who were to undergo elective orthopaedic or plastic surgery lasting more than 60 min, were randomly divided into two equal groups, one receiving propofol (PF) and the other propanidid (PD). All the patients received 0.5 mg atropine, 100 mg pethidine and 7.5 mg droperidol (10 mg if weight greater than 60 kg) intramuscularly 45 min before induction. Patients in group PF were then given 2 mg.kg-1 propofol over 1 min and 0.9 microgram.kg-1 fentanyl over 3 min, followed by a constant rate infusion of 5 mg.kg-1.h-1 propofol and 3 micrograms.kg-1.h-1 fentanyl. For PD patients, the doses of fentanyl were identical; they were given 10.6 mg.kg-1 propanidid over 3 min for induction, and 37 mg.kg-1.h-1 for maintenance. All the patients were intubated and ventilated mechanically. The usual anaesthetic parameters were monitored at induction, during surgery, and during recovery. Consciousness was lost more quickly with propofol (p less than 0.05), but the corneal reflex returned more rapidly in group PD (p less than 0.02). The time required for a full return to normal consciousness was identical in both groups. The fall, during induction, and the increase, during recovery, of Pasys were greater in group PD (p less than 0.05 and less than 0.001 respectively). Unbound, rather than total, plasma concentrations may be related to the anaesthetic action of propofol. Therefore, we measured plasma concentrations of propofol and recorded Nb wave latencies of auditory evoked potentials (AEP) during continuous infusion of propofol in 15 patients undergoing coronary artery bypass grafting (CABG) surgery. After induction of anaesthesia with fentanyl, propofol was infused continuously at a rate of 10 mg kg-1 h-1 for 20 min, and then the rate was reduced to 3 mg kg-1 h-1. Administration of heparin before cardiopulmonary bypass (CPB) did not affect total or unbound propofol concentration. Initiation of CPB decreased mean total propofol concentration from 2.6 to 1.7 micrograms ml-1 (P < 0.01). Simultaneously, mean unbound propofol concentration remained at 0.06 micrograms ml-1 because of a slight increase in the mean free fraction of plasma propofol (from 2.3 to 3.5%; P > 0.05). During hypothermic CPB, mean total propofol concentration increased to concentrations measured before bypass (to 2.1 micrograms ml-1; P > 0.05 vs value before CPB) and the mean unbound propofol concentration was at its highest (0.07 microgram ml-1; P < 0.05 vs value before heparin). After CPB and administration of protamine, the mean total propofol concentration remained lowered (1.7 micrograms ml-1; P < 0.05 vs value before heparin) and the mean unbound propofol concentration returned to the level measured before heparin (P < 0.001 vs value during hypothermia). The latency of the Nb wave from recordings of AEP increased after induction of anaesthesia, reached its maximum during hypothermia and was prolonged during the subsequent phases of the study. The latency of the Nb wave did not correlate with total or unbound propofol concentration. We conclude that the changes in total and unbound concentrations of plasma propofol were not parallel in patients undergoing CABG. During CPB or at any other time during the CABG procedure, the unbound propofol concentration did not decrease and Nb wave latency was prolonged compared with baseline values measured after induction of anaesthesia before the start of CPB.   相似文献

This study was designed to assess recovery from total intravenous anaesthesia with propofol for short ENT procedures. Twenty-six patients (ASA I and II) were assigned to two groups of thirteen: one breathed air (Laser laryngeal microsurgery), the second N2O-O2 (FIO2 : 0.5) (various ENT procedures). The induction sequence was exactly the same for both groups: oral premedication with 10 mg diazepam one hour before surgery, I mg pancuronium bromide, 2 micrograms X kg-1 fentanyl, denitrogenation within 3 min, after which propofol was delivered (2.5 mg X kg-1). When the eye-lash reflex had disappeared (time recorded), 1.5 mg X kg-1 suxamethonium was given and laryngotracheal intubation carried out. A continuous infusion of propofol (9 mg X kg-1 X h-1) was started. Surgery began 5 +/- 2 min after the start of propofol infusion. The durations of anaesthesia, surgery and propofol infusion were similar in both groups. To have good surgical conditions, it was necessary to give repeated doses of propofol for 15 patients. Thus, the total dose of propofol was significatively different between the two groups: 24.5 +/- 6.7 mg X kg-1 X h-1 in group "air" versus 16 +/- 3.6 mg X kg-1 X h-1 in group "N2O-O2" (p less than 0.001). Extubation occurred within 16 +/- 8 min in group "air", being more rapid in group "N2O-O2" (11 +/- 9 min; no significant difference). In combination with fentanyl, propofol was compared with etomidate for total intravenous anaesthesia in 21 women (ASA Grades I-II) admitted for elective hysterectomy. They received either propofol (bolus 1.5 mg kg-1, infusion 9 mg kg-1 h-1 for 10 min thereafter 6 mg kg-1 h-1) or etomidate (bolus 0.10 mg kg-1, infusion 3 mg kg-1 h-1 reduced to 0.6 mg kg-1 h-1). Fentanyl 10 micrograms kg-1 was given for induction followed by an infusion of 30 micrograms kg-1 h-1 for 10 min reduced to 6 micrograms kg-1 h-1 for the first hour and successively reduced over time. Induction was smooth and maintenance easy to manage in both groups. There was no difference in time from end of infusion until extubation, but the time until the patients could report their date of birth was significantly shorter in the propofol group. Nausea and vomiting were more pronounced in the etomidate group, and mental side-effects were only seen after etomidate. After 3 months, more patients in the etomidate group complained of reduced power of concentration. We conclude that total intravenous anaesthesia with either propofol or etomidate is equally easy to manage, but in the recovery situation propofol was advantageous in time and quality.  相似文献

We propose the use of an intravenous propofol/propofol auto-co-induction technique as an alternative to propofol/midazolam for induction of anaesthesia. We have studied 54 unpremedicated ASA 1 or 2 patients undergoing day-stay anaesthesia for minor orthopaedic surgery. All received 10 micrograms.kg-1 or alfentanil before induction, followed by either midazolam 0.05 mg.kg-1, propofol 0.4 mg.kg-1 or saline, and 2 min later, a propofol infusion at a rate of 50 mg.kg-1.h-1 until loss of eyelash reflex. We compared pre- and postinduction haemodynamic changes, complications at insertion of a laryngeal mask airway and recovery from anaesthesia in the three groups. Both co-induction techniques showed less postinduction hypotension and significant reduction of the total induction dose of propofol when compared to the control group. In the propofol/propofol group there was a decreased incidence of apnoea during induction of anaesthesia. These patients were discharged from hospital 2 h after the end of anaesthesia whereas patients in the midazolam/propofol group were discharged after 2 1/2 h (p < 0.001).  相似文献

Total intravenous anesthesia with propofol, fentanyl and ketamine (PFK) was given to two patients complicated with myotonic dystrophy. Case-1: A 42-year-old female underwent a hemithyroidectomy. Anesthesia was induced slowly with intravenous ketamine 20 mg and propofol 60 mg. Her tracheal intubation was performed smoothly without any muscle relaxants. Anesthesia was maintained with propofol infusion of 5 mg.kg-1.h-1, ketamine infusion of 0.3 mg.kg-1.h-1 and fentanyl 200 micrograms in total. She regained consciousness 20 minutes after the end of propofol infusion, and 15 minutes later, her trachea was extubated without any troubles. Case-2: A 41-year-old female underwent a removal of left parotid tumor. Anesthesia was induced slowly with ketamine 40 mg and propofol 100 mg intravenously. Anesthesia was maintained with propofol infusion of 5-10 mg.kg-1.h-1, ketamine infusion of 0.5 mg.kg-1.h-1 and fentanyl 350 micrograms in total. No muscle relaxant was used through the surgical procedure. Emergence from anesthesia was observed 10 minutes after the end of propofol infusion and her trachea was extubated. When a nasogastric tube was pulled out, her respiration stopped suddenly and she was intubated again only for two hours without any troubles. In both cases their serum CPK levels and rectal temperatures were very stable. PFK method would be a choice for patients with myotonic dystrophy.  相似文献

目的 探讨吗啡-芬太尼复合麻醉下非体外循环下冠状动脉旁路移植术患者的转归.方法 拟在非体外循环下行冠状动脉旁路移植术的患者72例,年龄41~64岁,随机分为2组(n=36):芬太尼复合麻醉组(F组)和吗啡-芬太尼复合麻醉组(MF组).静脉注射芬太尼10~20μg/kg、咪达唑仑0.1 mg/kg、依托咪酯0.3 mg/kg和维库溴铵0.1 mg/kg进行麻醉诱导,气管插管后行机械通气.麻醉维持:MF组静脉输注吗啡0.15 mg·kg-1·h-1+芬太尼8μg·kg-1·hZ-1,F组静脉输注芬太尼10μg·kg-1·h-1,2组均吸入0.5%~2.0%异氟烷,间断静脉注射维库溴铵4 rag.2组术后进行静脉镇痛,MF组和F组分别静脉输注吗啡0.75 mg/h吗啡和芬太尼10μg/h.分别于术前(基础状态)、术后1、2、3 d进行恢复质量评分(QoR评分).于拔除气管导管后15 min、4 h、24 h时进行Ramsay镇静评分.记录术后不良反应的发生情况.结果 与F组比较,MF组术后QoR评分升高,术后发热发生率降低(P<0.05或0.01),Ramsay镇静评分和其他不良反应的发生率差异无统计学意义(P>0.05).结论与芬太尼复合麻醉比较,吗啡-芬太尼复合麻醉有利于非体外循环下冠状动脉旁路移植术患者预后.  相似文献

The recovery times and the state of postoperative awakening were studied after anaesthesia with propofol or enflurane. The forty patients who were to undergo surgery for cold thyroid nodules were divided into two homogeneous groups which were similar in age, sex, weight and length of surgery. In one group, induction was carried out with propofol, fentanyl, pancuronium bromide, and maintained with 9 mg X kg-1 X h-1 propofol. In the other group, this was done with thiopentone, fentanyl, pancuronium bromide, followed by 0.8% enflurane. Repeat injections of fentanyl were given as required. The time between stopping giving the anaesthetic drug and eye opening was measured, as well as the performance in memorizing and sorting numbers; these two tests were carried out before and 2 h after surgery. The overall results did not show any differences between the two groups in recovery time or quality of awakening. In the propofol group, recovery time and quality were better than in the enflurane group if the patient was less than 50 years old or surgery lasted less than 100 min. In the propofol group, there were significant correlations between recovery time and age (r = 0.83), and between recovery time and duration of surgery (r = 0.87). The doses of this new product should therefore be modified according to the age of the patient and the length of surgery.  相似文献

We studied 114 female patients (ASA 1 or 2) who were within 20% of ideal body weight and who were scheduled to undergo gynaecological laparoscopy which required supplementation with an opioid (groups IA and PA), or dental procedures which did not require opioid supplementation (groups IO and PO). A computerised package of psychomotor tests was performed before surgery. Anaesthesia was induced with propofol 2.5 mg.kg-1 and all patients received atracurium 0.3 mg.kg-1 and 67% nitrous oxide in oxygen. Patients in group IA received isoflurane 1% (inspired), and alfentanil 10 micrograms.kg-1 as a bolus and 10 micrograms.kg-1.h-1 as an infusion. Patients in group PA received propofol 9 mg.kg-1.h-1 as an infusion, decreasing to 6 mg.kg-1.h-1 after 15 min, together with alfentanil 10 micrograms.kg-1.h-1. Patients in groups IO and PO received isoflurane and propofol in the regimens described for groups IA and PA, but without alfentanil. Recovery was assessed by a blinded observer who recorded times to awakening (eye opening) and orientation (giving date of birth), and who repeated the psychomotor tests at 1, 3 and 5 h. Linear analogue scales of mood, nausea and pain were obtained and other side effects were noted in the succeeding 48 h. A matched control group of 25 females (who were not anaesthetised) underwent psychomotor testing on four occasions in order to assess the 'learning effect' of repeated recovery testing. A 57-year-old female patient underwent combined off-pump coronary artery bypass grafting and abdominal aortic aneurysm replacement. Anesthesia was maintained with propofol, fentanyl, and thoracic epidural anesthesia. Propofol doses were adjusted to maintain bispectral index (BIS) between 40-60. Despite the remarkable hemodynamic changes, BIS remained stable at about 50 during the surgery. The average dose of propofol was 3.3 mg.kg-1.hr-1. The patient awoke an hour after the surgery and was extubated 1.5 hours thereafter. This case report suggests that BIS is a useful index to determine the depth of anesthesia during surgeries which induce marked hemodynamic changes.  相似文献
