Puerperal ovarian vein thrombosis commonly originates from purulent necrotic endomyometritis. The incidence is published to be 1 to 600 deliveries. According to the puerperal uterine drainage, the predominant location is the right ovarian vein in 90% of all cases. The leading symptoms are lower abdominal pain, fever and leucocytosis. Discrepancy between the given clinical picture and the insignificant findings on gynaecologic examinations is common.  相似文献

BACKGROUND: Postpartum ovarian vein thrombosis is a rare diagnosis with a significant associated risk of pulmonary embolism. The mainstay of therapy consists of intravenous antibiotics and anticoagulation. CASE: A 30-year-old woman presented with fever and abdominal pain 3 days after cesarean section and was found to have an ovarian vein thrombosis on computed tomography. Given her history of cerebral arteriovenous malformation and the attendant risk for cerebral hemorrhage with the use of anticoagulation, she was successfully treated with a removable suprarenal inferior vena cava (IVC) filter and intravenous antibiotics. CONCLUSION: For patients who fail to respond to anticoagulation alone or in whom anticoagulation is contraindicated, placement of a retrievable suprarenal IVC filter is a viable treatment option in the setting of postpartum ovarian vein thrombosis.  相似文献

BACKGROUND: Deep vein thrombosis is a rare complication of myoma uteri. There have been few reports on this association. CASE: A 42-year-old woman presented with a myoma uteri, 24 weeks in size, that had compressed the pelvic veins and caused unilateral deep vein thrombosis of the left leg. After anticoagulation therapy, a total abdominal hysterectomy was performed, with an uneventful recovery. CONCLUSION: Although deep vein thrombosis associated with myoma uteri is a rare event, it can be managed successfully with anticoagulants and hysterectomy. Myoma uteri with deep vein thrombosis is an indication for hysterectomy.  相似文献

A case of renal vein thrombosis following puerperal ovarian vein thrombophlebitis is reported. We review the syndrome of puerperal ovarian vein thrombophlebitis and emphasize the potential for renal vein and vena caval involvement. The utility of computed tomographic scanning for diagnostic confirmation of this postpartum complication is described.  相似文献

BACKGROUND: Echinococcal disease, although known to occur in most organs and body areas, is extremely rare in the female reproductive system. A literature search revealed only three anecdotal non-English language case reports of ovarian echinococcosis. CASE: A 20-year-old woman presented with epigastric pain. Abdominal ultrasonography and computed tomography showed three multicystic masses, one intrahepatic, one intraperitoneal and the third situated in the left ovary. Serologic examination confirmed the diagnosis of echinococcal disease. Marked regression of all the cysts occurred following a short course of albendazole. CONCLUSION: Echinococcal cysts should be included in the differential diagnosis of multicystic ovarian lesions if the patient has cysts elsewhere and is from an endemic area.  相似文献

BACKGROUND: Even low-voltage electric shock can initiate late vascular thromboses. We present the first reported case of antenatal deep vein thrombosis (DVT) in a pregnant woman that could be attributed to the electric shock she accidentally received several weeks prior to the DVT. CASE: A 19-year-old primigravida presented to the office at 29 weeks of gestation with signs and symptoms of DVT. A magnetic resonance imaging revealed an occluding thrombus involving the entire left common iliac and external iliac vein. She tested negative for all the biochemical abnormalities that could lead to a hypercoagulable state. Her prior pregnancy course had been unremarkable with the exception of an electric shock she had had secondary to touching exposed electric wires at home at 22 weeks of gestation. CONCLUSION: Pregnant women exposed to electric shock should be carefully monitored for such complications as DVT during the weeks following the exposure.  相似文献

A nonpuerperal patient with Crohn's disease and pelvic pain was incidently diagnosed with ovarian vein thrombosis. The patient's laboratory data were negative for an inherited hypercoagulopathy state. Ovarian vein thrombosis is a possible cause of pelvic pain in patients with Crohn's disease. Modern imaging technology has made it easier to diagnose the once-elusive ovarian vein thrombosis.  相似文献

ObjectiveWe present a case who developed ovarian vein thrombosis (OVT) after laparoscopic-assisted vaginal hysterectomy with bilateral salpingectomy to share our experience.Case reportA 46-year-old woman came to our hospital due to severe dysmenorrhea induced by adenomyosis. Medical treatments were given but with unsatisfactory effect. As the patient had completed family planning, a hysterectomy was scheduled. However, on the sixth postoperative day, the patient complained of low abdominal pain with fever on and off. After a series of examinations, right OVT was diagnosed. The patient was treated with antibiotics only. Under close surveillance, the OVT resolved spontaneously, and the patient was discharged.ConclusionDiagnosis of OVT requires highly suspicion owing to its rarity and non-specific presentation. OVT is a potentially serious venous thromboembolism that sometimes can be life threatening. Anticoagulant treatment is still controversial. Conventional Tomography with contrast medium could detect early OVT with high sensitivity and specificity.  相似文献

Ovarian vein trombosis (OVT) is a pathologic entity classically considered as a postpartum complication and only rarely associated with other diseases. Due to its vague symptoms, it is usually underdiagnosed. However its consequences can be fatal. We report a case of an incidental finding of ovarian thrombosis in an asymptomatic 45-year-old woman who underwent surgery due to the ultrasonographic finding of a para-ovarian cyst.  相似文献

Ovarian vein thrombosis (OVT) is a rare complication of pregnancy. However, recognition and treatment is critical because a delay in diagnosis could lead to significant maternal morbidity. The diagnosis of OVT remains a challenge because there is no known profile of risk factors. Current controversies concern radiologic diagnosis, appropriate treatment strategies including antibiotics and anticoagulation, treatment duration, and testing for thrombophilias. TARGET AUDIENCE: Obstetricians & Gynecologists, Family Physicians. LEARNING OBJECTIVES: After completion of this article, the reader should be able to explain that even though the occurrence of postpartum ovarian vein thrombosis (OVT) is rare the physician must consider it in a differential diagnosis in a patient with postdelivery fever unresponsive to antibiotics, state that missing the diagnosis can have devastating consequences, and recall that at times it is difficult to differentiate from septic thrombophlebitis.  相似文献
