目的 总结基于住院医师规范化培训的神经外科专业硕士研究生临床带教体会。 方法 在带教老师转变带教观念的基础上,对2013-2015年在首都医科大学附属北京天坛医院神经 外科六、七病区进行规范化培训的专业型硕士综合运用多种教学方法,包括通过直接“呈现”的方式 开展医德教育,通过“实践”和“强化”的方式进行临床工作作风和临床实践能力的培养,以及通过 “发现”的方式培养研究生的临床科研能力。 结果 研究期间参加规范化培训的专业型硕士累计20人;期间,其研究生临床能力考核、执业医师 资格考试通过率均达到100%;在教学主管部门对带教研究生的反馈问卷中,关于老师带教意识、带 教能力及效果方面,满意率均为100%。 结论 通过综合运用上述4种教学方式,有助于参加规范化培训的专业型硕士在临床以及科研能力 的提升。  相似文献

目的 对北京市神经内科住院医师规范化培训结业技能考核成绩的影响因素进行分析,探讨脑血 管病住院医师培训方案的改进。 方法 收集2017-2020年北京市神经内科住院医师规范化培训临床结业技能考核成绩,对技能考 核是否通过,以及辅助检查、病例考核、技能操作及人文沟通等分项成绩的可能影响因素进行多因 素分析。 结果 二元logistic回归分析结果显示,性别、学历、专业基地规模是技能考核通过与否的影响因素。 多元逐步回归分析结果显示,性别、专业基地规模、学员类型、学历是辅助检查成绩的影响因素;专业 基地规模是影响病例考核成绩的影响因素;学员类型是技能操作成绩的影响因素。 结论 女性、大的培训基地学员及博士学历学员通过住院医师规范化培训结业技能考核率更高。 在制订脑血管病学员培养方案时,应充分考虑人员特点,针对其短板进行重点培训,并努力提高专业 基地的质量和同质性。  相似文献

The structure of a three tiered system of training for human service workers within New Zealand is described as well as the teaching and facilitation methods used to deliver the content of the courses within a tertiary training environment. Underlying the training courses are the three themes of: the creation of valued lifestyles for people with disabilities; a commitment to biculturalism; and the development of effective communication skills. Heightened public awareness of autism and increased prevalence estimates of autism spectrum disorders (ASDs) has generated a sense of urgency within the public school system to identify children with these disorders for targeted intervention. Two multidisciplinary groups of professionals, one each from two separate school districts, were identified and trained to provide diagnostic and consultative services. This paper outlines a model process for school personnel to develop a basic level of training and competence in recognizing and serving students who have an ASD by (1) providing an overview of the legal and clinical issues involved in screening for children with ASD within the school system, (2) defining a school-based professional training process and (3) outlining a school-based ASD screening process.  相似文献

The authors conducted an assessment of mental health training, consultation, and research needs in Florida. The purpose was to document the current training, consultation, and information needs within the mental health care delivery system. The data collection included a six-page, self-administered mail questionnaire and a structured telephone interview of selected key informants throughout the state. The questions on the survey instrument asked for background information on the respondents as well as their training and research priorities for mental health services. Accountability procedures, financing issues, and clinical diagnostic and assessment techniques were among the most frequently rated priorities for training and research. Results are discussed with emphasis on the statewide dissemination of study findings, a national standardization of the needs assessment process, and the importance of systematic decision-making for administrators in the mental health service delivery system.  相似文献

Despite the importance of establishing shared scoring conventions and assessing interrater reliability in clinical trials in psychiatry, these elements are often overlooked. Obstacles to rater training and reliability testing include logistic difficulties in providing live training sessions, or mailing videotapes of patients to multiple sites and collecting the data for analysis. To address some of these obstacles, a web-based interactive video system was developed. It uses actors of diverse ages, gender and race to train raters how to score the Hamilton Depression Rating Scale and to assess interrater reliability. This system was tested with a group of experienced and novice raters within a single site. It was subsequently used to train raters of a federally funded multi-center clinical trial on scoring conventions and to test their interrater reliability. The advantages and limitations of using interactive video technology to improve the quality of clinical trials are discussed.  相似文献

Thanks to an ever increasing array of sophisticated techniques, neuroimaging has become an integral part of clinical neurology and a major tool in the neurosciences. Because, undoubtedly, the role of neuroimaging will grow even further, a Task-force on Neuroimaging was created by the EFNS in June 1996 to assess how academic neurology in Europe should adapt to such rapid changes. The present report summarizes the conclusions reached by this task-force. After a brief survey of the current optimal use and potential developments of neuroimaging in academic neurology, a set of recommendations and guidelines are proposed, which can be summarized as follows:

An open system model, constructed with concepts from general systems theory and cybernetics, is presented and applied to the psychotherapy process and to the evaluation of patient change. It is asserted that the patient-therapist system and the patient's social system operate interdependently, exchanging mutual causal effects. To initiate change the therapist must introduce negative feedback within a range of patient toleration. Once change is initiated, the crucial determinant is the response by the social system to the input of new information. Thus patient change is a function of both the therapist and the social system. Recovery from hemiparesis after stroke largely depends on widespread functional and structural neuronal plasticity. Based on animal studies of rehabilitative and pharmacological interventions on recovery, recent neuroscientific findings suggest that new rehabilitative interventions may also have benefits by modulating neuroplastic mechanisms in stroke patients. Neurofunctional imaging methods such as Positron Emission Tomography (PET) and functional Magnetic Resonance Imaging (fMRI) as well as transcranial magnetic stimulation (TMS) now permit direct studies of training-induced plasticity in stroke patients. Prospectively conducted studies using these techniques have investigated how plasticity relates to recovery of motor function and how intensive training modulates cortical reorganization. Despite technical and methodological differences, consistent neuroanatomic findings on training-induced reorganization of the motor system have been reported. These discoveries have important implications for the future development of neurorehabilitative training methods. With continuously improving methods to study cortical reorganization and over 100 randomized clinical trials on stroke rehabilitation published during the past five years, this promises to be an exciting time in stroke rehabilitation research.  相似文献
