BackgroundMorel-Lavallée lesions, also known as an internal degloving injuries, occur hours to months after high-speed shearing trauma, usually in the peri-trochanteric region. These are uncommon injuries, and are often missed as part of the trauma examination. Failure to diagnose or treat these lesions may result in complications, such as infected seromas, chronic cosmetic deformities, capsule formation, or skin necrosis. There are no formalized societal guidelines for management, but smaller studies have recommended compression alone for asymptomatic lesions, aspiration for small symptomatic lesions, and open debridement for large lesions.Case ReportA young woman presented with swelling, fluctuance, and paresthesia to her right hip after falling off her bicycle 1 week earlier. Physical examination showed a fluctuant and hypoesthetic area over the greater trochanter and point-of-care ultrasound showed a hypoechoic and compressible fluid collection between a fascial layer and a subcutaneous layer, confirming the diagnosis of a Morel-Lavallée lesion (internal degloving injury). Symptoms did not improve with compression alone, but did improve after fluid aspiration.Why Should an Emergency Physician Be Aware of This?Morel-Lavallée lesions are frequently missed traumatic injuries. Morel-Lavallée lesions can be diagnosed quickly and cost-effectively in the emergency department through the combination of a thorough history, physical examination, and bedside ultrasound. Although there are no formal societal guidelines, limited studies suggest management strategies, including compression, aspiration, and open debridement, with treatments varying by symptom severity and lesion size. 相似文献

We aimed to compare contrast-enhanced–guided liver biopsy (CEUSLB) and ultrasound-guided liver biopsy (USLB) in the diagnosis of focal liver lesions (FLLs) developed on a background of advanced chronic liver disease (ACLD). Between 2011 and 2019, patients diagnosed with liver tumors on a background of ACLD were evaluated for inclusion in the study. Patients were randomly assigned to the CEUSLB or USLB group. In total, 144 patients were randomly assigned to either CEUSLB (n = 79) or USLB (n = 65). Overall, in the CEUSLB group, the sensitivity was significantly better (94.74% vs. 74.6%, respectively; p = 0.001). Both the fragment length of the biopsy specimen and the single puncture success rate were statistically higher in the CEUSLB group (p = 0.022 and p = 0.0006, respectively). There was no difference in terms of major or minor complications (p = 0.682). CEUSLB is a feasible technique that increases the diagnostic sensitivity for liver tumors developed in ACLD. 相似文献

Salter–Harris II fractures of the distal radius can result in serious complications. The aim of this study was to measure the fracture–physis distance using point-of-care ultrasound (POCUS) to determine whether a certain distance is associated with Salter–Harris II fractures, compared with other fracture types, in a cohort of children with X-ray–identified distal radius fractures. Participants were from a parent diagnostic study conducted in an Australian tertiary pediatric emergency department, which prospectively evaluated the diagnosis of pediatric distal forearm fractures using POCUS compared against X-ray. Nurse practitioners, who underwent 2 h of training, administered a six-view POCUS protocol in clinically non-angulated pediatric forearm injuries prior to X-ray. This was a secondary analysis of data from the parent study. The 122 participants with X-ray–identified distal radius fractures from the parent study had their POCUS images interpreted by two emergency physician sonologists, who measured the fracture–physis distance. The median and maximum fracture–physis distances for Salter–Harris II fractures (n = 19) were 8.00 and 9.85 mm, whereas minimum and median distances for incomplete fractures (n = 22) were 10.20 and 15.98 mm, and those for complete fractures (n = 9) were 10.85 and 12.85 mm. Buckle fracture (n = 72) distances ranged from 4.35 to 26.55 mm, with a median of 13.65 mm. In children diagnosed with a distal radius fracture on X-ray, a fracture–physis distance cutoff of 1 cm differentiated Salter–Harris II fractures from other cortical breach fracture types, but not buckle fractures. Although this exploratory study suggests the “POCUS 1-cm rule” could be used as a secondary sign to augment the diagnosis of Salter–Harris II distal radius fractures using POCUS, further research is required to validate this measurement prospectively. 相似文献

Certain technical criteria must be met to ensure the treatment safety of magnetic resonance–guided high-intensity focused ultrasound. We retrospectively reviewed how our enrollment criteria were applied from 2014 to 2017 in a clinical trial of magnetic resonance–guided high-intensity focused ultrasound ablation of recurrent malignant and locally aggressive benign solid tumors. Among the 36 screened patients between 2014 and 2017, more than one-third were excluded for technical exclusion criteria such as the anatomic location and proximity to prosthetics. Overall, patients were difficult to accrue for this trial, given the incidence of these tumors. To increase potential accrual, screening exclusion criteria could be more generalized and centered on the ability to achieve an acceptable treatment safety margin, rather than specifically excluding on the basis of general anatomic areas. 相似文献

ObjectivesTo assess symptoms of post-traumatic stress disorder, anxiety, depression and health related quality of life in a sample of adult patients treated with veno-arterial extracorporeal membrane oxygenation.Design and settingThis is a cross-sectional study. The population were all patients discharged (2008–2018) from a thoracic surgical intensive ward at a tertiary university hospital in Norway. A sample of 20 patients was recruited.Main outcome measuresSymptoms of post-traumatic stress disorder were assessed using Impact of Events Scale-Revised, while symptoms of depression and anxiety were assessed using Hospital Anxiety and Depression Scale. RAND 36-Item Short Form Health Survey was applied to measure health-related quality of life.ResultsSymptoms of post-traumatic stress disorder were reported by 40% of the participants. Twenty percent reported symptoms of depression and anxiety. Compared to the general population, participants reported poorer health-related quality of life on all domains, and significantly worse on the domains physical function, general health and social function.ConclusionPatients in our study reported symptoms of post-traumatic stress disorder, anxiety, depression and impaired health-related quality of life following treatment with veno-arterial extra corporeal membrane oxygenation. Addressing possible emotional and psychological distress could represent a potential major improvement in health care provided to this group. Further research is needed to incorporate prophylactic methods, such as identifying vulnerable patients and implement corresponding interventions, into clinical practice. 相似文献

Uterine fibroids (UFs) are very common benign tumors of the female reproductive tract. According to recent reports, magnetic resonance–guided high-intensity ultrasound (MR-HIFU) appears to be a well-tolerated and efficient treatment option for UFs. However, MR-HIFU still presents several limitations. The treatment is rarely associated with achieving complete non-perfused volume (NPV). Not all patients are qualified for a final procedure, and selected women obtain very good results in such treatment. The primary objective of this experimental study was to assess the effect of transvaginal misoprostol and intravenous oxytocin preparation on UF volume change, sonication time and NPV after MR-HIFU procedure in women of reproductive age with symptomatic UFs. Secondary outcomes included the effect on the peri-procedural effectiveness of misoprostol and oxytocin. This study enrolled 247 women with symptomatic UFs; based on gynecologic examinations and magnetic resonance imaging (MRI) scans, 128 women qualified for MR-HIFU without pharmacologic treatment, 57 women qualified for the misoprostol/diclofenac group and 62 women qualified for the oxytocin group. Pharmacologic pre-treatment improved NPV compared with non-pharmacologic treatment (average NPV: controls 61.9% ± 25.8%; oxytocin 76.8% ± 20.7%; misoprostol/diclofenac 85.2% ± 15.1%; average sonication time: controls 120 min ± 56.4%; oxytocin 111 min ± 45.4%; misoprostol/diclofenac 80 min ± 47.7%). Statistical analysis did not reveal significant intergroup differences in UF volume changes after 6 mo (controls: n = 40, 37.4% ± 27.5%; oxytocin n = 25, 45.8% ± 31%; misoprostol/diclofenac n = 19, 33.4% ± 23.2%). The misoprostol/diclofenac group, which achieved the highest NPV immediately after the MR-HIFU procedure, was characterized by the lowest UF volume change percentages 6 mo later. The administration of vasoconstrictor drugs (oxytocin and misoprostol/diclofenac) to support MR-HIFU in UF treatment is a new issue that may improve the total effectiveness of this method. Randomized controlled trials are necessary to estimate the real effect of vasoconstrictors on MR-HIFU. 相似文献

Quantification of the dynamics of the carotid artery wall is useful in evaluating arteriosclerosis and atherosclerosis. As the carotid artery wall moves not only in the radial direction but also in the longitudinal direction, longitudinal movement should be considered in the analysis of the dynamic properties of the carotid artery wall. In this study, we propose a “lateral M-mode” method for visualizing the longitudinal movement of the intima–media complex (IMC). For the lateral M-mode, we set the target line in the longitudinal direction along the IMC and visualize the signals on the target line frame-by-frame by correcting the position of the target line along the radial displacement estimated by the phased tracking method. Differentiating the envelope signals between consecutive ultrasound beams was effective in visualizing the lateral movement of the IMC. The precision of the longitudinal displacement of the IMC estimated using the conventional block-matching method was validated by comparing it with the lateral M-mode. Because the conventional M-mode sequence plays an important role in evaluation of the dynamics of various tissues, the proposed “lateral M-mode” contributes to a detailed understanding of vascular dynamics and the development of diagnostic methods for vascular diseases. 相似文献

A 3-D ultrasound system was determined to provide reliable and valid results for scoliosis assessment in the coronal and sagittal planes. The objective of this study was to investigate whether 3-D ultrasound can detect coronal–sagittal coupling and to study its potential effect on curve progression in patients with adolescence idiopathic scoliosis (AIS) as per the traditional Cobb angle classification. Radiographic and ultrasonic coronal and sagittal curvatures of 126 patients with AIS were evaluated. Thoracic kyphosis (TK) and lumbar lordosis (LL) with different coronal deformity were compared correspondingly based on either main thoracic or (thoraco)lumbar curve groups. The TK and LL of patients with single curves were also compared with study the curve effect on sagittal curvatures. A prospective cohort of 51 patients were followed for an average of 23 months for preliminary progression investigation. TKs in patients with larger main thoracic Cobb angles was significantly smaller than those with smaller main thoracic Cobb angles, judging by the results obtained from ultrasound and X-ray. The TKs of patients with only single right main thoracic curves were significantly smaller than those of patients with only single left (thoraco)lumbar curves. In addition, patients with progressive curves were observed to be relative hypokyphotic during early visits. 相似文献
