Objectives:To evaluate and compare the dental and skeletal changes with conventional and miniscrew-supported maxillary expansion appliances in adolescents.Materials and Methods:Forty patients were divided into two groups, with one group receiving a tooth-borne expander and the other group receiving an expander supported by four miniscrews (bone-borne). Multiplanar coronal and axial slices obtained from cone-beam computed tomography images were used to measure the changes in transverse skeletal widths, buccal bone thickness, tooth inclination, and root length. Paired t-tests and independent-sample t-tests were used to compare the two expansion methods.Results:Bone-borne expansion increased the maxillary suture opening more than 2.5 times than tooth-borne expansion both anteriorly and posteriorly. Between the maxillary first premolars, sutural expansion accounted for 28% and 70% of the total transverse width increase in the tooth-borne and bone-borne expander groups, respectively. Similarly, 26% and 68% of the total expansion was of skeletal nature in the tooth-borne and bone-borne expander groups between the maxillary first molars. The pattern of expansion was variable, with most of the patients in both groups demonstrating a triangular-shaped sutural opening that was wider anteriorly. Subjects in the conventional group experienced significantly more buccal bone reduction and greater buccal inclination of the teeth. No significant differences were observed for root length measurements between the two groups.Conclusion:Use of bone-borne expansion in the adolescent population increased the extent of skeletal changes in the range of 1.5 to 2.8 times that of tooth-borne expansion and did not result in any dental side effects.  相似文献

Objective:To evaluate the immediate effects of rapid maxillary expansion (RME) on the transverse skeletal and dentoalveolar changes with bone-borne (C-expander) and tooth-borne type expanders using cone-beam computed tomography (CBCT) in late adolescents.Materials and Methods:A sample of 28 female late-adolescent patients was divided into two groups according to the type of expander: bone-borne (C-expander, n  =  15, age  =  18.1 ± 4.4 years) and tooth-borne (hyrax, bands on premolars and molars, n  =  13, age  =  17.4 ± 3.4 years). CBCT scans were taken at 0.2-mm voxel size before treatment (T1) and 3 months after RME (T2). Transverse skeletal and dental expansion, alveolar inclination, tooth axis, vertical height of tooth, and buccal dehiscence were evaluated on maxillary premolars and molars. Paired t-test, independent t-test, one-way analysis of variance, and Scheffé post hoc analysis were performed.Results:The C-expander group produced greater skeletal expansion, except in the region of the first premolar (P < .05 or < .01), which showed slight buccal tipping of the alveolar bone. The Hyrax group had more buccal tipping of the alveolar bone and the tooth axes, except in the region of the second molar (P < .05 or < .01 or < .001). Dental expansion at the apex level was similar in the banded teeth (the first premolar and the first molar). Vertical height changes were apparent on the second premolar in the hyrax group (P < .05 or < .01). Significant buccal dehiscence occurred at the first premolar in the hyrax group (P < .01 or < .001). There were no significant differences between tooth types for any variables in the C-expander group.Conclusions:For patients in late adolescence, bone-borne expanders produced greater orthopedic effects and fewer dentoalveolar side effects compared to the hyrax expanders.  相似文献

ObjectivesTo assess and compare spontaneous expansion of mandibular posterior teeth between tooth-borne (TB) and bone-borne (BB) rapid maxillary expansion (RME).Materials and MethodsThis study included 36 adolescents with bilateral maxillary crossbite receiving tooth-borne (average age: 14.4 ± 1.3 years) or bone-borne (average age: 14.7 ± 1.4 years) maxillary expansion. Cone beam computed tomography was acquired before expansion (T1) and after 6 months'' retention (T2). Specific linear and angular measurements were performed in the coronal view to assess buccal inclinations and widths of mandibular posterior units. All data were statistically analyzed.ResultsIn both groups there was a significant increase in buccal-lingual inclination of mandibular posterior teeth ranging from 1.67° to 2.30° in the TB group and from 1.46° to 2.11° in BB group. Mandibular posterior widths showed an increase ranging from 0.80 mm to 1.33 mm in TB group and from 0.64 mm to 0.96 mm in the BB group. No differences between groups were found for linear or angular measurements.ConclusionsA clinically significant gain of space in the mandibular arch should not be expected after RME.  相似文献

AimTo evaluate and compare the short-term (post-retention) skeletal and dental changes following bone-borne and tooth-borne surgically assisted rapid maxillary expansion (SARME) using cone beam computed tomography (CBCT).Subjects and methodsIn this randomized clinical study, 30 patients with transverse maxillary deficiency underwent either tooth-borne (n = 15) or bone-borne (n = 15) SARME. Before treatment and immediately after the consolidation period, CBCT was obtained and the nasal floor width, interdental root distance, palatal bone width and interdental cusp distance were measured at first premolar and first molar regions of maxilla.ResultsTwenty eight patients completed the study protocol. In both tooth-borne (n = 13) and bone-borne (n = 15) groups the highest degree of expansion occurred in the dental arch, followed by palatal bone, and nasal floor (V-shaped widening in coronal dimension). The amount and pattern of expansion was comparable between anterior and posterior maxillary regions in each group (parallel posteroanterior expansion) and between the two groups.ConclusionDental and skeletal effects of tooth-borne and bone-borne devices were comparable. The overall complication rate was negligible. Selection of an expansion device should be based on each individual patient's requirements. Future long-term clinical trial studies to evaluate the stability and relapse of these two techniques are recommended.  相似文献

ObjectiveTo evaluate the molar inclination and skeletal and alveolar bone changes when comparing tooth bone-borne (MSE) and tissue bone-borne type maxillary expanders (C-expander) using cone-beam computed tomography (CBCT) in late adolescence.Materials and MethodsA sample of 48 late-adolescent patients were divided into two groups according to the type of expander: MSE group (n = 24, age = 19.2 ± 5.9 years) and C-expander group (n = 24, age = 18.1 ± 4.5 years). CBCT scans were taken before treatment and 3 months after expansion. Transverse skeletal and dental expansion, alveolar inclination, tooth axis, buccal alveolar bone height, thickness, dehiscence, and fenestration were evaluated on the maxillary first molar. Paired t-test, independent t-test, Pearson''s chi-square test, and Spearman correlation analysis were performed.ResultsThe MSE group produced greater dental expansion (P < .05), whereas skeletal expansion was similar in both groups (P = .859). The C expander group had more alveolar bone inclination change (P < .01), and the MSE group had more buccal tipping of the anchorage teeth (P < .01 or .001). Buccal alveolar bone height loss and thickness changes were greater in the MSE group (P < .01 or <.001). Formation of dehiscences was more frequent in the MSE group (P < .001), whereas for fenestrations, there were no significant differences between the two groups. Buccal bone height loss in the MSE group had a negative correlation with initial buccal bone thickness.ConclusionsThe incorporation of teeth into bone-borne expanders resulted in an increase in the severity of side effects. For patients in late adolescence, tissue bone-borne expanders offer comparable skeletal effects to tooth bone-borne expanders, with fewer dentoalveolar side effects.  相似文献

ObjectivesTo evaluate changes in dimensions and volume of upper airway before and after mini-implant assisted rapid maxillary expansion (MARME) and observe correlations between changes of upper airway and vertical skeletal pattern in young adults.Materials and MethodsIn this retrospective study, 22 patients (mean age, 22.6 ± 4.5 years; 4 male 18 female) with transverse discrepancy underwent MARME. Cone beam computed tomography was taken before and 3 months after expansion. Vertical and horizontal dimensions and volume of the nasal cavity, nasopharyngeal, retropalatal, retroglossal and hypopharyngeal airway were compared before and after MARME. Correlations between changed volume and dimensions were explored, as well as the vertical skeletal pattern.ResultsNasal osseous width, maxillary width, volume of the nasal cavity and nasopharynx increased significantly (P < .05). Enlarged nasopharyngeal volume correlated with increased nasal width at the PNS plane (P < .05). There were no correlations between expanded volume and maxillary width. No measurements except nasal cavity volume had a correlation with Sum angle. Increased maxillary width correlated negatively with hard palate thickness (P < .05).Conclusions(1) MARME caused an increase in volume of the nasal cavity and nasopharynx, with expansion of nasal osseous width and maxillary width. (2) Enlarged nasal width at the PNS plane contributed to the increase in nasopharynx volume. Enlarged maxillary width showed no direct relation with increased volume. (3) In this study, it was unclear about the association between changes of the upper airway and vertical skeletal pattern because of complex structures. (4) Palate thickness affected skeletal expansion of the maxilla in MARME.  相似文献

ObjectivesTo assess changes in spheno-occipital synchondrosis after rapid maxillary expansion (RME) performed with conventional tooth-borne (TB) and bone-borne (BB) appliances.Materials and MethodsThis study included 40 subjects with transverse maxillary deficiency who received TB RME or BB RME. Cone-beam computed tomography images (CBCT) were taken before treatment (T0), and after a 6-month retention period (T1). Three-dimensional surface models of the spheno-occipital synchondrosis and basilar part of the occipital bone were generated. The CBCTs taken at T0 and T1 were registered at the anterior cranial fossa via voxel-based superimposition. Quantitative evaluation of Basion displacement was performed with linear measurements and Euclidean distances. The volume of the synchndrosis was also calculated for each time point as well as the Nasion-Sella-Basion angle (N-S-Ba°). All data were statistically analyzed to perform inter-timing and intergroup comparisons.ResultsIn both groups, there was a small increment of the volume of the synchondrosis and of N-S-Ba° (P < .05). Basion showed a posterosuperior pattern of displacement. However, no significant differences (P > .05) were found between the two groups.ConclusionsAlthough TB and BB RME seemed to have some effects on the spheno-occipital synchondrosis, differences were very small and clinically negligible.  相似文献

Objective:To quantitatively evaluate maxillary skeletal expansion using cone-beam computed tomography (CBCT) images and propose a novel way to quantify the dental tipping effects of temporary skeletal anchorage device–supported rapid maxillary expansion appliance (TSADRME).Materials and Methods:Images from 25 patients receiving rapid maxillary expansion with incorporated temporary skeletal anchorage devices (TSADs) before activation (T1) and after removal (T2) were analyzed to detect dentoskeletal changes.Results:A significant increase from T1 to T2 was found for all linear measurements except buccal maxillary width at the canines. The greatest buccal expansion was at the first molar, decreasing anteriorly. However, the greatest palatal expansion was at the first premolar. All younger subjects (8–16 years old) exhibited less dental tipping and greater expansion overall compared with the older subjects. There was great variability in dental tipping of first molars (mean = 4.31°), with some subjects demonstrating mild uprighting of these teeth.Conclusions:The TSADRME appliance is an effective, clinically useful device that results in mild molar tipping and may positively affect expansion in the area of TSAD placement.  相似文献

ObjectivesTo evaluate the volume, amount, and localization of root resorption in the upper first premolars by micro–computed tomography (micro-CT) after three different rapid maxillary expansion appliances and two different activation rhythms.Materials and MethodsThe patients were divided into three groups; Hyrax, acrylic cap splint (ACS), and full coverage acrylic bonded (FCAB) appliances. Each group was then divided into the following two subgroups: rapid maxillary expansion (RME) and semirapid maxillary expansion (SRME). After expansion was completed, the appliances were stabilized for 12 weeks during the retention period. For each group, 10 premolars (for a total of 60 premolars) were scanned with the micro-CT (SkyScan). The reconstructed 3D images of each root sample were divided into six regions. The resorption craters on these six different root surfaces were analyzed by special CTAn (SkyScan) software for direct volumetric measurements. Kruskal-Wallis one-way analysis of variance and Mann-Whitney U tests were used for statistical analysis.ResultsThe total volume of root resorption was less with FCAB than with ACS and Hyrax (P < .001). In all groups, a greater volume of resorption was found on the buccal surface than on the lingual surface (P < .001). No significant differences were found between the RME and SRME groups (P > .05).ConclusionsAll expansion appliances caused root resorption in the upper first premolar teeth, but FCAB may be safer in terms of root resorption. The resorption craters were generally concentrated on the buccal surface. There was no effect of activation rhythm on root resorption.  相似文献

AimTo three-dimensionally (3D) assess the long-term effects of tooth-borne and bone-borne surgically assisted rapid maxillary expansion (SARME).Subjects and methodsThis prospective cohort study comprised 45 consecutive skeletally mature non-syndromic patients with transverse maxillary hypoplasia. In 28 patients, a tooth-borne distractor (Hyrax) was used for expansion, whereas in the remaining 17 a bone-borne distractor (transpalatal distractor, TPD) was used. Cone beam computed tomography (CBCT) scans were performed before treatment (T0) and 22 months later, after fixed appliance treatment (T1). 3D models were constructed from CBCT data and superimposed using voxel-based matching. Distance maps between the superimposed models were computed to evaluate the amount of skeletal changes.ResultsThe distance maps of the superimposed models showed positive distances on the right and left posterior alveolar segments of the maxilla indicating lateral expansion. The anterior maxillary region showed negative distances or posterior displacement and remodelling of the anterior alveolar region. There was no statistically significant difference between TPD and Hyrax for the three alveolar segments (p values ranged 0.63–0.81).ConclusionBone-borne and tooth-borne SARME were found to produce comparable results at the end of fixed appliance treatment regarding skeletal changes.  相似文献

Objective:To quantitatively evaluate the effects of asymmetric rapid maxillary expansion (ARME) on cortical bone thickness and buccal alveolar bone height (BABH), and to determine the formation of dehiscence and fenestration in the alveolar bone surrounding the posterior teeth, using cone-beam computed tomography (CBCT).Materials and Methods:The CBCT records of 23 patients with true unilateral posterior skeletal crossbite (10 boys, 14.06 ± 1.08 years old, and 13 girls, 13.64 ± 1.32 years old) who had undergone ARME were selected from our clinic archives. The bonded acrylic ARME appliance, including an occlusal stopper, was used on all patients. CBCT records had been taken before ARME (T1) and after the 3-month retention period (T2). Axial slices of the CBCT images at 3 vertical levels were used to evaluate the buccal and palatal aspects of the canines, first and second premolars, and first molars. Paired samples and independent sample t-tests were used for statistical comparison.Results:The results suggest that buccal cortical bone thickness of the affected side was significantly more affected by the expansion than was the unaffected side (P < .05). ARME significantly reduced the BABH of the canines (P < .01) and the first and second premolars (P < .05) on the affected side. ARME also increased the incidence of dehiscence and fenestration on the affected side.Conclusions:ARME may quantitatively decrease buccal cortical bone thickness and height on the affected side.  相似文献

ObjectiveTo investigate the efficacy of microimplant-assisted rapid palatal expansion (MARPE) to treat skeletal maxillary discrepancies during the post-pubertal growth spurt stage.Materials and MethodsSixty patients with skeletal maxillary transverse deficiency during the post-pubertal growth spurt stage were randomly divided into MARPE and Hyrax groups. Thirty patients (mean age: 15.1 ± 1.6 years) were treated using the four-point MARPE appliance; 30 patients (mean age, 14.8 ± 1.5 years) were treated using the Hyrax expander. Cone beam computed tomography scans and dental casts were obtained before and after expansion. The data were analyzed using paired t-tests and independent t-tests.ResultsThe success rates of midpalatal suture separation were 100% and 86.7% for MARPE and Hyrax groups, respectively. Palatal expansion and skeletal to dental ratio at the first molar level were greater in the MARPE group (3.82 mm and 61.4%, respectively) than in the Hyrax group (2.20 mm and 32.3%, respectively) (P < .01). Reductions in buccal alveolar bone height and buccal tipping of the first molars were less in the MARPE group than in the Hyrax group (P < .01).ConclusionsMARPE enabled more predictable and greater skeletal expansion, as well as less buccal tipping and alveolar height loss on anchorage teeth. Thus, MARPE is a better alternative for patients with skeletal maxillary deficiency during the post-pubertal growth spurt stage.  相似文献

Objectives:To evaluate changes in the volume and cross-sectional area of the nasal airway before and 1 year after nonsurgical miniscrew-assisted rapid maxillary expansion (MARME) in young adults.Materials and Methods:Fourteen patients (mean age, 22.7 years; 10 women, four men) with a transverse discrepancy who underwent cone beam computed tomography before (T0), immediately after (T1), and 1 year after (T2) expansion were retrospectively included in this study. The volume of the nasal cavity and nasopharynx and the cross-sectional area of the anterior, middle, and posterior segments of the nasal airway were measured and compared among the three timepoints using paired t-tests.Results:The volume of the nasal cavity showed a significant increase at T1 and T2 (P < .05), while that of the nasopharynx increased only at T2 (P < .05). The anterior and middle cross-sectional areas significantly increased at T1 and T2 (P < .05), while the posterior cross-sectional area showed no significant change throughout the observation period (P > .05).Conclusions:The results demonstrate that the volume and cross-sectional area of the nasal cavity increased after MARME and were maintained at 1 year after expansion. Therefore, MARME may be helpful in expanding the nasal airway.  相似文献

Objective:To compare the dentoskeletal effects of rapid (RME) and slow (SME) maxillary expansion in patients with bilateral complete cleft lip and palate (BCLP).Materials and Methods:This was a secondary analysis of a previous randomized controlled trial (RCT). Forty-six patients (34 male, 12 female) with BCLP and posterior crossbite (mean age of 9.2 years) were randomly assigned to two study groups. Group RME comprised subjects treated with Haas/Hyrax expander. Group SME included patients treated with quad-helix appliance. Cone-beam computed tomography (CBCT) was performed before expansion (T1) and after appliance removal at the end of a 6-month retention period (T2) for a previous RCT that compared the transverse skeletal effects of RME and SME. CBCT-derived cephalometric images were generated and cephalometric analysis was performed using Dolphin Imaging Software (Chatsworth, Calif). Intergroup comparisons were performed using t tests (P < .05).Results:Baseline forms were similar between groups. No significant differences between RME and SME groups were found.Conclusions:Rapid and slow maxillary expansion produced similar sagittal and vertical changes in patients with BCLP. Both Haas/Hyrax and quad-helix appliances can be used in patients with vertical facial pattern. Clinical relevance: RME and SME can be equally indicated in the treatment of maxillary arch constriction in patients with BCLP.  相似文献

ObjectiveTo evaluate pharyngeal airway and maxillary sinus volumes following symmetric rapid maxillary expansion (RME) and asymmetric rapid maxillary expansion (ARME) treatment using cone-beam computed tomography (CBCT).Materials and MethodsThe study consisted of 60 patients presenting to the orthodontics clinic with an indication that they required symmetric or asymmetric rapid maxillary expansion treatment. Individuals were included if they were aged 12–15 years and had symmetric (RME group; 14 girls, 16 boys) or asymmetric (ARME group; 16 girls, 14 boys) maxillary deficiency. Maxillary sinus volume (mm3) and pharyngeal airway volume (upper, lower, and total; mm3) were evaluated using CBCT records. The parameters were compared before treatment (T1) and after 3 months in retention (T2).ResultsAll measurements at T2 were increased significantly compared with T1 in the RME group (P < .05). In the ARME group, changes in the lower pharyngeal airway and the nonaffected maxillary sinus volumes (non-affected side of maxillary sinus volumes) were not significant; however, the other measurements increased significantly from T1 to T2 (P < .05). Intergroup comparisons revealed that total pharyngeal airway volume and total maxillary sinus volume changes were significantly greater in the RME group.ConclusionsPharyngeal airway and maxillary sinus volumes increased with both RME and ARME treatment. Both were found to be effective for treating transverse maxillary deficiency.  相似文献

Objectives:To compare palatal symmetry, dimensions, and molar angulations following early mixed-dentition slow maxillary expansion with parameters in normal controls.Materials and Methods:A total of 30 patients treated with a Haas-type expander for unilateral posterior crossbite with functional shift were compared with 30 controls matched for dental age, gender, and molar relationship. Records were taken before (T1) and after expansion (T2). Palatal width, surface area, volume, and molar angulations were measured on digitized models. Surface area and volume were split in half then divided into anterior, middle, and posterior segments to measure symmetry. Student''s t-test was used to assess group differences.Results:Mean intercanine width increased 4.65 mm, and intermolar width increased 4.76 mm. The treated mean surface area increased 127.05 mm2 compared with 10.35 mm2 in controls. The treated palatal volume increased 927.55 mm3 compared with 159.89 mm3 for controls. At T1, the controls showed significant differences in surface area and volume between the anterior palatal segments. At T2, this difference occurred in surface area of middle palatal segments of the treated sample. First permanent molars showed an increased buccal and distal inclination after treatment, opposite to controls. The increase in buccal inclination was greater on the crossbite side.Conclusions:The Haas-type expander results in similar expansion across the canines and first molars. A palate that is symmetrical before expansion may become asymmetric after expansion but only in the middle segment. Changes in individual molar angulation following palatal expansion can be measured without radiation imaging.  相似文献

Objective:To evaluate the changes in nasal mucociliary clearance in orthodontic patients after rapid maxillary expansion (RME) therapy.Materials and Methods:Forty-two children (25 boys and 17 girls) participated in this study. The RME group consisted of 21 patients (mean age, 13.8 years), who had undergone RME at the initiation of orthodontic treatment. The control group consisted of 21 subjects (mean age, 13.6 years), who were attending the department of orthodontics for active orthodontic treatment. The nasal mucociliary clearance was assessed by the saccharin test. Saccharin transit times (STTs) were measured for each treated subject before expansion (T1), after RME (T2), and after a 3-month retention period (T3). Records were obtained at the same time intervals for each group.Results:The STT decreased significantly in the RME group after expansion and retention (P < .05). A statistically significant difference was found when the STTs of the control and RME groups were compared after expansion and retention (P < .05).Conclusions:The STTs of young orthodontic patients with maxillary narrowness and without any history of nasal or systemic disease were within normal limits. However, RME increased the mucociliary clearance in patients who had maxillary narrowness, having positive effects on nasal physiology and increasing nasal cavity volume.  相似文献

Objectives:To evaluate the effectiveness of platelet-rich plasma (PRP) with its growth factors in minimizing the side effects of rapid maxillary expansion (RME) on the periodontal tissue of anchoring teeth using cone-beam computed tomography (CBCT).Materials and Methods:A randomized, split-mouth clinical trial was conducted on 18 patients aged 12–16 years (14 ± 1.65) with a skeletal maxillary constriction who underwent RME using a Hyrax appliance. The sample was randomly divided into two groups: intervention and control sides. PRP was prepared and injected on the buccal aspect of supporting teeth in the intervention group. High-resolution CBCT imaging (H-CBCT) was carried out preoperatively (T0) and after 3 months of retention (T1) to study the buccal bone plate thickness (BBPT) and buccal bone crest level (BBCL) of anchoring teeth. Changes induced by expansion were evaluated using paired sample t-test (P < .05).Results:Results showed that there was no significant difference in BBPT and BBCL between the two groups after RME (P > .05). The prevalence of dehiscence and fenestrations was increased at (T1) in both groups and the percentage was higher in the PRP group.Conclusions:RME induced vertical and horizontal bone loss. Objective:To evaluate the long-term effects of rapid maxillary expansion (RME) and posterior bite block (BB) in prepubertal subjects with dentoskeletal open bite.Materials and Methods:The treatment group (TG) comprised 16 subjects (14 girls, 2 boys) with dentoskeletal open bite with a mean age of 8.1 ± 1.1 years treated with RME and BB. Three consecutive lateral cephalograms were available before treatment (T1), at the end of the active treatment with the RME and BB (T2), and at a follow-up observation at least 4 years after the completion of treatment (T3). The TG was compared with a control group (CG) of 16 subjects (14 girls, 2 boys) matched for sex, age, and vertical skeletal pattern. An independent sample t-test was used to compare the T1 to T3, T1 to T2, and T2 to T3 cephalometric changes between the TG and the CG.Results:In the long term, the TG showed a significantly greater increase in overbite (+1.8 mm), reduced extrusion of maxillary and mandibular molars (−3.3 mm), and, consequently, a significant decrease in facial divergence (−2.8°) when compared with untreated subjects.Conclusions:The RME and BB protocol led to successful and stable recovery of positive overbite in 100% of the patients considered. Correction of open bite was associated with reduced extrusion of maxillary and mandibular molars with a significant improvement in vertical skeletal relationships when compared with the CG.  相似文献
