Seven patients with congenital anosmia underwent detailed chemosensory evaluation, followed by the performance of biopsies of the olfactory region. Olfactory epithelium was not found in any of the biopsy specimens. It appears therefore that patients with congenital anosmia lack any olfactory epithelium. Several possible explanations for this finding are discussed. The most attractive hypothesis is that the olfactory placode forms either normally or abnormally during development but later degenerates and is replaced with respiratory epithelium. Only one patient in our series had congenital anosmia in association with a syndrome (Kallmann's syndrome), indicating that congenital anosmia is found more often as an isolated symptom.  相似文献

BACKGROUND: Kallmann's syndrome (KS) was first mentioned in 1944 as an association of anosmia and hypogonadotropic hypogonadism. Causes are multiple genetic defects the most common of which is the x-linked KS appearing mostly in men. However, autosomal dominant and autosomal recessive forms have also been described. PATIENT: We present a case of KS with normosmia (male, 39 years of age). All symptoms of hypogonadotropic hypogonadism were present. RESULTS: Psychophysical olfactory testing revealed left-sided anosmia with right-sided normosmia which was confirmed by electrophysiological measures of olfactory function. Magnetic resonance imaging indicated aplasia of the left olfactory tract and bulb, whereas the right-sided structures appeared to be normal. CONCLUSIONS: As indicated in this case with lateralized anosmia and contralateral normosmia, overall olfactory function strongly depends on the "best" nostril. Therefore, in many clinical situations, lateralized olfactory testing appears to be extremely important.  相似文献

目的总结先天性失嗅的分类、临床表现以及影像学特点。方法回顾性分析2004年8月-2006年9月收治的8例先天性失嗅病例的临床资料。4例为伴有其他异常的先天性失嗅患者,其中卡尔曼综合征(Kallmann syndrome)3例,鼻腔鼻窦发育异常1例;另外4例为孤立性失嗅。8例患者均行详细的病史采集、全面体检、T&T嗅觉检查、嗅觉事件相关电位测试、鼻内镜检查和鼻窦CT检查。7例行嗅觉通路MRI和性激素检测,2例行嗅黏膜活组织检查。结果8例患者自幼均未闻到过任何气味。除1例鼻腔鼻窦发育异常外,其余7例耳鼻咽喉科常规检查以及鼻内镜检查未见异常。主观嗅觉测试均为完全失嗅。嗅觉事件相关电位测试显示最大嗅刺激引不出嗅觉事件相关电位。CT检查显示1例鼻腔鼻窦未发育,其余7例未见异常。MRI检查:6例均为嗅球、嗅束缺失,嗅沟缺失或部分变浅;1例双侧嗅球发育差。内分泌检查:3例青春期后外生殖器以及男性第二性征发育不良,血清睾酮、雌二醇、促黄体生成素以及促卵泡生成素水平低于正常。其余4例性激素检测正常。嗅黏膜活检:1例未见嗅上皮典型结构;另1例为鳞状上皮组织伴慢性炎性病变。结论先天性失嗅诊断应依据病史、专科体检、嗅觉测试、鼻窦cT以及嗅路MRI检查结果。嗅路MRI对诊断有重要价值。  相似文献

The nasal cycle is a phenomenon characterized by cyclic changes in the airflow resistance of each nasal fossa from congestion or constriction of the nasal erectile tissue. This phenomenon is caused by alterations in the influence exerted by the autonomic nervous system and is believed to be regulated by a hypothalamic center. In order to verify this hypothesis on the hypothalamic regulation of the nasal cycle, the authors studied a group of subjects with Kallmann's syndrome. This disorder is characterized by coexisting hypothalamic hypogonadism and hyposmia or anosmia due to hypothalamic and olfactory center hypoplasia. An anomalous nasal cycle was found in all the subjects with Kallmann's syndrome. This disorder might be related to hypothalamus atrophy.  相似文献

目的 分析奥密克戎变异株感染后嗅觉功能障碍患者的焦虑、抑郁现状及其相关因素,为制定有效的心理干预措施提供理论依据。方法 于2022年12月—2023年1月重庆新型冠状病毒(SARS-CoV-2)奥密克戎变异株流行期间,通过问卷收集信息,采用广泛性焦虑障碍量表(GAD-7)和患者健康问卷(PHQ-9)评定焦虑、抑郁情绪,采用视觉模拟量表(VAS)评定嗅觉功能。采用χ2检验、多因素Logistic回归分析影响患者焦虑、抑郁情绪的相关因素。结果 共纳入患者367例,其中感染后嗅觉功能障碍患者182例,无嗅觉功能障碍患者185例。嗅觉功能障碍患者焦虑、抑郁情绪的检出率分别为46.7%、63.2%,均高于无嗅觉功能障碍的患者。多因素Logistic回归分析显示,嗅觉下降持续时间长(OR=1.660)、嗅觉功能VAS评分高(OR=8.993)是产生焦虑情绪的危险因素。女性(OR=2.964)、嗅觉功能VAS评分高(OR=4.423)是产生抑郁情绪的危险因素。结论 SARS-CoV-2感染后患者均出现一定程度的焦虑、抑郁情绪,伴有嗅觉功能障碍患者的焦虑、抑郁情绪的检出率更高,应加强对此类患者的心理疏导和人文关怀,以改善和促进其心理健康。  相似文献

Objectives: To measure the prevalence of and identify clinical characteristics associated with poor olfactory function in a large cohort of patients with chronic rhinosinusitis (CRS). Study Design: Multi‐institutional, cross sectional analysis. Methods: An objective measure of olfactory dysfunction, the Smell Identification Test, demographic data, clinical factors, and comorbidity data were collected from a cohort of 367 patients who presented with CRS at three tertiary care centers. Data were analyzed using univariate and multivariate analyses. Results: Sixty‐four percent of men and women aged 18 to 64 had olfactory dysfunction whereas 95% of patients older than or equal to 65 years had olfactory dysfunction (P < .001); no significant difference was noted by gender. By multivariate logistic regression analysis, patients with nasal polyposis [Odds ratio (OR) 2.4, 95% confidence interval (CI) 1.3–4.2, P = .003] and patients older than or equal to 65 years (OR 10.0, 95% CI 2.3–43.7, P = .002) were at increased risk of hyposmia. Patients with nasal polyposis (OR 13.2, 95% CI 5.7–30.7, P < .001), asthma (OR 4.2, 95% CI 1.8–9.8, P = .001), older than or equal to 65 years (OR 15.6, 95% CI 2.3–104.9, P = .005), and smokers (OR 7.6, 95% CI 1.8–31.6, P = .005) were at increased risk of anosmia. Conclusions: Poor olfactory function is common in patients with CRS. Age, nasal polyposis, smoking, and asthma were significantly associated with olfactory dysfunction in patients with CRS. Neither prior endoscopic sinus surgery nor a history of allergic rhinitis was associated with olfactory dysfunction. Septal deviation and inferior turbinate hypertrophy were associated with normal olfactory function.  相似文献

Objective —To present a statistical evaluation of a new olfactory test, Biolfa®.

Material and Methods —The olfactory test was carried out in individuals with normal olfactory function (n=67; 31 males; mean age 27.4 years) and in patients with mild, moderate or severe hyposmia (n=155; 61 males; mean age 54.6 years). The main diagnoses of the hyposmic patients were upper respiratory tract infection (32%), nasal polyposis (24%), head trauma (8%), idiopathic (8%), old age (5%) and chronic rhinitis (5%). In the first part of the test, olfactory thresholds were measured for three different substances (eugenol, aldehyde C14 and phenyl ethyl alcohol). The second part comprised an odor identification test for determining olfactory function for a large panel of common odors of Southern European countries.

Results —Mean olfactory thresholds were determined for subjects with normal olfaction and hyposmic patients; there was a statistically significant relationship between olfactory thresholds and degree of olfaction (mild to moderate hyposmia versus severe hyposmia). For the odor identification test, a global score was determined to allow the differentiation of normosmic subjects from patients with mild, moderate or severe hyposmia. The principles of decision theory (i.e. analysis of the receiver operating characteristic curve) were applied to the problem of evaluating the ability of Biolfa to distinguish subnormal subjects from hyposmic subjects.

Conclusion —This study provides a basis for the routine clinical use of Biolfa.  相似文献

Conclusion: The subjective increase in nasal patency after oral menthol application depends on olfactory function. In patients with relatively low olfactory sensitivity, this effect is small or absent. It was hypothesized that this may also modulate the sensation of nasal blockage. Objective: Menthol stimulates the nasal trigeminal nerve, resulting in a subjectively increased nasal airflow and patency without objectively measurable changes. Patients suffering from olfactory impairment also express a reduced nasal trigeminal sensitivity. The aim of this investigation was to examine the effect of menthol on nasal patency ratings in a group of patients suffering from olfactory dysfunction. Methods: A group of 92 patients with olfactory impairment was included in this study. Olfactory function was assessed using the ‘Sniffin’ Sticks.’ Patients were instructed to chew a menthol-containing gum for 30 s, after which they rated the change in nasal patency on a categorical scale. Results: Patients who rated the change in nasal patency as pronounced scored higher on the Sniffin’ Sticks olfactory test compared to patients who experienced little or no change in nasal patency after chewing the menthol-containing gum. On average, similar changes could be observed for all etiologies of olfactory impairment included in this study.  相似文献

目的 通过探究上呼吸道感染后嗅觉障碍脑灰质形态变化,得出上呼吸道感染后嗅觉障碍患者嗅觉中枢结构的影像学特点。方法 选取上呼吸道感染后嗅觉障碍的患者24例与嗅觉功能正常的健康对照受试者20例,通过基于体素的大脑皮质形态学体积测量(VBM)比较患者的大脑灰质和白质体积与对照组的差异。结果 上呼吸道感染后嗅觉障碍组患者TDI[气味察觉阈(T)、气味辨别能力(D)、气味识别能力(I)]总分(16.45±5.62)分,T为(2.33±0.93)分,D为(6.54±2.60)分,I为(7.58±3.22)分;嗅觉障碍平均时长为(19.00±6.33)个月;视觉模拟量表评分(VAS)为(7.79±2.41)分,与对照组比较均存在统计学差异(P<0.05)。上呼吸道感染后嗅觉障碍患者的大脑灰质总体积和嗅皮质体积明显小于对照组(P<0.05),大脑灰质总体积占全脑体积的比例和皮质厚度与嗅觉功能评分TDI值呈正相关(r=0.71,P<0.000 1;r=0.69,P=0.000 9)。结论 上呼吸道感染后嗅觉障碍患者大脑灰质总体积和嗅皮质体积减少,且嗅觉功能与大脑灰质体积比例及皮质厚度呈...  相似文献

Introduction

Idiopathic hypogonadotrophic hypogonadism with an olfactory deficit is defined as Kallmann syndrome and is distinct from normosmic idiopathic hypogonadotrophic hypogonadism.

Objective

Because olfactory perception not only consists of orthonasally gained impressions but also involves retronasal olfactory function, in this study we decided to comprehensively evaluate both retronasal and orthonasal olfaction in patients with idiopathic hypogonadotrophic hypogonadism.

Methods

This case-control study included 31 controls and 45 idiopathic hypogonadotrophic hypogonadism patients. All participants whose olfactory and taste functions were evaluated with orthonasal olfaction (discrimination, identification and threshold), retronasal olfaction, taste function and olfactory bulb volume measurement. The patients were separated into three groups according to orthonasal olfaction: anosmic idiopathic hypogonadotrophic hypogonadism, hyposmic idiopathic hypogonadotrophic hypogonadism and normosmic idiopathic hypogonadotrophic hypogonadism.

Results

Discrimination, identification and threshold scores of patients with Kallmann syndrome were significantly lower than controls. Threshold scores of patients with normosmic idiopathic hypogonadotrophic hypogonadism. were significantly lower than those of controls, but discrimination and identification scores were not significantly different. Retronasal olfaction was reduced only in the anosmic idiopathic hypogonadotrophic hypogonadism group compared to controls. Identification of bitter, sweet, sour, and salty tastes was not significantly different when compared between the anosmic, hyposmic, and normosmic idiopathic hypogonadotrophic hypogonadism groups and controls. Olfactory bulb volume was lower bilaterally in all patient groups when compared with controls. The olfactory bulb volume of both sides was found to be significantly correlated with threshold, discrimination and identification scores in idiopathic hypogonadotrophic hypogonadism patients.

Conclusion

1) There were no significant differences in gustatory function between controls and idiopathic hypogonadotrophic hypogonadism patients; 2) retronasal olfaction was reduced only in anosmic patients but not in orthonasally hyposmic participants, possibly indicating presence of effective compensatory mechanisms; 3) olfactory bulb volumes were highly correlated with olfaction scores in the hypogonadotrophic hypogonadism group. The current results indicate a continuum from anosmia to normosmia in idiopathic hypogonadotrophic hypogonadism patients.  相似文献

Although a reduced olfactory/gustatory function affects patients in all parts of life, this problem has not received much attention in Wegener’s granulomatosis (WG). The aim of this study was to assess the smell/taste function of WG patients. Demographic data of 16 WG patients (9 males, 7 females) were obtained. They all subjectively assessed their taste/smell function on visual analogue scale. Olfactory/gustatory functions of the patients were tested with ‘Sniffin’ Sticks and ‘Taste’ strips, respectively. The results were then compared with those from sex and age-matched control group (n = 16) and normative data. WG patients subjectively assessed their olfactory (p = 0.03) and gustatory (p = 0.02) function to be lower than control group. All the olfactory scores (odour identification, odour discrimination and threshold) in both genders were significantly below the scores in the control group. WG patients were hyposmic. For taste (total taste score, as well as scores for the qualities sweet, sour, salty and bitter), WG patients did not significantly differ from controls and were normogeusic. However, the gustatory scores showed the tendency of reduction as compared to the control group. In conclusion, WG patients truly suffer from olfactory/taste dysfunction, but this is worse with olfaction. It is, therefore, imperative that physicians should make their patients to be aware of these sensory dysfunctions and educate them on methods to cope with it for better quality of life.  相似文献

The olfactory (OF) and gustatory function in multiple sclerosis (MS) patients and possible influencing variables of the disease, such as depression or fatigue, were determined. In an open prospective controlled clinical study 30 patients with MS and 30 healthy controls matched for age, sex and smoking-habits were investigated. With Mini Mental State Examination cognitive dysfunction was excluded, with Expanded Disability Status Scale the patient’s ability to accomplish the tests was ensured. The severity of depression was measured with the self-reported Beck Depression Inventory. The orthonasal olfactory function was derived with olfactory event related potentials (OERP) and TDI-score (Threshold, Discrimination and Identification, Sniffin’ Sticks). Retronasal olfactory function was tested with Taste-Powder-score, gustatory function with Taste-strip-score. There was a significant loss of olfactory function measured with TDI-score [12/30 (40%), p = 0.002] and gustatory function [5/23 (21.7%), p < 0.001] in MS-patients, 23.8% (5/21) of MS-patients showed hyposmia with OERPs, significantly correlating with the TDI-score (p = 0.03). The Expanded Disability Status scale score inversely correlated with the TDI-score (p = 0.002). This study confirms the incidence of olfactory disorder in MS-patients and reveals a frequent gustatory deficit. The Identification subtest can be proposed as a marker of the OF in MS-patients: it includes complex cognitive tasks and may be influenced by depression and fatigue, which are common symptoms of MS. Objectives —The capability of three olfactory tests (European Test of Olfactory Capabilities, Sniffin’ Sticks and Cross-Cultural Smell Identification Test) to similarly classify subjects as normosmics, hyposmics and anosmics as well as the relation between test performance and suprathreshold ortho- and retronasal odor intensity ratings were examined.

Material and Methods —A total of 48 subjects (age range 15–84 years; mean age 49.5 years) completed the 3 olfactory tests, rated aqueous solutions of vanilla (0–0.31%) and lemon aroma (0–0.17%) for odor and flavor intensity and filled in a background questionnaire. Ten subjects had case histories indicating anosmia, with the remainder having subjectively normal olfaction.

Results —The test results were highly correlated and differentiated anosmic, hyposmic and normosmic subjects. At an individual level, some discrepancy was seen in the olfactory diagnoses given by the three tests. In principal component analysis, olfactory measurements were loaded on three components: (i) odor detection, discrimination and identification; (ii) suprathreshold intensity ratings; and (iii) threshold for n-butanol. Advanced age was related to impaired olfactory performance.

Conclusions —Although the three olfactory tests diagnosed the individuals slightly differently, all were considered to be valid for clinical evaluation of olfactory capabilities. The tests separated anosmics and normosmics highly significantly, and permitted an assessment of hyposmia. The suprathreshold odor intensity ratings reflected a different dimension of olfaction than the three olfactory tests.  相似文献

Conclusion. A special consulting team for patients with olfactory disorders would be able to verify, describe, and explain the characteristics of the disorders, also in cases where a patient experiences a disorder, but has a normal sense of smell. Objectives. The general purpose of this paper was to present our experience with patients seeking medical attention for olfactory disorders, and to describe how quantitative and qualitative olfactory disorders are diagnosed, what the etiologies are, and how quality of life is compromised in patients with olfactory disorders. Subjects and methods. This investigation was performed prospectively over a 10-year period by one physician responsible for the consulting team for patients with olfactory disorders. Based on a standardized clinical examination, a structured interview, and assessment of olfactory function, its aim was to diagnose quantitative and qualitative disorders in 303 consecutive patients. Health-related quality of life was assessed with the Nottingham Health Profile (NHP) in about one-third of patients. Results. In the majority of patients, a reduced sense of smell was found after testing. Often, but not always, this was combined with qualitative disorders. Dominating etiologies were infections, nasal polyposis, head trauma, and aging. Patients complaining of olfactory disorders experience a significantly reduced quality of life regarding the impact of their health problem on paid employment, household work, and social and family life.  相似文献

Conclusion: To the best of the authors’ knowledge, this study is the first to evaluate the olfactory function of adult patients diagnosed with GERD. The results revealed that adults with GERD have diminished olfactory function. Objective: This study aimed to evaluate the olfactory abilities of subjects using the ‘Sniffin’ Sticks’ olfactory test. ObjectivesOlfactory dysfunction is a frequent feature of COVID-19. Despite the growing evidence, current knowledge on the subject remains insufficient, so that data obtained with different tools, from multiple centers and in distinct scenarios are welcome. Yet, the predictive value of olfactory dysfunction in terms of the overall prognosis of COVID-19 is unknown. This study aims to evaluate the olfactory function of hospitalized patients with COVID-19 and the impact of the results on their clinical outcomes.MethodsPatients with severe acute respiratory distress syndrome (ARDS) admitted to a university tertiary hospital were recruited and divided into those with ARDS due to COVID-19, and those with ARDS of any other cause. Sociodemographic and clinical data were collected at baseline and the patients had their objective olfactory function evaluated by the Alcohol Sniff Test on admission and during hospital stay. The participants were then followed up until reaching an endpoint: hospital discharge, endotracheal intubation, transfer to the intensive care unit, or death. Patients with COVID-19 were also subgrouped and compared according to their olfactory thresholds and to their overall clinical outcomes. The obtained data was analyzed using R software. Level of significance was set at 0.05.ResultsEighty-two patients were included (of which 58 had COVID-19). 87.93% of the patients with COVID-19 had diminished olfactory dysfunction on admission. The mean length of hospital stay among patients with olfactory dysfunction was greater (7.84 vs 6.14 days) and nine individuals in this subgroup had poor overall outcomes. None of those with normal olfactory function developed critical COVID-19. The mean olfactory function was significantly worse among patients with COVID-19 and poor outcomes (3.97 vs 7.90 cm, P = .023).ConclusionObjective olfactory dysfunction is frequent in ARDS caused by SARS-CoV-2 infection. Patients with longitudinal poorer outcomes present worse olfactory thresholds on admission.  相似文献

The Scandinavian Odor-Identification Test (SOIT) was developed to address the need for a culturally valid odor-identifi cation test for clinical use on the Scandinavian population that (i) has good ability to generalize performance to olfactory status, (ii) assesses olfactory and trigeminal function separately, (iii) requires only limited cognitive demands, (iv) has normative data and good sensitivity and specificity, and (v) is fast, easy to administer and inexpensive. Sixteen odorous test stimuli were selected that were relatively identifiable, familiar, strong in intensity and pleasant according to healthy participants. Four response alternatives were then selected for each test stimulus based on a confusion matrix of identification rates obtained from healthy participants, in a manner that controlled for task difficulty. Results on the SOIT from healthy persons and hyposmic patients showed satisfactory test-retest reliability, split-half reliability and validity. Cut-off scores for olfactory diagnosis (normosmia, hyposmia, anosmia) based on normative data obtained from 171 healthy persons showed a satisfactory sensitivity and specificity of the SOIT. Assessment of 22 anosmic patients with the SOIT revealed that three of the test stimuli were significant trigeminal stimulants.  相似文献

The aim of this pilot study was to investigate olfactory function in unselected, cooperative patients with ischemic stroke and to gain information about olfactory perception in patients with distinct stroke localizations. Three ischemic stroke patients underwent olfactory testing using the Sniffin’ Sticks test and olfactory event-related potentials recording. All stroke patients were found to be normosmic or only slightly hyposmic when using a psychophysical testing device with age-adjusted norms. No responses or longer latencies on the affected side were found, however, when recording evoked potentials. This seems to argue for the idea that on the affected side it is often possible to document the objective lesion although the subjective perception of the patient appears normal.  相似文献
