Hill-Sachs Remplissage手术治疗骨缺损性复发性肩关节前脱位   总被引:2,自引:0,他引:2  
目的 探讨关节镜下Bankart重建术辅助Hill-Sachs Remplissage手术治疗存在明显骨缺损的复发性肩关节前脱位的疗效.方法 回顾性分析随访2年以上的应用关节镜下Bankart重建术辅助Hill-Sachs Remplissage手术治疗的复发性肩关节前脱位49例患者的病例资料,男42例,女7例;接受手术时年龄16.7~54.7岁,平均28.4岁.49例均为单向不稳定,合并明显的肩盂骨性损伤及巨大的Hill-Sachs损伤.术中采用金属缝合锚钉行Bankart修补,辅助后方冈下肌腱固定填充Hill-Sachs损伤.全部病例随访24~35个月,平均29.0个月,随访时采用ASES评分、Constant-Murley评分、Rowe评分进行功能评估,观察肩关节活动度变化.结果 术前及终末随访时肩关节平均前屈上举162.9°±17.1°和170.9°±7.4°(P=0.007),平均体侧外旋56.0°±17.6°和54.1°±17.1°(P=0.511);ASES评分为(84.7±11.3)分和(96.0±3.4)分(P=0.000),Constant-Murley评分为(93.3±8.7)分和(97.8±3.6)分(P=0.005),Rowe评分为(36.8±8.5)分和(89.8±12.5)分(P=0.000).终末随访时1例患者出现复发脱位,3例患者出现半脱位,失效率8.2%(4/49).此4例患者恐惧试验阳性.结论 肩关节镜下Bankart重建术辅助Hill-Sachs Remplissage手术是治疗存在明显骨缺损的复发性肩关节前脱位的有效方法之一.手术适应证的正确选择、熟练的关节镜下操作技术以及术后长期、严格的功能康复锻炼是手术成功的关键.
Abstract:
Objective The purpose of our study was to report the results of using arthroscopic Remplissage and Bankart repair in patients who had an engaging Hill-Sachs lesion with significant glenoid bone loss. Methods We retrospectively reviewed 49 consecutive patients who underwent arthroscopic Remplissage and Bankart repair for anterior shoulder instability with a mean duration of follow-up of 29.0 months (24-35 months). At the time of surgery the mean age of 42 men and 7 women was 28.4 years. All patients were diagnosed as recurrent anterior shoulder dislocation with a bony lesion of glenoid and an engaging HillSachs lesion. An arthroscopic Remplissage and Bankart repair using metal anchor was performed in all cases.ASES score, Constant-Murley score and Rowe score were used to evaluate the stability and the function of the shoulder. Results Patients' active forward elevation significantly(P=0.007) improved from 162.9°±17.1°preoperatively to 170.9°±7.4° at final follow-up. The external rotation was 56.0°±17.6° before the surgery compared with the 54.1°±17.1° postoperatively(P=0.511 ). The ASES score, Constant-Murley score and Rowe score was 84.7±11.3, 93.3±8.7 and 36.8±8.5 preoperatively compared with 96.0±3.4, 97.8±3.6 and 89.8±12.5 postoperatively. Significant difference could be found with regard to ASES score (P=0.000), ConstantMurley score (P=0.005) and Rowe score (P=0.000). One redislocation happened and a subluxation was noticed in three patients (8.3%). Conclusion Arthroscopic Remplissage combined with Bankart repair can achieve satisfactory for recurrent anterior shoulder dislocation accompany with engaging Hill-Sachs lesion.  相似文献

目的 探讨关节镜下Bankart修复术(缝合锚钉技术)治疗肩关节复发性前脱位的临床效果,并对术后复发不稳的可能危险因素进行分析.方法 2002年3月至2010年3月连续收治肩关节复发性前脱位患者259例,均采用关节镜下缝合锚钉技术进行Bankart修复手术,其中188例患者获得随访.患者手术时平均年龄25.3岁(13~58岁);其中男性143例、女性45例;运动员50名、非运动员138名.随访时采用美国肩肘关节外科协会评分系统(ASES)肩关节评分、Rowe评分以及患者满意度评价手术效果.术前肩关节ASES评分平均72.6分,Rowe评分平均33.4分.对于肩关节术后不稳的复发率、关节活动范围以及术后复发不稳的危险因素进行评估.结果 188例患者术后平均随访38.6个月(12~110个月).术后肩关节ASES评分平均91.9分,与术前比较差异具有统计学意义(P<0.001);术后Rowe评分平均81.9分,与术前比较差异具有统计学意义(P<0.001).患者手术满意度调查显示,满意152例、基本满意16例、不满意20例,满意率为89.4%.术后有24例患者复发脱位,总体复发率为12.8%;运动员患者复发率为28.0%,非运动员复发率为7.2%.术后患者平均肩关节外展外旋为75.2°,与术前比较无明显丧失(P>0.05).关节镜Bankart修复术后复发不稳与患者年龄及是否为运动员明显相关(P<0.05);而与术前病程长短、锚钉类型、锚钉数目、骨性Bankart损伤、合并肩关节上盂唇撕裂损伤、合并后或下方盂唇损伤、合并肩袖撕裂、关节松弛以及肩袖间隙闭合等因素无明显相关(P>0.05).结论 关节镜下采用缝合锚钉进行Bankart修复术是治疗肩关节复发性前脱位的有效方法,临床效果比较满意.年轻患者(≤20岁)和运动员患者是术后肩关节复发不稳的高危因素,必要时选择切开手术.
Abstract:
Objective To evaluate retrospectively the results of arthroscopic Bankart repair using suture anchors for recurrent anterior shoulder dislocation with a minimum 1-year follow-up and to assess risk factors for recurrence.Methods From March 2002 to March 2010,259 patients with recurrent anterior shoulder dislocation underwent arthroscopic Bankart repair with suture anchors.And 188 patients(50 athletes,138 nonathletes)were available for follow-up.The mean age at the time of surgery was 25.3 years (range,13-58 years).The mean follow-up was 38.6 months(range,12-110 months).All of the 188 patients were evaluated preoperatively and postoperatively with the American Shoulder and Elbow Society (ASES)shoulder score and Rowe score system.The rate of recurrent instability,range of motion,and risk factors for postoperative recurrence were evaluated.The ASES score was 72.6 preoperatively,and Rowe score was 33.4.Results The ASES scores improved significantly to 91.9 postoperatively(P<0.001).The Rowe scores improved to 81.9 postoperatively(P<0.001).And 152 patients were greatly satisfied with the results,16 satisfied and 20 unsatisfied.The satisfactory rate was 89.4%.24 patients(12.8%)suffered a recurrence after surgery,14 athletes and 10 nonathletes.The recurrence rates were 28.0% in the athlete group and 7.2% in the nonathlete group.On average there was no significant loss of external rotation postoperatively(average,75.2° preoperatively and 67.2° postoperatively).Patients under age 20,and athlete patients were associated with recurrence(P< 0.05).Other factors including length of time until surgery,type of anchors,number of anchors,presence of bony Bankart lesion,presence of a superior labrum,anterior and posterior tear,presence of posterior or inferior labrum lesion,presence of rotator cuff tear,ligamentous laxity and rotator interval closure did not influence the recurrence rate(P>0.05).Conclusions Arthroscopic Bankart repair is a good option for the treatment of recurrent anterior shoulder dislocation.Identification of risk factors for recurrence allows for consideration of open stabilization.In the series,patients under age 20 and athlete patients are the most important risk factors for recurrence.  相似文献

Background

Recently, arthroscopic Bankart repairs have become much more popular than open repairs for the treatment of recurrent anterior shoulder instability. However, it is unclear whether the modern arthroscopic Bankart repairs using suture anchors could restore equivalent stability to open repairs. We conducted a meta-analysis to compare arthroscopic Bankart repairs using suture anchors and open repairs in regard to clinical outcomes.

Methods

A literature review was performed according to the PRISMA guidelines. PubMed was searched from January 1966 to January 2017. Studies were identified using the terms ‘anterior shoulder dislocation’ or ‘recurrent anterior shoulder dislocation’ and ‘Bankart repair’. The search yielded 36 publications. After reading the full-text articles, we included four randomized controlled trials and five retrospective studies that compared arthroscopic and open repairs using suture anchors.

Results

No significant differences were found between the two procedures in frank re-dislocation and revision surgery due to recurrence. However, the overall recurrent instability including not only re-dislocation but also subluxation and apprehension was significantly higher in arthroscopic repairs than in open repairs, while a significantly higher Rowe score and lower loss of external rotation at 90° of abduction were observed following arthroscopic repairs compared to open repairs.

Conclusions

Modern arthroscopic Bankart repairs using suture anchors provide an equivalent outcome compared to open repairs in terms of apparent re-dislocation, but overall recurrent instability including subluxation or apprehension was still significantly higher in arthroscopic repairs than in open repairs.  相似文献

OBJECTIVE: Arthroscopic refixation of the labrum-ligament complex at the glenoid. INDICATIONS: Posttraumatic anterior or anterior-inferior shoulder instability with Bankart or ALPSA lesion (anterior labral periosteal sleeve avulsion). CONTRAINDICATIONS: Atraumatic shoulder instability. Instabilities due to blunted or frayed degeneration of the labrum-ligament complex. HAGL lesion (humeral avulsion of the glenohumeral ligaments) with humeral detachment of the glenohumeral ligaments. Larger bony glenoid defects. SURGICAL TECHNIQUE: Mobilization of the labrum-ligament complex from the neck of the glenoid, superior tightening and refixation at the glenoid rim with the aid of absorbable suture anchors. POSTOPERATIVE MANAGEMENT: Immobilization of the affected arm for 4 weeks in an immobilization bandage with abduction pillows. Daily pendulum exercises. Active flexion up to 70 degrees and abduction up to 40 degrees, all in neutral or internal rotation. Avoidance of external rotation for a total of 6 weeks. RESULTS: From January 1999 to December 2001, 58 patients with a Bankart or ALPSA lesion were treated with arthroscopic shoulder stabilization using absorbable suture anchors and slowly absorbable braided sutures. 56 patients underwent a follow-up clinical examination after, on average, 31 months (24-48 months). None of these patients had suffered more than five shoulder dislocations before the operation (average 2.8). Of the intraoperative lesions, a plain Bankart lesion was present in twelve patients (21.4%), 44 patients had an ALPSA lesion (78.6%), of which one in two were combined with an SLAP 2 or SLAP 3 lesion (superior labrum from anterior to posterior). In the evaluation using the Rowe Score, there was an excellent result for 40 patients (71.4%), and a good result for twelve (21.4%). Four patients suffered a repeat dislocation and were therefore classified as poor results (7.2%).  相似文献

目的 探讨关节镜下非打结型缝合锚钉修补Bankan损伤治疗复发性肩关节前脱位的疗效.方法 复发性肩关节前脱位患者14例,均为男性;年龄18~34岁,平均25.2岁;左侧4例,右侧10例,涉及主力侧12例;均为单向不稳.术前脱位次数为3~36次,平均13.5次.关节镜下采用可吸收非打结型缝合锚钉治疗Bankart损伤.术前及术后随访采用ASES评分及Constant-Murley功能评估.结果 所有患者均获得随访,随访时间11~22个月,平均17个月.术中发现9例合并Hill-Sachs损伤,3例合并骨性Bankart损伤,2例合并后下盂唇损伤,2例合并SLAPⅡ型损伤,1例合并关节内游离体.14例患者术前及末次随访时肩关节平均前屈上举为163.4°±8.6°和169.7°±4.2°;外展90°时,平均外旋角度分别为58.5°+13.6°和90.3 °±5.5.;术后外展90°时,患侧外旋角度较健侧受限8.4°±6.2°术前及末次随访时ASES评分为(77.4±3.7)分和(94.3±2.6)分,两者比较差异有统计学意义(P<0.05);其中VAS不稳定评分平均为(7.2±1.4)分和(1.2 ±0.6)分(P<0.01);Constant-Mudey评分平均为(78.1±4.6)分和(93.9±3.7)分.术后无一例患者发生再脱位,且均重返伤前工作岗位.结论 肩关节镜下Bankart 重建手术是治疗复发性肩关节脱位的有效方法 .非打结型缝合锚钉简化了手术操作步骤,减少手术时间和创伤.  相似文献

Background

To report the results of an arthroscopic percutaneous repair technique for partial-thickness tears of the anterosuperior cuff combined with a biceps lesion.

Methods

The inclusion criteria were evidence of the upper subscapularis tendon tear and an articular side partial-thickness tear of the supraspinatus tendon, degeneration of the biceps long head or degenerative superior labrum anterior-posterior, above lesions treated by arthroscopic percutaneous repair, and follow-up duration > 24 months after the operation. American Shoulder and Elbow Surgeons (ASES) score, constant score, the pain level on a visual analogue scale, ranges of motion and strength were assessed.

Results

The mean (± standard deviation) age of the 20 enrolled patients was 56.0 ± 7.7 years. The forward flexion strength increased from 26.3 ± 6.7 Nm preoperatively to 38.9 ± 5.1 Nm at final follow-up. External and internal rotation strength was also significantly increased (14.2 ± 1.7 to 19.1 ± 3.03 Nm, 12.3 ± 3.2 to 18.1 ± 2.8 Nm, respectively). Significant improvement was observed in ASES and constant scores at 3 months, 1 year and the time of final follow-up when compared with preoperative scores (p < 0.001). The mean subjective shoulder value was 86% (range, 78% to 97%).

Conclusions

The implementation of complete rotator cuff repair with concomitant tenodesis of the biceps long head using arthroscopic percutaneous repair achieved full recovery of normal rotator cuff function, maximum therapeutic efficacy, and patient satisfaction.  相似文献

Lesions of the superior labrum can be a source of significant shoulder pain and disability. SLAP (superior labrum anterior-posterior) tears have been classified into many different types. A type IV SLAP tear is a bucket-handle tear of the superior labrum with extension into the biceps tendon. This relatively uncommon SLAP tear, if present, has been shown to be frequently associated with other pathology including Bankart lesions. We present an arthroscopic technique for combined repair of a type IV SLAP tear and Bankart lesion. Steps include initial reduction of the bucket-handle portion of the superior labral injury, repair of the anterior-inferior labral detachment, and, finally, repair of the superior labrum and biceps tendon split.  相似文献

Between April 1990 and April 1994, 100 patients with a preoperative diagnosis of anterior instability underwent a diagnostic arthroscopy of the shoulder. Patients with isolated SLAP lesions were excluded from the study. Patients with multidirectional instability, bony Bankart lesions, and large Hill-Sachs lesions were also excluded. Football players and dominant arm throwing athletes were also excluded. Thirty patients remained who had post-traumatic, unidirectional, anterior instability and a repairable Bankart lesion and did not have any exclusions as noted above. All of these patients underwent an initial attempt at an arthroscopic Sure-Tac stabilization (Smith & Nephew, Andover, MA). Twenty-three patients met our criteria for a secure fixation but 7 did not. These 7 underwent an immediate arthrotomy and open Bankart repair. All of the patients were available at follow-up at an average of 47 months (range, 36 to 72 months). The patients were evaluated by the Rowe shoulder rating scale. There have been two cases of recurrent subluxation and one case of recurrent dislocation in the Sure-Tac group. All three occurred over 2 years later. All three had recurrent Bankart lesions and underwent an arthrotomy and Bankart repair. There were no cases of recurrence of subluxation or dislocation in our initial open Bankart repair group. Sure-Tac arthroscopic anterior stabilization of the shoulder can initially give good results but these results appear to deteriorate over time and increased activity of the patient. We currently do not recommend a Sure-Tac repair even in a selective group of patients with an isolated Bankart lesion. This is based on our excellent results following an open Bankart repair and a 13% recurrence rate following Sure-Tac stabilization in carefully selected patients.Arthroscopy 1998 May-Jun;14(4):389-94  相似文献

Purpose

With increased sports participation and medical community awareness, there appears to be an increase in pediatric musculoskeletal injuries. Our purpose was to identify the intra-articular injury pattern seen within the pediatric shoulder.

Methods

A retrospective review was performed at two tertiary-care children’s hospitals between 2008 and 2011 on all patients who underwent magnetic resonance imaging (MRI) and subsequent shoulder arthroscopy. Exclusion criteria included: girls >14 years old and boys >16 years old. Demographics, MRI and arthroscopic findings were recorded. Labral pathology was grouped: Zone I (Bankart lesions, 3–6 o’clock for right shoulder), Zone II (posterior labral lesions, 6–11 o’clock), Zone III (SLAP lesions, 11–1 o’clock), and Zone IV (anatomic variants, 1–3 o’clock).

Results

One hundred and fifteen children met criteria, mean age 14.4 years (range 8–16). There were 24 girls and 91 boys, with 70 right shoulders. Of 108 children, labral pathology involved: 72 Zone I (16 isolated anterior), 56 Zone II (15 isolated posterior), 38 Zone III (four isolated superior), and three had an isolated Buford complex. Seventy had more than one labral zone injured, and 31 (30 %) had more than two zones injured. Non-labral pathology included partial rotator cuff tears and humeral avulsions of the glenohumeral ligament.

Conclusion

With 94 % of intra-articular pathology being labral tears, the distribution of proportion in children differs from adults; moreover, 23 % involved only the posterior or posterosuperior labrum. Treating surgeons should be prepared to find anterior tears extending beyond the zone of a classic Bankart lesion and an association with C rotator cuff tears.  相似文献

We report our experience with arthroscopic repair of the Bankart lesion following traumatic unidirectional anterior shoulder dislocation. Thirty consecutive patients (7 women, 23 men; average age, 26.5 years) were followed for an average of 38 months (minimum 2-year follow-up) after arthroscopic Bankart suture repair for recurrent shoulder dislocation. The study included patients who had pure shoulder dislocations (excluding those with instability secondary to subluxation, multidirectional instability, or an atraumatic origin), had experienced an initial frank shoulder dislocation (documented radiographically or requiring the assistance of medical personnel for reduction), and had a Bankart lesion, visualized arthroscopically. Clinical evaluation using the Rowe functional grading system showed 11 patients rated as excellent, 8 as good, 3 as hair, and 8 as poor. Six of 8 patients were rated as poor because they frankly redislocated following their arthroscopic shoulder stabilization. Our study shows a 27% failure rate in this group. Critical reevaluation of the transglenoid arthroscopic Bankart procedure is mandatory to identify the appropriate patient population for this procedure.  相似文献

Introduction and importanceThe two major etiologies of shoulder superior labral tears anterior to posterior (SLAP) are traumatic and degenerative processes. Bucket handle tears of the superior labrum represent one-third of labral lesions. However, in this article, we present a double bucket handle tear which has been reported once in the literature.Presentation of caseA 25-year-old male presented with complaint of chronic pain in his right shoulder with a remote history of traumatic dislocation. Physical examination revealed a positive apprehension test. Shoulder magnetic resonance imaging (MRI) showed a superior labral tear with a Hill-Sach lesion. Arthroscopy showed a double bucket handle tear of superior labrum and mild biceps tendonitis along with Bankart lesion. The tear was resected and the Bankart lesion was repaired followed by supervised physical therapy. Good clinical outcomes in form of resolution of pain and shoulder instability at six months were obtained.DiscussionSLAP tears are common shoulder lesion that is reported differently in the literature. Arthroscopic studies had reported the incidence between 3.9%-11.8. The diagnosis of such lesion relies on the clinical presentation and imaging. Knesek et al. classified SLAP lesions based on the integrity of the biceps anchor and the type of labral tear (Knesek et al., 2013). The standard treatment of symptomatic SLAP lesions is Arthroscopic debridement. However, non-operative management was described in the literature.ConclusionDouble bucket handle injuries of the superior labrum are reported in literature once. These lesions can be treated with arthroscopic debridement and Bankart repair and followed by supervised physical therapy.  相似文献

Background/objectives

Despite the improvements in the methods of arthroscopic stabilization of anterior shoulder instability, a recurrence rate of as high as 30% is reported in the literature. In this context, we report the outcome of arthroscopic Bankart repair in anterior shoulder instability, with the use of bio-absorbable suture anchors for patients that were followed up for at least two years from the date of surgery.The arthroscopic method offers a less invasive technique of Bankart repair for traumatic anterior shoulder instability. We would like to report the 2-year clinical outcomes of bio-absorbable suture anchors used in traumatic anterior dislocations of the shoulder.

Methods

Data from 79 shoulders in 74 patients were collected over 4 years (2005–2009). Each patient was followed up over a period of 2 years. The patients underwent arthroscopic Bankart repair using bio-absorbable suture anchors for their anterior shoulder instability. These surgeries were performed at a single institution by a single surgeon over the time period. The patients were assessed with two different outcome measurement tools. The University of California at Los Angeles (UCLA) shoulder rating scale and the Simple Shoulder Test (SST) score. The scores were calculated before surgery and at the 2-year follow-up. The recurrence rates, range of motion as well post-operative function and return to sporting activities were evaluated.

Results

SST results from the 12 domains showed a significant improvement from a mean of 6.1 ± 3.1 to 11.1 ± 1.8 taken at the 2-year follow-up (p < 0.0001). Data from the UCLA scale showed a pre and post-operative mean of 20.2 ± 5.0 and 32.4 ± 4.6 respectively (p < 0.0001). 34 had excellent post-operative scores, 35 had good scores, 1 had fair score and 3 had poor scores. 75% of the patients returned to sports while 7.6% developed a recurrence of shoulder dislocation or subluxation.

Conclusion

Arthroscopic Bankart repair with the use of suture anchors is a reliable treatment method, with good clinical outcomes, excellent post-operative shoulder motion and low recurrence rates.  相似文献

BACKGROUND: The purpose of this study was to evaluate prospectively the surgical outcomes of arthroscopic repair of anterior capsulolabral lesions with use of suture anchors in a large series of patients who were followed for two to six years. METHODS: We evaluated the results of arthroscopic Bankart repair with use of suture anchors and nonabsorbable sutures in 167 patients with traumatic recurrent anterior instability of the shoulder. The mean age at the time of the operation was twenty-five years. Preoperatively and at the time of follow-up (at a mean of forty-four months), the patients were assessed with three objective outcome measurement tools (the Rowe score, the University of California at Los Angeles [UCLA] shoulder rating scale, and the American Shoulder and Elbow Surgeons [ASES] score) and two subjective measurement tools (pain and function visual analog scales). The recurrence rate, range of motion, and risk factors for postoperative recurrence were evaluated. RESULTS: All shoulder scores improved after surgery (p < 0.001). According to the Rowe scale, 130 patients (78%) had an excellent score; twenty-nine (17%), a good score; six (4%), a fair score; and two (1%), a poor score. Overall, the rate of postoperative recurrence of instability was 4% (one dislocation, two subluxations, and four positive results on the anterior apprehension test). Postoperative recurrence was related to an osseous defect of >30% of the entire glenoid circumference. In the patients with recurrent postoperative instability, the episodes were less frequent than they had been preoperatively and shoulder function was related to activity level. A revision arthroscopic Bankart repair stabilized three of the four shoulders in which it was performed. One hundred and fifty-two patients (91%) returned to >/=90% of their preinjury activity level. The mean loss of external rotation (and standard deviation) was 2.0 degrees +/- 4.0 degrees. CONCLUSIONS: We found that, in contrast to previous reports on the results of arthroscopic repair, arthroscopic capsulolabral repair with use of suture anchors can provide satisfactory outcomes in terms of recurrence rate, activity, and range of motion.  相似文献

Australian Rules football (ARF) is a potentially violent, overhead, body-contact sport. We reviewed 56 shoulders in patients who sustained their initial traumatic anterior subluxation or dislocation during ARF and who underwent reconstructive surgery for traumatic anterior instability, whether by arthroscopic or by open techniques. Patients were followed up for a mean of 29.4 months after operation, and clinical evaluation was performed with the Rowe grading system. Three types of surgical procedures were performed: arthroscopic suture repair, arthroscopic Bankart repair with an absorbable polyglyconate tack, and open capsular shift with repair of the Bankart lesion. Shoulders treated with arthroscopic suture repair had a 70% rate of recurrent subluxation or dislocation on return to ARF Dislocations treated arthroscopically with the biodegradable tack had a 38% rate of recurrence of instability; three fourths of the recurrences were after minimal to moderate trauma. Shoulders treated with an open capsular shift and Bankart procedure had a 30% rate of recurrent instability, with half of the recurrences caused by violent trauma. In the open group there were no failures in patients who did not return to ARF. We suggest that arthroscopic repair in shoulders with anterior instability and recurrent dislocation does not adequately address the plastic deformation of the anterior capsule that may occur after repeated episodes of dislocation. We advocate open shoulder procedures in ARF athletes to address all areas of the capsulolabral pathologic condition and to provide the most secure repair possible with minimal chance of recurrence.  相似文献

目的探讨肩关节镜下缝线锚钉修复合并关节盂唇上部从前到后的损伤(SLAP)的疗效。方法 2007年至2009年,结合体格检查、MRI和关节镜诊断为关节盂唇前后沿伸撕裂的SLAP损伤患者12例,关节镜下采用缝线锚钉治疗。其中SLAP-Ⅴ型损伤(Bankart损伤+SLAPⅡ型损伤)7例,SLAP-Ⅷ型损伤(SLAPⅡ型损伤合并后下方盂唇撕裂)3例以及一种新的SLAP损伤类型2例,即SLAPⅢ型损伤+前后盂唇撕裂并脱位。术前及术后随访采用ASES评分及Constant-Murley功能评估。结果所有患者平均随访27个月。术前及终末随访ASES评分为(77.4±3.7)分vs(94.3±2.6)分,两者比较差异有统计学意义(t=28.1,P〈0.05);Constant-Murley评分为(78.1±4.6)对(93.9±3.7)分(t=28.9,P〈0.05)。术后无一例患者发现再脱位,且均重返伤前工作岗位。结论随着肩关节镜技术的不断进步,肩关节盂唇严重损伤的修复更加有效、微创及简便。  相似文献

Introduction

The increasing number of surgically treated superior labrum anterior to posterior (SLAP) lesions in the past decade calls for an investigation of the long-term clinical results and assessment of possible predictive factors that could have an influence on the postoperative outcome.

Patients and methods

Out of 77 patients 60 were treated surgically with arthroscopic SLAP repair. Of these 78?% of the patients were available at final follow-up and the mean duration of follow-up was 57.7 months (range 26?94 months). All patients were followed up clinically by evaluation of the Constant-Murley (CMS), Rowe (RS) and the subjective shoulder value (SSV) scores. Patients were subdivided according to age at time of surgery, concomitant rotator cuff pathology, existing isolated SLAP lesions, existing accompanying injury, etiology of injury and pre-existing cartilage lesion at the time of surgery.

Results

The median age and gender-adjusted CMS was 89?% (range 38–106?%). The average functionality of the operated shoulder in SSV was 90?% and 87?% of patients were very satisfied or satisfied with the clinical outcome. Both the age and generally accompanying lesions and in particular partial lesions of the rotator cuff had no effect on the postoperative outcome. Pre-existing cartilage lesions resulted in significantly lower functionality of the shoulder as part of the SSV (p?=?0.0221).Both absolute and age and gender-related CMS (p?=?0.0104) and SSV (p?=?0.0418) were significantly higher in the posttraumatic group than the group with degenerative etiology.

Discussion

Clinical results after arthroscopic repair of SLAP lesions are stable in an average long-term of approximately 5 years. The degenerative and recurrent microtraumatic etiology and pre-existing cartilage damage can be predictive factors for a worse postoperative outcome.  相似文献
