The french population is growing older every year. As a result, the practice of medicine will continue to shift toward the care of elderly patients. Infections are a common problem in the elderly. The most common infections are urinary and respiratory tract infections. Whenever possible, clinicians should collect appropriate cultures before initiating antibiotic therapy. In practice, clinical and microbiological diagnosis of infection in the elderly are generally not well documented. Because the elderly appear to be at an increased risk for more severe complications of infection, it is usually appropriate to begin treatment with empiric antibiotics. Drug dosages and galenic forms should be appropriate for elderly patients and selected according to the suspected causative organism, the severity of infection, and potential drug interactions and toxicity. Adverse drug reactions, pharmacokinetic changes (renal dysfunction) and drug interactions are common in the elderly. The calculation of creatinine clearance is the most useful measure of renal function in elderly patients. Amoxicillin with clavulanic acid, cephalosporins and quinolones are the first-line antibiotics in the elderly. 相似文献
