目的 评价经食管超声心动图(TEE)对指导小儿先天性心脏病(先心病)外科手术和介入治疗的应用价值及其安全性。方法 2000年11月至2005年1月,在上海市复旦大学附属儿科医院心血管中心接受外科开胸手术或经导管介入治疗的先心病患儿中有317例进行了TEE检查,年龄2个月至17岁6个月(平均4.7岁)。采用HP/SONOS2500超声诊断仪,频率5.5~7.5MHz双平面经食管探头,全麻状态下进行。结果 与术前经胸超声心动图(TTE)比较,术前TEE检查对诊断作出修正或补充者51例(16.1%),其中因此而修正了手术治疗方案25例(7.9%)。术后TEE检查发现有并发症或残余问题57例(18.0%),其中8例(2.5%)因此立即再次手术。所有病例均未因TEE检查而引起并发症。结论 TEE可安全地应用于小儿先心病围术期检查,对术前诊断做出修正或补充,术后可及时发现并发症或残余问题。 Abstract Objective To evaluate the utility of transesophageal echocardiography (TEE) for surgical and interventional repairs in children with congenital heart disease (CHD) and its safety as well.Methods 317 patients with CHD underwent TEE examinations at the age of 2 months~17 years and 6 months(mean 4.7 years).HP/SONOS2500 ultrasonic instrument with transesophageal biplane probe of 5.5~7.5MHz was used to perform TEE under general anesthesia.Results Compared with preoperative transthoracic echocardiography(TTE),TEE had new findings or made revision of the diagnoses in 51 cases(16.1%) before operations,which led to the changes of surgical or interventional precedure in 25 cases(7.9%).TEE had found residual problems or evolving situations in 57 cases(18.0%) after operations,which led to a second immediate surgical intervention in 8 cases(2.5%).There were no detectable complications due to TEE in all cases.Conclusion TEE is valuable in providing meaningful preoperative emendations or additions and finding residual problems after operations. 目的 探讨彩色多普勒超声心动图检测胎儿先天性心脏病 (先心病 )的可行性。 方法  2 96例胎儿分为两组 :14 2例有高危因素的胎儿 ,15 4例正常妊娠胎儿。应用多普勒测定胎儿心脏各切面 ,并在生后作超声心动随访。 结果 在 14 2例有高危因素的胎儿中 ,发现胎儿先心病 5例 ,其中右心发育不良综合征 1例、法洛四联症 2例、单纯性室间隔缺损 (室缺 ) 2例。漏诊 2例 ,为单纯性室缺1例及房间隔缺损 1例。 15 4例正常妊娠胎儿中未发现先心病。生后超声心动随访或引产尸检结果与产前诊断相符。 结论 彩色多普勒超声心动图在诊断胎儿先心病方面是比较准确和安全的 ,对高危孕妇需加强胎儿先心病的检测  相似文献

新生儿先天性心脏病385例彩色多普勒超声诊断及其临床意义   总被引:16,自引:1,他引:16  
目的 研究新生儿先天性心脏病(简称先心病)的类型并判断其预后,以提出减少发病及死亡的对策。方法 分析1991-1999年新生儿先心病385例的彩色多普勒超声特点,并对87例非青紫先心病进行随访2个月-7年,观察其预后。结果 本组病例常见类型的顺位是:左向右分流畸形(69.4%),大动脉转位(10.4%),肺动脉出口梗阻(6.2%),左室出口梗阻(4.4%),肺静脉连接异常(3.6%)。新生儿期众议院的335例中,106例(47.8%)是在生后1周内入院,335例中重症先心病50例(14.9%)。本组病例死亡及抢救无望家长放弃者共79例(21%),其中47%出生日龄在1周内,重症先心病中大动脉转位、左心发育不良、肺动脉狭窄或闭锁发病及死亡均早,死亡及放弃中以大动脉转位占首位(33%)。室间隔缺损≤3mm者,经随访65%自然关闭;而≥6mm者中,仍未闭或经手术矫治共占92%,此组病例51%合并肺动脉高压。房间隔缺损5-6mm者,经随访60%自然关闭;7-10mm者,67%未闭,33%手术。动脉导管未闭>5mm者100%手术。结论 为减少先心病的发病和减少死亡,应重视胎儿心脏超声检查,早期发现重症先心病及时终止妊娠;生后对先心病也应早做超声诊断,发现重症及早外科治疗,轻者定期随访。  相似文献

目的研究糖尿病孕妇胎儿的心功能变化。方法应用多普勒超声心动图测定23例妊娠期糖尿病孕妇胎儿(糖尿病孕妇胎儿组)和54例正常胎儿(正常胎儿组)心脏的解剖数值:室间隔厚度(IVS)、主动脉(AO)、肺动脉(PA)、二尖瓣(MV)、三尖瓣(TV)的直径,并测定主、肺动脉血流的峰速(PFV)、平均速度(V)、加速时间(ACT)、每搏输出量(SV)、心输出量(CO)、心脏收缩功能参数(ForceF)。测定二、三尖瓣血流舒张早期峰值流速(PFVe)、舒张晚期峰值流速(PFVa)、平均速度(V)、表示舒张功能的参数。结果糖尿病孕妇胎儿组与正常胎儿组相比,IVS明显增厚,AO、PA、MV、TV直径均增大。主、肺动脉血流的PFV、SV、ForceF均增加,ACT无明显变化。二、三尖瓣的PFVa明显增加、二、三尖瓣血流舒张早期峰值流速及舒张晚期峰值流速比值(E/A)明显下降。结论糖尿病孕妇的胎儿体重、心脏相对较大,心脏收缩力增强,心输出量增加。IVS的明显增厚使舒张功能受到影响。  相似文献

目的:研究宫内发育迟缓胎儿的心功能变化。方法:应用多普勒超声心动图测定64例正常胎儿(正常胎儿组)和36例IUGR胎儿(IUGR组)主动脉、肺动脉血流的峰速(PFV)、平均速度(V)、加速时间(ACT)、每搏输出量(SV)、心输出量(CO),以此表示心脏收缩功能的参数。测定二、三尖瓣血流的舒张早期峰值流速(PFVe)、舒张晚期峰值流速(PFVa)、E/A、V表示舒张功能的参数。结果:在正常胎儿组中,主动脉的PFV、V、ACT均大于肺动脉,而左心室的SV、左心室输出量(LCO)、均小于右心室。二尖瓣的PFVe、PFVa、均小于三尖瓣。IUGR组与正常胎儿组比较,主、肺动脉的PFV、V、SVCO、均明显下降,肺动脉的ACT明显减少,主动脉的ACT明显增加,左心室SV、LCO、均大于右心室。二尖瓣及三尖瓣的E/A明显下降,三尖瓣的PFVe、PFVa、V明显下降,二尖瓣的PFVe、PFVa均大于三尖瓣。结论:在IUGR胎儿中可能发生“脑微效应”,即胎儿缺氧后,出现血流灌注再分布。  相似文献

目的 探讨Tei指数在监测妊娠合并心脏病伴肺动脉高压患者右心室功能中的作用及右心室压力负荷增加对左心室功能的影响. 方法选择妊娠合并心脏病患者58例(先天性心脏病36例,风湿性心脏病14例,心律失常8例),其中26例伴有肺动脉高压(肺动脉高压组),根据肺动脉压力情况分为轻度 [30~49 mm Hg(1 mm Hg=0.133 kPa)] 11例,中度 (50~79 mm Hg) 9例,重度 (≥80 mm Hg) 6例,余32例为非肺动脉高压组.以15例正常妊娠妇女作为对照组,分别计算左、右心室Tei指数. 结果(1)肺动脉高压组右心室等容舒张时间(IRT)为(93±52)ms,等容收缩时间(ICT)为(66±41)ms,较对照组[(39±19)、(38±20) ms]及非肺动脉高压组[(59±12)、(43±19) ms]均明显延长,差异均有统计学意义(P<0.01,P<0.05);而肺动脉高压组右心室射血时间(ET)为(239±46)ms, 较对照组[(299±38) ms]及非肺动脉高压组[(250±41)ms]均明显缩短,差异也均有统计学意义(P<0.01,P<0.05);肺动脉高压组右心室Tei指数为0.72±0.49,明显高于对照组的0.38±0.12和非肺动脉高压组的0.43±0.16,差异也均有统计学意义(P<0.01,P<0.05).(2)肺动脉高压组左心室IRT[(99±27)ms]长于对照组[(88±20) ms]和非肺动脉高压组[(95±15) ms],差异有统计学意义(P<0.01);但3组左心室ICT比较,差异无统计学意义(P>0.01),肺动脉高压组左心室ET为[(202±26)ms]短于对照组[(290±21)ms]和非肺动脉高压组[(220±36)ms],差异有统计学意义(P<0.05,P<0.05);肺动脉高压组左心室Tei指数(0.77±0.38)明显高于对照组(0.43±0.15)和非肺动脉高压组(0.58±0.21),差异也有统计学意义(P<0.01,P<0.05).(3) 右心室Tei指数与肺动脉收缩压具显著正相关关系(r=0.84,P<0.01).(4)肺动脉高压组中,重度患者右心室Tei指数(0.75±0.43)较轻度患者(0.68±0.35)明显升高,差异有统计学意义(P<0.01),较中度患者(0.71±0.14)也明显升高(P<0.05). 结论(1)Tei指数是评价妊娠合并心脏病伴肺动脉高压患者右心室功能简便而准确的多普勒超声新指标,并且Tei指数高低可反映疾病的严重程度;(2)右心室压力负荷增加对左心室功能有明显影响.  相似文献

Objective To investigate clinical value of Tei index used for monitoring right ventricular (RV) function in pregnant women complicating cardiac disease accompanying pulmonary hypertension(PH) and the influence on left ventricular(LV) function due to overload pressure of RV. Methods Fifty-eight pregnant women complicating cardiac disease (including 32 cases with congenital heart disease,14 cases with rheumatic heart disease and 8 cases with arrhythmia)were enrolled in this study, among 26 cases coexisted with pulmonary hypertension. According to the pressure of PH, those patients were divided into three groups:11 cases in mild group[30-49 mm Hg (1 mm Hg=0.133 kPa)],9 cases in moderate group (50-79 mm Hg)and 6 cases in severe group (≥80 mm Hg). In the mean time, 15 healthy pregnant women were matched as control. Tei index were measured for LV and RV respectively. Results (1) The isovolumetric relaxation time [IRT,(93±52) ms] and isovolumetric contraction time [ICT,(66±41) ms] of RV in PH group were significantly higher than normal controls [(39±19) ms in IRT and (38±20) ms in ICT] and the other patients without PH group[(59±12) ms in IRT and (43±19)ms in ICT, P<0.01, P<0.05;P<0.05,P<0.05]; however, ejection time (ET) was(239±46) ms significantly shortened in PH group (P<0.05,P<0.01) when compared with (250±41) ms in patients without PH and (299±38) ms in normal controls. Tei index in PH group were 0.72±0.49, which were significantly higher than 0.38±0.12 in normal controls and 0.43±0.16 in patients without PH (P<0.01,P<0.05). (2) The IRT[(99±27)ms] and ICT[(71±40) ms] of LV in PH group were significantly higher than in normal controls [(88±20) ms,(50±24)ms] (P<0.01,P<0.01). ET of LV in PH group [(202±26)ms] were significantly shortened that (290±21)ms in normal controls and (220±36)ms in patients without PH (P<0.01,P<0.05). Tei index of LV in PH group were significantly higher than 0.43±0.15 in normal controls and 0.58±0.21 in patients without PH (P<0.01,P<0.05). (3) Positive correlation between Tei index of RV and pressure of pumonaroy artery were observed (r=0.84, P<0.01). (4) Tei index of RV in severe PH were significantly higher than mild PH (0.75±0.43 vs. 0.68±0.35, P<0.01) and moderate PH (0.75±0.43 vs. 0.71±0.14, P<0.05). Conclusions (1)The Tei index is a novel efficient Doppler index in assessing RV function of pregnant women complicating cardiac disease accompanying PH. In the mean time, 15 healthy pregnant women were matched as control. Tei index were measured for LV and RV respectively. Results (1) The isovolumetric relaxation time [IRT,(93±52) ms] and isovolumetric contraction time [ICT,(66±41) ms] of RV in PH group were significantly higher than normal controls [(39±19) ms in IRT and (38±20) ms in ICT] and the other patients without PH group[(59±12) ms in IRT and (43±19)ms in ICT, P<0.01, P<0.05;P<0.05,P<0.05]; however, ejection time (ET) was(239±46) ms significantly shortened in PH group (P<0.05,P<0.01) when compared with (250±41) ms in patients without PH and (299±38) ms in normal controls. Tei index in PH group were 0.72±0.49, which were significantly higher than 0.38±0.12 in normal controls and 0.43±0.16 in patients without PH (P<0.01,P<0.05). (2) The IRT[(99±27)ms] and ICT[(71±40) ms] of LV in PH group were significantly higher than in normal controls [(88±20) ms,(50±24)ms] (P<0.01,P<0.01). ET of LV in PH group [(202±26)ms] were significantly shortened that (290±21)ms in normal controls and (220±36)ms in patients without PH (P<0.01,P<0.05). Tei index of LV in PH group were significantly higher than 0.43±0.15 in normal controls and 0.58±0.21 in patients without PH (P<0.01,P<0.05). (3) Positive correlation between Tei index of RV and pressure of pumonaroy artery were observed (r=0.84, P<0.01). (4) Tei index of RV in severe PH were significantly higher than mild PH (0.75±0.43 vs. 0.68±0.35, P<0.01) and moderate PH (0.75±0.43 vs. 0.71±0.14, P<0.05). Conclusions (1)The Tei index is a novel efficient Doppler index in assessing RV function of pregnant women complicating cardiac disease accompanying PH. And the fluctuation of Tei index might reflect seriousness of the disease. (2) The overload pressure of RV pressure due to PH has significant influence on LV function.  相似文献

目的:探讨彩色多普勒超声诊断胎儿心脏病的临床价值。方法:应用Acuson128×P/10彩色多普勒超声诊断仪检查368例孕20~40周的心脏病高危胎儿,并将产前超声检查与胎儿尸体检查或分娩后彩色多普勒超声检查结果进行对照。结果:产前超声检查发现11例胎儿心脏病,5例经引产后胎儿尸体检查证实,5例经产后彩色多普勒超声检查证实,1例假阳性,1例假阴性。结论:彩色多普勒超声是产前诊断胎儿心脏病的重要方法。  相似文献

目的探讨超声心动图对胎儿先天性心脏结构异常诊断的价值.方法2004-01-2005-10江门市妇幼保健院对724例胎儿心脏进行了二维、M型、彩色多普勒血流显像检查.取胎儿四腔心切面,三血管平面,大血管短轴切面,左心室长轴切面,主动脉长轴切面,右室两腔切面等多切面检查.结果724例胎儿超声心动图检查中15例诊断胎儿先天性心脏结构异常,4例伴有多发畸形.13例进行了治疗性引产,其中9例均经尸解证实超声诊断结构,4例拒绝尸解;1例心包积液;1例超声诊断为卵圆孔增大,出生后复查心脏结构未见异常.709例产前超声心动图检查未发现异常者3例于出生后发现先天性心脏病.结论超声心动图对胎儿先天性心脏结构异常的诊断具有重要价值.四腔心切面、三血管平面是超声诊断先天性心脏结构异常最重要的切面.  相似文献
