丙型肝炎病毒(HCV)和人类免疫缺陷病毒(HIV)合并感染的情况十分多见,在经输血或单采血浆而感染HIV者中甚至高达90%以上。高效抗逆转录病毒治疗(HAART)的应用使HIV相关的综合征和病死率大大下降。却使HCV慢性感染在共感染者中的致病作用日益凸现。本研究旨在通过比较HIV/HCV共感染组与HCV单纯感染组的肝组织病理学改变,观察HIV/HCV合并感染者肝组织的病理学特征。  相似文献

人类免疫缺陷病毒相关性肾病(human immunodeficiency virus-associatied nephrophath,HIVAN)是人类免疫缺陷病毒(humman immunodeficiency virus,HIV-1)感染导致的一种肾脏疾病,以蛋白尿、氮血症为主要病理改变,预后差,近年来,由于HIV的传播HIVAN的发病率也随之不断增加,已引起临床医师的高度重视,在过去几年中,人们对HIVAN的发病机制,临床过程和病理改变有了更新的认识,药理治疗也取得显著效果。本文将对HIVAN的现状及治疗进展进行综述。  相似文献

目的 首次报道我国于1999年5月开始对人类免疫缺陷病毒(HIV)-1感染者的规范化高效抗逆转录病毒治疗。方法 用齐多夫定+拉米夫定(AZT+3TC,商品名:双汰芝)联合硫酸茚地那韦(indinavir,商品名:佳息患)对15例HIV感染或艾滋病患者进行为期1年的治疗。随访指标为病毒载量和T淋巴细胞亚群分析。结果 15例随访1年,用药3个月后HIV-1 RNA平均值至198拷贝/ml,比治疗前的90743RNA拷贝/ml下降2.7log。用药后12个月CD4细胞计数平均增加67个/μl,CD8细胞计数平均减少192个/μl,CD4/CD8比例从0.35增加到0.56,15例中2例未作T淋巴细胞亚群分类,13例治疗后3、6、9、12个月CD4^ 童贞细胞(CD45RA+CD62L+)数呈现平稳上升趋势,在治疗1年时平均升高42个/μl。而CD8^ 童贞细胞(CD45RA+CD62L+)数平均升高19个/μl。所有患者用药后出现消化道反应,3例出现一过性黄疸,2例出现泌尿系结石。结论 与国外临床报道的治疗效果相一致,15例HIV-1不同阶段感染者经治疗后病毒载量水平明显降低,CD4平均细胞数有所增加,在具有不同病毒基因亚型的病例显示同样的治疗效果。  相似文献

目的分析182例HIV/AIDS实施高效抗反转录病毒治疗(highly active antiretroviral therapy,HAART)的效果及其影响因素,为进一步有效治疗提供依据。方法采用回顾性调查方法,对我院接受规范HAART的182例HIV/AIDS患者的CD4+T淋巴细胞绝对计数和病毒载量进行分析。结果 182例中,157例治疗后CD4+T淋巴细胞计数增加,且在治疗后前6个月内增加速度最快,随着治疗时间延长则较为缓慢。病毒载量在治疗后的6个月内下降最快,大部分降至血浆检测不到的水平,6个月后比较稳定;但随着治疗时间延长,部分患者的病毒载量出现反弹,可能与患者服药依从性差和病毒耐药性的出现有关。治疗前后CD4+T淋巴细胞计数差异有统计学意义(P0.05)。多因素logistic回归分析表明年龄和病毒载量是影响HAART疗效的危险因素,基线CD4+T淋巴细胞计数和按时服药是保护因素。结论 HAART能明显增加CD4+T淋巴细胞计数,降低病毒载量,有效控制机会感染,治疗效果显著。影响HAART疗效的因素是多方面的,在治疗过程中应综合考虑各种可能影响因素以提高疗效,对符合条件的患者应尽早进行规范的抗病毒治疗。  相似文献

目的 探讨获得性免疫缺陷综合征(AIDS)患者胃黏膜组织中人类免疫缺陷病毒(HIV)感染的分布、数量及高效抗逆转录病毒治疗(HAART)前、后的变化.方法 选取伴有消化道症状的AIDS患者35例,对照组为HIV-1阴性者10例,均进行胃镜检查.PCR法制备地高辛标记HIV-1 LTR、gag基因的双链cDNA探针,对胃黏膜组织冰冻切片采取核酸原位杂交法观察HIV感染情况,并与外周血单个核细胞(PBMC)涂片对比.结果 ①AIDS患者胃镜下多表现为不同程度的慢性浅表性胃炎(CSG)和慢性萎缩性胃炎(CAG),AIDS患者临床消化道症状、胃镜下表现及病理结果未见明显特征性.②AIDS患者胃黏膜组织内HIV阳性显色多见于单个核细胞(MMC)中,胃黏膜上皮、腺上皮细胞和问质细胞中亦有阳性杂交信号.③胃窦黏膜组织与胃体黏膜组织HIV阳性率差异无统计学意义(P>0.05).④AIDS患者胃黏膜HIV阳性率为(1.97±3.25)%,AIDS未治疗组与HAART治疗组各组间胃窦和胃体部HIV阳性率差异均无统计学意义(P值均>0.05).PBMC中HIV总阳性细胞率为(12.38±9.17)%;AIDS未治疗组PBMC中HIV阳性细胞率为(19.37±9.23)%,AIDS治疗1~4年组为(4.25±3.47)%,两组间差异有统计学意义(P<0.05).结论 胃黏膜是HIV感染的靶部位之一,HAART治疗对AIDS患者胃黏膜感染收效甚微.  相似文献
