目的:评价双期螺旋CT经动脉门脉造影(CTAP)能否降低非肿瘤性灌注缺损(NPD)的发生率。方法:36例经活检证实的原发性肝癌患者接受双期螺旋CTAP检查。在注入造影剂30s后开始第1期扫描,第1期扫描结束后10-15s开始第2期扫描。两期扫描所见分别与碘油CT、肝动脉造影CT及重复性CT检查结果相联系。结果:两期扫描的NPD发生率分别为36.1%和8.3%(P<0.05)。结论:双期螺旋CTAP能明显降低NPD的发生率。  相似文献

A new technique of CT during arterial portography (CTAP) performed with a left brachial artery approach is presented. We used 4-F, 50-cm J-shaped sheath introducer with a straight dilator and angled 4-F catheter with side holes. Catheter insertion to the superior mesenteric artery (SMA) can be performed without any trouble by the help of a J-shaped long sheath introducer positioned in the aortic arch. In our experience, 10 min compression of the puncture site and 2 h observation after catheter removal brought no local complication. This method can be performed safely on outpatient basis. Both computed tomography arterial portography (CTAP) and CT hepatic arteriography (CTHA) are CT techniques with angiographic assistance. The detection sensitivity of these techniques is high because marked lesion contrast can be obtained using direct delivery of contrast materials to the liver parenchyma or the tumors. The use of CTAP and CTHA may improve therapeutic results after transarterial embolization therapy for hepatocellular carcinomas because of their high diagnostic accuracy. Findings on CTAP or CTHA can sometimes help characterize the hepatic focal lesions. Thus, CTAP and CTHA are frequently performed as pretreatment examinations, although they are invasive compared to intravenous (IV) contrast-enhanced CT or magnetic resonance imaging. However, there are some potential pitfalls, such as nontumorous perfusion abnormalities. CTAP and CTHA are less effective for evaluation of patients with cirrhosis and portal hypertension. This article presents a current overview of CTAP and CTHA technique for diagnosis of hepatic neoplasms.  相似文献

目的:研究肝动脉造影CT和经动脉门脉造影CT在肝癌介入治疗中的应用价值和意义。方法:10例原发性肝癌介入治疗前行肝动脉造影CT(CTHA)和经动脉门脉造影CT(CTAP)检查;术中行数字减影肝动脉造影(DSA)。结果:CTHA、CTAP联合检查与常规CT、DSA比较,分别多发现新癌灶11个(26/37)和12个(25/37)。准确判断非复发癌灶2个和1个坏死灶。结论:CTHA、CTAP是肝癌介入治疗前准确判断肿瘤数量和存活度最敏感和准确的方法,对于介入治疗方案的实施、疗效评价有很重要的作用。  相似文献

Prostaglandin E1 (PGE1) is a vasodilator that increases portal venous flow. Hepatic CT during arterial portography (CTAP) was performed in 42 patients with and without PGE1 to compare peak hepatic enhancement and nontumorous abnormalities. Although no significant differences in peak hepatic enhancement were observed (71 +/- 12 HU for CTAP with PGE1; 74 +/- 34 HU for CTAP without PGE1), the number of nontumorous abnormalities for CTAP with PGE1 (n = 11) was significantly lower than that for CTAP without PGE1 (n = 24) (p < 0.01, Wilcoxon signed rank test). CTAP combined with PGE1 therefore represents a useful method to study lesions of the liver, as the number of nontumorous abnormalities observed is significantly reduced and liver parenchyma can be scanned more evenly.  相似文献

PURPOSE: To evaluate the portal and arterial blood supplies to dysplastic nodules in the cirrhotic liver with computed tomography (CT) during arterial portography (CTAP) and CT hepatic arteriography (CTHA). MATERIALS AND METHODS: Nineteen histopathologically proved low-grade dysplastic nodules and 13 high-grade dysplastic nodules in 17 patients with liver cirrhosis were evaluated with CTAP and CTHA for the presence of portal and arterial blood supplies to the nodules. The nodules ranged from 0.4 to 4.5 cm in diameter (mean, 1.6 cm). RESULTS: The portal supply was present in 14 of the 19 (74%) low-grade dysplastic nodules and in seven of the 13 (54%) high-grade dysplastic nodules. The hepatic arterial supply was increased in four of the 19 (21%) low-grade dysplastic nodules, present in nine (47%), and absent in six (32%). The arterial supply was increased in four of the 13 (31%) high-grade dysplastic nodules, present in four (31%), and absent in five (38%). CONCLUSION: The portal and arterial supplies to the low- and high-grade dysplastic nodules were variable and inconsistent. Therefore, it is difficult to detect and characterize the dysplastic nodules on the radiologic images on the basis of the blood supply.  相似文献

We examined a patient with a hepatocellular carcinoma and multiple adenomatous hyperplasias in the cirrhotic liver. Helical CT hepatic arteriography (CTA) showed the adenomatous hyperplasias as areas of discrete hypoattenuation, and the combined CT during arterial portography (CTAP) showed corresponding areas of subtle hyperattenuation. Such imaging findings at combined CTA and CTAP were seen in only one patient in a series of more than 80 patients in whom we performed angiographically-assisted CT. We demonstrate these unusual imaging findings of adenomatous hyperplasia in the report.  相似文献

AIMS: The purpose of our study was to evaluate the observer performance with combined helical CT during arterial portography (CTAP) and biphasic CT hepatic arteriography (CTHA) in the pre-operative detection of malignant hepatic tumours. METHODS: Computed tomography images obtained in 41 patients with suspected hepatic tumours were retrospectively reviewed. In a blind fashion, three off-site, independent radiologists reviewed CTAP and early-phase CTHA combined for the first review, then late-phase CTHA was added for the second review. Statistical analysis was conducted on lesion-by-lesion and segment-by-segment bases; a total of 328 liver segments including 65 segments with 74 malignant hepatic tumours ranging in size from 5 to 100 mm (mean, 21.4 mm) were analysed. RESULTS: Sensitivity for detection of liver segments harbouring tumours of CTAP and biphasic CTHA combined (82%) was identical to that of CTAP and early-phase CTHA combined (82%). Specificity of CTAP and biphasic CTHA combined (93%) was greater than that of CTAP and early-phase CTHA combined (90%, P < 0.005). The mean confidence level for the 74 tumours significantly increased by adding late-phase CTHA (P < 0.0005). The mean confidence level for 100-142 benign perfusion abnormalities detected with CTAP and early-phase CTHA combined significantly decreased by adding late-phase CTHA (P < 0.0005). CONCLUSION: By combining late-phase CTHA with CTAP and early-phase CTHA information, the specificity for the detection of malignant hepatic tumours rises significantly, allowing more accurate preoperative tumour detection.  相似文献

The standard method for the evaluation of hepatic perfusion during hepatic artery infusion (HAI) chemotherapy is planar hepatic artery perfusion scintigraphy (HAPS). Planar HAPS was performed with 2 mCi of [99mTc] macroaggregated albumin infused at 1 ml/min and compared with single photon emission CT (SPECT) HAPS and with a new study, CT performed during the slow injection of contrast material through the HAI catheter (HAI-CT). Thirteen patients underwent 16 HAI-CT studies, 14 planar HAPS studies, and 9 SPECT HAPS studies. In 13 of 14 studies (93%) HAI-CT and planar HAPS were in complete agreement as to the perfusion pattern of intrahepatic metastases and normal liver. In nine studies where all modalities were performed, the findings identified by HAI-CT and planar HAPS agreed in all cases, whereas the results of two SPECT scans disagreed with the other studies. With respect to perfusion of individual metastases, 14 of 14 HAI-CT studies, 12 of 13 planar HAPS studies, and 9 of 9 SPECT HAPS studies correctly demonstrated the perfusion status of individual lesions as indicated by the pattern of changes in tumor size determined on CT obtained before and after the perfusion studies. Hepatic artery infusion CT was superior for delineation of individual metastases, particularly small lesions, and for the evaluation of nonperfused portions of the liver. Planar HAPS detected extrahepatic perfusion in four patients, and this was not detected by HAI-CT. We conclude that HAI-CT and scintigraphy are complementary techniques. Hepatic artery infusion CT has advantages for the evaluation of intrahepatic perfusion, and planar HAPS is superior to HAI-CT for the detection of extrahepatic perfusion.  相似文献

To evaluate the accuracy of a fast three-dimensional (3D) reconstruction technique in determining the segmental location of hepatic metastases, 14 patients (40 metastases) were prospectively investigated before surgery with dynamic two-dimensional (2D) computed tomography during sequential arterial portography (CTAP). All patients underwent subsequent hepatic tumor resection within 4 days. After computer-generated mapping of hepatic venous structures with high attenuation and metastases with low attenuation, 3D reconstruction was performed. Thirty-six of the 40 lesions (90%) were detected with 2D and 3D CTAP. The accuracy in determining the segmental location of hepatic metastases was 78% (28 of the 36 metastases) for 2D CTAP and 94% (34 of the 36 metastases) for 3D CTAP. The difference in localization rates between 2D CTAP and 3D CTAP was statistically significant. The 3D CTAP technique provides vital data unattainable with other imaging modalities that improve the preoperative assessment of the resectability of hepatic metastases and allows planning a safer surgical approach.  相似文献

PURPOSE: To determine the appearance of regenerative nodules in patients with liver cirrhosis at computed tomography (CT) during arterial portography (CTAP) and CT hepatic arteriography (CTHA). MATERIALS AND METHODS: CTAP and CTHA of the liver were performed in 28 consecutive patients with hepatocellular carcinoma (HCC) who were scheduled to undergo partial resection of the liver. Helical CTAP was performed after contrast material injection into the superior mesenteric artery followed by helical CTHA after contrast material injection into the hepatic artery. CT scans were analyzed for the presence of identifiable nodules and their size; results were correlated with gross and microscopic findings. RESULTS: Resected livers showed cirrhosis in 20 patients, chronic hepatitis in four, and normal liver in four. Among the 20 patients with cirrhosis, regenerative nodules were demonstrated as enhancing 3-10 mm nodules surrounded by lower attenuation fibrous septa 0.8-1.5 mm thick at CTAP in seven patients and nonenhancing nodules of the same size surrounded by enhancing fibrous septa at CTHA in 15 patients. The degree of fibrosis determined the conspicuity of nodules. CONCLUSION: Regenerative nodules in cirrhotic liver are visualized as enhancing nodules surrounded by lower attenuation thin septa at CTAP and nonenhancing nodules surrounded by enhancing fibrous septa at CTHA. CTHA is more sensitive than CTAP in depicting regenerative nodules (P < .005).  相似文献

Fourteen patients with hepatocellular carcinoma (HCC) were examined with computed tomography (CT) during arterial portography (CTAP) and with CT after intraarterial injection of iodized oil. The detectability of main lesions and associated daughter nodules or intrahepatic metastases was assessed. Hepatic resection was subsequently performed in all 14 patients. The results of the imaging studies were compared with the surgical and pathologic findings by means of a lesion-by-lesion analysis. A total of 34 masses were identified in the resected specimens: 18 main tumor masses and 16 intrahepatic metastases. For CTAP, the detection rate of main tumors was 94%; for iodized-oil CT, 82%. However, the daughter-nodule detection rates for both techniques were poor-only 38% detected for CTAP and 50% for iodized-oil CT. Although these two techniques remain important preoperative imaging methods in patients with HCC, the results of this study suggest that small daughter nodules (less than 5 mm in diameter) may go undetected with both techniques.  相似文献

目的通过螺旋CT动脉门静脉造影(SCTAP)、螺旋CT动脉造影(SCTA)与肝动脉造影比较,显示小肝癌的血流动力学特性及影像特征。方法22例原发性肝细胞癌(HCC)或HCC术后肝内转移病人(男18人、女4人,年龄34~65岁,平均44.6岁),共79个病灶(0.1~3cm,平均直径1.8cm)。5例SCTAP与17例SCTA,结果分别与肝动脉造影术比较。结果5例SCTAP显示28个小病灶中,26个低密度,2个等密度;17例SCTA发现51个病灶,42个表现为高密度,9个表现为等密度;两组血管造影分别显示17、32个病灶,其中高密度41个,等密度8个。与血管造影相比,SCTAP显示小于2cm的病灶及所有小于3cm的病灶数差异具有显著性(P<0.05)或高度显著性(P<0.01);SCTA对小于3cm的病灶数较血管造影有显著性差异(P<0.05。)结论SCTAP及SCTA可准确反映小肝癌的血流动力学特性,在显示小肝癌方面优于肝动脉造影,可以为临床提供更丰富的影像学资料。  相似文献

AIM: To evaluate the diagnostic accuracy of the combination of computed tomography (CT) during arterial portography (CTAP) and double-phase CT hepatic arteriography (CTHA) with multidetector-row CT (MDCT) for the evaluation of hepatocellular carcinomas (HCCs) in patients with cirrhosis. MATERIALS AND METHODS: The combination of CTAP and double-phase CTHA was performed on 46 patients with 54 nodular HCCs. Three readers reviewed the images obtained with CTAP alone, first-phase CTHA alone, double-phase CTHA, and the combination of CTAP and double-phase CTHA. The review of the images was conducted on a segment-by-segment basis, with 368 hepatic segments, including 50 segments with 54 HCCs, reviewed for detection of HCCs with the aid of a five-point confidence scale. Diagnostic accuracy was evaluated by comparing the receiver-operating characteristic (ROC) analysis results. RESULTS: The sensitivity for detecting HCCs was significantly higher with either double-phase CTHA or the combination of CTAP and double-phase CTHA than with first-phase CTHA alone (90 and 93 versus 85%, respectively, p<0.01). The specificity for detecting HCCs was significantly higher with the combination of CTAP and double-phase CTHA than with CTAP alone (97 and 94%, respectively, p<0.01). The positive predictive values for detecting HCCs were significantly higher with double-phase CTHA than with first-phase CTHA alone (86 and 82%, respectively, p<0.05). The area under the ROC curve (Az) values were significantly higher with the combination of CTAP and double-phase CTHA (0.983) than with first-phase CTHA alone (0.959; p<0.05). CONCLUSION: The combination of CTAP and double-phase CTHA with MDCT significantly enhances the detection of HCC.  相似文献

目的 探讨DSA、CT和经肠系膜上动脉门静脉灌注CT成像对肝转移瘤的血液供应显示状况.方法 回顾性分析100例原发病灶经手术和(或)病理证实的肝转移瘤患者资料,均进行了CT平扫、多期CT增强扫描、选择性腹腔动脉和超选择性肝固有动脉DSA检查,其中,56例还经肠系膜上动脉插管行肠系膜上动脉的门静脉灌注CT成像(P(1TAP)检查,计算转移瘤中心区域、肿瘤边缘、门静脉和正常肝实质的时间-密度曲线(TDC)灰度密度(K值),观察肝转移瘤血液供应来源.DSA图像用Photoshop软件进行定量分析,CT图像用去卷积灌注软件进行分析.结果 DSA表现:肝固有动脉造影TDC显示肿瘤中心K值峰值平均为(67±12)%,肿瘤边缘K值峰值平均为(76±15)%,正常肝实质K值峰值平均为(51±10)%.腹腔动脉造影TDC显示,肿瘤中心及肿瘤边缘K值表现为快速上升,然后为缓慢上升的平台,而正常肝实质则呈现持续缓慢上升的态势.PCTAP扫描表现:肿瘤在30 s的时间内,密度变化几乎呈直线,无增强表现.结论 肝动脉是肝转移瘤的主要血液供应来源,门静脉几乎不参与肝转移瘤血液供应.  相似文献
