Classification of scalpula fractures

Fractures of the scapula are often associated with concomitant injuries of adjacent anatomic structures (e.g., chest). Scapula fractures are basically subdivided into intra-articular and extra-articular fractures.

Treatment of extra-articular fractures

It could be shown within literature that extra-articular fractures usually heal uneventfully with nonoperative treatment. Exceptions are fractures of the scapular neck, which can displace secondarily because of the tension of the triceps muscle and should therefore be operated even in cases of moderate fracture displacement.

Treatment of intra-articular fractures

Intra-articular fractures (glenoid fractures) are frequently operated even in cases of small intra-articular step-offs. However, there are no exactly defined limits of nonoperative treatment within the current literature.

SSSC (superior shoulder suspensory complex)

A special theoretical model for definition of stability is the SSSC (superior shoulder suspensory complex), which describes a ring of three struts of bony (glenoid, coracoid, clavicula, spine, acromion) and ligament structures (coracoclavicular ligaments, acromioclavicular ligaments), whereby the displacement of two of the three struts compromises the stability of the ring and, thus, should be treated operatively.  相似文献

Background

There are only a few studies available about antero-inferior glenoid rim fractures, which are rare injuries in clinical practice. The purpose of this study was to evaluate the results and complications of non-operatively treated antero-inferior glenoid rim fractures. In this article preliminary results will be presented.

Patients and methods

Over a 14-year period (1997–2010) a total of 14 patients with an antero-inferior glenoid rim fracture and a fragment size of ³?5 mm treated non-operatively were retrospectively identified. Outcomes were the clinical results and complications, as well as prospective assessment of long-term functional status (age and gender-related Constant/Murley score and Rowe score) of the shoulder. Radiographic analysis included determination of the degree of glenohumeral arthritis.

Results

Included in this study were 7 females and 7 males with a mean age of 62 years (range 35–87 years). The average follow-up time was 5.7 years (range 1–14 years) including a mean age and gender-related Constant/Murley score of 94.1 % (range 70–100 %) and a mean Rowe score of 90.4 points (range 50–100 points). At the time of injury joint step-off and displacement averaged 4.5 mm (range 0–14 mm), 4 patients (4/14, 28.6 %) had glenohumeral arthritis, which was asymptomatic in 2 (2/14, 14.3 %) and 2 had symptomatic arthritis (2/14, 14,3 %). Both patients with asymptomatic arthritis showed recurrent instability during the early treatment after primary dislocation. Within the study population no further instability was identified.

Conclusion

Antero-inferior glenoid rim fractures are rare and evidence regarding results and functional outcome is limited. Non-operative treatment leads to good mid-term and long-term functional results.  相似文献

Background

Surgical treatment of scapular fractures with posterior approach is frequently associated with postoperative infraspinatus hypotrophy and weakness. The aim of this retrospective study is to compare infraspinatus strength and functional outcomes in patients treated with the classic Judet versus modified Judet approach for scapular fracture.

Patients and methods

20 cases with scapular neck and body fracture treated with posterior approach for lateral border plate fixation were reviewed. In 11 of 20 cases, we used the modified Judet approach (MJ group), and in 9 cases we used the classic Judet approach (CJ group). All fractures were classified according to the AO classification system. At follow-up examinations, patients had X-ray assessment with acromiohumeral distance (AHD) measurement, clinical evaluation, active range of motion (ROM) examination, Constant Shoulder Score, and Disability of the Arm, Shoulder and Hand (DASH) Score. Infraspinatus strength assessment was measured using a dynamometer during infraspinatus strength test (IST) and infraspinatus scapular retraction test (ISRT).

Results

Demographic data did not significantly differ between the CJ group and MJ group, except for mean follow-up, which was 4.15 years in the CJ group and 2.33 in the MJ group (p < 0.001). All X-ray examinations showed fracture healing. AHD was significantly decreased in the CJ group (p = 0.006). We did not find significant differences in active ROM between the MJ and CJ groups in the injured arm (p < 0.05). The Constant Score was 75.83 (±14.03) in the CJ group and 82.75 (±10.72) in the MJ group (p = 0.31); DASH Score was 10.16 in the CJ group and 6.25 in the MJ group (p = 0.49). IST showed mean strength of 8.38 kg (±1.75) in the MJ group and 4.61 kg (±1.98) in the CJ group (p = 0.002), ISRT test was 8.7 (±1.64) in the MJ group and 4.95 (±2.1) in the CJ group (p = 0.002). Infraspinatus hypotrophy was detected during inspection in six patients (five in the CJ group and one in the MJ group); it was related to infraspinatus strength weakness in IST and ISRT (p < 0.001).

Conclusions

Infraspinatus-sparing surgical approach for scapular fracture avoids infraspinatus hypotrophy and external-rotation strength weakness. We suggest use of the modified Judet approach for scapular fracture and to restrict the classic Judet approach to only when the surgeon believes that the fracture is not easily reducible with a narrower exposure.

Purpose

Bilateral scapular fracture is a very rare injury. Most of these fractures result from electrical shock or epileptic seizure. We treated six patients with such injuries, all of them caused by direct violence. The aim of this study was to report on the patients and to present an overview of the cases published so far.

Methods

Between January 2011 and August 2012, we treated six patients with bilateral scapular fractures (four men and two women, age range 20–78 years). Another 11 cases were found in the literature. All cases were analysed in terms of injury mechanism, fracture pattern and the manner of diagnosis.

Results

Our six patients increased the total number of recorded cases to 17 and the number of patients with traumatic bilateral scapular fractures from four to ten. In five of our cases, the injuries were classified as being the result of high-energy trauma. Computed tomography (CT) examination of the affected scapulae was performed in all six cases, in five in combination with 3D CT reconstruction; in one polytraumatised female patient, only axial CT scans were obtained. In all five high-energy trauma cases, bilateral fracture of the scapular body was recorded, of which one was classified as open. Four of the 11 cases found in the literature were caused by direct violence: in six patients, the fractures resulted from muscle spasms associated with epileptiform seizure or electrical shock, and one patient suffered a pathological fracture associated with amyloidosis. The most frequently recorded fracture in all 17 patients (34 fractures) was of the scapular body, i.e. 24 fractures, followed by 12 fractures of the glenoid fossa.

Conclusion

According to data in the literature, bilateral scapular fracture is a rare injury. One reason may be that the potential incidence is often neglected. With the increasing number of patients with polytrauma, the potential for scapular fracture should always be taken into account, together with the fact that this injury may be bilateral. Of vital importance in diagnosing these injuries is CT scanning, including 3D CT reconstructions.  相似文献

Objectives

To compare clinical outcomes between titanium elastic nails (TENS), open reduction internal fixation (ORIF) and non-operative treatment options at mean 30 months in Allman type 1 fractures of the clavicle.

Design

Retrospective case matched.

Setting

Tertiary Trauma Centre, UK.

Patients

Treatment method was based on surgeon choice although there was no difference between groups in terms of age or gender. Those treated non-operatively tended to have a less severe fracture pattern. There were 25 patients treated with TENS, 24 with ORIF and 41 treated conservatively. Main outcome measurements are qDASH questionnaire, patient satisfaction question and clinical complication rate.

Results

Patients managed with TENS had significantly better clinical outcomes than both plate fixation and non-operatively managed groups.

Conclusions

This study supports the use of TENS fixation of middle third clavicle fractures in significantly displaced Allman type 1 injuries.  相似文献

The authors operated on 22 patients with scapular body fractures, with a mean age of 35 years. The minimal follow-up was 12 months. All patients were treated from the Judet posterior approach. The study included 14 cases of an isolated body fracture, three of scapular body fracture combined with fracture of the scapular neck and five of glenoid fossa fracture. In all 14 cases where a 3D computed tomography (CT) reconstruction was performed prior to operation, intraoperative findings corresponded to this reconstruction. In eight cases without preoperative 3D CT reconstruction, the correct type of fracture was identified in only two cases. We also identified three basic types of fractures of the lateral border of the scapula. The anatomical relationship between the glenoid fossa and scapular body, congruency and stability of the shoulder joint was achieved in all cases. The average constant score was 94.  相似文献

Purpose

In the Netherlands, over 20,000 patients sustain a hip fracture yearly. A first hip fracture is a risk factor for a second, contralateral fracture. Data on the similarity of the treatment of bilateral femoral neck fractures is only scarcely available. The objectives of this study were to determine the cumulative incidence of non-simultaneous bilateral femoral neck fractures and to describe the patient characteristics and treatment characteristics of these patients.

Methods

A database of 1,250 consecutive patients with a femoral neck fracture was available. Patients with a previous contralateral femoral neck fractures were identified by reviewing radiographs and patient files. Patient characteristics, previous fractures, hip fracture type and details on treatment were collected from the patient files.

Results

One hundred nine patients (9 %, 95 % confidence interval 7–10 %) had sustained a non-simultaneous bilateral femoral neck fracture. The median age at the first fracture was 81 years; the median interval between the fractures was 25 months. Overall, 73 % was treated similarly for both fractures in terms of non-operative treatment, internal fixation or arthroplasty. In patients with identical Garden classification (30 %), treatment similarity was 88 %.

Conclusions

The cumulative incidence of non-simultaneous bilateral femoral neck fractures was 9 %. Most patients with identical fracture types were treated similarly. The relatively high risk of sustaining a second femoral neck fracture supports the importance of secondary prevention, especially in patients with a prior wrist or vertebral fracture.  相似文献

Background

The treatment of radial neck fractures in children varies according to the displacement, angulation, and skeletal maturity. There is a general agreement that displaced radial neck fractures with more than 30° angulations (Judet type III and IV fractures) should be surgically treated. There are several treatment possibilities for Judet type III and IV fractures including percutaneous pin reduction, elastic stable intramedullary nailing (ESIN), and open reduction with or without internal fixation. In this retrospective study we compared the clinical and radiographical outcomes, and complications following intramedullary versus percutaneous pinning in displaced radial neck fractures in children.

Materials and methods

Between 2000 and 2011, 20 patients were treated using closed reduction: in 12 cases we used percutaneous pinning, and in 8 cases we used ESIN. According to Judet classification the two groups were composed as follows: 10 (77 %) type III and 3 (23 %) type IV fractures in the percutaneous pinning group; 4 (57 %) type III, and 3 (43 %) type IV fractures in the ESIN group.

Results

After an average of 42 months, excellent results in Mayo elbow performance scores (MEPS) were obtained in 71 and 69 % of ESIN and percutaneous pinning groups respectively, with good results in the remaining cases apart from one fair case (8 %) in the percutaneous pinning group. After a radiological evaluation, all fractures healed in excellent or good alignment. When comparing the two groups, the subjects treated with the ESIN technique had higher range of motion (ROM) in flexion, extension and pronation. No patients developed complications, except three cases of asymptomatic enlargements of the radial head, reported only in the percutaneous pinning group.

Conclusion

In this research the clinical outcome, assessed with the MEPS, and the radiological alignment, were comparable between the subjects that were treated with percutaneous pinning and those with ESIN techniques; whereas the ESIN technique demonstrated higher ROM in flexion, extension and pronation. The ESIN technique seems to be the ideal approach both for the higher ROM values and for the absence of complications.  相似文献

Purpose

Three-dimensional computerised tomography (3DCT) can provide comprehensive patho-anatomy of complex bone on a single image. Though important, the key articular quadrilateral [Q] surface has not been a part of the systems developed for classifying acetabulum fractures. The purpose of the study was to simplify the complexity of classification by the direct sign of the broken Q surface which lies opposite the entire floor of the acetabulum.

Methods

The study reviewed 84 acetabular fractures using 3DCT images of the interior lateral view (IL) taken between June 2002 to December 2009. Fractures were traditionally classified using the anatomical disruption, plane of the fracture line breaking through or not through the bone column described by Judet and Letournel.

Results

The 3D images clearly show the primary site of impaction acting on the acetabulum and the whole course of fracture. The image could not illustrate disruption of the lips of acetabulum and congruity of hip joints in 20 cases of wall (W) fracture. There were 30 transverse (T) fractures classified when the acetabulum was divided horizontally from front to back into upper and lower parts and 34 cases of column (C) fracture when the main vertical lines run and collide along the anterior and posterior column.

Conclusions

This study showed that the well-known complex fractures can be satisfactorily classified with the broad flat inner plane of the Q surface.  相似文献

Anatomical neck fractures of the scapula are rare. The authors have found in the literature only four radiologically documented fractures of the anatomical neck of the scapula. Two of them were published by Hardegger et al., the third case was published by Arts and Louette. The last case, in fact only a radiograph and a rather poor 3D CT reconstruction of a fracture of the anatomical neck of scapula, was published by Jeong and Zuckerman. Together with author’s two patients, the group of radiologically verified anatomical neck fractures of the scapula comprises six cases in total (four men, one woman, one gender unspecified). Analysis of the radiographs showed that in all these cases, the fracture line separated only the glenoid fossa from the scapular body, with a short spike of the lateral border of the scapula. The proximal part of the vertical fracture line ran into the coracoglenoid notch, between the upper border of the glenoid and the base of the coracoid process. The distal part of the fracture line crossed the lateral border of the scapular body 2–4 cm distal to the inferior pole of the glenoid fossa. The gleniod fragment was always formed by the glenoid fossa and a short spike of the lateral border of the scapular body. In five cases, the glenoid fragment, together with the humeral head, was displaced distally and the humeral head came to lie below the level of the coracoid process. At the same time, the glenoid fragment rotated into a valgus position. Only in one case, did the radiographs fail to show valgus displacement and the fracture was angulated in the transverse plane. In all six cases, the subacromial space between the acromion and the humeral head was widened. All fractures were operated on via a Judet posterior approach. In five cases, the outcome of the operation was assessed at 3, 5, 12, 21 and 120 months after surgery, three-being rated as excellent or very good, one as good and one as poor.  相似文献

Introduction

Posttraumatic and postoperative osteomyelitis (PPO) with bacteria colonisation during trauma and associated surgery is an increasing clinical problem. This study investigated the treatment of PPO by surgical revision including irrigation, debridement, and temporary hardware maintenance. In addition, a drainage was inserted as persisting fistula to control osteomyelitis until fracture healing was achieved. Trauma- and osteomyelitis-related factors that influenced the study outcome were determined.

Patients and methods

67 consecutive patients with PPO were included. At onset of PPO, patients had incomplete fracture healing. Patients were subdivided by time of PPO occurrence (acute, subacute or chronic), initial soft tissue trauma, anatomical location, and initial fracture type (AO classification). The study outcome measures included radiographic and clinical follow-up.

Results

59 patients could be followed for an average of 23 months after revision surgery. A bone healing was achieved by 89 % of patients after 14.7 ± 13.4 weeks. Fractures of the lower extremity, open fractures and comminuted C-type fractures took significantly longer to achieve bone healing (p < 0.05 each). Time of PPO occurrence did not influence bone healing. After fracture consolidation, no re-infection was found.

Conclusions

This study showed high rates of bone healing, indicating that this strategy with persisting fistula should be considered as alternative treatment option in patients with PPO.  相似文献

Background

Scapular body fractures generally occur as a result of high-energy, direct trauma to the shoulder sustained in automobile accidents. While such mechanisms have been well described, little is known about scapular body fractures sustained during sporting activities.

Questions/Purposes

We sought to systematically review the literature on scapular body fracture sustained during sporting activity, recording rates and mechanisms of injury, management strategies, and return-to-sport times.

Methods

Following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, we performed a systematic review of studies conducted between 1985 and 2017. Inclusion criteria were studies examining scapular body fractures sustained during sporting activity, fracture management, and patient outcomes. Exclusion criteria were studies on non-sporting-related fractures and those not reporting fracture management or patient outcomes.

Results

Nine studies encompassing ten cases of scapular body fracture sustained during sporting activity were identified, with acute trauma responsible for 70% of fractures. No patient sustained any associated injuries. Fractures were treated conservatively in 90% of cases, with no reported complications. Mean overall time to return to sport was 2.5 months, while no significant difference in return to sport was appreciated in athletes with acute versus fatigue fractures.

Conclusion

Scapular body fractures in athletes occur primarily from muscle contraction against a resisted force in the upper extremity during contact sports. Unlike non-sporting fractures, these fractures usually involve low-energy mechanisms without associated injury and conservative treatment is usually successful.

Background

Fractures of the distal third of the clavicle represent 10–30?% of all clavicle fractures . Frequently, these fractures result in instability due to a combination of bony and ligamentous injury. Thus, assessment of the stability is essential for adequate treatment of these fractures.

Aim

This article presents a review of the different classification systems for distal clavicle fractures with respect to anatomical and functional factors to allow for comprehensive assessment of stability. Furthermore, the different treatment options for each fracture type are analyzed.

Results and discussion

Fractures to the distal third of the clavicle without instability can be treated conservatively with satisfactory outcome. In contrast, instability may result in symptomatic non-union under conservative treatment; therefore, distal clavicle fractures with instability should be treated operatively with respect to the functional demands of the patient. Operative treatment with locked plating in combination with coracoclavicular fixation results in excellent functional results. Arthroscopically assisted fracture fixation may be beneficial in terms of a minimally invasive approach as well as assessment and treatment of associated glenohumeral lesions.

Purpose

The combination of ipsilateral femoral neck and shaft fractures remains a treatment challenge in orthopedic surgery because both fracture types constitute separate entities and require specific treatment concepts.

Material and methods

In a case control study, incidence, treatment strategies, and outcomes of this injury were analyzed. All patients with femoral fractures treated between 1 January 2001 and 31 July 2007 at a level I trauma center were included in the study.

Results

Twenty-one out of 1,935 patients (1.1%) sustained 22 combined fractures of the femoral neck and shaft. Also considering the combination of femoral shaft fractures with fractures of the acetabulum and the distal femur (knee), the proportion of chain injuries of the femur was 3.1%. The rate of multiply injured patients in the group of patients with ipsilateral femoral neck and shaft fractures was 64%. The majority of the patients could be treated with a single implant for both fracture components. The leading fracture component was the femoral neck fracture in eight cases. All fractures consolidated after 4.7 months on average; one pseudarthrosis of the femoral neck was observed. All fractures were discovered in the course of primary diagnostic measures; in 73% of the patients, a computed tomography (CT) body scan was done. Fifty-nine percent of the patients with ipsilateral femoral neck and shaft fractures received primary definitive operative care. Complications included two torsional failures that needed correction and one case of postoperative infection that was easily treated.

Conclusion

Treatment of ipsilateral femoral neck and shaft fractures is still demanding, but diagnosis has improved with regular use of CT body scans in the management of multiply injured patients. Furthermore, possibilities for operative treatment have been advanced by the introduction of the long proximal femoral nail and the antegrade femoral nail, two implants supporting stabilization of these fracture entities.  相似文献

目的探讨肩胛骨不稳定骨折的手术治疗。方法随访分析1999年1月至2004年1月收治的经手术治疗的16例不稳定肩胛骨骨折患.根据Hardegger分类方法,肩胛盂窝骨折4例、解剖颈骨折6例、外科颈骨折4例、粉碎性骨折2例。结果16例患有12例获得完整随访资料.随访时间3个月~4年,平均21月,采用Hardegger肩关节功能评定标准,优4例,良6例,可2例。优良率83.3%.平均骨折临床愈合时间8周。结论a)不稳定肩胛骨骨折畸形愈合严重影响肩关节功能,需行手术切开复位内固定治疗。b)对肩胛骨骨折治疗前需要进行全面的X线摄片检查和CT检查,以全面了解骨折情况,判断骨折的稳定性及有无手术指征。c)对有手术指征的不稳定肩胛骨骨折应根据不同骨折的部位和形态,选择相应的手术人路和内固定物进行手术治疗。  相似文献

Purpose

For femoral neck fractures, recent scientific evidence supports cemented hemiarthroplasty (HA) over uncemented HA and suggests that total hip arthroplasty (THA) should be performed more frequently. We report the current surgical trends in treating femoral neck fractures in Finland.

Methods

The study was conducted using the Finnish National Hospital Discharge Register and included all Finns at least 50 years of age who underwent surgery for femoral neck fractures from 1998 through 2011. Age- and sex-specific incidence rates and annual proportion of each treatment method were calculated.

Results

During 1998–2011, a total of 49,514 operations for femoral neck fracture were performed in Finland. The proportion of uncemented HA increased from 8.1 % in 2005 to 22.2 % in 2011. During the same time, the proportion of cemented HA decreased from 63.9 to 52.5 %, internal fixation decreased from 23.2 to 16.1 % and THA increased from 4.9 to 9.2 %.

Conclusions

Between 2005 and 2011, the proportion of uncemented HA for femoral neck fractures increased markedly in Finland, while cemented HA and internal fixation declined. During this time, the use of THA nearly doubled. The current evidence-based guidelines for treatment of femoral neck fractures were mainly followed, but the increase in uncemented HA procedures contradicts recent scientific evidence.  相似文献

Background

Femoral neck fractures are a major public health problem. Multiple-screw fixation is the most commonly used surgical technique for the treatment of stable femoral neck fractures.

Questions/purposes

We determined (1) the proportion of hips that had conversion surgery to THA, and (2) the proportion of hips that underwent repeat fracture surgery after percutaneous screw fixation of stable (Garden Stages I and II) femoral neck fractures in patients older than 65 years and the causes of these reoperations.

Methods

We performed a retrospective study of all patients older than 65 years with stable femoral neck fractures secondary to low-energy trauma treated surgically at our institution between 2005 and 2008. We identified 121 fractures in 120 patients older than 65 years as stable (Garden Stage I or II); all were treated with percutaneous, cannulated screw fixation in an inverted triangle without performing a capsulotomy or aspiration of the fracture hematoma at the time of surgery. The average age of the patients at the time of fracture was 80 years (range, 65–100 years). Radiographs, operative reports, and medical records were reviewed. Fracture union, nonunion, osteonecrosis, intraarticular hardware, loss of fixation, and conversion to arthroplasty were noted. Followup averaged 11 months (range, 0–5 years) because all patients were included, including those who died. The mortality rate was 40% for all patients at the time of review.

Results

Twelve patients (10%) underwent conversion surgery to THA at a mean of 9 months after the index fracture repair (range, 2–24 months); the indications for conversion to THA included osteonecrosis, nonunion, and loss of fixation. Two others had periimplant subtrochanteric femur fractures treated by surgical repair with cephalomedullary nails and two patients had removal of hardware.

Conclusions

Revision surgery after osteosynthesis for stable femoral neck fractures was more frequent in this series than previously has been reported. The reasons for this higher frequency of reoperation may be related to poor bone quality, patient age, and some technical factors, which leads us to believe other treatment options such as nonoperative management or hemiarthroplasty may be viable options for some of these patients.

Background

Radial head arthroplasty is considered to be the treatment of choice in non-reconstructable radial head fractures in the acute fracture situation. Despite the promising short-term results in the current literature, replacement of the radial head remains controversially discussed as long-term results are still missing. In our study, we report our 7.8-year results after treatment with the bipolar radial head prosthesis of Judet.

Materials and methods

Between 1997 and 2004, 34 patients were treated with Judet??s bipolar radial head prosthesis in our department. After a mean of 94 months (range 15?C139), 28 of these 34 patients could be re-examined. While 20 patients were treated with radial head arthroplasty primarily in a fracture situation, 7 patients were treated secondarily after failure of fixation. One patient was treated for a tumor of the proximal radius.

Results

According to the Mayo Elbow Performance Score, 16 patients achieved an excellent, 10 patients a good, 1 patient a fair, and 1 patient a poor result. The mean Disabilities of the Arm, Shoulder and Hand Questionnaire (DASH) score was 12.0 (range 0?C50.1). There was no distinct difference between primary and secondary implantation in terms of the clinical outcome. The elbow flexion averaged 125° (range 100?C150°); the mean extension deficit was 20° (range 0?C60°). The mean pronation was 66° (range 0?C90°) with a mean supination of 67° (range 0?C90°). The most common complications were osteoarthritis of the ulnohumeral joint (n=18) and degenerative changes of the humeral capitellum with erosions (n=12). Finally, 3 patients suffered a dislocation after the surgical intervention, 1 patient had an infection, and 1 patient developed a radioulnar synostosis.

Conclusion

目的探讨改良Judet手术入路治疗复杂肩胛骨骨折的适应证及疗效。方法自1997年1月至2005年10月经改良Judet手术入路治疗肩胛骨骨折21例,其中男性15例,女性6例,平均年龄34岁(18~62岁)。根据Hardegger的分型方法,肩胛体部骨折11处、肩胛颈骨折10处、盂缘骨折8处、盂窝骨折7处、肩胛冈骨折9处、肩峰骨折6处。其中多部位复杂骨折15例,伴有腋神经损伤2例,肩胛上神经损伤4例。通过改良Judet手术入路,对不同类型的肩胛骨骨折使用重建钢板或拉力螺钉等固定。结果18例患者获得随访,随访时间6个月至4年,平均21个月。根据Rowe疗效评价标准,优12例,良3例,可2例,差1例,优良率为83%。结论改良Judet入路操作简单,暴露充分,大多数类型的肩胛骨骨折可通过改良Judet入路完成,尤其适用于肩胛体部骨折,肩胛颈骨折,肩胛冈骨折以及不存在喙突、前部肩胛盂骨折的多部位复杂骨折,经改良Judet入路行骨折复位内固定是肩胛骨骨折手术治疗的一种安全有效方法。  相似文献
