Carcinoid tumour of the larynx: A case report and clinico-pathological review

Carcinoid tumour of the larynx is a rare condition but one that is being increasingly recognised. We report the case of a 66 year old lady with an advanced malignant laryngeal carcinoid, metastasing to the regional lymph nodes, thyroid and submandibular glands. Surgery is the treatment of choice in this condition - radiotherapy and chemotherapy being ineffective. Prognosis is poor, and most patients eventually die of distant metastases. The clinical, biochemical and histological findings are reviewed.     

多原发肿瘤含肾神经鞘瘤1例报道并文献复习

多原发肿瘤并不常见,其中包含肾神经鞘瘤者更为罕见。本研究报道1例合并肾神经鞘瘤的多原发肿瘤的诊治。患者为女性,曾患左卵巢皮样囊肿、右后纵隔神经鞘瘤、左前臂神经鞘瘤、甲状腺乳头状癌,并行手术治疗。1个月前体检发现左肾肿物伴左肾上腺肿物,于北京大学第三医院行后腹腔镜左肾上腺结节切除术、左肾部分切除术。手术顺利完成,无中转开放,术中动脉阻断时间18 min,出血20 mL。病理检查示左侧肾富于细胞型神经鞘瘤伴局灶囊性变,肿瘤直径约3 cm。免疫组织化学S-100（+）,Ki-67（15%+）。短期随访无复发。本研究患者神经鞘瘤为异时性,分别予手术切除,其肾神经鞘瘤为良性,行腹腔镜肾部分切除术不但创伤小、恢复快,更重要的是可以最大限度的保留肾组织,避免术前病理的不确定性而行根治性肾切除术。     

Multiple endocrine neoplasia type 2B. A case report and review of the syndrome

This paper presents a case of the very rare multiple endocrine neoplasia Type 2B syndrome. It highlights that because of the presence of superficial neuromas in this condition, there is the possibility for early diagnosis. Recent knowledge of the molecular genetics of this syndrome and the ability to screen family members is also stressed since early thyroidectomy is now recommended to prevent the development of thyroid carcinoma which is the main determinant of prognosis.     

Multiple carcinoids in the stomach with hypergastrinemia and type A gastritis: a case report

We have experienced a case of the stomach with hypergastrinemia and type A gastritis with multiple carcinoids in a 37-year-old woman. An upper gastrointestinal series revealed multiple minute polyps in the upper body of the stomach. All polyps were diagnosed as carcinoid using endoscopic biopsies. She had neither symptom or signs of typical carcinoid disease. The serum gastrin level was as high as 725 pg/ml. Total gastrectomy was performed, and the diagnosis of multiple gastric carcinoids (sm, no) with type A gastritis was histologically confirmed. After the operation, the serum gastrin level returned to normal, and the patient has been doing well and is disease-free to date at 7 years after the operation. This case suggested that multiple gastric carcinoid lesions may be precipitated by chronic atrophic gastritis accompanying hypergastrinemia. In the treatment of multiple gastric carcinoids with type A gastritis, total gastrectomy with lymph node dissection should be standard operative procedure, in order to resect the fundic gland area completely which could be the origin of carcinoids and endocrine cell micronest.     

甲外多发性球血管瘤1例报道并文献复习

目的 探讨球血管瘤的临床病理特点及诊断和鉴别诊断,以减少误诊.方法 用回顾性方法分析1例发生于甲外部位的多发性球血管瘤并进行文献复习.结果 多发性球血管瘤很少见,其临床症状与经典型血管球瘤相似,以疼痛为主.病理组织学上表现为其血管成分为较大的静脉,似海绵状血管瘤,瘤细胞沿血管内壁成层状排列或成簇聚集.治疗方法为手术切除.结论 多发性球血管瘤是血管球瘤的一种少见类型,病理学上应与海绵状血管瘤和动静脉血管瘤等鉴别.     

Multiple myeloma with pulmonary embolism: a case report

We report a rare case of a patient who died suddenly, in whom bilateral pulmonary artery thrombosis with multiple myeloma was found at autopsy. An estimate of the incidence of pulmonary embolism in myeloma patients based on postmortem examinations is about 3.2%. Hypercoagulability and decreased fibrinolytic capacity due to multiple myeloma were the probable causes of multiple thromboses.     

胸壁多发性神经纤维瘤1例并文献复习

目的探讨多发性神经纤维瘤的遗传特性、临床特点及诊断治疗的进展。方法报道1例胸壁多发性神经纤维瘤的病例资料、家系情况及病理结果，并复习文献。结果单凭影像学对胸壁多发性神经纤维瘤定性诊断较困难，最后确诊需病理组织学证实；目前尚无有效的疗法能够阻止或逆转神经纤维瘤的病程。结论多发性神经纤维瘤是一种常染色体显性遗传病，应对患者进行密切观察，定期检查，尽可能做到早发现、早治疗；应提高对胸壁多发性神经纤维瘤的认识，注意鉴别诊断，避免误诊。     

胃平滑肌肉瘤——附胃多发性平滑肌肉瘤1例

本文复习了胃平滑肌肉瘤的有关文献,讨论了胃平滑肌肉瘤临床和肿瘤的病理特点以及诊断和治疗问题。胃多发性平滑肌肉瘤十分罕见,查阅国内、外文献仅1例报道。本文报道为第2例,并提出了多中心生长问题。
