Attempts at manual reduction under general anesthesia of a chronically dislocated temporomandibular joint were unsuccessful. Light elastic traction was applied to modified dentures, and after 7 weeks the condyles were partially repositioned. This article describes a new application in three-dimensional modelling. By adding the contours of important soft tissue structures to the bone contours, it is possible to produce a combined bone and soft tissue model. The advantages of this technique are shown in a patient suffering from a parapharyngeal tumour. The model not only enabled the precise localisation of the tumour in relation to the surrounding bone, but also identified important structures like the internal carotid artery in relation to the tumour. 相似文献

Oral infections caused by flies are rarely encountered in clinical practice, and consequently, there is a paucity of information in the medical and dental literature about these conditions. In the present article, we present a concise review on oral myiasis or fly‐blown disease. A variety of fly species can infest the oral tissues and produce an exotic clinical picture. Oral myiasis is mainly encountered in the tropics and subtropics, but can also be encountered in the western part of the world due to the increase of globalization, immigration, and global warming. Commonly‐reported symptoms of oral myiasis include pain, swelling, itchy sensation, and feeling of something moving in the mouth. The surgical debridement of infected tissue with the removal of maggots is the treatment of choice in most cases of oral myiasis. 相似文献

Dentinogenesis imperfecta is a hereditary disorder resulting in defective dentin in both the primary and secondary dentitions. The complications of dentinogenesis imperfecta are difficult to manage and provide a challenge to the dentist. This case report concerns treating an African American patient with dentinogenesis imperfecta who appeared for treatment with endodontic pathosis. It illustrates the need for appropriate and timely restorative treatment to prevent pulpal pathosis. Also demonstrated is the difficulty of endodontically treating dentinogenesis imperfecta teeth because of pulpal obliteration and abnormal dentin mineralization Early and correct diagnosis of dentinogenesis imperfecta is imperative to enable appropriate preventive interventions and optimal dental treatment. Although pulpal pathosis is rarely reported with dentinogenesis imperfecta, endodontic treatment is occasionally necessary and has a guarded prognosis if initiated after pulp canal obliteration has occurred. 相似文献

Myiasis is a condition caused by the invasion of tissues by larvae of Diptera flies. This phenomenon is well documented in the skin especially among animals and people in tropical and subtropical areas. The condition causes extensive tissue destruction as the larvae, at least for a certain period, feed on the host's dead or living tissue, liquid body substances, or ingested food. Mouth breathing during sleep, poor oral hygiene, alcoholism, senility, mental disability, cerebral palsy, and hemiplegia may facilitate the development of myiasis. We present a case report of oral myiasis in a 22‐year‐old male with cerebral palsy and severe mental retardation treated successfully by manual removal of the larvae by topical application of turpentine oil and oral systemic therapy with ivermectin. 相似文献

Myiasis was first described by Hope in 1840. Since then, many cases of myiasis affecting different human organs have been reported. Oral myiasis is very rare in healthy persons but occurs mainly in the tropics and associated with inadequate public and personal hygiene. In this case report, we present clinical findings gingival myiasis. The patient was 26-year-old farmer and his chief complaint was discomfort and swelling in his maxillary gingiva. The patient's medical and family histories were non-contributory and oral hygiene was not acceptable. The diagnosis was based on the characteristic clinical features and the visual presence of wriggling larvae about 1 cm in size. The larvae were identified as Calliphoridae. Treatment consisted of removal of the maggots from the gingival sulcus, followed by scaling and oral hygiene instruction. One week later, a periodontal flap operation was done. The patient was followed-up for 6 months and healing was uneventful. 相似文献

Myiasis is an infection caused by the deposition of fly larvae in tissues, and its involvement in the human oral cavity is uncommon. Herein, we have performed a data analysis of published cases of oral myiasis. A search was performed in PubMed, Ovid, Web of Science, Scopus, and LILACS. Geographic distribution, demographic data, associated factors, clinical features, fly types, treatment, and presence of sequelae were analyzed. A total of 122 articles reported the cases of 157 infected individuals. The most affected countries were India (41%) and Brazil (29.5%). Male predominance (67.5%) and a mean of 41.9 years of age were observed. The gingiva (29%) was the most affected site, followed by palate (25%) and lip (21%). There were different forms and combinations of treatments: manual removal of larvae and surgical debridement, application of asphyxiating substances, antibiotic therapy, and use of ivermectin. The condition predominantly affects individuals with neurological and/or locomotor disabilities, of low socioeconomic status, with poor oral hygiene and chemical dependence and individuals with previous injuries or with the absence of lip sealing. The establishment of a standard treatment protocol, enabling comparison in future studies and providing uniformity in treatment strategies offered by health services is strongly recommended. 相似文献

A workshop to discuss primary oral melanomas was convened at the annual Western Society of Teachers of Oral Pathology meeting in Bannf, Alberta, Canada. Fifty oral melanomas, identified from the files of the participants, were reviewed in order to better understand the clinical features, histologic spectrum, and natural history of these perplexing lesions. Results confirmed that oral melanomas occur in adults almost three times more frequently in men than women and have a decided predilection for the palate and gingiva. Some lesions exhibit a clinically detectable and prolonged in situ growth phase, whereas others seem to lack this property and exhibit only or predominantly invasive characteristics. Recurrences, metastases, and death from tumor were characteristic of the follow-up of a limited number of patients. Until definitive prospective data are collected that elucidate natural history, oral mucosal melanomas should be tracked separately from cutaneous lesions. All oral pigmented lesions that are not clinically diagnostic should be biopsied. Lesions with equivocal histopathologic features might be referred to as “atypical melanocytic proliferation” and should be excised. Recognition of lesions in an early in situ phase and aggressive treatment should have a favorable effect on prognosis. To enhance future or prospective study of these rare neoplasms, guidelines for reporting oral melanomas are suggested. 相似文献

Self-inflicting injuries are common among special children. Frequent in developing countries, cysticercosis is a parasitic infection that rarely involves the mouth. This study reports a case of oral cysticercosis in a 13-year-old female patient who had an asymptomatic nodule in the right labial mucosa. Filarial worms are nematodes that dwell in the subcutaneous tissues and lymphatics of human hosts. In India, filariasis is predominantly caused by a species of nematode called Wuchereria bancrofti. The disease is transmitted through the bite of blood sucking mosquitoes. The salient clinical feature of the disease is lymphangitis leading to elephantiasis of the legs, arms, scrotum and breast. Oral or perioral involvement is very rare. We report a case of filariasis which was diagnosed after biopsy of an innocuous cheek nodule. 相似文献

Leiomyoma of the oral cavity is infrequently found because of the scarcity of smooth muscle in the mouth. A review of the literature yielded only 124 cases reported from 1884 to 1987. A new case of primary leiomyoma of the lower buccal sulcus is presented. Although the recurrence rate has been very low, wide surgical excision is recommended because of the high incidence of malignancy of this tumor in the oral cavity contrasted with other parts of the body. 相似文献

Actinomycosis was first described as a clinical entity over 100 years ago. However, the fundamental characteristics of this entity have not been fully discussed, and major questions remain unanswered, such as the highly diversified pathogenicity of the phenomenon according to numerous published case reports and clarification of solid diagnostic criteria. Even the frequency of cervicofacial actinomycosis occurrence is unclear; some authors consider it to be rare and others to be common. We present 11 cases examined and treated in our department within the last 14 years along with a review of the literature.
