The use of cryopreserved aged human oocytes in a diagnostic test of sperm fertilizing ability was evaluated. Oocytes arising from assisted conception cycles and showing no signs of fertilization 48 h post-insemination were cryopreserved by one of two methods. An ultrarapid method using dimethyl sulphoxide gave poor post-thaw results, with only 5/69 (7.2%) oocytes surviving. Oocytes frozen by a slow method using propanediol as the cryoprotectant gave better survival rates (359/594; 60%). Fertilization by donor spermatozoa of these thawed oocytes was poor (15/63; 24%) when the zona pellucida was left intact. To improve this, the zona was enzymatically removed using pronase. These zona-free oocytes were then inseminated with spermatozoa from a fertile donor or from men previously exhibiting fertilization failure in an in-vitro fertilization treatment cycle. The fertilization rate in the patient group (41/91; 45%) was significantly lower than in the donor group (16/18; 89%) (P less than 0.02). There was also a significant (P less than 0.03) reduction in the median number of pronuclei per oocyte (2.9 versus 4.5). These results show that aged oocytes can be effectively cryopreserved to establish a bank for use in a test to identify men with impaired sperm fertilizing capacity.  相似文献

BACKGROUND: Round spermatid injections are associated with disappointing clinical outcomes, and although these cells have been shown to mature into late spermatids in vitro, the developmental potential of such gametes remains to be demonstrated. METHODS: Round spermatids were isolated from 12 testicle samples of patients with obstructive azoospermia, hypoplasia, complete maturation arrest, and incomplete Sertoli cell-only syndrome. They were cultured for 7 days at 32 degrees C, 5% CO(2)in air, in microdrops of Vero cell-conditioned medium containing 10% synthetic serum substitute. RESULTS: From the 238 round spermatids cultured, 25.2% attained the elongating and 5.5% the elongated spermatid stage (3-4 days per step). Relatively higher maturation rates were found in cases with obstructive azoospermia, but differences were significant only for elongated spermatids (9.3%). No differences were found in maturation rates between cases with non-obstructive azoospermia (4.3% of elongated spermatids). Experimental microinjections with elongating and elongated spermatids revealed a low fertilization rate (40.9%) but a normal blastocyst formation rate (60%). CONCLUSIONS: Late spermatids resulting from in-vitro culture of round spermatids in conditioned medium, either in controls in cases with a spermiogenetic block, appeared able to successfully fertilize the human oocyte and elicit normal embryo development.  相似文献

Genital tract abnormalities and adverse pregnancy outcome are well known in women exposed in utero to diethylstilboestrol (DES). Data about adverse reproductive performance in women exposed to DES have been published, including controversial reports of menstrual dysfunction, poor responses after ovarian stimulation, oocyte maturation and fertilization abnormalities. We compared oocyte quality, in-vitro fertilization results and embryo quality for women exposed in utero to DES with a control group. Between 1989 and 1996, 56 DES-exposed women who had 125 in-vitro fertilization (IVF) attempts were retrospectively compared to a control group of 45 women with tubal disease, who underwent 73 IVF attempts. Couples suffering from male infertility were excluded. The parameters compared were oocyte quality (maturation abnormalities, immature oocyte, mature oocyte), fertilization and cleavage rate (per treated and metaphase II oocytes), and embryo quality (number and grade). We found no significant difference in oocyte maturational status, fertilization rates, cleavage rates, embryo quality and development between DES-exposed subjects and control subjects. These results suggest that in-utero exposure to DES has no significant influence on oocyte quality and fertilization ability as judged during IVF attempts.  相似文献

At 16–18 h after insemination, 5.5% of the inseminatedoocytes displayed only one pronucleus. The incidence of single-pronucleatedoocytes was not correlated with the age of the patient. Cytogeneticanalysis of 41 embryos derived from single-pronucleated oocytesrevealed a haploid chromosome complement in 12.2%, a diploidchromosome complement in 80.5% and a triploid set in 7.3% ofthe embryos. In one diploid metaphase, a Y chromosome couldbe clearly demonstrated. A total of 312 single-pronucleatedoocytes were evaluated twice (at 16–18 h and at 20–24h after insemination). In 25% of the single-pronucleated oocytesa second pronucleus was observed 4–6 h later, suggestingasynchronous or delayed pronuclear formation. The cleavage ofthese embryos was similar to the cleavage of embryos with twopronuclei at the first evaluation. Parthenogenetic activationand asynchronous pronuclei may both be mechanisms leading tothe morphological observation of a single pronucleus. In clinicalpractice, the second repeat observation of single-pronucleatedoocytes became part of the standard procedure.  相似文献

Human oocyte cryopreservation results in poor survival and subsequentfertilization rates. It has been suggested that freeze-thaw-inducedchanges in the zona pellucida may impair sperm penetration orattachment. The aim of this study was to compare fertilizationand cleavage rates in cryopreserved oocytes inseminated by conventionalin-vitro fertilization (IVF) or intracytoplasmic sperm injection(ICSI). A total of 220 oocytes, obtained from volunteers whohad undergone ovarian stimulation, were cryopreserved usinga slow freeze-rapid thaw protocol with 1.5 M propanediol asthe cryoprotectant. Surviving oocytes (n= 74, 34.4%) were randomlyallocated for fertilization by conventional IVF (group 1) orICSI (group 2) using cryopreserved spermatozoa from a singledonor of proven fertility. Fertilization was achieved in five(13.5%) of the oocytes in group 1 and 17 (45.9%) in group 2(P < 0.005), with only one oocyte in group 1 exhibiting normalfertilization as opposed to 16 (43.2%) in group 2 (P < 0.001).Similarly, one oocyte fertilized by IVF cleaved, while all fertilizedwith ICSI cleaved (P < 0.001). We conclude that althoughthe survival of oocytes is poor following cryopreservation,fertilization and cleavage rates can be enhanced significantlyusing ICSI. These data also suggest that the method of cryopreservationused in this study affected the zona pellucida, such that normalsperm attachment or penetration was impaired.  相似文献

This study reports the subsequent embryo development of cryopreservedmature human oocytes following insemination or intracytoplasmicsperm injection (ICSI). Metaphase II oocytes were cryopreservedusing a slow freezingrapid thawing procedure employing the cryoprotectant1,2- propanediol. The study was conducted at two centres. Thenormal insemination of cryopreserved oocytes was undertakenin one centre, and ICSI of cryopreserved oocytes in the other.Both methods resulted in a 50% normal fertilization rate. Alow rate of abnormal fertilization was observed in the inseminatedgroup of oocytes (5%) compared with 21% for the ICSI oocytes;this was not significantly different. Embryo development wasassessed daily for 7 days. All normal fertilized cryopreservedoocytes in both groups cleaved on day 2, with a similar appearanceto in-vitro fertilization and ICSI embryos. In the normal inseminatedoocytes, there was a significant decrease in the number of embryoscleaving on day 3 (33%) compared with the development of ICSIoocytes, with a subsequent gradual reduction over days 4 and5 (22 and 11% respectively) resulting in one early blastocyston day 7 (11%). In contrast, all ICSI-generated embryos continuedto cleave on day 3, with a gradual reduction over subsequentdays (day 4, 86%; day 5, 57%; day 6, 43%; day 7, 29%). By day7, two of the blastocysts had started to hatch, resulting ina 66% hatching rate of blastocysts formed from ICSI of cryopreservedoocytes. This is the first study to show normal developmentto the hatching blastocyst stage following ICSI of cryopreservedhuman oocytes.  相似文献

An auto-controlled study was conducted in couples with tubal infertility and normozoospermic semen. The fertilization rates and embryonic development in sibling oocytes treated, using the same semen sample, either by conventional in-vitro fertilization (IVF) or intracytoplasmic sperm injection (ICSI) at the same time were compared. Sibling oocyte-cumulus complexes (OCC) of 56 different couples with tubal infertility and normozoospermic semen were randomly divided in order of retrieval into two groups inseminated either by conventional IVF or by ICSI. Of the retrieved OCC in the same cohort, 53.0 +/- 31.2 and 62.0 +/- 26.6% showed two distinct pronuclei after conventional IVF and ICSI respectively (not significant). Complete fertilization failure occurred after conventional IVF in 12.5% (7/56 couples). After ICSI, the comparable figure was 3.6% (2/56). The number of cases was too small to apply a statistical test to this difference. Total cleavage rates were quite similar: 86.7 +/- 28.0 and 90.1 +/- 21% of the zygotes developed into transferable embryos after IVF and ICSI respectively (not significant). Similarly, no difference in embryo quality was observed. Although injection and insemination of the oocytes were performed at the same time in the two groups, at 42 h post-insemination more embryos were at the four-cell stage after ICSI (P < 0.001) than after conventional IVF, where more embryos were still at the two-cell stage (P < 0.02). Embryo transfer was possible in all 56 couples, resulting in 16 positive serum human chorionic gonadotrophin tests (28.6% per embryo transfer), from which a clinical pregnancy resulted in 15 couples. The best embryos were selected for transfer independently of the insemination procedure, but preferably from the same origin. There appeared to be no difference in implantation potency of the embryos obtained with either technique after the non-randomized transfers.  相似文献

We compared the outcome of ooplasmic round spermatid nuclear injections (ROSNI) versus intact round spermatid injections (ROSI). Rabbit round spermatid nuclei and intact round spermatids were recovered and injected into rabbit oocytes (groups A and B, respectively). Fertilization, cleavage and embryonic development rates were compared. In additional studies, five protocols for cryopreservation of round spermatids and two protocols for cryostorage of round spermatids were applied. The outcome of ROSNI techniques using frozen-thawed or cryostored-warmed round spermatids was evaluated. The cleavage rate and the overall morula plus blastocyst development rate were significantly larger in group A than group B. ROSNI procedures are superior to ROSI techniques in the rabbit. The largest fertilization, cleavage and embryonic development rates after ROSNI techniques using cryopreserved or cryostored round spermatids were demonstrated in groups of round spermatids in which a mixture of seminal plasma plus test yolk buffer was employed as an extender, and dimethyl sulphoxide plus a high concentration of glycerol served as cryoprotectants. It appears that the seminal plasma contains factors protecting round spermatids during cryopreservation or cryostorage, and/or the employment of two cryoprotectants has a beneficial role in the maintenance of round spermatid reproductive capacity.  相似文献

We have reported recently the first birth after intrauterinetransfer of embryos obtained by injection of round spermatidsinto oocytes in cases of unexpected azoospermia. Here we providea complete documentation of the series of 11 cases in whichthis novel method of infertility treatment was employed. Infour of these cases, elongated spermatids were identified inthe ejaculate, and it was decided to perform elongated spermatidinjection (ELSI). In the other six cases, only round spermatidswere present, and round spermatid injection (ROSI) was done.In one case, ROSI was given preference to ELSI because of avery poor viability status of elongated spermatids present inthe ejaculate. Fertilization of at least one oocyte was achievedin 10 of the 11 treatment cycles; the fertilization rate inthese 10 cycles ranged between 7 and 100% with a mean valueof 45%. All of the two-pronucleated zygotes cleaved and weretransferred to the patient’s uterus. A singleton pregnancywas achieved in two ROSI cycles. Both pregnancies developeduneventfully and resulted in the birth of normal infants. Thesedata show that intra-ooplasmic injection of spermatids obtainedfrom the ejaculate may become the treatment of first choicein patients with non-obstructive azoospermia.  相似文献

This study was undertaken to assess the potential of aged humanoocytes from failed in-vitro fertilization attempts as a modelfor the study of fertilization events after subzonal sperm insertion(SUZI). Criteria of aged oocyte suitability for this purposewere (i) the absence of nuclei, (ii) the presence of a polarbody, (iii) the absence of cell division or fragmentation, (iv)marked ooplasmic contraction in hyperosmotic medium, and (v)rapid ooplasmic relaxation after returning into normo-osmoticmedium following the micromanipulation. Micro-injection techniqueswere essentially the same as for SUZI with fresh oocytes. Oocytesthat fused with the micro-injected spermatozoa developed pronucleiof typical internal structure. However, the number of pronucleiwas often higher than that theoretically expected if each spermnucleus incorporated into the oocyte gave rise to a single pronucleus.Thus, the use of aged oocytes implies the need for a specificmethod to assess the frequency of fusion in sperm samples examined.The results suggest that the method described here can be appliedin a preliminary diagnostic test before a therapeutic SUZI attemptand in studies aimed at the optimization of sperm treatmentprotocols to increase the fusion capacity of subzonally insertedspermatozoa.  相似文献

The fertilization rate after ooplasmic micro-injection in mammals is low. In hamsters, micro-injection of sperm nuclei into oocytes resulted in the release of the second polar body and the formation of female and male pronuclei, but disappearance of the cortical granules was minimal. These unfavourable results seem to be associated with insufficient activation of oocytes by the current techniques of micro-injection. Therefore, we attempted to increase the fertilization rate by pretreating the oocytes with a Ca2+ ionophore, A23187 (10 microM, 3 min). Sperm nuclei were then micro-injected into the treated oocytes and 5 h later the percentage of oocytes at the pronuclear stage was examined: 53.1% for untreated controls and 85.7% for the treated group (P less than 0.05). However, when the number of pronucleate oocytes was divided by the number of A23187-activated oocytes, the percentage was 81.0% for the controls and 85.7% for the treated group. These findings suggest that pretreatment of oocytes with ionophore does not facilitate fertilization of activated oocytes, but that it indirectly enhances their development to the pronuclear stage by increasing the efficiency of egg activation.  相似文献

The impact of intracytoplasmic sperm injection (ICSI) on cryopreservedzygotes and embryos was evaluated by comparing embryo survivaland implantation between embryos derived from ICSI and thosederived from standard insemination procedures. The study includedpatients whose excess zygotes and embryos were cryopreservedbetween September 1993 and December 1994 and who subsequentlyunderwent a frozen embryo transfer. Embryo survival, clinicalpregnancy rates per transfer and pregnancy outcome were compared.Three hundred and thirty eight cryopreservation cycles, duringwhich 1471 embryos were cryopreserved, were included in thisstudy. Of those, 961 were derived from oocytes fertilized byinsemination in vitro and 510 were derived from oocytes fertilizedby ICSI. A total of 690 of the embryos (451 in the inseminationgroup and 239 in the ICSI group) have since undergone a thawcycle. The embryo survival rates were similar between the twogroups (70.5 and 73.2%, insemination and ICSI respectively)and were not significantly affected by the stage at cryopreservation.There was no significant difference in pregnancy rates per transfer(31.8 and 32.3%), the preclinical pregnancy loss rate (16.7and 23.8%), or the clinical miscarriage rate (16.7 and 23.8%)between the insemination and the ICSI groups respectively. Itis concluded that ICSI does not have an adverse impact on thesurvival and successful implantation of cryopreserved and thawedembryos.  相似文献

Micro-centrifugation (MC) at 6500 r.p.m. (3352 g) has not beenused previously for spermatozoal concentration and subsequentfertilization. We investigated MC for micro-insemination spermatozoaltransfer (MIST) of human spermatozoa from normal donors intohamster oocytes. MC resulted in reduced penetration of hamsteroocytes, both after MIST [77.4% (41/53) versus 87.8% (43/49)for control; NS] and after exposure to zona-free hamster oocytes[60.8% (79/130) versus 72.7% (88/121) for control; P < 0.05].However, MIST under the zona resulted in better incorporationof sperm nuclei when compared with zona-free hamster oocytes,for spermatozoa exposed to micro-centrifugation (77.4 versus60.8%, P < 0.05) and controls (87.8 versus 72.7%, P <0.05). Polyspermy was higher after MIST [22.0% (9/41) versus13.9% (11/79); NS] for MC + , and [25.6% (11/43) versus 13.6%(12/88); NS] for MC-. We conclude that MC does have a negative,but minimal effect on the fertility of human spermatozoa withrespect to hamster oocytes.  相似文献

Oocytes (unfertilized and preovulatory) and embryos (normal and polypronuclear), which were donated to research by patients undergoing procedures of assisted reproductive treatment, were analysed for cytogenetic abnormalities. A total of 362 oocytes and embryos were analysed. The unfertilized oocytes with readable metaphases (53.4%) gave 25.2% chromosomal abnormality with diploidy being the main aberration observed. A high incidence of premature chromosome condensation (PCC) was observed and the incidence of PCC in oocytes exposed to colcemid was significantly higher (14/62, 22.6%) than in those not exposed to this treatment (3/41, 7.3%, P less than 0.05). When chromosomal anomalies and PCC in the unfertilized oocytes were correlated to various patient criteria such as stimulation regimen, number of human menopausal gonadotrophin ampoules, peak oestradiol levels, age of patient and number of previous attempts, none of the criteria tested had any significant relationship to the incidence of chromosomal abnormality. However a significant increase in the incidence of PCC was noted in the gonadotrophin-releasing hormone (GnRH) 'flare' group (6/15, 40.0%) compared to the GnRH 'down-regulation' group (11/88, 12.5%). The incidence of chromosomal abnormalities among preovulatory oocytes was 16.7% and diploidy was the only abnormality noted. For embryos arising from two-pronuclear oocytes, the chromosomal constitution related mainly to embryo quality. The rate of chromosomal abnormality for apparently good quality embryos was 23.5% and for poor or fragmented embryos 83.3%. The majority (77.3%) of the readable metaphase plates for polypronuclear 1-cell and cleaved embryos showed grossly abnormal chromosome complements but 19% of the cleaved embryos contained sets of normal diploid chromosomes.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

The purpose of the study was to examine the stress associatedwith in-vitro fertilization (IVF) concurrently with other physicaland relational variables, and to compare these reactions withthose reported during a menstrual cycle without treatment. Women(n = 20) completed a daily symptom checklist for one completemenstrual cycle without treatment and one complete IVF cycle.The checklist included items related to stress, optimism, physicaldiscomfort and marital and social relationships. Daily ratingsduring IVF were compared with those obtained during the no-treatmentmenstrual cycle. IVF was associated with more stress, optimismand physical discomfort than a menstrual cycle without treatment,and with greater changes to marital and social relationships.The pattern of results shows that the stress associated withIVF is less salient when examined in the context of reactionsin other areas of functioning. The findings suggest that theemotional impact of IVF might be less pronounced during theactual treatment process than is generally assumed from studiesfocusing on the impact of treatment failure. Variables suchas optimism and physical discomfort which have previously receivedless attention in the literature were significantly affectedby IVF treatment.  相似文献

BACKGROUND: This study was conducted to investigate the effects of supplementation with free radical scavengers on the survival and fertilization rates of freeze-thawed mouse oocytes. METHODS: Superovulated oocytes with cumulus cells were cryopreserved by slow freezing in propanediol combined with a rapid thawing protocol. The cryopreservation medium was supplemented with the antioxidant enzymes superoxide dismutase (SOD) and catalase, and with the nitric oxide (NO) scavenger, haemoglobin (Hb). RESULTS: The addition of 50 IU/ml SOD showed significantly higher survival and fertilization capabilities compared with control (P < 0.01). Oocyte survival was greatly increased by concomitant addition of SOD with 10 IU/ml catalase (P < 0.01). On the other hand, the NO donor (sodium nitroprusside) inhibited survival and fertilization rates (P < 0.05). Significantly decreased survival and fertilization rates were also observed following the addition of high concentrations (10(-3) to 10(-6) nmol/l) of the NO synthase inhibitor N(G)-nitro-L-arginine methyl ester (L-NAME). In contrast, significantly better oocyte survival and fertilization rates were detected with low concentrations (10(-7) nmol/l) of L-NAME. Oocyte survival potential was significantly increased by addition of Hb (1 microg/ml, P < 0.05). Moreover, oocyte survival and fertilization rates were significantly promoted by the concomitant addition of SOD with Hb (P < 0.01). CONCLUSIONS: These results suggest that supplementation of free radical scavengers, particularly combinations of SOD with NO scavengers in freezing and thawing media, improved the post-thaw survival and fertilization rates of cryopreserved mouse oocytes.  相似文献

A retrospective analysis in 50 couples of 53 cycles of intracytoplasmic sperm injection (ICSI) with immotile spermatozoa from testicular-retrieved spermatozoa was performed to evaluate whether total immotile spermatozoa achieved after testicular sperm extraction could fertilize ova and result in pregnancies. We assessed the efficacy of ICSI with totally immotile testicular spermatozoa extracted from the testes of azoospermic patients with severe spermatogenic failure (group 1) and compared these results with those from spermatozoa which were recovered after several hours of incubation and were motile (group 2) at the time of injection. In 19 cycles, only totally immotile spermatozoa were injected at the time of ICSI. For the remaining 34 cycles, at least one motile spermatozoon was found for injection. The oocyte fertilization rates were 51% for group 1 and 62% for group 2 (P < 0.02). Eighteen of 19 cycles in group 1 (90%) and all 34 (100%) cycles in group 2 had embryos for replacement. The mean number of embryos per cycle was 5.2 +/- 0.8 and 7.5 +/- 0.9 in groups 1 and 2 respectively; this and the embryo quality (cumulative embryo scoring = 40 +/- 8 for group 1 and 50 +/- 7 for group 2), and clinical pregnancy rates (15.8% per oocyte retrieval in group 1 and 23.5% in group 2) were not significantly different between groups. Fertilization, cleavage and pregnancy can be achieved with intracytoplasmic testicular sperm injection from patients with immotile spermatozoa, at levels comparable with those of ICSI using motile spermatozoa.  相似文献

This study reports the obstetric outcome of pregnancies obtained after the transfer of cryopreserved or fresh embryos where the initial procedure was standard in-vitro fertilization (IVF) and intracytoplasmic sperm injection (ICSI). Pregnancies obtained after frozen IVF (n = 245) or frozen ICSI (n = 177) were compared with a control group of pregnancies after fresh embryo transfer in standard IVF (n = 245) and ICSI (n = 177) cycles were selected as controls. The controls were matched according to maternal age, parity and date of embryo transfer. In the standard IVF group, the biochemical pregnancy rates in the cryopreserved and fresh groups were 18.8 and 9.8% respectively (P < 0.01). In the ICSI group, the biochemical pregnancy rates in the cryopreserved and fresh groups were 16.4 and 6.8% respectively (P < 0.01). The miscarriage rates were comparable between the cryopreserved and fresh groups. However, in the frozen ICSI group the miscarriage rate (26.0%) was significantly higher than in the frozen conventional IVF group (13.1%) (P = 0.001). The frequencies of preterm deliveries, infants with very low birthweight and intrauterine deaths were similar in the groups. The low birthweight rates in the frozen IVF (16.1%) and ICSI (12.1%) groups were significantly lower than those in the fresh IVF (32.2%) and ICSI (32.7%) groups (P < 0.001). The major malformation rates in the frozen IVF (2.4%) and ICSI (2.9%) groups were not different from the major malformation rates in the fresh IVF (4.5%) and ICSI (2.4%) groups. In conclusion, the cryopreservation process had no negative impact on the outcome of pregnancies over 20 weeks of gestation. Long-term follow-up studies are needed in order to prove the safety of the freezing-thawing process.  相似文献

The morphological changes caused by freezing and thawing human testicular spermatozoa have been assessed here. Retrieval of testicular biopsies was carried out on six patients with obstructive azoospermia preparatory to intracytoplasmic sperm injection (ICSI). Light microscope analysis was carried out on testicular cells and ultrastructural analysis was carried out on spermatozoa and different spermatid stages before and after the freezing procedure. Upon examination under light microscopy, all germ cells presented increased vacuolization in their cytoplasm and shrinkage or swelling of the nuclei and cytoplasmic membranes. These altered structures were accentuated in the spermatocyte I cell which often presented disrupted membranes. The ultrastructural findings under transmission electron microscopy demonstrated that after freezing and thawing the major types of cryoinjury were the swelling and rupture of inner and outer acrosomal and plasma membranes. The acrosome material often appeared as dispersed material or as condensed spots or was even lost. Such damage was observed mainly at the spermatozoa and late spermatid stages. We conclude that the freezing and thawing of testicular biopsies causes similar morphological damage to testicular spermatozoa and frozen-thawed ejaculated spermatozoa. It is still unclear whether these changes in testicular spermatozoa after freezing and thawing may compromise its use in the ICSI procedure.  相似文献

Zona-free human oocytes are frequently encountered in in-vitro fertilization (IVF) laboratories. The oocytes escape out of the zona pellucida, following zona fracture, which can occur during oocyte retrieval or manipulation, but occasionally may be the result of increased zona fragility. Some of the zona-free oocytes are mature and morphologically healthy; nevertheless, all are typically discarded. In this report, we demonstrate that zona-free oocytes can be fertilized normally using intracytoplasmic sperm injection (ICSI) and can subsequently develop without zona to the blastocyst stage in vitro. We therefore suggest that those mature and morphologically normal zona-free oocytes may be rescued, fertilized with ICSI and then cultured to the blastocyst stage for subsequent transfer or cryopreservation.  相似文献
