Background
Systemic chemotherapy has become established as the standard treatment for advanced incurable gastric cancer. In the metastatic setting, systemic chemotherapy can prolong the survival time, improve symptom control and help to maintain a better quality of life.Material and methods
Testing the human epidermal growth factor receptor 2 (HER2) expression status of the primary tumor and/or metastases is warranted before initiation of first-line treatment.Results
In cases of HER2 overexpression or amplified tumors, trastuzumab in combination with cisplatin and 5?fluorouracil (5-FU) or capecitabine are the standard forms of treatment. Tumors that are HER2 negative or weakly positive are treated with a platinum-fluoropyrimidine combination as doublet or as a triplet with docetaxel in younger patients who can tolerate more intensive treatment. Taxane derivatives, such as docetaxel and paclitaxel as well as irinotecan are accepted treatment options for patients who show progress during or after first-line chemotherapy. The anti-vascular endothelial growth factor receptor 2 (anti-VEGFR2) antibody ramucirumab demonstrated efficacy with prolongation of survival both as monotherapy and in combination with paclitaxel.Objective

Background
An early diagnosis by optimal diagnostics and tumor staging are required for correct management of gastric cancer. Modern imaging techniques are used to determine the best therapeutic options within surgical and multimodal concepts and to avoid surgery for inoperable cases.Objective
Overview about the current state of the art in TNM staging of gastric cancer using modern imaging techniques.Material and methods
A review of the literature on TNM staging of gastric cancer using endoscopy, endoscopic ultrasonography (EUS), EUS-guided fine needle aspiration biopsy and tomographic imaging techniques, such as multidetector computed tomography (CT), magnetic resonance imaging (MRI), positron emission tomography CT (PET-CT) and PET-MRI as well as laparoscopy.Results
Endoscopy is still the first-line diagnostic method and gold standard for early detection, histological diagnosis and exclusion of gastric cancer. The EUS technique is used to assess the extent of intramural infiltration (T staging). The determination of the exact N stage using EUS, multidetector CT or MRI remains a problem but can be improved by EUS-guided fine needle aspiration biopsy of paragastric lymph nodes if clinically necessary. Multidetector CT and MRI are used to detect extramural growth of gastric cancer, for diagnosis of local infiltration and of distant metastases (M staging). Laparoscopy is the method of choice for the diagnosis of peritoneal cancer not detectable by conventional imaging.Conclusion
Following initial endoscopic diagnosis the TNM classification (cTNM) of gastric cancer can be carried out using endoscopy, EUS, CT and MRI; however, there are diagnostic loopholes in determination of the N staging and non-invasive detection of peritoneal cancer.8.

For patients with gastric cancer an interdisciplinary approach is important in order to select the most appropriate therapy. The clinical symptoms are often non-specific. The organ most commonly affected by metastases is the liver. Tumor markers play a relatively unimportant role in laboratory investigations. In addition to optical endoscopic procedures, ultrasound endoscopy has now become firmly established for diagnosis and staging of gastric cancer. In addition to endosonography additional imaging techniques are used for staging and preoperative evaluation of gastric cancer. PET is a very promising development, but is not yet a standard procedure. Laparoscopy allows an exact assessment of the degree of invasion of the lymph nodes of compartment II and after opening the bursa omentalis a direct estimation of infiltration the pancreas and left liver lobe in cases of advanced tumors. 相似文献

Background
The incidence of gastric cancer in Germany is 24.2 per 100,000 inhabitants per year (cancer registry 2011) and in 15?% of these patients peritoneal carcinomatosis is simultaneously detected at the time of the primary diagnosis.Objective
Patients with peritoneal carcinomatosis due to metastases from gastric cancer should be evaluated for the possibility of cytoreductive surgery and hyperthermic intraperitoneal chemotherapy (HIPEC). The indications and outcome for cytoreductive surgery and HIPEC are discussed in this paper.Methods
The study included a systematic review of the literature and development of criteria which simplify the decision of patient selection for cytoreductive surgery and HIPEC. This review highlights the patient outcome and outlines possible future diagnostic and therapeutic options.Results
Completeness of cytoreductive surgery is the most important prognostic factor for patients with peritoneal carcinomatosis from metastasized gastric cancer. The median survival time can be significantly prolonged in selected patients from 1–3 months to 11 months using a multimodal therapeutic regimen including chemotherapy, cytoreductive surgery and HIPEC.Conclusion
To achieve an optimal outcome (complete cytoreduction) with improvement in patient survival, patient selection performed by a multidisciplinary group of specialists (e.g. surgeon, oncologist and anesthesiologist) is crucial. Neoadjuvant and adjuvant chemotherapy is generally recommended in order to achieve an improved rate of complete cytoreduction and to potentially prolong disease-free and overall survival times.12.

Zusammenfassung Magenfrühkarzinome sind durch eine maximale Infiltration der Mukosa oder der Submukosa des Magens definiert, unabhängig vom Vorhandensein von Lymphknotenmetastasen. Sie können überall im Magen auftreten. Ein in der Therapie wichtiges Verfahren ist die endoskopische Mukosaresektion (EMR). Die Indikationen und Grenzen der EMR beim Magenfrühkarzinom basieren weitgehend auf Ergebnissen aus Operationsserien und japanischen Mukosektomie-Serien (Mukosakarzinome; keine Submukosainfiltration; erhabene, flache und leicht eingesenkte Tumorwuchsart, aber kein Tumorulkus; Tumorgröße bis 2 cm; histologisch gut oder mäßig differenzierte Karzinome; keine Invasion von Gefäßen). The prognosis after surgical treatment of locally advanced gastric cancer remains poor. The 5-year survival rate for patients with stage II and stage III cancers is reported to be 14–57%. The bad prognosis is determined by the limited rate of R0 resections and the high probability of systemic relapse. Both problems could be ameliorated by neoadjuvant treatment. Neoadjuvant chemotherapy was demonstrated to be effective and safe in several phase II studies. For initially irresectable non-metastatic tumours, neoadjuvant chemotherapy can be regarded as a standard of care. In cases of response, secondary resection should be considered. Results from recently published phase II trials characterised preoperative chemoradiation also to be active and feasible in selected patients, but the amount of clinical data is still rather limited. First results from the randomised phase III “MAGIC” trial indicate a prognostic benefit for patients who underwent neoadjuvant chemotherapy. Results from other study groups will be presented in the near future. Therefore, neoadjuvant chemotherapy is a modality for which a rising evidence and influence on routine treatment strategies in locally advanced gastric cancer can be expected. 相似文献

verbesserten Verfahren zur perkutanen Radiotherapie die intraoperative Radiotherapie einen interessanten Ansatz zur lokal

Publikation der ersten Ergebnisse einer gro?en randomisierten Intergroup-Studie im letzten Jahr hat die Diskussion um die
Vorteile einer postoperativen simultanen Radiochemotherapie angeregt. Der vorliegende Beitrag diskutiert methodische Grundlagen
