Topical fluorides have been used in dental practice for over 30 years. An outline of the development of various fluoride preparations during this period of time is presented and the results of clinical trials into the caries-inhibitory properties of different topical fluoride agents are reviewed. The purpose of the paper is to draw together the evidence on which the rationale for topical fluoride treatment is based.  相似文献

This paper summarizes current trends in the clinical diagnosis of occlusal caries in response to the RTI/UNC review and reflects the dilemma felt by many dentists who understand the difficulty in accurately assessing the extent and activity of pit and fissure caries in many of their patients. They are unsure if they should be aggressive in instrumenting suspicious lesions and provide small restorations, some of which may not be indicated. Alternatively, should they wait until signs are more clear-cut and provide larger restorations? Discussed here is the advantage of practicing dentists who obtain immediate false-positive feedback when they instrument a tooth with no clinical caries and false-negative feedback when a recall patient exhibits progression of an equivocal lesion. They should be encouraged to use this feedback as part of their diagnostic procedure and explain to their patients the difficulty of providing an accurate and precise diagnosis with existing tests.  相似文献

This paper reviews the use of professionally applied topical fluorides (PATF) in caries prevention. PATFs are indicated for children and adults with one or more decayed smooth surfaces and/or those who are at high caries risk. Frequency of administration depends on the patient's caries risk, and is usually every 6 months. The effectiveness of fluoride varnish and gel applications has been well established in caries prevention trials involving permanent teeth. Although both types are effective, varnish may be preferred because it is easier to apply, reduces the risk of fluoride over-ingestion, and has greater patient acceptance. Fluoride foams are similar products to gels, but have not been tested clinically. The use of in-office two-part rinses is not recommended because they have not been proven effective. A cleaning, or prophylaxis, is not necessary before the application of topical fluoride for caries prevention. In conclusion, when used appropriately, PATFs are a safe, effective means of reducing caries risk among high-risk populations.  相似文献

Fluoride therapy continues to be the best defense in the battle against dental caries. The decision to utilize topical fluorides is no longer age dependent. Changing disease patterns require dentists to critically evaluate the caries risk of each patient and develop a fluoride treatment plan based upon the needs of the individual patient. A variety of professional and self-applied fluoride products are available and new fluoride delivery systems have recently entered the market. A critical review of literature, combined with an understanding of the advantages and disadvantages of each topical fluoride system will assist the dentist in selecting the product best suited for each patient.  相似文献

To investigate the effectiveness of topical fluorides in preventing fissure caries, we conducted a randomized controlled trial with parallel groups. In total, 501 children (1,539 molars, 3,078 sites), mean age 9.1 years, who had at least one sound permanent first molar with deep fissures or fissures with signs of early caries were recruited. They were randomly allocated among four groups: (1) resin sealant, single placement; (2) 5% NaF varnish, semi-annual application; (3) 38% silver diamine fluoride (SDF) solution, annual application; and (4) placebo control. Follow-up examinations were conducted every 6 months by a masked examiner. After 24 months, 485 children (97%) were examined. Proportions of pit/fissure sites with dentin caries in the sealant, NaF, SDF, and control groups were 1.6%, 2.4%, 2.2%, and 4.6%, respectively. A multi-level logistic regression analysis accounting for the effects of data clustering and confounding factors showed that fissures in any of the three treatment groups had significantly lower risks of carious cavity development into dentin than did controls (p < 0.05). We concluded that placement of resin sealant, semi-annual application of NaF varnish, and annual application of SDF solution are all effective in preventing pit and fissure caries in permanent molars (ClinicalTrials.gov number CT01446107).  相似文献

Inadvertent ingestion of fluoride from topical procedures poses the risk of dental fluorosis in age-susceptible users. This risk can be minimized for mouthrinsing by restricting it to those above the age-susceptible limit. Operators administering topical fluoride treatments can take practical precautions that have been shown to reduce the amount of fluoride retained and, thus, reduce ingestion. Clinical and epidemiological studies have failed to establish that dentifrice ingestion by young children constitutes a serious fluorosis risk. Discussion of risk must consider the effect of perceived risks upon the public.  相似文献

PURPOSE: The purpose of this study was to assess the efficacy of antimicrobial therapy in the prevention of early childhood caries (ECC). METHODS: The study population consisted of 83 subjects (age: 12 to 19 months (x= 15.6); gender: 40 females and 43 males). Inclusion criteria included: (1) unremarkable medical history; (2) presence of 4 maxillary primary incisors (MPI) with no visible defects; (3) clinically caries free; (4) use of a nursing bottle at naptime and/or bedtime which contained a cariogenic substrate; (5) 2 consecutive ms positive cultures from pooled MPI plaque. The subjects were randomized into 2 groups. The 39 subjects in the experimental group and the 44 subjects in the control group were evaluated every 2 months during the study period. At each evaluation, the subjects had 10% povidone iodine (experimental group) or placebo (control group) applied to their dentition. Treatment failure was defined as the appearance of a white spot lesion(s) on any of the MPI during the study period. RESULTS: Using the Kaplan-Meier procedure, the estimated percents (+/- SES) of participants to experience 12 months of disease-free survival were 91 +/- 5% for those receiving treatment and 54 +/- 9% for those in the control group. Via the log-rank test, the hazard of treatment failure is statistically significantly higher in the placebo group (log-rank statistic 10.28, two-sided P=0.0013). CONCLUSIONS: These observations indicate that topical antimicrobial therapy increases disease-free survival in children at high risk for ECC.  相似文献

PURPOSE: The purpose of the present paper is: 1) to review recent concepts of the causes of caries; 2) to illustrate parameters of causes of cases and of incidence; and 3) to outline the consequences for caries prevention and oral health promotion. The paper is divided in three sections. Section 1 reviews recent theories of the causes of caries and summarizes that it is necessary to make a clear distinction between the causes of a case of caries and causes of occurrence of caries in populations. Cases relate to the reasons why individuals get sick, while incidence relates to why so many (or so few) within a population get sick. In section 2 the difference between the causes of a case and of the occurrence in a population is illustrated. A new social epidemiology moves beyond the focus on individual level risk factors to a multi-level perspective. Applied to oral diseases the paradigm of social epidemiology bridges our understanding of the biological determinants of caries with an understanding of the societal determinants of caries. Social epidemiology is the branch of epidemiology that studies the social distribution and social determinants of states of health. Individuals are embedded in societies and populations. In section 3 the insight acquired above is applied to the choice of disease-preventive and oral health-promotive strategies. Prevention of caries and promotion of oral health must be rooted in the understanding of caries as it occurs in populations.  相似文献

Early childhood caries (ECC) is a virulent form of dental caries that can destroy the primary dentition of toddlers and preschool children. It occurs worldwide, afflicting predominantly disadvantaged children. High-risk North American populations include Hispanic and Native American children, as well as children enrolled in Head Start, a federally funded program for preschool children living in poverty. The prevalence of EEC among these children ranges from 11% to 72%. ECC is an infectious disease, and Streptococcus mutans is the most likely causative agent; diet also plays a critical role in the acquisition and clinical expression of this infection. Early acquisition of S. mutans is a key event in the natural history of the disease. Acquisition may occur via vertical or horizontal transmission. Primary oral colonization by S. mutans coupled with caries-promoting feeding behaviours results in accumulation of these organisms to levels exceeding 30% of the total cultivable plaque flora which in turn leads to rapid demineralization of tooth structure. Treatment of ECC is costly because the cooperative capacity of babies and preschool children usually necessitates the use of general anesthesia. Treatment usually consists of restoration or surgical removal of carious teeth along with recommendations regarding feeding habits. However, this approach has resulted in unacceptable clinical outcomes, and relapse rates of approximately 40% have been reported within the first year after dental surgery. Primary prevention of ECC has largely been restricted to counselling parents about caries-promoting feeding behaviours. This approach has also had minimal success. Newer strategies addressing the infectious component through use of topical antimicrobial therapy appear promising.  相似文献

Abstract — Acidulated phosphate fluoride (APF) and stannous fluoride (SnF2) solutions applied to acetic acid etched and unetched human enamel shows that two 2 minute applications of APF on the etched enamel significantly softened it, but there was no significant change when SnF2 was used.  相似文献
