The aetiology of community-acquired pneumonia in childhood was studied in the total population of 8851 children in the area of 4 municipalities in eastern Finland. All cases of community-acquired pneumonia (n = 201) were registered during a surveillance period of 12 months between September 1, 1981 and August 31, 1982. The diagnosis of pneumonia was verified radiologically in all identified cases. The diagnosis of chlamydial infection was based on an antibody response measured by complement fixation (CF), by enzyme immunoassay (EIA; IgG or IgM) or by microimmunofluorescence (MIF; IgG or IgM), and the diagnosis of mycoplasmal infection on CF alone. In total, 29 cases of Chlamydia sp. infection were diagnosed; 20 were caused by Chlamydia pneumoniae. Thus, C. pneumoniae was an aetiological agent in 10%, of the 201 pneumonia cases: the proportion was 9% for children aged 5-9 y and 31% for those aged 10 y or more. In the study population, the total incidence of C. pneumoniae pneumonia was 2.3/1000/y. Mycoplasma pneumoniae serology (CF) was positive in 44 patients (22%); the total incidence of M. pneumoniae pneumonia was 5.0/1000/y. Serological evidence of both Chlamydiae and M. pneumoniae was detected in 9 (41%) patients. Our results indicate that C. pneumoniae is an important cause of community-acquired pneumonia in school-aged children. Diagnostic serological response to Chlamydia species or M. pneumoniae was found in 42% of pneumonia patients between 5 and 9 y of age and in 67% of patients aged 10 y or more. Thus, we suggest that macrolides should be considered as an empirical antimicrobial treatment for community-acquired pneumonia, especially in school-aged outpatients.  相似文献

BACKGROUND: The prevalence of community-acquired pneumonia (CAP) caused by atypical pathogens in Thai children is unknown. OBJECTIVE: To examine the prevalence of Mycoplasma pneumoniae and Chlamydophila pneumoniae infections in paediatric patients (aged 2-15 years) with CAP in three academic hospitals using standardised laboratory techniques. The characteristics of atypical pneumonia were also compared with other causes of CAP. METHODS: Diagnosis of current infection was based on a four-fold or more rise in antibody serum samples or persistently high antibody titres together with the presence of mycoplasmal or chlamydial DNA in secretions. RESULTS: Of 245 patients with CAP, 17.5% of cases were caused by atypical pathogens (M. pneumoniae 14.3%, C. pneumoniae 2.8% and co-infection 0.4%). We also found atypical pathogens in young children aged 2-5 years. The clinical and laboratory findings did not distinguish atypical pneumonia from other CAPs. Segmental or lobar consolidation on chest X-rays was more common in atypical pneumonia, while dyspnoea was more prominent in other CAPs. CONCLUSION: Our data show a high prevalence of M. pneumoniae and C. pneumoniae in Thai children with CAP, including in children aged 2-5 years.  相似文献

Serological methods are routinely used in the diagnosis of viral and atypical bacterial respiratory infections. Recently, they have also been applied to typical bacteria, such as Streptococcus pneumoniae. The aim of this study was to determine the aetiology of paediatric community-acquired pneumonia (CAP) in both ambulatory and hospitalized patients, by using antibody assays. During a 15-month prospective surveillance, paired sera were studied for antibodies to 14 microbes in 101 children with symptoms of acute infection and infiltrates compatible with pneumonia on chest radiographs. A potential causative agent was detected in 66 (65%) patients. Evidence of bacterial, viral and mixed viral-bacterial infection was demonstrated in 44%, 42% and 20% of the CAP cases, respectively. The most commonly found agents included Mycoplasma pneumoniae (27%), Pneumococcus (18%) and respiratory syncytial virus (17%). Human metapneumovirus (hMPV) was detected in 5 (5%) children. Pneumococcal infections were evenly distributed among the age groups studied. Our results confirm the role of S. pneumoniae in paediatric CAP at all ages, those of M. pneumoniae at >2 y of age and emphasize the emerging role of hMPV. The high proportion of mixed viral-bacterial infections highlights the need to treat all children with CAP with antibiotics.  相似文献

Simkania negevensis, a recently found Chlamydia-like organism, has been associated with respiratory infections in children and adults with pneumonia, but S. negevensis findings have been common also without any infection. The aims of the present paper were to evaluate S. negevensis in the aetiology of paediatric community-acquired pneumonia (CAP), its seroprevalence in north Italian children, and whether there is cross-reactivity between S. negevensis and Chlamydia pneumoniae serology. Antibodies to S. negevensis were measured by microimmunofluorescence (MIF) in 101 frozen paired sera obtained from children with CAP. Serological evidence (> or =4-fold increase or decrease in IgM or IgG) of acute S. negevensis infection was achieved in 5 (5%) cases. Two were mixed infections with Mycoplasma pneumoniae and 1 with respiratory syncytial virus. In total, 20-30% of the children had measurable antibodies to S. negevensis, with no association with age. No cross-reactivity was observed between antibodies to S. negevensis and C. pneumoniae. S. negevensis appears to be a real, though rare, cause of CAP in children.  相似文献

OBJECTIVE AND BACKGROUND: Childhood community-acquired pneumonia (CAP) remains a leading cause of morbidity and mortality worldwide. The features of childhood CAP vary between countries. The aim of this study was to delineate the clinical characteristics, complications, spectrum of pathogens and patterns of antimicrobial resistance associated with hospitalized cases of childhood CAP in Singapore. METHODS: A retrospective study was conducted of patients discharged from Singapore's only children's hospital over a 3-year period with a principal diagnosis of CAP. RESULTS: A total of 1702 children, with a median age of 4.2 years (range: 1 month-16.3 years) were enrolled. A pathogen was identifiable in 38.4% of cases, including Mycoplasma pneumoniae in 20.3%, typical respiratory bacteria in 10.3% (64.6%Streptococcus pneumoniae; 21.7% non-typeable Haemophilus influenzae), viruses in 5.5% and mixed bacterial/viral infections in 2%. The majority of M. pneumoniae infections were in school-aged children (>5 years). Severity of infection was greater in CAP caused by typical bacteria, as reflected by length of hospital stay, CRP level, white cell and absolute neutrophil counts. Mortality from typical bacterial infections (8.9%) exceeded that from M. pneumoniae (0.3%) and viral pneumonias (0%) (P < 0.001). Aminopenicillins were often prescribed empirically for suspected S. pneumoniae and H. influenzae infections; however, resistance to these agents was frequently documented among S. pneumoniae (58.5%) and H. influenzae isolates (51%). CONCLUSION: In Singaporean children hospitalized with CAP, M. pneumoniae is the most commonly identified causative organism, followed by common respiratory viruses, S. pneumoniae and H. influenzae. Streptococcus pneumoniae and H. influenzae are associated with greater severity of infection than other organisms, and have high levels of resistance to commonly prescribed antibiotics.  相似文献

目的 对引起社区获得性肺炎 (CAP)的病原体进行流行病学调查 ,加强对非典型致病原重要性的认识。方法 收集 2 0 0 1年 11月至 2 0 0 2年 6月CAP患者共 10 3例。分别检测患者急性期及恢复期肺炎支原体、嗜肺军团菌、肺炎衣原体血清抗体。应用聚合酶链反应 (PCR)分别扩增肺炎支原体P1黏附蛋白基因及肺炎衣原体 16SrRNA基因。应用酶免疫测定 (EIA)方法检测尿中嗜肺军团菌抗原。取患者急性期痰标本进行细菌培养 ,应用常规方法分离鉴定细菌。结果  10 3例CAP患者 ,病原体检出率为 48.5%。 2 3例 (2 2 .3 % )检出肺炎支原体 ,3例 (2 .9% )检出嗜肺军团菌 ,2例 (1.9% )检出肺炎衣原体。 12例分离出肺炎链球菌 (11.7% )、9例分离出流感嗜血杆菌 (8.7% )、7例分离出肺炎克雷伯菌 (6.8% )。 6例患者存在混合感染 (5.8% ) ,其中 5例为肺炎支原体混合其他病原体。结论 非典型病原体尤其是肺炎支原体感染在CAP中占据重要地位 ;肺炎链球菌和流感嗜血杆菌仍为常见的致病细菌 ;混合感染不容忽视  相似文献

Mycoplasma pneumoniae is a common cause of community-acquired pneumonia (CAP) in children, but there has been no clinical report on M. pneumoniae infections in Vietnamese children. We investigated the clinical features of M. pneumoniae infection when the pathogen was detected in the respiratory tract in hospitalized children aged 1-15 years due to lower respiratory tract infections or CAP in Vietnamese children. Throat swabs from 47 patients (18.6%) of 252 patients with a clinical diagnosis of CAP were PCR positive (male, 34; female, 13), and 21 throat swabs (8.3%) showed culture positive for M. pneumoniae. The M. pneumoniae pathogen could be detected by PCR and/or culture in 52 patients (male, 36; female, 16). The major clinical signs in the 52 patients were fever (>38 degrees C) in 100%, pharyngitis in 100%, tachypnea in 94%, dry cough in 86.5%, and rough breathing in 83% of patients. The average term of illness prior to hospitalization was 7.5+/-4.1 days, and the average number of hospitalized days was 7.9+/-3.5 days. Beta-lactam group antibiotics, which were ineffective against M. pneumoniae infection, were used in 37 cases (71%).  相似文献

STUDY OBJECTIVES: To determine microbial agents causing community-acquired pneumonia (CAP) in Southeast Asia. DESIGN: A prospective study. SETTING: Three general hospitals in Thailand. PATIENTS: Two hundred forty-five adult patients fulfilling the clinical criteria of CAP from September 1998 to April 2001. INTERVENTIONS: Investigations included sputum Gram stain and culture, blood culture, pleural fluid culture (if presented), urine antigen for Legionella pneumophila and Streptococcus pneumoniae, and serology for Mycoplasma pneumoniae, Chlamydia pneumoniae, and L pneumophila. RESULTS: There were 98 outpatients and 147 hospitalized patients included in the study, and an organism was identified in 74 of 98 outpatients (75.5%) and 105 of 147 of the hospitalized patients (71.4%). C pneumoniae (36.7%), M pneumoniae (29.6%), and S pneumoniae (13.3%) were the most frequent causative pathogens found in outpatients, while S pneumoniae (22.4%) and C pneumoniae (16.3%) were the most common in hospitalized patients. There was a significantly higher incidence of C pneumoniae (36.7% vs 16.3%, respectively; p < 0.001) and M pneumoniae (29.6% vs 6.8%; p < 0.001, respectively) in the outpatients than in the hospitalized patients. The incidence of S pneumoniae, L pneumophila, and mixed infections was not different between the groups. Mixed infections were presented in 13 of 98 outpatients (13.3%) and 9 of 147 hospitalized patients (6.1%), with C pneumoniae being the most frequent coinfecting pathogen. CONCLUSIONS: The data indicate that the core organisms causing CAP in Southeast Asia are not different from those in the Western countries. The guidelines for the treatment of patients with CAP, therefore, should be the same.  相似文献

Mycoplasma pneumoniae is a common cause of community-acquired pneumonia (CAP) in children and young adults, but limited information about its prevalence in the elderly is available. The polymerase chain reaction (PCR) with primers targeting the cytadhesine P1 gene and the 16S rRNA gene was analyzed for detecting M. pneumoniae in throat washings of 84 patients, aged 60-96 years, with clinical diagnosis of CAP, from September 2002 through August 2004, in Santiago, Chile. PCR results were compared with serology performed by indirect immunofluorescence (IFI). Specimens from 11 of 84 patients (13.1%) were positive for M. pneumoniae by any test. The IFI test was positive in 8 (72.7%) patients and PCR in 7 (63.6%) cases. The acute phase sera allowed diagnosis of M. pneumoniae in 5 of 11 patients (45.4%), 4 of them showing an IgM response. PCR was negative in 4 patients with positive serology and 3 patients were positive only by PCR. The two PCR primers showed 100% correlation, and a similar sensitivity; no inhibitory specimens for PCR were detected. In conclusion, M. pneumoniae should be considered as a potential etiologic agent of CAP in the elderly. Its detection must be performed by a combination of PCR and serology.  相似文献

BACKGROUND: The contribution of Chlamydia spp in respiratory tract infections in paediatric population from India has not been studied in detail. METHODS: Sixty children under five years of age who were admitted with acute lower respiratory tract infection during a one year period were investigated for Chlamydial aetiology of respiratory infection. Diagnosis was based on antigen detection by direct immunofluorescence (DIF) in throat swab along with anti-Chlamydial immunoglobulin G (IgG) antibody demonstration by solid phase enzyme immunoassay (EIA). RESULTS: Chlamydia spp antigen was detected in seven (11.6%) cases, C. pneumoniae in six (10%) and C. trachoniatis in one (1.6%). Chlamydia spp IgG antibody in serum was demonstrated in 24 (40%) cases, of which C. pneumoniae IgG was denconstrated in 18 (30%) cases. Taking the criteria of antigen detection (n=7) and high IgG antibody titre of > or = 1:512 (n=5) for a positive case, 12 (20%) children were found to be suffering from recent Chlamydial infection. CONCLUSION: Chlamydia spp plays a significant role in respiratory tract infections in Indian paediatric population. Diagnostic procedure like antigen detection in throat swab is rapid, less cumbersome and feasible and should be more widely used along with antibody demonstration to determine the aetiological agent early in the course of illness.  相似文献

In Thailand, the death rate from community-acquired pneumonia (CAP), especially severe CAP, has increased steadily over the past decade. To optimize the outcome, rapid start of appropriate antibiotics and supportive care are the mainstay of management. We therefore assessed the local etiology and outcome of adult patients with severe CAP admitted between January 1, 1999 and December 31, 2001. One hundred and five of 383 patients (27.4%) met the ATS criteria for severe CAP. The mean age was 56.9 (SD 18.2) years. The male to female ratio was 60:45. Duration of symptoms before admission was 5.3 (SD 4.0) days. Most of them (91.4%) had co-morbidity, diabetes mellitus being most common. A microbiological pathogen was isolated in 62 patients (59%). The pathogens most commonly isolated were B. pseudomallei (29.4%), S. pneumoniae (20.6%), K. pneumoniae (19.1%), and H. influenzae (11.8%). Other less common pathogens were E. coli (5.9%), S. aureus (5.9%), M. pneumoniae (1.5%), M. catarrhalis (1.5%), P. aeruginosa (1.5%), P. fluorescens (1.5%), and S. stercoralis (1.5%). Hospitalization averaged 14.7 (SD 14.3) days and mortality was 21%. Clinicals in 17.1 % of patients did not improve and they transferred home. Most (81.9%) patients required mechanical ventilation, while 60 (57.1%) developed septic shock, and 13 (12.3%) acute renal failure. Severe CAP carried high mortality, despite intensive care. Empirical therapy for B. pseudomallei should be considered, where endemic, and for patients with diabetes mellitus or chronic renal failure.  相似文献

In an ideal clinical setting, empiric antimicrobial treatment prescribed in adult community acquired pneumonia (CAP) should be based on national etiological surveillance and in vitro susceptibility assays. Available information about etiology in ambulatory patients and intensive care unit (ICU) patients is scarce, compared to information obtained in hospitalized patients. In studies designed to explore the etiology of pneumonia, no microorganism is detected in 40-50% of patients, a fact that represents limited yields in diagnostic methods. In all settings, Streptococcus pneumoniae is the main respiratory pathogen recovered in adults CAP, being responsible of about 16% of cases among ambulatory patients and about 22% of those admitted to hospital and ICU. About one third of cases are caused by a small group of microorganisms: Haemophilus influenzae, Mycoplasma pneumoniae, Chlamydia pneumoniae, respiratory viruses, Staphylococcus aureus, gramnegative bacillus, Legionella sp; each one is isolated in less than 10% of cases. In general, microorganism distribution varies scarcely in the following attending settings: ambulatory patients, common wards and ICU. An exception is represented by a higher frequency of gram negative bacillus, S. aureus and Legionella sp in ICU, and of C. pneumoniae in the ambulatory setting. In Chile, CAP etiology in hospitalized adult patients is similar to foreign reports; no systematic information has been collected about the etiology in neither ambulatory patients nor in severe CAP.  相似文献

Since community-acquired pneumonia (CAP) is a common disease with a high morbidity rate, it is important to obtain information concerning its etiology and susceptibility to antibiotics across different geographic areas. This study presents data obtained in 5 Latin American counties in the course of an international clinical trial that evaluated the efficacy and safety of treatment with either moxifloxacin or amoxicillin administered for 10 days to patients suspected of having CAP caused by a pneumococcal infection. Details are given of the pathogens identified, the patterns of sensitivity to antibiotics observed, and the clinical and microbiological results obtained.A total of 84 patients were studied, of whom 70 (83.3%) were evaluated at the end of the trial to determine the efficacy and safety of the treatment received. Gram-positive bacteria were found in samples from 29 patients (80.5%). The pathogen was Streptococcus pneumoniae in 28 of those cases (77.7%). Gram-negative bacteria were found in 7 patients (19.4%), the most common being Haemophilus influenzae in 3 patients (8.3%). The presence of atypical microorganisms was detected in 18 of the 70 patients (25%), mainly Mycobacterium pneumoniae (n=11), and in 6 cases (8.5%) the infection was mixed. Ten strains of S. pneumoniae (35.7%) were shown to be susceptible to penicillin, 2 (7.1%) were highly resistant, and 16 (57.1%) showed moderate resistance. The clinical success rate at the final visit after treatment was 94.1% for moxifloxacin and 91.7% for amoxicillin. The results of this trial demonstrate a high prevalence of S. pneumoniae with reduced susceptibility to penicillin in patients with CAP in Latin America. It also revealed a high incidence of atypical pathogens and mixed infection in 8.6% of patients. This information should be taken into account when establishing protocols for empirical treatment of CAP in Latin America.  相似文献

At the Kilimanjaro Christian Medical Centre, a tertiary referral hospital in northern Tanzania, both the number of paediatric cases of lower respiratory-tract infection (LRTI) and the associated mortality increased between 2000 and 2001. Molecular diagnostic tools were used to enhance the identification of the pathogens responsible for this perceived increase. All 72 children aged between 2 and 60 months who were admitted with LRTI over a 3-month period were enrolled in the study. Induced sputum was collected from each child and, if the parents consented, the subjects were also tested for HIV. The sputum samples were each checked for bacteria by culture and, in amplification assays, for the DNA of Pneumocystis jiroveci, Mycobacterium tuberculosis and Streptococcus pneumoniae. Twenty-two (50%) of the 44 children tested for HIV had HIV-1 antibodies. Although only two children, both aged <6 months, were found PCR-positive for P. jiroveci, and only one was found positive for M. tuberculosis, 46 (including one of those found positive for P. jiroveci and the child found positive for M. tuberculosis) were found PCR-positive for S. pneumoniae. It therefore appears that most paediatric cases of LRTI who present at the hospital are attributable to S. pneumoniae, and that infections with this pathogen are entirely responsible for the observed increase in the incidence of LTRI in the local children. The increase seen in LRTI-associated mortality among the children may be the result of pneumococcal antibiotic resistance.  相似文献

We applied a multiplex polymerase chain reaction (PCR) and culture to detect Streptococcus pneumoniae and detected 3 other respiratory pathogens--Haemophilus influenzae, Moraxella catarrhalis, and Alloiococcus otitidis--simultaneously by PCR, in the nasopharynx of 386 children aged under 5 y. S. pneumoniae was the most common pathogen carried by children in all age groups, with the rate ranging from 15.8% in children aged 3-4 y to 28.6% in children aged 2-3 y. H. influenzae and M. catarrhalis showed similar carriage rates across all the age groups. Only 2 young children (0.5%) carried A. otitidis. Higher carriage of S. pneumoniae was found in children who had not received the heptavalent pneumococcal conjugate vaccine (PCV7). Cefotaxime non-susceptibility was high (51.4%) in S. pneumoniae nasopharyngeal isolates. Serotype 6B was the most common in fully immunized carriers and also in those who received catch-up immunization. Due to low PCV7 coverage in Taiwan, the carriage of vaccine and non-vaccine serotypes of S. pneumoniae in children remains common.  相似文献

OBJECTIVE: The aim of the study was to evaluate the association between antibodies to Chlamydia pneumoniae and the onset of asthma in children. METHODOLOGY: In 1996-2000, 122 children aged 1-6 years, who were treated for new asthma as inpatients or outpatients in our hospital, were recruited. For each patient, two controls, matched by age, sex and municipality, were randomly selected from the same population. In 2000, 104 serum samples were available from patients (85%) and 120 from controls (49%) for microimmunofluorescence (MIF) assay for C. pneumoniae and C. trachomatis antibodies, and for enzyme immunoassay (EIA) for C. pneumoniae antibodies. RESULTS: In EIA, the median IgG concentrations were 20 EIU (EIA units) in the patients, and 16 EIU in the controls. IgG was positive (> 30 EIU) in 37 (36%) patients and in 36 (31%) controls. IgA was positive (> 12 EIU) in four (4%) patients and in eight (7%) controls. In MIF, four (4%) patients and seven (6%) controls were IgG positive, and seven were also IgA positive. IgM antibodies were detected in four children by EIA, and in none by MIF. CONCLUSION: IgG antibodies to C. pneumoniae, though common in 1 to 6-year-old children as detected by EIA, did not differ between newly diagnosed asthma patients and controls in this case-control study.  相似文献

OBJECTIVES: To assess any change in the microbiological profile of community-acquired pneumonia (CAP) in our region over the last 20 years. METHODS: We compared hospital admissions aged between 15 and 74 (n = 61) in Norfolk (UK) for CAP over a 19-month period in 1982-3 (ST1) with all admissions aged over 16 (n = 99) over a 14-month period in 1999-2000 (ST2). Data were collected for ST1 as part of a prospective multicentred research study, in a period of high Mycoplasma pneumoniae activity. ST2 was a prospective study of clinical practice. Chlamydophila species were differentiated in ST2 using whole-cell immunofluorescence. RESULTS: A microbiological diagnosis was made in 38 (62%) in ST1 compared with 48 (48%) in ST2. Streptococcus pneumoniae remained the most common pathogen (26% in ST1, 25% in ST2). The incidence of M. pneumoniae was 18% in ST1 and 4% in ST2. The proportion of viral pathogens identified was similar: nine (15%) in ST1 and 14 (14%) in ST2. No cases of Chlamydophila pneumoniae were diagnosed in ST2. CONCLUSIONS: The microbiological profile of CAP in Norfolk (UK) has not changed over the last 20 years and C. pneumoniae is not a frequent pathogen.  相似文献

We investigated the etiology of acute lower respiratory infection (ALRI) in children under 5 admitted to Srinagarind Hospital. The causative bacteria and viruses were determined by hemoculture and viral isolation from blood and nasopharyngeal aspirate samples. Antigens of respiratory syncytial virus (RSV) and Chlamydia trachomatis were detected using EIA. The 74 children less than 5 years of age with ALRI enrolled in our study were diagnosed with pneumonia (75.7%), croup (16.2%), and bronchiolitis (8.1%), respectively. Examination of blood or nasopharyngeal aspirate revealed viral or bacterial infections in 26 and 22 cases, respectively, whereas 5 of the children aged under 1 year (10%) were diagnosed with pneumonia caused by Chlamydia trachomatis. RSV was the most common virus detected (24.3%) and was associated with pneumonia and bronchiolitis, while the parainfluenza virus was the primary cause of croup. In cases of pneumonia, bacterial infections were identified in almost all of the cases: and Streptococcus pneumoniae and Haemophilus influenzae were the most commonly isolated (at 8.9% each). Mixed infections were detected in 8 cases (10.8%). The incidence of RSV infection peaked during the especially warm and cool seasons, whereas the bacterial infections were primarily associated with the relatively cool season. Our study indicates that a combined pneumococcal and Hib vaccine and a RSV vaccine would reduce the high rate of pneumonia in children under 5 years of age in Northeast Thailand.  相似文献

The prevalence of antibodies to viruses associated with respiratory illnesses (influenza, corona, R.S., adeno, parainfluenza Mycoplasma pneumoniae) has been investigated in 119 children under 3 years of age (Koulikoro region Mali) and 50 persons 15-19 years of age (Bamako/Mali). In the surveillance period (March 1982-September 1982) an outbreak of respiratory disease in association with RS virus took place. A rise of CF titer has been found in 90.8% of the children; 59.1% of them presented a fourfold or greater increase of titer. The GMT values rose from 1:7 to 1:48. Concerning influenza viruses a higher incidence of positive reactions has been observed only for H3N2 viruses. A fourfold CF rise of titer has been observed for adenovirus and mycoplasma pneumoniae in 3 cases each, for parainfluenza type I and III in 2 cases each, and for parainfluenza II in 1 case. In persons 15-19 years of age the incidence of positive CF reactions was relatively high (68%-98%), but the GMT values were moderate (1:12-1:25). These data confirm that the frequency of viral respiratory diseases is the same in tropical countries as in countries with a temperate climate.  相似文献

BACKGROUND: There has not been a comprehensive multi-center study investigating the microbial profile of community acquired pneumonia (CAP) in Taiwan. METHODS: A prospective study of adult CAP patients requiring hospitalization between December 2001 and April 2002 was carried out in 13 hospitals in Taiwan. Etiology was determined based on laboratory data from blood and sputum cultures plus serology from paired serum and urine antigen detection tests. RESULTS: Etiology was assigned to 99 (58.9%) of the 168 patients having the most complete data for etiology determination, with mixed infection in 21 (12.5%) patients. More than half (51.8%) of the patients were>60 years and 63.7% of the patients were males. The most common etiologic agent was Streptococcus pneumoniae (40, 23.8%), the majority (60%, 24 cases) of which was detected by positive urine antigen test. Other common agents included Mycoplasma pneumoniae (24, 14.3%), Chlamydia pneumoniae (12, 7.1%), Influenza A virus (11, 6.5%), Klebsiella pneumoniae (8, 4.8%) and Haemophilus influenzae (8, 4.8%). The prevalence of S. pneumoniae and M. pneumoniae was highest in patients>60 years (25/87, 28.7%), and<44 years (12/59, 19%), respectively; while K. pneumoniae comprised a larger proportion (4/22, 18%) in the 45-59 years group. CONCLUSIONS: S. pneumoniae was the most common etiology agent in adult patients hospitalized due to CAP in Taiwan and the spectrum of other major pathogens was similar to studies conducted elsewhere in the world. Empiric treatment recommendations developed in other parts of the world may be appropriately adapted for local use after taking into account local resistance profiles. Our data also support the recommendation that urine antigen test be added as an adjunct to adult CAP etiology diagnosis protocol.  相似文献
