背景:医用生物蛋白胶具有封闭组织创面、减少渗出、止血、促进伤口愈合和防止粘连等功能。 目的:系统评价医用生物蛋白胶在甲状腺手术中应用的效果。 方法:应用计算机检索中国期刊网全文数据库、万方数据库、维普数据库、中国生物医学数据库网络版 2000年1月至2013年4月公开发表的医用生物蛋白胶在甲状腺手术中应用的随机对照临床试验文献,对纳入的研究采用Rev Man 5.2 统计学软件进行Meta分析。 结果与结论:共纳入6项研究,合计797例患者。Meta分析结果显示,使用医用生物蛋白胶组术后第1天引流量[比值比为-27.36,95%可信区间(-33.86,-20.87),P < 0.000 01]和术后总引流管引流量[加权均数差为-38.73,95%可信区间(-44.78,-32.67),P < 0.000 01 ]低于未使用医用生物蛋白胶组,术后切口拆线时间短于未使用医用生物蛋白胶组[比值比为-2.00,95%可信区间(-2.17,-1.83),P < 0.000 01]。两组术后3 d发热[比值比为1.53,95%可信区间(0.59,3.96),P=0.38]、切口感染[比值比为0.86,95%可信区间(0.12,6.15),P=0.88]及术后血肿[比值比为0.86,95%可信区间(0.21,3.48),P=0.83]等并发症无差异。说明在甲状腺手术应用医用生物蛋白胶在不增加术后并发症的情况下,可明显减少引流液的量。中国组织工程研究杂志出版内容重点:生物材料;骨生物材料; 口腔生物材料; 纳米材料; 缓释材料; 材料相容性;组织工程全文链接:  相似文献

研究医用生物蛋白胶及其主体成分纤维蛋白原的潜在免疫原性。采用SDS-PAGE电泳、Western blot分离确定主体胶成分中的纤维蛋白原蛋白,建立间接ELISA法检测抗体条件;新西兰白兔创伤实验及BALB/c小鼠实验考察医用生物蛋白胶及纤维蛋白原的免疫原性。结果表明,生物蛋白胶主体成分以0.5μg/ml包被时,可有效检测抗纤维蛋白原抗体。新西兰白兔创伤实验中,实验组兔血清1周后出现抗体,2周后抗体水平明显升高,6周后抗体水平下降,呈先升后降的动态变化趋势;小鼠实验中,NBT试验未发现阳性中性粒细胞,MTT检测到T细胞呈现弱增殖能力。研究表明,医用生物蛋白胶主体胶成分中的纤维蛋白原在兔出现一过性抗体,医用生物蛋白胶对小鼠未表现出明显的免疫原性。  相似文献

背景:目前在翼状胬肉切除联合角膜缘上皮移植中,对于医用生物蛋白胶能否代替缝线固定自体球结膜植片还存在一定争议。 目的:采用Meta分析比较医用生物蛋白胶与缝线在翼状胬肉切除联合角膜缘上皮移植中应用的效果。 方法:查阅PubMed数据库、CNKI全文数据库、中国生物医学文献数据库、维普数据库和万方数据库,从建库初始至2015年6月比较医用生物蛋白胶与缝线在翼状胬肉切除联合角膜缘上皮移植中应用效果的随机对照试验,采用Meta分析两组手术时间、术后并发症发生率及复发率。 结果与结论:共纳入14篇随机对照试验,879例患者。Meta分析结果显示,纤维蛋白胶组手术时间、术后并发症发生率、术后6个月及1年的复发率均低于缝线组[MD=-15.58,95%CI(-18.96,-12.21),P < 0.000 01;OR= 0.39,95%CI(0.19,0.68),P=0.02;OR=0.28,95%CI(0.12,0.65),P=0.003];OR=0.23,95%CI(0.07,0.76),P=0.02]。表明翼状胬肉切除联合自体角膜缘干细胞移植中应用生物蛋白胶促进植片贴附,可缩短手术时间、降低术后并发症、减少翼状胬肉术后复发。 中国组织工程研究杂志出版内容重点:生物材料;骨生物材料; 口腔生物材料; 纳米材料; 缓释材料; 材料相容性;组织工程  相似文献

背景:生物蛋白胶广泛应用于各种外科手术中对富含小血管创面渗血的处理。 目的:通过在肝部分切除中应用组织可吸收性医用生物蛋白胶,分析其对手术时间及肝部分切除后止血效果的影响。 方法:对42例肝部分切除患者随机等分为实验组和对照组。实验组在肝部分切除后,缺损处喷洒医用生物蛋白胶封闭,并以明胶海绵剪成多块小片段混合医用生物蛋白胶以覆盖断面来代替常规缝合断面。对照组仅缝合收拢肝组织缺损。 结果与结论:实验组使用的生物蛋白胶可迅速附着于创面,实验组患者肝部分切除的手术时间、出血量、引流量及渗血持续时间均明显小于对照组(P < 0.05),且2组患者的住院天数和住院费用的差异无显著性意义(P > 0.05)。提示在肝部分切除中应用医用生物蛋白胶可以在不增加患者费用及住院时间的同时,有效降低手术时间、减少肝部分切除后出血。 关键词:生物蛋白胶;可吸收性材料;明胶海绵;肝部分切除;生物材料 doi:10.3969/j.issn.1673-8225.2012.12.021  相似文献

背景:纤维蛋白胶是一种生物蛋白制剂,由纤维蛋白原、凝血酶及其他生物活性因子组成,具有止血、趋化及促细胞有丝分裂等特性,有着良好的生物相容性。 目的:对纤维蛋白胶在骨科的应用进行综述。 方法:以“fibrin  glue, biomaterial,application” 为英文检索词,以“纤维蛋白胶,骨科,应用” 为中文检索词,检索PubMed、BP、SD、万方及维普等数据库1995-01/2011-05相关文献,选取纤维蛋白胶在骨科中的应用有关文献30篇进行总结。 结果与结论:纤维蛋白胶是一种具有多向生物活性的生物蛋白,能迅速形成立体网状结构,起到良好的止血作用。在注射部位局部形成凝胶圈,起封闭与粘合作用。具有预防脊柱手术后硬脊膜外腔粘连,抑制局部炎症扩散,减轻水肿压迫,促进术后切口愈合的作用。能充当生物基质,应用于骨或软骨组织工程。关键词:纤维蛋白胶;骨科;应用;生物相容性;综述文献 缩略语注释:BMP:bonemorphogeneticprotein,骨形态发生蛋白 doi:10.3969/j.issn.1673-8225.2012.16.036  相似文献

The purpose of the study was to evaluate the hemostatic effectiveness of fibrin glue (FG) prepared by a modification of cryoprecipitation technique in experimental rat liver surgery. FG component 1 was prepared by triple or 'recycled' cryoprecipitation method from single-donor plasma. Rats subjected to liver incision, partial and total lobectomy were treated with FG on the surgical cut surface or underwent standard surgical technique. The efficacy of FG-treatment was evaluated on the basis of the 24-hour survival ratio and peripheral blood hematological parameters. The mean values of fibrinogen, FXIII, fibronectin and horizontal tensile strength of FG were 54.2 +/- 19.9 g/l, 13.5 +/- 3.6 IU/ml, 3103.1 +/- 148.9 mg/l, and 1.076 +/- 0.18 N/cm2, respectively. The survival of FG-treated rats subjected to partial and total lobectomy was significantly higher in comparison to the FG-nontreated animals, accompanied with higher values of red blood cell counts, hemoglobin concentration and hematocrit. When liver incision was performed, although there were no differences in survival rate, FG-treated animals had significantly higher values of the tested hematological parameters. The presented results demonstrated that by using 'recycled' cryoprecipitation it is possible to obtain high quality single-donor FG with successful hemostatic therapeutical effects, as confirmed in the experimental rat model of liver surgery.  相似文献

Blood-derived biomaterials include fibrin sealant (FS) (also called fibrin glue), platelet gel (PG), and platelet fibrin glue. They are used in many surgical fields because of their functional properties and unique physical advantages compared to synthetic products. FS can be made industrially by the fractionation of large plasma pools, or from single plasma donations. Thanks to a high content in fibrinogen, FS exhibits, after activation by thrombin and formation of a strong fibrin clot, tissue sealing and haemostatic properties. PG and platelet fibrin glue are made from single blood donations (platelet concentrates combined or not with cryoprecipitate). Owing to their richness in platelet, PG and PFG can release, upon thrombin activation, a myriad of growth factors that can stimulate cell growth and differentiation, generating much interest for hard and soft tissues regeneration and healing, as well as, increasingly, cell therapy protocols to replace fetal bovine serum. Blood-derived biomaterials have the advantages, over synthetic glues and other biomaterials, of being physiologically compatible with human tissues, and of not inducing tissue necrosis or other tissue reactions. They can be readily colonized by cells and are totally biodegradable in a matter of days to weeks. These blood-derived biomaterials are used increasingly as tissue engineering tools, allowing surgeons to influence and improve the in vitro or in vivo cellular environment to enhance the success of tissue grafting. We review here the three main types of biomaterials that can be made from human blood and describe their biochemical and physiological properties as well as their clinical applications.  相似文献

Fibrin glue (FG) is used in a variety of clinical applications and in the laboratory for localized and sustained release of factors potentially important for tissue engineering. However, the effect of different fibrinogen concentrations on FG scaffold delivery of bioactive adeno-associated viruses (AAVs) has not been established. This study was performed to test the hypothesis that FG concentration alters AAV release profiles, which affect AAV bioavailability. Gene transfer efficiency of AAV-GFP released from FG was measured using HEK-293 cells. Bioactivity of AAV transforming growth factor-beta1 (TGF-β(1)) released from FG was assessed using the mink lung cell assay, and by measuring induction of cartilage-specific gene expression in human mesenchymal stem cells (hMSCs). Nondiluted FG had longer clotting times, smaller pore sizes, thicker fibers, and slower dissolution rate, resulting in reduced release of AAV. AAV release and gene transfer efficiency was higher with 25% and 50% FG than with the 75% and 100% FG. AAV-TGF-β(1) released from dilute-FG transduced hMSCs, resulting in higher concentrations of bioactive TGF-β(1) and greater upregulation of cartilage-specific gene expression compared with hMSC from undiluted FG. This study, showing improved release, transduction efficiency, and chondrogenic effect on hMSC of bioactive AAV-TGF-β(1) released from diluted FG, provides information important to optimization of this clinically available scaffold for therapeutic gene delivery, both in cartilage regeneration and for other tissue engineering applications.  相似文献

Most research done on hypothyroidism has focused on physiological and biochemical aspects of the tissues, whilst there has been little work on tissue morphology especially on salivary glands. The present study has used hypothyroid Wistar rats as a model for investigating the effects of hypothyroidism on submandibular gland structure. Two groups of Wistar rats were studied. One was made hypothyroid with methimazole and the second was an untreated euthyroid group (control). They were euthanised after 10 weeks. Submandibular glands were removed and studied. Systematic random transverse sections were obtained from submandibular glands and subjected to morphometric analysis. Volume density and absolute volume of granular, striated and excretory ducts and interlobular connective tissue were estimated by point counting. Volume-weighted mean particle volume of serous and mucous acini was also determined by unbiased stereology. Absolute volume of granular ducts in the submandibular gland of hypothyroid rats was reduced by approximately 50% (p<0.009) whilst that of the striated and excretory ducts and interlobular connective tissue was unaffected. Volume of serous acini was also significantly (p<0.03) lower in hypothyroid rats. These changes suggest that hypothyroidism has an effect on submandibular gland structure, and that this effect occurs mostly in two major exocrine compartments (granular duct and serous acinus) of the gland.  相似文献
