Purpose: To determine the frequency of hypointense appearance of dural sinus thrombosis on T2-weighted images, which may mimic a normal flow void, and when possible correlate with appearance on T1-weighted images. Methods and Materials: Retrospective review of radiology files showed 51 patients with a discharge diagnosis of dural sinus thrombosis who underwent MR imaging during the period 1986-1998. These images were reviewed by an experienced neuroradiologist for appearance on T2-weighted images. This process yielded five cases in which a hypointense appearance on this pulse sequence simulated a normal flow void. An additional two cases were added from the teaching files of two other institutions giving a total of seven cases (13% of studies). The resulting study population consisted of five women and two men (mean age 27.1 years). T1 weighted images were available in five patients. In two patients MR venography was available, but not T1-weighted images. The diagnosis of dural sinus thrombosis was based solely on absence of flow void on T1-weighted images in one case, solely on absence of flow void on MR venography in two cases and absence of flow void on T1-weighted images in conjunction with MR venography or gradient echo findings in 4 patients. All images were obtained on a 1.5 T magnet (GE Medical Systems; Milwaukee, Wisconsin). Results: In all patients hypointense signal of thrombus was isointense with normal flow voids in other dural sinuses on T2-weighted images. In all cases in which T1-weighted images were available, the signal intensity of thrombus was isointense to gray matter. Conclusion: Hypointense appearance of thrombus on T2-weighted images is a potential pitfall in the MR diagnosis of dural sinus thrombosis. Because thrombus in this stage of evolution appears isointense to gray matter on T1-weighted images, careful attention must be paid to other sequences to avoid this pitfall.  相似文献

Intraperitoneal gelatin sponge can mimic a mass lesion on magnetic resonance (MR) images. To determine the MR imaging characteristics of gelatin sponge over time, a 15 x 10 x 4-mm piece of gelatin sponge soaked in saline was surgically implanted in the peritoneal cavity of 14 mice. Two mice underwent a sham operation. Contiguous axial spin-echo images of the abdomen were obtained with T1-weighted, spin-density, and T2-weighted sequences preoperatively and over a 6-week period postoperatively. Gelatin sponge initially appears as a heterogeneous mass of low signal intensity on T1-weighted images and increasing intensity on spin-density and T2-weighted images, containing multiple round foci of very low signal intensity, attributable to air, at all sequences. Over time, signal intensity further increases and becomes more homogeneous on spin-density and T2-weighted images, although foci of air persist to 3 weeks. By 2-4 weeks, the mass is no longer discrete. Foci of air should help differentiate gelatin sponge from tumor and add gelatin sponge to the differential diagnosis of abscess.  相似文献

Angioleiomyomas are smooth muscle tumors that occur most often in the extremities and are usually less than 2 cm in diameter. We present 2 case reports of giant angioleiomyoma. Case 1 was a 51-year-old woman with a 5-year history of a slowly growing mass 12x10 cm in diameter in the right posterior thigh. The tumor was larger than any previously reported. Case 2 was a 72-year-old man with an 8-year history of a slowly growing mass 6x6 cm in diameter in the left palm. The tumor was unique in that it grew to a giant size in the hand of the elderly patient. Magnetic resonance imaging of both cases confirmed the tumors' presence in the subcutis. T1-weighted images showed homogenous signal isointense to muscle. On T2-weighted images, areas of hyperintensity and hypointensity intermingled. Three-dimensional gadolinium-enhanced magnetic resonance angiography showed rapid initial enhancement followed by a gradual increase in enhancement, suggesting that the tumors were rich in vessels. Microscopic examination revealed numerous vascular channels embedded within a huge smooth muscle component.  相似文献

Colorectal mucinous carcinoma: findings on MRI   总被引:3,自引:0,他引:3  
PURPOSE: The purpose of this work was to define the characteristic MR features of colorectal mucinous carcinomas and to correlate the mucin pool with the signal intensity of this tumor. METHOD: MRI of 12 cases of pathologically proven colorectal carcinoma containing mucin was evaluated. We analyzed the signal intensity of tumor on T1- and T2-weighted MR images and correlated the area of intratumoral high signal intensity on T2-weighted images with the mucinous pool on the pathologic specimens. Two radiologists independently estimated the area of high signal intensity in the tumor on T2-weighted images and one pathologist estimated the amount of mucinous pool in the pathologic specimen. RESULTS: In 9 (75%) of 12 cases, focal or diffuse high signal intensity areas were detected on T2-weighted fast spin echo images. In seven cases in which mucin pools were seen macroscopically, partial (n = 3) or diffuse high signal intensity areas were noted on the T2-weighted images. Among the five cases in which microscopic mucinous pools were detected on the pathologic slides, three cases showed no high signal foci on MR images, and in the remaining two cases, high signal intensity areas were noted as small foci. CONCLUSION: Intratumoral high signal intensity on T2-weighted fast spin echo MR images occurs in mucinous carcinomas and correlates with the mucin pools on pathologic specimens.  相似文献

PURPOSE: To investigate the relationship between the degree of contrast enhancement in fluid-attenuated inversion recovery (FLAIR) sequences and tumor signal intensity on T2-weighted images. MATERIALS AND METHODS: A total of 96 patients suspected of having brain tumors were examined by MR imaging, and whenever a brain tumor with an enhancing part larger than the slice thickness was demonstrated on postcontrast T1-weighted images, postcontrast FLAIR images were additionally acquired. The tumor signal intensity on the T2-weighted images was visually classified as follows: equal or lower compared with normal cerebral cortex (group 1), higher than normal cortex (group 2), and as high as cerebrospinal fluid (CSF) (group 3). When a lesion contained several parts with different signal intensities on T2-weighted images, we assessed each part separately. In each group, we visually compared pre- and postcontrast FLAIR images and assessed whether tumor contrast enhancement was present. When contrast enhancement was present on FLAIR sequence, the degree of contrast enhancement in T1-weighted and FLAIR sequences was visually compared. RESULTS: Postcontrast T1-weighted images showed 46 enhancing lesions, including 48 parts, in 31 MR examinations. FLAIR images of the lesion-parts in group 1 (N=18) did not show significant contrast enhancement. In group 2 (N=12), all the parts were enhanced in FLAIR sequences, and three parts were enhanced more clearly in the FLAIR sequences than in the T1-weighted sequences. In group 3 (N=18), all the parts were enhanced equally or more clearly in the FLAIR sequences than in the T1-weighted sequences. CONCLUSION: The signal intensity in FLAIR sequences is largely influenced by both T1 and T2 relaxation time; there is a close relationship between the signal intensity of brain tumors on T2-weighted images and the degree of contrast enhancement on FLAIR sequences. When tumors have higher signal intensity than normal cortex on T2-weighted images, additional postcontrast FLAIR imaging may improve their depiction.  相似文献

To evaluate the ability of spin-echo MRI to differentiate between slow flow and mural thrombus in aortic diseases, we reviewed the spin-echo MRI of 10 patients with intraaortic thrombus that had been documented by CT in 8 patients and aortography in 2 patients. Six patients had aortic aneurysms with mural thrombi, and four had aortic dissections with adherent mural thrombi within the false lumen. Five of seven gated oblique sagittal or coronal T1-weighted studies demonstrated hyperintense slow flow signal within the residual lumen. This hyperintense slow flow signal was accompanied by a parallel hypointense rim due to a boundary layer dephasing phenomenon. Eight axial T2-weighted MR studies demonstrated a hypointense zone due to fresh clot at the edge of a mural thrombus on even-echo images consistently. The hypointense boundary layer between slow flow and mural thrombus on either gated T1-weighted MRI or second-echo T2-weighted MRI not only predicted the presence of flow within the residual lumen but also clearly separated the area of slow flow from that of mural thrombus in 9 of 10 patients. Proper interpretation of spin-echo images may obviate the need for phase display imaging or gradient-echo imaging in differentiating between slow flow and mural thrombus.  相似文献

Three patients with surgically inaccessible giant carotid aneurysms/pseudoaneurysms and one patient with carotid cavernous fistula had endovascular occlusion with detachable silicone balloons filled with Cholografin. MR was performed before the procedures in three cases and again 18 hr to 44 days after embolization in all four cases. The age-related changes of arterial thrombi, as well as the optimal timing and value of different pulse sequences in the noninvasive follow-up, were evaluated. Arterial thrombi have some characteristics in common with intracerebral hematomas, being isointense on T1-weighted spin-echo images during acute phase and subsequently acquiring hyperintense signals on both T1- and T2-weighted spin-echo images during the subacute and chronic phases. Additional observations are that (1) hyperacute (less than 24 hr old) thrombus is hyperintense on T2-weighted spin-echo sequences; (2) hemosiderin is less conspicuous in chronic intraluminal thrombi than in intracerebral hematomas of comparable size; and (3) thrombosis is initiated at a site remote from the apex of the aneurysm and then progresses centripetally. The Cholografin-filled balloon is hypointense to gray matter on T1-weighted spin-echo images and isointense to both hyperacute and chronic thrombus on T2-weighted spin-echo images. The optimal timing and sequence for MR follow-up of a thrombosed aneurysm with conventional spin-echo technique is beyond 7 days on T1-weighted spin-echo images. The in vivo appearance of Cholografin-filled silicone balloons does not change appreciably on T1- and T2-weighted spin-echo sequences up to 6 weeks if filled according to the manufacturer's specification.  相似文献

In a prospective study, MR images were evaluated in seven patients with femoropopliteal venous thrombosis with symptoms of less than 5 days duration. T1-weighted (600/25 [TR/TE]), intermediate (2000/30), and T2-weighted (2000/100) spin-echo series and a gradient-recalled acquisition in the steady state (GRASS) series were compared. Using venography as the standard for diagnosis, we found GRASS to be the most sensitive of the MR techniques, showing thrombi in all patients. It provided good contrast between the low-intensity thrombus and high-intensity flowing blood and also between thrombus and intermediate- or high-intensity perivascular tissues. The T1-weighted series was the least sensitive technique. All thrombi showed heterogeneity in the transaxial image with differences in signal between the peripheral and central regions. A higher intensity signal in the center than in the periphery at some level of the thrombus was found in six of seven T2-weighted or GRASS images. Heterogeneity in the signal intensity was more frequent in distal portions of thrombi, whereas the most proximal extent was homogeneous in appearance in six of seven cases. The heterogeneous appearance may be related to the greater age of the distal thrombus, because deep venous thrombi are known to begin in the calf and extend proximally over time. We conclude, on the basis of our experience with a small number of patients, that the GRASS MR technique is more sensitive for detecting acute deep venous thrombosis than T1-weighted, intermediate, and T2-weighted MR images.  相似文献

PURPOSE: To prospectively evaluate use of diffusion-weighted (DW) magnetic resonance (MR) images and apparent diffusion coefficient (ADC) maps for determination of the consistency of macroadenomas. MATERIALS AND METHODS: The study protocol was approved by the institutional ethics committee, and informed consent was obtained from all patients. Twenty-two patients with pituitary macroadenoma (10 men, 12 women; mean age, 54 years +/- 17.09 [standard deviation]; range, 21-75 years) were examined. All patients underwent MR examination, which included T1-weighted spin-echo and T2-weighted turbo spin-echo DW imaging with ADC mapping and contrast material-enhanced T1-weighted spin-echo imaging. Regions of interest (ROIs) were drawn in the macroadenomas and in normal white matter on DW images, ADC maps, and conventional MR images. Consistency of macroadenomas was evaluated at surgery and was classified as soft, intermediate, or hard. Histologic examination was performed on surgical specimens of macroadenomas. Mean ADC values, signal intensity (SI) ratios of tumor to white matter within ROIs on conventional and DW MR images, and degree of enhancement were compared with tumor consistency and with percentage of collagen content at histologic examination by using analysis of variance for linear trend. RESULTS: The mean value of ADC in the soft group was (0.663 +/- 0.109) x 10(-3) mm(2)/sec; in the intermediate group, (0.842 +/- 0.081) x 10(-3) mm(2)/sec; and in the hard group, (1.363 +/- 0.259) x 10(-3) mm(2)/sec. Statistical analysis revealed a significant correlation between tumor consistency and ADC values, DW image SI ratios, T2-weighted image SI ratios, and percentage of collagen content (P < .001, analysis of variance). No other statistically significant correlations were found. CONCLUSION: Findings in this study suggest that DW MR images with ADC maps can provide information about the consistency of macroadenomas.  相似文献

Solitary fibrous tumor of the pleura: MR appearance and enhancement pattern   总被引:7,自引:0,他引:7  
PURPOSE: The aim of the current study was to characterize the MR appearance of solitary fibrous tumor of the pleura (SFTP). METHOD: Twenty-two consecutive patients with histologically confirmed SFTP were retrospectively evaluated with MRI. RESULTS: Tumors demonstrated low signal intensity (n = 18, 82%), iso signal intensity (n = 2, 9%), and high signal intensity (n = 2, 9%) on T1-weighted images. On T2-weighted images, tumors were observed as low signal intensity (n = 10, 45%), high signal intensity (n = 2, 9%), and mixed signal intensity (n = 10, 45%). Solid component in the proliferation of tumor cells corresponded to low signal intensity in 12 tumors (55%) on T2-weighted images. On gadolinium-enhanced T1-weighted images, inhomogeneous enhancement (n = 18, 82%), nodular enhancement (n = 6, 27%), and homogeneous enhancement (n = 4, 18%) were observed. The enhanced lesion within the tumors had dense tumor tissue and dilated microvessels in the pathologic specimen. CONCLUSION: SFTP shows variable appearance and enhancement pattern on MRI according to morphologic tumor heterogeneity.  相似文献

OBJECTIVE: We wanted to evaluate the role of diffusion-weighted imaging (DWI) and the apparent diffusion coefficient (ADC) for detecting breast tumors, as compared with the T1- and T2-weighted images. MATERIALS AND METHODS: Forty-one female patients underwent breast MRI, and this included the T1-, T2-, DWI and dynamic contrast-enhanced images. Sixty-five enhancing lesions were detected on the dynamic contrast-enhanced images and we used this as a reference image for detecting tumor. Fifty-six breast lesions were detected on DWI and the histological diagnoses were as follows: 43 invasive ductal carcinomas, one mucinous carcinoma, one mixed infiltrative and mucinous carcinoma, seven ductal carcinomas in situ (DCIS), and four benign tumors. First, we compared the detectability of breast lesions on DWI with that of the T1- and T2-weighted images. We then compared the ADCs of the malignant and benign breast lesions to the ADCs of the normal fibroglandular tissue. RESULTS: Fifty-six lesions were detected via DWI (detectability of 86.2%). The detectabilities of breast lesions on the T1- and T2-weighted imaging were 61.5% (40/65) and 75.4% (49/65), respectively. The mean ADCs of the invasive ductal carcinoma (0.89+/-0.18 x 10(-3)mm(2)/second) and DCIS (1.17+/-0.18 x 10(-3)mm(2)/ second) are significantly lower than those of the benign lesions (1.41+/-0.56 x 10(-3)mm(2)/second) and the normal fibroglandular tissue (1.51+/-0.29 x 10(-3)mm(2)/ second). CONCLUSION: DWI has a high sensitivity for detecting breast tumors, and especially for detecting malignant breast tumors. DWI was an effective imaging technique for detecting breast lesions, as compared to using the T1- and T2-weighted images.  相似文献

A case of magnetic resonance imaging of adrenocortical carcinoma extending into the inferior vena cava is presented. Magnetic resonance was able to outline the intraluminal extent of tumor thrombus accurately. Signal intensity ratios (mean 0.98 +/- 0.02) of tumor thrombus/primary tumor on T1- and T2-weighted sequences were useful for tissue characterization of tumor thrombus. Time course of the signal intensity of thrombus before and after intravenous administration of 0.1 mmol/kg gadolinium-diethylenetriamine pentaacetic acid was identical to the primary tumor which aided in differentiation from nontumor thrombus.  相似文献

The value of dynamic-contrast enhanced MR imaging using FLASH technique was studied in 13 patients with postoperative recurrent cervical cancer verified histopathology and in 9 patients without recurrence. Dynamic FLASH imaging and conventional spin-echo T1- and T2-weighted sequences were compared in a prospective fashion with regard to accuracy of the diagnosis of recurrent tumor using biopsy results as the gold standard. The contrast between the recurrent tumor and the surrounding pelvic tissues was also analyzed. The accuracy of depicting recurrent tumor on dynamic images (82%) was superior to that of pre- and postcontrast T1-weighted images and T2-weighted images (64%, 68%, and 64%, respectively). The contrast between the recurrent tumor and pelvic fat was greater on precontrast T1-weighted and dynamic images than on T2-weighted and postcontrast T1-weighted images. The dynamic images clearly showed involvement of the surrounding pelvic organs, because enhancement was observed exclusively in the tumor in the early dynamic phase. Accuracy regarding involvement of the urinary bladder or rectal wall on pre- and postcontrast T1-weighted images and T2-weighted images was lower than that on the dynamic images. Dynamic MR imaging has potential for use in the detection and evaluation of the extent of recurrent postoperative cervical cancer.  相似文献

Hemangioblastoma of the conus medullaris with MRI has not been reported before. A 75-year-old man had a history of falling due to weakness of his left lower limb. MRI revealed a well-defined oval mass in the conus medullaris. The tumor had an isointense signal relative to spinal cord on T1-weighted images, hyperintense signal areas intermixed with punctate spots of hypointensity on T2-weighted images, and heterogeneous obvious enhancement on gadolinium-enhanced T1-weighted images. Associated abnormally tortuous vessels were noted in the dura proximal to the tumor. Histological findings were compatible with the diagnosis of hemangioblastoma. Hemangioblastoma should be included in the differential diagnosis in patients with an enhancing tumor and adjacent engorged vessels of the spinal cord.  相似文献

We report here on a case of solitary fibrous tumor of the retroperitoneum, and the tumor displayed a predominantly myxoid histology. A 56-year-old man presented with an incidentally detected retroperitoneal mass. On the MR images, the mass was observed as having iso-signal intensity on the T1-weighted images and high signal intensity on the fat-saturated T2-weighted images. The mass showed intense enhancement on the Gd-DTPA enhanced T1-weighted images. At surgery, a well-defined solid mass was found in the left retroperitoneum. The histological diagnosis was made as solitary fibrous tumor with a predominantly myxoid histology.  相似文献

OBJECTIVE: Tumor thickness in oral tongue cancer is an important independent prognostic factor for local recurrence, nodal metastasis, and patient survival. An accurate preoperative assessment of tumor thickness is therefore essential for optimal treatment planning. The aim of our study was to evaluate the accuracy of MRI findings for the preoperative measurement of tumor thickness. SUBJECTS AND METHODS. Eighteen patients with oral tongue cancer underwent preoperative MRI of the tongue. After surgery, the glossectomy specimens were serially sectioned. The radiologic tumor thickness of contrast-enhanced T1-weighted and T2-weighted images was compared with the histologic tumor thickness using our proposed tumor thickness staging classifications. These included stage I (tumor < or = 3 mm), stage II (> 3 mm but < or = 9 mm) and stage III (> 9 mm). RESULTS: The overall accuracy in assessment of proposed tumor thickness staging using contrast-enhanced T1-weighted and T2-weighted images was 83% and 56%, respectively. The radiologic tumor thickness as measured on contrast-enhanced T1-weighted and T2-weighted images had significant correlation with histologic tumor thickness (R = 0.938 and 0.941, respectively). CONCLUSION: MR images provide satisfactory accuracy for the measurement of tumor thickness and staging of oral tongue cancer. Preoperative MRI is recommended to assist in treatment planning for patients with this disease.  相似文献

The purpose of this study was to evaluate the spectrum of appearances of gastrointestinal carcinoid tumors at magnetic resonance imaging (MRI) and to elucidate patterns of appearances of carcinoid liver metastases on precontrast and postgadolinium images. The MR examinations of 29 patients (11 men, 18 women; age range, 33-87 years) with histologically confirmed gastrointestinal carcinoid tumors, representing our complete 9.5 years of experience with this entity, were retrospectively reviewed. Twelve patients had MR examinations prior to resection or biopsy of the primary tumor (preoperative group); 17 patients were imaged postsurgically (postoperative group). All MR studies were performed at 1.5 T and comprised T1-weighted spoiled gradient echo (SGE), T2-weighted fat-suppressed turbo spin echo, HASTE, and serial postgadolinium T1-weighted SGE sequences without and with fat suppression. Morphology, signal intensity, and contrast enhancement of primary tumors and of metastases to the mesentery, peritoneum, and liver were evaluated. Primary tumors were visualized in 8 of 12 patients and best demonstrated on postgadolinium T1-weighted fat-suppressed images. The appearance of primary tumors was a nodular mass originating from the bowel wall (4 of 12 patients) or regional uniform bowel wall thickening (4 of 12 patients) with moderate intense enhancement on postgadolinium images. In 4 of 12 patients the primary tumor was prospectively not seen. Mesenteric metastases, seen in eight patients, presented as nodular masses and were associated with mesenteric stranding in seven patients. A total of 156 liver metastases were evaluated in 16 patients. On precontrast T1- and T2-weighted images, 117 metastases (75%) were hypointense and hyperintense, respectively. A total of 146 metastases (94%) were hypervascular, showing moderate intense enhancement during the hepatic arterial phase, and 9 metastases (6%) were hypovascular. Twenty-three metastases (15%) were visible only on immediate postgadolinium images. MRI is able to demonstrate findings in carcinoid tumors, including the primary tumor, mesenteric metastases, and liver metastases. Liver metastases are commonly hypervascular and may be demonstrable only on immediate postgadolinium images.  相似文献

A 41-year-old man presented with an asymptomatic mass in the right medial thigh. Magnetic resonance imaging (MRI) revealed a well-demarcated, 10-cm mass in the right adductor muscles. The margins of the mass exhibited high signal intensity and the rest showed low or iso signal intensity on T1-weighted MR images. However, the high signal intensity was decreased on T2-weighted images with fat suppression. The central part of the tumor was of inhomogeneous high signal intensity on T2-weighted images; after Gd-DTPA injection it enhanced inhomogeneously on T1-weighted images with fat suppression. On dynamic computed tomography (CT) in the arterial phase, there were strongly enhancing spotty areas in the tumor. At surgery, a yellow-whitish tumor was resected and a pathological diagnosis of angiomyolipoma (AML) in the thigh was made. This study was devoted to tumor differentiation in liver MR T1-weighted imaging with superparamagnetic iron oxide (SPIO). Twenty-one patients with 40 liver lesions were studied at 1.5 T. Before and at least 45 minutes after SPIO administration, turbo-field-echo (TFE) T1-weighted, TFE T1 × T2*-weighted (MXT), and fat-suppressed turbo-spin-echo T2-weighted images were acquired. A quantitative analysis was performed blindly. On TFE T1-weighted images, the signal enhancement was ?33% ± 12 for the liver, ?24% ± 2 for adenomas and focal nodular hyperplasia, +60% ± 33 for the hemangiomas; metastases and cyst enhancement were not significant. After SPIO on TFE T1-weighted images, the hemangioma-to-liver signal ratio (149% ± 18) was definitely higher than the mean metastasis-to-liver signal ratio (90% ± 16). This T1-related differentiation ability lacked dramatically on TFE MXT images and, in one case, was reduced on post-SPIO TFE T1-weighted images by a long imaging delay after SPIO administration (2 hours).  相似文献
