OBJECTIVES: Exposure to the radioactive daughters of radon is associated with increased risk of lung cancer in mining populations. An investigation of incidence of lung cancer following a clinical survey of Ontario uranium miners was undertaken to explore whether risk associated with radon is modified by factors including smoking, radiographic silicosis, clinical symptoms, the results of lung function testing, and the temporal pattern of radon exposure. METHODS: Miners were examined in 1974 by a respiratory questionnaire, tests of lung function, and chest radiography. A random selection of 733 (75%) of the original 973 participants was followed up by linkage to the Ontario Mortality and Cancer Registries. RESULTS: Incidence of lung cancer was increased threefold. Risk of lung cancer among miners who had stopped smoking was half that of men who continued to smoke. There was no interaction between smoking and radon exposure. Men with lung function test results consistent with airways obstruction had an increased risk of lung cancer, even after adjustment for cigarette smoking. There was no association between radiographic silicosis and risk of lung cancer. Lung cancer was associated with exposures to radon daughters accumulated in a time window four to 14 years before diagnosis, but there was little association with exposures incurred earlier than 14 years before diagnosis. Among the men diagnosed with lung cancer, the mean and median dose rates were 2.6 working level months (WLM) a year and 1.8 WLM/year in the four to 14 year exposure window. CONCLUSIONS: Risk of lung cancer associated with radon is modified by dose and time from exposure. Risk can be substantially decreased by stopping smoking.  相似文献

The reliability of exposure data directly affects the reliability of the risk estimates derived from epidemiological studies. Measurement uncertainty must be known and understood before it can be corrected. The literature on occupational exposure to radon ((222)Rn) and its decay products reveals only a few epidemiological studies in which uncertainty has been accounted for explicitly. This work examined the sources, nature, distribution and magnitude of uncertainty of the exposure of French uranium miners to radon ((222)Rn) and its decay products. We estimated the total size of uncertainty for this exposure with the root sum square (RSS) method, which may be an alternative when repeated measures are not available. As a result, we identified six main sources of uncertainty. The total size of the uncertainty decreased from about 47% in the period 1956-1974 to 10% after 1982, illustrating the improvement in the radiological monitoring system over time.  相似文献

Lung cancer among Dutch coal miners: a case-control study   总被引:5,自引:0,他引:5  
A case-control study was conducted in the southern part of The Netherlands to investigate the risk of lung cancer in coal miners; 381 age-matched pairs of primary lung cancer cases and controls, diagnosed between 1972 and 1988, were selected from the pathology department of the University Hospital in the region. Information about past employment in coal mines was obtained through the registers of the collaborative pension fund for Dutch miners. 20% of the cases were (at some time) employed in coal mining, compared with 21% of the controls (odds ratio 0.95; 95% confidence interval: 0.65-1.38). 9% of both cases and controls had an underground work history (odds ratio 0.96; 95% confidence interval: 0.56-1.65). The duration of underground coal mining did not differ substantially between cases and referents (average duration: respectively 117 and 108 months). No relation between specific histologic tumor cell types and coal mining could be demonstrated. The study gives no indication that workers in Dutch coal mines have an increased risk of developing lung malignancies.  相似文献

A case-control study was conducted to describe lung cancer risk in a cohort of New Mexico underground U miners. The subjects included 65 cases and 230 age-matched controls, most with exposures below 3.50 J h m-3 (1000 WLM). The risk for lung cancer was increased for all cumulative exposures to Rn progeny of 0.35 J h m-3 (100 WLM) or greater. The odds ratios were unchanged with control for cigarette smoking. With exclusion of subjects with exposures above 3.50 J h m-3 (1000 WLM), the estimated excess relative risk was 0.3% per mJ h m-3 (1.1% per WLM). The risk was greater for younger subjects and the data were consistent with a multiplicative interaction between cigarette smoking and exposure to Rn progeny.  相似文献

Studies of miners provide the basis for public health efforts to reduce residential radon progeny exposure. Because the preponderance of households do not have members who smoke indoors, studies of non-smoking miners contribute essential data for risk assessments for residential radon progeny exposure. We studied a cohort of 2,209 never-smokers who were underground uranium miners employed in the western U.S. from 1956 to the early 1990's and who participated in a screening program for lung cancer conducted by Saccomanno and colleagues. After determining the vital status and cause of death in the cohort, we conducted a nested case-control study of 55 lung cancer deaths in males and 3 age-matched controls for each case. The relative risk of lung cancer was 29.2 (95% CI 5.1, 167.2) for miners with greater than 1,450 WLM compared with those exposed to less than 80 WLM. Temporal factors affected risk, including average dose rate, which was inversely associated with lung cancer risk, and the length of time since last exposure, which was directly associated with decreased risk. As in studies of non-smokers and smokers combined, the exposure response relationship in never-smokers was consistent with a decreased slope at higher WLM, which resulted, in part, from an inverse dose rate effect.  相似文献

Public concern was expressed regarding the possibility of adverse health effects with the disposal of radioactive waste in Port Hope, Ontario. A case-control study was carried out to estimate the relative importance of domestic radon gas exposure in the causation of lung cancer in the town over a ten-year period. Twenty-seven cases met the entry criteria. Statistical analyses of results did not provide conclusive results that linked an increased risk of lung cancer and elevated domestic alpha radiation levels, when all factors were considered. However, a very strong association was demonstrated between cigarette smoking and lung cancer.  相似文献

BACKGROUND: Radon is a radioactive gas that tends to accumulate in indoor environment. A causal relationship between lung cancer and radon exposure has been demonstrated in epidemiologic studies of miners. The objective of this paper is to present the results of case-control studies of lung cancer risk associated with indoor radon exposure. METHODS: Case-control studies published since 1990 are included in this review. This type of protocol is particularly well suited for studying the relationship between indoor radon exposure and lung cancer risk, taking into account possible confounding factors such as tobacco smoking. The characteristics and results of these studies are summarized. The limitations associated with each of these studies are also discussed. RESULTS: The results of available studies are relatively concordant and suggest a positive association between lung cancer risk and indoor radon exposure with an estimated excess relative risk of about 6 to 9% per 100Bq/m3 increase in the observed time-weighted average radon concentration. The order of magnitude of this estimation agrees with extrapolations from miners but some studies may suffer from inadequate statistical power. CONCLUSION: At present, efforts are underway to pool together the data from the existing studies of indoor radon. This pooling analysis with thousands of cases and controls will provide a more precise estimate of the lung cancer risk from indoor radon exposure and explore the effect of modifying factors, such as smoking.  相似文献

United States uranium miners who smoked have death rates from lung cancer that are intermediate between the rates predicted by the additive and multiplicative models (on a ratio scale) across all age groups. Age specific patterns of interaction have not been thoroughly examined, and most analyses have been internal ones in which there was no truly non-exposed group. Here age specific death rates of lung cancer among ever smoking uranium miners have been examined for conformity with the additive and multiplicative models. The multiplicative model fits well for the youngest and oldest categories, but poorly for the middle age ranges. In the middle age range, predicted rates under the multiplicative model were quite high, surpassing the corresponding United States death rates for all causes combined. If the multiplicative model is assumed to hold across all ages, one hypothesis that might explain the observed age specific patterns is that the full expression on the multiplicative model might not be seen at certain ages due to a limited pool of miners susceptible to lung cancer. These data, however, have several limitations such as small numbers of deaths from lung cancer among never smokers, the use of qualitative rather than quantitative smoking and radon exposure data, and ignorance of the underlying biological mechanisms of interaction.  相似文献

After 1946, the WISMUT Company developed the third-largest uranium-mining province in the world in the German Democratic Republic. METHODS: A case-control study among former WISMUT miners was conducted to investigate the lung cancer risk in relation to attained age, time since exposure, exposure duration, and exposure rate. It consisted of 505 patients with lung cancer and 1,073 controls matched to cases according to the year of birth. The cumulative exposure to radon and radon decay products was calculated as the sum of yearly exposures and expressed in Working Level Months (WLM). Cases had a mean cumulative exposure of 552 WLM compared to 420 WLM in controls. RESULTS: There was a statistically significant increase in lung cancer risk for cumulative exposures above 800 WLM. Under the assumption of a linear risk model, there was a significant increase in the relative risk of 0.10 per 100 WLM after adjusting for smoking and asbestos exposure. For current smokers the increase in relative risk was lower (0.05 per 100 WLM), whereas it was higher (0.20 per 100 WLM) among nonsmokers and longtime ex-smokers. After correcting in a sensitivity analysis for the fact that the controls of this study had a higher average exposure than the population of WISMUT workers they were recruited from, the adjusted ERR increased to 0.24 per 100 WLM. Lung cancer risk declined with time since exposure, except for exposures received 45 or more years ago. No inverse dose rate effect was observed.  相似文献

Several studies have shown that miners, in both uranium and nonuranium mines, have an increased lung cancer mortality, probably caused by exposure to radon and its daughters. The excess mortality has been observed primarily among smoking miners but some recent studies have also indicated a considerably increased risk among nonsmoking miners. This study, among a group of iron ore miners, was undertaken to further elucidate the somewhat unclear and presumably complex relationship of mining, smoking, and lung cancer. The results show a 16-fold increase in lung cancer mortality among miners versus nonminers. Even nonsmoking miners seem to be at a rather high risk of developing lung cancer, but there was a tendency for the most heavy smoking miners to die earlier and to have a slightly shorter induction-latency period for development of lung cancer than was found among the nonsmoking miners.  相似文献

A case-control study of 1625 histologically confirmed cases of lung cancer and 3091 controls matched for sex, age, hospital admission, and interviewer was conducted in France between 1976 and 1980. The results presented concern the effects of different occupations on the occurrence of lung cancer among 1334 male cases and 2409 matched controls. Occupations were coded blindly according to the International Standard Classification of Occupations. An excess risk of lung cancer was observed for the following occupations after adjustment for cigarette exposure: farmers (RR = 1.24, p less than 0.06), miners and quarrymen (RR = 2.14, p less than 0.02), plumbers and pipe fitters (RR = 1.80, p less than 0.04), motor vehicle drivers (RR = 1.42, p less than 0.01).  相似文献

Recently there has been considerable public and regulatory concern that radon, produced by the decay of naturally occurring uranium, can accumulate in homes, offices, and schools at levels that may substantially increase the risk of lung cancer. The major cause of lung cancer is smoking, and radon appears to interact multiplicatively with smoking in causing lung cancer. Thus, the most effective way to reduce the increased risk of lung cancer resulting from radon exposure is to cease smoking. In this paper, a model for the risks associated with radon exposure that was developed by a committee of the National Academy of Sciences is used to calculate the benefits, in terms of reduction in lifetime risk of lung cancer, of ceasing to smoke, ceasing radon exposure, or ceasing both. Ceasing to smoke is considerably more beneficial than ceasing radon exposure, and thus policymakers addressing the health effects of radon should place priority on encouraging individuals to stop smoking.  相似文献

Airborne radon has been recognized as a probable cause of lung cancer since the epidemiological studies made in the mining communities of Schneeberg and Jachymov during the 1920s and 1930s. Uranium miners are generally exposed to high concentrations of radon, and the growth of the uranium mining industry in the past 20 years has therefore stimulated a great amount of work on the risk from inhalation of radon and its daughters. However, there is still no general agreement on either the physical measurements in which the maximum permissible concentration in air should be expressed or the numerical value of this concentration.

There are no uranium mines being worked in the United Kingdom but there are considerable numbers of coal and other mines. It was therefore decided to make some radon measurements in a selection of mines in the UK to see if there was any prospect of obtaining useful dose-risk data. Measurements were made in 22 mines in all - 12 coal mines and 10 other mines - and the highest concentrations were found in three haematite mines in West Cumberland and in the two tin mines which were visited. The concentrations in these five mines were, in general, greater than 0·3 of a working level (WL) and in many locations were greater than 1 WL.

Because of these findings and also because an excess incidence of lung cancer among West Cumberland haematite miners has been previously reported, a further study of the mortality experience of West Cumberland miners has been carried out by Boyd, Doll, Faulds, and Leiper (1970). Their findings are reported in this Journal (p. 97).
